Objective. To study the epidemiology of rotavirus and estimate rotavirus- associated morbidity and mortality in children <= 5 years of age in Brazil in 2004 before introducing the rotavirus vaccine in Brazil`s National Immunization Program ( Programa Nacional de Imunizacoes, PNI). Methods. To estimate rotavirus morbidity, published studies ( 1999 - 2006) addressing incidence of acute diarrhea among children <= 5 years of age and frequency of rotavirus infection among children with diarrhea in Brazil were reviewed. Diarrhea episodes were divided into three categories of severity by level of care: mild cases requiring only home- based care; moderate cases requiring a visit to an outpatient healthcare facility; and severe cases requiring hospitalization. To estimate rotavirus mortality, information on the number of registered deaths from diarrhea in children <= 5 years of age was obtained from the Mortality Information System ( Sistema de Informacao, sobre Mortalidade, SIM) of Brazil`s public healthcare system ( Sistema Unico de Sa de, SUS) and the proportion of deaths due to rotavirus was calculated. Results. Rotavirus infections were estimated to cause 3 525 053 episodes of diarrhea, 655 853 visits to outpatient healthcare facilities, 92 453 hospitalizations, and 850 deaths of children <= 5 years of age each year in Brazil. Conclusion. Rotavirus infections are an important cause of child morbidity and mortality in Brazil.

The objective of this study was to verify the possible association between the Sp1-binding site polymorphism and genital prolapse. A case-control study was conducted in 107 patients with stages III and IV genital prolapse. The control group included 209 women with stages 0 and I. The polymorphism of type I collagen Sp1-binding site was identified by amplification of the first intron of the COL1A1 gene. We did not find differences in the prevalence of the GT and TT genotypes between the groups (p=0.34), even when we grouped patients with at least one polymorphic allele (GT and TT) and compared them with patients without the polymorphic allele (GG; p=0.17) The presence of at least one vaginal delivery, family history for prolapse, and macrosomatic fetus were independent risk factors for prolapse. In conclusion, the COL1A1 Sp1-binding site was not significantly associated with genital prolapse among our study subjects.

Objective. The objective of this study was to conduct a cost-effectiveness analysis of a universal rotavirus vaccination program among children : 5 years of age in Brazil. Methods. Considering a hypothetical annual cohort of approximately 3 300 000 newborns followed over 5 years, a decision-tree model was constructed to examine the possible clinical and economic effects of rotavirus infection with and without routine vaccination of children. Probabilities and unit costs were derived from published research and national administrative data. The impact of different estimates for key parameters was studied using sensitivity analysis. The analysis was conducted from both healthcare system and societal perspectives. Results. The vaccination program was estimated to prevent approximately 1735 351 (54%) of the 3 210 361 cases of rotavirus gastroenteritis and 703 (75%) of 933 rotavirus-associated deaths during the 5-year period. At a vaccine price of 18.6 Brazilian reais (R$) per dose, this program would cost R$121 673 966 and would save R$38 536 514 in direct costs to the public healthcare system and R$71 778 377 in direct and indirect costs to society. The program was estimated to cost R$1 028 and R$1 713 per life-years saved (LYS)from the societal and healthcare system perspectives, respectively. Conclusions. Universal rotavirus vaccination was a cost-effective strategy for both perspectives. However, these findings are highly sensitive to diarrhea incidence rate, proportion of severe cases, vaccine coverage, and vaccine price.

A new tuberculosis vaccine is urgently needed. Prime-boost strategies are considered very promising and the inclusion of BCG is highly desirable. In this investigation, we tested the protective efficacy of BCG delivered in the neonatal period followed by boosters in the adult phase with a DNA vaccine containing the hsp65 gene from Mycobacterium leprae (pVAXhsp65). Immune responses were characterized by serum anti-hsp65 antibody levels and IFN-gamma and IL-5 production by the spleen. Amounts of these cytokines were also determined in lung homogenates. Protective efficacy was established by the number of colony-forming units (CFU) and histopathological analysis of the lungs after challenge with Mycobacterium tuberculosis. Immunization with BCG alone triggered a significant reduction of CFU in the lungs and also clearly preserved the pulmonary parenchyma. BCG priming also increased the immunogenicity of pVAXhsp65. However, boosters with pVAXhsp65 or the empty vector abolished the protective efficacy of BCG. Also, higher IL-5 levels were produced by spleen and lungs after DNA boosters. These results demonstrated that neonatal BCG immunization followed by DNAhsp65 boosters is highly immunogenic but is not protective against tuberculosis.

