ABSTRACT: BACKGROUND: We have read the letter by Bhoyrul et al. in response to our recently published article "Safety and effectiveness of bariatric surgery: Roux-en-Y gastric bypass is superior to gastric banding in the management of morbidly obese patients". We strongly disagree with the content of the letter. RESULTS AND DISCUSSION: Bhoyrul et al. base their letter mostly on low level evidence such as single-institutional case series (level IV evidence) and expert opinion (level V evidence). Surprisingly, they do not comment on the randomized controlled trial, which clearly favours gastric bypass over gastric banding. CONCLUSION: The letter by Bhoyrul et al. is based on low level evidence and is itself biased, unsubstantiated, and not supported by the current literature.

Type 1 diabetes mellitus is a chronic disease characterized by blood glucose levels out of normal range due to inability of insulin production. This dysfunction leads to many short- and long-term complications. In this paper, a system for tele-monitoring and tele-management of Type 1 diabetes patients is proposed, aiming at reducing the risk of diabetes complications and improving quality of life. The system integrates Wireless Personal Area Networks (WPAN), mobile infrastructure, and Internet technology along with commercially available and novel glucose measurement devices, advanced modeling techniques, and tools for the intelligent processing of the available diabetes patients information. The integration of the above technologies enables intensive monitoring of blood glucose levels, treatment optimisation, continuous medical care, and improvement of quality of life for Type 1 diabetes patients, without restrictions in everyday life activities.

Das Web 2.0 eröffnet Wissenschaftlerinnen und Wissenschaftlern neue Möglichkeiten mit Wissen und Informationen umzugehen: Das Recherchieren von Informationen und Quellen, der Austausch von Wissen mit anderen, das Verwalten von Ressourcen und das Erstellen von eigenen Inhalten im Web ist einfach und kostengünstig möglich. Dieser Artikel thematisiert die Bedeutung des Web 2.0 für den Umgang mit Wissen und Informationen und zeigt auf, wie durch die Kooperation vieler Einzelner das Schaffen von neuem Wissen und von Innovationen möglich wird. Diskutiert werden der Einfluss des Web 2.0 auf die Wissenschaft und mögliche Vor- und Nachteile der Nutzung. Außerdem wird ein kurzer Überblick über Studien gegeben, die die Nutzung des Web 2.0 in der Gesamtbevölkerung untersuchen. Im empirischen Teil des Artikels werden Methode und Ergebnisse der Befragungsstudie „Wissenschaftliches Arbeiten im Web 2.0“ vorgestellt. Befragt wurden Nachwuchswissenschaftlerinnen und Nachwuchswissenschaftler in Deutschland zur Nutzung des Web 2.0 für die eigene wissenschaftliche Arbeit. Dabei zeigt sich, dass insbesondere die Wikipedia von einem Großteil der Befragten intensiv bis sehr intensiv für den Einstieg in die Recherche verwendet wird. Die aktive Nutzung des Web 2.0, z.B. durch das Schreiben eines eigenen Blogs oder dem Mitarbeiten bei der Online-Enzyklopädie Wikipedia ist bis jetzt noch gering. Viele Dienste sind unbekannt oder werden eher skeptisch beurteilt, der lokale Desktopcomputer wurde noch nicht vom Web als zentraler Speicherort abgelöst.

As education providers increasingly integrate digital learning media into their education processes, the need for the systematic management of learning materials and learning arrangements becomes clearer. Digital repositories, often called Learning Object Repositories (LOR), promise to provide an answer to this challenge. This article is composed of two parts. In this part, we derive technological and pedagogical requirements for LORs from a concretization of information quality criteria for e-learning technology. We review the evolution of learning object repositories and discuss their core features in the context of pedagogical requirements, information quality demands, and e-learning technology standards. We conclude with an outlook in Part 2, which presents concrete technical solutions, in particular networked repository architectures.

