PURPOSE: Several studies observed a female advantage in the prognosis of cutaneous melanoma, for which behavioral factors or an underlying biologic mechanism might be responsible. Using complete and reliable follow-up data from four phase III trials of the European Organisation for Research and Treatment of Cancer (EORTC) Melanoma Group, we explored the female advantage across multiple end points and in relation to other important prognostic indicators. PATIENTS AND METHODS: Patients diagnosed with localized melanoma were included in EORTC adjuvant treatment trials 18832, 18871, 18952, and 18961 and randomly assigned during the period of 1984 to 2005. Cox proportional hazard models were used to calculate hazard ratios (HRs) and 95% CIs for women compared with men, adjusted for age, Breslow thickness, body site, ulceration, performed lymph node dissection, and treatment. RESULTS: A total of 2,672 patients with stage I/II melanoma were included. Women had a highly consistent and independent advantage in overall survival (adjusted HR, 0.70; 95% CI, 0.59 to 0.83), disease-specific survival (adjusted HR, 0.74; 95% CI, 0.62 to 0.88), time to lymph node metastasis (adjusted HR, 0.70; 95% CI, 0.51 to 0.96), and time to distant metastasis (adjusted HR, 0.69; 95% CI, 0.59 to 0.81). Subgroup analysis showed that the female advantage was consistent across all prognostic subgroups (with the possible exception of head and neck melanomas) and in pre- and postmenopausal age groups. CONCLUSION: Women have a consistent and independent relative advantage in all aspects of the progression of localized melanoma of approximately 30%, most likely caused by an underlying biologic sex difference.

GOAL: To evaluate the impact of the Ross operation, recently (1997) introduced in our unit, for the treatment of patients with congenital aortic valve stenosis. METHODS: The period from January 1997 to December 2000 was compared with the previous 5 years (1992-96). Thirty-seven children (< 16 yrs) and 49 young adults (16-50 yrs) with congenital aortic valve stenosis underwent one of these treatments: percutaneous balloon dilatation (PBD), aortic valve commissurotomy, aortic valve replacement and the Ross operation. The Ross operation was performed in 16 patients, mean age 24.5 yrs (range 9-46 yrs) with a bicuspid stenotic aortic valve, 7/10 adults with calcifications, 2/10 adults with previous aortic valve commissurotomy, 4/6 children with aortic regurgitation following PBD, and 1/6 children who had had a previous aortic valve replacement with a prosthetic valve and aortic root enlargement. RESULTS: PBD was followed by death in two neonates (fibroelastosis); all other children survived PBD. Although there were no deaths, PBD in adults was recently abandoned, owing to unfavourable results. Aortic valve commissurotomy showed good results in children (no deaths). Aortic valve replacement, although associated with good results (no deaths), has been recently abandoned in children in favour of the Ross operation. Over a mean follow-up of 16 months (2-40 months) all patients are asymptomatic following Ross operation, with no echocardiographic evidence of aortic valve regurgitation in 10/16 patients and with trivial regurgitation in 6/16 patients. CONCLUSIONS: The approach now for children and young adults with congenital aortic valve stenosis should be as follows: (1) PBD is the first choice in neonates and infants; (2) Aortic valve commissurotomy is the first choice for children, neonates and infants after failed PBD; (3) The Ross operation is increasingly used in children after failed PBD and in young adults, even with a calcified aortic valve.

HIV-positive adolescents face a number of challenges in dealing with their disease and its treatment. In this qualitative study, twenty-nine HIV-positive adolescents aged 13 to 20 years (22 girls), who live in Switzerland, were asked, in a semi-structured interview (duration of 40-110 minutes), to describe their perceptions and experiences with the disease itself and with therapeutic adherence. While younger adolescents most often thought of their disease as fate, older adolescents usually knew that they had received it through vertical transmission, although the topic appeared to be particularly difficult to discuss for those living with their HIV-positive mothers. Based on their attending physician's assessment, 18 subjects were judged highly adherent, 4 fairly and 7 poorly adherent. High adherence appeared linked with adequate psychological adjustment and effective coping mechanisms, as well as with the discussion and adoption of explicit medication-taking strategies. The setting and organisation of health care teams should allow for ongoing discussions with HIV-positive adolescents that focus on their perceptions of their disease, how they cope with it and with the treatment, and how they could improve their adherence.

