Estudo piloto para a avaliação da aplicabilidade de instrumento de pesquisa nas versões impressa e eletrônica sobre riscos e agravos à saúde do trabalhador. Objetiva avaliar comparativamente a aplicação do instrumento sobre riscos e danos no formato eletrônico e formato impresso em trabalhadores de enfermagem. O local do estudo foi o Centro de Material e Esterilização (CME) e a população os profissionais da unidade. Os participantes foram divididos através de sorteio em dois grupos de vinte e oito trabalhadores nomeados grupo impresso e grupo eletrônico. O primeiro grupo respondeu o questionário em formato impresso e o segundo o mesmo questionário no formato eletrônico através de uma home page. Os dados foram analisados através da estatística descritiva simples. Após a aplicação do questionário nos dois formatos, obteve-se resposta de 27 trabalhadores, sendo 20 (71,4%) do grupo impresso e 7 (36,4%) do grupo eletrônico. A aplicação do questionário revelou que no formato eletrônico não há possibilidade de item sem resposta e a análise dos dados pode ser imediata; é uma metodologia limpa e possui menor custo direto de aplicação. Entretanto houve uma taxa de resposta menor que no questionário no formato impresso. Na forma impressa, os pontos negativos foram o custo direto de aplicação mais elevado, possibilidade maior de erros, maior tempo gasto para a coleta e criação de banco de dados. Como pontos positivos tem-se a possibilidade de aplicação a trabalhadores sem experiência em informática, consequentemente apresenta maior taxa de respostas. Ambos os grupos observaram que o questionário possui boas instruções e fácil compreensão, além de curto tempo para resposta. Os trabalhadores observaram a existência de riscos físicos, ergonômicos, químicos, biológicos e de acidentes. As varizes, problemas osteomusculares e problemas oculares foram os mais relatados pelos profissionais como agravos à saúde relacionada ao trabalho. Concluiu-se que este grupo de trabalhadores ainda não está preparado para a realização de pesquisas no formato eletrônico e que a forma impressa ainda tem melhor aceitabilidade. A diferença de respostas nos grupos deveu-se a características pessoais dos respondentes e não a forma de coleta de dados. Ambas as metodologias identificaram fatores de riscos e agravos à saúde dos trabalhadores. Conclui-se que deve ser indicada a aplicação do questionário eletrônico em trabalhadores capacitados no uso da tecnologia computacional. Quanto à evidência da análise dos dados coletados, sugere-se a melhoria da planta física; contratação de profissionais capacitados; treinamento em equipe relacionado aos riscos e medidas de proteção a saúde do trabalhador.

A perda auditiva por exposição ao ruído é um problema de saúde ocupacional, não reconhecido nas escolas de música. Em Portugal, a legislação relativa a ruído ocupacional não possui indicações específicas para músicos, existindo apenas um código de conduta europeu, proveniente da Diretiva 2003/10/CE que estabelece as orientações gerais relativas a como devem ser protegidos do ruído músicos e trabalhadores de setores de entretenimento. Avaliou-se o nível sonoro contínuo equivalente (LAeq), individualmente no decorrer das atividades letivas, o que permitiu determinar o nível exposição pessoal diária ao ruído (Lex,8h) de 20 docentes de música. Paralelamente, os docentes preencheram um questionário relativo a fatores intrínsecos e individuais e todos efetuaram audiogramas tonais simples. Os dados recolhidos foram estatisticamente tratados através do programa Statistical Package for Social Sciences (SPSS) versão 21. Existem atividades letivas que implicam níveis de exposição pessoal diária ao ruído superiores ao nível de ação inferior (25%), pelo que se devem adotar medidas para sensibilizar e alertar os docentes para a adoção de medidas de proteção. A atividade dos docentes expostos a níveis de exposição pessoal diária mais elevados correspondeu a aulas de grupo e a aulas individuais, com utilização de instrumentos musicais direcionais. A manifestação de sintomatologia relevante relativa a perda auditiva (audição de zumbidos, dificuldades de perceção do diálogo e dificuldade em adormecer) e a evolução da surdez profissional, não parecem estar diretamente relacionadas com os níveis de exposição pessoal diária ao ruído nem atividades com exposição ao ruído desenvolvidas nos tempos livres.

