We aimed to characterize the HIV-1 epidemic of the Belgian and Colombian cohorts using an integrated approach that includes socio-demographic information, clinical data, and viral sequences, analyzed with statistical and phylogenetic approaches.

The advent of highly active antiretroviral therapy (HAART) improved HIV infection prognosis. However, adverse metabolic and morphologic effects emerged, highlighting a lack of investigation into the role of nutritional interventions among this population. The present study evaluated the impact of a nutritional counseling program on prevention of morphologic and metabolic changes in patients living with HIV/AIDS receiving HAART. A 12-month randomized clinical trial was conducted with 53 adults of both genders in use of HAART. Subjects were allocated to either an intervention group (IG) or a control group (CG). Nutritional counseling was based on the promotion of a healthy diet pattern. Anthropometrical, biochemical, blood pressure, and food intake variables were assessed on four separate occasions. Sub scapular skin-fold results showed a significant tendency for increase between time 1 (Mean IG = 14.9 mm; CG = 13.6 mm), time 3 (Mean IG = 16.7 mm; CG = 18.2 mm), and time 4 (Mean IG = 16.4 mm; CG = 17.7 mm). Lipid percentage intake presented a greater increase among controls (time 1 mean = 26.3%, time 4 mean = 29.6%) than among IG subjects (time 1 mean = 29.1%, time 4 mean = 28.9%). Moreover, participants allocated to the IG presented an increase in dietetic fiber intake of almost 10 grams. The proposed nutritional counseling program proved to be effective in improving diet by reducing fat consumption and increasing fiber intake.

Objectives: Human papillomavirus (HPV) infection is a major risk factor for cervical disease. Using baseline data from the HIV-infected cohort of Evandro Chagas Clinical Research Institute at Fiocruz, Rio de Janeiro, Brazil, factors associated with an increased prevalence of HPV were assessed. Methods: Samples from 634 HIV-infected women were tested for the presence of HPV infection using hybrid capture 11 and polymerase chain reaction. Prevalence ratios (PR) were estimated using Poisson regression analysis with robust variance. Results: The overall prevalence of HPV infection was 48%, of which 94% were infected with a high-risk HPV. In multivariate analysis, factors independently associated with infection with high-risk HPV type were: younger age (<30 years of age; PR 1.5, 95% confidence interval (CI) 1.1-2.1), current or prior drug use (PR 1.3, 95% CI 1.0-1.6), self-reported history of HPV infection (PR 1.2, 95% CI 0.96-1.6), condom use in the last sexual intercourse (PR 1.3, 95% CI 1.1-1.7), and nadir CD4+ T-cell count <100 cells/mm(3) (PR 1.6, 95% CI 1.2-2.1). Conclusions: The estimated prevalence of high-risk HPV-infection among HIV-infected women from Rio de Janeiro, Brazil, was high. Close monitoring of HPV-related effects is warranted in all HIV-infected women, in particular those of younger age and advanced immunosuppression. This descriptive study aimed to investigate the relationship between expression of immunological (TCD4 +) and virological (viral load) parameters, lipodystrophy syndrome and lifestyle variables of people living with HIV who underwent a program of physicalexercise. Initially, the sample was composed by 17 persons, recorded at the Department of Giselda Trigueiro Hospital Care (GTH), Natal-RN . With the passing of the stages of intervention (physical training program- PTP) the number of participants has changed (17, 9, 8 and 7) as the training phases (I, II, III and IV). Data collected were on total cholesterol, High Density Lipoprotein (HDL) and triglycerides. The % fat (% F), body mass index (BMI), waist-hip ratio (WHR), lean mass (LM), central fat (BF), total (TF) and peripheral (PF), weight and height were used to measure the morphological parameters. For control of variables (TCD4 +, viral load, Triglycerides, Cholesterol, HDL), the information contained in the blood tests every four months were investigated. After Phase I and II, it was used a structured interview. Then sampling was carried out considering the pre-and post-tests 1, 2, 3 and 4 (after 16, 32, 48 and 64 weeks of training, respectively). Daily, the intensity of the work was checked by the scale of perceived exertion for exercises adapted to resistance34. Procedures used were descriptive statistics (dispersion, absolute and relative frequencies, means, standard deviations and minimum and maximum values) as well as Spearman linear correlation adopting a significance level of p ≤ 0.05. Positive changes were observed for TCD4 + and viral load in all phases of the PTP. For the morphological components, the loss of central subcutaneous fat (CSF) and total subcutaneous fat (TSF) for both sexes and the decrease in % BF among women were the most dramatic results after the training phase I. For men, results were more significant to % F and LM in Phases I and II and peripheral subcutaneous fat (PSF) declined in all phases of the exercise. As for women, results were more expressive for % F and TM in the phases I and II and the PSF decreased in all phases of the exercise. Whereas for women, the waist/hip ratio (WCQ) and 0% F decreased and showed a positive association with triglycerides (WHR r * 0.82, p 0.042, r 0.88 TSF *, p 0.019 and r 1.00 ** CSF, p <0.001) and among men with limb subcutaneous fat (LSF)* r 0.65, p 0.029). The PTP provided improvement in the health, self-esteem and quality of life, proving to be a possible strategy to positively influence the expression of immunological parameters (TCD4 +) and virological (viral load) and morphological components of people living with HIV causing no deleterious effects in these parameters

