Résumé: La Scoliose Idiopathique de l’Adolescent (SIA) est une condition débilitante qui peut avoir comme résultat une douleur importante, une altération du fonctionnement quotidien et une détérioration de la qualité de vie. Pour les patients qui ne répondent pas au traitement conservateur, la fusion vertébrale, en utilisant des greffes osseuses, est devenue un traitement de choix pour stabiliser la colonne. Des connaissances plus pointues à propos des facteurs impliqués dans l’ostéogénèse et la formation de l’os peuvent raccourcir le processus de guérison et permettre aux patients de réintégrer leurs activités dans un laps de temps plus court. Les plaquettes peuvent jouer un rôle important dans la première étape de la guérison des fractures car elles sont une source autologue de plusieurs facteurs de croissance qui soutiennent la prolifération et la différenciation des ostéoblastes in vivo et in vitro. Au cours des dernières années, plusieurs tentatives ont été réalisées afin de trouver des traitements additionnels pour : 1) Raccourcir le temps de guérison des fractures relativement long ; 2) Obtenir une plus courte période de convalescence pour les patients qui ont besoin de prothèses ; 3) Corriger plus facilement plusieurs maladies congénitales; 4) Améliorer le processus de fusion vertébrale et 5) Développer de nouvelles approches thérapeutiques, notamment au niveau des processus régularisant le remodelage osseux et la régénération des tissus osseux. Dans le cadre de la présente étude, j’ai étudié la contribution possible du facteur de croissance de l’insuline (IGF) et du facteur vasculaire endothélial de croissance (VEGF) sur la maturation de l’ostéoblaste scoliotique dans des cultures cellulaires in vitro et j’ai comparé les résultats avec celles obtenues dans les mêmes conditions mais en stimulant les ostéoblastes avec de la mélatonine. Cette étude préliminaire a été réalisée sur des échantillons d’os récoltés de quatre patients atteints par la Scoliose Idiopathique de l‘Adolescent (SIA), ainsi que sur des échantillons d’os issus de quatre sujets témoins (cas traumatiques). Les résultats montrent que l’IGFs et le VEGFs possèdent une action d’inhibition sur la prolifération d’ostéoblastes scoliotiques et non scoliotiques, et que cette action est proportionnelle à la concentration de ces facteurs. Les ostéoblastes scoliotiques tendent à avoir une prolifération cellulaire plus rapide et plus élevée que les témoins non scoliotiques. De façon générale les ostéoblastes provenant de patients scoliotiques ont une ostéogénèse in vitro plus accélérée que le sujet non scoliotique. De plus, il semble que la mélatonine joue un rôle physiologique dans la différenciation de l’ostéoblaste scoliotique et elle semble aider à avoir une différenciation plus précoce que chez les non traités. Les ostéoblastes scoliotiques expriment un défaut d’expression de l’IGF 1 et d’IGF 1R en présence de la mélatonine. En conclusion, le VEGF A et l’IGF 1 peuvent également promouvoir la différenciation et la prolifération des ostéoblastes humains scoliotiques en culture primaire.

