Fragestellung: In der vorliegenden Untersuchung wurde geprüft, ob sich neu gelistete Herztransplantationskandidaten mit ischämischer Herzinsuffizienz (IKMP) und dilatativer Kardiomyopathie (DKMP) nach Kontrolle von Krankheitsschwere und Geschlecht in psychosozialen Variablen unterscheiden. Zudem wurde untersucht, ob psychosoziale Risikofaktoren für IKMP-Patienten stärker mit der 1-Jahres-Mortalität auf der Warteliste assoziiert sind als für DKMP-Patienten. Methode: Im Rahmen der multizentrischen, prospektiv angelegten Studie „Warten auf ein neues Herz“ bearbeiteten 160 DKMP- (16 % Frauen) und 122 IKMP-Patienten (13 % Frauen) kurz nach Aufnahme auf die Warteliste einen Fragebogen, der subjektiven Gesundheitszustand, Angst, Depressivität, soziale Unterstützung, Netzwerkgröße, Ärger, Ärgerausdruck und wartezeitspezifische Belastungen erfasste. Medizinische Daten zum Zeitpunkt der Listung und Veränderungen im Wartelistenstatus im 1-Jahres-Follow-Up wurden von Eurotransplant bereitgestellt. Ergebnisse: IKMP-Patienten waren im Vergleich mit DKMP-Patienten älter, häufiger ehemalige Raucher, hatten häufiger vorherige Herzoperationen erlebt, litten seltener unter einer Erregungsleitungsstörung, wurden seltener mit Aldosteronantagonisten sowie häufiger mit Katecholaminen behandelt. Nach Kontrolle dieser Variablen und unter Einbezug des Geschlechts berichteten nur Männer mit IKMP höhere Angstwerte und mehr Anger-In als Männer mit DKMP. Ein höheres Mortalitätsrisiko für IKMP-Patienten ein Jahr nach Aufnahme auf die Warteliste konnte nicht belegt werden. Auch zeigte sich keine Interaktion zwischen psychosozialer Belastung und Grunderkrankung hinsichtlich der Mortalität. Niedrige emotionale Unterstützung ging unabhängig von der Grunderkrankung mit einem dreifach erhöhten Mortalitätsrisiko einher. Fazit: Männer mit IKMP sind stärker durch negative Emotionen belastet als Männer mit DKMP. Eine weitere Risikogruppe stellen Personen mit niedriger emotionaler Unterstützung dar. Psychosoziale Risikofaktoren sollten daher bereits bei Aufnahme auf die Warteliste erfasst und Betreuungsangebote gezielt auf diese beiden Gruppen abgestimmt werden.

BACKGROUND Posttraumatic Stress Disorder (PTSD) may occur in patients after exposure to a life-threatening illness. About one out of six patients develop clinically relevant levels of PTSD symptoms after acute myocardial infarction (MI). Symptoms of PTSD are associated with impaired quality of life and increase the risk of recurrent cardiovascular events. The main hypothesis of the MI-SPRINT study is that trauma-focused psychological counseling is more effective than non-trauma focused counseling in preventing posttraumatic stress after acute MI. METHODS/DESIGN The study is a single-center, randomized controlled psychological trial with two active intervention arms. The sample consists of 426 patients aged 18 years or older who are at 'high risk' to develop clinically relevant posttraumatic stress symptoms. 'High risk' patients are identified with three single-item questions with a numeric rating scale (0 to 10) asking about 'pain during MI', 'fear of dying until admission' and/or 'worrying and feeling helpless when being told about having MI'. Exclusion criteria are emergency heart surgery, severe comorbidities, current severe depression, disorientation, cognitive impairment and suicidal ideation. Patients will be randomly allocated to a single 45-minute counseling session targeting either specific MI-triggered traumatic reactions (that is, the verum intervention) or the general role of psychosocial stress in coronary heart disease (that is, the control intervention). The session will take place in the coronary care unit within 48 hours, by the bedside, after patients have reached stable circulatory conditions. Each patient will additionally receive an illustrated information booklet as study material. Sociodemographic factors, psychosocial and medical data, and cardiometabolic risk factors will be assessed during hospitalization. The primary outcome is the interviewer-rated posttraumatic stress level at three-month follow-up, which is hypothesized to be at least 20% lower in the verum group than in the control group using the t-test. Secondary outcomes are posttraumatic stress levels at 12-month follow-up, and psychosocial functioning and cardiometabolic risk factors at both follow-up assessments. DISCUSSION If the verum intervention proves to be effective, the study will be the first to show that a brief trauma-focused psychological intervention delivered within a somatic health care setting can reduce the incidence of posttraumatic stress in acute MI patients. AIM This study assessed the mental health of parents of children with inflammatory bowel disease (IBD), compared their mental health with age-matched and gender-matched references and examined parental and child predictors for mental health problems. METHODS A total of 125 mothers and 106 fathers of 125 children with active and inactive IBD from the Swiss IBD multicentre cohort study were included. Parental mental health was assessed by the Symptom Checklist 27 and child behaviour problems by the Strengths and Difficulties Questionnaire. Child medical data were extracted from hospital records. RESULTS While the mothers reported lower mental health, the fathers' mental health was similar, or even better, than in age-matched and gender-matched community controls. In both parents, shorter time since the child's diagnosis was associated with poorer mental health. In addition, the presence of their own IBD diagnosis and child behaviour problems predicted maternal mental health problems. CONCLUSIONS Parents of children with IBD may need professional support when their child is diagnosed, to mitigate distress. This, in turn, may help the child to adjust better to IBD. Particular attention should be paid to mothers who have their own IBD diagnosis and whose children display behaviour problems.

