OBJETIVOS: descrever a impotência no pós-operatório de cirurgia cardíaca quanto a freqüência, intensidade, características definidoras e comparar a impotência em pacientes em pós-operatório de cirurgias de válvula (VAL) e de coronária (RM). MÉTODO: a presença de impotência foi definida pela análise de entrevistas de 75 pacientes em pós-operatório de cirurgia cardíaca (60% em pós-operatório de RM e 40% de VAL). RESULTADOS: 44 (58,7%) pacientes apresentaram impotência. Não houve diferenças quanto a freqüência (p=0,84) e intensidade (p=0,73) de impotência entre os pós-operados de VAL e os de RM. As características definidoras mais freqüentes nos pacientes com impotência foram: "expressões verbais relativas à falta de controle ou de influências sobre uma situação" (77,3%), "dúvida para planejar o futuro e estabelecer objetivos" (75,0%), "expressão de dúvida acerca do desempenho de papéis" (63,6%) e "expressões de insatisfação e frustração pela inabilidade no desempenho de tarefas e/ou atividades pessoais" (56,8%).

Pesquisa de campo, prospectiva, quantitativa, descritiva-exploratória, realizada na UTI geral/adulto de um hospital privado do município de São Paulo. Objetivos: avaliar o NAS - Nursing Activities Score - como medida de carga de trabalho de enfermagem; sua aplicabilidade por turnos e sua correspondência com o quantitativo de enfermagem efetivo. Classificados 33 pacientes: idade média: 70,4 anos (+/-16,5), 66,7% do sexo masculino; permanência média na UTI: 17 dias (+/- 20,4); SAPSII: 41,7 (+/-17,9); risco de morte: (RM) 33,5% (+/- 26,8); 63,6% transferidos para Unidades de Cuidados Semi-Intensivos, 18,2% evoluíram a óbito. Obtiveram-se 396 medidas por turnos (134-manhã; 132-tarde; 130-noturno), média de 55,4 (+/-12,3) e 147 medidas de NAS de 24h, média de 69,6 (+/-18,2). O instrumento mostrou-se mais adequado à aplicação em 24 horas que por turnos, tendendo a refletir o número de profissionais efetivo, revelando-se interessante instrumento de classificação de pacientes e carga de trabalho de enfermagem em terapia intensiva.

This paper derives a model of markets with system goods and two technological standards. An established standard incurs lower unit production costs but causes a negative externality. The paper derives the conditions for policy intervention and compares the effect of direct and indirect cost-reducing subsidies in two markets with system goods in the presence of externalities. If consumers are committed to the technology by purchasing one of the components, direct subsidies are preferable. For a medium-low cost difference between technological standards and a low externality cost it is optimal to provide a direct subsidy only to the first technology adopter. As the higher the externality cost raises, the more technology adopters should be provided with direct subsidies. This effect is robust in all extensions. In the absence of consumers commitment to a technological standard indirect and direct subsidies are both desirable. In this case, the subsidy to the first adopter is lower then the subsidy to the second adopter. Moreover, for the low cost difference between technological standards and low externality cost the fi rst fi rm chooses a superior standard without policy intervention. Finally, a perfect compatibility between components based on different technological standards enhances an advantage of indirect subsidies for medium-high externality cost and cost difference between technological standards. The West Liberty Foods turkey cooperative was formed in 1996 to purchase the assets and assume operations of Louis Rich Foods (an investor-owned processing rm), which, at the time, announced the imminent shutdown of its West Liberty, Iowa, processing facility. We study the creation and performance of this �new generation� cooperative using eld interviews with grower members and company management. We describe changes, before and after the buyout, in the contractual apparatus used for procuring live turkeys, and in the communication requirements, work expectations, and nancial positions of growers. During the private ownership period, most of the inputs (except labor and facilities) were provided by the rm; there was substantial supervision of the growers' actions; growers faced little price and production risk; and growers' equity was due largely to ownership of land and other farm assets. Our interviews reveal that, after cooperative formation, growers were exposed to considerable additional risk; monitoring of growers by the rm was less intensive; grower time and effort commitments to turkey production increased substantially; and a signicant fraction of rm (cooperative) equity came from growers' willingness to leverage their farm and personal assets (and hence indirectly their existing relationships with local lenders). We argue that some of these changes are consistent with a nancial contract where asset pledging and its corollary risk generate higher work effort by growers and a reduction in agency rents. These economies likely compensate for an organizational deadweight loss traditionally associated with cooperative governance.