De nombreuses études ont démontré que les adolescents qui subissent une chirurgie correctrice de scoliose éprouvent des douleurs postopératoires intenses et cela pendant plusieurs jours après leur chirurgie. L’originalité de cette étude de type quasi-expérimental, consistait à réaliser un DVD sur une technique d’imagerie guidée et de relaxation combinée à une intervention éducative (gestion de la douleur) sur la douleur, l’anxiété, le coping et le retour aux activités d’adolescents ayant subi une chirurgie correctrice de scoliose. Prenant appui sur la théorie du portillon (Melzack & Casey, 1968) et sur la théorie transactionnelle du stress et coping (Lazarus & Folkman, 1984), quatre hypothèses ont été retenues en vue de vérifier l’effet de cette intervention. Trente et un adolescents, âgés entre 13 et 19 ans ont été répartis en deux groupes de façon aléatoire. Les participantes du groupe expérimental devaient visionner un DVD sur une technique d’imagerie guidée et de relaxation combinée à de l’information sur la gestion de la douleur en période préopératoire, au moment du congé de l’hôpital et ce, au moins trois fois par semaine sur une période d’un mois post congé. De plus, ce groupe continuait à recevoir les soins usuels. Les participants du groupe contrôle ne recevaient que les soins usuels. Les instruments de mesure utilisés furent le questionnaire concis de la douleur (QCD) (Larue F, Colleau, Brasseur, & Cleeland, 1995), l’inventaire d’anxiété situationnelle et de trait d’anxiété (IASTA-Y) (Gauthier & Bouchard, 1993) et l’inventaire du coping face à la douleur pédiatrique (Spicher, 2003). Quatre temps de mesure ont été planifiés, soit la journée avant la chirurgie, au congé de l’hôpital, au 14ième jour post congé et lors de la visite à la clinique d’orthopédie à un mois post congé. Les résultats montraient que les deux groupes étaient équivalents en ce qui concernait les caractéristiques sociodémographiques et les variables dépendantes. Les participants du groupe expérimental présentaient une douleur générale ressentie dans le dernier 24 heures plus basse au 14ième jour (p = 0,012) et un mois après leur congé de l’hôpital (p = 0,049). Certaines activités quotidiennes ont démontré une différence significative au 14ième jour soit : les activités scolaires ou de travail (p = 0,046), et les activités de soutien vital (manger, dormir) (p = 0,024), Par contre, nous n’avons pas observé de différence significative au niveau de l’anxiété situationnelle et des stratégies adaptatives de coping entre les deux groupes. L’auteur conclut que le niveau de douleur postopératoire des adolescents qui ont subi une chirurgie correctrice de scoliose peut être réduit par un visionnement d’un DVD sur une technique d’imagerie guidée et de relaxation combinée à une intervention éducative (information sur la douleur). De plus, ceci semble avoir un effet positif sur le retour plus rapide de certaines activités quotidiennes.

Introducción: En la práctica neuroquirurgica el uso de tornillos pediculares torácicos ha venido en aumento en el tratamiento de diferentes patologías de la espinales. Desde la descripción original, se confirma la adecuada canalización del trayecto mediante el uso del palpador, sin embargo la validez y seguridad de dicho instrumento es limitada y existe riesgo de complicaciones complejas. En este estudio se comprueba la seguridad y validez del uso del palpador para diagnosticar la integridad del trayecto pedicular torácico. Metodología: Se canalizaron pedículos torácicos en especímenes cadavéricos los cuales de manera aleatoria se clasificaron como normales (íntegros) o anormales (violados). Posteriormente cuatro cirujanos de columna, con diferentes grados de experticia, evaluaron el trayecto pedicular. Se realizaron estudios de concordancia obteniendo coeficiente Kappa, porcentaje total de precisión, sensibilidad, especificidad, VPP y VPN y el área bajo la curva ROC para determinar la precisión de la prueba. Resultados: La precisión y validez en el diagnostico del trayecto pedicular y localización del sitio de violación tienen relación directa con la experiencia y entrenamiento del cirujano, el evaluador con mayor experiencia obtuvo los mejores resultados. El uso del palpador tiene una buena precisión, área bajo la curva ROC 0.86, para el diagnostico de las lesiones pediculares. Discusión: La evaluación precisa del trayecto pedicular, presencia o ausencia de una violación, es dependiente del grado de experiencia del cirujano, adicionalmente la precisión diagnostica de la violación varía según la localización de esta.

El canal lumbar estrecho de tipo degenerativo, es una enfermedad que se presenta en pacientes entre la quinta y la sexta década de vida; es la causa más común de cirugía lumbar después de los 65 años. Este trabajo busca determinar cuáles son los factores asociados a la presentación de eventos adversos o re-intervención en cirugía de canal lumbar estrecho en la Fundación Santa Fe de Bogotá en los años comprendidos entre 2003 y 2013. Métodos: se realizó un estudio de prevalencia de tipo analítico, en donde se analizaron 249 pacientes sometidos a intervención quirúrgica por cirugía de canal lumbar estrecho.

JUSTIFICATIVA E OBJETIVOS: Complicações neurológicas da anestesia subaracnóidea, apesar de raras, podem determinar seqüelas importantes. O objetivo deste estudo é apresentar estas complicações com a finalidade de esclarecer os fatores desencadeantes, o que facilita o diagnóstico das lesões. CONTEÚDO: São apresentadas as seguintes complicações: lesão de nervo desencadeada pela agulha e cateter, cefaléia pós-punção, síndrome da artéria espinhal anterior, hematoma espinhal, meningite bacteriana, meningite asséptica, aracnoidite adesiva, síndrome da cauda eqüina e sintomas neurológicos transitórios. CONCLUSÕES: O conhecimento dos fatores desencadeantes de complicações neurológicas determinadas pela anestesia subaracnóidea pode prevenir as lesões, diagnosticar e tratar mais precocemente e, desse modo, mudar o prognóstico das mesmas.

