We have previously reported that human breast carcinoma (HBC) cell lines expressing the mesenchymal intermediate filament protein vimentin (VIM+) are highly invasive in vitro, and highly metastatic in nude mice when compared to their VIM- counterparts. Since only VIM+ cell lines can be induced to activate matrix metalloproteinase-2 (MMP-2) upon stimulation with Concanavalin A (Con A), we have examined here membrane type 1 MMP (MT1-MMP), a cell surface activator of MMP-2. Northern analysis reveals baseline expression of MT1-MMP in five of the six VIM+ cell lines studied (MDA-MB-231, MDA-MB-435, BT-549, Hs578T, MCF-7(ADR)), each of which showed variable activation of exogenous MMP-2 after treatment with Con A. In contrast, the four VIM-, poorly invasive HBC cell lines studied (MCF-7, T47D, MDA-MB 468, ZR-75-1) lacked baseline MT1-MMP mRNA expression, and showed no induction of either MT1-MMP expression or MMP-2-activation with Con A. Such differential MT1-MMP expression was confirmed in vivo using in situ hybridization analysis of nude mouse tumor xenografts of representative cell lines. Western analysis of the MDA-MB-231 cells revealed baseline membrane expression of a 60 kDa species, which was strongly induced by Con A treatment along with a weaker band co-migrating with that from MT1-MMP-transfected COS-1 cells (63 kDa), presumably representing latent MT1-MMP. MT1-MMP immunofluorescence strongly decorated Con A-stimulated MDA-MB-231 cells in a manner consistent with membranous staining, but did not decorate the unstimulated MDA-MB-231 cells or MCF-7 cells under either condition. Collectively, the results suggest the constitutive production of active MT1-MMP which is unavailable for either MMP-2 activation or immuno-decoration until Con A treatment. Since VIM expression arises by virtue of the so-called epithelial to mesenchymal transition (EMT) in invasive embryonic epithelia, we propose that this represents a major metastasis mechanism in breast carcinomas. MT1-MMP on the surface of such 'fibroblastoid' carcinoma cells may mediate a paracrine loop for the utilization of stromally produced MMP-2, and contribute to the poorer survival associated with VIM+ breast carcinomas.

Recently, two landmark reports on antiangiogenic therapy were published: Paez-Ribes and colleagues and Ebos and colleagues . The Board of the Metastasis Research Society (MRS) congratulates the authors for their informative articles that help to explain the puzzle of why antiangiogenic agents have had a relatively minor or no significant impact on patient survival. Using four model systems and several different strategies, these researchers showed that inhibition of angiogenesis reduced primary tumor growth and microvessel density in keeping with many earlier reports, but strikingly, accelerated invasion and metastasis.

This chapter considers the key characteristics of different types of child abuse and neglect, and outlines the nature and justifiability of mandatory reporting laws. The issue of whether these laws may be useful for child protection in developing countries with emerging economies is an important one. ‘Developing country’ is a term used by various institutions to describe a nation which has a lower living standard, industrial base, and human development index (HDI) compared to other countries (World Bank 2012; United Nations Development Programme 2013). In the context of developing countries, the chapter addresses two questions: first, might some forms of maltreatment be more suited to mandatory reporting than others? Second, what options for child protection may be considered by developing countries, taking into account children’s needs, cultural conditions and practices, economic imperatives, and the different levels of preparedness to implement child protection strategies?

This program of research used a mixed methods approach to explore the cultural, social and psychosocial factors that influence women's alcohol consumption. Results indicated that there were a number of common influencing factors across women of all ages but also a number of key influences and behaviours that were distinct for younger and older women. These findings emphasised the need for age-specific interventions that target these influences to reduce women's exposure to alcohol-related harm. This research is one of the first studies to examine alcohol consumption of both younger and older women.

