Les taux de mortalité maternelle et néonatale restent importants dans les pays en développement. L’ampleur de ces phénomènes est liée à une constellation de facteurs. Mais une part importante des issues défavorables de la grossesse et de la naissance est attribuable à des causes évitables et des comportements modifiables. Les interventions éducatives prénatales ont été élaborées dans le but d’adresser les facteurs affectant la demande de soins maternels et néonatals efficaces. Les stratégies éducatives ciblant les femmes enceintes incluent les conseils individuels, les sessions de groupes et la combinaison des deux stratégies. Ces stratégies visent à améliorer les connaissances sur les questions de santé maternelle et néonatale et à favoriser l’utilisation adéquate de soins qualifiés et les pratiques hygiéniques à domicile. L’Organisation Mondiale de la Santé (OMS) a diffusé dans les pays en développement des guides de pratiques en soins maternels et néonatals incluant les conseils de préparation à la naissance, lors des visites prénatales de routine. Toutefois, peu de données sont disponibles quant à l’efficacité et l’implantation effective de l’éducation prénatale dans les dits pays. Cette thèse cherche à mieux comprendre l’impact des programmes d’éducation prénatale implantés dans deux contextes à risques maternels et néonatals élevés au Burkina Faso. Rédigée sous forme d’articles, la thèse propose trois objectifs spécifiques : 1) examiner l’efficacité théorique des programmes d’éducation prénatale pour réduire la mortalité maternelle et néonatale dans les pays en développement; 2) évaluer l’association entre différents facteurs organisationnels et l’exposition des femmes aux conseils de préparation à la naissance qui font habituellement partie intégrante des programmes d’éducation prénatale implantés dans les services prénatals de routine; et 3) déterminer l’impact de recevoir des conseils de préparation à la naissance sur la probabilité d’accouchement institutionnel. Pour répondre au premier objectif, une méta-analyse de données issues d’essais randomisés a été effectuée. Concernant les réponses aux deux autres objectifs, les données d’une étude de cohorte rétrospective ont été utilisées. Cette étude observationnelle, conçue spécialement pour la thèse, a été menée dans deux districts à risques maternels et néonatals élevés (Dori et Koupela) du Burkina Faso. Les résultats observés à travers les trois investigations sont utiles pour l’avancement des connaissances et la pratique. La méta-analyse révèle que les interventions éducatives expérimentales sont associées à une réduction de 24% de la mortalité néonatale. Cette réduction atteint 30% dans les milieux à très forte mortalité néonatale. En situation de routine, divers facteurs organisationnels peuvent limiter ou faciliter la transmission des conseils éducatifs aux femmes usagères de soins prénatals. Au, Burkina Faso, les données analysées indiquent des fortes disparités entre les deux districts à l’étude. Les femmes du district de Koupela étaient significativement plus exposées aux conseils que celles de Dori. Au delà de cette disparité régionale, deux autres facteurs organisationnels sont fortement associés à l’exposition des femmes aux conseils de préparation à la naissance lors des visites prénatales de routine. Il s’agit de la disponibilité de supports de communication imagés dans l’établissement et le volume réduit de consultations par jour (moins de 20 consultations en moyenne versus 20 ou plus) augurant de moindres charges de travail pour le personnel. Enfin, les conseils reçus par les femmes sur les signes de complications obstétricales et sur les coûts des soins sont significativement associés à une probabilité plus élevée d’accoucher en institution; et ce, seulement dans le district de Dori où le taux d’accouchements institutionnels était relativement faible. En conclusion, l’éducation prénatale est bénéfique pour la sante maternelle et néonatale. Cependant, l’implantation et les effets sont hétérogènes selon les milieux. D’autres études expérimentales et observationnelles sont requises pour renforcer les évidences et investiguer plus en profondeur les facteurs de réussite afin de mieux orienter l’intervention. Les expérimentations futures devraient mesurer des issues de grossesses relatives à la mère (l’assistance qualifiée, les soins postpartum et la mortalité maternelle). Des études de cohorte prospectives avec des grands échantillons représentatifs permettraient de documenter de façon plus valide les événements et les expositions aux interventions durant la grossesse, l’accouchement et le postpartum.

