In this study, we demonstrate that the prototype B. breve strain UCC2003 possesses specific metabolic pathways for the utilisation of lacto-N-tetraose (LNT) and lacto-N-neotetraose (LNnT), which represent the central moieties of Type I and Type II human milk oligosaccharides (HMOs), respectively. Using a combination of experimental approaches, the enzymatic machinery involved in the metabolism of LNT and LNnT was identified and characterised. Homologs of the key genetic loci involved in the utilisation of these HMO substrates were identified in B. breve, B. bifidum, B. longum subsp. infantis and B. longum subsp. longum using bioinformatic analyses, and were shown to be variably present among other members of the Bifidobacterium genus, with a distinct pattern of conservation among human-associated bifidobacterial species.

OBJECTIVE: Necrotising enterocolitis (NEC) causes significant mortality in premature infants. The involvement of the innate immune system in the pathogenesis of NEC remains unclear. M-, L- and H-ficolins recognize microorganisms and activate the complement system, but their role in host defense is largely unknown. This study investigated whether ficolin concentrations are associated with NEC. STUDY DESIGN: Case-control study including 30 premature infants with NEC and 60 controls. M-, L- and H-ficolins were measured in cord blood using time-resolved immunofluorometric assays. Multivariate logistic regression was performed. RESULTS: Of the 30 NEC cases (median gestational age, 29.5 weeks), 12 (40%) were operated and 4 (13%) died. No difference regarding ficolin concentration was found when comparing NEC cases versus controls (p>0.05). However, infants who died of NEC had significantly lower M-ficolin cord blood concentrations than NEC survivors (for M-ficolin <300ng/ml; multivariate OR 12.35, CI 1.03-148.59, p=0.048). In the entire study population, M-, L- and H-ficolins were positively correlated with gestational age (p<0.001) and birth weight (p<0.001). Infants with low M-ficolin required significantly more often mechanical ventilation after birth multivariate (OR 10.55, CI 2.01-55.34, p=0.005). CONCLUSIONS: M-, L- and H-ficolins are already present in cord blood and increase with gestational age. Low cord blood concentration of M-ficolin was associated with higher NEC-associated fatality and with increased need for mechanical ventilation. Future studies need to assess whether M-ficolin is involved in multiorgan failure and pulmonary disease.

Introducción. La leche humana es el primer alimento escogido para alimentar a los recién nacidos (RN), especialmente a los prematuros (RNPT y a los de peso muy bajo (RNMBP). Cuando esta leche no está disponible, deben ser recetadas fórmulas especializadas, que promuevan un crecimiento y desarrollo adecuados. El uso de los hidrolizados proteicos (HP) en la UTI neonatal ha sido promovido, debido al riesgo potencial que los RNPT/MBP presentan, por la inmadurez gastrointestinal, problemas de absorción e intolerancia alimentaria. Sin embargo, tales formulaciones no fueran elaboradas para atender las necessidades específicas de estos niños, y pueden, cuando son utilizadas, ocasionar un déficit nutricional. Objetivo. Comparar la progresión de la nutrición enteral (NE) (a través de la oferta de volumen, calorías y proteínas), la evolución ponderal, y las complicaciones clínicas entre recién nacidos de peso muy bajo (RNPMB) alimentados con fórmula para prematuros (FP) e hidrolizado proteico (HP) en la UTI neonatal. Casuística y métodos. Fueran estudiados dos grupos de RNPMB: grupo 1:13RNPMB con edad gestacional media de nacimiento de 30 semanas, peso medio de nacimiento de 1 167 gramos, que recibieron FP. Grupo 2:8 RN con edad gestacional media de nacimiento de 29 semanas, peso medio de nacimiento de 1 141 gramos, alimentados con HP. Las ofertas de volumen, la de calorías y la de proteínas fueran diariamente calculadas para todos los niños en ambos grupos. La nutrición parenteral (NP) que complementaría a la terapia nutricional en las primeras semanas, también fue calculada. Los RNMBP fueron pesados diariamente, por la mañana, en balanza digital (escala de 10 g) debidamente calibrada. El peso medio diario fue calculado hasta el dia 15 de vida. Fueron construidas curvas de crescimento, a través del polinomio de segundo grado para ambos grupos. Para el análisis estadístico se utilizó el test de medias. Resultados. ) La oferta media de volumen, calorías y proteínas fue mayos en la primera semana en el grupo 2, pero de una forma no significativa (p=0.38; 0.34 y 0.05, respectivamente). En las semanas subsequentes, no hubo diferencias significativas en la progresión de NE entre los grupos. No fueron observadas complicaciones clínicas, sólo ocurrieron dos casos de distensión abdominal en el grupo 1, pero no fueron caracterizados como enterocolitis necrosante (ECN). No hubo diferencias estadísticas relacionadas con el aumento de peso entre los dos grupos durante el periodo de estudio; sin embargo, el grupo 1 presentó una tendencia de mejor evolución. conclusión. La prograsión de la NE fue semejante con la utilización de los dos tipos de fórmulas, no feron encontradas diferencias en el aumento de peso de los grupos, y no hubo complicaciones clínicas en ninguno de ellos. No hubo ventajas en el uso del HP en la UTI neonatal; por tanto, no está indicado para RNPMB, sin disfunción intestinal, debido a que su composición nutricional no prevé las necesidades de este grupo de niños

