This study aimed to investigate complementary feeding practices during children's first year of life in Botucatu, SP, Brazil. Practices were described according to the age range and the breastfeeding (BF) practice. Data were collected during a multi-vaccination campaign through the interview of 1,238 individuals who accompanied children younger than one year old being vaccinated. Differences associated with the BF situation were identified by the chi-square test and Fisher's exact test. The early introduction of complementary food was observed, which led to the low frequency of exclusive breastfeeding (36.9% <4 months). Children younger than 4 months old consumed tea (30.7%); children between 4 and 6 months old consumed fruits (54.1%), soups (39.9%) and home-made food (19.2%). Juice was offered only to 15.2% of children younger than 4 months old who were completed weaned, 60% of the children was offered water. Data show that the consistence of the food consumed was inappropriate: children between 6 and 8 months old were offered the family's regular food (48.8%) and children older than 8 months (71.6%) were offered soup. Therefore, interventions focused on complementary feeding are justified on the city.

Objective: The present study aimed at evaluating the occurrence and recurrence of middle ear effusion and possible associated factors in the first two years of life of 190 newborns and infants, participants in the interdisciplinary prevention, detection, and intervention program at the Clínica de Educação para Saúde of Universidade do Sagrado Coração. Methods: Newborns and infants were monthly submitted to anamneses, otoscopy, behavioral hearing assessment using sound instruments and pure tones (pediatric audiometry) and tympanometry. Results: The results revealed that 68.4% of infants presented one or more episodes of middle ear effusion during their two first years, with more recurrence among males. Peak occurrence was between four and 12 months of age and, the earlier the first episode, the higher the probability of recurrence. Greatest incidence was during May and August. It was found that, of the variables investigated, the period of exclusive breastfeeding actuated as a protector factor. With respect of risk factors, it was observed that passive smoking, gastro-esophageal reflux and respiratory allergy were related with the recurrences of effusion. Conclusion: Findings revealed the importance of periodic auditory follow-up for infants during their first two years of life, considered to be the critical period of auditory system maturation, during which sensory deprivation can be responsible for damage to the development of speech, language and other auditory abilities. © 2007 Sociedad Chilena de Pediatría.

Objective: To evaluate the results of the kangaroo mother method in Brazil. Methods: A prospective cohort study comparing 16 units that have or do not have the second phase of the kangaroo mother method: eight were national centers of excellence for the kangaroo mother method (study group) and eight were part of the Brazilian Neonatal Research Network (control group). A total of 985 newborn infants with birth weights of 500 to 1,749 g were enrolled. Multivariate analyses employedmultiple linear regression and Poisson regression with robust adjustment. Results: The adjusted analysis (controlled for birth weight, gestational age, Score for Neonatal Acute Physiology Perinatal Extension II, Neonatal Therapeutic Intervention Scoring System, and maternal age and educational level) demonstrated that mean length of hospital stay (p = 0.14) and intercurrent clinical conditions in the intermediate or kangaroo unit were equal for both groups. Weight (p = 0.012), length (p = 0.039) and head circumference (p = 0.006) at 36 weeks' corrected gestational age were all lower at the kangaroo units. The kangaroo units exhibited superior performance in relation to exclusive breastfeeding at discharge (69.2 vs. 23.8%, p=0.022). Conclusions: The evidence suggests that the humanization strategy adopted by the Brazilian Ministry of Health is a safe alternative to conventional treatment and a good strategy for promoting breastfeeding. Introduction: wheezing is one of the most common respiratory symptoms in childhood. Regardless of the cause, it is a reason to seek medical care in emergency rooms, especially if there is recurrence of episodes. Very common in childhood, recurrent wheezing has its first episodes in the first year of life. We sought to examine the risk factors for recurrent wheezing in infants in the first year of life. Methods: this is a cross-sectional quantitative study in which a standardized questionnaire of the International Study of Wheezing in Infants, translated and validated in Brazil, consisting of objective questions, applied 40 mothers were enrolled in two Family Health units. Results: the risk factors found were: smoking during pregnancy, family history of asthma, rhinitis and allergic dermatitis, the presence of at least one pet in the home at the time of birth and age at first cold less than or equal to three months of life. No significant relationships were found between males and wheezing, exclusive breastfeeding or numbers of colds in the first year of life. Conclusion: our findings are different from those reported in the literature.

O objetivo foi identificar os determinantes da saúde ruim em populações ribeirinhas menores de dois anos, residentes no Pará, Brasil. Foram avaliadas 202 crianças, considerando-se saúde ruim como variável desfecho, sendo composta pela combinação do estado nutricional, desenvolvimento físico-motor e intercorrências no último mês. Utilizou-se modelo multinível de análise hierárquica, considerando-se como preditoras da saúde ruim variáveis com p < 0,05 após ajuste. A razão de chance bruta apontou que o estado de saúde ruim é maior para as crianças de famílias que têm casa própria, são de maior idade e têm probabilidade de aleitamento materno exclusivo aos dois, três, quatro e cinco meses. Após ajuste, observa-se que crianças provenientes de famílias com casa própria têm 2,76 vezes mais chance de ter saúde ruim; esta também aumenta com a idade, chegando a ser 5,04 vezes maior entre as crianças de 18 a 23 meses, comparativamente às menores de 7 meses. Ter casa própria e mais idade representam, nessas comunidades, mais tempo de exposição ao risco de saúde ruim.

Background: Verifying the association with non-nutritives suction habits and practice of breastfeeding. Material and Methods: 87 pregnants were interviewed and were monthly home visits until 6 mounths old babies, the studied variables were: social-economic-culture conditions, breast feeding pratical and non-nutritives suction habits. Results: 94.7% for mothers started breastfeeding their babies in the first month, but only 49.7% exclusively children breastfeeding. The sixth month nobody mother non-exclusively breastfed ones and 50% were weaning. The habits and pacifier users were significant association with exclusively children breastfed (p=0.0065 and 0.0270 respectively) and children breastfed (both p<0.0001). Conclusions: The early weaning rate was high and low rate of exclusive breastfeeding, existing association between non-nutritives suction habits and pacifier users with breastfeeding.
