Background: Providing support for research is one of the key issues in the ongoing attempts to improve Primary Care. However, when patient care takes up a significant part of a GP's time, conducting research is difficult. In this study we examine the working conditions and profile of GPs who publish in three leading medical journals and propose possible remedial policy actions. Findings: The authors of all articles published in 2006 and 2007 in three international Family Medicine journals - Annals of Family Medicine, Family Practice, and Journal of Family Practice - were contacted by E-mail. They were asked to complete a questionnaire investigating the following variables: availability of specific time for research, time devoted to research, number of patients attended, and university affiliation. Only GPs were included in the study. Three hundred and ten relevant articles published between 2006 and 2007 were identified and the authors contacted using a survey tool. 124 researchers responded to our questionnaire; 45% of respondents who were not GPs were excluded. On average GPs spent 2.52 days per week and 6.9 hours per day on patient care, seeing 45 patients per week. Seventy-five per cent of GPs had specific time assigned to research, on average 13 hours per week; 79% were affiliated to a university and 69% held teaching positions. Conclusions: Most GPs who publish original articles in leading journals have time specifically assigned to research as part of their normal working schedule. They see a relatively small number of patients. Improving the working conditions of family physicians who intend to investigate is likely to lead to better research results.

This paper forwards an opinion about authors' and journals' motivations for scientific writing. Personal and institutional motivations are listed and discussed and, in regard to biodiversity sciences, I propose that a nationalistic motivation is also pertinent in a biodiversity-rich country such as Brazil. Curiosity and competitiveness should be combined for better results. Finally I discuss ground-breaking science within a post-modern perspective, and how the mere act of scientific writing might trigger both scientific and social revolutions.

Fabio Scarano (2008) shows an attractive picture of the motivations leading to the final goal of the scientific enterprise, i.e. why scientists must publish their findings. Moreover, he proposes that scientists must aim for creativity and originality through question-driven papers, rather than unenlightening descriptive ones. I agree, but I will show that this view, albeit necessary, is incomplete. The most important flaw is that he does not show how that, in order to be creative and original one needs a deep understanding of a domain of knowledge. I will argue that these qualities cannot be reached in a theoretical vacuum. It must be remembered that the scientific enterprise is a complex cognitive process. One can only advance, learn and understand from the springboard of what one already knows. The improvement of established theories or the proposition of new ones can only be possible through a deep analysis, synthesis and integration of accepted scientific knowledge. This is only possible through the scrutiny of the concepts, propositions and predictions of accepted theories. Going deeper into Scarano's ideas, I propose that to further our comprehension of nature and to give a basis for the generation of knowledge, Brazilian ecologists should look for a specific set of question-driven papers. These are what I will call the 'why-question' papers. Only why-question driven papers can provide accounts which advance scientific knowledge and foster explanations of the mechanisms behind ecological processes.

Tässä pro gradu -tutkielmassa selvitettiin, miten aikakauslehden eli emobrändin tuttuus vaikuttaa halukkuuteen osallistua verkkopalvelun eli brändilaajennuksen sisällöntuotantoon joukkoistamalla. Kysymys liittyy muuttuneeseen tilanteeseen median kulutuksessa. Teknologia on kehittynyt, paperisten mediatuotteiden kysyntä vähentynyt ja aikakauslehdet kehittävät digitaalisia palvelujaan. Medialla kuitenkaan ole yksinoikeutta sisällöntuotantoon. Kuka tahansa voi tehdä ja julkaista sisältöä. Monet mediat ovat ottaneet lukijat mukaan sisällöntuotantoon. Aiempien tutkimusten perusteella emobrändin tuttuus vaikuttaa myönteisesti asi-akkaiden halukkuuteen käyttää brändilaajennusta. Se vaikuttaa myös ostopäätökseen ja lisää luottamusta verkkopalvelua kohtaan. Tässä tutkimuksessa tuote oli tuttu niille, jotka ovat lehden tilaajia. Lukijalähtöistä sisältöä ovat mm. blogit, verkkokeskustelut, lukijoiden tarinat, runot, kuvat, kilpailut ja kyselyt. Tutkielman aineisto kerättiin verkkokyselyn avulla ja siihen vastasi 437 vastaajaa. Tulosten perusteella emobrändin tuttuus vaikuttaa myönteisesti tilaajien halukkuuteen käyttää aikakauslehden verkkopalvelua. Tilaajat käyvät palvelussa muita käyttäjiä useammin ja osallistuvat tai haluaisivat osallistua ei-tilaajia innokkaammin sisällöntuotantoon. He ovat myös ei-tilaajia kiinnostuneempia lukijalähtöisestä sisällöstä.

With the new age of Internet of Things (IoT), object of everyday such as mobile smart devices start to be equipped with cheap sensors and low energy wireless communication capability. Nowadays mobile smart devices (phones, tablets) have become an ubiquitous device with everyone having access to at least one device. There is an opportunity to build innovative applications and services by exploiting these devices’ untapped rechargeable energy, sensing and processing capabilities. In this thesis, we propose, develop, implement and evaluate LoadIoT a peer-to-peer load balancing scheme that can distribute tasks among plethora of mobile smart devices in the IoT world. We develop and demonstrate an android-based proof of concept load-balancing application. We also present a model of the system which is used to validate the efficiency of the load balancing approach under varying application scenarios. Load balancing concepts can be apply to IoT scenario linked to smart devices. It is able to reduce the traffic send to the Cloud and the energy consumption of the devices. The data acquired from the experimental outcomes enable us to determine the feasibility and cost-effectiveness of a load balanced P2P smart phone-based applications.