OBJETIVO: No contexto de acesso universal à terapia antiretroviral, os resultados do Programa de Aids dependem da qualidade do cuidado prestado. O objetivo do estudo foi avaliar a qualidade do cuidado dos serviços ambulatoriais que assistem pacientes de Aids. MÉTODOS: Estudo realizado em sete Estados brasileiros, em 2001 e 2002. Foi avaliada a qualidade do atendimento a pacientes com Aids quanto à disponibilidade de recursos e a organização do trabalho de assistência. Um questionário com 112 questões estruturadas abordando esses aspectos, foi enviado a 336 serviços. RESULTADOS: A taxa de resposta foi de 95,8% (322). Os indicadores de disponibilidade de recursos mostram uma adequação maior do que os indicadores de organização do trabalho. Não faltam antiretrovirais em 95,5% dos serviços, os exames de CD4 e Carga Viral estão disponíveis em quantidade adequada em 59 e 41% dos serviços, respectivamente. em 90,4% dos serviços há pelo menos um profissional não médico (psicólogo, enfermeiro ou assistente social). Quanto à organização, 80% não agendavam consulta médica com hora marcada; 40,4% agendavam mais que 10 consultas médicas por período; 17% não possuíam gerentes exclusivos na assistência e 68,6% não realizavam reuniões sistemáticas de trabalho com a equipe. CONCLUSÕES: Os resultados apontam que além de garantir a distribuição mais homogênea de recursos, o programa precisa investir no treinamento e disseminação do manejo do cuidado, conforme evidenciado nos resultados da organização de trabalho.

The "HIV/Aids-Quality of Life" (HAT-Qol) is a specific multifunctional instrument used to measure the life quality of HIV infected persons. It is divided into nine domains: general activity, sexual activity, secrecy about HIV seropositivity, concern about health, financial concern, awareness about HIV, satisfaction with life, issues about medications and belief in the doctor. The current study analyzed the life quality of HIV infected individuals-who attended the DST/Aids Program in Maringa city, Parana state-regarding the use or not of antiretroviral therapy (TARV) and their demographic, epidemiological and clinical characteristics. Data were collected by retrospective analysis from 1,200 medical charts of patients registered in the program. The HAT-Qol instrument was applied before routine medical consultation. One hundred and sixty-nine patients, who had HIV infection confirmed, were divided into two groups, G1 with 118 individuals receiving antiretroviral therapy and G2 with 51 individuals who were not under this therapy.Result analysis, regarding social and demographic characteristics, revealed no difference among responses related to gender, educational degree and sexual option. Age influenced satisfaction with sexual activity and marital status. Regarding HIV awareness, the lowest response index or worst quality of life came from, respectively, men between 50 and 69 years old and patients who did not have regular partners compared with the ones who did. Additionally, it was observed that the time of diagnosis influenced general activities, HIV awareness, concern about health and financial issues, satisfaction with life and topics about medications. The variables were compared in both groups. There was no influence on the use or not of antiretroviral therapy regarding age, sexual activity, HIV diagnosis time and the domains that evaluated general activities, financial concern, awareness of HIV and satisfaction with life. In relation to time of diagnosis, there was an influence only in persons who had been diagnosed two or three years before, in which a lower quality of life was observed among individuals who were not under antiretroviral therapy. It was not possible to compare variables about medication use, HIV plasmatic viral rate ant time of diagnosis, because G2 individuals were not receiving the antiretroviral therapy. Furthermore, no comparison was made regarding marital status and HIV awareness, because there were no married individuals in G2. Thus, the analysis of the results showed that the use of antiretroviral treatment did not influence the life quality of HIV patients studied by the HAT-Qol scale.

