Neurocognitive impairment constitutes a core feature of bipolar illness. The main domains affected are verbal memory, attention, and executive functions. Deficits in these areas as well as difficulties to get functional remission seem to be increased associated with illness progression. Several studies have found a strong relationship between neurocognitive impairment and low functioning in bipolar disorder, as previously reported in other illnesses such as schizophrenia. Cognitive remediation strategies, adapted from work conducted with traumatic brain injury patients and applied to patients with schizophrenia, also need to be adapted to individuals with bipolar disorders. Early intervention using functional remediation, involves neurocognitive techniques and training, but also psychoeducation on cognition-related issues and problem-solving within an ecological framework.

Background Depression is one of the more severe and serious health problems because of its morbidity, disabling effects and for its societal and economic burden. Despite the variety of existing pharmacological and psychological treatments, most of the cases evolve with only partial remission, relapse and recurrence. Cognitive models have contributed significantly to the understanding of unipolar depression and its psychological treatment. However, success is only partial and many authors affirm the need to improve those models and also the treatment programs derived from them. One of the issues that requires further elaboration is the difficulty these patients experience in responding to treatment and in maintaining therapeutic gains across time without relapse or recurrence. Our research group has been working on the notion of cognitive conflict viewed as personal dilemmas according to personal construct theory. We use a novel method for identifying those conflicts using the repertory grid technique (RGT). Preliminary results with depressive patients show that about 90% of them have one or more of those conflicts. This fact might explain the blockage and the difficult progress of these patients, especially the more severe and/or chronic. These results justify the need for specific interventions focused on the resolution of these internal conflicts. This study aims to empirically test the hypothesis that an intervention focused on the dilemma(s) specifically detected for each patient will enhance the efficacy of cognitive behavioral therapy (CBT) for depression. Design A therapy manual for a dilemma-focused intervention will be tested using a randomized clinical trial by comparing the outcome of two treatment conditions: combined group CBT (eight, 2-hour weekly sessions) plus individual dilemma-focused therapy (eight, 1-hour weekly sessions) and CBT alone (eight, 2-hour group weekly sessions plus eight, 1-hour individual weekly sessions). Method Participants are patients aged over 18 years meeting diagnostic criteria for major depressive disorder or dysthymic disorder, with a score of 19 or above on the Beck depression inventory, second edition (BDI-II) and presenting at least one cognitive conflict (implicative dilemma or dilemmatic construct) as assessed using the RGT. The BDI-II is the primary outcome measure, collected at baseline, at the end of therapy, and at 3- and 12-month follow-up; other secondary measures are also used. Discussion We expect that adding a dilemma-focused intervention to CBT will increase the efficacy of one of the more prestigious therapies for depression, thus resulting in a significant contribution to the psychological treatment of depression. BACKGROUND: Recent literature has distinguished the negative symptoms associated with a diminished capacity to experience (apathy, anhedonia) from symptoms associated with a limited capacity for expression (emotional blunting, alogia). The apathy-anhedonia syndrome tends to be associated with a poorer prognosis than the symptoms related to diminished expression. The efficacy of drug-based treatments and psychological interventions for these symptoms in schizophrenia remains limited. There is a clear clinical need for new treatments. METHODS: This pilot study tested the feasibility of a program to reduce anhedonia and apathy in schizophrenia and assessed its impact on 37 participants meeting the ICD-10 criteria for schizophrenia or schizoaffective disorders. Participants were pre- and post-tested using the Scale for the Assessment of Negative Symptoms (SANS) and the Calgary Depression Scale for Schizophrenia (CDSS). They took part in eight sessions of the Positive Emotions Program for Schizophrenia (PEPS)--an intervention that teaches participants skills to help overcome defeatist thinking and to increase the anticipation and maintenance of positive emotions. RESULTS: Thirty-one participants completed the program; those who dropped out did not differ from completers. Participation in the program was accompanied by statistically significant reductions in the total scores for Avolition-Apathy and Anhedonia-Asociality on the SANS, with moderate effect sizes. Furthermore, there was a statistically significant reduction of depression on the CDSS, with a large effect size. Emotional blunting and alogia remain stable during the intervention. DISCUSSION: Findings indicate that PEPS is both a feasible intervention and is associated with an apparently specific reduction of anhedonia and apathy. However, these findings are limited by the absence of control group and the fact that the rater was not blind to the treatment objectives. CONCLUSIONS: PEPS is a promising intervention to improve anhedonia and apathy which need to be tested further in a controlled study. This paper intends to elaborate the relationship between Kelly's Personal Construct Theory (PCT) and the systemic therapies beyond their notable similarities. Kelly's constructive alternativism is situated in the context of the current constructivist orientation that the family therapy movement seems to be adopting. A model of change is presented based on PCT's experience cycle. From this cycle, the relationship between behaviors and constructions is elaborated incorporating Procter's (52, 53) notions of the Family Construct System (FCS) andposition. This model allows for interventions both at behavioral and construction levels, as well as allowing for a certain technical eclecticism while, at the same time, retaining a strong theoretical coherence. This approach is discussed in the context of the debate about strategizing, power, and control held by authors such as Golann, Hoffman, and Tomm. Finally, some implications for research are outlined.