Tämän insinöörityön tarkoituksena oli luoda Insinööritoimisto Olof Granlund Oy:lle malliprojekti ilmanvaihtokoneen ohjauksen mahdollisesta sähköenergian säästöpotentiaalista. Insinöörityötä voidaan myöhemmin käyttää referenssinä tarjottaessa yrityksen palvelua asiakkaille energiakatselmointityössä kiinteistöihin, joissa on suuria ilmanvaihtokone-yksiköitä. Ensin työssä esitellään käytössä olevat, tutkittavat ilmanvaihtokoneet ja niiden toiminta. Sitten selvitetään ilmanvaihtolaitteiden ja niiden ohjauksen eri osien toiminta, sekä ohjausinformaatio, joka välittyy eri laitteiden välillä. Seuraavaksi työssä tutkitaan rakennusautomaation osuutta, sen hyötyä ja rakennetta kiinteistössä. Tämän jälkeen selvitetään hankkeen viranomaismääräykset. Sitten selvitetään, mistä koostuvat kustannukset ilmanvaihtokoneen ohjauksen uusimisessa. Lopuksi vielä tutkitaan, kuinka pitkä on tehtävän investoinnin takaisinmaksuaika, sekä mitkä ovat saneerauksen hiilidioksidi-päästövaikutukset. insinöörityön Lopputuloksena syntyi yrityksen sisäinen kansio. Kansion esimerkkilaskelmia ja laskentamenetelmiä on tarkoitus käyttää jatkossa referenssinä kiinteistönpito-osastolla sähkö- ja lvi-ryhmissä, niiden henkilöiden käytössä, jotka ovat tekemisissä energiakatselmointien kanssa.

Opinnäytetyömme on osa Keski-Uudenmaan pelastuslaitoksen KUOSCE -hanketta, jonka tarkoitukse-na on selvittää perustason sairaankuljettajien taitoja hoitamisen ja tutkimisen eri osa-alueilla, sekä luoda perustason osaamisen kehittymisen malli ensihoidon koulutukseen ja Keski-Uudenmaan pelastuslaitok-sen käyttöön. Opinnäytetyön tarkoituksena on kuvata tutkittuun tietoon perustuen, minkälaista elvytysosaamista perustason sairaankuljettajat tarvitsevat. Toisena tarkoituksena on arvioida elvytysosaamista arvioivan mittarin luotettavuutta ja käyttökelpoisuutta. Olemme kuvanneet kirjallisuuskatsauksessamme tutkimuksiin perustuen elvytyksessä tarvittavaa osaamista sekä osaamisessa todettuja puutteita niin sairaalassa kuin sen ulkopuolisessa ensihoidossa. Tutkimuksissa todettiin, että sairaankuljettajien elvytystaidoissa on paljon parannettavaa. Lähes jokaisella osa-alueella on osaamispuutteita. Elottomuuden tunnistamiseen kuluu liian pitkä aika, painelupaikka määritellään väärin, oikean painelusyvyyden hallitsemisessa on vaikeuksia, ventilaation oikean suhteen toteuttaminen on käytännössä vaikeaa. Tutkimusten mukaan sairaankuljettajien tiedolliset taidot elvytystilanteissa ovat hyvät, mutta käytännön taidoissa on parannettavaa. Mittarin luotettavuuden ja käyttökelpoisuuden arvioimiseksi järjestimme Helsingin ammattikorkeakoulu Stadian valmistuville ensihoitajaopiskelijoille (n=16) simuloidun elvytystilanteen Antti- elvytysnukella. Totesimme mittarin olevan pääosin luotettava, mutta havaitsimme mittarissa olevan myös sen luotettavuutta heikentäviä tekijöitä, kuten arvioijien eriävä mielipide testaustilanteessa johtuen mittarin vaikeasta seurattavuudesta.