Après des années d’efforts, l’Afrique Sub-saharienne n’a connu qu’une faible amélioration de ses indicateurs de santé maternelle. Assurer l’accès aux soins obstétricaux d’urgence (SOU) pour toutes les femmes est une stratégie efficace pour réduire la mortalité maternelle. Cependant, ces soins sont dispendieux et ces dépenses peuvent être « catastrophiques ». Afin d’en réduire le fardeau, le Mali a instauré la gratuité de la césarienne et un système de référence-évacuation. L’objectif de cette étude est d’examiner la prévalence et les facteurs contribuant aux dépenses catastrophiques liées aux SOU dans la région de Kayes, Mali. Elle vise aussi à étudier les conséquences socioéconomiques de ces dépenses au sein des ménages. L’étude a révélé que les dépenses lors d’urgences obstétricales sont en moyenne de 71535 FCFA (US$ 152). Entre 20.7% et 53.5% des ménages ont encouru des dépenses catastrophiques supérieures à 15% et 5% de leur revenu annuel respectivement. Les ménages de femmes sans éducation, du milieu rural et ayant souffert d’infection post-partum sont les plus à risque d’encourir des dépenses catastrophiques. La césarienne n’est pas associée à une probabilité réduite de dépense catastrophique malgré la gratuité. Faire des dépenses élevées ne garantie pas la survie de la mère puisque entre 19,4% et 47,1% des décès maternels ont encouru des dépenses catastrophiques. Enfin, les ménages s’endettent et vendent fréquemment des biens pour faire face aux dépenses ce qui créé des difficultés financières importantes à long terme. La création de nouvelles politiques de financement sera nécessaire à l’amélioration de la santé maternelle au Mali.

Contexte : Environ 20 % des femmes enceintes présentent un risque élevé de dépression prénatale. Les femmes immigrantes présentent des symptômes dépressifs élevés pendant la grossesse, le début de la période suivant l'accouchement et comme mères de jeunes enfants. Tandis que les disparités ethniques dans la symptomatologie dépressive pendant la grossesse ont été décrites, la combinaison de la longueur du séjour dans le pays d’accueil et la région d'origine sont rarement évalués dans des études qui portent sur la santé des immigrants au Canada. En outre, les études auprès des femmes immigrantes enceintes ont souvent un échantillon de taille qui ne suffit pas pour démêler les effets de la région d'origine et de la durée du séjour sur la santé mentale. De plus, au Canada, presque une femme sur cinq est un immigrant, mais leur santé mentale au cours de la grossesse, les niveaux d'exposition aux facteurs de risque reconnus pour la dépression prénatale et comment leur exposition et la vulnérabilité face à ces risques se comparent à celles des femmes enceintes nés au Canada, sont peu connus. De plus, le processus d'immigration peut être accompagné de nombreux défis qui augmentent le risque de violence subie par la femme. Néanmoins, les preuves existantes dans la littérature sont contradictoires, surtout en ce qui concerne le type de violence évaluée, les minorités ethniques qui sont considérées et l'inclusion de l'état de santé mentale. Objectifs : Tout d'abord, nous avons comparé la santé mentale de femmes immigrantes et les femmes nées au Canada au cours de la grossesse en tenant compte de la durée du séjour et de la région d'origine, et nous avons évalué le rôle des facteurs socio-économiques et du soutien social dans la symptomatologie dépressive prénatale. Deuxièmement, nous avons examiné la répartition des facteurs de risque contextuels de la symptomatologie dépressive prénatale selon le statut d'immigrant et la durée du séjour au Canada. Nous avons ensuite évalué l'association entre ces facteurs de risque et les symptômes de dépression prénataux et ensuite comparé la vulnérabilité des femmes nés au Canada et les femmes immigrantes à ces facteurs de risque en ce qui concerne les symptômes de la dépression prénatale. En troisième lieu, nous avons décrit la prévalence de la violence pendant la grossesse et examiné l'association entre l'expérience de la violence depuis le début de la grossesse et la prévalence des symptômes de la dépression prénatale, en tenant compte du statut d’immigrant. Méthodes : Les données proviennent de l'étude de Montréal sur les différences socio-économiques en prématurité. Les femmes ont été recrutées lors des examens de routine d'échographie (16 à 20 semaines), lors de la prise du sang (8-12 semaines), ou dans les centres de soins