Abstract Background: BNP has been extensively evaluated to determine short- and intermediate-term prognosis in patients with acute coronary syndrome, but its role in long-term mortality is not known. Objective: To determine the very long-term prognostic role of B-type natriuretic peptide (BNP) for all-cause mortality in patients with non-ST segment elevation acute coronary syndrome (NSTEACS). Methods: A cohort of 224 consecutive patients with NSTEACS, prospectively seen in the Emergency Department, had BNP measured on arrival to establish prognosis, and underwent a median 9.34-year follow-up for all-cause mortality. Results: Unstable angina was diagnosed in 52.2%, and non-ST segment elevation myocardial infarction, in 47.8%. Median admission BNP was 81.9 pg/mL (IQ range = 22.2; 225) and mortality rate was correlated with increasing BNP quartiles: 14.3; 16.1; 48.2; and 73.2% (p < 0.0001). ROC curve disclosed 100 pg/mL as the best BNP cut-off value for mortality prediction (area under the curve = 0.789, 95% CI= 0.723-0.854), being a strong predictor of late mortality: BNP < 100 = 17.3% vs. BNP ≥ 100 = 65.0%, RR = 3.76 (95% CI = 2.49-5.63, p < 0.001). On logistic regression analysis, age >72 years (OR = 3.79, 95% CI = 1.62-8.86, p = 0.002), BNP ≥ 100 pg/mL (OR = 6.24, 95% CI = 2.95-13.23, p < 0.001) and estimated glomerular filtration rate (OR = 0.98, 95% CI = 0.97-0.99, p = 0.049) were independent late-mortality predictors. Conclusions: BNP measured at hospital admission in patients with NSTEACS is a strong, independent predictor of very long-term all-cause mortality. This study allows raising the hypothesis that BNP should be measured in all patients with NSTEACS at the index event for long-term risk stratification.

Células de Saccharomyces carlsbergensis (IZ-1834) foram crescidas e adaptadas, progressivamente ao meio de caldo de cana esterilizado e contendo 0,001%, 0,002%, 0,003%, 0,004% e 0,00% de emulsan Al (v/v) além do tratamento testemunho isento de Emulsan Al. Estas células, levadas a Warburg fermentaram: glicose, rafinose. Não fermentaram: galactose, maltose e lactose. Não exibiram praticamente fermentação endógena. Glicose e caldo de cana tiveram seus rendimentos elevados, no caso das células utilizadas serem aquelas adaptadas aos diferentes teores de Emulsan Al. A fermentação da rafinose sofreu notável aceleração quando as células utilizadas foram adaptadas à Emulsan Al, sendo que a aceleração foi da ordem de 2 a 3 vêzes àquela observada no experimento com células crescidas em meio isento de Emulsan Al. A diferença da permeabilidade entre um e outro tipo de célula parece explicar o fenômeno.

The reproductive pattern of Elachistocleis bicolor (Guérin Méneville, 1838) was studied at Serra da Bodoquena, from October 2000 to September 2001. Reproduction occurred in the wet season (October to April) and was correlated to high continuous pluviometric precipitation. The species presents sexual size dimorphism, with females larger than males. The number of mature eggs per ovary was 620 ± 251 (n=39) and mature eggs measured 1.15 ± 0.30 mm (n=40). Elachistocleis bicolor presented significant relations between snout-vent length and number of mature eggs (n=39; r²=0.25; p=0.001), individual weight and number of mature eggs (n=41, r²=0.30; p=0.002), snout-vent length and ovarian weight (n=35; r²=0.47; p<0.01), and individual weight and ovarian weight (n=36; r²=0.55; p<0.01). Weight and volume are better to study size-fecundity relationships than snout-vent length. The females invested 22.7 ± 6.3 % (n=35) of their weights in reproduction and the variance associated to this variable was high, related to the reproductive mode of the species.

