A primeira referência a controlo de infecção em Portugal remonta a 1930 mas é só em 1979 que é publicada a primeira circular informativa da Direcção-Geral dos Hospitais a qual divulgava a Resolução 31 do Conselho da Europa sobre a institucionalização das Comissões de Controlo de Infecção. Em 1986 é recomendado a todas as unidades de saúde o controlo de infecção, também pela DGH e, novamente, seguindo uma disposição do Conselho da Europa. Em 1993 aquela Direcção Geral decide pela necessidade de institucionalização das CCIH mas é só em 1996 que são criadas as CCIH em todas as unidades hospitalares públicas e privadas com definição, afectação de recursos humanos, físicos e financeiros e definida a composição e as atribuições. Três anos depois nasce o Programa Nacional de Controlo de Infecção com o objectivo de divulgar a verdadeira dimensão do problema e promover as medidas necessárias para a prevenção da infecção. O PNCI foi criado na DGS em 1996, transferido para o INS Dr. Ricardo Jorge em 1999, tendo regressado à DGS em 2006. No ano seguinte foi aprovado pelo Sr. Ministro da Saúde Dr. Correia de Campos e publicado em DR o Programa Nacional de Controlo de Infecção Associada aos Cuidados de Saúde. No mesmo ano é determinada pela DGS a reestruturação das CCI em todas as unidades de saúde, definida a organização, constituição e atribuições dos agrupamentos de Centros de Saúde, Administrações Regionais e Unidades de Cuidados Continuados. O PNCI tem missão bem definida e projectos desenvolvidos em áreas de vigilância epidemiológica, desenvolvimento de normas e boa prática e funções de consultoria e apoio. Em vigilância epidemiológica tem em campo os projectos HELICS-UCI, HELICS - Cirurgia, Infecções nosocomiais da corrente sanguínea, infecções em UCI - recém-nascidos e Inquéritos Nacionais de Prevalência. Têm sido emanadas inúmeras normas de boa prática e protocolos divulgados no sítio da DGS, micro sítio do PNCI. Os vários estudos têm gerado informação sob o ponto de vista nosológico, microbiológico, de resistência bacteriana e de uso de antibióticos, de importância fundamental para intervenção dirigida e avaliação de resultados.

BACKGROUND: The use of cardiac output monitoring may improve patient outcomes after major surgery. However, little is known about the use of this technology across nations. METHODS: This is a secondary analysis of a previously published observational study. Patients aged 16 years and over undergoing major non-cardiac surgery in a 7-day period in April 2011 were included into this analysis. The objective is to describe prevalence and type of cardiac output monitoring used in major surgery in Europe. RESULTS: Included in the analysis were 12,170 patients from the surgical services of 426 hospitals in 28 European nations. One thousand four hundred and sixteen patients (11.6 %) were exposed to cardiac output monitoring, and 2343 patients (19.3 %) received a central venous catheter. Patients with higher American Society of Anesthesiologists (ASA) scores were more frequently exposed to cardiac output monitoring (ASA I and II, 643 patients [8.6 %]; ASA III-V, 768 patients [16.2 %]; p < 0.01) and central venous catheter (ASA I and II, 874 patients [11.8 %]; ASA III-V, 1463 patients [30.9 %]; p < 0.01). In elective surgery, 990 patients (10.8 %) were exposed to cardiac output monitoring, in urgent surgery 252 patients (11.7 %) and in emergency surgery 173 patients (19.8 %). A central venous catheter was used in 1514 patients (16.6 %) undergoing elective, in 480 patients (22.2 %) undergoing urgent and in 349 patients (39.9 %) undergoing emergency surgery. Nine hundred sixty patients (7.9 %) were monitored using arterial waveform analysis, 238 patients (2.0 %) using oesophageal Doppler ultrasound, 55 patients (0.5 %) using a pulmonary artery catheter and 44 patients (2.0 %) using other technologies. Across nations, cardiac output monitoring use varied from 0.0 % (0/249 patients) to 27.5 % (19/69 patients), whilst central venous catheter use varied from 5.6 % (7/125 patients) to 43.2 % (16/37 patients). CONCLUSIONS: One in ten patients undergoing major surgery is exposed to cardiac output monitoring whilst one in five receives a central venous catheter. The use of both technologies varies widely across Europe.