prénatals. L’échelle de dépistage Center for Epidemiologic Studies (CES-D) a été utilisée pour évaluer la symptomatologie dépressive à 24-26 semaines de grossesse chez 1495 immigrantes et 3834 femmes nées au Canada. Les niveaux d'exposition à certains facteurs de risque ont été évalués selon le statut d'immigrant et la durée de séjour à l'aide des tests Chi-2 ou test- t. L'échelle de dépistage Abuse Assessment screen (AAS) a été utilisée pour déterminer la fréquence et la gravité de la violence depuis le début de la grossesse. La relation avec l'agresseur a été également considérée. Toutes les mesures d'association ont été évaluées à l'aide de régressions logistiques multiples. Des termes d'interaction multiplicative furent construits entre chacun des facteurs de risque et statut d'immigrant pour révéler la vulnérabilité différentielle entre les femmes nés au Canada et immigrantes. Résultats : La prévalence des symptômes de dépression prénatales (CES-D > = 16 points) était plus élevée chez les immigrantes (32 % [29,6-34,4]) que chez les femmes nées au Canada (22,8 % (IC 95 % [21.4-24.1]). Des femmes immigrantes présentaient une symptomatologie dépressive élevée indépendamment du temps depuis l'immigration. La région d'origine est un fort indice de la symptomatologie dépressive : les prévalences les plus élevées ont été observées chez les femmes de la région des Caraïbes (45 %), de l’Asie du Sud (43 %), du Maghreb (42 %), de l'Afrique subsaharienne (39 %) et de l’Amérique latine (33 %) comparativement aux femmes nées au Canada (22 %) et celle de l'Asie de l’Est où la prévalence était la plus faible (17 %). La susceptibilité de présenter une dépression prénatale chez les femmes immigrantes était attenuée après l’ajustement pour le manque de soutien social et de l'argent pour les besoins de base. En ce qui concerne la durée du séjour au Canada, les symptômes dépressifs ont augmenté avec le temps chez les femmes d’origines européenne et asiatique du sud-est, diminué chez les femmes venant du Maghreb, de l’Afrique subsaharienne, du Moyen-Orient, et de l’Asie de l'est, et ont varié avec le temps chez les femmes d’origine latine et des Caraïbes. Les femmes immigrantes étaient beaucoup plus exposées que celles nées au Canada à des facteurs de risques contextuels indésirables comme la mésentente conjugale, le manque de soutien social, la pauvreté et l'encombrement au domicile. Au même niveau d'exposition aux facteurs de risque, les femmes nées au Canada ont présenté une plus grande vulnérabilité à des symptômes de la dépression prénatale en l'absence de soutien social (POR = 4,14 IC95 % [2,69 ; 6.37]) tandis que les femmes immigrées ont présentées une plus grande vulnérabilité à des symptômes de la dépression prénatale en absence d'argent pour les besoins de base (POR = 2,98 IC95 % [2.06 ; 4,32]). En ce qui concerne la violence, les menaces constituent le type de la violence le plus souvent rapporté avec 63 % qui ont lieu plus d'une fois. Les femmes immigrantes de long terme ont rapporté la prévalence la plus élevée de tous les types de violence (7,7 %). La violence par le partenaire intime a été la plus fréquemment rapportées (15 %) chez les femmes enceintes les plus pauvres. Des fortes associations ont été obtenues entre la fréquence de la violence (plus d'un épisode) et la symptomatologie dépressive (POR = 5,21 [3,73 ; 7,23] ; ainsi qu’entre la violence par le partenaire intime et la symptomatologie dépressive (POR = 5, 81 [4,19 ; 8,08). Le statut d'immigrant n'a pas modifié les associations entre la violence et la symptomatologie dépressive. Conclusion: Les fréquences élevées des symptômes dépressifs observées mettent en évidence la nécessité d'évaluer l'efficacité des interventions préventives contre la dépression prénatale. La dépression chez les femmes enceintes appartenant à des groupes minoritaires mérite plus d'attention, indépendamment de leur durée de séjour au Canada. Les inégalités d’exposition aux facteurs de risque existent entre les femmes enceintes nées au Canada et immigrantes. Des interventions favorisant la réduction de la pauvreté et l'intégration sociale pourraient réduire le risque de la dépression prénatale. La violence contre les femmes enceintes n'est pas rare au Canada et elle est associée à des symptômes de la dépression prénatale. Ces résultats appuient le développement futur du dépistage périnatal de la violence, de son suivi et d'un système d'aiguillage culturellement ajusté.