Muitos estudos têm sido realizados para o entendimento da neuropatogênese das encefalites virais a partir de trabalhos experimentais, porém, nenhum estudo experimental foi dedicado à compreensão da neuropatogênese de membros da família Picornaviridae isolados de morcegos na região amazônica. O vírus Juruaçá, um desses agentes, parcialmente caracterizado como membro da família Picornaviridae por Araújo e colaboradores (2006), causou lesões no encéfalo de camundongos neonatos com presença de gliose reativa, apesar de não provocar efeito citopático (ECP) em cultivos primários de células do sistema nervoso central (SNC), sugerindo que este agente viral seja responsável pela morte dos animais devido a uma intensa resposta imune. O objetivo desse trabalho foi investigar a resposta imune no SNC e alterações celulares causadas pelo vírus Juruaçá em camundongos albinos da linhagem BALB/c neonatos a partir de análises histopatológicas, de ativação microglial e da expressão de citocinas, óxido nítrico (NO) e espécies reativas de oxigênio (ROS). Para tanto, foram realizados processamento de amostras para histopatologia, ensaios imunoenzimáticos, imunohistoquímicos e de imunofluorescência, além de testes para quantificação de NO e ROS e análises estatísticas. Nossos resultados demonstraram que o vírus Juruaçá induz lesões por todo o encéfalo, com maior intensidade no parênquima cortical. Os testes imunohistoquímicos demonstraram a presença de antígenos virais e de micróglias reativas distribuídos por todo o encéfalo e região anterior da medula espinhal. Micróglias com aspecto ameboide, demonstrando intensa ativação, foram observadas principalmente no córtex cerebral, bulbo olfatório, núcleo olfatório anterior, prosencéfalo e diencéfalo próximo ao ventrículo lateral. A produção das citocinas anti-inflamatórias (IL-10, IL-4) diminuiu ao longo do tempo, enquanto que as pró-inflamatórias (IL-12, IL-6, IL-1β, TNF-α, IFN-γ) aumentaram significativamente a partir do 8º dia. Os ensaios para detecção de ROS demonstraram grande produção de radicais superóxido desde o 4º dia, já a produção de NO foi sempre menor nos animais infectados. Provavelmente, a ativação das células gliais, principalmente micróglias, e consequente produção de citocinas pró-inflamatórias e ROS promoveram uma ação devastadora sobre as células do SNC, que coincide com a intensificação dos sinais clínicos. Diante do exposto, ficou evidente que os nossos resultados indicam que o vírus Juruaçá é responsável por uma doença de cunho inflamatório que leva a óbito 100% de camundongos neonatos infectados.

Autogenous bone grafts are considered to be the gold standard in bone regeneration because of their osteogenic activity; however, due to limited availability of intraoral donor sites and the need to resolve the demands of patients requires an alternative to these. Two male patients were submitted to implant surgery in two stages with 6 months intervals between each of them: the first was exodontia and placement of DBM graft into the socket; the second stage was the drill with a 2 mm internal diameter trephine in center of the alveolar ridge previously grafted with DBM and subsequent implant placement. The samples were analyzed under histological techniques. A very mature bone was observed at 6 months after DBM graft placement in the sockets, showing it to be a good alternative as bone graft.

BACKGROUND: There are several techniques for screw insertion in upper cervical spine surgery, and the use of the 3.5-mm screw is usually the standard. However, there is no consensus regarding the feasibility of using these screws in the pediatric population. OBJECTIVE: To determine the measurement of the lamina angle, lamina and pedicle length and thickness, and lateral mass length of the topographic axial view of the axis vertebra of 2- to 10-year-old children to guide the use of surgical screws. METHODS: Seventy-five computed tomography scans from 24- to 120-month-old patients were studied. Measurements were taken in an axial view of C2 and correlated with 2 age groups and both sexes. Statistical analysis was performed with the Student t test. RESULTS: In the 24- to 48-month age group, only 5.5% of the lamina and 8.3% of the pedicles had thicknesses < 3.5 mm. In the 49- to 120-month age group, there were no lamina thickness values < 3.5 mm, and 1.2% of pedicle thicknesses were < 3.5 mm. Both age groups had no lamina and pedicle lengths < 12 mm and no lateral mass lengths > 12 mm. CONCLUSION: In the majority of cases, the use of 3.5-mm lamina and pedicle screws in children is feasible. A base value of 45 degrees for the spinolaminar angle can be adopted as a reference for insertion of screws in the C2 lamina. This information can be particularly useful for decision making during preoperative planning for C1-C2 or craniocervical arthrodesis in children.