Prepreg-materiaalit ovat olleet käytössä veneiden ja lentokoneiden valmistuksessa vuosia. Vasta muutamia vuosia sitten prepreg-materiaalia alettiin soveltaa apuvälinealaan. Koska prepreg-ortoosien valmistaminen on vain yksi alaluokka ortoosien valmistamisessa, opetusmateriaalia opiskelijoille ei ole paljon tarjolla. Tulevaisuudessa prepreg-materiaalien käyttö ortoosien valmistuksessa tulee pysymään samana tai sen käyttö tulee jopa kasvamaan. Siksi tietoutta prepreg-aineiden käytöstä ja koko valmistusprosessin kulusta oli tärkeää saada apuvälinetekniikan opiskelijoille. Päämääräni oli tehdä keräämistäni tiedoista prepreg-materiaalien käyttöopas. Ennen oppaan valmistamista, keräsin materiaalia oppaaseen erilaisista lähteistä. Haastattelin prepreg- ortoosien asiantuntijaa, luin prepreg-materiaalien valmistajien komposiittitietoja ja muita prepreg- tuotteista olevia teknisiä tietoja. Materiaalin halusin koota helppoon ja kompaktiin tilaan, joten ideana oli tuottaa CD-ROM, jolta tieto olisi helposti poimittavissa. Jotta CD-ROM täyttäisi vaatimustasoni, opiskelin myös oppaan suunnitteluun liittyvää kirjallisuutta. Prepreg-materiaalien käyttöoppaasta tuli loppujen lopuksi 33 sivua pitkä. Opas koostuu 7 pääotsikosta aina materiaaleista prepreg-ortoosin valmistusprosessiin. Pääotsikoiden alla on monia alaotsikoita, jotka kertovat ominaisuuksia ja teknisiä tietoja prepreg-materiaaleista. Opas valmistettiin PowerPoint- ohjelmalla, mikä teki siitä helppokäyttöisen. Jokaiseen osioon pääsee hyperlinkkien kautta painamalla haluamaansa otsikkoa sisällysluettelossa. Moniin kappaleisiin on lisätty kuvia, jotka tuovat lisää informaatiota tekstiin. Oppaan ulkoasu ja värimaailma on myös suunniteltu aiheen mukaan. Oppaasta tulevat hyötymään monet opiskelijat ja jopa työssä olevat, jotka haluavat tietoja prepreg- materiaaleista. Tiedon kokoaminen yhteen helpottaa tiedon hakemista ja säästää paljon aikaa. CD-ROM-opas säilyy myös paremmin kuin paperinen.

Barrels are discrete cytoarchitectonic neurons cluster located in the layer IV of the somatosensory¦cortex in mice brain. Each barrel is related to a specific whisker located on the mouse snout. The¦whisker-to-barrel pathway is a part of the somatosensory system that is intensively used to explore¦sensory activation induced plasticity in the cerebral cortex.¦Different recording methods exist to explore the cortical response induced by whisker deflection in¦the cortex of anesthetized mice. In this work, we used a method called the Single-Unit Analysis by¦which we recorded the extracellular electric signals of a single barrel neuron using a microelectrode.¦After recording the signal was processed by discriminators to isolate specific neuronal shape (action¦potentials).¦The objective of this thesis was to familiarize with the barrel cortex recording during whisker¦deflection and its theoretical background and to compare two different ways of discriminating and¦sorting cortical signal, the Waveform Window Discriminator (WWD) or the Spike Shape Discriminator (SSD).¦WWD is an electric module allowing the selection of specific electric signal shape. A trigger and a¦window potential level are set manually. During measurements, every time the electric signal passes¦through the two levels a dot is generated on time line. It was the method used in previous¦extracellular recording study in the Département de Biologie Cellulaire et de Morphologie (DBCM) in¦Lausanne.¦SSD is a function provided by the signal analysis software Spike2 (Cambridge Electronic Design). The¦neuronal signal is discriminated by a complex algorithm allowing the creation of specific templates.¦Each of these templates is supposed to correspond to a cell response profile. The templates are saved¦as a number of points (62 in this study) and are set for each new cortical location. During¦measurements, every time the cortical recorded signal corresponds to a defined number of templates¦points (60% in this study) a dot is generated on time line. The advantage of the SSD is that multiple¦templates can be used during a single stimulation, allowing a simultaneous recording of multiple¦signals.¦It exists different ways to represent data after discrimination and sorting. The most commonly used¦in the Single-Unit Analysis of the barrel cortex are the representation of the time between stimulation¦and the first cell response (the latency), the representation of the Response Magnitude (RM) after¦whisker deflection corrected for spontaneous activity and the representation of the time distribution¦of neuronal spikes on time axis after whisker stimulation (Peri-Stimulus Time Histogram, PSTH).¦The results show that the RMs and the latencies in layer IV were significantly different between the¦WWD and the SSD discriminated signal. The temporal distribution of the latencies shows that the¦different values were included between 6 and 60ms with no peak value for SSD while the WWD¦data were all gathered around a peak of 11ms (corresponding to previous studies). The scattered¦distribution of the latencies recorded with the SSD did not correspond to a cell response.¦The SSD appears to be a powerful tool for signal sorting but we do not succeed to use it for the¦Single-Unit Analysis extracellular recordings. Further recordings with different SSD templates settings¦and larger sample size may help to show the utility of this tool in Single-Unit Analysis studies.