Recent advances in Information and Communication Technology (ICT), especially those related to the Internet of Things (IoT), are facilitating smart regions. Among many services that a smart region can offer, remote health monitoring is a typical application of IoT paradigm. It offers the ability to continuously monitor and collect health-related data from a person, and transmit the data to a remote entity (for example, a healthcare service provider) for further processing and knowledge extraction. An IoT-based remote health monitoring system can be beneficial in rural areas belonging to the smart region where people have limited access to regular healthcare services. The same system can be beneficial in urban areas where hospitals can be overcrowded and where it may take substantial time to avail healthcare. However, this system may generate a large amount of data. In order to realize an efficient IoT-based remote health monitoring system, it is imperative to study the network communication needs of such a system; in particular the bandwidth requirements and the volume of generated data. The thesis studies a commercial product for remote health monitoring in Skellefteå, Sweden. Based on the results obtained via the commercial product, the thesis identified the key network-related requirements of a typical remote health monitoring system in terms of real-time event update, bandwidth requirements and data generation. Furthermore, the thesis has proposed an architecture called IReHMo - an IoT-based remote health monitoring architecture. This architecture allows users to incorporate several types of IoT devices to extend the sensing capabilities of the system. Using IReHMo, several IoT communication protocols such as HTTP, MQTT and CoAP has been evaluated and compared against each other. Results showed that CoAP is the most efficient protocol to transmit small size healthcare data to the remote servers. The combination of IReHMo and CoAP significantly reduced the required bandwidth as well as the volume of generated data (up to 56 percent) compared to the commercial product. Finally, the thesis conducted a scalability analysis, to determine the feasibility of deploying the combination of IReHMo and CoAP in large numbers in regions in north Sweden.

IoT consists of essentially thousands of tiny sensor nodes interconnected to the internet, each one of which executes the programmed functions under memory and power limita- tions. The sensor nodes are distributed mainly for gathering data in various situations. IoT envisions the future technologies such as e-health, smart city, auto-mobiles automa- tion, construction sites automation, and smart home. Secure communication of data under memory and energy constraints is major challenge in IoT. Authentication is the first and important phase of secure communication. This study presents a protocol to authenticate resource constraint devices in physical proximity by solely using the shared wireless communication interfaces. This model of authentication only relies on the abundance of ambient radio signals to authenticate in less than a second. To evaluate the designed protocol, SkyMotes are emulated in a network environment simulated by Contiki/COOJA. Results presented during this study proves that this approach is immune against passive and active attacks. An adversary located as near as two meters can be identified in less than a second with minimal expense of energy. Since, only radio device is used as required hardware for the authentication, this technique is scalable and interoperable to heterogeneous nature of IoT.

Internet of Things or IoT is revolutionizing the world we are living in, similarly the way Internet and the web did few decades ago. It is changing how we interact with the things surrounding us. Electronic health and remote patient monitoring are the ways of utilizing these technological improvements towards the healthcare. There are many applications of IoT in eHealth such as, it will open the gate to provide healthcare to the remote areas of the world, where healthcare through traditional hospital systems cannot be provided. To connect these new eHealth IoT systems with the existing healthcare information systems, we can use the existing interoperability standards commonly used in healthcare information systems. In this thesis we implemented an eHealth IoT system based on Health Level 7 interoperability standard for continuous data transmission. There is not much previous work done in implementing the HL7 for continuous sensor data transmission. Some of the previous work was limited to sensors which are not continuous in nature and some of it is only theatrical architecture. This thesis aims to prove that it is possible to implement an eHealth IoT system by using sensors which require continues data transmission, such as respiratory sensors, and to connect it with the existing eHealth information system semantically by using HL7 interoperability standard. This system will be beneficial in implementing eHealth IoT systems for those patients, who requires continuous healthcare personal monitoring. This includes elderly people and patients, whose health need to be monitored constantly. To implement the architecture, HL7 v2.5 is selected due to its ease of implementation and low size. We selected some open source technologies because of their open licenses and large developer community. We will also review the most efficient technology available in every layer of eHealth IoT system and will propose an efficient system.

Abstract: There is a lot of hype around the Internet of Things along with talk about 100 billion devices within 10 years time. The promise of innovative new services and efficiency savings is fueling interest in a wide range of potential applications across many sectors including smart homes, healthcare, smart grids, smart cities, retail, and smart industry. However, the current reality is one of fragmentation and data silos. W3C is seeking to fix that by exposing IoT platforms through the Web with shared semantics and data formats as the basis for interoperability. This talk will address the abstractions needed to move from a Web of pages to a Web of things, and introduce the work that is being done on standards and on open source projects for a new breed of Web servers on microcontrollers to cloud based server farms. Speaker Biography -Dave Raggett : Dave has been involved at the heart of web standards since 1992, and part of the W3C Team since 1995. As well as working on standards, he likes to dabble with software, and more recently with IoT hardware. He has participated in a wide range of European research projects on behalf of W3C/ERCIM. He currently focuses on Web payments, and realising the potential for the Web of Things as an evolution from the Web of pages. Dave has a doctorate from the University of Oxford. He is a visiting professor at the University of the West of England, and lives in the UK in a small town near to Bath.

The cloud is playing a very important role in wireless sensor network, crowd sensing and IoT data collection and processing. However, current cloud solutions lack of some features that hamper the innovation a number of other new services. We propose a cloud solution that provides these missing features as multi-cloud and device multi-tenancy relying in a whole different fully distributed paradigm, the actor model.
