BACKGROUND: Pediatric rheumatic diseases have a significant impact on children's quality of life and family functioning. Disease control and management of the symptoms are important to minimize disability and pain. Specialist clinical nurses play a key role in supporting medical teams, recognizing poor disease control and the need for treatment changes, providing a resource to patients on treatment options and access to additional support and advice, and identifying best practices to achieve optimal outcomes for patients and their families. This highlights the importance of investigating follow-up telenursing (TN) consultations with experienced, specialist clinical nurses in rheumatology to provide this support to children and their families. METHODS/DESIGN: This randomized crossover, experimental longitudinal study will compare the effects of standard care against a novel telenursing consultation on children's and family outcomes. It will examine children below 16 years old, recently diagnosed with inflammatory rheumatic diseases, who attend the pediatric rheumatology outpatient clinic of a tertiary referral hospital in western Switzerland, and one of their parents. The telenursing consultation, at least once a month, by a qualified, experienced, specialist nurse in pediatric rheumatology will consist of providing affective support, health information, and aid to decision-making. Cox's Interaction Model of Client Health Behavior serves as the theoretical framework for this study. The primary outcome measure is satisfaction and this will be assessed using mixed methods (quantitative and qualitative data). Secondary outcome measures include disease activity, quality of life, adherence to treatment, use of the telenursing service, and cost. We plan to enroll 56 children. DISCUSSION: The telenursing consultation is designed to support parents and children/adolescents during the course of the disease with regular follow-up. This project is novel because it is based on a theoretical standardized intervention, yet it allows for individualized care. We expect this trial to confirm the importance of support by a clinical specialist nurse in improving outcomes for children and adolescents with inflammatory rheumatisms. From practice to research in a couple and family therapy consultation - This article presents a study that took place at a Couples and Family outpatient clinic. Multiple levels are involved in couples and family therapy. The definition of relevant variables allowing the evaluation of a systemic therapeutic intervention's efficiency is thus particularly difficult. The aim of our study is to assess the short- and medium-term evolution on four different levels: (1) individual symptomatology; (2) conjugal satisfaction; (3) parental and co-parental relationship quality, and (4) global family functioning for a brief systemic intervention (BSI). BSI consists in a therapeutic treatment of a maximum of six sessions. The different levels are assessed by validated self-reported questionnaires before and after BSI as well as at a three-month follow-up. The therapeutic alliance is also measured by a self-reported questionnaire (WAI) after each session. The results of a pilot sample of N = 10 couples and families showed the efficiency of BSI on most of the variables measured and a greater therapeutic change for women than for men. No therapeutic effect was observed for men's individual symptomatology or for global family functioning. These results suggest that BSI has a different impact on the four levels measured. The importance of the therapeutic alliance on the therapeutic success is also confirmed.

Family cohesion and adaptability, as operationalised in the Family Adaptability and Cohesion Scales III (FACES III), are two hypothesised dimensions of family functioning. We tested the properties of a French version of FACES III in school-children (mean age: 13 years; S.D:0.85) recruited from the general population and their parents. Separate confirmatory factor analyses were performed for adolescents and adults. The results of both analyses were compatible with a two-factor structure similar to that proposed by the authors of the original instrument. However, orthogonality between the two factors was only supported in the adult data. Internal reliability estimates were 0.78 and 0.68 in adolescents and 0.82 and 0.65 in adults, for cohesion and adaptability respectively.