Objective: To carry out an anatomical study of the axis with the use of computed tomography (CT) in children aged from two to ten years, measuring the lamina angle, lamina and pedicle length and thickness, and lateral mass length. Methods: Sixty-four CTs were studied from patients aged 24 to 120 months old, of both sexes and without any cervical anomaly. The measurements obtained were correlated with the data on age and sex of the patients. Statistical analysis was performed using the Students "t" tests. Results: We found that within the age range 24-48 months, 5.5% of the lamina and 8.3% of the pedicles had thicknesses of less than 3.5mm, which is the minimum thickness needed for insertion of the screw. Between 49 and 120 months, there were no lamina thicknesses of less than 3.5mm, and 1.2% of the pedicle thicknesses were less than 3.5mm values. Neither of the age groups had any lamina and pedicle lengths of less than 12mm, or lateral mass lengths greater than 12mm. Conclusion: The analysis of the data obtained demonstrates that most of the time, is possible to use a 3.5mm pedicle screw in the laminas and pedicles of the axis in children. OBJETIVO: Estudo anatômico do áxis através de tomografia computadorizada (TC) em crianças de dois a dez anos de idade, mensurando a angulação das lâminas, espessura e comprimento de lâminas e pedículos e espessura da massa lateral. MÉTODOS: Estudou-se 64 TCs da coluna cervical de indivíduos com idades entre 24 e 120 meses, de ambos os sexos e sem deformidades cervicais. Correlacionaram-se as variáveis estudadas com os grupos etários e sexo dos pacientes. A análise estatística foi realizada por meio do teste t. RESULTADOS: Verificou-se que na faixa etária entre 24-48 meses, 5,5% das lâminas e 8,3% dos pedículos possuem espessura menores do que 3,5mm, espessura necessária para colocação de um parafuso. Entre 49-120 meses não há lâminas com espessuras menores do que 3,5mm e 1,2% dos pedículos possuem espessura menor do que 3,5mm. Em ambos os grupos etários não há comprimento de lâminas e pedículos menores do que 12 mm e massas laterais maiores do que 12 mm. CONCLUSÃO: A análise das dimensões obtidas no estudo permite, na maioria dos casos, a colocação de parafusos de 3,5mm nas lâminas e pedículos do áxis de crianças. OBJETIVO: Análise retrospectiva de prontuários de pacientes com instabilidade C1-C2 de causas traumáticas e não-traumáticas, submetidos à artrodese C1-C2. MÉTODOS: Foi realizada análise retrospectiva de prontuários de 20 pacientes do ambulatório de coluna do IOT-HCFMUSP com idades entre 7 e 83 anos (média de 43 anos), de ambos os sexos. Os parâmetros radiográficos para instabilidade foram baseados na medida do intervalo atlanto-axial superior a 3 mm em adultos e a 5 mm em crianças, utilizando-se medidas obtidas através de radiografia simples analisada no perfil. RESULTADOS: Foram operados 20 pacientes com instabilidade cervical alta, a maioria de origem traumática. A técnica cirúrgica mais utilizada foi a artrodese descrita por Magerl. Não foram observadas lesões vasculares. Foi registrada complicação infecciosa em dois pacientes. Obteve-se uma taxa de consolidação da artrodese de 85% e não foram necessárias cirurgias de revisão. CONCLUSÃO: Todas as técnicas utilizadas produziram a consolidação óssea satisfatória e foram excelentes para controlar a instabilidade atlanto-axial.