An experiment on restricted suckling of crossbred dairy cows was conducted at the Livestock Research Centre, Tanga in northeast Tanzania. Thirty-six Bos taurus (Holstein Friesian and Jersey) x Bos indicus (East African Zebu) cows were allocated alternately as they calved to suckling their calves for either 12 or 24 weeks after calving. Cows grazed improved pastures and were offered 4 kg concentrate daily. Milking occurred twice daily by hand; calves were allowed to suck residual milk for 30 min following each milking. Calves were also allowed access to grazing and were offered a maximum of I kg concentrate daily to 24 weeks of age. Weaning age had no significant effect on lactation milk yield for human consumption, the mean (SE) yield being 1806 (102.0) L and 1705 (129. 1) L for 12- and 24-week weaning, respectively. Cows from the two treatments suffered similar losses of live weight and body condition score during lactation and neither group had returned to the original body condition score 40 weeks following calving. Post-partum anoestrous intervals were prolonged. Although not significant, cows suckling calves to 24 weeks had a mean interval to first oestrus extended by 38 days compared with cows suckling calves to 12 weeks. The mean (SE) daily live weight gains of the calves to 52 weeks were 263 (14.1) g/day and 230 (18.1) g/day for calves weaned at 12 and 24 weeks, respectively, such that 12-month weights were 119 (5.6) kg and 110 (7.3) kg, respectively. Twelve-week-weaned calves consumed more concentrate (p < 0.05) from 13 to 24 weeks than did 24-week weaned calves. Calculation of residual milk consumption removed by calves from birth to 12 weeks indicated that it accounted for 28% of total yield. No benefits in cow and calf performance and welfare were found to justify prolonging the suckling period to 24 weeks.

Background: Acupuncture is commonly used to reduce pain during labour despite contradictory results. The aim of this study is to evaluate the effectiveness of acupuncture with manual stimulation and acupuncture with combined manual and electrical stimulation (electro-acupuncture) compared with standard care in reducing labour pain. Our hypothesis was that both acupuncture stimulation techniques were more effective than standard care, and that electro-acupuncture was most effective. Methods: A longitudinal randomised controlled trial. The recruitment of participants took place at the admission to the labour ward between November 2008 and October 2011 at two Swedish hospitals. 303 nulliparous women with normal pregnancies were randomised to: 40 minutes of manual acupuncture (MA), electro-acupuncture (EA), or standard care without acupuncture (SC). Primary outcome: labour pain, assessed by Visual Analogue Scale (VAS). Secondary outcomes: relaxation, use of obstetric pain relief during labour and post-partum assessments of labour pain. The sample size calculation was based on the primary outcome and a difference of 15 mm on VAS was regarded as clinically relevant, this gave 101 in each group, including a total of 303 women. Results: Mean estimated pain scores on VAS (SC: 69.0, MA: 66.4 and EA: 68.5), adjusted for: treatment, age, education, and time from baseline, with no interactions did not differ between the groups (SC vs MA: mean difference 2.6, 95% confidence interval [CI] -1.7-6.9 and SC vs EA: mean difference 0.6 [95% CI] -3.6-4.8). Fewer number of women in the EA group used epidural analgesia (46%) than women in the MA group (61%) and SC group (70%) (EA vs SC: odds ratio [OR] 0.35; [95% CI] 0.19-0.67). Conclusions: Acupuncture does not reduce women's experience of labour pain, neither with manual stimulation nor with combined manual and electrical stimulation. However, fewer women in the EA group used epidural analgesia thus indicating that the effect of acupuncture with electrical stimulation may be underestimated. These findings were obtained in a context with free access to other forms of pain relief.