The secretive 2011 Anti-Counterfeiting Trade Agreement – known in short by the catchy acronym ACTA – is a controversial trade pact designed to provide for stronger enforcement of intellectual property rights. The preamble to the treaty reads like pulp fiction – it raises moral panics about piracy, counterfeiting, organised crime, and border security. The agreement contains provisions on civil remedies and criminal offences; copyright law and trademark law; the regulation of the digital environment; and border measures. Memorably, Susan Sell called the international treaty a TRIPS Double-Plus Agreement, because its obligations far exceed those of the World Trade Organization's TRIPS Agreement 1994, and TRIPS-Plus Agreements, such as the Australia-United States Free Trade Agreement 2004. ACTA lacks the language of other international intellectual property agreements, which emphasise the need to balance the protection of intellectual property owners with the wider public interest in access to medicines, human development, and transfer of knowledge and technology. In Australia, there was much controversy both about the form and the substance of ACTA. While the Department of Foreign Affairs and Trade was a partisan supporter of the agreement, a wide range of stakeholders were openly critical. After holding hearings and taking note of the position of the European Parliament and the controversy in the United States, the Joint Standing Committee on Treaties in the Australian Parliament recommended the deferral of ratification of ACTA. This was striking as representatives of all the main parties agreed on the recommendation. The committee was concerned about the lack of transparency, due process, public participation, and substantive analysis of the treaty. There were also reservations about the ambiguity of the treaty text, and its potential implications for the digital economy, innovation and competition, plain packaging of tobacco products, and access to essential medicines. The treaty has provoked much soul-searching as to whether the Trick or Treaty reforms on the international treaty-making process in Australia have been compromised or undermined. Although ACTA stalled in the Australian Parliament, the debate over it is yet to conclude. There have been concerns in Australia and elsewhere that ACTA will be revived as a ‘zombie agreement’. Indeed, in March 2013, the Canadian government introduced a bill to ensure compliance with ACTA. Will it be also resurrected in Australia? Has it already been revived? There are three possibilities. First, the Australian government passed enhanced remedies with respect to piracy, counterfeiting and border measures in a separate piece of legislation – the Intellectual Property Laws Amendment (Raising the Bar) Act 2012 (Cth). Second, the Department of Foreign Affairs and Trade remains supportive of ACTA. It is possible, after further analysis, that the next Australian Parliament – to be elected in September 2013 – will ratify the treaty. Third, Australia is involved in the Trans-Pacific Partnership negotiations. The government has argued that ACTA should be a template for the Intellectual Property Chapter in the Trans-Pacific Partnership. The United States Trade Representative would prefer a regime even stronger than ACTA. This chapter provides a portrait of the Australian debate over ACTA. It is the account of an interested participant in the policy proceedings. This chapter will first consider the deliberations and recommendations of the Joint Standing Committee on Treaties on ACTA. Second, there was a concern that ACTA had failed to provide appropriate safeguards with respect to civil liberties, human rights, consumer protection and privacy laws. Third, there was a concern about the lack of balance in the treaty’s copyright measures; the definition of piracy is overbroad; the suite of civil remedies, criminal offences and border measures is excessive; and there is a lack of suitable protection for copyright exceptions, limitations and remedies. Fourth, there was a worry that the provisions on trademark law, intermediary liability and counterfeiting could have an adverse impact upon consumer interests, competition policy and innovation in the digital economy. Fifth, there was significant debate about the impact of ACTA on pharmaceutical drugs, access to essential medicines and health-care. Sixth, there was concern over the lobbying by tobacco industries for ACTA – particularly given Australia’s leadership on tobacco control and the plain packaging of tobacco products. Seventh, there were concerns about the operation of border measures in ACTA. Eighth, the Joint Standing Committee on Treaties was concerned about the jurisdiction of the ACTA Committee, and the treaty’s protean nature. Finally, the chapter raises fundamental issues about the relationship between the executive and the Australian Parliament with respect to treaty-making. There is a need to reconsider the efficacy of the Trick or Treaty reforms passed by the Australian Parliament in the 1990s.

