La biblioteca escolar es un servicio de información básico para todos los miembros de una comunidad educativa, que forma parte de los espacios docentes de los centros y de los procesos pedagógicos que tienen lugar en ellos. Las bibliotecas escolares funcionan como centros de recursos para las actividades de enseñanza-aprendizaje, están constituidas por un conjunto sistematizado y dinámico de servicios y fondos documentales que permiten a los usuarios desarrollar hábitos lectores y buscar y valorar las fuentes de información, entre otras relevantes funciones. Los recursos de información que albergan son uno de sus principales activos, pero si colección documental no está organizada, las tareas de búsqueda y localización de la información resultarán complicadas y la calidad de los recursos obtenidos, cuestionable. Los bibliotecarios deben conocer en profundidad las características específicas del fondo documental y las fuentes disponibles; las técnicas y herramientas adecuadas para procesar y tratar el fondo bibliográfico, así como los métodos de recuperación de la información más convenientes. En este contexto, el objetivo de este trabajo es analizar de forma pormenorizada los procesos de indización y clasificación que se realizan en las bibliotecas escolares para procesar y recuperar la información que albergan su colecciones, así como describir las características más relevantes de las herramientas específicas que se usan en las bibliotecas escolares españolas, brasileñas y portuguesas, adaptadas a las características de los usuarios que utilizan sus servicios y acuden a ellas para resolver necesidades de información. Para lograr este propósito, se analiza el concepto de biblioteca escolar de forma crítica, se estudian sus funciones y se examinan las técnicas y los instrumentos que permiten organizar la información. Entre otras herramientas, se estudian listas de encabezamientos de materia como los Encabezamientos de materia para libros infantiles y juveniles y la Lista de Encabezamientos de materia para las bibliotecas públicas; sistemas de clasificación, como la Clasificación Decimal Universal (edición de bolsillo) o la clasificación por centros de interés y tesauros especializados como el Tesauro de la Educación UNESCO-OIE y el Tesauro Europeo de la Educación, entre otros.

The process of Competences Recognition, Validation and Certification , also known as Accreditation of Prior Learning (APL), is an innovative means of attaining school certificates for individuals without an academic background. The main objective of this process is to validate what people have learned in informal contexts, in order to attribute academic certificates. With the increasing interest of the qualification of workers and governmental support, more and more Portuguese organizations promote this process within their facilities and their work hours. This study explores the relationship between the promotion of this Human Resource Development Programme and employee’s attitudes (Job Satisfaction and Organizational Commitment) and behaviours (Extra-role Organizational Citizenship Behaviours) towards the organization they work for. Results of a cross-sectional survey of Portuguese Industrial Workers (N=135) showed that statistical significant results are in the higher levels of Voice Behaviours (a dimension of Extra-role Organizational Citizenship Behaviour in the groups of workers who were involved or had graduated from the firm promoted APL process.

The present study describes the experience of dental caries in Indians communities of the Xingu, in order to supply parameters for further analysis of trends of the disease in Indians. We performed oral health examination in 288 Indians from four communities (Yawalapiti, Aweti, Mehinaku and Kamaiura) living in the southern part of the Xingu National Park, using international criteria defined by the World Health Organization. The outcome measures were the DMFT and dmft scores, and the care index. Indians of the Upper Xingu presented high levels of caries, in all age groups. The average DMFT for 11 to 13-year-old children - 5.93 - was lower than the index measured in 1993 for 12-year-old schoolchildren in nearby cities - 8.23 -, whose United Nations' human development index ranked medium. However, Indians presented a much lower care index, per age group, than these cities, and a high ratio of missing teeth for persons above 20 years old. These observations indicate low incorporation of dental care services. The irregularity of the services programmed for these communities, and the changing dietary and cultural patterns, mainly derived from their contact with the non-indigenous population of Brazil, reinforce the pressing need for health promotion initiatives aimed at these groups.