Partindo das realidades empíricas de Cabo Verde e diásporas, este texto explora, até certo ponto, as limitações heurísticas de certas noções sugeridas pela mainstream do modelo neoliberal da economia sobre o «informal» em África. O texto debruça-se sobre diferentes dimensões, espaços e protagonistas de práticas de economia informal em rede. As repercussões identitárias das dinâmicas destas redes entre os cabo-verdianos, apreendidas através da observação de certas formas de sociabilidade dos actores sociais considerados, são igualmente exploradas.

En pacients amb càncer de mama la ressonància magnètica és superior a altres exploracions per a monitoritzar la resposta a la quimioteràpia neoadjuvant. Analitzem la correlació entre resposta completa radiològica(RCr) valorada amb RM i resposta completa patològica (RCp), globalment i en funció dels diferents immunofenotips. Es tracta d´un estudi clínic, retrospectiu, on s´han inclòs 103 pacients tractades amb quimioteràpia neoadjuvant. Totes les pacients incloses disposaven de RM inicial i de RM post-quimioteràpia. La sensibilitat de la RM per a detectar RCp és d´un 82% i l´especificitat d´un 71%. El valor de sensibilitat més alt, del 100%, s´ha objectivat en els tumors triple negatius. La concordança global entre RCr i RCp és d´un 74%.

I describe the customer valuations game, a simple intuitive game that can serve as a foundation for teaching revenue management. The game requires little or no preparation, props or software, takes around two hours (and hence can be finished in one session), and illustrates the formation of classical (airline and hotel) revenue management mechanisms such as advanced purchase discounts, booking limits and fixed multiple prices. I normally use the game as a base to introduce RM and to develop RM forecasting and optimization concepts off it. The game is particularly suited for non-technical audiences.

The network revenue management (RM) problem arises in airline, hotel, media,and other industries where the sale products use multiple resources. It can be formulatedas a stochastic dynamic program but the dynamic program is computationallyintractable because of an exponentially large state space, and a number of heuristicshave been proposed to approximate it. Notable amongst these -both for their revenueperformance, as well as their theoretically sound basis- are approximate dynamic programmingmethods that approximate the value function by basis functions (both affinefunctions as well as piecewise-linear functions have been proposed for network RM)and decomposition methods that relax the constraints of the dynamic program to solvesimpler dynamic programs (such as the Lagrangian relaxation methods). In this paperwe show that these two seemingly distinct approaches coincide for the network RMdynamic program, i.e., the piecewise-linear approximation method and the Lagrangianrelaxation method are one and the same.