Skin exposures to chemicals may lead, through percutaneous permeation, to a significant increase in systemic circulation. Skin is the primary route of entry during some occupational activities, especially in agriculture. To reduce skin exposures, the use of personal protective equipment (PPE) is recommended. PPE efficiency is characterized as the time until products permeate through material (lag time, Tlag). Both skin and PPE permeations are assessed using similar in vitro methods; the diffusion cell system. Flow-through diffusion cells were used in this study to assess the permeation of two herbicides, bentazon and isoproturon, as well as four related commercial formulations (Basagran(®), Basamais(®), Arelon(®) and Matara(®)). Permeation was measured through fresh excised human skin, protective clothing suits (suits) (Microchem(®) 3000, AgriSafe Pro(®), Proshield(®) and Microgard(®) 2000 Plus Green), and a combination of skin and suits. Both herbicides, tested by itself or as an active ingredient in formulations, permeated readily through human skin and tested suits (Tlag < 2 h). High permeation coefficients were obtained regardless of formulations or tested membranes, except for Microchem(®) 3000. Short Tlag, were observed even when skin was covered with suits, except for Microchem(®) 3000. Kp values tended to decrease when suits covered the skin (except when Arelon(®) was applied to skin covered with AgriSafe Pro and Microgard(®) 2000), suggesting that Tlag alone is insufficient in characterizing suits. To better estimate human skin permeations, in vitro experiments should not only use human skin but also consider the intended use of the suit, i.e., the active ingredient concentrations and type of formulations, which significantly affect skin permeation.

Within the framework of state security policy, the focus of this dissertation are the relations between how new security threats are perceived and the policy planning and bureaucratic implementation that are designed to address them. In addition, this thesis explores and studies some of the inertias that might exist in the core of the state apparatus as it addresses new threats and how these could be better managed. The dissertation is built on five thematic and interrelated articles highlighting different aspects of when new significant national security threats are detected by different governments until the threats on the policy planning side translate into protective measures within the society. The timeline differs widely between different countries and some key aspects of this process are also studied. One focus concerns mechanisms for adaptability within the Intelligence Community, another on the policy planning process within the Cabinet Offices/National Security Councils and the third focus is on the planning process and how policy is implemented within the bureaucracy. The issue of policy transfer is also analysed, revealing that there is some imitation of innovation within governmental structures and policies, for example within the field of cyber defence. The main findings of the dissertation are that this context has built-in inertias and bureaucratic seams found in most government bureaucratic machineries. As much of the information and planning measures imply security classification of the transparency and internal debate on these issues, alternative assessments become limited. To remedy this situation, the thesis recommends ways to improve the decision-making system in order to streamline the processes involved in making these decisions. Another special focus of the thesis concerns the role of the public policy think tanks in the United States as an instrument of change in the country’s national security decision-making environment, which is viewed from the perspective as being a possible source of new ideas and innovation. The findings in this part are based on unique interviews data on how think tanks become successful and influence the policy debate in a country such as the United States. It appears clearly that in countries such as the United States think tanks smooth the decision making processes, and that this model with some adaptations also might be transferrable to other democratic countries.

El trabajo de grado fue inspirado en la primera experiencia laboral del autor en Proaño Representaciones, una empresa comercializadora del Equipos de Protección Personal (EPPs) y seguridad industrial en Quito, Ecuador. Durante la primera etapa de esta experiencia se pudo ser parte de diferentes iniciativas, especialmente en el tema de la gestión de la comercialización, dentro de ellas se evidenciaba el interés de la empresas por introducirse en le mercado de la seguridad ocupacional en los países vecinos, Colombia y Perú. De la mano de lo anterior, se le presento a la empresa una propuesta donde se planteaba hacer una investigación de mercados en el sector de la salud ocupacional en las principales ciudades de Colombia, Bogotá y Medellín, la iniciativa en un principio tenia dos frentes, la intervención en los canales de distribución (pequeños distribuidores y fabricantes) y por otro lado la participación en contrataciones estatales. Pero a lo largo del desarrollo de la iniciativa se replanteo la iniciativa, los dos ejes no podían ser trabajados simultáneamente, se decidió que fueran pasos a seguir, ósea, en un principio intervenir los canales de distribución en Bogotá y Medellín, y después la participación en la contracción estatal. Durante el desarrollo de este trabajo de grado se tuvo la oportunidad de ser parte de círculos de aprendizaje especializados, visitas a empresas protagonistas del sector como por ejemplo ARSEG, todo lo anterior se llevo a cabo tanto en la ciudad de Bogotá y Medellín. Igualmente desarrollar esta iniciativa permitió un acercamiento a entidades de gobierno como lo fue el Ministerio del Trabajo.