Concussion, a frequent injury in sports, is rarely evoked and often trivialized in children and teenagers. Knowledge of the diverse symptoms and signs to seek for is essential to an appropriate and secure management. The initial treatment relies on cognitive and physical rest followed by a progressive return to school and subsequently sport activities. The aim of this article is to review an injury whose prognosis is generally favourable, but whose rare complications can prove dramatic.

This study compared the relative effectiveness of two computerized remedial reading programs in improving the reading word recognition, rate, and comprehension of adolescent readers demonstrating significant and longstanding reading difficulties. One of the programs involved was Autoskill Component Reading Subskills Program, which provides instruction in isolated letters, syllables, and words, to a point of rapid automatic responding. This program also incorporates reading disability subtypes in its approach. The second program, Read It Again. Sam, delivers a repeated reading strategy. The study also examined the feasibility of using peer tutors in association with these two programs. Grade 9 students at a secondary vocational school who satisfied specific criteria with respect to cognitive and reading ability participated. Eighteen students were randomly assigned to three matched groups, based on prior screening on a battery of reading achievement tests. Two I I groups received training with one of the computer programs; the third group acted as a control and received the remedial reading program offered within the regular classroom. The groups met daily with a trained tutor for approximately 35 minutes, and were required to accumulate twenty hours of instruction. At the conclusion of the program, the pretest battery was repeated. No significant differences were found in the treatment effects of the two computer groups. Each of the two treatment groups was able to effect significantly improved reading word recognition and rate, relative to the control group. Comprehension gains were modest. The treatment groups demonstrated a significant gain, relative to the control group, on one of the three comprehension measures; only trends toward a gain were noted on the remaining two measures. The tutoring partnership appeared to be a viable alternative for the teacher seeking to provide individualized computerized remedial programs for adolescent unskilled readers. Both programs took advantage of computer technology in providing individualized drill and practice, instant feedback, and ongoing recordkeeping. With limited cautions, each of these programs was considered effective and practical for use with adolescent unskilled readers.

This study examined the bone mineral content (BMC) in young women with Adolescent Idiopathic Scoliosis (AIS), treated with a brace (27.9 ±21.6 months, for 18.0±5.4 h/d) during adolescence (AIS-B, n = 15, 25.6 ±5.8 yrs), versus women with AIS but no treatment (AIS-NB, n = 15, 24.0 ±4.0 yrs), and women without AIS (C, n = 19, 23.5 ±3.8 yrs). After controlling for lean body mass, calcium and vitamin D daily intake, and strenuous physical activity, femoral neck BMC was lower in the AIS-B compared with AIS-NB and C (all p’s < .05). In summary, women with AIS, braced during their growing years are characterized by low lower limb BMC. However, the lack of a relationship between brace treatment duration and BMC, suggests that bracing was not the likely mechanism.

The purpose of this project was to raise awareness surrounding child and adolescent mental health in an effort to reduce preconceived stigmas in relation to this specialized field. This project presented a literature review of the current state of child and adolescent mental health in Canada today, including the prevalence and several treatment options for young people confronting mental health challenges. Consideration of the powerful role of the education system upon youth with mental health issues became evident, specifically regarding early identification and prevention. A needs assessment was conducted to gather feedback from the clinical practitioners of a Section 23 classroom within a Southern Ontario hospital. This assessment was used to develop an informational and pedagogical workshop resource to extend practitioner understanding of this pertinent issue and support the social and emotional needs of young people confronting mental heath challenges. Results of the assessment indicated the significant need for such a workshop resource, and these responses were used to guide the development of Group Chat: A Workshop to Support the Emotional and Social Needs of Youth. The latter was subsequently presented to participants, whereby evaluative questionnaires indicated the efficacy and usefulness of this workshop resource to both practitioners and students alike.