Many revenue management (RM) industries are characterized by (a) fixed capacities in theshort term (e.g., hotel rooms, seats on an airline flight), (b) homogeneous products (e.g., twoairline flights between the same cities at similar times), and (c) customer purchasing decisionslargely influenced by price. Competition in these industries is also very high even with just twoor three direct competitors in a market. However, RM competition is not well understood andpractically all known implementations of RM software and most published models of RM donot explicitly model competition. For this reason, there has been considerable recent interestand research activity to understand RM competition. In this paper we study price competitionfor an oligopoly in a dynamic setting, where each of the sellers has a fixed number of unitsavailable for sale over a fixed number of periods. Demand is stochastic, and depending on howit evolves, sellers may change their prices at any time. This reflects the fact that firms constantly,and almost costlessly, change their prices (alternately, allocations at a price in quantity-basedRM), reacting either to updates in their estimates of market demand, competitor prices, orinventory levels. We first prove existence of a unique subgame-perfect equilibrium for a duopoly.In equilibrium, in each state sellers engage in Bertrand competition, so that the seller withthe lowest reservation value ends up selling a unit at a price that is equal to the equilibriumreservation value of the competitor. This structure hence extends the marginal-value conceptof bid-price control, used in many RM implementations, to a competitive model. In addition,we show that the seller with the lowest capacity sells all its units first. Furthermore, we extendthe results transparently to n firms and perform a number of numerical comparative staticsexploiting the uniqueness of the subgame-perfect equilibrium.

Revenue management (RM) is a complicated business process that can best be described ascontrol of sales (using prices, restrictions, or capacity), usually using software as a tool to aiddecisions. RM software can play a mere informative role, supplying analysts with formatted andsummarized data who use it to make control decisions (setting a price or allocating capacity fora price point), or, play a deeper role, automating the decisions process completely, at the otherextreme. The RM models and algorithms in the academic literature by and large concentrateon the latter, completely automated, level of functionality.A firm considering using a new RM model or RM system needs to evaluate its performance.Academic papers justify the performance of their models using simulations, where customerbooking requests are simulated according to some process and model, and the revenue perfor-mance of the algorithm compared to an alternate set of algorithms. Such simulations, whilean accepted part of the academic literature, and indeed providing research insight, often lackcredibility with management. Even methodologically, they are usually awed, as the simula-tions only test \within-model" performance, and say nothing as to the appropriateness of themodel in the first place. Even simulations that test against alternate models or competition arelimited by their inherent necessity on fixing some model as the universe for their testing. Theseproblems are exacerbated with RM models that attempt to model customer purchase behav-ior or competition, as the right models for competitive actions or customer purchases remainsomewhat of a mystery, or at least with no consensus on their validity.How then to validate a model? Putting it another way, we want to show that a particularmodel or algorithm is the cause of a certain improvement to the RM process compared to theexisting process. We take care to emphasize that we want to prove the said model as the causeof performance, and to compare against a (incumbent) process rather than against an alternatemodel.In this paper we describe a \live" testing experiment that we conducted at Iberia Airlineson a set of flights. A set of competing algorithms control a set of flights during adjacentweeks, and their behavior and results are observed over a relatively long period of time (9months). In parallel, a group of control flights were managed using the traditional mix of manualand algorithmic control (incumbent system). Such \sandbox" testing, while common at manylarge internet search and e-commerce companies is relatively rare in the revenue managementarea. Sandbox testing has an undisputable model of customer behavior but the experimentaldesign and analysis of results is less clear. In this paper we describe the philosophy behind theexperiment, the organizational challenges, the design and setup of the experiment, and outlinethe analysis of the results. This paper is a complement to a (more technical) related paper thatdescribes the econometrics and statistical analysis of the results.

We present a leverage theory of reputation building with co-branding. We showthat under certain conditions, co-branding that links unknown firms in a new sectorwith established firms in a mature sector allows the unknown firms to signal a highproduct quality and establish their own reputation. We compare this situationwith a benchmark in which both sectors are new and firms signal their qualityonly with prices. We investigate how this comparison is affected by the nature ofthe technology linking the two sectors and a cross-sector inference problem thatconsumers might face in identifying the true cause of product failure. We find thatco-branding facilitates the process in which a Þrm in the new sector to signal itsproduct quality only if the co-branding sectors produce complementary inputs andconsumers face a cross-sector inference problem. We apply our insight to economicsof superstars, multinational firms and co-authorship.