Devido à falta de estruturas rígidas para determinação de idade em crustáceos, métodos que se baseiam na determinação de idade através da distribuição da freqüência de comprimentos são comumente utilizados. Neste trabalho, foi comparado o crescimento do siri-azul, Callinectes sapidus Rathbun, 1896, em laboratório e a partir de indivíduos de campo. Os indivíduos de laboratório, após a coleta, tiveram seu crescimento acompanhado individualmente. A temperatura foi mantida constante (25±2°C) e a salinidade a 20±5. Os indivíduos do campo foram coletados entre 2002 e 2004 em pontos pré-determinados no estuário da Laguna dos Patos, com rede de portas. A biometria, nos dois casos, constou da medida da largura da carapaça (LC mm). As estimativas das curvas de crescimento, segundo o modelo de Bertalanffy para os indivíduos de campo, foram obtidas pelo acompanhamento do deslocamento modal (MPA). O tamanho máximo de largura de carapaça utilizado foi mantido fixo em todas as análises (157,78 mm para fêmeas e 162,71 mm para machos). Para os indivíduos mantidos em laboratório, os parâmetros e a longevidade obtidos foram k=0,001/dia; t o=-1,53; longevidade de 3117 dias (8,5 anos) para machos e k=0,002/dia; t o=-29,5; longevidade de 2795 dias para fêmeas (7,7 anos). Os parâmetros de crescimento e a longevidade estimados para os animais coletados em campo foram k=0,004/dia; t o=-4,23; 1267 dias (3,5 anos) para machos e k=0,004/dia; t o=-3,71; 1260 dias (3,45 anos) para fêmeas. Foram encontradas diferenças significativas entre animais de laboratório e de campo, sugerindo que a espécie responde de forma diferente ao ambiente em que está inserida.