After the birth of a child, during the post-partum period, the exercise of fatherhood, the sexual relationships either marital or sexual as well as the definition of the male role are all aspects that can change. These changes are guarded and can contribute to strengthen the familiar bond or deprive it. Thus, this study aims to understand the male experience during the partner s puerperal period. In this period, the male interacts and participates according to his understanding of the post-partum period. The male experiences different situations with his partner, family and social environment. This research is descriptive-exploratory and has a qualitative approach. It was developed with 15 men that live with their live with their partners during the puerperal period. The data was collected through a semi-structured interview. These data was treated according to content analysis proposed by Bardin. These were analyzed through Symbolic Interacionism according to Blumer. Thus, three themes emergedshares of care in the home environment, meanings attributed to the puerperium, emotions emerged during the post-partum - the seven subcategories-taking care of the wife and child, supporting the family, moment to rest, fulfillment through fatherhood, sense of joy, feeling of concern and sense of exclusion. It was seen that the subjects experience post-partum mainly by taking care of their partner and child and providing support of the family. Besides the mentioned aspects, there was meaning attributed to the puerperal period, considering it a period of rest for the partner and above all, fatherhood. Thus, even understanding that it is excluded, the subjects expressed joy and worriness towards the child s future, related to the insecurity in which the country goes through in current conjuncture

The consultation for women during the postpartum period should occur between the seventh and tenth days, and 42 days after childbirth, to decrease the incidence of maternal and neonatal morbidity and mortality. However, the effectiveness of such assistance in primary health care has not been achieved, especially in the forty-second day of puerperium. Facing this reality, the research aimed to understand the views of women about postpartum consultation. This is an exploratory and descriptive research with qualitative approach, developed in the municipality of Lajes/RN, Brazil, with women inscribed on the four teams that make up the Family Health Strategy. Data were collected through semistructured interviews with 15 women who met the following criteria: be enrolled in ESF; have health mental preserved, have been entered in the Humanization Program of Prenatal and Birth, and that was, at maximum, 60 days postpartum. The data were organized according to the precepts of content analysis according to Bardin, generating three categories: prevention of puerperal complications, feelings related to life changes after childbirth, and postpartum care. This process of coding and categorizing a central theme emerged: the experience of women in the postpartum period. The data were analyzed according to the principles of symbolic interactionism, according to Blumer. The study revealed that the meanings attributed to the postpartum period for prevention of complications were directly related to home, to the consultation and postpartum care provided by family members and health professionals. The interviewees strictly complied with the rest under the influence of the context in which they were entered. But that has not happened with the postpartum revision because few mothers underwent this procedure. Therefore, the interaction of the interviewed people in their living standard as well as the feelings that permeated the post-partum were crucial to consider whether or not the post-partum visit as significant. According to the results, it was noted that disability guidelines and counter-references has impaired the access of women to postpartum review. Thus, further studies are needed on the subject, as well as a reorientation of health care activities in view of the consolidation of postpartum consultation in primary care

O caráter multifatorial das complicações e da mortalidade materna torna difícil e demorada a tarefa de seu controle a longo prazo. A atenção profissional à mulher gestante e/ou parturiente representa seguramente elemento chave para a obtenção de bons resultados, tanto maternos quanto perinatais. Partindo-se do pressuposto de que atenção médica profissional ao parto de maneira adequada tem a capacidade de diminuir a ocorrência de complicações associadas à morbidade e mortalidade materna, são apresentadas resumidamente as evidências sobre algumas intervenções incluídas nesta atenção. São enfocadas as evidências derivadas de estudos realizados com extremo rigor metodológico e científico, os ensaios controlados randomizados, sobre intervenções capazes de reduzir as complicações e a mortalidade materna. Estas principais intervenções referem-se basicamente a: atenção institucional ao parto, atendimento profissional capacitado, utilização de parteiras tradicionais em determinados contextos, uso de tecnologias apropriadas incluindo o partograma, local do parto, posição para o parto, uso de episiotomia, tipo de parto, uso de ocitócicos na fase ativa do parto, realização de esforços de puxo no período expulsivo, manejo da dequitação e profilaxia da hemorragia puerperal. Ainda que o efeito de prevenir mortes seja difícil de ser avaliado pela baixa freqüência, sua utilização de forma racional e padronizada, por meio de manuais e normatizações de condutas de serviços, tem um efeito positivo sobre a qualidade da atenção ao nascimento. Isso faz parte do contexto técnico e humano do direito que toda mulher tem ao melhor atendimento possível nesse momento tão especial de sua vida.