Pancreatic exocrine dysfunction has been frequently recorded in protein-energy malnutrition in underdeveloped countries. In addition, the pancreas requires optimal nutrition for enzyme synthesis and potentially correctable pancreatic enzyme insufficiency may play a role in the continuation of protein-energy malnutrition. This problem has not been previously evaluated in Australian Aborigines. We have applied a screening test for pancreatic dysfunction (human immunoreactive trypsinogen [IRT] assay) to the study of 398 infants (6-36 months) admitted to the Alice Springs Hospital over a 20-month period. All infants were assessed by anthropometric measures and were assigned to to three nutritional groups (normal, moderate or severely malnourished) and two growth groups (stunted or not stunted). Of the 198 infants who had at least a single serum cationic trypsinogen measurement taken, normal values for serum IRT (with confidence limits) were obtained from 57 children, who were normally nourished. IRT levels were significantly correlated with the degree of underweight but there was no correlation with the degree of stunting or age. Mean IRT levels for the moderate and severely underweight groups were significantly greater than the mean for the normal group (P < 0.01). Seventeen children (8.6%) had trypsinogen levels in excess of the 95th percentile for the normally nourished group, reflecting acinar cell damage or ductal obstruction. We conclude that pancreatic dysfunction may be a common and important overlooked factor contributing to ongoing malnutrition and diseases in malnourished Australian Aboriginal children.

- Objectives To develop and test a valid and reliable assessment of wheelchair skills for individuals with spinal cord injuries (SCI); the Queensland Evaluation of Wheelchair Skills (QEWS). - Setting Hospital, Australia. - Methods Phase 1: Four Delphi panel rounds with clinical experts were used to develop the QEWS. Phase 2: Intra-rater and inter-rater reliability of the QEWS items were examined in 100 people with SCI. Phase 3a: Concurrent validity was investigated by examining the association between QEWS total scores and physiotherapists’ global ratings of wheelchair skill performance. Phase 3b: Construct validity was tested in 20 people with recent SCI by examining change in QEWS total scores between when they first mobilised in a wheelchair and scores obtained 10 weeks later. - Results Phase 1: The QEWS was developed. Phase 2: The intra-class correlation coefficients reflecting the intra-rater reliability and the inter-rater reliability for the QEWS total score were 1.00 and 0.98, with scores being within one point of each other 96 and 91% of the time, respectively. Phase 3a: The QEWS total scores were comparable with the global rating of wheelchair skill performance (r2=0.93). Phase 3b: The QEWS scores changed by a median (interquartile range (IQR)) of 4 (1 to 6) points over the 10-week period following first wheelchair mobilisation. - Conclusion The QEWS is a valid and reliable tool for measuring wheelchair skills in individuals with SCI. The QEWS is efficient and practical to administer and does not require specialised equipment.

Emerging contaminants (ECs) are chemical compounds commonly present in water. It is only recently that this family of compounds is being recognized as significant water pollutants (. ECs include a wide variety of chemicals such as pharmaceutical and personal care products (PPCPs), pesticides, hydrocarbons and hormones, among others, that once released into the environment exert adverse impacts on the human and wildlife endocrine system. Natural attenuation and conventional treatment processes are not capable of removing these micro-pollutants detected in wastewater influent and effluent and surface and drinking water. The main challenges related with presence of ECs in stormwater in the context of reuse are: a) Development of suitable laboratory test methodologies and protocols for ECs identification and quantification b) Identification of the sources of ECs in the urban environment; c) Understanding their impacts on human and/or ecosystem health; and d). Development of cost-effective removal technologies which are appropriate for large as well as small-scale application.

In line with cultural psychology and developmental theory, a single case approach is applied to construct knowledge on how children s interaction emerge interlinked to historical, social, cultural, and material context. The study focuses on the negotiation of constraints and meaning construction among 2-to 3-year-old children, a preschool teacher, and the researcher in settings with water. Water as an element offers a special case of cultural canalization: adults selectively monitor and guide children s access to it. The work follows the socio-cultural tradition in psychology, particularly the co-constructivist theory of human development and the Network of Meanings perspective developed at the University of São Paulo. Valsiner s concepts of Zone of Free Movement and Zone of Promoted Action are applied together with studies where interactions are seen as spaces of construction where negotiation of constraints for actions, emotions, and conceptions occur. The corpus was derived at a Finnish municipal day care centre. During a seven months period, children s actions were video recorded in small groups twice a month. The teacher and the researcher were present. Four sessions with two children were chosen for qualitative microanalysis; the analysis also addressed the transformations during the months covered by the study. Moreover, the data derivation was analyzed reflectively. The narrowed down arenas for actions were continuously negotiated among the participants both nonverbally and verbally. The adults expectations and intentions were materialized in the arrangements of the setting canalizing the possibilities for actions. Children s co-regulated actions emerged in relation to the adults presence, re-structuring attempts, and the constraints of the setting. Children co-constructed novel movements and meanings in relation to the initiatives and objects offered. Gestures, postures, and verbalizations emerged from the initially random movements and became constructed to have specific meanings and functions; meaning construction became abbreviated. The participants attempted to make sense of the ambiguous (explicit and implicit) intentions and fuzzy boundaries of promoted and possible actions: individualized yet overlapping features were continuously negotiated by all the participants. Throughout the months, children s actions increasingly corresponded adults (re-defined) conceptions of water researchers as an emerging group culture. Water became an instrument and a context for co-regulations. The study contributes to discussions on children as participants in cultural canalization and emphasizes the need for analysis in early childhood education practices on the implicit and explicit constraint structures for actions.

