Although Parkinson’s disease (PD) is a complex disease for which appropriate nutrition management is important, limited evidence is currently available to support dietetic practice. Existing PD-specific guidelines do not span all phases of the Nutrition Care Process (NCP). This study aimed to document PD-specific nutrition management practice by Australian and Canadian dietitians. DAA members and PEN subscribers were invited to participate in an online survey (late 2011). Eighty-four dietitians responded (79.8% Australian). The majority (70.2%) worked in the clinical setting. Existing non-PD guidelines were used by 52.4% while 53.6% relied on self-initiated literature reviews. Weight loss/malnutrition, protein intake, dysphagia and constipation were common issues in all NCP phases. Respondents also requested more information/evidence for these topics. Malnutrition screening (82.1%) and assessment (85.7%) were routinely performed. One-third did not receive referrals for weight loss for overweight/obesity. Protein intake meeting gender/age recommendations (69.0%), and high energy/high protein diets to manage malnutrition (82.1%) were most commonly used. Constipation management was through high fibre diets (86.9%). Recommendations for spacing of meals and PD medications varied with 34.5% not making recommendations. Nutritional diagnosis (70.2%) and stage of disease (61.9%) guided monitoring frequency. Common outcome measures included appropriate weight change (97.6%) and regular bowel movements (88.1%). With limited PD-specific guidance, dietitians applied best available evidence for other groups with similar issues. Dietitians requested evidence-based guidelines specifically for the nutritional management of PD. Guideline development should focus on those areas reported as commonly encountered. This process can identify the gaps in evidence to guide future research.

Background: Malnutrition is a common problem for residents of nursing homes and long-term care hospitals. It has a negative influence on elderly residents and patients health and quality of life. Nutritional care seems to have a positive effect on elderly individuals nutritional status and well-being. Studies of Finnish elderly people s nutrition and nutritional care in institutions are scarce. Objectives: The primary aim was to investigate the nutritional status and its associated factors of elderly nursing home residents and long-term care patients in Finland. In particular, to find out, if the nursing or nutritional care factors are associated with the nutritional status, and how do carers and nurses recognize malnutrition. A further aim was to assess the energy and nutrient intake of the residents of dementia wards. A final objective was to find out, if the nutrition training of professionals leads to changes in their knowledge and further translate into better nutrition for the aged residents of dementia wards. Subjects and methods: The residents (n=2114) and patients (n=1043) nutritional status was assessed in all studies using the Mini Nutritional Assessment test (MNA). Information was gathered in a questionnaire on residents and patients daily routines providing nutritional care. Residents energy and nutrient intake (n=23; n=21) in dementia wards were determined over three days by the precise weighing method. Constructive learning theory was the basis for educating the professionals (n=28). A half-structured questionnaire was used to assess professionals learning. Studies I-IV were cross-sectional studies whereas study V was an intervention study. Results: Malnutrition was common among elderly residents and patients living in nursing homes and hospitals in Finland. According to the MNA, 11% to 57% of the studied elderly people suffered from malnutrition, and 40-89% were at risk of malnutrition, whereas only 0-16% had a good nutritional status. Resident- and patient-related factors such as dementia, impaired ADL (Activities of Daily Living), swallowing difficulties and constipation mainly explained the malnutrition, but also some nutritional care related factors, such as eating less than half of the offered food portion and not receiving snacks were also related to malnutrition. The intake of energy and some nutrients by the residents of dementia wards were lower than those recommended, although the offered food contained enough energy and nutrients. The proportion of residents receiving vitamin D supplementation was low, although there is a recommendation and known benefits for the adequate intake of vitamin D. Nurses recognized malnutrition poorly, only one in four (26.7%) of the actual cases. Keeping and analysing food diaries and reflecting on nutritional issues in small group discussions were effective training methods for professionals. The nutrition education of professionals had a positive impact on the energy and protein intake, BMIs, and the MNA scores of some residents in dementia wards. Conclusions: Malnutrition was common among elderly residents and patients living in nursing homes and hospitals in Finland. Although residents- and patient related factors mainly explained malnutrition, nurses recognized malnutrition poorly and nutritional care possibilities were in minor use. Professionals nutrition education had a positive impact on the nutrition of elderly residents. Further studies describing successful nutritional care and nutrition education of professionals are needed.

