The understanding of fish immune system structure and function is essential for the development of new technologies and products to improve productivity. This is the first review on immune system of fish with Brazilian studies. Aquaculture in Brazil has shown massive growth in recent years due to methods of culture intensification. However, these procedures led to disease outbreaks, as well as the chemotherapy and the misuse of antibiotics. A viable alternative to avoid the use of chemicals and prevent economic losses is the administration of immunostimulants and prebiotcs, which act by increasing the innate immune system. In Brazil there is a lack of studies on fish immune system, except by some groups that have studied the effects of the immunostimulants administration in various species.

Pregnancy affects both maternal and fetal metabolism, and even in non-diabetic women, it exerts a diabetogenic effect. Among pregnant women, 2% to 14% develop gestational diabetes. Pregnancy can also occur in women with preexisting diabetes, which may predispose the fetus to many alterations in organogenesis, restrict growth, and the mother, to some diabetes-related complications, such as retinopathy and nephropathy, or to acceleration of the course of these complications, if they are already present. Women with gestational diabetes generally start their treatment with diet and lifestyle changes; when these changes are not enough for optimal glycemic control, insulin therapy must then be considered. Women with type 2 diabetes using oral hypoglycemic agents are advised to change to insulin therapy. Those with preexisting type 1 diabetes should start intensive glycemic control. As basal insulin analogues have frequently been used off-label in pregnant women, there is a need to evaluate their safety and efficacy. The aim of this review is to report the use of both short- and long-acting insulin analogues during pregnancy and to enable clinicians, obstetricians, and endocrinologists to choose the best insulin treatment for their patients.

[ES] El objetivo general del trabajo fue ampliar los conocimientos sobre el uso de fármacos utilizados en el tratamieno del dolor como los antiinflamatorios no esteroides (AINEs), los opioides y antimigrañosos, princpalmente para esclarecer posibles invonvenientes asociados a una utilización incorrecta. Se diseño y llevó a cabo un estudio descriptivo transversal. Mediante un cuestionario previamente elaborado, se entrevistó a 403 pacientes y se obtuvieron datos de 1378 dispensaciones entre 2009 y 2010, desde siete farmacias comunitarias. El análisis de los datos situó a los AINEs como el grupo más destacado en la población estudiada respecto a casi todas las variables que se definieron, con valores máximos en frecuencia de utilización, dispensación sin receta, efectos adversos y otros incovenientes. Su amplio consumo puede estar relacionado con muchos de los efectos adversos especificados por los pacientes encuestados y también con el aumento del uso de otros medicamentos. La información sanitaria no está adaptada a la comunidad estudiada, lo que puede magnificar los problemas e influir sobre la salud y el nivel de satisfacción, mientras la contribución de los AINEs al gasto farmacéutico cada vez está siendo más elevado.

[ES] En la última década ha crecido significativamente el uso de fármacos y diversos productos de cuidado personal en nuestra sociedad. Estos compuestos, que pertenecen a los actualmente denominados contaminantes emergentes, no son totalmente eliminados en las estaciones depuradoras de aguas residuales y pueden incorporarse al medio ambiente a través de los emisarios submarinos, las aguas de riego o los lodos usados como abonos en agricultura. Muchos de estos compuestos tienen actividad tóxica o mutagénica y además pueden sufrir bioacumulación y biomagnificación en organismos marinos. Dado que las concentraciones de estos contaminantes son muy pequeñas es imprescindible desarrollar procedimientos de extracción y preconcentración que permitan cuantificarlas. Con las metodologías que se han optimizado en el grupo de investigación, hemos conseguido detectar y determinar la presencia de muchos de estos compuestos tanto en muestras líquidas como sólidas, incluyendo emisarios submarinos, agua de mar (en playas y en mar abierto), lodos y sedimentos.

Infantile hemangiomas (IHs) are the most common benign neoplastic pathology of childhood; their natural history generally involves three phases: after the onset, which usually occurs in the first weeks of life, there is the proliferation phase where the IH reaches its maximum development and it is followed by the spontaneous involution which leads to the IH regression. The duration and the extent of these phases may vary widely even though in most of the cases the involution process begins around twelve months of life and the regression, complete or partial, is completed around seventh-ninth year of life. The majority of the IHs does not require any treatment. However, 10%-20% is likely to develop serious complications, functional impairments or aesthetic alterations and entail a timely treatment. Although there is no treatment protocol currently shared, therapies usually used in cases with a complication risk consist in: systemic or intralesional steroids as a first choice; interferon α, vincristine and/or bleomicin as second or third choice and/or surgical treatment. Propranolol, a non-selective beta-blocker, has been used for cardiovascular diseases even in childhood for decades. Since 2008 it has been widely used in the IHs treatment, although it is still "off-label". In literature there are hundreds of cases and some clinical studies that show the effectiveness and safety of this drug for this indication. Thanks to a multidisciplinary team (Dermatologists, Cardiologists, Paediatricians, and Radiologists) of S. Orsola-Malpighi Hospital, a clinical study, which has been previously approved by the ethics committee, is carried out in order to evaluate the efficacy and safety of systemic propranolol in the treatment of IHs in paediatric age. At the end of 2012, 78 patients underwent this treatment: the results we have obtained so far show a good efficacy and safety profile in agreement with the data provided by the literature.

recombinant activated factor VII (rFVIIa) is used off-label for massive bleeding. There is no convincing evidence of the benefits of this practice and the minimal effective dose is unknown. The aim of the study was to evaluate our in-house guideline recommending a low dose of 60 μg/kg for off-label use of rFVIIa.

The purpose of this investigation was to describe the use of linezolid in pediatric inpatient facilities. A retrospective multicenter survey including data from nine participating tertiary care pediatric inpatient facilities in Germany and Austria was undertaken. Data on 126 off-label linezolid treatment courses administered to 108 patients were documented. The survey comprises linezolid treatment in a broad spectrum of clinical indications to children of all age groups; the median age was 6.8 years (interquartile range 0.6-15.5 years; range 0.1-21.2 years; ten patients were older than 18 years of age but were treated in pediatric inpatient units). Of the 126 treatment courses, 27 (21%) were administered to preterm infants, 64 (51%) to pediatric oncology patients, and 5% to patients soon after liver transplantation. In 25%, the infection was related to a medical device. Linezolid iv treatment was started after intensive pre-treatment (up to 11 other antibiotics for a median duration of 14 days) and changed to enteral administration in only 4% of all iv courses. In 39 (53%) of 74 courses administered to children older than 1 week and younger than 12 years of age, the dose was not adjusted to age-related pharmacokinetic parameters. In only 17 courses (13%) was a pediatric infectious disease consultant involved in the clinical decision algorithm. Linezolid seemed to have contributed to a favorable outcome in 70% of all treatment courses in this survey. Although retrospective, this survey generates interesting data on the off-label use of linezolid and highlights several important clinical aspects in which the use of this rescue antibiotic in children might be improved.