Nous avons travaillé à Bello Horizonte, Etat de Minas, avec le venin de 4 espèces de Scorpions: Tityus bahiensis (C. L. KOCK, 1836). Tityus serrulatus (LUTZ-MELLO, 1922). Tityus dorsomaculatus (LUTZ – MELLO, 1922). Bothriurus (espèce em étude), sur un total de 13.640 individus. Nous avons essayé et observe l’action du venin sur 97 espèces differentes d’êtres vivants – depuis les chlamydozoaires jusqu’à l’«Homo sapiens». Nous avons cherché à déterminer une unité toxique «plus précise, plus régulierè». Les étalons dits «unité vésicule», «unité morsure» sont inconstants et sans rigueur. Tout au plus, peuvent ils server à l’étude de l’action générale du venin, et cela meme, dans certains cas seulement. Nous avons employé la pesée pour determiner l’unité toxique. Ce qui est important pour qui étudie ces sujets ce n’est pás lê nombre de vésicules, mais bien la quantité de venin humide ou desséché qu’elles contiennent. La balance, pour notre travail, est um moyen indicateur de bien plus grande précision que la «vésicule» ou la «morsure». Nous sommes parvenus à prouver qu’il existe une relation constante entre le poid brut des vésicules et la quantité de venin humide ou desséché qu’elles contiennent dans leur intérieur. Donc em pesant les vésicules, nous pesons indirectement le venin. Peu nous importe qu’il y ait 10 ou 100 vésicules. Il nous importe seulement de savoir combien elles pèsent, et de déterminer par ce fait, la quantité proportionnelle de vain pur. La technique générale est la suivante: Nous pesons um certain nombre de vésicules. Nous triturons ensuite, dans um mortier stérilisé et nous emulsionnons, par l’addition consécutive d’eau distillée, stérilisée. Nous filtrons l’émulsion sur le papier filtre employé em chimie, préalablement taré et desséché dans une atmosphere de chlorure de calcium. Après le filtrage on sèche à nouveau les papiers filtre employés d'abord à l'étuve et ensuite dans la même atmosphère de chlorure de calcium. Nous pesons plusieurs fois et on obtient la moyenne de ces pesées. On soustrait de cette dernière pesée le taux des substances non venimeuses, glandulaires, également dissoutes et calculées à 23 du poids brut et celles retenues par les papiers,-on obtient ainsi la moyenne réelle du venin pur contenu dans les vésicules utilisèés. Une simple divisiôn suffit pour fixer la moyenne de chacune. Ces données ont été vérifiées par les expériences faites avec du venin pur, largement obtenu dans notre Laboratoire. Nous avons trouvé de la sorte pour une vésicule de Tityus serrulatus: 0,gr.000,386 de T. bahiensis: 0,gr.001.261.24 de venin pur ce qui donne. 7/15,96 pour la 1ère. 1/8,36 pour la 2ème du poids sec de chaque vésicule. Le poids sec, pour une moyenne obtenue de 1.000 vésicules, fut de 0,gr.008,236 pour Tityus bahiensis. Maximum 0,gr.011. Minimum 0,gr.004.4 pour chacun. Pour Tityus serrulatus, en 1.049 vésicules le poids fut de 0,gr.006,08. Maximum 0,gr.014.03. Minimum 0,gr.003,1 pour chacun. C'est pour cette raison que l'unité-vésicule est incertaine. 2 poules A et B.; l'une, A, pesant 2 K.030 gr. reçoit dans une veinè, une émulsion en sèrum physiologique à 8,50/%, stérilisé, de 19 vésicules totales de Tityus serrulatus, et présence de légers phénomènes toxiques. L'autre, B, pesant 2 K.320 gr. meurt avec tous les phénomènes classiques de l'empoisonnement, par l'injection endoveineuse del'émulsion de 16 vésicules totales de venin de Tityus serrulatus! Les premières 19 vésicules pesaient 0,gr.58; les 16 derniéres-84 milligrammes. Les premières contenaient 0,gr.003. 634 et les secondes 0,gr.005.263 de venin pur! La moyenne obtenue de 6346 scorpions, (entre T. bahiensis et T. serrulatus) nous a fourni pour chacun: 0,gr.000,131,53 de venin pur, par piqûre. Si l'on spécifie: Pour 5.197 T. bahiensis. La moyenne pour une piqûre est 0,gr.000.106.15. Pour 1.149 T. serrulatus, la moyenne pour une piqûre est.......0,gr.000.246.30. La quantité a varié, selon les individus, de 0,gr.000.035.71, à 0,gr.000.436.01 de venin pur, pour une piqûre. D'après ce qui vient d'étre dit, on peut voir combien la quantité de venin éjaculé varie, chaque fois, chez les scorpions. L'unité-piqûre ne peut done pas ètre utiliseé pour des expériences dèlicates. Le mieux est de se servir de venin pur, et c'est ce que nous avons fait pour les expériences minutieuses. Quand on n'en possède pas, on peut établir pour chaque série des expériences à tenter-la dose minima mortelle en poids (grammes et fractions) de vésicules. D'après les bases ici consignées, et avec une trés petite erreur, on peut calculer la quantité de venin pur de cette dóse. Ce calcul est d'ailleurs dispensable. On peut s'en rapporter simplement au poids sec des vésicules totales et dire que la D. m. m. est de tant de milligr. secs. Comme le venin se conserve mal dans les vésicules, il faut, dans ce procédé, doser la D. m. m. toutes les fois que l'on veut procéder á une sériê d'expériences. Le venin desséché rappelle, d'après le temps de conservations au Laboratoire, celui de Crotatus terrificus et celui des Lachesis (quand il est vieux). Il est retenu au passage en partie, par les bougies Berkfeld et Chamberland. La conservation en état de dessication est la meilleure. Ainsi gardé, à l'abri de la lumierè, aux approches de 0,gr., pendant 8 mois, il perd à peine 1,2 à 1,4 de sa valeur primitive. L'echauffement à 100 gr. trouble une dissolution de venin dans l'eau distilleé; sans atteindre toutefois son pouvoir toxique, quand on l'injecte par la voie intra-cérébrale. Nous avons fait l'experience par 11 voies diverses. Sur des animaux sensibles, nous n'avons pas obtenu de phénomènes toxiques, apparemment, par les voies suivantes: 1) buccale; 2) gastrique; 3) rectale; 4) chambre oculaire antérieure; 5) cornéenne; 6) trachéenne; 7) meningée {sur; intra; 8) simple contact, bien que direct, avec le systemè nerveux central. OBJECTIVE. The purpose of this study was to evaluate the prevalence of mesenteric venous thrombosis (MVT) in the Swiss Inflammatory Bowel Disease Cohort Study and to correlate MVT with clinical outcome. MATERIALS AND METHODS. Abdominal portal phase CT was used to examine patients with inflammatory bowel disease (IBD). Two experienced abdominal radiologists retrospectively analyzed the images, focusing on the superior and inferior mesenteric vein branches and looking for signs of acute or chronic thrombosis. The location of abnormalities was registered. The presence of MVT was correlated with IBD-related radiologic signs and complications. RESULTS. The cases of 160 patients with IBD (89 women, 71 men; Crohn disease [CD], 121 patients; ulcerative colitis [UC], 39 patients; median age at diagnosis, 27 years for patients with CD, 32 years for patients with UC) were analyzed. MVT was detected in 43 patients with IBD (26.8%). One of these patients had acute MVT; 38, chronic MVT; and four, both. The prevalence of MVT did not differ between CD (35/121 [28.9%]) and UC (8/39 [20.5%]) (p = 0.303). The location of thrombosis was different between CD and UC (CD, jejunal or ileal veins only [p = 0.005]; UC, rectocolic veins only [p = 0.001]). Almost all (41/43) cases of thrombosis were peripheral. MVT in CD patients was more frequently associated with bowel wall thickening (p = 0.013), mesenteric fat hypertrophy (p = 0.005), ascites (p = 0.002), and mesenteric lymph node enlargement (p = 0.036) and was associated with higher rate of bowel stenosis (p < 0.001) and more intestinal IBD-related surgery (p = 0.016) in the outcome. Statistical analyses for patients with UC were not relevant because of the limited population (n = 8). CONCLUSION. MVT is frequently found in patients with IBD. Among patients with CD, MVT is associated with bowel stenosis and CD-related intestinal surgery.