The aims of this study were 1) to clarify the factors associated with family functioning in cancer patient’s families with dependant children, 2) to examine children’ mental health when they are exposed to parental cancer, 3) to explore the subjective experience of having cancer during pregnancy, and finally, 4) to describe the implementation of a childcentred family intervention for cancer patients’ families with dependant children in an adult oncology setting. The study groups were collected between May 1st 2002 and April 30th 2004. They consisted of one European group collected from six different countries (N = 381) and two Finnish clinical groups (N = 85 and N = 2). The first Finnish clinical group of 85 cancer patient families with dependant children included a sub-sample of 54 families with children aged 11-17 years. The second Finnish clinical group consisted of two pregnant cancer patients. Additionally, a control group (N = 59) consisting of a sub-sample of 49 families with children aged 11-17 years was used. Quantitative methods (FAD, BDI, YSR, SOC, SF-8) and qualitative methods (observation, interviews, diaries, videotapes) were used exclusively and/or in combination. The results can be summarised as follows: 1) cancer “per se “ did not impair family functioning, children’s mental health, early interaction between ill mothers and their infant, 2) maternal depression or the ill parent’s depression were significantly associated with impairment in family functioning, 3) the individual’s good sense of coherence was associated with improvement in family functioning, and 4) a child-centred family intervention, which aims to give space for elaborating on cancer in the family, validates the sense of coherence and children’s feelings, and promotes open communication was welcomed. It is important to note that in the European study group, the prevalence of depression was 35 % (BDI > 16) among ill mothers, and 28% among healthy mothers, 28% among ill fathers, and 13% among healthy fathers. Early screening and effective treatment of depression in cancer patients and their partners is of paramount importance for the mental health of children and the well-being of the family. Pregnant cancer patients are in need of psychosocial support.

The aim of the present dissertation was to capture a picture of child and adolescent mental health in Romania, in the context of almost 25 years of changes following the Romanian Revolution of December ’89. A three-part study was carried out in order to provide consistent answers to the pre-defined objectives: to appraise the development of child and adolescent mental health services in Romania (Part I), to explore the characteristics of clinically-referred patients in a Romanian child and adolescent psychiatry department (Part II), to examine the children’s mental health state and its connections with family functioning and associated risk factors (Part III). A multi-method research approach was used, comprising one qualitative analysis and two quantitative research studies. Part I consisted of a comparative qualitative analysis of the answers given by 10 mental health professionals at a 12-questions open ended interview about the current situation in child and adolescent mental health in Romania, on three topics: changes, challenges, solutions. Part II involved a descriptive quantitative analysis of certain variables (e.g. age, gender, primary diagnosis, co-morbidities, time of hospitalization) conducted on the patients who had been admitted to the Child and Adolescent Psychiatry Department at “Prof. Dr. Alexandru Obregia” Psychiatry Hospital, Bucharest in 1991 and in 2013. Part III was conducted on 342 subjects enrolled in two clinical groups and one school group, this study being performed through a cross-sectional analysis on multi-informant child and adolescent mental health problems and competencies (CBCL, YSR, SDQ P, SDQ SR) and their interrelation with household information (HQ) and family functioning (FAD). Outlining the results it can be stated that: 1) The CAMH System in Romania is definitely set on the path of reorganization, including a higher involvement of beneficiaries and of the community. 2) The characteristics of the admitted patients have changed significantly during the last almost 25 years since `89 December Revolution, under the influence of word wide trends in child psychiatry and of administrative aspects of the mental health network in Romania. 3) The rates of main diagnoses and co-morbidities confirm the reports in literature, with Autism Spectrum Disorder being the most frequent childhood psychiatric disorders in this study. 4) The children’s mental health problems in the psychiatry group are comparable to those reported for other clinical populations. 5) Significant score differences were observed according to various household features and also meaningful associations between a child’s clinical status and different aspects of family functioning. The Romanian Child and Adolescent Psychiatry has started to adopt the norms and standards of the European Union. In the 25 years that have elapsed after the 1989 Revolution, many changes have occurred in Romanian CAMH, but many unresolved issues have also risen. Therefore, the major contribution of this thesis is that it provides a coherent and updated overview of the present-day situation from three different perspectives- those of mental healthcare professionals, the one observed in clinical patients and the one reported by children’s families.

