The influenza virus has been a challenge to science due to its ability to withstand new environmental conditions. Taking into account the development of virus sequence databases, computational approaches can be helpful to understand virus behavior over time. Furthermore, they can suggest new directions to deal with influenza. This work presents triplet entropy analysis as a potential phylodynamic tool to quantify nucleotide organization of viral sequences. The application of this measure to segments of hemagglutinin (HA) and neuraminidase (NA) of H1N1 and H3N2 virus subtypes has shown some variability effects along timeline, inferring about virus evolution. Sequences were divided by year and compared for virus subtype (H1N1 and H3N2). The nonparametric Mann-Whitney test was used for comparison between groups. Results show that differentiation in entropy precedes differentiation in GC content for both groups. Considering the HA fragment, both triplet entropy as well as GC concentration show intersection in 2009, year of the recent pandemic. Some conclusions about possible flu evolutionary lines were drawn. © 2013 Elsevier B.V.

Despite the severity of pneumonia in patients with pandemic influenza A infection (H1N1), no validated risk scores associated with H1N1 pneumonia were tested. In this prospective observational study, we analyzed data of consecutive patients in our emergency room, hospitalized because of pneumonia between July and August 2009 in a public hospital in Brazil. The following pneumonia scoring systems were applied: the SMART-COP rule; the Pneumonia Severity Index; and the CURB-65 rule. Of 105 patients with pneumonia, 53 had H1N1 infection. Among them, only 9.5% that had a low risk according to SMART-COP were admitted to ICU, compared with 36.8% of those with the Pneumonia Severity Index score of 1-2 and 49% of those with CURB-65 score of 0-1. The SMART-COP had an accuracy of 83% to predict ICU admission. The SMART-COP rule presented the best performance to indicate ICU admission in patients with H1N1 pneumonia. Introducción: Considerado como grupo de riesgo específico en la estrategia de inmunización contra la gripe A (H1N1), el colectivo de trabajadores sanitarios ha sido objeto de este estudio desde la perspectiva de sus actitudes y creencias hacia la inmunización, con especial énfasis en la influencia de las fuentes de información para tomar la decisión de vacunarse. Métodos: Estudio observacional de carácter transversal dirigido a trabajadores sanitarios en activo de la provincia de Alicante y realizado mediante cuestionario cara a cara a una muestra aleatoria por cuotas según categoría profesional en trabajadores de hospitales y centros de salud. Resultados: Las fuentes de información difieren entre subgrupos: los médicos utilizaron revistas científicas y/o congresos, las enfermeras la obtuvieron a través de Sanidad y otras enfermeras, el resto de trabajadores optaron por la televisión y/o el médico de familia. De los 3 colectivos estudiados, los médicos son los que menos sensación de gravedad han percibido frente a la gripe A (H1N1) (59,4%), son los que más confían en la vacuna (42,3%), los que más la recomiendan (44,4%), los que mejor han seguido las recomendaciones para evitar el contagio (93%) y los más vacunados (18,3%). El 75,5% de los sanitarios valoró la información recibida como regular, mala o muy mala. La totalidad admitió que se creó alarma social. Discusión: El éxito de futuras campañas de inmunización contra la gripe en personal sanitario podría incrementarse si fueran diseñadas actividades informativas segmentadas y orientadas a cada subgrupo del colectivo, adecuando la estrategia y mejorando la calidad de la información.

OBJECTIVE To explore social equity, health planning, regulatory and ethical dilemmas in responding to a pandemic influenza (H5N1) outbreak, and the adequacy of protocols and standards such as the International Health Regulations (2005). APPROACH This paper analyses the role of legal and ethical considerations for pandemic preparedness, including an exploration of the relevance of cross-jurisdictional and cross-cultural perspectives in assessing the validity of goals for harmonisation of laws and policies both within and between nations. Australian and international experience is reviewed in various areas, including distribution of vaccines during a pandemic, the distribution of authority between national and local levels of government, and global and regional equity issues for poorer countries. CONCLUSION This paper finds that questions such as those of distributional justice (resource allocation) and regulatory frameworks raise important issues about the cultural and ethical acceptability of planning measures. Serious doubt is cast on a 'one size fits all' approach to international planning for managing a pandemic. It is concluded that a more nuanced approach than that contained in international guidelines may be required if an effective response is to be constructed internationally. IMPLICATIONS The paper commends the wisdom of reliance on 'soft law', international guidance that leaves plenty of room for each nation to construct its response in conformity with its own cultural and value requirements.