We report the case of a one-day-old newborn infant, female, birth weight 1900 g, gestational age 36 weeks presenting with necrotizing fasciitis caused by E. coli and Morganella morganii. The newborn was allowed to fall into the toilet bowl during a domestic delivery. The initial lesion was observed at 24 hours of life on the left leg at the site of the venipuncture for the administration of hypertonic glucose solution. Despite early treatment, a rapid progression occurred resulting in a fatal outcome. We call attention to the risk presented by this serious complication in newborns with a contaminated delivery, and highlight the site of the lesion and causal agents.

Preterm infants in neonatal intensive care units frequently receive red blood cells (RBC) transfusions due to the anemia of prematurity. A number of variables related to gestational age, severity of illness and transfusion practices adopted in the neonatal unit where the neonate was born may contribute to the prescription of RBC transfusions. This study aimed to analyse the frequency and factors associated with RBC transfusions in very-low-birth-weight preterm infants. A prospective cohort of 4283 preterm infants (gestational age: 29.9 ± 2.9 weeks; birth weight: 1084 ± 275 g) carried out at 16 university hospitals in Brazil between January 2009 and December 2011 was analysed. Factors associated with RBC transfusions were evaluated using univariate and multiple logistic regression analysis. A total of 2208 (51.6%) infants received RBC transfusions (variation per neonatal unit: 34.1% to 66.4%). RBC transfusions were significantly associated with gestational age (OR: -1.098; 95%CI: -1.12 to -1.04), SNAPPE II score (1.01; 1.00-1.02), apnea (1.69; 1.34-2.14), pulmonary hemorrhage (2.65; 1.74-4.031), need for oxygen at 28 days of life (1.56; 1.17-2.08), clinical sepsis (3.22; 2.55-4.05), necrotising enterocolitis (3.80; 2.26-6.41), grades III/IV intraventricular hemorrhage (1.64; 1.05-2.58), mechanical ventilation (2.27; 1.74-2.97), use of umbilical catheter (1.86; 1.35-2.57), parenteral nutrition (2.06; 1.27-3.33), >60 days of hospitalization (5.29; 4.02-6.95) and the neonatal unit where the neonate was born. The frequency of RBC transfusions varied among neonatal intensive care units. Even after adjusting for adverse health conditions and therapeutic interventions, the neonatal unit continued to influence transfusion practices in very-low birth-weight infants.

Forty Escherichia coli strains isolated primarily from neonatal meningitis, urinary tract infections and feces were screened for the presence of virulence genes with a newly developed microarray on the array tube format. A total of 32 gene probes specific for extraintestinal as well as intestinal E. coli pathotypes were included. Eighty-eight percent of the analyzed strains were positive for the K1-specific probe on the microarray and could be confirmed with a specific antiserum against the K1 capsular polysaccharide. The gene for the hemin receptor ChuA was predominantly found in 95% of strains. Other virulence genes associated with K1 and related strains were P, S, and F1C fimbriae specific for extraintestinal E. coli, the genes for aerobactin, the alpha-hemolysin and the cytotoxic necrotizing factor. In two strains, the O157-specific catalase gene and the gene for the low-molecular-weight heat-stable toxin AstA were detected, respectively. A total of 19 different virulence gene patterns were observed. No correlation was observed between specific virulence gene patterns and a clinical outcome. The data indicate that virulence genes typical of extraintestinal E. coli are predominantly present in K1 strains. Nevertheless, some of them can carry virulence genes known to be characteristic of intestinal E. coli. The distribution and combination of virulence genes show that K1 isolates constitute a heterogeneous group of E. coli.