The parents of premature infants, especially the mothers, are at increased risk for distress. Infants born prematurely are at risk for developmental problems. The aim of this study was to investigate whether the psychological well-being of both parents is associated with child development in very low birth weight (VLBW, ≤1500g) children. The burden of prematurity-related morbidity to the children and to the family was also assessed. A cohort of 201 VLBW infants born during 2001–2006 in the Turku University Hospital, Finland, and their parents were studied (I–IV). One study included a control group (n=166) of full-term infants (IV). The psychological well-being of the parents was evaluated by assessments of depressive symptoms, parenting stress, the sense of coherence and general family functioning. Cognitive, behavioral, and socio-emotional development, and the health-related quality of life (HRQoL) of the children were determined when the children were 2 to 8 years old. The psychological well-being of the parents was associated with the cognitive, behavioral and social development of the VLBW children. The VLBW infants with prematurity-related morbidities had a poorer HRQoL and the general functioning of the family was inferior compared to the control children and their families. 64.5% of the VLBW children survived without morbidities. Most of the VLBW children did not have significant behavior problems (93%), had normal social skills (63%), had no emotional problems (64%), and had no problems in executive functioning (62%). Only 3% of the surviving VLBW infants had significant cognitive delay. In conclusion, the depressive symptoms and stress of the parents can be risk factors for disadvantageous child development, while a strong sense of coherence can be protective. Parents of the premature children with developmental delays might also experience more depressive symptoms and stress than other parents. Prematurity-related morbidities were a burden to the VLBW child as well as to the family.

Changes within the family unit have resulted in changes in interactions between grandparents and older grandchildren. Existing research indicates that these relationships can result in positive outcomes. A relevant task for researchers is to continue to explore th~se intergenerational relationships. This qualitative phenomenological study explores the question: What functional patterns exist when grandparents interact with older grandchildren? Six grandparent-older grandchild pairs agreed to be involved. Kennedy's (1992) formulation of grandparent-older grandchild activity clusters was reviewed and revised. Activities were clustered related to socialization, companionship, support, entertainment, and education. Findings unique to this study indicate that shared activities were mutually chosen with consideration of activity tolerance, and were consistently evaluated as enjoyable. Partners were chosen because of a comfortable relationship established through frequent past and present interactions, and not because of family lineage preferences. Both grandparents and older grandchildren stated a desire to have a generation peer share activities with them. Exploration of dimension concepts for the "McMaster Model of Family Functioning" indicated that these relationships have potential to contribute to healthy family functioning. The implications for practice, theory development and further research are suggested.

Les parents d’enfants aux prises avec un trouble déficitaire de l’attention avec ou sans hyperactivité (TDA/H) sont à risque de dépression. Ces parents rapportent aussi des relations familiales plus problématiques, des expériences plus stressantes, un sentiment d’auto-efficacité plus faible à l’égard de leur rôle parental et des pratiques parentales plus coercitives ou inadéquates en comparaison aux parents d’enfants sans ce diagnostic. Plusieurs recherches ont relevé que les parents d’enfants ayant un TDA/H qui ont participé à un programme d’entraînement aux habiletés parentales (PEHP) rapportent une amélioration générale des difficultés énumérées précédemment. Le changement d’attitude et de pratiques parentales est souvent relié à une diminution des symptômes du TDA/H chez les enfants. L’intervention peut donc contribuer à améliorer la condition du parent et par le fait même celle de son enfant. Toutefois, le TDA/H est un trouble chronique qui peut affecter certaines caractéristiques personnelles du parent pouvant interférer avec la capacité de ce dernier à bénéficier d’une intervention. Peu d’études s’attardent aux caractéristiques du parent pouvant affecter l’efficacité de l’intervention. Le but de la présente étude est d’étudier le lien bidirectionnel entre certaines caractéristiques personnelles (dépression, stress, sentiment d’auto-efficacité) et les pratiques parentales dans un contexte d’intervention qui s’adresse aux parents d’enfants ayant un TDA/H. Les résultats démontrent que le sentiment d’auto-efficacité initial du parent est prédicteur d’une discipline inconstante, et ce, indépendamment de la condition expérimentale. De plus, les pratiques positives initiales sont prédicteurs d’un sentiment d’auto-efficacité élevé au post-test et ce, indépendamment de la condition expérimentale.