Desde la epistemología constructivista se considera la cognición como la actividad básica del vivir: interpretar la experiencia, otorgar significado al flujo de los acontecimientos. Por tanto, no se la ve como una actividad intelectual, racional o mental sino holística porque implica al sujeto en su totalidad; aunque se pueda manifestar en forma de pensamientos, emociones, imágenes, etc. Las distintas terapias cognitivas de inspiración constructivista comparten la visión de la persona como un agente que construye de forma proactiva los acontecimientos, incorporando los significados generados en un sistema cuyo núcleo es el sentido de identidad. Los esfuerzos para promover el cambio pueden provocar resistencias a menos que se armonicen con la necesidad esencial de la persona de proteger su sentido de identidad, continuidad y, en consecuencia, su unicidad.

La hipnosis y la terapia cognitiva parecen reflejar, en principio, dos concepciones muy distintas de la intervención psicológica. Sin embargo, ambas contemplan entre sus objetivos la reorganización de las estructuras cognitivas del cliente. Este objetivo común las hace compatibles y podría permitir a las terapias cognitivas nuevas formas de intervenir en tales estructuras. La hipnosis promueve que el cliente haga por sí mismo este proceso de reestructuración sin el control racional y consciente, por lo que se puede modificar estructuras inaccesibles a la conciencia, el lenguaje y, por tanto, al análisis cognitivo.

Obsessive Compulsive Disorder (OCD) involves excessive worry coupled with engaging in rituals that are believed to help alleviate the worry. Pervasive Developmental Disorders (PODs) are characterized by impairments in social interaction, communication, and the presence of repetitive and/or restrictive behaviours (American Psychiatric Association, 2000). Research suggests that as many as 81% of children with a POD also meet criteria for a diagnosis ofOCD. Currently, only a handful of studies have investigated the use of Cognitive Behavioural Therapy (CBT) in treating OCD in children with autism (Reaven & Hepburn, 2003 ; Sze & Wood, 2007; Lehmkuhl, Storch, Bodtish & Geflken, 2008). In these case studies. the use of a multi-modal CBT treatment package was successful in alleviating OCD behaviours. The current study used function-based CBT with parent involvement and behavioural supplements to treat 2 children with POD and OCD. Using a multiple baseline design across behaviours and participants, parents reported that their child 's anxiety was alleviated and these gains were maintained at 6-month follow-up. According to results of the Children 's Yale-Brown Obsessive Compulsive Scale (Goodman, Price, Rasmussen, Riddle, & Rapoport, 1986) from preto post-test, OCD behaviours of the children decreased II"om the severe to the mild range. In addition, the parents rated the family's level of interference related to their child 's OCD as substantially lower. Last, the CBT treatment received high ratings of consumer satisfaction.