Management of pandemic influenza relies on complex coordination of many different dimensions of the health and social care systems, emergency services, levels of national and local government, civil society, communications and media, and cultural expectations. Law is one small but important component of those arrangements, which touch on fundamental ethical debates. This review demonstrates that the Asian region is actively engaging those issues in diverse ways in light of their varied socioeconomic and cultural backgrounds, but scope remains for prioritising further research into these relationships.

Exposure to influenza viruses is necessary, but not sufficient, for healthy human hosts to develop symptomatic illness. The host response is an important determinant of disease progression. In order to delineate host molecular responses that differentiate symptomatic and asymptomatic Influenza A infection, we inoculated 17 healthy adults with live influenza (H3N2/Wisconsin) and examined changes in host peripheral blood gene expression at 16 timepoints over 132 hours. Here we present distinct transcriptional dynamics of host responses unique to asymptomatic and symptomatic infections. We show that symptomatic hosts invoke, simultaneously, multiple pattern recognition receptors-mediated antiviral and inflammatory responses that may relate to virus-induced oxidative stress. In contrast, asymptomatic subjects tightly regulate these responses and exhibit elevated expression of genes that function in antioxidant responses and cell-mediated responses. We reveal an ab initio molecular signature that strongly correlates to symptomatic clinical disease and biomarkers whose expression patterns best discriminate early from late phases of infection. Our results establish a temporal pattern of host molecular responses that differentiates symptomatic from asymptomatic infections and reveals an asymptomatic host-unique non-passive response signature, suggesting novel putative molecular targets for both prognostic assessment and ameliorative therapeutic intervention in seasonal and pandemic influenza.

OBJECTIVES: This study aimed at investigating whether data from medical teleconsultations may contribute to influenza surveillance. METHODS: International Classification of Primary Care 2nd Edition (ICPC-2) codes were used to analyse the proportion of teleconsultations due to influenza-related symptoms. Results were compared with the weekly Swiss Sentinel reports. RESULTS: When using the ICPC-2 code for fever we could reproduce the seasonal influenza peaks of the winter seasons 07/08, 08/09 and 09/10 as depicted by the Sentinel data. For the pandemic influenza 09/10, we detected a much higher first peak in summer 2009 which correlated with a potential underreporting in the Sentinel system. CONCLUSIONS: ICPC-2 data from medical teleconsultations allows influenza surveillance in real time and correlates very well with the Swiss Sentinel system.