Background: A DNA vaccine (pVAXhsp65) containing the gene of a heat-shock protein (hsp65) from Mycobacterium leprae showed high immunogenicity and protective efficacy against tuberculosis in BALB/c mice. A possible deleterious effect related to autoimmunity needed to be tested because hsp65 is highly homologous to the correspondent mammalian protein. In this investigation we tested the effect of a previous immunization with DNAhsp65 in the development of experimental autoimmune encephalomyelitis (EAE), a rat model of multiple sclerosis. Methods: Female Lewis rats were immunized with 3 pVAXhsp65 doses by intramuscular route. Fifteen days after the last DNA dose the animals were evaluated for specific immunity or submitted to induction of EAE. Animals were evaluated daily for weight loss and clinical score, and euthanized during the recovery phase to assess the immune response and inflammatory infiltration at the central nervous system. Results: Immunization with pVAXhsp65 induced a specific immune response characterized by production of IgG(2b) anti-hsp65 antibodies and IFN-gamma secretion. Previous immunization with pVAXhsp65 did not change EAE clinical manifestations (weight and clinical score). However, the vaccine clearly decreased brain and lumbar spinal cord inflammation. In addition, it downmodulated IFN-gamma and IL-10 production by peripheral lymphoid organs. Conclusion: Our data demonstrated that this vaccine does not trigger a deleterious effect on EAE development and also points to a potential protective effect. Copyright (C) 2010 S. Karger AG, Basel

Heat-shock proteins (HSPs) are currently one of the most promising targets for the development of immunotherapy against tumours and autoimmune disorders. This protein family has the capacity to activate or modulate the function of different immune system cells. They induce the activation of monocytes, macrophages and dendritic cells, and contribute to cross-priming, an important mechanism of presentation of exogenous antigen in the context of MHC class I molecules, These various immunological properties of HSP have encouraged their use in several clinical trials. Nevertheless, an important issue regarding these proteins is whether the high homology among HSPs across different species may trigger the breakdown of immune tolerance and induce autoimmune diseases. We have developed a DNA vaccine codifying the Mycobacterium leprae Hsp65 (DNAhsp65), which showed to be highly immunogenic and protective against experimental tuberculosis. Here, we address the question of whether DNAhsp65 immunization could induce pathological autoimmunity in mice. Our results show that DNAhsp65 vaccination induced antibodies that can recognize the human Hsp60 but did not induce harmful effects in 16 different organs analysed by histopathology up to 210 days after vaccination. We also showed that anti-DNA antibodies were not elicited after DNA vaccination. The results are important for the development of both HSP and DNA-based immunomodulatory agents.

P>According to the hygiene hypothesis, the increased incidence of allergic and autoimmune diseases in developed countries is mainly explained by the decreased contact between the human population and certain environmental agents as lactobacillus, mycobacteria and helminths. In this study, we evaluated the effect of multiple infections with Strongyloides venezuelensis on the development of experimental autoimmune encephalomyelitis (EAE) in Lewis rats. Multiple infections before EAE induction were not able to change the evolution of the disease. No alterations were observed in weight loss, clinical score and inflammation intensity at the central nervous system. The presence of significant levels of parasite-specific IgG1 but not IgG2b suggested a Th2 polarization. However, the percentage and absolute number of CD4+CD25+Foxp3+ T cells were not changed, being their levels in the spleen and lymph nodes of infected rats comparable to the ones found in normal animals. These results suggest that a Th2-polarized response without concomitant expansion of Foxp3+ regulatory T cells was not able to modify EAE progression. Even though these results do not threaten the hygiene hypothesis, they suggest that this paradigm might be an oversimplification. They also emphasize the need of a study to compare the immunoregulatory ability associated with different helminth spp.