AIMS/HYPOTHESIS: Several susceptibility genes for type 2 diabetes have been discovered recently. Individually, these genes increase the disease risk only minimally. The goals of the present study were to determine, at the population level, the risk of diabetes in individuals who carry risk alleles within several susceptibility genes for the disease and the added value of this genetic information over the clinical predictors. METHODS: We constructed an additive genetic score using the most replicated single-nucleotide polymorphisms (SNPs) within 15 type 2 diabetes-susceptibility genes, weighting each SNP with its reported effect. We tested this score in the extensively phenotyped population-based cross-sectional CoLaus Study in Lausanne, Switzerland (n = 5,360), involving 356 diabetic individuals. RESULTS: The clinical predictors of prevalent diabetes were age, BMI, family history of diabetes, WHR, and triacylglycerol/HDL-cholesterol ratio. After adjustment for these variables, the risk of diabetes was 2.7 (95% CI 1.8-4.0, p = 0.000006) for individuals with a genetic score within the top quintile, compared with the bottom quintile. Adding the genetic score to the clinical covariates improved the area under the receiver operating characteristic curve slightly (from 0.86 to 0.87), yet significantly (p = 0.002). BMI was similar in these two extreme quintiles. CONCLUSIONS/INTERPRETATION: In this population, a simple weighted 15 SNP-based genetic score provides additional information over clinical predictors of prevalent diabetes. At this stage, however, the clinical benefit of this genetic information is limited.

PURPOSE: To determine prognostic factors and evaluate outcomes of transcatheter arterial embolization in severely injured patients in hemodynamically unstable condition with multicompartmental bleeding.¦MATERIALS AND METHODS: Between June 2000 and May 2008, 36 consecutive patients treated with transcatheter arterial embolization for major retroperitoneal bleeding associated with at least one additional source of bleeding were retrospectively reviewed. Mean Injury Severity Score (ISS) was 49.4 ± 15.8. Univariate and multivariate analyses were performed to identify parameters associated with failure of embolization, need for additional surgery to control bleeding, and fatal outcome at 30 d.¦RESULTS: Embolization was technically successful in 35 of 36 patients (97.2%) and resulted in immediate and sustained (> 24 h) hemodynamic improvement in 29 (80.5%). Additional hemostatic surgery was necessary after embolization in six patients (16.6%). Fifteen patients (41.6%) died within 30 d. Failure to restore hemodynamic stability was correlated with the rate of administration of packed red blood cells (P = .014), rate of administration of fresh frozen plasma (FFP; P = .031), and systolic blood pressure (SBP) immediately before embolization (P = .002). The need for additional surgery was correlated with FFP administration rate before embolization (P = .0002) and hemodynamic success (P = .003). Death was correlated with Glasgow Coma Scale score at admission (P = .001), ISS (P = .014), New Injury Severity Score (P = .016), number of injured sites (P = .012), SBP before embolization (P = .042), need for vasopressive drugs before embolization (P = .037), and hemodynamic success (P = .0004).¦CONCLUSIONS: In patients in hemodynamically unstable condition, transcatheter arterial embolization effectively controls bleeding and improves hemodynamic stability. Immediate survival is related to hemodynamic condition before embolization, and 30-d mortality is mainly related to associated brain trauma.