RESUMO: Objectivo: O exercício tem sido identificado como um factor de promoção da qualidade de vida em mulheres submetidas a cirurgia por cancro de mama. Mas os níveis de actividade física tendem a reduzir após o diagnóstico de cancro de mama e a manterem-se baixos após fim dos tratamentos. O objectivo deste estudo é verificar se um programa de exercício físico supervisionado, associado a estratégias motivacionais em mulheres submetidas a cirurgia por cancro da mama e que mantêm a intervenção usual praticada, é mais efectivo no aumento dos níveis de actividade física, na redução do índice de massa corporal e na melhoria da qualidade de vida, do que o tratamento usualmente praticado. Metodologia: Trata-se de um estudo piloto experimental, aleatorizado e controlado. Cumpriram os critérios de inclusão no estudo 37 utentes submetidas a cirurgia por cancro de mama no Hospital Fernando Fonseca, Amadora, e submetidos à intervenção usual da fisioterapia. Foram aleatoriamente distribuídas pelos grupos experimental (n=19) e de controlo (n=18). O contacto telefónico foi feito por um elemento externo, cego em relação à distribuição dos sujeitos. Alguns elementos desistiram ou não puderam participar no estudo. O número de sujeitos final foi de 11 para o grupo experimental e de 10 para o grupo de controlo, idênticos na média de idades. Todos os sujeitos mantiveram a intervenção usual da fisioterapia. Os sujeitos do grupo experimental foram ainda submetidos ao programa de exercício e promoção da actividade física. Todo o grupo foi avaliado no início do programa, no final do mesmo (12 semanas) e após 6 meses de follow-up, nos seguintes indicadores: nível de actividade física, pelo International Physical Activity Questionnaire (IPAQ), índice de massa corporal e qualidade de vida, pelos questionários: European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) e Supplementary Questionnaire Breast Cancer Module (QLQ-BR23). Resultados: O programa implementado demonstrou-se efectivo no aumento dos níveis de actividade física (p=0,017) e na qualidade de vida associada ao estado funcional (p=0,016) e ao estado de saúde global auto-percepcionado (p=0,030) no final do programa. Foi ainda possível identificar resultados positivos noutras variáveis estudadas, como por exemplo a fadiga auto-reportada, mas que não se demonstraram estatisticamente significativos, facto que pode atribuir-se à reduzida dimensão da amostra. Conclusões: Apesar de algumas limitações, este estudo é de grande a importância para a comunidade científica preocupada com crescente problemática do cancro de mama. Mais uma vez se demonstra que o exercício físico é uma estratégia importante para a melhoria do estado de saúde de pacientes confrontados com doença. Esta comprovação é de grande interesse para os profissionais de saúde e particularmente para os fisioterapeutas dedicados à promoção da saúde nesta população.---------------------ABSTRACT: Purpose: Exercise has been identified as a quality of life promoting factor, amongst women after breast cancer surgery. But the physical activity levels reduce significantly after a diagnosis of breast cancer and remain low after treatment is completed. The aim of this study is to determine whether a supervised group exercise program allied to motivating strategies in women after breast cancer surgery in conjunction with standard treatment, is more effective in improving physical activity levels, reducing body mass index and promoting quality of life, than standard treatment on its own. Methods: This is a pilot randomized controlled trial. Thirty-seven women, submitted to breast cancer surgery in the Hospital Fernando Fonseca, Amadora, and submitted to standard physiotherapy intervention, completed the inclusion criteria. They were randomly allocated into intervention group (n=19) and control group (n=18). The telephone contact was done by an external element, blind to the subjects’ allocation. Some elements declined or could not participate. The final intervention group was n=11 and the control n=10, similar in age. All subjects maintained the standard physiotherapy intervention. The experimental group was submitted to the exercise and health promotion program. The complete group was evaluated in the beginning of the program, at the final (after 12 weeks) and after 6 months follow-up, using as outcomes: physical activity index with the international Physical Activity Questionnaire (IPAQ), the body mass index and quality of life using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) and Supplementary Questionnaire Breast Cancer Module (QLQBR23). Results: The studied program demonstrated to be effective in improving the physical activity index (p=0,017) and quality of live related to physical functioning (p=0,016) and to global health status (p=0,030) at the end of the program. It was possible to find positive results in some other outcomes, such as fatigue, although the differences were not statistically significant. We believe that these results can be attributed to the small sample size. Conclusions: Although we can identify some methodological constrains, this is a very important study for scientific community working on the breast cancer subject. Once more, exercise was identified as an important strategy to improve global heath status in breast cancer patients. This represents an important contribution to the health professionals and mostly for physiotherapists working on health promotion subject.