This study conducts a cost-effectiveness analysis of a childhood universal varicella vaccination program in Brazil. An age and time-dependent dynamic model was developed to estimate the incidence of varicella for 30 years. Assuming a single-dose schedule; vaccine efficacy of 85% and coverage of 80%, the program could prevent 74,422,058 cases and 2905 deaths. It would cost R$ 3,178,396,110 and save R$ 660,076,410 to the society and R$ 365,602,305 to the healthcare system. The program is cost-effective (R$ 14,749 and R$ 16,582 per life-year saved under the societal and the healthcare system`s perspective, respectively). The program`s cost-effectiveness is highly sensitive to the vaccine price and number of doses. (C) 2008 Elsevier Ltd. All rights reserved.

Purpose: The objective of this study was to evaluate the stress on the cortical bone around single body dental implants supporting mandibular complete fixed denture with rigid (Neopronto System-Neodent) or semirigid splinting system (Barra Distal System-Neodent). Methods and Materials: Stress levels on several system components were analyzed through finite element analysis. Focusing on stress concentration at cortical bone around single body dental implants supporting mandibular complete fixed dentures with rigid ( Neopronto System-Neodent) or semirigid splinting system ( Barra Distal System-Neodent), after axial and oblique occlusal loading simulation, applied in the last cantilever element. Results: The results showed that semirigid implant splinting generated lower von Mises stress in the cortical bone under axial loading. Rigid implant splinting generated higher von Mises stress in the cortical bone under oblique loading. Conclusion: It was concluded that the use of a semirigid system for rehabilitation of edentulous mandibles by means of immediate implant-supported fixed complete denture is recommended, because it reduces stress concentration in the cortical bone. As a consequence, bone level is better preserved, and implant survival is improved. Nevertheless, for both situations the cortical bone integrity was protected, because the maximum stress level findings were lower than those pointed in the literature as being harmful. The maximum stress limit for cortical bone (167 MPa) represents the threshold between plastic and elastic state for a given material. Because any force is applied to an object, and there is no deformation, we can conclude that the elastic threshold was not surpassed, keeping its structural integrity. If the force is higher than the plastic threshold, the object will suffer permanent deformation. In cortical bone, this represents the beginning of bone resorption and/or remodeling processes, which, according to our simulated loading, would not occur. ( Implant Dent 2010; 19:39-49)

Objective The purpose of this study was to evaluate the retention force of an O-ring attachment system in different inclinations to the ideal path of insertion, using devices to compensate angulations. Material and methods Two implants were inserted into an aluminum base, and ball attachments were screwed to implants. Cylinders with O-rings were placed on ball attachments and connected to the test device using positioners to compensate implant angulations (0 degrees, 7 degrees, and 14 degrees). Plexiglass bases were used to simulate implant angulations. The base and the test device were positioned in a testing apparatus, which simulated insertion/removal of an overdenture. A total of 2900 cycles, simulating 2 years of overdenture use, were performed and 36 O-rings were tested. The force required for each cycle was recorded with computer software. Longitudinal sections of ball attachment-positioner-cylinder with O-rings of each angulation were obtained to analyze the relationship among them, and O-ring sections tested in each angulation were compared with an unused counterpart. A mixed linear model was used to analyze the data, and the comparison was performed by orthogonal contrasts (alpha=0.05). Results At 0 degrees, the retention force decreased significantly over time, and the retention force was significantly different in all comparisons, except from 12 to 18 months. When the implants were positioned at 7 degrees, the retention force was statistically different at 0 and 24 months. At 14 degrees, significant differences were found from 6 and 12 to 24 months. Conclusions Within the limitations of this study, it was concluded that O-rings for implant/attachments perpendicular to the occlusal plane were adequately retentive over the first year and that the retentive capacity of O-ring was affected by implant inclinations despite the proposed positioners. O sistema logístico para distribuição de produtos acabados caracteriza-se pela integração dos serviços de comunicação, transporte e financeiros com a finalidade de atender às demandas do consumidor final. Estima-se que no estado do Espírito Santo, o consumo de carne de frango seja de 44,4 quilos per capita por ano. Para atender a esta demanda, o estado conta com matadouros-frigoríficos distribuídos pelo seu território, bem como, com a participação de outras empresas localizadas no país. Em sistemas de transportes, são característicos Problemas de Roteamento de Veículos (VRP), que precisam ser estudados, caracterizados e otimizados, normalmente, através de rotinas computacionais, que permitem avaliar maior quantidade de variáveis. O presente trabalho teve por objetivo caracterizar um VRP de um matadouro-frigorífico da região do Sul do Espírito Santo e desenvolver um aplicativo computacional que seja suporte para os gestores de logística, servindo para avaliar e propor rotas, e analisar parâmetros logísticos do processo de distribuição de produtos. No desenvolvimento do aplicativo computacional foi necessário caracterizar o sistema logístico da empresa, coletar e analisar os dados das operações logísticas, desenvolver as rotinas computacionais que representassem o sistema em estudo, verificar a confiabilidade dos resultados fornecidos pelo aplicativo, validá-lo e então, poder realizar as experimentações. O aplicativo desenvolvido permitiu reproduzir dados do sistema estudado e avaliar rotas segundo parâmetros logísticos. Pode-se concluir que o aplicativo computacional desenvolvido é útil aos gestores de logística, permitindo a avaliação das rotas praticadas e de novas configurações de rotas.