Nutrition affects bone health throughout life. To optimize peak bone mass development and maintenance, it is important to pay attention to the dietary factors that enhance and impair bone metabolism. In this study, the in vivo effects of inorganic dietary phosphate and the in vitro effects of bioactive tripeptides, IPP, VPP and LKP were investigated. Dietary phosphate intake is increased through the use of convenience foods and soft drinks rich in phosphate-containing food additives. Our results show that increased dietary phosphate intake hinders mineral deposition in cortical bone and diminishes bone mineral density (BMD) in the aged skeleton in a rodent model (Study I). In the growing skeleton (Study II), increased phosphate intake was observed to reduce bone material and structural properties, leading to diminished bone strength. Studies I and II revealed that a low Ca:P ratio has negative effects on the mature and growing rat skeleton even when calcium intake is sufficient. High dietary protein intake is beneficial for bone health. Protein is essential for bone turnover and matrix formation. In addition, hydrolysis of proteins in the gastrointestinal tract produces short peptides that possess a biological function beyond that of being tissue building blocks. The effects of three bioactive tripeptides, IPP, VPP and LKP, were assessed in short- and long-term in vitro experiments. Short-term treatment (24 h) with tripeptide IPP, VPP or LKP influenced osteoblast gene expression (Study III). IPP in particular, regulates genes associated with cell differentiation, cell growth and cell signal transduction. The upregulation of these genes indicates that IPP enhances osteoblast proliferation and differentiation. Long-term treatment with IPP enhanced osteoblast gene expression in favour of bone formation and increased mineralization (Study IV). The in vivo effects of IPP on osteoblast differentiation might differ since eating frequency drives food consumption, and protein degradation products, such as bioactive peptides, are available periodically, not continuously as in this study. To sum up, Studies I and II raise concern about the appropriate amount of dietary phosphate to support bone health as excess is harmful. Studies III and IV in turn, support findings of the beneficial effects of dietary protein on bone and provide a mechanistic explanation since cell proliferation and osteoblast function were improved by treatment with bioactive tripeptide IPP.

Some recipes for fish utilization in particular zones of Nigeria are assessed, giving details of their modes of preparation. The traditions of use of fish in the recipes shown appear to be based on the successful agricultural practices in particular zones and areas of Nigeria. This has helped to promote taste and values, since easily procured food items are cheaper for use in particular areas than others. The use of such fish items in the recipes are useful for promoting protein intake and stemming of malnutrition, especially in the rural areas, as well as helping to improve the incomes of women-in-agriculture

70-day growth trial was conducted with Heteroclarias: Heterobranchus bidorsalis X Clarias gariepinus (mean weight 0.64~c0.006g) fed diets based on various inclusion levels of Maggot Meal. The fishmeal in the control diet was replaced with maggot meals at 25%, 50%, 75% and 100% levels to supply 40% crude protein in the final diets. The trails were conducted in glass tanks (60cmx30cmx30cm). Evaluation of growth parameters and nutrient utilization of the fish was based on weight gains, protein intake, protein efficiency ratio, net protein utilization, feed conversion efficiency and carcass analysis. Best growth and feed conversion efficiency were obtained with the 75% dietary inclusion of maggot meal. There was no significant differences (P>0.055) between the group of fish on 50% and 75% dietary inclusion maggot meal in growth performance and protein efficiency ratio but, there was a significant (P<0.05) difference in the NPU (Net Protein Utilization) and protein gain between the control diet and those fed on maggot meals. There was no marked variation in the survival rate of fish on all diets