PURPOSE: To assess the sensitivity and false positive rate (FPR) of neurological examination and somatosensory evoked potentials (SSEPs) to predict poor outcome in adult patients treated with therapeutic hypothermia after cardiopulmonary resuscitation (CPR). METHODS: MEDLINE and EMBASE were searched for cohort studies describing the association of clinical neurological examination or SSEPs after return of spontaneous circulation with neurological outcome. Poor outcome was defined as severe disability, vegetative state and death. Sensitivity and FPR were determined. RESULTS: A total of 1,153 patients from ten studies were included. The FPR of a bilaterally absent cortical N20 response of the SSEP could be calculated from nine studies including 492 patients. The SSEP had an FPR of 0.007 (confidence interval, CI, 0.001-0.047) to predict poor outcome. The Glasgow coma score (GCS) motor response was assessed in 811 patients from nine studies. A GCS motor score of 1-2 at 72 h had a high FPR of 0.21 (CI 0.08-0.43). Corneal reflex and pupillary reactivity at 72 h after the arrest were available in 429 and 566 patients, respectively. Bilaterally absent corneal reflexes had an FPR of 0.02 (CI 0.002-0.13). Bilaterally absent pupillary reflexes had an FPR of 0.004 (CI 0.001-0.03). CONCLUSIONS: At 72 h after the arrest the motor response to painful stimuli and the corneal reflexes are not a reliable tool for the early prediction of poor outcome in patients treated with hypothermia. The reliability of the pupillary response to light and the SSEP is comparable to that in patients not treated with hypothermia.

The prevalence of MVT did not differ between CD (35/121 [28.9%]) and UC (8/39 [20.5%]) (p = 0.303). The location of thrombosis was different between CD and UC (CD, jejunal or ileal veins only [p = 0.005]; UC, rectocolic veins only [p = 0.001]). Almost all (41/43) cases of thrombosis were peripheral. MVT in CD patients was more frequently associated with bowel wall thickening (p = 0.013), mesenteric fat hypertrophy (p = 0.005), ascites (p = 0.002), and mesenteric lymph node enlargement (p = 0.036) and was associated with higher rate of bowel stenosis (p < 0.001) and more intestinal IBD-related surgery (p = 0.016) in the outcome. Statistical analyses for patients with UC were not relevant because of the limited population (n = 8). CONCLUSION: MVT is frequently found in patients with IBD. Among patients with CD, MVT is associated with bowel stenosis and CD-related intestinal surgery.