O soro de leite é um subproduto da fabricação do queijo, seja por acidificação ou por processo enzimático. Em condições ideais, a caseína do leite se agrega formando um gel, que posteriormente cortado, induz a separação e liberação do soro. É utilizado de diversas formas em toda a indústria alimentícia, possui rica composição em lactose, sais minerais e proteínas. A desidratação é um dos principais processos utilizados para beneficiamento e transformação do soro. Diante disto, o objetivo deste trabalho foi avaliar a influência dos métodos de secagem: liofilização, leito de espuma (nas temperaturas de 40, 50, 60, 70 e 80ºC) e spray-dryer (nas temperaturas de 55, 60, 65, 70 e 75ºC), sobre as características de umidade, proteína, cor e solubilidade do soro, bem como estudar o seu processo de secagem. O soro foi obtido e desidratado após concentração por osmose reversa, testando 11 tratamentos, em 3 repetições, utilizando um delineamento inteiramente casualizado. Os resultados demonstraram que o modelo matemático que melhor se ajustou foi o modelo de Page, apresentado um coeficiente de determinação ajustado acima de 0,98 e erro padrão da regressão em todas as temperaturas abaixo de 0,04 para o método por leito de espuma. Para o método de liofilização os respectivos valores foram 0,9975 e 0,01612. A partir disso, pode-se elaborar um modelo matemático generalizado, apresentando um coeficiente de determinação igual a 0,9888. No caso do leito de espuma, observou-se que à medida que se aumenta a temperatura do ar de secagem, o tempo de secagem diminui e os valores do coeficiente de difusão efetiva aumentam. Porém, a redução no tempo de secagem entre os intervalos de temperatura, diminui com o aumento da mesma. A energia de ativação para a difusão no processo de secagem do soro foi de 26,650 kJ/mol e para todas as avaliações físico-químicas e tecnológicas, a análise de variância apresentou um valor de F significativo (p<0,05), indicando que há pelo menos um contraste entre as médias dos tratamentos que é significativo.