O tema sobre a convivência familiar e comunitária como um direito de crianças e adolescentes, à primeira vista, se mostra bastante abstrato e ocupando ao mesmo tempo um lugar-comum. A sociedade aceita facilmente o fato de que toda criança tem direito a nascer e crescer em um lar saudável, protegido e seguro. Entretanto, muitas são as crianças que enfrentam situações de extrema vulnerabilidade pessoal e social, tendo que se afastar de suas famílias de origem para viver em situação de abrigamento, como medida protetiva. Este artigo tem como objetivo principal, a partir do que está preconizado no Plano Nacional de Promoção, Proteção e Defesa do Direito de Crianças e Adolescentes à Convivência Familiar e Comunitária e em leis anteriores (Constituição Federal Brasileira de 1988 e na Lei n. 8069/90 - Estatuto da Criança e do Adolescente), apresentar uma análise sobre os desafios e transformações que serão imprescindíveis para que esse direito seja efetivado, abordando os principais aspectos que envolvem as responsabilidades das políticas sociais brasileiras, com ênfase na educação pública.

Introduction: Falls among older adults is a public health problem, therefore it is necessary preventive actions, however the adherence is the major problem faced by practitioners and researchers working on falls prevention programs. Objective: To evaluate the variables related to the adherence to falls prevention programs among the elderly enrolled in a Basic Health Unit (BHU). Methods: Was performed an observational cross-sectional analytical study. All elderly registered in a BHU and able to ambulate independently were invited to participate in a falls prevent program. The Elderly who Adhered to the Program (EAP) were evaluated at BHU; and the Elderly Not Adhered to the Program (ENAP) were identified and assessed at home. The assessment for both groups was performed using an evaluation form containing personal data, measures and clinical scales to assess cognitive status, balance, mobility, fear of falling, handgrip strength. Data were analyzed with SPSS 20.0. In addition to this assessment, the ENAP underwent a semi structured interview, in which we used the qualitative approach based on the figure of the Collective Subject Discourse. Results: The study included 222 elderly, 111 EAP and 111ENAP, most aged between 70 and 79 years (48.2%), female (68.5%), married (52.3%) and illiterate (47.7%). Consolidated as protective factors for adherence, worst rates of physical activity (p = 0.001), balance (p = 0.010) and cognition (p = 0.007). The interview of ENAP identified two themes: "Local implementation of programs for the prevention of falls" and "Relationship between BHU and the elderly health care," and found that the elderly who did not adhere were unable to displace and did not mention that primary care programs are related to health care in elderly. Conclusions: Elderly who do not adhere to the program differ from elderly who adhere as worst indices of cognition, balance and physical activity which implies greater risk of falling; and they were unable to participate in falls prevention program and by to be caregiver and showed displacement difficult

Os objetivos do trabalho foram: quantificar as exposições dérmicas e respiratórias potenciais proporcionadas ao tratorista em pulverizações de agrotóxicos, na cultura de goiaba, com o turbopulverizador; avaliar a eficiência de duas vestimentas de proteção individual; avaliar a segurança e classificar estas condições de trabalho em seguras ou inseguras para cada agrotóxico considerado. A exposição dérmica potencial proporcionada ao tratorista pela condição de trabalho foi de 3.807,3 mL de calda/7h. A eficiência do conjunto de proteção individual Agro Light foi de 96,7 %, e a do Azeredo, 96,2 %. A exposição dérmica potencial nas regiões do corpo mais expostas do tratorista à pulverização, em ordem decrescente, foram os pés, os braços, as coxas+pernas-frente e o tronco-atrás. A exposição dérmica não-controlada pelos conjuntos de proteção individual foi igualmente distribuída em todas as regiões do corpo do tratorista devido aos controles das exposições dérmicas que proporcionaram às regiões mais expostas do corpo. Foram classificadas como seguras as pulverizações de três dos 17 agrotóxicos considerados e sete com o uso das vestimentas de proteção individual. As demais foram classificadas como inseguras.