L’approche cognitive du trouble obsessionnel-compulsif (TOC) propose un lien bidirectionnel entre les émotions et les cognitions. Cependant, même si des études montrent une association entre les émotions et le TOC, aucune étude ne s’est attardée à la relation entre les émotions, les cognitions et les comportements au cours d’une thérapie cognitive. La présente étude a pour but d’examiner la relation entre les processus cognitif, béhavioral et émotionnel au cours d’une thérapie basée sur les inférences (TBI) chez des personnes souffrant du TOC. Plus précisément, nous avons observé comment les émotions et les symptômes du TOC s’influencent et comment ils s’influencent à travers le temps. Les patients ont rempli un journal de bord tout au long du processus thérapeutique, notant (de 0 à 100) des émotions clés, ainsi que les croyances et les comportements ciblés durant la thérapie. Des analyses à mesures répétées ont été utilisées afin de maximiser le potentiel des données longitudinales. Les résultats montrent que l’anxiété, la tristesse et la joie ont des trajectoires similaires aux croyances et aux comportements au cours de la thérapie. Les forces et limites de l’étude sont discutées. Les implications des résultats pour le traitement des émotions et des pensées à différents moments de la thérapie sont aussi discutées.

Il s'agit d'une hypothèse largement répandue que l’égo-dystonie et l’égo-syntonie caractérisent les obsessions dans les troubles des conduites alimentaires (TCA) et que ces facteurs sont cliniquement pertinents pour la conceptualisation et le traitement des TCA. Cependant, les résultats empiriques sur ce sujet sont rares. Compte tenu du chevauchement reconnu entre les TCA, notamment l'anorexie et la boulimie (BN), et le trouble obsessionnel-compulsif (TOC) dans la phénoménologie et les caractéristiques psychologiques, un programme de thérapie cognitive basée sur les inférences (TBI) de 24 semaines, démontré efficace dans le traitement des TOC, a été adapté pour traiter les TCA. La recherche sur le TOC suggère que la transformation des pensées intrusives en obsessions est liée à la mesure dans laquelle les pensées intrusives menacent des perceptions fondamentales du soi et de l’identité. Cette thèse a pour objectif d'examiner le lien entre l’égo-dystonie et les TCA. Pour se faire, nous avons exploré le lien entre la nature égo-dystone des obsessions chez les patients souffrant d'un TCA et la peur de l'image de soi. Nous avons également étudié la relation entre la sévérité des symptômes TCA et l’égo-dystonie dans les obsessions. En outre, nous avons investigué les différences dans la présence de pensées égo-dystones et de peur face à son identité entre des sujets non-cliniques et des personnes atteintes d’un TCA. Enfin, nous avons comparé le degré d’égo-dystonie dans les pensées de personnes atteintes d’un TCA à celui dans les pensées d’individus souffrant d’un TOC. L’égo-dystonie dans les pensées a été mesurée par l'Ego Dystonicity Questionnaire (EDQ) et le degré de peur face à l’identité a été mesuré par le Fear of Self Questionnaire (FSQ) d’une part dans un échantillon de femmes souffrant d’un TCA (n = 57) et d’autre part dans un échantillon de participantes non-cliniques (n = 45). Les résultats révèlent que l’égo-dystonie et la peur face à l’identité étaient fortement corrélées à la fois dans l’échantillon clinique et non-clinique. Les scores de l’EDQ n’étaient pas significativement corrélés à la sévérité des symptômes TCA à l'exception de la sous-échelle d’irrationalité de l’EDQ qui était fortement associée à la sévérité des comportements compulsifs compensatoires. Les participantes souffrant d'un TCA avaient des scores significativement plus élevés à l’EDQ et au FSQ que les sujets non-cliniques. Ensuite, une étude de cas décrit l’application du programme de thérapie cognitive TBI pour une femme de 35 ans avec un diagnostic de BN. La pathologie TCA s’est significativement améliorée au cours de la TBI et six mois suivant la thérapie. Cette étude de cas met en évidence l'importance de cibler les idées surévaluées, les doutes et le raisonnement face au soi et à l’identité dans le traitement psychologique pour les TCA. Enfin, l’objectif final de cette thèse était d’examiner les changements au niveau (1) des symptômes TCA, (2) du degré d’égo-syntonie dans les obsessions, et (3) des mesures de peur face à l’identité, de motivation, d’humeur et d’anxiété au cours de la TBI et au suivi post six mois. L’égo-dystonie, la peur face à l’identité, les symptômes TCA et le stade motivationnel ont été mesurés chez 15 femmes souffrant de BN au cours du traitement et six mois après la TBI. Quatre vingt pourcent de l’échantillon, soit 12 des 15 participantes, ont démontré une diminution cliniquement significative des symptômes TCA et 53% ont cessé leurs comportements compensatoires au suivi post six mois. Les retombées cliniques relatives au traitement des TCA sont discutées.