RESUMO - Actualmente, a diabetes mellitus é a doença crónica que representa o maior desafio aos sistemas de saúde, e é um importante problema de saúde pública, tanto no que concerne ao crescente aumento do número de pessoas afectadas pela doença, incapacidade, morbilidade e mortalidade prematura, como nos custos envolvidos no controlo e tratamento das suas complicações. Nesse sentido, e consciente do grande peso económico e epidemiológico que a diabetes mellitus acarreta, este trabalho versa sobre: (1) identificar a prevalência da diabetes mellitus, em 2010, na região do Alto Alentejo; (2) conhecer as evidências sobre as intervenções e cuidados prestados aos diabéticos desta região; e (3) analisar os custos directos com o internamento hospitalar, cujo diagnóstico principal foi a diabetes, no ano 2010, nesta região. Através deste estudo, constatamos que a diabetes, na região do Alto Alentejo, no universo dos diabéticos identificados (n=8.767), representou uma prevalência de 7,7 diabéticos por 100 habitantes. Quanto ao género, a prevalência é superior no sexo feminino (8,1%), em relação ao sexo masculino (7,2%). A média da idade foi de 67,5 anos, e verificou-se que a diabetes aumenta em grande escala com a idade, encontrando-se uma taxa de prevalência superior em idades acima dos 60 anos. Referente ao seguimento dos cuidados de saúde prestados pela ULS aos doentes com diabetes mellitus, fica muito aquém do que as Guidelines definem, para os cuidados prestados a estes doentes, tanto em termos de seguimento da própria doença, como na prevenção das suas complicações. Em relação ao internamento hospitalar, em 2010, registaram-se 255 episódios cujo diagnóstico principal foi a diabetes mellitus (250.xx), correspondendo estes casos a um total de 761.990€. A demora média de internamento foi de 10 dias (11,5 para o sexo masculino e 8,5 para o sexo feminino) e, a média das idades situa-se nos 65,5 anos.

Em avaliação dos prontuários médicos de 3.139 pacientes picados por serpentes do gênero Bothrops atendidos no Hospital Vital Brazil (HVB), de 1981 a 1990, observou-se maior acometimento do sexo masculino (75,7%). Em 1.412 casos (45,0%) a serpente foi identificada, sendo 1.376 B. jararaca, 20 B. jararacussu, 11 B. neuwiedi, 2 B. moojeni, 2 B. alternatus e 1 B. pradoi. As regiões anatômicas mais comumente picadas foram: pé (47,5%) e mão (21,3%). O torniquete foi realizado em 38,2% dos casos e sua freqüência diminuiu durante esse período (p < 0,001). As manifestações clínicas mais freqüentes foram: dor (95,6%), edema (95,4%), equimose (56,1%), bolha (13,8%), necrose (16,5%), abscesso (11,0%), sangramento extratecidual (12,3%), insuficiência renal (1,6%) e choque (0,7%). A coagulação sanguínea foi avaliada em 2.990 casos e estava alterada em 1.730 (57,9%). Ocorreram 21 amputações (0,7%) e 9 óbitos (0,3%). A dose de soro administrada no HVB diminuiu ao longo desse período (p < 0,001).

We present a calibrated model of the UK mobile telephony market with four mobile networks; calls to and from the fixed network; network-based price discrimination; and call externalities. Our results show that reducing mobile termination rates broadly in line with the recent European Commission Recommendation to either pure long-run incremental cost ; reciprocal termination charges with fixed networks; or Bill & Keep (i.e. zero termination rates), increases social welfare, consumer surplus and networks profits. Depending on the strength of call externalities, social welfare may increase by as much as £ 990 million to £ 4.5 billion per year, with Bill & Keep leading to the highest increase in welfare. We also apply the model to estimate the welfare effects of the 2010 merger between Orange and T-Mobile under different scenarios concerning MTRs, and predict that consumer surplus decreases strongly.

The GxE interaction only became widely discussed from evolutionary studies and evaluations of the causes of behavioral changes of species cultivated in environments. In the last 60 years, several methodologies for the study of adaptability and stability of genotypes in multiple environments trials were developed in order to assist the breeder's choice regarding which genotypes are more stable and which are the most suitable for the crops in the most diverse environments. The methods that use linear regression analysis were the first to be used in a general way by breeders, followed by multivariate analysis methods and mixed models. The need to identify the genetic and environmental causes that are behind the GxE interaction led to the development of new models that include the use of covariates and which can also include both multivariate methods and mixed modeling. However, further studies are needed to identify the causes of GxE interaction as well as for the more accurate measurement of its effects on phenotypic expression of varieties in competition trials carried out in genetic breeding programs.