Considering the impact of cysticercosis on public health, especially the neurologic form of the disease, neurocysticercosis (NC), we studied the frequency of positivity of anti-Taenia solium cysticercus antibodies in serum samples from 1,863 inhabitants of Cássia dos Coqueiros, SP, a municipal district located 80 km from Ribeirão Preto, an area considered endemic for cysticercosis. The 1,863 samples were tested by enzyme linked immunosorbent assay (ELISA) using an antigenic extract from Taenia crassiceps vesicular fluid (Tcra). The reactive and inconclusive ELISA samples were tested by immunoblotting. Of the 459 samples submitted to immunoblotting, 40 were strongly immunoreactive to the immunodominant 18 and 14 kD peptides. Considering the use of immunoblotting as confirmatory due to its high specificity, the anti-cysticercus serum prevalence in this population was 2.1%.

RESUMO - Nas últimas décadas, a especialização dos cuidados médicos tem conduzido a uma fragmentação do sistema de prestação, que, associada a uma deficiente coordenação entre serviços, cuidados e prestadores, torna a navegabilidade dos utentes nos sistemas de saúde uma tarefa complexa. Um novo modelo de organização, assente na procura de valor para os cidadãos, deve adoptar uma abordagem sistémica, que tenha subjacente uma coordenação integrada de serviços, numa perspectiva de ciclo de cuidados. Reorientar a prestação de cuidados para a obtenção de resultados e valor em saúde, exige uma reengenharia em torno da estrutura, organização e avaliação1 dos cuidados, requerendo, nomeadamente: i) instrumentos e ferramentas que auxiliem e estruturem este novo modelo; ii) assumpção dos papéis definidos para cada um dos actores do sistema, nomeadamente ao nível da coordenação; iii) encorajamento à adopção de modelos de contratualização, pagamento e competição, que responsabilizem os actores envolvidos não só pela prática que desenvolvem, mas pelos resultados em saúde. Estes mecanismos constituem uma oportunidade para expandir e sustentar abordagens, programas e intervenções integradas. Investir num sistema de pagamento por valor em saúde — P4V — payment for value, traduz uma aposta na relação entre diagnóstico, tratamento, resultados clínicos e custos, enquanto estratégia para assegurar ganhos em qualidade dos cuidados, eficiência dos processos e valor em saúde para o cidadão. Neste contexto, a gestão da doença enquanto modelo direccionado para o reforço da perspectiva e participação activa do cidadão, e avaliação compreensiva de novas formas de organização e gestão do sistema de prestação, constitui um instrumento para informar e sustentar esse processo de reengenharia do sistema. Um modelo que procura assegurar o encontro entre o estado da arte na prestação de cuidados e um nível óptimo, garantindo a qualidade de vida expectável para a pessoa com doença crónica. ----------------- ---------ABSTRACT – In the last decades advanced medical sciences trend to specialized care and fragmented health systems, leaving patients with a challenge on navigating services and care, requiring them to see a sequence of specialists each delivering discrete interventions. To overcome these challenges, every health system must redefine health care delivery to use its resources more efficiently and improve quality of care through an organization of the system as a whole. A system currently organized around value for patients, entails a framework that comprises the entire set of activities needed to address a patient´s medical condition, over the full cycle of care. Value- based care delivery therefore requires an integrated practice, both across services and time, and implies a movement through new structures, organization models, evaluation efforts and payment systems that enables, catalyze and reinforces the extension and sustainability of the steps needed to the change required. A shift from a payment for performance to a payment for value focuses attention on maximizing the overall value of care, and encourages coordination and integration between components of care that extends from screening, diagnoses, all the way through treatment, outcomes and costs, and ensuring an incentive for potentially high value types of care as well as innovation. These leave the actors of the system with the task of best allocating and valuing components of care. Disease management as a model designed to structure patient engagement and involvement in their care, and assure a comprehensive evaluation and monitoring of new organization and care delivery strategies align an opportunity as a source of information and sustainability for the progress of a growing number of likeminded efforts now underway across care delivery for chronic diseases. Local Tourist Systems (LTS) can be analyzed according to an investigation structure that derives from industrial economics on industrial districts, local productive systems or learning regions. LTS concept is a useful analytical tool that can seize the resorts diversity and organization. Resorts can be conceived both as clusters or industrial districts, either with a perfect agreement between productive sphere and local community or a mere industrial juxtaposition without any economic or social connection. On the other hand tourist clusters analysis has cross referred almost exclusively to socio-economic criteria. Environmental issues were almost disregarded. Approaches swing from the “greening” of products and practices to initiatives focused on an integrated approach, linking environment and tourist development. This paper tries to discuss how to favor – inside a tourist destination - the creation of clusters grounded on sustainable tourism. The prevalence of Trypanosoma cruzi infection was evaluated in Berilo, Minas Gerais (MG), Brazil, from January to July 1997. A serological survey using the indirect immunofluorescence test (IFT) in dried blood collected on filter-paper was performed in a sample of 2,261 individuals. The overall prevalence rate of T. cruzi infection was 18%, and reached 50% in individuals older than 30 years from rural areas. The percentage of seropositivity was 0.17% among individuals younger than 10 years old, suggesting that vectorial transmission is controlled in the area. A decrease in prevalence rates among people born after 1960 and 1970 was observed and this appears to be correlated with the beginning of control programs. A reduction in T. cruzi infection rates was observed when comparing our results with the rates estimated in a serologic study carried out in Berilo in 1983(11).

