In the present study, Biomphalaria snails collected from five Egyptian governorates (Giza, Fayoum, Kafr El-Sheikh, Ismailia and Damietta), as well as reference control Biomphalaria alexandrina snails from the Schistosome Biological Supply Center (SBSC) (Theodor Bilharz Research Institute, Egypt), were subjected to species-specific polymerase chain reaction (PCR) assays to identify the collected species. All of the collected snails were found to be B. alexandrina and there was no evidence of the presence of Biomphalaria glabrata. Randomly amplified polymorphic DNA (RAPD)-PCR assays showed different fingerprints with varying numbers of bands for the first generation (F1) of B. alexandrina snail populations (SBSC, Giza, Fayoum, Kafr El-Sheikh, Ismailia and Damietta). The primer OPA-1 produced the highest level of polymorphism and amplified the greatest number of specific bands. The estimated similarity coefficients among the B. alexandrina populations based on the RAPD-PCR profiles ranged from 0.56 (between SBSC and Ismailia snails) to 0.72 (between Ismailia and Kafr El-Sheikh snails). Experimental infection of the F1 of progeny from the collected snails with Schistosoma mansoni (SBSC strain) showed variable susceptibility rates ranging from 15% in the Fayoum snail group to 50.3% in SBSC snails. A negative correlation was observed between the infection rates in the different snail groups and the distances separating their corresponding governorates from the parasite source. The infection rates of the snail groups and their similarity coefficients with SBSC B. alexandrina snails were positively correlated. The variations in the rates of infection of different B. alexandrina groups with S. mansoni, as well as the differences in the similarity coefficients among these snails, are dependent not only on the geographical distribution of the snails and the parasite, but also on the genetic variability of the snails. Introduction of this variability into endemic areas may reduce the ability of the parasite to infect local hosts and consequently reduce schistosomiasis epidemiology.

Trata-se do relato do processo de implementação do sistema informatizado de gestão de materiais no Hospital Universitário da USP. O sistema foi intitulado Sistema de Gestão de Materiais (SGM). A implementação apresentou quatro fases: a escolha do modelo e da ferramenta informacional, a reestruturação do processo logístico de materiais do hospital, a reestruturação das áreas de apoio e a implantação do próprio sistema SGM. O estudo realizado no Centro Cirúrgico após a implantação do sistema demonstrou que, comparando o consumo e o estoque de materiais do SGM em relação ao Sistema Tradicional, houve uma diminuição da quantidade consumida e do custo dos materiais estocados na Unidade.

BACKGROUND: The clinical profile and outcome of nosocomial and non-nosocomial health care-associated native valve endocarditis are not well defined. OBJECTIVE: To compare the characteristics and outcomes of community-associated and nosocomial and non-nosocomial health care-associated native valve endocarditis. DESIGN: Prospective cohort study. SETTING: 61 hospitals in 28 countries. PATIENTS: Patients with definite native valve endocarditis and no history of injection drug use who were enrolled in the ICE-PCS (International Collaboration on Endocarditis Prospective Cohort Study) from June 2000 to August 2005. MEASUREMENTS: Clinical and echocardiographic findings, microbiology, complications, and mortality. RESULTS: Health care-associated native valve endocarditis was present in 557 (34%) of 1622 patients (303 with nosocomial infection [54%] and 254 with non-nosocomial infection [46%]). Staphylococcus aureus was the most common cause of health care-associated infection (nosocomial, 47%; non-nosocomial, 42%; P = 0.30); a high proportion of patients had methicillin-resistant S. aureus (nosocomial, 57%; non-nosocomial, 41%; P = 0.014). Fewer patients with health care-associated native valve endocarditis had cardiac surgery (41% vs. 51% of community-associated cases; P < 0.001), but more of the former patients died (25% vs. 13%; P < 0.001). Multivariable analysis confirmed greater mortality associated with health care-associated native valve endocarditis (incidence risk ratio, 1.28 [95% CI, 1.02 to 1.59]). LIMITATIONS: Patients were treated at hospitals with cardiac surgery programs. The results may not be generalizable to patients receiving care in other types of facilities or to those with prosthetic valves or past injection drug use. CONCLUSION: More than one third of cases of native valve endocarditis in non-injection drug users involve contact with health care, and non-nosocomial infection is common, especially in the United States. Clinicians should recognize that outpatients with extensive out-of-hospital health care contacts who develop endocarditis have clinical characteristics and outcomes similar to those of patients with nosocomial infection. Interaction between CD40, a member of the tumor necrosis factor receptor (TNFR) superfamily, and its ligand CD40L, a 39-kDa glycoprotein, is essential for the development of humoral and cellular immune responses. Selective blockade or activation of this pathway provides the ground for the development of new treatments against immunologically based diseases and malignancies. Like other members of the TNF superfamily, CD40L monomers self-assemble around a threefold symmetry axis to form noncovalent homotrimers that can each bind three receptor molecules. Here, we report on the structure-based design of small synthetic molecules with C3 symmetry that can mimic CD40L homotrimers. These molecules interact with CD40, compete with the binding of CD40L to CD40, and reproduce, to a certain extent, the functional properties of the much larger homotrimeric soluble CD40L. Architectures based on rigid C3-symmetric cores may thus represent a general approach to mimicking homotrimers of the TNF superfamily.