Une préoccupation grandissante face aux risques biologiques reliés aux maladies infectieuses est palpable tant au niveau international que national ou provincial. Des maladies émergentes telles que le SRAS ou la grippe A/H1N1 ont amené une prise en charge des risques pandémiques et à l’élaboration de mesures d’urgence pour maîtriser ces risques : développer une culture de sécurité est devenu une priorité de recherche de l’OMS. Malgré tout, peu d’écrits existent face à cette volonté de sécuriser la santé et le bien-être par toute une série de dispositifs au sein desquels les discours occupent une place importante en matière de culture de sécurité face aux risques biologiques. Une réflexion sociopolitique était nécessaire pour les infirmières qui sont aux premières loges en dispensant des soins à la population dans une perspective de prévention et de contrôle des infections (PCI) dans laquelle elles se spécialisent. Dès lors, ce projet avait pour buts d’explorer la perception du risque et de la sécurité face aux maladies infectieuses auprès des infirmières cliniciennes et gestionnaires québécoises; d’explorer plus spécifiquement l'existence ou l'absence de culture de sécurité dans un centre de santé et de services sociaux québécois (CSSS); et d’explorer les discours en présence dans le CSSS en matière de sécurité et de risques biologiques face aux maladies infectieuses et comment ces discours de sécurité face aux risques biologiques se traduisent dans le quotidien des infirmières. Les risques biologiques sont perçus comme identifiables, mesurables et évitables dans la mesure où les infirmières appliquent les mesures de préventions et contrôle des infections, ce qui s’inscrit dans une perspective positiviste du risque (Lupton, 1999). La gestion de ces risques se décline au travers de rituels de purification et de protection afin de se protéger de toute maladie infectieuse. Face à ces risques, une culture de sécurité unique est en émergence dans le CSSS dans une perspective de prévention de la maladie. Toutefois, cette culture de sécurité désirée est confrontée à une mosaïque de cultures qui couvrent différentes façons d’appliquer ou non les mesures de PCI selon les participants. La contribution de cette recherche est pertinente dans ce nouveau domaine de spécialité que constituent la prévention et le contrôle des infections pour les infirmières québécoises. Une analyse critique des relations de pouvoir tel qu’entendu par Foucault a permis de soulever les questions de surveillance infirmière, de politique de l’aveu valorisée, de punition de tout écart à l’application rigoureuse des normes de PCI, de contrôle de la part des cadres infirmiers et d’assujettissement des corps relevant des mécanismes disciplinaires. Elle a permis également de documenter la présence de dispositifs de sécurité en lien avec la tenue de statistiques sur les patients qui sont répertoriés en tant que cas infectieux, mais également en termes de circulation des personnes au sein de l’établissement. La présence d’un pouvoir pastoral est perceptible dans la traduction du rôle d’infirmière gestionnaire qui doit s’assurer que ses équipes agissent de la bonne façon et appliquent les normes de PCI privilégiées au sein du CSSS afin de réguler les taux d’infections nosocomiales présents dans l’établissement. En cas de non-respect des mesures de PCI touchant à l’hygiène des mains ou à la vaccination, l’infirmière s’expose à des mesures disciplinaires passant de l’avertissement, la relocalisation, l’exclusion ou la suspension de l’emploi. Une culture du blâme a été décrite par la recherche d’un coupable au sein de l’institution, particulièrement en temps de pandémie. Au CSSS, l’Autre est perçu comme étant à l’origine de la contamination, tandis que le Soi est perçu comme à l’abri de tout risque à partir du moment où l’infirmière respecte les normes d’hygiène de vie en termes de saines habitudes alimentaires et d’activité physique. Par ailleurs, les infirmières se doivent de respecter des normes de PCI qu’elles connaissent peu, puisque les participantes à la recherche ont souligné le manque de formation académique et continue quant aux maladies infectieuses, aux risques biologiques et à la culture de sécurité qu’elles considèrent pourtant comme des sujets priorisés par leur établissement de santé. Le pouvoir produit des effets sur les corps en les modifiant. Cette étude ethnographique critique a permis de soulever les enjeux sociopolitiques reliés aux discours en présence et de mettre en lumière ce que Foucault a appelé le gouvernement des corps et ses effets qui se capillarisent dans le quotidien des infirmières. Des recherches ultérieures sont nécessaires afin d’approfondir ce champ de spécialité de notre discipline infirmière et de mieux arrimer la formation académique et continue aux réalités infectieuses cliniques.

El nuevo orden internacional derivado de la Guerra Fría se caracterizó por la multiplicación de nuevas amenazas a la seguridad y la construcción de bloques regionales con el propósito de enfrentarlas. Esta investigación plantea que bajo tales circunstancias, en América del Norte, fue adoptada una agenda ampliada y profundizada en materia de seguridad que permitió articular las seguridades económica, militar y la bioseguridad. En este sentido, la configuración de dicha agenda fue posible gracias a la adopción de una retórica neoliberal de seguridad económica desde la puesta en marcha del Tratado de Libre Comercio en 1994, la cual luego del 11 de septiembre de 2001 fue articulada con la agenda de seguridad militar propuesta por el gobierno estadounidense en materia de lucha antiterrorista, que a su turno permitió la adopción de una retórica y unas medidas extraordinarias en materia de bioseguridad, motivada por los ataques bioterroristas con ántrax en EE.UU., el brote de SARS en Canadá y la pandemia de AH1N1 en México.