INTRODUCTION: from an epidemiological point of view, more than 120 species of triatomine (Hemiptera, Reduviidae) are known. The occurrence and positivity for Trypanosoma cruzi in triatomines in 16 municipalities of the Triângulo Mineiro and Alto Paranaíba were evaluated from January 2002 to December 2004. METHODS: the triatomines were captured basically according to the classic norms of the National Health Foundation. The parasitological exams of the triatomines were conducted according to the technique described by the Ministry of Health. During the study period, 990 specimens of triatomines were captured and of these, 771 could be examined. RESULTS: five species were identified: Triatoma sordida, Panstrongylus diasi, Panstrongylus megistus, Panstrongylus geniculatus and Rhodnius neglectus. Triatoma sordida represented 71.5% of all the triatomines captured, followed by Panstrongylus megistus (18%), Rhodnius neglectus (9.3%), Panstrongylus diasi (0.8%) and Panstrongylus geniculatus (0.4%). Of the total number of triatomines examined, 2.7% were positive for Trypanosoma cruzi. Panstrongylus megistus was the species that presented the highest rates of infection by Trypanosoma cruzi (8.3%), followed by Rhodnius neglectus (2.9%) and Triatoma sordida (1.4%). CONCLUSIONS: there is a need to adapt to new circumstances in epidemiology, with greater emphasis on entomological surveillance, since the potential for adaptation of secondary species of triatomines exists, especially where Chagas' disease is already under control.

Introduction: The Baixada Maranhense Microregion currently has the highest prevalence of schistosomiasis in the State of Maranhão, likely because this parasitosis is characterized as an occupational disease, and increased contact with water increases the risk of infection by Schistosoma mansoni. This paper reports the results of the first comprehensive freshwater malacological survey performed in the Baixada Maranhense Microregion. Methods: Freshwater mollusks were collected from the twenty-one municipalities of the Baixada Maranhense Microregion and from Bacurituba and Cajapió and were evaluated for infection by trematodes. Results: A total of 9,129 mollusks were collected (sixteen species), which included the first records of six species in the State of Maranhão: Gundlachia radiata, G. ticaga, Hebetancylus moricandi, Plesiophysa guadeloupensis, Pomacea bridgesii diffusa and Omalonyx sp. Biomphalaria glabrata was found in five municipalities, whereas B. straminea was found in nine. Biomphalaria glabrata and B. straminea were observed in syntopy in Pinheiro and São Bento. Of the 990 specimens of B. glabrata and the 2,109 specimens of B. straminea that were exposed to and/or analyzed for the presence of larval trematodes, only a single specimen of B. glabrata (0.1%) from São Bento shed S. mansoni. Other larval trematodes were first observed in mollusks from the State of Maranhão. Conclusions: These results indicate that the study area is epidemiologically important due to the presence of two natural vectors of schistosomiasis and the active transmission of schistosomiasis, which was confirmed in the infected specimen that was collected in this study.

IntroductionWe compared feces culturing in charcoal or vermiculite to obtain Strongyloides venezuelensis larvae.MethodsFeces (5g) from infected rats was mixed with vermiculite (10g) or coal (10g) in plastic cups and incubated at 28°C for 48h. Larvae were recovered using Baermann-Moraes method.ResultsSignificantly higher number of positive larval cultures were recovered from vermiculite than from charcoal (15/17 and 4/17, respectively; p < 0.001; 990.6 ± 307.5 and 215 ± 78.1 larvae, p = 0.027).ConclusionsVermiculite yields more larvae and provides cleaner pellets, improving larvae identification and facilitating their use for other purposes.