Background: Caesarean section rates in Brazil have been steadily increasing. In 2009, for the first time, the number of children born by this type of procedure was greater than the number of vaginal births. Caesarean section is associated with a series of adverse effects on the women and newborn, and recent evidence suggests that the increasing rates of prematurity and low birth weight in Brazil are associated to the increasing rates of Caesarean section and labour induction. Methods: Nationwide hospital-based cohort study of postnatal women and their offspring with follow-up at 45 to 60 days after birth. The sample was stratified by geographic macro-region, type of the municipality and by type of hospital governance. The number of postnatal women sampled was 23,940, distributed in 191 municipalities throughout Brazil. Two electronic questionnaires were applied to the postnatal women, one baseline face-to-face and one follow-up telephone interview. Two other questionnaires were filled with information on patients' medical records and to assess hospital facilities. The primary outcome was the percentage of Caesarean sections (total, elective and according to Robson's groups). Secondary outcomes were: post-partum pain; breastfeeding initiation; severe/near miss maternal morbidity; reasons for maternal mortality; prematurity; low birth weight; use of oxygen use after birth and mechanical ventilation; admission to neonatal ICU; stillbirths; neonatal mortality; readmission in hospital; use of surfactant; asphyxia; severe/near miss neonatal morbidity. The association between variables were investigated using bivariate, stratified and multivariate model analyses. Statistical tests were applied according to data distribution and homogeneity of variances of groups to be compared. All analyses were taken into consideration for the complex sample design. Discussion: This study, for the first time, depicts a national panorama of labour and birth outcomes in Brazil. Regardless of the socioeconomic level, demand for Caesarean section appears to be based on the belief that the quality of obstetric care is closely associated to the technology used in labour and birth. Within this context, it was justified to conduct a nationwide study to understand the reasons that lead pregnant women to submit to Caesarean sections and to verify any association between this type of birth and it's consequences on postnatal health.

Admission blood lactate concentration has been shown to be a useful indicator of disease severity in human medicine and numerous studies have associated hyperlactatemia with patients at high risk of death who should be treated aggressively regardless of the cause of the lactate generation. The degree and duration of hyperlactacidaemia also have been correlated with the subsequent development of organ failure. Similarly, in a small number of studies about equine colic, blood lactate concentration has been investigated as a useful prognostic variable . In neonatal foals blood lactate was studied first by Magdesian (2003) who described venous blood lactate concentration in 14 normal foals during the initial 48 hours post-partum. A preliminary study about lactate concentration in foals presenting to a neonatal intensive care unit reported that surviving foals had earlier lactate clearance. The measurement of blood lactate concentration is traditionally available with a wet chemistry laboratory method or with blood-gas analyzers, for clinicians working at university or large private hospital. But this methods may not be easily accessible to many practitioners in field conditions. Several relatively inexpensive, easy to use and rapid pocket size monitors to measure lactate concentration have been validated in human patients and athletes. None of these portable lactate analyzer have been evaluated in clinically normal neonatal foals or in foals referred to a neonatal intensive care unit. The aims of this study were to validate the Lactate Scout analyzer in neonatal foals, investigating the correlation between lactate concentration in whole blood measured with the portable monitor and measured in plasma with the reference laboratory analyzer. The effect of hematocrit (Hct) on the accuracy of Lactate Scout was also evaluated. Further, we determined the utility of venous lactate measurement in critically-ill foals, describing lactate values in the most frequent neonatal pathologies, evaluating serial blood lactate measurements during hospitalization and investigating its prognostic value. The study also describes normal range for lactate in healthy neonatal foals during the first 72 hours of life.