O objetivo do presente estudo foi elaborar e caracterizar um extrato hidrossolúvel à base de gergelim para ser usado como possível substituto do leite de vaca e extrato de soja. Foi proposto um modelo de cinética de absorção de água durante a etapa de maceração no qual estimou o tempo ideal de batimento do gergelim macerado e se essa etapa influencia no rendimento do produto. Posteriormente com o objetivo de saber a influência do tempo de batimento (0,45 min a 14,55 min) e a concentração do extrato (1,925% a 23,07%) foram avaliadas através de um delineamento composto central rotacional (DCCR), ou seja, um delineamento 22 incluindo 4 ensaios nas condições axiais e 5 repetições no ponto central, totalizando 13 experimentos. Para todos os ensaios do delineamento realizou-se análises de viscosidade, cor, sólidos solúveis totais e rendimento. As mesmas análises foram realizadas em amostras comerciais de leite de vaca e extrato hidrossolúvel de soja com o intuito de comparação. Após a realização dessa etapa foi possível à seleção de algumas amostras para elaboração do extrato hidrossolúvel de gergelim e então realizou-se análises físico-químicas (proteína, cinzas, lipídios, carboidratos) e determinou-se a capacidade antioxidante por ABTS e teor de compostos fenólicos de tais amostras. Quanto à análise de rendimento não houve diferença significativa da quantidade de extrato obtido em diferentes temperaturas de maceração pelo teste de Tukey (p> 0,05). O DCCR foi realizado no programa Statistica e selecionaram-se os ensaios 1, 2, 6, 7, PC (ponto central) que foram as amostras que mais se assemelharam com as amostras comerciais estudadas. Nessas amostras realizou-se as análises físico-químicas além de capacidade antioxidante e compostos fenólicos. O conteúdo de sólidos totais na amostra 6 foi superior as demais amostras, diferindo estatisticamente das demais amostras (p> 0,05). Já para o teor de lipídios, cinzas e proteínas encontrados nas amostras 6 e PC foram maiores que as demais amostras, enquanto a amostra 7 exibiu o menor teor de lipídios e proteínas, considerando que o teor de cinzas das amostras 1, 2 e 7 foram estatisticamente iguais. O teor de carboidratos calculado por diferença foi superior nas amostras 1, 2 e 7 e com o menor valor na amostra 6. Os valores de capacidade antioxidante e compostos fenólicos não se diferenciaram estatisticamente entre si ao nível de 5% de probabilidade. Concluiu-se portanto que a etapa de maceração não influenciou no rendimento do produto final. A partir das análises físico-químicas pode-se concluir que o extrato hidrossolúvel de gergelim em determinadas amostras apresentou um melhor desempenho quanto ao valor nutricional comparado ao extrato de soja.

Diversas drogas ototóxicas causam efeitos nocivos ao ser humano, como zumbido, várias formas de deficiência auditiva e vertigem. Dentre os principais agentes considerados ototóxicos, encontra-se o álcool. OBJETIVO: Verificar as influências do alcoolismo no aparelho vestíbulo-coclear. FORMA DE ESTUDO: Estudo de coorte contemporânea com corte transversal. MATERIAL E MÉTODO: A amostra compreendeu 37 indivíduos para o grupo experimental, freqüentadores do grupo de Alcoólicos Anônimos da cidade de Santa Maria-RS e 37 indivíduos não alcoólicos, com a mesma idade e sexo, de cada indivíduo do estudo, a fim de formar o grupo controle. Todos foram submetidos à anamnese, exame otorrinolarigológico, avaliação audiológica básica e avalição vectoeletronistagmográfica. RESULTADOS: Observou-se que 67,57% dos indivíduos do Grupo Experimental apresentaram alteração na audiometria e 24,32% apresentaram alteração na vectoeletronistagmografia computadorizada. Nos indivíduos do grupo Controle, 27,03% apresentaram audiometria alterada e 10,81% apresentaram resultados alterados na vectoeletronistagmografia computadorizada. CONCLUSÃO: O álcool interfere na audição e no equilíbrio do indivíduo, causando efeito deletério no organismo humano.

Segundo a literatura, a disfunção vestibular infantil pode afetar consideravelmente a habilidade de comunicação e o desempenho escolar. OBJETIVO: Estudar a função vestibular em crianças com dificuldades escolares e suas queixas vestibulares. ESTUDO DE CASO: Estudo Clínico com coorte transversal. MATERIAL E MÉTODOS: Foram estudadas 50 crianças entre 7 e 12 anos, que freqüentavam escolas públicas de Piracicaba durante os anos de 2004 e 2005. Os procedimentos foram: anamnese; exame otorrinolaringológico; exame audiológico e avaliação vestibular. RESULTADOS: Das crianças avaliadas, 62,0% não relataram dificuldades escolares e 38,0% referiram ter dificuldades. A queixa geral mais comum foi de tontura (36,0%), e o sintoma mais comum no ambiente escolar foi de cefaléia (50,0%). Encontramos 74,2% de exame vestibular normal nas crianças sem dificuldades escolares e 31,6% de normalidade nas crianças com dificuldades. Encontramos alterações vestibulares de origem periférica irritativa tanto unilateral como bilateral, num total de 68,4% para as crianças com dificuldades escolares e um total de 25,8% para crianças sem dificuldades escolares. CONCLUSÃO: A queixa de atordoamento, o sintoma de náuseas e as dificuldades em ler e copiar apresentaram uma relação estatisticamente significante. Todas as alterações vestibulares encontradas foram de origem periférica irritativa. Os dados revelaram uma relação estatisticamente significante.