[EUS] Elikadura oso garrantzitsua da errendimendurako txapelketa batean. Horrez gain, lehiaketa batzuetan, kirolariak kategoria ezberdinetan sailkatzen dira beren pisuaren arabera, kyokushin karatean bezala. Arte martzial honetako lehiakide askok txapelketaren aurreko egunetan dieta gogorra egin behar dute, ahalik eta kategoria baxuen egoteko asmoz. Batzuetan, barauan edo afaldu gabe joaten dira hori lortzeko. Horregatik, ikerketa honetan pisua hartu eta gero eta lehiatu aurretik elikadurazko estrategia ezberdinak aztertu ziren. Kontrola egin baino lehen, karbohidratoak eta proteinak hartu behar izan zituzten partaideek, eta indarra, erresistentzia, bizkortasuna eta potentzia baloratu egin zen. Kancho Oyama gimnasioan egin ziren frogak. Horietan 12 karatekak hartu zuten parte, batzuk emakumezkoak (4) eta beste batzuk gizonezkoak (8), 18 urte baino gehiagokoak izanda. Ikerketaren emaitzak, partaide gehienak indarrean, erresistentzia eta bizkortasunean karbohidratoekin eta potentzian proteinekin hobetu egin zutela markatu zuten. Pacientes com doença renal crônica (DRC) na fase não dialítica são normalmente orientados a seguir uma dieta hipoproteica e hipossódica. Estudos nacionais e internacionais mostram que a adesão a essa dieta tem sido baixa e difícil de ser mantida, pois requer mudanças importantes no hábito alimentar. O objetivo do presente estudo foi avaliar o impacto de um programa de educação nutricional sobre a adesão à dieta hipoproteica em pacientes com DRC em tratamento conservador. Trata-se de um ensaio clínico randomizado, prospectivo com duração de 5 1,5 meses. Foram acompanhados 85 pacientes com DRC na fase não dialítica, atendidos em dois Ambulatórios de Nutrição e Doenças Renais do Hospital Universitário Pedro Ernesto. Os pacientes foram divididos de forma aleatória em 2 grupos: Intervenção (n=39) e Controle (n=46). Os pacientes do Grupo Intervenção foram submetidos a um programa de educação nutricional, além da orientação de dieta hipoproteica (0,6 a 0,75 g/kg/dia). Os pacientes do Grupo Controle foram submetidos apenas à orientação de dieta hipoproteica (0,6 a 0,75 g/kg/dia). A avaliação da adesão foi feita a partir da estimativa do consumo de proteína por recordatório alimentar de 24 horas. Adotou-se como critério de adesão apresentar ao final do estudo redução de ao menos 20% da ingestão proteica inicial. A avaliação nutricional e laboratorial foi realizada no início e no término do estudo. Os parâmetros antropométricos avaliados foram peso, estatura, dobras cutâneas do tríceps, bíceps, subescapular e supra-ilíaca e perímetro da cintura e do braço. As laboratoriais foram creatinina, uréia, potássio, fósforo, glicose e albumina no plasma e sódio e uréia na urina de 24 horas. Ao avaliar o amostra total, 51,8% dos pacientes eram do sexo masculino, com média de idade de 63,4 11,0 anos, IMC indicativo de sobrepeso (28,8 5,4 kg/m2) e filtração glomerular estimada (FGe) de 32,6 12,2 mL/mim/1,73m2. As características iniciais não diferiram entre os Grupos Intervenção e Controle. Ambos os grupos apresentaram melhora dos parâmetros laboratoriais e antropometricos, com redução significante da uréia plasmática e da glicemia no Grupo Controle (P < 0,05 vs início do estudo) e do IMC em ambos os grupos (P < 0,05 vs início do estudo). Após o período de acompanhamento, o Grupo Intervenção e o Grupo Controle apresentaram ingestão proteica significantemente diferente (0,62 0,2 vs 0,77 0,26 g/kg/dia, respectivamente). A ingestão de sódio não mudou de forma significante em ambos os grupos no inicio e término do acompanhamento. A Adesão à ingestão proteica foi observada em 74,4% do Grupo Intervenção e em 47,8% do Grupo Controle (P < 0,05). A análise de regressão logística multivariada revelou que pertencer ao Grupo Intervenção e sexo masculino se associaram com a Adesão (P <0,05), mesmo após corrigir para outras variáveis testadas. Com base nos achados desse estudo, pode-se concluir que o programa de educação nutricional foi uma ferramenta eficaz no tratamento dietoterápico do paciente com DRC na pré-diálise, pois promoveu melhora na adesão à dieta hipoproteica, além de ter promovido melhora dos parâmetros antropométricos e laboratoriais.

Introductions of exotic finfish between 1948 and 1953 are reported in this paper, with a brief reference to earlier and later introductions. Exotic fish were introduced principally to develop the potential for aquaculture in fresh and brackish waters in order to increase the availability of fish for rural communities through the biological control of aquatic vegetation. The algal feeding tilapia (Sarotherodon mossambicus) has created a new food industry in inland and brackishwaters. It has supplemented marine fishery production in a community where animal protein intake consists mainly of fish. It is also being cultured in flooded rice fields and used in the control of malaria. This excellent table fish has not had any adverse environmental impact.