Extensive literature outlined that the quality of the mother-foetus relationship is considered the main feature with regard to the quality of postnatal mother-infant interaction and also to the child’s psychical development. Nowadays the relationship between the pregnant woman and her foetus is viewed as the central factor of the somatic dialogue between the functioning of the maternal and the foetal organisms. This dialogue is responsible for the physic development of the child, as well as of its psychosomatic structure. Therefore the research area has necessarily had to extend to the analysis of psychological processes concerning: the pregnancy, the couple that is bound by parenthood, the influence of intergenerational dynamics. In fact, the formation of maternal identity, as well as that of the relationship between the woman and the foetus, refers to the pregnant woman’s relationship with her parents, especially with her mother. The same pregnancy, considered as a psychosomatic event, is directly influenced by relational, affective and social factors, particularly by the quality of the interiorized parental relations and the quality of the current relationships (such as that with her partner and with her family of origin). Some studies have begun to investigate the relationship between the pregnant woman and the foetus in term of “prenatal attachment” and its relationship with socio-demographic, psychological e psychopathological aspects (such as pre and post partum depression), but the research area is still largely unexplored. The present longitudinal research aimed to investigate the quality of the pregnant womanfoetus relationship by the prenatal attachment index, the quality of the interiorized relationship with woman’s parents, the level of alexithymic features and maternity social support, in relation with the modulation of the physiology of delivery and of postpartum, as well as of the physical development of the child. A consecutive sample of 62 Italian primipara women without any kind of pathologies, participated in the longitudinal study. In the first phase of this study (third trimester of the pregnancy), it has investigated the psychological processes connected to the affective investment of the pregnant women towards the unborn baby (by Prenatal Attachment Inventory), the mothers’ interiorized relationship with their own parents (by Parental Bonding Instrument), the social and affective support from their partner and their family of origin are able to supply (by Maternity Social Support Scale), and the level of alexithymia (by 20-Toronto Alexithymia Scale). In the second phase of this study, some data concerning the childbirth course carried out from a “deliverygram” (such as labour, induction durations and modalities of delivery) and data relative to the newborns state of well-being (such as Apgar and pH indexes). Finally, in the third phase of the study women have been telephoned a month after the childbirth. The semistructured interview investigated the following areas: the memory concerning the delivery, the return to home, the first interactions between the mother and the newborn, the breastfeeding, the biological rhythms achieved from newborns. From the data analysis a sample with a good level of prenatal attachment and of support social and a good capability to mental functioning emerged. An interesting result is that the most of the women have a great percentage of “affectionless control style” with both parents, but data is not sufficient to interpret this result. Moreover, considering the data relative to the delivery, medical and welfare procedures, that have been necessary, are coherent with the Italian mean, while the percentage of the caesarean section (12.9%) is inferior to the national percentage (30%). The 29% of vaginal partum has got epidural analgesia, which explains the high number (37%) of obstetrician operations (such as Kristeller). The data relative to the newborn (22 male, 40 female) indicates a good state of well-being because Apgar and pH indexes are superior to 7 at first and fifth minutes. Concerning the prenatal phase, correlation analysis showed that: the prenatal attachment scores positively correlated with the expected social support and negatively correlated with the “externally oriented thinking” dimension of alexithymia; the maternity social support negatively correlated with total alexithymia score, particularly with the “externally oriented thinking” dimension, and negatively correlated with maternal control of parental bonding. Concerning the delivery data, there are many correlations (after all obvious) among themselves. The most important are that the labour duration negatively correlated with the newborn’s index state of well-being. Finally, concerning the data from the postpartum phase the women’ assessments relative to the partum negatively correlated with the duration of the delivery and positively correlated with the assessment relative to the return to home and the interaction with the newborn. Moreover the length of permanence in the hospital negatively correlated with women’s assessments relative to the return to home that, in turn, positively correlated with the quality of breastfeeding, the interaction between the mother and the newborn and the biological regulation of the child. Finally, the women’ assessments relative to breastfeeding positively correlated with the mother-child interactions and the biological rhythms of children. From the correlation analysis between the variables of the prenatal phase and the data relative to the delivery, emerged that the prenatal attachment scores