Die Dissertationsschrift widmet sich der Erforschung des Online-Lernens mittels Weblogs unter Anwendung der E-Portfolio Methode als einer seit mehreren Jahren verstärkt aufkommenden Lern- und Präsentationsform im Bildungskontext. Über mehrere Lehrveranstaltungen des Studiengangs "Angewandte Medien- und Kommunikationswissenschaft" an der Technischen Universität Ilmenau hinweg wurden drei Fallstudien gebildet. Innerhalb dieser wurde das Führen von eigenen E-Portfolio Blogs durch Studierende über einen Zeitraum von etwa drei Jahren evaluiert. Als Evaluationsziel wurde anhand spezifischer Fragestellungen ermittelt, wie das damit einhergehende selbstgesteuert-konnektive Lernen zu entsprechendem Erfolg führen kann. Hierbei wurde insbesondere die Teildimension Medienkompetenz im Spannungsfeld von Lernaktivität, Wissenserwerb und Informations-/Wissensmanagement betrachtet sowie weitere intervenierende Variablen, wie zum Beispiel Aufwand oder Akzeptanz, berücksichtigt. Inhaltlich wurden zunächst begriffliche Grundlagen dargestellt, die Nutzung von E-Portfolios in Theorie und Praxis beschrieben, Medienkompetenz-Ansätze detailliert aufgezeigt sowie in den Kontext von E-Portfolios gebracht und schließlich eine umfangreiche Analyse des Forschungsstandes aufbereitet. Diese gingen mit Erkenntnissen aus einer qualitativen Vorstudie in Form von fünf leitfadengestützten Experteninterviews einher. Die darauf aufbauende Hauptstudie widmete sich anschließend der Erhebung und Auswertung quantitativer Daten anhand von Online-Befragungen mit den Studierenden zu fünf Zeitpunkten aus intra- und interindividueller Perspektive. Als markanteste empirische Erkenntnis der Arbeit kann festgehalten werden, dass es durch das selbstgesteuert-konnektive Lernen mit E-Portfolio Blogs zu einer nachhaltigen Förderung der Medienkompetenz kommt, die sich auch in signifikanten Zusammenhängen mit den anderen Teildimensionen und intervenierenden Variablen widerspiegelt. Darüber hinaus bieten sich aber auch Potenziale für eine steigende Lernaktivität, einen ansteigenden Wissenserwerb und ein verbessertes Informations-/Wissensmanagement, die es aber noch weiterführend zu erforschen gilt. Demgegenüber können allerdings der entstehende und kontinuierlich hohe Aufwand sowie die erforderliche (Eigen-) Motivation als entscheidende Herausforderungen dieser Lernmethode identifiziert werden.

Fine roots are the most dynamic portion of a plant's root system and a major source of soil organic matter. By altering plant species diversity and composition, soil conditions and nutrient availability, and consequently belowground allocation and dynamics of root carbon (C) inputs, land-use and management changes may influence organic C storage in terrestrial ecosystems. In three German regions, we measured fine root radiocarbon (14C) content to estimate the mean time since C in root tissues was fixed from the atmosphere in 54 grassland and forest plots with different management and soil conditions. Although root biomass was on average greater in grasslands 5.1 ± 0.8 g (mean ± SE, n = 27) than in forests 3.1 ± 0.5 g (n = 27) (p < 0.05), the mean age of C in fine roots in forests averaged 11.3 ± 1.8 yr and was older and more variable compared to grasslands 1.7 ± 0.4 yr (p < 0.001). We further found that management affects the mean age of fine root C in temperate grasslands mediated by changes in plant species diversity and composition. Fine root mean C age is positively correlated with plant diversity (r = 0.65) and with the number of perennial species (r = 0.77). Fine root mean C age in grasslands was also affected by study region with averages of 0.7 ± 0.1 yr (n = 9) on mostly organic soils in northern Germany and of 1.8 ± 0.3 yr (n = 9) and 2.6 ± 0.3 (n = 9) in central and southern Germany (p < 0.05). This was probably due to differences in soil nutrient contents and soil moisture conditions between study regions, which affected plant species diversity and the presence of perennial species. Our results indicate more long-lived roots or internal redistribution of C in perennial species and suggest linkages between fine root C age and management in grasslands. These findings improve our ability to predict and model belowground C fluxes across broader spatial scales.

BACKGROUND Critical incidents in clinical medicine can have far-reaching consequences on patient health. In cases of severe medical errors they can seriously harm the patient or even lead to death. The involvement in such an event can result in a stress reaction, a so-called acute posttraumatic stress disorder in the healthcare provider, the so-called second victim of an adverse event. Psychological distress may not only have a long lasting impact on quality of life of the physician or caregiver involved but it may also affect the ability to provide safe patient care in the aftermath of adverse events. METHODS A literature review was performed to obtain information on care giver responses to medical errors and to determine possible supportive strategies to mitigate negative consequences of an adverse event on the second victim. An internet search and a search in Medline/Pubmed for scientific studies were conducted using the key words "second victim, "medical error", "critical incident stress management" (CISM) and "critical incident stress reporting system" (CIRS). Sources from academic medical societies and public institutions which offer crisis management programs where analyzed. The data were sorted by main categories and relevance for hospitals. Analysis was carried out using descriptive measures. RESULTS In disaster medicine and aviation navigation services the implementation of a CISM program is an efficient intervention to help staff to recover after a traumatic event and to return to normal functioning and behavior. Several other concepts for a clinical crisis management plan were identified. CONCLUSIONS The integration of CISM and CISM-related programs in a clinical setting may provide efficient support in an acute crisis and may help the caregiver to deal effectively with future error events and employee safety.

The execution of a project requires resources that are generally scarce. Classical approaches to resource allocation assume that the usage of these resources by an individual project activity is constant during the execution of that activity; in practice, however, the project manager may vary resource usage over time within prescribed bounds. This variation gives rise to the project scheduling problem which consists in allocating the scarce resources to the project activities over time such that the project duration is minimized, the total number of resource units allocated equals the prescribed work content of each activity, and various work-content-related constraints are met. We formulate this problem for the first time as a mixed-integer linear program. Our computational results for a standard test set from the literature indicate that this model outperforms the state-of-the-art solution methods for this problem.