The "gold standard" for treatment of intervertebral disc herniations and degenerated discs is still spinal fusion, corresponding to the saying "no disc - no pain". Mechanical prostheses, which are currently implanted, do only have medium outcome success and have relatively high re-operation rates. Here, we discuss some of the biological intervertebral disc replacement approaches, which can be subdivided into at least two classes in accordance to the two different tissue types, the nucleus pulposus (NP) and the annulus fibrosus (AF). On the side of NP replacement hydrogels have been extensively tested in vitro and in vivo. However, these gels are usually a trade-off between cell biocompatibility and load-bearing capacity, hydrogels which fulfill both are still lacking. On the side of AF repair much less is known and the question of the anchoring of implants is still to be addressed. New hope for cell therapy comes from developmental biology investigations on the existence of intervertebral disc progenitor cells, which would be an ideal cell source for cell therapy. Also notochordal cells (remnants of the embryonic notochord) have been recently pushed back into focus since these cells have regenerative potential and can activate disc cells. Growth factor treatment and molecular therapies could be less problematic. The biological solutions for NP and AF replacement are still more fiction than fact. However, tissue engineering just scratched the tip of the iceberg, more satisfying solutions are yet to be added to the biomedical pipeline.

With an official life time of over 5 years, Spine Tango can meanwhile be considered the first international spine registry. In this paper we present an overview of frequency statistics of Spine Tango for demonstrating the genesis of questionnaire development and the constantly increasing activity in the registry. Results from two exemplar studies serve for showing concepts of data analysis applied to a spine registry. Between 2002 and 2006, about 6,000 datasets were submitted by 25 centres. Descriptive analyses were performed for demographic, surgical and follow-up data of three generations of the Spine Tango surgery and follow-up forms. The two exemplar studies used multiple linear regression models to identify potential predictor variables for the occurrence of dura lesions in posterior spinal fusion, and to evaluate which covariates influenced the length of hospital stay. Over the study period there was a rise in median patient age from 52.3 to 58.6 years in the Spine Tango data pool and an increasing percentage of degenerative diseases as main pathology from 59.9 to 71.4%. Posterior decompression was the most frequent surgical measure. About one-third of all patients had documented follow-ups. The complication rate remained below 10%. The exemplar studies identified "centre of intervention" and "number of segments of fusion" as predictors of the occurrence of dura lesions in posterior spinal fusion surgery. Length of hospital stay among patients with posterior fusion was significantly influenced by "centre of intervention", "surgeon credentials", "number of segments of fusion", "age group" and "sex". Data analysis from Spine Tango is possible but complicated by the incompatibility of questionnaire generations 1 and 2 with the more recent generation 3. Although descriptive and also analytic studies at evidence level 2++ can be performed, findings cannot yet be generalised to any specific country or patient population. Current limitations of Spine Tango include the low number and short duration of follow-ups and the lack of sufficiently detailed patient data on subgroup levels. Although the number of participants is steadily growing, no country is yet represented with a sufficient number of hospitals. Nevertheless, the benefits of the project for the whole spine community become increasingly visible.

BACKGROUND Clinical observations indicate that the presence of nucleus pulposus (NP) tissue during spinal fusion hinders the rate of disc ossification. While the underlying mechanism remains unknown, this observation could be due to incomplete removal of NP cells (NPCs) that secrete factors preventing disc calcification, such as bone morphogenetic protein (BMP) antagonists including noggin and members of the DAN (differential screening selected gene aberrative in neuroblastoma) family. METHODS Monolayer human bone marrow-derived mesenchymal stem cells (MSCs) were cocultured withNPCs and annulus fibrosus cells (AFCs) embedded in alginate for 21 days. At the end of coculture, MSCs were stained for mineral deposition by alizarin red, and relative expression of bone-related genes [Runt-related transcription factor 2, (RUNX2), Osteopontin (OPN), and Alkaline phosphatase (ALP)] and ALP activity were analyzed. Relative expression of three BMP antagonists, chordin (CHRD), gremlin (GREM1), and noggin (NOG), was determined in primary human NPCs and AFCs. These cells were also stained for Gremlin and Noggin by immunocytochemistry. RESULTS Alizarin red staining showed that MSC osteogenesis in monolayer cultures was inhibited by coculture with NPCs or AFCs. ALP activity and RT-PCR analyses confirmed these results and demonstrated inhibition of osteogenesis of MSC in the presence of disc cells. NOG was significantly up-regulated in MSCs after coculture. Relative gene expression of intervertebral disc (IVD) cells showed higher expression of GREM1 in NPCs than in AFCs. CONCLUSIONS We show that primary IVD cells inhibit osteogenesis of MSCs. BMP inhibitors NOG, GREM1 and CHRD were expressed in IVD cells. GREM1 appears to be differentially expressed in NPCs and AFCs. Our results have implications for the design and development of treatments for non-union in spinal fusion.