INTRODUCTION The pentasaccharide fondaparinux is widely approved for prophylaxis and treatment of thromboembolic diseases and therapy of acute coronary syndrome. It is also used off-label in patients with acute, suspected or antecedent heparin-induced thrombocytopenia (HIT). The aim of this prospective observational cohort study was to document fondaparinux' prescription practice, tolerance and therapy safety in a representative mixed German single-centre patient cohort. PATIENTS AND METHODS Between 09/2008 - 04/2009, 231 consecutive patients treated with fondaparinux were enrolled. Medical data were obtained from patient's records. The patients were clinically screened for thrombosis (Wells score), sequelae of HIT (4T's score), and bleeding complications (ISTH-criteria) and subjected to further assessment (i.e. sonography, HIT-diagnostics), if necessary. The mortality rate was assessed 30 days after therapy start. RESULTS Overall, 153/231 patients had a prophylactic, 74/231 patients a therapeutic, and 4/231 patients a successive prophylactic/therapeutic indication. In 11/231 patients fondaparinux was used due to suspected/antecedent HIT, in 5/231 patients due to a previous cutaneous delayed-type hypersensitivity to heparins. Other indications were rare. Three new/progressive thromboses were detected. No cases of HIT, major bleedings, or fatalities occurred. CONCLUSIONS Fondaparinux was well tolerated and was safe in prophylaxis and therapy; prescriptions mostly followed the current approval guidelines and were rarely related to HIT-associated indications (<5% of prescriptions), which is in contrast to previous study results in the U.S. (>94% of prescriptions were HIT-associated). A trend towards an individualised fondaparinux use based on the compound's inherent properties and the patients' risk profiles, i.e., antecedent HIT, bone fractures, heparin allergy, was observed.

Infant mortality as a problematic situation has been recognized for some 130 years in one form or another. It has undergone various changes in its empirical dimensions relative to whom we study within the population, what we study--low birth-weight vs. pre-term births--and how we study it--whether demographically or medically. An analysis of the process by which the condition was raised by claims makers as an intolerable situation among America's urban residents reveals that demographic and medical data were sparse. Nonetheless, a judgement about the meaning and significance of the condition was made, and that interpretation led to the promulgation of systems to both document and address the condition as it has come to be defined.^ This investigation depicts the historical context and natural history of infant mortality as one of a number of social problems that came to be defined through the interplay among groups and individuals making claims and how their claims came to the public policy agenda as worthy of collective resources--who won, who lost and why. The process of social definition focuses attention on the claims makers and the ways they contrast the meaning, origins and remedies for this troubling condition. The historical context becomes the frame of reference for understanding the actions of the claims makers and the meaning and significance they attached to the problem.^ We purport that "context" provides a closer reality than disjoined "value free" accounts. Context provides the evidence for the definition, who participated in the process, why and by what means.^ The role of women in the definitional process reveals the differences in approaches utilized by the women of the settlement house reform movement and African-American women working at the grass-roots. Much of the work done by these two groups provided options to the problem's remedy; however, their differences paved the way to our current (principally medically-oriented) definition and its inherent limitations. ^