In many primitively eusocial wasp species new nests are founded either by a single female or by a small group of females. In the single foundress nests, the lone female develops her ovaries, lays eggs as well as tends her brood. In multiple foundress nests social interactions, especially dominance-subordinate interactions, result in only one `dominant' female developing her ovaries and laying eggs. Ovaries of the remaining `subordinate' cofoundresses remain suppressed and these individuals function as workers and tend the dominant's brood. Using the tropical, primitively eusocial polistine wasp Ropalidia marginata and by comparing wasps held in isolation and those kept as pairs in the laboratory, we demonstrate that social interactions affect ovarian development of dominant and subordinate wasps among the pairs in opposite directions, suppressing the ovaries of the subordinate member of the pair below that of solitary wasps and boosting the ovaries of dominant member of the pair above that of solitary females. In addition to being of physiological interest, such mirror image effects of aggression on the ovaries of the aggressors and their victims, suggest yet another mechanism by which subordinates can enhance their indirect fitness and facilitate the evolution of worker behavior by kin selection. O consumo de etanol durante a gestação é um grave problema de saúde pública. Durante o desenvolvimento, o sistema nervoso é especialmente susceptível aos efeitos tóxicos do etanol e a exposição ao etanol durante este período pode gerar um amplo espectro de distúrbios neurocomportamentais, sendo o mais frequente, a hiperatividade. Recentemente, estudos têm sugerido que distúrbios na plasticidade neuronal podem estar relacionados com a hiperatividade. Os inibidores de PDE são drogas que agem impedindo a degradação de segundos mensageiros celulares como AMPc e GMPc, mantendo a ativação de proteínas quinases e de fatores de transcrição como o CREB, levando a expressão de genes relacionados à plasticidade. Neste trabalho, avaliamos através do teste de campo aberto se a administração de Vinpocetina ou Rolipram (inibidores de PDE) seria capaz de amenizar ou reverter a hiperatividade de camundongos Suíços expostos ao etanol no período correspondente ao terceiro trimestre de gestação humana. Para tanto, foram realizadas duas etapas: na primeira etapa, durante o período neonatal, os animais receberam injeções intraperitoneais de etanol (5g/Kg em solução salina a 25%, no 2, 4, 6 e 8 dias de vida pós-natal - PN2 a PN8) ou de salina, e 4 horas antes do teste comportamental no campo aberto (10 min), em PN30, receberam Vinpocetina (10mg/Kg ou 20mg/Kg diluídas em DMSO ip) ou somente DMSO ip. Na segunda etapa, os animais foram expostos ao etanol ou à salina no período neonatal nas mesmas condições da primeira etapa e no dia do teste comportamental receberam Rolipram (0,5mg/Kg diluídas em DMSO ip ou somente DMSO ip). Posteriormente aos testes, foram coletados o córtex cerebral frontal e o hipocampo dos animais para avaliação dos níveis de AMPc. Os resultados comportamentais indicam que somente o tratamento com Vinpocetina (20mg/Kg) reverteu a hiperatividade de camundongos expostos ao etanol, resultado que não foi observado com o tratamento com Rolipram. Desta forma, a dosagem dos níveis de AMPc foi realizada apenas nos animais que receberam injeção de Vinpocetina (20mg/Kg). A exposição neonatal ao etanol reduziu significativamente os níveis de AMPc no córtex e no hipocampo. O tratamento com Vinpocetina gerou um aumento nos níveis de AMPc no córtex e restaurou estes níveis no hipocampo. Nossos resultados sugerem que a reversão da hiperatividade pelo tratamento com Vinpocetina pode estar associada ao aumento da plasticidade neural induzida por esta droga.