Introduisant à la lecture de la pensée de Claude Lefort, l'article montre que sa pensée du politique se construit entièrement dans l'horizon d'une pensée de la liberté politique, voire des conditions de celle-ci. A l'opposé d'un libéralisme d'inspiration constructiviste, la pensée de Lefort insiste sur la thèse que ce qui assure la liberté politique dans la démocratie n'est pas tant l'idée d'une propriété de soi dont jouirait chacun en vertu d'un droit naturel que l'attachement à la liberté comme forme des rapports sociaux constitutifs de l'espace démocratique.

Extracranial carotid aneurysm is a rare vascular manifestation of Behçet disease. To our knowledge, only 32 cases have been reported. This article presents a complex case of a 28-year-old man who was first treated by vein graft reconstruction. At 12 months of follow-up, a nonanastomotic false aneurysm of the vein graft occurred and was treated by interposition of prosthetic graft. Two months later, an anastomotic pseudoaneurysm between the two grafts was excluded by two stent grafts. Based on our experience and a review of the literature, we compared the outcomes of prosthetic and autologous vein reconstructions and discussed the role of carotid ligation and immunosuppressive treatment.

BACKGROUND: Radiofrequency (RF) ablation is used to obtain local control of unresectable tumors in liver, kidney, prostate, and other organs. Accurate data on expected size and geometry of coagulation zones are essential for physicians to prevent collateral damage and local tumor recurrence. The aim of this study was to develop a standardized terminology to describe the size and geometry of these zones for experimental and clinical RF. METHODS: In a first step, the essential geometric parameters to accurately describe the coagulation zones and the spatial relationship between the coagulation zones and the electrodes were defined. In a second step, standard terms were assigned to each parameter. RESULTS: The proposed terms for single-electrode RF ablation include axial diameter, front margin, coagulation center, maximal and minimal radius, maximal and minimal transverse diameter, ellipticity index, and regularity index. In addition a subjective description of the general shape and regularity is recommended. CONCLUSIONS: Adoption of the proposed standardized description method may help to fill in the many gaps in our current knowledge of the size and geometry of RF coagulation zones.

A pitaya vermelha é uma cactácea cujos frutos são de interesse comercial crescente por produtores e consumidores. No entanto, ainda há diversos aspectos sobre seu cultivo que precisam ser elucidados, proporcionando rentabilidade ao produtor. Diante disso, realizou-se o presente trabalho, que teve como objetivo obter informações quanto à propagação vegetativa desta fruteira, utilizando-se de diferentes fontes de material em função do tempo de cura. O experimento foi realizado no Ripado de Fruticultura, pertencente ao Departamento de Produção Vegetal da Faculdade de Ciências Agrárias e Veterinárias - Unesp - Câmpus de Jaboticabal-SP, utilizando estacas de plantas de pitaya de diferentes origens (planta adulta, estacas de brotações de plantas adultas recém-enraizadas e planta em início de desenvolvimento, originária de semente), submetidas a 3 períodos de cura: 0; 7 e 14 dias. As avaliações foram quanto a: enraizamento; volume de raiz; comprimento da maior raiz (cm); massas fresca e seca das raízes (gramas); número e tamanho das brotações nas estacas (cm). Foram realizadas 5 repetições, com 10 estacas cada, totalizando 150 estacas de cada material. Nas condições em que o experimento foi realizado, pode-se concluir que a estaquia deve ser realizada tão logo feita a segmentação dos cladódios e que as estacas obtidas de brotações de plantas recém-enraizadas apresentam melhores resultados.

Objetivando a verificação da existência de plântulas originadas de embrião zigótico em viveiros comerciais de mudas cítricas, realizou-se o presente trabalho, utilizando a técnica de fAFLP e quatro espécies de porta-enxertos para citros. Verificou-se uma base genética estreita entre as espécies testadas, além de grande variabilidade entre os materiais, independentemente do viveiro em que foram coletados, o que permite concluir que a seleção visual, comumente realizada nos viveiros, é ineficiente.

As ressecções hepáticas laparoscópicas vêm ganhando adeptos entre cirurgiões pois evidenciam recuperação rápida, menor permanência hospitalar e melhor resultado estético. A utilização do dispositivo laparoscópico de radiofreqüência foi pela primeira vez utilizada, com sucesso, no Brasil, para a ressecção de carcinoma hepatocelular no segmento VI, em dois pacientes cirróticos. Apesar do sangramento intraoperatório continuar sendo um grande desafio para o cirurgião durante ressecções hepáticas laparoscópicas, em ambos os casos, a exclusão vascular hepática foi dispensável e não houve necessidade de hemotransfusão. Os pacientes receberam alta hospitalar no quarto dia pós-operatório.