Background: Metabolic and morphological changes associated with excessive abdominal fat, after the introduction of Antiretroviral Therapy, increase the risk of cardiovascular disease in people living with HIV/AIDS(PLWHA). Accurate methods for body composition analysis are expensive and the use of anthropometric indices is an alternative. However the investigations about this subject in PLWHA are rare, making this research very important for clinical purpose and to advance scientific knowledge. The aim of this study is to correlate results of anthropometric indices of evaluation of body fat distribution with the results obtained by Dual-energy X-Ray Absorptiometry(DEXA) , in people living with HIV/AIDS. Methods. The sample was of 67 PLWHA(39 male and 28 female), aged 43.6+7.9 years. Body mass index, conicity index, waist/hip ratio, waist/height ratio and waist/thigh were calculated. Separated by sex, each index/ratio was plotted in a scatter chart with linear regression fit and their respective Pearson correlation coefficients. Analyses were performed using Prism statistical program and significance was set at 5%. Results: The waist/height ratio presented the highest correlation coefficient, for both male (r=0.80, p<0.001) and female (r=0.87, p <001), while the lowest were in the waist/thigh also for both: male group (r=0.58, p<0.001) and female group (r=0.03, p=0.86). The other indices also showed significant positive correlation with DEXA. Conclusion: Anthropometric indices, especially waist/height ratio may be a good alternative way to be used for evaluating the distribution of fat in the abdominal region of adults living with HIV/ADIS. © 2012 Segatto et al.; licensee BioMed Central Ltd.

Estudo referente a avaliação do Programa de HIV/aids em um hospital de referência, na concepção dos usuários. O objetivo geral da pesquisa foi descrever a concepção dos usuários na unidade de internação e ambulatorial, sobre o programa de HIV/aids em um hospital universitário de referência na cidade de Belém-PA. Para o estudo foram traçados como objetivos específicos: apresentar as sugestões dos usuários do programa para a melhoria no desenvolvimento das atividades do programa e fornecer as ferramentas mínimas para a construção de um modelo de avaliação para o programa. No referencial foi utilizado o panorama da epidemia de HIV/AIDS, políticas de saúde no Brasil e a aids, e a avaliação de programas e serviços em saúde. O método e a abordagem foram qualitativo, descritivo do tipo estudo de caso. Foi utilizado um roteiro de entrevistas semi-estruturado sobre a temática. A pesquisa foi desenvolvida nas dependências do HUJBB, mais especificamente nos serviços do Programa de HIV/aids, em que foram entrevistados usuários do SAE e os internados na clínica de doenças infecto-parasitárias. Através da análise dos dados, foram divididas as falas em quatro categorias: caracterização do usuário quanto ao acesso ao serviço; a motivação do usuário pela qualidade do serviço; dificuldades frente às atividades e orientações do serviço; expectativas para a melhoria do programa. O resultado da pesquisa esclarece as restrições e qualidades do programa; a insatisfação do usuário no acesso ao serviço; o alto grau de motivação bem como a ausência da mesma; as principias dificuldades dos usuários em aderir as orientações e atividades do programa, como o medo de encarar a morte, a falta de recurso para chegar ao serviço, o preconceito vivido pelas pessoas com HIV/aids e a promiscuidade sexual; e as expectativas dos usuários para o programa: rapidez no atendimento, melhor infraestrutura. Conclui-se com as melhorias que podem contribuir para a qualidade de vida dos usuários, subsidiar demanda suficiente de profissionais para atender no programa e oferecer tratamento completo às pessoas que vivem com HIV/aids atendidas no programa do HUJBB.