There is much interest in the identification of the main drivers controlling changes in the microbial community that may be related to sustainable land use. We examined the influence of soil properties and land-use intensity (N fertilization, mowing, grazing) on total phospholipid fatty acid (PLFA) biomass, microbial community composition (PLFA profiles) and activities of enzymes involved in the C, N, and P cycle. These relationships were examined in the topsoil of grasslands from three German regions (Schorfheide-Chorin (SCH), Hainich-Dun (HAI), Schwabische Alb (ALB)) with different parent material. Differences in soil properties explained 60% of variation in PLFA data and 81% of variation in enzyme activities across regions and land-use intensities. Degraded peat soils in the lowland areas of the SCH with high organic carbon (OC) concentrations and sand content contained lower PLFA biomass, lower concentrations of bacterial, fungal, and arbuscular mycorrhizal PLFAs, but greater enzyme activities, and specific enzyme activities (per unit microbial biomass) than mineral soils in the upland areas of the HAI and ALB, which are finer textured, drier, and have smaller OC concentrations. After extraction of variation that originated from large-scale differences among regions and differences in land-use intensities between plots, soil properties still explained a significant amount of variation in PLFA data (34%) and enzyme activities (60%). Total PLFA biomass and all enzyme activities were mainly related to OC concentration, while relative abundance of fungi and fungal to bacterial ratio were mainly related to soil moisture. Land-use intensity (LUI) significantly decreased the soil C:N ratio. There was no direct effect of LUI on total PLFA biomass, microbial community composition, N and P cycling enzyme activities independent of study region and soil properties. In contrast, the activities and specific activities of enzymes involved in the C cycle increased significantly with LUI independent of study region and soil properties, which can have impact on soil organic matter decomposition and nutrient cycling. Our findings demonstrate that microbial biomass and community composition as well as enzyme activities are more controlled by soil properties than by grassland management at the regional scale. Online reputation management deals with monitoring and influencing the online record of a person, an organization or a product. The Social Web offers increasingly simple ways to publish and disseminate personal or opinionated information, which can rapidly have a disastrous influence on the online reputation of some of the entities. The author focuses on the Social Web and possibilities of its integration with the Semantic Web as resource for a semi-automated tracking of online reputations using imprecise natural language terms. The inherent structure of natural language supports humans not only in communication but also in the perception of the world. Thereby fuzziness is a promising tool for transforming those human perceptions into computer artifacts. Through fuzzy grassroots ontologies, the Social Semantic Web becomes more naturally and thus can streamline online reputation management. For readers interested in the cross-over field of computer science, information systems, and social sciences, this book is an ideal source for becoming acquainted with the evolving field of fuzzy online reputation management in the Social Semantic Web area.

Online reputation management deals with monitoring and influencing the online record of a person, an organization or a product. The Social Web offers increasingly simple ways to publish and disseminate personal or opinionated information, which can rapidly have a disastrous influence on the online reputation of some of the entities. This dissertation can be split into three parts: In the first part, possible fuzzy clustering applications for the Social Semantic Web are investigated. The second part explores promising Social Semantic Web elements for organizational applications,while in the third part the former two parts are brought together and a fuzzy online reputation analysis framework is introduced and evaluated. Theentire PhD thesis is based on literature reviews as well as on argumentative-deductive analyses.The possible applications of Social Semantic Web elements within organizations have been researched using a scenario and an additional case study together with two ancillary case studies—based on qualitative interviews. For the conception and implementation of the online reputation analysis application, a conceptual framework was developed. Employing test installations and prototyping, the essential parts of the framework have been implemented.By following a design sciences research approach, this PhD has created two artifacts: a frameworkand a prototype as proof of concept. Bothartifactshinge on twocoreelements: a (cluster analysis-based) translation of tags used in the Social Web to a computer-understandable fuzzy grassroots ontology for the Semantic Web, and a (Topic Maps-based) knowledge representation system, which facilitates a natural interaction with the fuzzy grassroots ontology. This is beneficial to the identification of unknown but essential Web data that could not be realized through conventional online reputation analysis. Theinherent structure of natural language supports humans not only in communication but also in the perception of the world. Fuzziness is a promising tool for transforming those human perceptions intocomputer artifacts. Through fuzzy grassroots ontologies, the Social Semantic Web becomes more naturally and thus can streamline online reputation management.

Given the increasing interest in using social software for company-internal communication and collaboration, this paper examines drivers and inhibitors of micro-blogging adoption at the workplace. While nearly one in two companies is currently planning to introduce social software, there is no empirically validated research on employees’ adoption. In this paper, we build on previous focus group results and test our research model in an empirical study using Structural Equation Modeling. Based on our findings, we derive recommendations on how to foster adoption. We suggest that micro-blogging should be presented to employees as an efficient means of communication, personal brand building, and knowledge management. In order to particularly promote content contribution, privacy concerns should be eased by setting clear rules on who has access to postings and for how long they will be archived.