As infecções respiratórias de etiologia viral constituem um problema alarmante de Saúde Pública, sendo responsáveis pelo elevado e constante aumento dos índices de morbimortalidade registados no Mundo associados ao vírus influenza. O presente estudo teve como objetivo avaliar a prevalência dos anticorpos IgG e IgM em soros de utentes com requisições para análises serológicas ao vírus influenza A e B. Os utentes foram atendidos entre 1 de Abril de 2009 e 30 de Abril de 2011. Outro objectivo foi determinar a epidemiologia do vírus pandémico A (H1N1) 2009 nos indivíduos com sintomatologia de gripe durante o período entre Julho de 2009 e Julho de 2010 utilizando a técnica de RT-PCR em amostras de exsudado (ou aspirado) nasofaríngeo. Tendo por base o universo de amostragem de 981 indivíduos, constatou-se que 10,7 e 8,2% da população analisada apresenta valores positivos de anticorpos IgM e IgG indicativos de infecção pelo vírus influenza A e B, respectivamente. Constatou-se, também, que entre os 1934 indivíduos submetidos a procedimentos de diagnóstico laboratorial para a detecção de infecção pelo vírus pandémico A (H1N1) 2009, cerca de 747 (38,6%) estavam infectados. Verificou-se que, a população mais jovem foi mais susceptível à infecção pelo vírus influenza A (H1N1) 2009. Isto difere da típica época de gripe sazonal, na qual as pessoas mais idosas estão mais propensas a tornarem-se infectadas e a desenvolver doença grave por influenza A e/ou B. A prevalência de gripe na RAM é reduzida – um dos aspectos plausíveis que justifiquem esta afirmação poderá dever-se às características genéticas da população da RAM estudada. Embora seja de elevada relevância salientar que o Programa Regional de Vacinação (PRV) da RAM tem alcaado reconhecimento nacional e internacional devido às excelentes taxas de cobertura vacinal, fruto da atitude entre cidadãos e profissionais de saúde.

Neste debate, historiadores latino-americanos comparam a pandemia de gripe de 1918-1919 com a que varre o continente em 2009, sobretudo as experiências de México, Argentina e Brasil. Analisam as estratégias adotadas nos dois momentos, com ênfase em isolamento, vigilância em portos e aeroportos, intervenções nas cidades. Comparam a atuação dos Estados nacionais e governos locais, a posição dos médicos e dos meios de comunicação e o comportamento das populações, especialmente no tocante ao medo e à morte. Analisam o desempenho das estruturas de assistência às populações e as medidas terapêuticas e profiláticas recomendadas por órgãos públicos de saúde, por interesses privados ligados à venda de medicamentos e pelas medicinas populares e caseiras. O debate trata, ainda, da influência que a experiência de 1918 teve sobre as avaliações da crise atual, bem como do legado que deixará para o futuro.

In 1970, searching for the interspecies transmission of influenza viruses led to the first study on influenza viruses in domestic animals. Birds and mammals, including human beings, are their natural hosts; however, other animals may also play a role in the virus epidemiology. The objective was to investigate the incidence of influenza viruses in adult dogs raised in rural (9, 19.56%) and urban (37, 80.43%) areas in the state of Sao Paulo, Brazil. Dog serum samples were examined for antibodies to influenza viruses by the hemagglutination inhibition (HI) test using the corresponding antigens from the circulating viruses in Brazil. Dogs from rural areas presented antibodies to influenza A H3N2, and influenza A H7N7 and H3N8. In rural areas, dog sera displayed mean titers as 94.37, 227.88, 168.14, 189.62 HIU/25 mu L for subtypes H1N1, H3N2, H7N7, H3N8, respectively. About 84% and 92% of dogs from urban areas exhibited antibodies to human influenza A H1N1 and H3N2, respectively, with statistical difference at p < 0.05 between the mean titers of antibodies to H1N1 and H3N2. About 92% and 100% were positive for H7N7 and H3N8, respectively. In dogs from urban areas, the mean titers of antibodies against influenza A H1N1, H3N2, H7N7 and H3N8, were 213.96, 179.42, 231.76, 231.35 HIU/25 mu L respectively. The difference among them was not statistically significant at p > 0.05. In conclusion, these dogs were positive for both human and equine influenza viruses. The present study suggests the first evidence that influenza viruses circulate among dogs in Brazil.