The origins for this work arise in response to the increasing need for biologists and doctors to obtain tools for visual analysis of data. When dealing with multidimensional data, such as medical data, the traditional data mining techniques can be a tedious and complex task, even to some medical experts. Therefore, it is necessary to develop useful visualization techniques that can complement the expert’s criterion, and at the same time visually stimulate and make easier the process of obtaining knowledge from a dataset. Thus, the process of interpretation and understanding of the data can be greatly enriched. Multidimensionality is inherent to any medical data, requiring a time-consuming effort to get a clinical useful outcome. Unfortunately, both clinicians and biologists are not trained in managing more than four dimensions. Specifically, we were aimed to design a 3D visual interface for gene profile analysis easy in order to be used both by medical and biologist experts. In this way, a new analysis method is proposed: MedVir. This is a simple and intuitive analysis mechanism based on the visualization of any multidimensional medical data in a three dimensional space that allows interaction with experts in order to collaborate and enrich this representation. In other words, MedVir makes a powerful reduction in data dimensionality in order to represent the original information into a three dimensional environment. The experts can interact with the data and draw conclusions in a visual and quickly way.

The use of data mining techniques for the gene profile discovery of diseases, such as cancer, is becoming usual in many researches. These techniques do not usually analyze the relationships between genes in depth, depending on the different variety of manifestations of the disease (related to patients). This kind of analysis takes a considerable amount of time and is not always the focus of the research. However, it is crucial in order to generate personalized treatments to fight the disease. Thus, this research focuses on finding a mechanism for gene profile analysis to be used by the medical and biologist experts. Results: In this research, the MedVir framework is proposed. It is an intuitive mechanism based on the visualization of medical data such as gene profiles, patients, clinical data, etc. MedVir, which is based on an Evolutionary Optimization technique, is a Dimensionality Reduction (DR) approach that presents the data in a three dimensional space. Furthermore, thanks to Virtual Reality technology, MedVir allows the expert to interact with the data in order to tailor it to the experience and knowledge of the expert.

El presente Trabajo Fin de Grado (TFG) surge de la necesidad de disponer de tecnologías que faciliten el Procesamiento de Lenguaje Natural (NLP) en español dentro del sector de la medicina. Centrado concretamente en la extracción de conocimiento de las historias clínicas electrónicas (HCE), que recogen toda la información relacionada con la salud del paciente y en particular, de los documentos recogidos en dichas historias, pretende la obtención de todos los términos relacionados con la medicina. El Procesamiento de Lenguaje Natural permite la obtención de datos estructurados a partir de información no estructurada. Estas técnicas permiten un análisis de texto que genera etiquetas aportando significado semántico a las palabras para la manipulación de información. A partir de la investigación realizada del estado del arte en NLP y de las tecnologías existentes para otras lenguas, se propone como solución un módulo de anotación de términos médicos extraídos de documentos clínicos. Como términos médicos se han considerado síntomas, enfermedades, partes del cuerpo o tratamientos obtenidos de UMLS, una ontología categorizada que agrega distintas fuentes de datos médicos. Se ha realizado el diseño y la implementación del módulo así como el análisis de los resultados obtenidos realizando una evaluación con treinta y dos documentos que contenían 1372 menciones de terminología médica y que han dado un resultado medio de Precisión: 70,4%, Recall: 36,2%, Accuracy: 31,4% y F-Measure: 47,2%.---ABSTRACT---This Final Thesis arises from the need for technologies that facilitate the Natural Language Processing (NLP) in Spanish in the medical sector. Specifically it is focused on extracting knowledge from Electronic Health Records (EHR), which contain all the information related to the patient's health and, in particular, it expects to obtain all the terms related to medicine from the documents contained in these records. Natural Language Processing allows us to obtain structured information from unstructured data. These techniques enable analysis of text generating labels providing semantic meaning to words for handling information. From the investigation of the state of the art in NLP and existing technologies in other languages, an annotation module of medical terms extracted from clinical documents is proposed as a solution. Symptoms, diseases, body parts or treatments are considered part of the medical terms contained in UMLS ontology which is categorized joining different sources of medical data. This project has completed the design and implementation of a module and the analysis of the results have been obtained. Thirty two documents which contain 1372 mentions of medical terminology have been evaluated and the average results obtained are: Precision: 70.4% Recall: 36.2% Accuracy: 31.4% and F-Measure: 47.2%.