Apesar de diversos estudos sobre nutrição de prematuros terem sido realizados, ainda não existe consenso sobre a melhor estratégia nutricional a ser adotada. Atualmente a taxa de crescimento dessa população não é semelhante àquela encontrada no ambiente intrauterino. O presente estudo tem por objetivo avaliar se o maior aporte proteico enteral durante a internação hospitalar promove melhora dos índices antropométricos na alta hospitalar. Realizou-se um ensaio clínico randomizado com 117 prematuros nascidos entre janeiro de 2009 e julho de 2013 com peso ≤ 1500 gramas e idade gestacional≤32 semanas em uma unidade terciária de saúde, excluídos os nascidos com malformações graves, aferindo-se os índices antropométricos ao nascimento e na alta hospitalar. Randomizou-se os prematuros por meio de sorteio em dois grupos. O grupo 1 (n=53), foi submetido a um aporte protéico enteral diário de 4,5 gramas/kg/dia, enquanto o grupo 2 (n=64), recebeu 3,5 gramas/kg/dia. Avaliou-se se a nutrição enteral com aporte protéico maior que o comumente utilizado em unidades de terapia intensiva neonatais e também descrito na literatura, promove diferenças antropométricas na alta hospitalar. Na análise dos resultados, verificou-se diferença estatisticamente significativa para retorno ao peso de nascimento (p=0,02), crescimento de escore-Z em relação ao peso de nascimento (p=0,03) e crescimento escore-Z em relação ao comprimento de nascimento (p=0,02) quando comparados o grupo 1 ao 2. Não houve diferenças estatisticamente significativas nas incidências de enterocolite necrotizante (p=0,70, RR 0,88), déficit ponderal na alta (p=0,27, RR 0,70), restrição de crescimento na alta (p= 0,39, RR 0,82) e déficit de perímetro cefálico na alta (p=0,45, RR 0,67). Concluiu-se, apesar das limitações metodológicas do estudo, que os participantes do grupo 1 apresentaram menor decréscimo de escores-Z em relação ao peso de nascimento e ao comprimento de nascimento quando comparados ao grupo 2, além de necessidade de menor tempo para recuperação do peso de nascimento. Não houve diferença entre os grupos para tempo de internação hospitalar, assim como para intercorrências de interesse (enterocolite necrotizante, déficit ponderal na alta, restrição de crescimento na alta e déficit de perímetro cefálico na alta).

Increased management attention to the fisheries tn Uganda is fuelled by five major factors:socio-economic demands, human population increase,stock depletion, biodiversity loss and,environmental degradation.Fish exports from Uganda to overseas and regional markets have rapidly increased since the mid 1990s and it is estimated that total exports are worth at least US$ 200m three quarters of the value due mostly to Nile perch exports to overseas markets.Exports to regional markets are dominated by variously processed tilapia "mukene" (Rastrineobo/a argentea),Nile perch "angara"(A/estes baremose) and cat fishes.Virtually all major water bodies contribute to this trade. A combinatilm of exports and an increased human population has seen the per capita fish consumption in Uganda drop from 15kg in the early 1990s to almost 10kg by 2005.this figure is below the WHO recommended fish protein intake of 17kg. the apparent fish gap in a liberalised economic framework has stimulated interest in commercial fish farming

The fisheries sector in Cambodia contributes 8%–12% to national GDP and 25% - 30% to agricultural GDP, with an estimated 4.5 million people involved in fishing and associated trades. Fish and other aquatic animals are important food sources, contributing an estimated national average of 60% - 70% of total animal protein intake. Of the 2013 total fish production, 550,000 metric tons were harvested from freshwater habitats, of which rice field fisheries and small-scale family fisheries contributed approximately 20%. The productivity and value of rice field fisheries to households in rural Cambodia has been highlighted in a number of previous studies. The Fisheries Administration of the Ministry of Agriculture, Forestry and Fisheries plans to increase productivity from rice field fisheries and aquaculture at an annual rate of 15% to maintain supply for a growing population. This report draws mainly on the baseline and monitoring data from the Rice Field Fisheries Enhancement Project (RFFEP) during its implementation between 2012 and 2014. Reference is also made to the Fish on Farms project to highlight the relative contribution of fish from small-scale aquaculture compared to wild-caught fish.