Purpose: Adolescent idiopathic scoliosis (AIS) is often associated with low bone mineral content and density (BMC, BMD). Bracing, used to manage spine curvature, may interfere with the growth-related BMC accrual, resulting in reduced bone strength into adulthood. The purpose of this study was to assess the effects of brace treatment on BMC in adult women, diagnosed with AIS and braced in early adolescence. Methods: Participants included women with AIS who: (i) underwent brace treatment (AIS-B, n = 15, 25.6 ± 5.8 yrs), (ii) underwent no treatment (AIS, n = 15, 24.0 ± 4.0 yrs), and (iii) a healthy comparison group (CON, n = 19, 23.5 ± 3.8 yrs). BMC and body composition were assessed using dual-energy X-ray absorptiometry. Differences between groups were examined using a oneway ANOVA or ANCOVA, as appropriate. Results: AIS-B underwent brace treatment 27.9 ± 21.6 months, for 18.0 ± 5.4 h/d. Femoral neck BMC was lower (p = 0.06) in AIS-B (4.54 ± 0.10 g) compared with AIS (4.89 ± 0.61 g) and CON (5.07 ± 0.58 g). Controlling for lean body mass, calcium and vitamin D daily intake, and strenuous physical activity, femoral neck BMC was statistically different (p = 0.02) between groups. A similar pattern was observed at other lower extremity sites (p < 0.05), but not in the spine or upper extremities. BMC and BMD did not correlate with duration of brace treatment, duration of daily brace wear, or overall physical activity. Conclusion: Young women with AIS, especially those who were treated with a brace, have significantly lower BMC in their lower limbs compared to women without AIS. However, the lack of a relationship between brace treatment duration during adolescence and BMC during young adulthood, suggests that the brace treatment is not the likely mechanism of the low BMC.

Les progrès spectaculaires réalisés dans le traitement de la fibrose kystique font en sorte qu'un nombre de plus en plus élevé de familles comptant un adolescent atteint de ce problème de santé, peut maintenant envisager d'effectuer la transition depuis l'adolescent vers l'âge adulte. À ce jour, les recherches à ce sujet demeurent peu nombreuses, principalement en ce qui a trait à la nature des interations entre les adolescents, leurs parents et les professionnels de la santé qui préparent ces familles en vue du transfert, depuis un établissement pédiatrique vers un établissement adulte. L'appréhension de ce phénomène s'est réalisée dans une perspective constructiviste et systémique, ce qui a permis de mettre en relief certaines des multiples facettes du phénomène et de jeter un éclairage novateur sur des dynamiques entre les membres de la famille et entre ces derniers et les professionnels de la santé. Le but de cette étude de cas, de type qualitavive, était de modéliser de manière systémique, le processus de transtion pour des familles ayant un adolescent atteint de la fibrose kystique qui est en phase pré transfert, depuis l'établissement pédiatrique vers le milieu adulte. Des entretiens semi-dirigés ont été réalisés avec sept familles comptant un adolescent atteint de la fibrose kystique. De plus, un entretien de groupe a également été effectué avec une équipe interprofessionnelle oeuvrant dans une clinique qui traite des adolescents atteints de la fibrose kysitque. L'analyse qualitative des données a mené au développement d'un modèle systémique de la transition pour ces familles. Ce modèle souligne qu'en évoluant de façon parallèle, les familles et les professionnels de la santé poursuivent la même finalité de favoriser le développement de l'autonomie de l'adolescent. Ce processus de transition chez ces familles s'inscrit, par ailleurs, dans un espace-temps signifié par le transfert inter-établissements, avec peu de considération pour la souffrance parentale liée au pronostic de la maladie. Ainsi, le développement de l'autonomie est marqué par la confiance qui doit s'établir entre l'adolescent et son parent, en passant par la surveillance du parent envers l'adolescent et par la responsabilisation graduelle chez ce dernier. Cette étude propose donc une modélisation systémique de ce processus de transition qui contribue non seulement au développement du concept de la transition en sciences de la santé, mais aussi de la pratique clinique et de la recherche dans le domaine des soins à la famille aux prises avec un adolescent atteint de la fibrose kystique.