La enterocolitis necrotizante es de las enfermedades más frecuentes en la etapa neonatal con alta morbi mortalidad, requiriendo en muchos casos resección quirúrgica, predisponiendo de manera directa al desarrollo del síndrome de intestino corto. El síndrome de intestino corto conlleva a una serie de complicaciones nutricionales, infecciosas y metabólicas importantes no identificadas en nuestro medio y que están asociadas a factores directos e indirectos que aún no se han descrito. Las características clínicas dependerán del segmento resecado, extensión, compromiso de la válvula ileocecal, grado de adaptación intestinal y complicaciones posquirúrgicas. Su manejo es principalmente nutricional y requiere un equipo multidisciplinario para complementar las necesidades principales del tratamiento y mejorar el pronóstico. Objetivos: En este estudio de graduación para optar al título de especialista en medicina pediátrica se describirán las características epidemiológicas, clínicas, el manejo quirúrgico y de soporte en niños que se someten a resección intestinal por enterocolitis necrotizante grado III y desarrollan síndrome de intestino corto, así como su prevalencia y complicaciones frecuentes observadas en el período entre enero 2006 a diciembre 2010. Material y Métodos: El estudio se realizó a través de una revisión sistemática de expedientes clínicos de pacientes con diagnóstico de enterocolitis necrotizante. La recolección de éstos datos se realizó utilizando la base de datos del departamento de estadística del HNNBB en un periodo de 5 años. Se registraron 216 casos de enterocolitis necrotizante y de éstos se depuraron según los criterios de inclusión y exclusión hasta extraer solamente a aquellos a los cuales se les realizó cirugía intestinal y se identificaron los que cumplían con la definición de síndrome de intestino corto, siendo 17 y 6 respectivamente. Resultados: Posterior a esta recolección se aporta información actual y real a las diferentes disciplinas involucradas en el manejo del niño con SIC secundario a resección quirúrgica por ECN. Se describen los factores epidemiológicos y clínicos que mantienen relación directa o indirecta con el desarrollo de síndrome de intestino corto y también el tratamiento médico y quirúrgico que influyeron en el mismo.

Antecedentes: la enterocolitis necrosante es la enfermedad gastrointestinal médica y quirúrgica más frecuente de la etapa neonatal. Objetivo: conocer su prevalencia y sus factores de riesgo en recién nacidos de 1,000 a 1,500 g en la unidad de cuidados intensivos eonatales del Hospital Universitario Dr. José Eleuterio González. Material y métodos: estudio observacional, analítico, retrospectivo de casos y controles. Se recopilaron los datos de los expedientes clínicos de los pacientes nacidos en el Hospital Universitario durante el periodo del 1 de agosto del 2005 al 31 de julio del 2006 con peso de 1,000 a 1,500 g. Se excluyeron pacientes que nacieron en otro hospital o que tuvieran malformaciones congénitas. Se analizaron los datos mediante las pruebas de la χ2, la t de Student y razón de momios. Resutados: de los 55 pacientes que reunieron los criterios de inclusión, 18% tuvieron enterocolitis necrosante grado II o mayor. Se encontraron como factores de riesgo con significación estadística al Apgar de 0 a 6 al primer minuto, uso de presión positiva continua nasal, uso de aminofi lina y sepsis tardía (p < 0.05). Conclusiones: su prevalencia es mayor en el hospital en donde se efectuó el presente estudio que la reportada en países desarrollados o en hospitales privados de países en vías de desarrollo, pero con menor mortalidad. Los factores de riesgo con significación estadística fueron los relacionados con episodios de isquemia, reperfusión e infección.

To compare neonatal deaths and complications in infants born at 34-36 weeks and six days (late preterm: LPT) with those born at term (37-41 weeks and six days); to compare deaths of early term (37-38 weeks) versus late term (39-41 weeks and six days) infants; to search for any temporal trend in LPT rate. A retrospective cohort study of live births was conducted in the Campinas State University, Brazil, from January 2004 to December 2010. Multiple pregnancies, malformations and congenital diseases were excluded. Control for confounders was performed. The level of significance was set at p<0.05. After exclusions, there were 17,988 births (1653 late preterm and 16,345 term infants). A higher mortality in LPT versus term was observed, with an adjusted odds ratio (OR) of 5.29 (p<0.0001). Most complications were significantly associated with LPT births. There was a significant increase in LPT rate throughout the study period, but no significant trend in the rate of medically indicated deliveries. A higher mortality was observed in early term versus late term infants, with adjusted OR: 2.43 (p=0.038). LPT and early term infants have a significantly higher risk of death.