Com o advento da Terapia Anti -Retroviral, a Aids assumiu características de doença crônica, em especial nos países onde o acesso aos medicamentos é efetivamente garantido. O Brasil é tomado como modelo por possuir um programa que tem dado boas respostas à epidemia. Em novembro de 1996, foi promulgada, pelo Sistema Único de Saúde (SUS), a lei que dispõe sobre a obrigatoriedade do acesso gratuito a todos os que necessitarem de medicamentos anti -retrovirais. Os resultados obtidos com o tratamento – a redução progressiva da carga viral e a manutenção e/ou restauração do funcionamento do sistema imunológico – têm sido associados a benefícios marcantes na saúde física das pessoas soropositivas e permitido que elas retomem e concretizem seus projetos de vida. Porém, o acesso universal aos medicamentos que possibilita o tratamento para portadores do HIV gratuitamente ainda enfrenta problemas de adesão. Em uma compreensão mais restrita, adesão pode ser definida como o comportamento de uma pessoa – tomar remédio, seguir uma dieta ou fazer mudanças no estilo de vida – que corresponde às recomendações da equipe de saúde. Nesse contexto, esse estudo se propõe a analisar as representações sociais de sujeitos soropositivos sobre o tratamento anti-retroviral e suas implicações no processo de adesão a este tratamento, caracterizando as imagens e os sentidos que estes sujeitos soropositivos que aderiram ou não aderiram à terapia anti -retroviral possuem sobre este tipo de tratamento e as implicações na sua vida, destacando as objetivações e as ancoragens que compõem suas representações sociais. A metodologia foi pautada nas formulações teóricas sobre pesquisa qualitativa, priorizando -se a entrevista no enfoque do Método de Explicitação do Discurso Subjacente (MEDS), realizadas na Unidade de Referência em Doenças Infecciosas e Parasitárias Especiais (UREDIPE), vinculada à Secretaria de Estado de Saúde do estado do Pará (SESPA) e no Hospital Universitário João de Barros Barreto (HUJBB), mais especificamente na Clinica de Doenças Infecciosas e Parasitárias (DIP).

A coinfecção do vírus da hepatite C (HCV) em pessoas portadoras do vírus da imunodeficiência humana (HIV) é freqüentemente observada em virtude destes vírus apresentarem similaridade em suas rotas de transmissão, principalmente no que se refere à via parenteral. No Brasil, a prevalência depende da área geográfica considerada, variando de 8,9% a 54%. Nos coinfectados, a progressão da doença pelo HCV é usualmente mais agressiva e apresenta alto nível de viremia, como também, há um risco maior de associação do HCV com a cirrose hepática e/ou hepatocarcinoma. O objetivo do presente estudo foi estimar a prevalência de HCV e fatores de risco associados à coinfecção em pessoas soropositivas para HIV na cidade de Imperatriz Maranhão. Participaram 249 pacientes soropositivos para HIV atendidos no SAE do Programa Municipal de DST/AIDS de Imperatriz do Maranhão. Foi coletado de cada voluntário 10 mL de sangue periférico para realização do teste sorológico, onde foi realizada pesquisa de anticorpos IgG HCV específicos e testes de Biologia Molecular (RT-PCR) para pesquisa do RNA viral e genotipagem. Entre os pacientes observou-se similaridade entre a frequência dos gêneros, 49% masculino e 51% feminino, com média de idade de 40 anos. Foi observado que 98% possuem baixo nível de instrução e 63% possuem renda mensal de até um salário mínimo. A soroprevalência do anti-HCV foi de 2.4% (6/249). Na comparação dos fatores de risco pesquisados entre os pacientes reagentes e não reagentes na pesquisa sorológica de anticorpos HCV específicas demonstraram que a presença de tatuagens e piercing foi o único fator que se mostrou significantes, sendo mais frequente nos reagentes. Esse foi o primeiro estudo que investiga a coinfecção HIV e HCV na cidade de Imperatriz, Maranhão e a identificação de pacientes coinfectados foi de fundamental importância para o serviço que a partir de então irá realizar o acompanhamento destes pacientes.