La scoliose idiopathique de l’adolescent est une déformation 3D du rachis. La littérature comporte une multitude d’études sur la prédiction de l’évolution et l’identification de facteurs de risque de progression. Pour l’instant les facteurs de risque établis sont l’amplitude de la déformation, la maturité squelettique et le type de courbure. Plusieurs autres champs ont été explorés comme les aspects génétiques, biochimiques, mécaniques, posturaux et topographiques, sans vraiment apporter beaucoup de précision à la prédiction de l’évolution. L’avancement de la technologie permet maintenant de générer des reconstructions 3D du rachis à l’aide des radiographies standard et d’obtenir des mesures de paramètres 3D. L’intégration de ces paramètres 3D dans un modèle prédictif représente une avenue encore inexplorée qui est tout à fait logique dans le contexte de cette déformation 3D du rachis. L’objectif général de cette thèse est de développer un modèle de prédiction de l’angle de Cobb à maturité squelettique à partir de l’information disponible au moment de la première visite, soit l’angle de Cobb initial, le type de courbure, l’âge osseux et des paramètres 3D du rachis. Dans une première étude, un indice d’âge osseux a été développé basé sur l’ossification de l’apophyse iliaque et sur le statut du cartilage triradié. Cet indice comporte 3 stades et le second stade, qui est défini par un cartilage triradié fermé avec maximum 1/3 d’ossification de l’apophyse iliaque, représente le moment pendant lequel la progression de la scoliose idiopathique de l’adolescent est la plus rapide. Une seconde étude rétrospective a permis de mettre en évidence le potentiel des paramètres 3D pour améliorer la prédiction de l’évolution. Il a été démontré qu’à la première visite il existe des différences pour 5 paramètres 3D du rachis entre un groupe de patients qui sera éventuellement opéré et un groupe qui ne progressera pas. Ces paramètres sont : la moyenne da la cunéiformisation 3D des disques apicaux, la rotation intervertébrale à la jonction inférieure de la courbure, la torsion, le ratio hauteur/largeur du corps vertébral de T6 et de la colonne complète. Les deux dernières études sont basées sur une cohorte prospective de 133 patients avec une scoliose idiopathique de l’adolescent suivi dès leur première visite à l’hôpital jusqu’à maturité squelettique. Une première étude a permis de mettre en évidence les différences morphologiques à la première visite entre les patients ayant progresser de plus ou moins de 6°. Des différences ont été mise en évidence pour la cyphose, l’angle de plan de déformation maximal, la rotation ntervertébrale l’apex, la torsion et plusieurs paramètres de «slenderness». Ensuite une seconde étude a permis de développer un modèle prédictif basé sur un modèle linéaire général en incluant l’indice d’âge osseux développé dans la première étude, le type de courbure, l’amplitude de l’angle de Cobb à la première visite, l’angle de déformation du plan maximale, la cunéiformisation 3D des disques T3-T4, T8-­T9, T11-­T12 et la somme des cunéiformisation 3D de tous les disques thoraciques et lombaires. Le coefficient de détermination multiple pour cette modélisation est de 0.715. Le modèle prédictif développé renforce l’importance de considérer la scoliose idiopathique dans les trois dimensions et il permettra d’optimiser la prédiction de l’évolution au moment de la première visite.