INTRODUCTION : The tuberculin test is a diagnostic method for detecting latent tuberculosis (TB) infection, especially among disease contact cases. The objective of this study was to analyze the prevalence and evolution of Mycobacterium tuberculosis infection among TB contact cases. METHODS : A retrospective cohort study was performed in a reference center for TB. The study population consisted of 2,425 patients who underwent a tuberculin test from 2003 to 2010 and whose results indicated contact with individuals with TB. The data were collected from the registry book of the tuberculin tests, patient files and the Information System Records of Notification Grievance. To verify the evolution of TB, case records through September 2014 were consulted. Data were analyzed using the Statistical Package for the Social Sciences (SPSS). In all hypothesis tests, a significance level of 0.05 was used. RESULTS : From the studied sample, 435 (17.9%) contacts did not return for reading. Among the 1,990 contacts that completed the test, the prevalence of latent TB infection was 35.4%. Of these positive cases, 50.6% were referred to treatment; the dropout rate was 42.5%. Among all of the contacts, the TB prevalence was 1.8%, from which 13.2% abandoned treatment. CONCLUSIONS : The collected data indicate the need for more effective public policies to improve TB control, including administering tests that do not require a return visit for reading, enhancing contact tracing and encouraging actions that reinforce full treatment adherence.

OBJECTIVE: To determine the utility of B-type natriuretic peptide (BNP) in the diagnosis of congestive heart failure (CHF) in patients presenting with dyspnea to an emergency department (ED). METHODS: Seventy patients presenting with dyspnea to an ED from April to July 2001 were included in the study. Mean age was 72±16 years and 33 (47%) were male. BNP was measured in all patients at the moment of admission to the ED. Emergency-care physicians, blinded to BNP values, were required to assign a probable initial diagnosis. A cardiologist retrospectively reviewed the data (blinded to BNP measurements) and assigned a definite diagnosis, which was considered the gold standard for assessing the diagnostic performance of BNP. RESULTS: The mean BNP concentration was higher in patients with CHF (n=36) than in those with other diagnoses (990±550 vs 80±67 pg/mL, p<0.0001). Patients with systolic dysfunction had higher BNP levels than those with preserved systolic function (1,180±641 vs 753±437 pg/mL, p=0.03). At a blood concentration of 200 pg/mL, BNP showed a sensitivity of 100%, specificity of 97.1%, positive predictive value of 97.3%, and negative predictive value of 100%. The application of BNP could have potentially corrected all 16 cases in which the diagnosis was missed by the emergency department physician. CONCLUSION: BNP measurement is a useful tool in the diagnosis of CHF in patients presenting to the ED with dyspnea.

OBJETIVO: Verificar a ação do hormônio de crescimento (GH) sobre a remodelação miocárdica em ratos infartados. MÉTODOS: Ratos Wistar foram submetidos ao infarto e tratados com GH (IAM-GH; n=8), ou com o veículo (IAM; n=8). Ratos controles foram submetidos ao infarto simulado (C; n=8). Após 30 dias, os animais foram submetidos ao estudo funcional por meio de ecocardiograma e as alterações da contratilidade miocárdica estudadas no músculo papilar isolado do ventrículo esquerdo (VE). RESULTADOS: O ecocardiograma identificou aumento nos diâmetros (mm) diastólico (C=7,32±0,49; IAM=8,50±0,73; IAM-GH= 9,34± 0,73; P<0,05) e sistólico (C=3,38±0,47, IAM= 5,16±1,24; IAM-GH=5,96±1,54; P<0,05) no VE dos animais com infarto. Os animais do grupo IAM-GH apresentaram menor fração (%) de ejeção (C=0,9±0,03; IAM=0,76±0,12; IAM-GH=0,72±0,14; P<0,05 para C vs IAM-GH) em relação aos controles. O estudo do músculo papilar isolado do ventrículo esquerdo mostrou que o grupo IAM-GH apresentou alterações (C=1,50±0,59; IAM= 1,28±0,38; IAM-GH=1,98±0,41; P<0,05 para C vs IAM-GH) somente na tensão de repouso (TR - g/mm²) e no delta do tempo para a tensão desenvolvida decrescer 50% (TR50, ms) após estimulação com cálcio (C=23,75±9,16; IAM=-16,56±14,82; IAM-GH=-4,69±8,39; P<0,05 para C vs IAM-GH) e no delta da tensão desenvolvida (TD, g/mm²) após estimulação com isoproterenol (C=0,99±0,17; IAM=0,54±0,62; IAM-GH= 0,08±0,75; P<0,05 para C vs IAM-GH), em relação aos animais controle. CONCLUSÃO: A administração precoce do GH no modelo de infarto experimental em ratos pode resultar em efeitos adversos no processo de remodelação ventricular.