Este estudo apoia-se na abordagem sociocultural, em uma perspectiva interacionista da relação biologia cultura, beneficiando-se também do olhar da psicologia evolucionista para os fenômenos humanos. Estas abordagens, a partir de uma visão do homem como biologicamente cultural fazem-se relevantes para o estudo de crenças e conhecimento sobre o desenvolvimento de crianças com síndrome de Down (SD). Esta síndrome tem prevalência de um a cada 700 nascimentos, não importando classe social, racial ou local de nascimento dos pais, ou seja, é universal. A revisão da literatura revelou uma carência de estudos psicológicos sobre o contexto de desenvolvimento dessas crianças, inclusive as crenças de seus cuidadores e de profissionais de saúde. Sendo assim, o objetivo desse trabalho foi investigar crenças e conhecimento de dois grupos (pais e profissionais de saúde) sobre o desenvolvimento de crianças com síndrome de Down até dois anos de idade no Estado do Rio de Janeiro. Participaram da pesquisa 101 pessoas sendo 60 pais com filhos de até oito anos com síndrome de Down e 41 profissionais de saúde, médicos ou residentes do Instituto Fernandes Figueira, IFF/Fiocruz. Foram utilizados os seguintes instrumentos: Questionário com duas perguntas abertas sobre crenças sobre síndrome de Down que foram respondidas livremente pelos participantes; inventário sobre concepção de desenvolvimento infantil (ICDI); inventário sobre conhecimento de desenvolvimento infantil (KIDI) modificado, adaptado para crianças com síndrome de Down. Os dados foram analisados em aspectos qualitativos e quantitativos. A aplicação dos instrumentos foi realizada individualmente, em local conveniente para o participante ou no IFF/Fiocruz e após a assinatura do termo de consentimento. Os dados dos três instrumentos foram tratados e reduzidos. As respostas ao instrumento de crenças foram organizadas em categorias e comparadas. Escores nas diferentes subescalas do ICDI foram calculados e, em cada grupo (pais e profissionais) analisaram-se as concepções sobre desenvolvimento predominantes, estabelecendo-se comparações entre eles. Escores nas diferentes partes do KIDI foram ainda calculados (porcentagem de acertos). Foram feitas comparações intra e entre grupos. Os resultados foram tratados em cada um dos aspectos: crenças sobre SD, concepções e conhecimento sobre desenvolvimento. Os resultados obtidos mostram que as crenças dos pais estão distribuídas em oito categorias com três focos distintos (na criança, nos pais ou nos dois) e a dos profissionais em nove categorias, também, com três focos distintos (na SD, no médico e na criança e família). O resultado obtido no ICDI indica que os participantes valorizam mais as concepções de aprendizagem e interacionismo do que de maturação e que não há diferença significativa entre os grupos. Para o KIDI observou-se diferença significativa entre os grupos tanto no resultado geral de percentual de acertos como nos resultados em cada subescala. Espera-se que os resultados obtidos possam contribuir para a literatura sobre psicologia do desenvolvimento e síndrome de Down.

Este estudo investigou a implementação da Política Nacional de Educação Permanente da Saúde (PNEPS) no Estado do Rio de Janeiro, durante o ano de 2006. A PNEPS, fundamentalmente, visa mudança das práticas de saúde por meio da educação permanente em serviço pela problematização do cotidiano do trabalho em saúde. No percurso da descentralização, preconizada tanto pelo Sistema Único de Saúde como pela PNEPS, o território de eleição para a investigação foi o do Município de Teresópolis, apresentado segundo os parâmetros utilizados para o cálculo do Índice de Desenvolvimento Humano. A pesquisa se concentrou na Secretaria Municipal de Saúde e nas rodas de consenso do Pólo de Educação Permanente em Saúde da Região Serrana do Estado do Rio de Janeiro. A metodologia utilizou a triangulação de dados procedentes das técnicas da observação participante, da consulta a fontes documentais pertinentes e de dez entrevistas semi-estruturadas feitas com gestores da Secretaria Municipal de Saúde. O material narrativo, das entrevistas, foi transcrito e submetido à análise do discurso. O campo aportou dados inusitados para a análise da implementação da PNEPS. Tanto a prefeitura como a UNIFESO compartilham da mesma liderança política, com repercussões na gestão do Sistema Único de Saúde e na educação formal em saúde. Embora o Programa de Saúde da Família gere demandas para a PNEPS, o cruzamento e a sobrecarga das ações assistenciais com as educativas, nesta instância, mediadas pelo mesmo profissional da saúde, também preceptor da UNIFESO, trazem repercussões para ambas as iniciativas. Principalmente, obstaculizam propostas educativas para as demandas de trabalhadores e de usuários. Finalmente, no que concerne às políticas públicas, o estudo demonstrou a presença do modelo centro-periferia em escala municipal, à semelhança daquele de dimensões mundial e federal, expresso pela descentralização de ações com centralização de recursos.