Les écrits rapportent qu’entre 10% et 40% des personnes atteintes de troubles mentaux ont présenté des comportements agressifs depuis l’établissement du diagnostic, et qu’entre 50% et 65% de ces comportements sont perpétrés envers les membres de la famille (Estroff et al., 1998). Or, les aidants naturels se sentent fréquemment démunis devant le comportement impulsif et agressif de leur proche atteint d’un trouble de santé mentale (Bonin & Lavoie-Tremblay, 2010) et la majorité des proches aidants ne reçoivent pas de soutien professionnel (Doornbos, 2001). À cet effet, une équipe de chercheurs et d’organismes communautaires ont mis sur pied un projet de formation adaptée par et pour les familles relativement à la gestion des comportements agressifs de leur proche atteint d’un trouble de santé mentale s’appuyant sur l’approche OMEGA communautaire (Bonin et al., 2011). La présente recherche est inspirée du modèle de Fresan et ses collaborateurs (2007), qui inclut les familles et leur proche atteint dans la prévention de la rechute et le rétablissement de ce dernier. Cette étude avait donc pour but de décrire et analyser, par un devis mixte, les perceptions de l’agressivité, du fonctionnement familial ainsi que de l’observance au traitement médical de personnes atteintes de troubles mentaux, suite à une formation de gestion des comportements agressifs adaptée par et pour des membres de familles, et suivie par leur proche aidant. Des questionnaires auto-administrés ont été remis et complétés par 25 personnes atteintes et comprenaient trois instruments qui étaient : 1. un questionnaire sociodémographique; 2. le Basis-24 (Eisen, 2007) permettant d’évaluer la condition mentale de la personne atteinte; 3. l’Échelle de perception de l’agressivité permettant de mesurer la fréquence et le niveau des comportements agressifs (De Benedictis et al., 2011); et 4. l'Échelle du fonctionnement familial permettant de mesurer le fonctionnement familial (Epstein, Baldwin, & Bishop, 1983). Des questions supplémentaires au questionnaire portaient sur l’observance au traitement médical et sur les changements observés. Des analyses descriptives et corrélationnelles bivariées ont été réalisées pour analyser les données quantitatives. Par la suite, des entrevues individuelles ont été réalisées auprès de huit personnes atteintes de troubles mentaux dont le membre de famille avait suivi la formation de gestion des comportements agressifs. La méthode d’analyse classique de Miles et Huberman (2003) a été utilisée pour réaliser l’analyse des données. L’analyse des données a permis de dégager les perceptions des personnes atteintes de troubles mentaux au regard de la formation de gestion des comportements agressifs que leur membre de famille avait suivi. Ainsi, le programme de formation OMÉGA adaptée par et pour les familles permettrait d’une part de diminuer l’agressivité et, d’autre part, d’améliorer le climat familial. De plus, selon les personnes atteintes, leur membre de famille était plus à l’écoute et faisait moins preuve d’intrusion, ce qui diminuaient les conflits et favorisaient une meilleure dynamique familiale. Cette étude a mis en évidence qu’il est nécessaire d’impliquer les familles dans les programmes de prévention de la rechute de la maladie mentale dans les soins de première ligne. Cette étude contribue ainsi à l’amélioration des connaissances sur la dynamique famille-patient et sur le rétablissement des personnes atteintes de troubles mentaux. Aussi, cette recherche propose des recommandations pour la recherche, la pratique, la formation et la gestion en sciences infirmières.

Au Québec, le trouble du déficit de l’attention/hyperactivité (TDA/H) est celui qui requiert le plus grand nombre de consultations en pédopsychiatrie (50 % à 75 %). À ce jour, l’intervention multimodale (traitement pharmacologique, programme d’entraînement aux habiletés parentales (PEHP) et programme d’intervention cognitive comportementale (PICC) auprès des enfants ayant un TDA/H) a obtenu de bons résultats à long terme. Dans cette étude, nous avons évalué les changements dans le fonctionnement familial suite à un PEHP. La conception de ce PEHP repose sur les deux approches : l’approche systémique familiale de Calgary (Wright & Leahey, 2013) et l’approche de solution collaborative et proactive (Greene, 2014). Le Family Assessment Device (FAD; Epstein, Baldwin, et Bishop, 1983), version courte, a été utilisé pour mesurer le fonctionnement général (FG) de la famille. La collecte de données a été réalisée auprès de deux groupes (groupe participant et groupe témoin) et à deux temps de mesure (avant et après le PEHP). L’échantillon contient 28 familles participantes et 18 familles témoins. L’analyse de variance à mesures répétées (ANOVA) a été utilisée pour tester l’effet des variables indépendantes (Temps et Intervention) sur la variable dépendante (FG). Les résultats indiquent que les parents qui participent à un PEHP perçoivent un fonctionnement familial général amélioré par rapport au groupe témoin. L’interprétation des changements à la suite du PEHP donne des pistes d’intervention infirmières à ces familles afin d’éviter les impacts de ce trouble sur le fonctionnement familial à long terme.