Objective: To develop a standard weight descriptor that can be used for estimation of patient size for obese patients. Patients and methods: Data were available from 3849 patients: 2839 from oncology patients (index data set) and 1010 from general medical patients (validation data set). The patients had a wide range of age (16-100 years), weight (25-165kg) and body mass index (BMI) [12-52 kg/m(2)] in both data sets. From the normal-weight patients in the oncology data set, an equation for male and female patients was developed to predict their normal weight as the sum of the lean body mass and normal fat body mass. The equations were evaluated by predicting the weight of patients in the general medical data set who had a normal BMI (30 kg/m(2)).

This thesis proposes a novel graphical model for inference called the Affinity Network,which displays the closeness between pairs of variables and is an alternative to Bayesian Networks and Dependency Networks. The Affinity Network shares some similarities with Bayesian Networks and Dependency Networks but avoids their heuristic and stochastic graph construction algorithms by using a message passing scheme. A comparison with the above two instances of graphical models is given for sparse discrete and continuous medical data and data taken from the UCI machine learning repository. The experimental study reveals that the Affinity Network graphs tend to be more accurate on the basis of an exhaustive search with the small datasets. Moreover, the graph construction algorithm is faster than the other two methods with huge datasets. The Affinity Network is also applied to data produced by a synchronised system. A detailed analysis and numerical investigation into this dynamical system is provided and it is shown that the Affinity Network can be used to characterise its emergent behaviour even in the presence of noise.

The aim of this thesis was to describe and evaluate visual acuity, macular edema and patientreported outcomes (PRO) following anti-VEGF treatment of diabetic macular edema (DME) patients in a real-world setting. Using a longitudinal study design, a cohort of DME patients was followed from baseline to 1 year after treatment start. Data were collected from two eye clinics at two county hospitals. Social background characteristics, medical data and PRO were measured before treatment initiation, at four month and after 1 year. A total of 57 patients completed the study. Mean age was 69 years and the sample was equally distributed regarding sex. At baseline, the patients described their general health as low. One year after treatment initiation, 30 patients had improved visual acuity and 27 patients had no improvement in visual acuity. The patients whose visual acuity improved reported an improvement in several subscales in patient-reported outcome measures (PROM), which was in contrast to the group that experienced a decline in visual acuity, where there was no improvement in PROM. Outcomes from the study can be useful for developing and providing relevant information and support to patients undergoing this treatment.

Aims To examine objective visual acuity measured with ETDRS, retinal thickness (OCT), patient reported outcome and describe levels of glycated hemoglobin and its association with the effects on visual acuity in patients treated with anti-VEGF for visual impairment due to diabetic macular edema (DME) during 12 months in a real world setting. Methods In this cross-sectional study, 58 patients (29 females and 29 males; mean age, 68 years) with type 1 and type 2 diabetes diagnosed with DME were included. Medical data and two questionnaires were collected; an eye-specific (NEI VFQ-25) and a generic health-related quality of life questionnaire (SF-36) were used. Results The total patient group had significantly improved visual acuity and reduced retinal thickness at 4 months and remains at 12 months follow up. Thirty patients had significantly improved visual acuity, and 27 patients had no improved visual acuity at 12 months. The patients with improved visual acuity had significantly improved scores for NEI VFQ-25 subscales including general health, general vision, near activities, distance activities, and composite score, but no significant changes in scores were found in the group without improvements in visual acuity. Conclusions Our study revealed that anti-VEGF treatment improved visual acuity and central retinal thickness as well as patient-reported outcome in real world 12 months after treatment start.