A Estratégia Saúde da Família na Cidade do Rio de Janeiro não pode ser pensada como um instrumento isolado no contexto de saúde local. A sua interação com a rede de saúde da cidade precisa ser considerada para que ocorra um fluxo de pacientes entre os níveis de atenção. O Rio de Janeiro é dividido em dez Áreas Programáticas com coordenações de saúde próprias. O tamanho e a diversidade das regiões da cidade do Rio de Janeiro faz com que estas áreas tenham necessidades particulares e, consequentemente exijam respostas diferenciadas para as questões de saúde. A área em foco neste estudo é a 3.1 onde se localiza o Complexo do Alemão. O Complexo é marcado pela pobreza, violência e exclusão social. No ano 2000 tinha o Índice Desenvolvimento Humano (IDH) de 0,474, um dos piores entre os bairros do Rio de Janeiro. O cenário de vulnerabilidade foi determinante para que o Complexo fosse uma das primeiras áreas a ter intervenções do Programa de Agentes Comunitários de Saúde e do Programa Saúde da Família na cidade do Rio de Janeiro. Este estudo procura relacionar as características da Estratégia Saúde da Família na Cidade do Rio de Janeiro com as características do modelo implantado no Complexo do Alemão, a partir de dados dos sistemas de informação e de busca bibliográfica sobre a região. Há características que são comuns ao Rio de Janeiro e ao Complexo do Alemão, como por exemplo, a falta de uma programação de referência e contra-referência para atender a demanda dos pacientes da Estratégia Saúde da Família nos níveis secundário e terciário de cuidados. Existem também características que são próprias do Complexo do Alemão, como por exemplo, a quantidade de equipes compatível com a população segundo as diretrizes do Ministério da Saúde, o que contrasta com a cidade como um todo que tem uma quantidade de equipes ainda pequena em relação à população.

O presente trabalho tem como objetivo analisar micropoliticamente a utilização das chamadas tecnologias da informação e comunicação em um Centro de Educação Infantil e Primária (CEIP), estabelecimento da rede pública de ensino da cidade de Barcelona, evidenciando seus desdobramentos no modo de gestão das práticas escolares e seus efeitos no processo de saúde/adoecimentos dos educadores. Para isso será considerado tanto o referencial produzido através do conjunto das análises bibliográficas sobre a temática, como do acompanhamento das atividades realizadas no/pelo grupo de Desenvolvimento Humano, Intervenção Social e Interculturalidade (DEHISI), da Universidade Autônoma de Barcelona, no referido estabelecimento. O trabalho realizado na escola a partir o projeto Shere Rom na Escola visa, para além do fomento à utilização das novas tecnologias como mediadores do processo educacional, abordar questões evidenciadas no cotidiano escolar, como fragmentação, isolamento, dificuldades de trabalhar a diversidade e de produzir uma articulação entre a comunidade escolar: professores, alunos e responsáveis e a comunidade de entorn. Se, por um lado, a introdução de computadores e softwares como dispositivos inovadores da metodologia educativa foi rapidamente assimilada pelos alunos, dificuldades foram encontradas ao considerarmos o corpo docente e de pesquisadores no tocante à utilização dos equipamentos, pela falta de dispositivos que permitam efetivamente a discussão das práticas e questões do cotidiano da escola. Neste sentido, o projeto se constitui como um desafio rumo à coletivização das ações e uma gestão participativa da escola.