BACKGROUND: Preoperative central neurologic deficits in the context of acute type A dissection are a complex comorbidity and difficult to handle. The aim this study was to analyze this subgroup of patients by comparing them with neurologically asymptomatic patients with type A dissection. Results may help the surgeon in preoperative risk assessment and thereby aid in the decision-making process. METHODS: We reviewed the data of patients admitted for acute type A dissection during the period from 1999 to 2010. Associated risk factors, time to surgery from admission, extension of the dissection, localization of central nervous ischemic lesions, and the influence of perioperative brain protective strategies were analyzed in a comparison of preoperative neurologically deficient to nondeficient patients. RESULTS: Forty-seven (24.5%) of a total of 192 patients had new-onset central neurologic symptoms prior to surgery. Concomitant myocardial infarction (OR 4.9, 95% CI 1.6-15.3, P = 0.006), renal failure (OR 5.9, 95% CI 1.1-32.8, P = 0.04), dissected carotid arteries (OR 9.2, 95% CI 2.4-34.7, P = 0.001), and late admission to surgery at >6 hours after symptom onset (OR 2.7, 95% CI 1.1-6.8, P = 0.04) were observed more frequently in neurologically deficient patients. These patients had a higher 30-day in-hospital mortality on univariate analysis (P = 0.01) and a higher rate of new postoperative neurologic deficits (OR 9.2, 95% CI 2.4-34.7, P = 0.02). Neurologic survivors had an equal hospital stay, and 67% of them had improved symptoms. CONCLUSIONS: The predominance of neurologic symptoms at admission may be responsible for an initial misdiagnosis. The concurrent central nervous system ischemia and myocardial infarction explains a higher mortality rate and a more extensive "character" of the disease. Neurologically deficient patients are at higher risk of developing new postoperative neurologic symptoms, but prognosis for the neurologic evolution of survivors is generally favorable.

OBJECTIVE: Positive occipital sharp transients of sleep (POSTS) are considered a normal variant seen in non-REM sleep; their asymmetrical presentation and relationship with EEG abnormalities have received scarce attention to date. We analyzed these features in a large prospective EEG recordings' sample. METHODS: In this case-control study, over 6 months we collected consecutive patients showing POSTS on their EEG. They were matched with consecutive control subjects (two for each). Demographical data, asymmetries for POSTS and alpha activity, and lateralized or diffuse occurrence of EEG abnormalities (slowing, epileptiform transients) were compared among these two groups. RESULTS: Out of 1254 EEG studies, 102 (8%) patients showed POSTS. They were younger (p=0.031), and more likely to show EEG abnormalities (p=0.008) - including epileptiform transients (p=0.002) - than controls. However, this relationship was influenced by age and recording length. Thirty nine POSTS recordings (38%) had a consistent amplitude asymmetry, but this was not associated with specific EEG abnormalities or alpha asymmetry. CONCLUSION: POSTS are a normal EEG variant, occurring in less than 10% of unselected EEG recordings, mostly in younger adults, without gender predominance. Amplitude asymmetries are found in over one third of subjects. SIGNIFICANCE: POSTS asymmetry, as opposed to other sleep transients, should be considered as normal.

BACKGROUND/OBJECTIVES: To measure resting metabolic rate (RMR), activity energy expenditure (AEE), total energy expenditure (TEE) and physical activity pattern, that is, duration and intensity (in metabolic equivalents, METs) of activities performed in late pregnancy compared with postpartum in healthy, well-nourished women living in Switzerland. SUBJECTS/METHODS: Weight, height, RMR, AEE, TEE and physical activity patterns were measured longitudinally in 27 healthy women aged 23-40 years at 38.2+/-1.5 weeks of gestation and 40.0+/-7.2 weeks postpartum. RESULTS: The RMR during late pregnancy was 7480 kJ per day, that is, 1320+/-760 kJ per day (21.4%) higher than the postpartum RMR (P<0.001). Absolute changes in RMR were positively correlated with the corresponding changes in body weight (r=0.61, P<0.001). RMR per kg body weight was similar in late pregnancy vs postpartum (P=0.28). AEE per kg during pregnancy and postpartum was 40+/-13 and 50+/-20 kJ/kg, respectively (P=0.001). There were significant differences in daily time spent at METs<1.5 (1067 vs 998 min, P=0.045), at 2.5< or =METs <3.0 (58 vs 82 min, P=0.002) and METs> or =6 (1 vs 6 min, P=0.014) during pregnancy and postpartum, respectively. CONCLUSIONS: Energy expenditure in healthy women living in Switzerland increases in pregnancy compared with the postpartum state. Additional energy expenditure is primarily attributed to an increase in RMR, which is partly compensated by a decrease in AEE. The decrease in physical activity-related energy costs is achieved by selecting less demanding activities and should be taken into account when defining extra energy requirements for late pregnancy in Switzerland.