The fisheries sector is crucial to the Bangladeshi economy and wellbeing, accounting for 4.4% of national Gross Domestic Product (GDP) and 22.8% of agriculture sector production, and supplying ca.60% of the national animal protein intake. Fish is vital to the 16 million Bangladeshis living near the coast, a number that has doubled since the 1980s. Here we develop and apply tools to project the long term productive capacity of Bangladesh marine fisheries under climate and fisheries management scenarios, based on downscaling a global climate model, using associated river flow and nutrient loading estimates, projecting high resolution changes in physical and biochemical ocean properties, and eventually projecting fish production and catch potential under different fishing mortality targets. We place particular interest on Hilsa shad (Tenualosa ilisha), which accounts for ca.11% of total catches, and Bombay duck (Harpadon nehereus), a low price fish that is the second highest catch in Bangladesh and is highly consumed by low income communities. It is concluded that the impacts of climate change, under greenhouse emissions scenario A1B, are likely to reduce the potential fish production in the Bangladesh Exclusive Economic Zone (EEZ) by less than 10%. However, these impacts are larger for the two target species. Under sustainable management practices we expect Hilsa shad catches to show a minor decline in potential catch by 2030 but a significant (25%) decline by 2060. However, if overexploitation is allowed catches are projected to fall much further, by almost 95% by 2060, compared to the Business as Usual scenario for the start of the 21st century. For Bombay duck, potential catches by 2060 under sustainable scenarios will produce a decline of less than 20% compared to current catches. The results demonstrate that management can mitigate or exacerbate the effects of climate change on ecosystem productivity.

Only few cases of classical phenylketonuria (PKU) in premature infants have been reported. Treatment of these patients is challenging due to the lack of a phenylalanine-free amino acid solution for parenteral infusion. The boy was born at 27 weeks of gestation with a weight of 1000 g (P10). He received parenteral nutrition with a protein intake of 3 g/kg/day. On day 7 he was diagnosed with classical PKU (genotype IVS10-11G>A/IVS12+ 1G>A) due to highly elevated phenylalanine (Phe) level in newborn screening (2800 micromol/L). His maximum plasma Phe level reached 3696 micromol/L. Phe intake was stopped for 4 days. During this time the boy received intravenous glucose and lipids as well as little amounts of Phe-free formula by a nasogastric tube. Due to a deficit of essential amino acids and insufficient growth, a parenteral nutrition rich in branched-chain amino-acids and relatively poor in Phe was added, in order to promote protein synthesis without overloading in Phe. Under this regimen, Phe plasma levels normalized on day 19 when intake of natural protein was started. The boy has now a corrected age of 2 years. He shows normal growth parameters and psychomotor development. Despite a long period of highly elevated Phe levels in the postnatal period our patient shows good psychomotor development. The management of premature infants with PKU depends on the child's tolerance to enteral nutrition. It demands an intensive follow-up by an experienced team and dedicated dietician. Appropriate Phe-free parenteral nutrition would be necessary especially in case of gastro-intestinal complications of prematurity.

Cette étude vise à estimer l’apport en glutamine (Gln) alimentaire chez des athlètes soumis à un protocole de supplémentation en glutamine ainsi qu’à clarifier les informations diffusées au grand public en ce qui concerne les sources alimentaires de glutamine. Des études cliniques ont démontré que la supplémentation en glutamine pouvait réduire la morbidité et la mortalité chez des sujets en phase critique (grands brulés, chirurgie…). Le mécanisme en cause semble impliquer le système immunitaire. Cependant, les études chez les sportifs, dont le système immunitaire a de fortes chances d’être affaibli lors de périodes d’entraînement prolongées impliquant des efforts longs et intenses, n’ont pas été concluantes. Or, ces études négligent systématiquement l’apport alimentaire en glutamine, si bien qu’il est probable que les résultats contradictoires observés puissent en partie être expliqués par les choix alimentaires des sujets. Puisque la méthode conventionnelle de dosage des acides aminés dans les protéines alimentaires transforme la glutamine en glutamate, les tables de composition des aliments présentent la glutamine et le glutamate ensemble sous la dénomination « glutamate » ou « Glu », ce qui a comme conséquence de créer de l’ambiguïté. La dénomination « Glx » devrait être utilisée. Partant de la probabilité qu’un apport en Glx élevé soit un bon indicateur de l’apport en glutamine, nous avons créé un calculateur de Glx et avons évalué l’alimentation de 12 athlètes faisant partie d’une étude de supplémentation en glutamine. Nous avons alors constaté que l’apport en Glx était directement proportionnel à l’apport en protéines, avec 20,64 % ± 1,13 % de l’apport protéique sous forme de Glx. Grâce à quelques données sur la séquence primaire des acides aminés, nous avons pu constater que le rapport Gln/Glx pouvait être très variable d’un type de protéine à l’autre. Alors que le ratio molaire Gln/Glx est de ~95 % pour les α et β-gliadines, il n’est que de ~43 % pour la caséine, de ~36 % pour la β-lactoglobuline, de ~31 % pour l’ovalbumine et de ~28 % pour l’actine. Il est donc possible que certaines protéines puissent présenter des avantages par rapport à d’autres, à quantité égale de Glx.