To evaluate intervention practices associated with hypothermia at both 5 minutes after birth and at neonatal intensive care unit (NICU) admission and to determine whether hypothermia at NICU admission is associated with early neonatal death in preterm infants. This prospective cohort included 1764 inborn neonates of 22-33 weeks without malformations admitted to 9 university NICUs from August 2010 through April 2012. All centers followed neonatal International Liaison Committee on Resuscitation recommendations for the stabilization and resuscitation in the delivery room (DR). Variables associated with hypothermia (axillary temperature <36.0 °C) 5 minutes after birth and at NICU admission, as well as those associated with early death, were analyzed by logistic regression. Hypothermia 5 minutes after birth and at NICU admission was noted in 44% and 51%, respectively, with 6% of early neonatal deaths. Adjusted for confounding variables, practices associated with hypothermia at 5 minutes after birth were DR temperature <25 °C (OR 2.13, 95% CI 1.67-2.28), maternal temperature at delivery <36.0 °C (OR 1.93, 95% CI 1.49-2.51), and use of plastic bag/wrap (OR 0.53, 95% CI 0.40-0.70). The variables associated with hypothermia at NICU admission were DR temperature <25 °C (OR 1.44, 95% CI 1.10-1.88), respiratory support with cold air in the DR (OR 1.40, 95% CI 1.03-1.88) and during transport to NICU (OR 1.51, 95% CI 1.08-2.13), and cap use (OR 0.55, 95% CI 0.39-0.78). Hypothermia at NICU admission increased the chance of early neonatal death by 1.64-fold (95% CI 1.03-2.61). Simple interventions, such as maintaining DR temperature >25 °C, reducing maternal hypothermia prior to delivery, providing plastic bags/wraps and caps for the newly born infants, and using warm resuscitation gases, may decrease hypothermia at NICU admission and improve early neonatal survival.

The aim of this study was to evaluate by clinical and laboratory parameters how cystic fibrosis (CF) affects growth and nutritional status of children who were undergoing CF treatment but did not receive newborn screening. A historical cohort study of 52 CF patients younger than 10 years of age were followed in a reference center in Campinas, Southeast Brazil. Anthropometric measurements were abstracted from medical records until March/2010, when neonatal screening program was implemented. Between September/2009 and March/2010, parental height of the 52 CF patients were also measured. Regarding nutritional status, four patients had Z-scores ≤ -2 for height/age (H/A) and body mass index/age (BMI/A). The following variables were associated with improved H/A ratio: fewer hospitalizations, longer time from first appointment to diagnosis, longer time from birth to diagnosis and later onset of respiratory disease. Forced vital capacity [FVC(%)], forced expiratory flow between 25-75% of FVC [FEF25-75(%)], forced expiratory volume in the first second [FEV1(%)], gestational age, birth weight and early respiratory symptoms were associated with IMC/A. Greater number of hospitalizations, diagnosis delay and early onset of respiratory disease had a negative impact on growth. Lower spirometric values, lower gestational age, lower birth weight, and early onset of respiratory symptoms had negative impact on nutritional status. Malnutrition was observed in 7.7% of cases, but 23% of children had nutritional risk.

We report a case of a 67 year-old-male patient admitted to the intensive care unit in the post-coronary bypass surgery period who presented cardiogenic shock, acute renal failure and three episodes of sepsis, the latter with pulmonary distress at the 30th post-operative day. The patient expired within five days in spite of treatment with vancomycin, imipenem, colistimethate and amphotericin B. At autopsy severe adenovirus pneumonia was found. Viral pulmonary infections following cardiovascular surgery are uncommon. We highlight the importance of etiological diagnosis to a correct treatment approach.

O objetivo de nosso estudo foi realizar tipagem molecular de 25 amostras clínicas de Candida spp, isoladas de crianças com candidemia, internadas na unidade de terapia intensiva neonatal de um Hospital Universitário entre 1998 a 2006. Dados demográficos e clínicos foram obtidos de prontuários para conhecimento dos aspectos clínicos e epidemiológicos. Identificação das leveduras foi feita por método convencional e a susceptibilidade antifúngica por método de microdiluição. O perfil genético foi determinado pela técnica de RAPD-PCR. Candida albicans (11; 44%) e Candida parapsilosis (10; 40%) foram as mais isoladas. Dezessete (68%) dos recém-nascidos tinham peso inferior a 1.500g. Prematuridade (92%), uso de cateter venoso central (100%), foram as condições de risco mais associados. Dezenove (76%) pacientes foram a óbito. Apenas uma cepa de Candida parapsilosis, mostrou ser sensível dose dependente ao fluconazol. Na análise molecular, foram observados 11 padrões genéticos distintos. Somente em dois casos foi observada relação epidemiológica, sugerindo mesma fonte de infecção.