A causa de los conflictos armados, como el de Colombia, se han desplazado por la fuerza a millones de personas, entre ellas una importante parte de la población infantil. Este estudio tuvo como objetivo evaluar la salud mental de los niños desplazados internos en edad preescolar en Bogotá Colombia, e identificar los determinantes de la salud mental en estos niños. Métodos: Estudio transversal realizado entre 279 niños que asisten a cuatro jardines infantiles en un barrio marginal de Bogotá. La salud mental infantil se evaluó con el instrumento validado de Comportamiento Infantil (CBCL) 1,5-5 años, aplicados a padres y cuidadores. Se realizo un análisis univariado y multivariado de regresión logística para evaluar la asociación entre el desplazamiento y la salud mental de los niños y para identificar las relaciones con la salud mental en los niños desplazados. Resultados: los Niños desplazados (n = 90) se identificaron con más frecuencia sobre los puntos de corte límite para las escalas CBCL que los no desplazados (n = 189) (por ejemplo, problemas totales 46,7 vs 22,8%;p \ 0,001). La asociación entre el desplazamiento y la presencia de problemas CBCL totales se mantuvo después del ajuste por factores socio-demográficos (OR Ajustado 3.3 del 95%: 1,5; 6,9). Donde la salud mental del cuidador explica en parte la asociación. En los niños desplazados, la salud mental del cuidador (p \ 0,01) y el funcionamiento familiar (p \ 0,01) se asociaron independientemente con la salud mental de los niños. La exposición a eventos traumáticos y el apoyo social también se asociaron con la salud mental del niño, sin embargo, las asociaciones no fueron independientes. Conclusión: En este barrio marginal de Bogotá, los niños en edad preescolar registrados como desplazados internos presentan peor salud mental que los no desplazados. El funcionamiento familiar y la salud mental del cuidador fueron fuerte e independientemente asociados con la salud mental de los niños y niñas desplazados.

Marital conflicts have been largely investigated in research, especially because of its impact on child development. Based on a literature review, this article aims to present some reflections about marital conflicts and its possible associations with children's behavior, particularly through parental practices. Emphasis is made on a systemic view of causality between marital conflict and child behavior, which offers a better comprehension of the family functioning. This perspective also allows us to enlarge the analysis scope beyond linear correlations between both variables.

The entrance of women in the labor market carries implications for the organization and structure of family functioning, leading to the proposition of new settings, family arrangements with direct interference in the mother-child and family dynamics.A major challenge for women is to reconcile time for household chores, academic, external work and be able to stay with their children in order to be able establish an affective bond and harmonious.In this sense, one can say that in married life the roles played by man and woman has been increasingly confused and that family configuration has been designed differently from the traditional family structure as well as the mother-child relationship is also touched by these events.

Objetivou-se identificar o padrão de funcionamento familiar nas relações de cuidado cotidiano de adultos na quarta idade. Trata-se de estudo de natureza diagnóstico-avaliativa de idosos de 80 e mais anos, dependentes de cuidados, e de seus familiares cuidadores, selecionados entre os cadastrados nas unidades de Saúde da Família de um distrito de periferia de Belém-PA, avaliando-os quanto à dinâmica de família, qualidade de vida e estilo de vida relacionado à saúde. Observou-se que a maioria dos idosos avaliou sua família como tendo boa funcionalidade. Porém, os demais dados relativos à qualidade de vida de idosos e cuidadores, como também, estilo de vida dos cuidadores recaíram no nível mediano, levando a inferir certa dificuldade no padrão de funcionamento familiar. Conclui-se que os múltiplos resultados obtidos sinalizam implicações práticas de atenção à unidade familiar e confirmam a necessidade de avaliação multidimensional em intervenção de família.