positively correlated with the dilatation stage scores and with the newborn’s Apgar index at first minute, the paternal care dimension of parental bonding positively correlated with the lengths of the various periods of childbirth like so the paternal control dimension with placental stage. Moreover, emerged that the expected social support positively correlated with the lengths of the various periods of childbirth and that the global alexithymia scores, particularly “difficulty to describe emotions” dimension, negatively correlated with total childbirth scores. From the correlation analysis between the variables of the prenatal phase and variable of the postpartum phase emerged that the total alexithymia scores positively correlated with the time elapsed from the childbirth to the breastfeeding of the child, the difficulty to describe emotions dimension of the alexithymia negatively correlated with the quality of the breastfeeding, the “externally oriented thinking” dimension of the alexithymia negatively correlated with mother-child interactions, and finally the paternal control dimension of the parental bonding negatively correlated with the time elapsed from the child to the breastfeeding of the child. Finally, from the analysis of the correlation between the data of the partum and the women’s assessments of the postpartum phase, emerged the negative correlation between the woman’s assessment relative to the delivery and the quantitative of obstetrician operations and the lengths of the various periods of childbirth, the positive correlation between the women’s assessment about the length of delivery periods and the real lengths of the same ones, the positive relation between woman’s assessment relative to the delivery and the Apgar index of children. In conclusion, there is a remarkable relation between the quality of the relationship the woman establishes with the foetus that influences the course of the pregnancy and the delivery that, in turn, influences the postpartum outcome, particularly relative to the mother-children relationship. Such data should be confirmed by heterogeneous populations in order to identify vulnerable women and to project focused intervention.

Abstract Objective: To evaluate the effectiveness of human recombinant activated factor VII (rhFVIIa, NovoSeven) in avoiding hysterectomy postpartum in the management of severe postpartum hemorrhage (PPH). Methods: We performed a prospective cohort study at our university tertiary care center. Patients with severe post partum hemorrhage (blood loss >2000 mL) and failed medical and uterus-preserving surgical management, were treated with intravenous bolus administration of rhVIIa. Main outcome measures were cessation of bleeding, postpartum hysterectomy and thromboembolic events. Results: In 20/22 patients included, PPH was caused primarily by uterine atony, including 7 (32%) with additional lower genital tract lesion; in two women, it was due to pathologic placentation (placenta increta, 9%). One case of amniotic fluid embolism and one woman with uterine inversion were included. Recombinant hFVIIa was successful in stopping the PPH and in preventing a hysterectomy in 20/22 women (91%). The remaining two patients with persistent bleeding despite rhFVIIa treatment, who underwent postpartum hysterectomy, had placenta increta. No thromboembolic event was noticed. Conclusions: This study describes the largest single center series of rhFVIIa treatment for fertility preservation in severe postpartum hemorrhage published to date. Our data suggest that administration of rhFVIIa is effective in avoiding postpartum hysterectomy after conservative medical and surgical measures have failed. Although randomized studies are lacking, rhFVIIa should be considered as a second-line therapeutic option of life-threatening postpartal bleeding, in particular if preservation of fertility is warranted and hysterectomy is to be avoided.

BACKGROUND We report on the design and implementation of a study protocol entitled Acupuncture randomised trial for post anaesthetic recovery and postoperative pain - a pilot study (ACUARP) designed to investigate the effectiveness of acupuncture therapy performed in the perioperative period on post anaesthetic recovery and postoperative pain. METHODS/DESIGN The study is designed as a randomised controlled pilot trial with three arms and partial double blinding. We will compare (a) press needle acupuncture, (b) no treatment and (c) press plaster acupressure in a standardised anaesthetic setting. Seventy-five patients scheduled for laparoscopic surgery to the uterus or ovaries will be allocated randomly to one of the three trial arms. The total observation period will begin one day before surgery and end on the second postoperative day. Twelve press needles and press plasters are to be administered preoperatively at seven acupuncture points. The primary outcome measure will be time from extubation to 'ready for discharge' from the post anaesthesia care unit (in minutes). The 'ready for discharge' end point will be assessed using three different scores: the Aldrete score, the Post Anaesthetic Discharge Scoring System and an In-House score. Secondary outcome measures will comprise pre-, intra- and postoperative variables (which are anxiety, pain, nausea and vomiting, concomitant medication). DISCUSSION The results of this study will provide information on whether acupuncture may improve patient post anaesthetic recovery. Comparing acupuncture with acupressure will provide insight into potential therapeutic differences between invasive and non-invasive acupuncture techniques.