Background: Routine screening of scoliosis is a controversial subject and screening efforts vary greatly around the world. METHODS: Consensus was sought among an international group of experts (seven spine surgeons and one clinical epidemiologist) using a modified Delphi approach. The consensus achieved was based on careful analysis of a recent critical review of the literature on scoliosis screening, performed using a conceptual framework of analysis focusing on five main dimensions: technical, clinical, program, cost and treatment effectiveness. FINDINGS: A consensus was obtained in all five dimensions of analysis, resulting in 10 statements and recommendations. In summary, there is scientific evidence to support the value of scoliosis screening with respect to technical efficacy, clinical, program and treatment effectiveness, but there insufficient evidence to make a statement with respect to cost effectiveness. Scoliosis screening should be aimed at identifying suspected cases of scoliosis that will be referred for diagnostic evaluation and confirmed, or ruled out, with a clinically significant scoliosis. The scoliometer is currently the best tool available for scoliosis screening and there is moderate evidence to recommend referral with values between 5 degrees and 7 degrees. There is moderate evidence that scoliosis screening allows for detection and referral of patients at an earlier stage of the clinical course, and there is low evidence suggesting that scoliosis patients detected by screening are less likely to need surgery than those who did not have screening. There is strong evidence to support treatment by bracing. INTERPRETATION: This information statement by an expert panel supports scoliosis screening in 4 of the 5 domains studied, using a framework of analysis which includes all of the World Health Organisation criteria for a valid screening procedure.

Cognitive behaviour therapy (CBT) for young people with obsessive compulsive disorder (OCD) has become the treatment of first choice. However, the literature is largely based on studies emphasising exposure and response prevention. In this study, we report on a randomised controlled trial of CBT for young people carried out in typical outpatient clinic conditions which focused on cognitions. A randomised controlled trial compares 10 sessions of manualised cognitive behavioural treatment with a 12-week waiting list for adolescents and children with OCD. Assessors were blind to treatment allocation. 21 consecutive patients with OCD aged between 9 and 18 years were recruited. The group who received treatment improved more than a comparison group who waited for 3 months. The second group was treated subsequently using the same protocol and made similar gains. In conclusion, CBT can be delivered effectively to young people with OCD in typical outpatient settings.

Background: The aim of this study was to evaluate stimulant medication response following a single dose of methylphenidate (MPH) in children and young people with hyperkinetic disorder using infrared motion analysis combined with a continuous performance task (QbTest system) as objective measures. The hypothesis was put forward that a moderate testdose of stimulant medication could determine a robust treatment response, partial response and non-response in relation to activity, attention and impulse control measures. Methods: The study included 44 children and young people between the ages of 7-18 years with a diagnosis of hyperkinetic disorder (F90 & F90.1). A single dose-protocol incorporated the time course effects of both immediate release MPH and extended release MPH (Concerta XL, Equasym XL) to determine comparable peak efficacy periods post intake. Results: A robust treatment response with objective measures reverting to the population mean was found in 37 participants (84%). Three participants (7%) demonstrated a partial response to MPH and four participants (9%) were determined as non-responders due to deteriorating activity measures together with no improvements in attention and impulse control measures. Conclusion: Objective measures provide early into prescribing the opportunity to measure treatment response and monitor adverse reactions to stimulant medication. Most treatment responders demonstrated an effective response to MPH on a moderate testdose facilitating a swift and more optimal titration process.

Background Major depressive disorders (MDD) are a debilitating and pervasive group of mental illnesses afflicting many millions of people resulting in the loss of 110 million working days and more than 2,500 suicides per annum. Adolescent MDD patients attending NHS clinics show high rates of recurrence into adult life. A meta-analysis of recent research shows that psychological treatments are not as efficacious as previously thought. Modest treatment outcomes of approximately 65% of cases responding suggest that aetiological and clinical heterogeneity may hamper the better use of existing therapies and discovery of more effective treatments. Information with respect to optimal treatment choice for individuals is lacking, with no validated biomarkers to aid therapeutic decision-making. Methods/Design Magnetic resonance-Improving Mood with Psychoanalytic and Cognitive Therapies, the MR-IMPACT study, plans to identify brain regions implicated in the pathophysiology of depressions and examine whether there are specific behavioural or neural markers predicting remission and/or subsequent relapse in a subsample of depressed adolescents recruited to the IMPACT randomised controlled trial (Registration # ISRCTN83033550). Discussion MR-IMPACT is an investigative biomarker component of the IMPACT pragmatic effectiveness trial. The aim of this investigation is to identify neural markers and regional indicators of the pathophysiology of and treatment response for MDD in adolescents. We anticipate that these data may enable more targeted treatment delivery by identifying those patients who may be optimal candidates for therapeutic response.