La fibromialgia es un síndrome doloroso crónico de alta complejidad en su diagnóstico (que hasta la fecha es clínico) y en su manejo. Se caracteriza por la presencia simultánea de numerosos problemas psicológicos en los pacientes, de los cuales se reconoce su asociación con el síndrome pero no la direccionalidad de la asociación. En consecuencia, la atención de las personas que lo desarrollan requiere del despliegue de numerosos esfuerzos interdisciplinarios, con un alto fracaso terapéutico y una mayor cronificación de los síntomas. La Terapia Cognoscitivo Comportamental ha demostrado ser una valiosa herramienta para el abordaje eficaz de las necesidades afectivas, emocionales y comportamentales de esta población. En consecuencia, el presente trabajo de grado tuvo como objetivo realizar una revisión de la literatura hallada en las bases de datos especializadas, sobre la evidencia de las Terapias de Cognoscitivo Comportamentales de Tercera Generación en el manejo de la fibromialgia. Se seleccionaron tres de estas terapias, a saber: la Terapia de Aceptación y Compromiso (TAC), la Terapia Cognoscitiva Basada en Mindfulness (TCBM) y la Psicoterapia Analítica Funcional (PAF). En primer lugar se presenta un breve recorrido histórico de la Terapia Comportamental con el fin de apreciar las diferencias y las similitudes de la Terapia Comportamental en sus tres generaciones. Posteriormente se presenta cada una de las tres terapias seleccionadas, contextualizándolas en cuanto a sus objetivos y evidencia empírica en el campo de la Psicología de la Salud y en específico del dolor crónico y de la fibromialgia. Finalmente, se presentan algunas conclusiones generales y se discute sobre su eficacia y el papel del en estas, en especial para el manejo de la persona con fibromialgia.

En la actualidad tanto el consumo de alcohol y marihuana, como las conductas suicidas (ideación e intención suicida) en adultos jóvenes, constituyen un problema de salud pública cuyo impacto a nivel social y emocional, irrumpe el estilo de vida no sólo de quien es consumidor y lleva a cabo conductas suicidas sino al núcleo familiar y social circundante. Hay una especial preocupación por el temprano consumo de estas sustancias, oscilando entre los 15 años en el caso de los hombres y en las mujeres a los 18 años. El objetivo de esta revisión teórica es revisar la evidencia teórica y empírica sobre la conducta suicida (ideación e intención) y el consumo de alcohol y marihuana en adolescentes y adultos jóvenes. Entre otros, los resultados muestran que el consumo de alcohol y la conducta suicida están relacionados y que no hay evidencia empírica sobre la relación entre el consumo de marihuana y esta conducta.

El objetivo de este estudio fue identificar las tendencias en la investigación de la depresión infantil mediante un análisis bibliométrico de artículos publicados entre enero de 2010 y enero de 2015, en las bases de datos EBSCO, Scopus y Scielo. Se incluyeron 146 artículos, escritos en inglés, español y portugués. En el trabajo se identifican los autores más productivos y las tendencias de los autores con respecto al sexo, profesión y producción; así como los países, instituciones y revistas que más publicaron sobre dicho trastorno. La edad y la ansiedad son las variables que con más frecuencia se asocian al tema; de igual forma, se identificó el Children Depression Inventory (CDI) como el instrumento más común para medir la depresión infantil. En este periodo de tiempo destacan los artículos centrados en investigaciones sobre poblaciones con diferentes estados de salud, diseños correlacionales, de corte transversal y escritos por múltiples autores. Al analizar los resultados obtenidos se observa un creciente interés y preocupación de los investigadores por conocer la frecuencia de los trastornos del estado de ánimo y más específicamente de la depresión en niños, y la asociación e influencia que esta puede tener para su desarrollo.