OBJECTIVE: To evaluate the accidents with biological material among medical students interning in a trauma emergency room and identify key related situations, attributed causes and prevention. METHODS: we conducted a study with a quantitative approach. Data were collected through a questionnaire applied via internet, with closed, multiple-choice questions regarding accidents with biological material. The sample comprised 100 students. RESULTS: thirty-two had accidents with biological material. Higher-risk activities were local anesthesia (39.47%), suture (18.42%) and needle recapping (15.79%). The main routes of exposure to biological material were the eyes or mucosa, with 34%, and syringe needle puncture, with 45%. After contamination, only 52% reported the accident to the responsible department. CONCLUSION: The main causes of accidents and routes of exposure found may be attributed to several factors, such as lack of training and failure to use personal protective equipment. Educational and preventive actions are extremely important to reduce the incidence of accidents with biological materials and improve the conduct of post-exposure. It is important to understand the main causes attributed and situations related, so as general and effective measures can be applied.

Public health efforts were initiated in the United States with legislative actions for enhancing food safety and ensuring pure drinking water. Some additional policy initiatives during the early 20th century helped organize and coordinate relief efforts for victims of natural disasters. By 1950's the federal government expanded its role for providing better health and safety to the communities, and its disaster relief activities became more structured. A rise in terrorism related incidents during the late 1990's prompted new proactive policy directions. The traditional policy and program efforts for rescue, recovery, and relief measures changed focus to include disaster preparedness and countermeasures against terrorism.^ The study took a holistic approach by analyzing all major disaster related policies and programs, in regard to their structure, process, and outcome. Study determined that United States has a strong disaster preparedness agenda and appropriate programs are in place with adequate policy support, and the country is prepared to meet all possible security challenges that may arise in the future. The man-made disaster of September 11th gave a major thrust to improve security and enhance preparedness of the country. These new efforts required large additional funding from the federal government. Most existing preparedness programs at the local and national levels are run with federal funds which is insufficient in some cases. This discrepancy arises from the fact that federal funding for disaster preparedness programs at present are not allocated by the level of risks to individual states or according to the risks that can be assigned to critical infrastructures across the country. However, the increased role of the federal government in public health affairs of the states is unusual, and opposed to the spirit of our constitution where sovereignty is equally divided between the federal government and the states. There is also shortage of manpower in public health to engage in disaster preparedness activities, despite some remarkable progress following the September 11th disaster.^ Study found that there was a significant improvement in knowledge and limited number of studies showed improvement of skills, increase in confidence and improvement in message-mapping. Among healthcare and allied healthcare professionals, short-term training on disaster preparedness increased knowledge and improved personal protective equipment use with some limited improvement in confidence and skills. However, due to the heterogeneity of these studies, the results and interpretation of this systematic review may be interpreted with caution.^

Background. MRSA (methicillin-resistant Staphylococcus aureus) is a multi-drug resistant bacterium that is quite prevalent in social environments where close person-to-person contact and crowding are an issue. In dental settings, the likelihood of transmission of MRSA may be higher than among other healthcare practitioners because of the close proximity between a patient's nose (where MRSA colonizes) and the field of procedure (the mouth) to the dental professional. Objective. To estimate the prevalence of MRSA nasal colonization among dental professionals (dentists and dental hygienists) in the Greater Houston Metropolitan Area, Texas, and analyze its associations with demographic, professional and personal protective equipment-related variables. Methods. 800 dental professionals (400 dentists and 400 dental hygienists) were randomly selected in the Greater Houston Metropolitan Area. Multiple waves of nasal swab kits and a self-administered questionnaire were mailed to increase the response rate of the study population. The swabs were cultured on chromagenic agar growth medium and bacterial growth results were evaluated after 18 hours. Positively selected bacterial colonies were confirmed as MRSA by further culturing these isolated bacteria on blood agar plates. Associations between positive nasal swabs and self-reported professional practice patterns, personal protective equipment use and demographics were analyzed using multiple logistic regression. Main Results. Completed questionnaires and nasal swabs were received from 496 study participants (68%). Fourteen cultures were positive for MRSA (4.2% among dentists and 1.6% among dental hygienists, p=0.07). After adjusting for gender, dental hygienists had a significantly lower prevalence of nasal colonization of MRSA as compared to dentists (OR: 0.20, 95% CI: 0.05–0.75). No other significant associations or interactions were found. Conclusion. The prevalence of nasal colonization with MRSA among dentists is similar to that reported for health care workers in general, whereas prevalence among dental hygienists is only slightly above that of the general population (1%). Differences in practice patterns and use of personal protective equipment did not explain this difference in this study, and was possibly due either to residual confounding or unexplored risk factors. Increased prevalence of MRSA among dentists warrants further investigation as to the reason for the increased rate and to allow implementation of measures to avoid transmission and progression to disease. ^