The chemical analysis of flesh and seed of date palm fruit (Kentichi) was evaluated. Carbohydrates were the predominant component in all studied date cultivars (~78.69-83.46 g/100g dry matter), followed by moisture content (~9.23-11.17%), along with moderate amount of fat (~0.56-7.10 g/100g dry matter), protein (~2.16-2.80 g/100g dry matter), and ash (~1.18-1.64 g/100 g dry matter). Some antioxidants (Ascorbic acid, total phenolic, total flavonoid, chlorophyll and carotenoids) were found in different values in both date fruit and seed. The physicochemical properties and antioxidant activity of both flesh and seed oil which was extracted using Hexane, Soxhlet and Modified Bligh - Dyer extraction methods were determined. The experimental results showed that temperature, different solvents and extraction time had significant effect on the yield of the date palm oil and physicochemical properties. Date Flesh oil showed an important free radical scavenging activity towards 1-1-diphenyl-2-picrylhydrazyl (DPPH) free radical.

Les lésions de la moelle épinière ont un impact significatif sur la qualité de la vie car elles peuvent induire des déficits moteurs (paralysie) et sensoriels. Ces déficits évoluent dans le temps à mesure que le système nerveux central se réorganise, en impliquant des mécanismes physiologiques et neurochimiques encore mal connus. L'ampleur de ces déficits ainsi que le processus de réhabilitation dépendent fortement des voies anatomiques qui ont été altérées dans la moelle épinière. Il est donc crucial de pouvoir attester l'intégrité de la matière blanche après une lésion spinale et évaluer quantitativement l'état fonctionnel des neurones spinaux. Un grand intérêt de l'imagerie par résonance magnétique (IRM) est qu'elle permet d'imager de façon non invasive les propriétés fonctionnelles et anatomiques du système nerveux central. Le premier objectif de ce projet de thèse a été de développer l'IRM de diffusion afin d'évaluer l'intégrité des axones de la matière blanche après une lésion médullaire. Le deuxième objectif a été d'évaluer dans quelle mesure l'IRM fonctionnelle permet de mesurer l'activité des neurones de la moelle épinière. Bien que largement appliquées au cerveau, l'IRM de diffusion et l'IRM fonctionnelle de la moelle épinière sont plus problématiques. Les difficultés associées à l'IRM de la moelle épinière relèvent de sa fine géométrie (environ 1 cm de diamètre chez l'humain), de la présence de mouvements d'origine physiologique (cardiaques et respiratoires) et de la présence d'artefacts de susceptibilité magnétique induits par les inhomogénéités de champ, notamment au niveau des disques intervertébraux et des poumons. L'objectif principal de cette thèse a donc été de développer des méthodes permettant de contourner ces difficultés. Ce développement a notamment reposé sur l'optimisation des paramètres d'acquisition d'images anatomiques, d'images pondérées en diffusion et de données fonctionnelles chez le chat et chez l'humain sur un IRM à 3 Tesla. En outre, diverses stratégies ont été étudiées afin de corriger les distorsions d'images induites par les artefacts de susceptibilité magnétique, et une étude a été menée sur la sensibilité et la spécificité de l'IRM fonctionnelle de la moelle épinière. Les résultats de ces études démontrent la faisabilité d'acquérir des images pondérées en diffusion de haute qualité, et d'évaluer l'intégrité de voies spinales spécifiques après lésion complète et partielle. De plus, l'activité des neurones spinaux a pu être détectée par IRM fonctionnelle chez des chats anesthésiés. Bien qu'encourageants, ces résultats mettent en lumière la nécessité de développer davantage ces nouvelles techniques. L'existence d'un outil de neuroimagerie fiable et robuste, capable de confirmer les paramètres cliniques, permettrait d'améliorer le diagnostic et le pronostic chez les patients atteints de lésions médullaires. Un des enjeux majeurs serait de suivre et de valider l'effet de diverses stratégies thérapeutiques. De telles outils représentent un espoir immense pour nombre de personnes souffrant de traumatismes et de maladies neurodégénératives telles que les lésions de la moelle épinière, les tumeurs spinales, la sclérose en plaques et la sclérose latérale amyotrophique.

Dans cet article je considère un récent défi à l’égalitarisme développé par Michael Huemer. Le challenge de Huemer prend la forme d’un dilemme : les égalitaristes peuvent être soit atomistes soit holistes en ce qui concerne la valeur de l’égalité. S’ils sont atomistes, alors ils doivent acceptés que l’égalité n’ait pas de valeur intrinsèque ; s’ils sont holistes, alors leur point ce vue est inconsistant avec une intuitive mais très plausible forme de conséquentialisme. Je montre que ce dilemme ne doit pas perturber les égalitaristes. Ils peuvent être holistes en ce qui concerne la valeur et adhérer en même temps au conséqeuntialisme.