This work aims to evaluate the impact of drug treatment on infection by Ascaris lumbricoides (Al), Trichuris trichiura (Tt) and hookworms (Hook) in a rural community from the sugar-cane zone of Pernambuco, Brazil. Four parasitological surveys were carried out from March 2001 to March 2002. Individual diagnosis was based on eight slides (four by the Kato-Katz method and four by the Hoffman method) per survey. Infected subjects were assigned to two groups for treatment with either albendazole (n = 62) or mebendazole (n = 57). Prevalence of infection fell significantly (p < 0.05) one month after treatment: Al (from 47.7% to 6.6%); Tt (from 45.7% to 31.8%) and Hook (from 47.7% to 24.5%). One year after treatment, infections by Tt and Hook remained significantly below pre-control levels. A substantial decrease in single-infection cases and multiple infections was found. Egg-negative rate was significant for Al (94.0%), Hook (68.3%) but not for Tt (45.5%), and did not differ significantly between subjects treated with mebendazole or albendazole. Egg counts fell significantly in the individuals remaining positive for Tt. It is recommended that antihelminthic treatment should be selective and given at yearly intervals preferably with albendazole, due to its cost-effectiveness.

Parasitic infections caused by intestinal protozoan and helminths affect more than two billion people worldwide and chemotherapy is the most commonly used therapeutic procedure. Considering the problems created by parasitic infections and the incorrect use of drugs, the aim of this work was to detect the frequency of enteroparasites infection and to estimate the use of chemotherapeutic agents in children living in the periphery of the city of Porto Alegre, RS, Brazil. Ninety-six preschool age children, who had parasitological exams and who used antiparasitic drugs, were analyzed. The efficacy of treatment was evaluated by stool examination repeated six months after treatment. The same diagnostic test was used to evaluate parasitological cure, which was defined as absence of eggs and cysts in the stool. From these children, 79 (82.3%) were contaminated by some species of parasite, the most prevalent were Ascaris lumbricoides, Trichuris trichiura and Giardia lamblia. The most commonly used drugs were mebendazole (86% of prescriptions) and metronidazole (30.3%). The cure rate in the 79 children, examined 6 months after treatment, was 65.3% for A. lumbricoides and 66.1% for T. trichiura. This study suggests that a continuous education program regarding the prevention and treatment of parasitic infections is an essential tool for their eradication.

The objective was to estimate the prevalence of Blastocystis hominis, to evaluate the effectiveness of different techniques for its diagnosis as well as to estimate the prevalence of other intestinal parasites in the community of Campo Verde, a district of Pitanga. The work was carried out from August to October 2004. Samples of feces from children and adults were collected and submitted to the techniques of direct wet mount, flotation in zinc sulphate solution, tube sedimentation, sedimentation in formalin-ether and staining by Kinyoun and iron hematoxylin methods. From 181 studied individuals, 128 (70.7%) showed protozoa and/or helminths in stool samples. The most prevalent species were Endolimax nana (33.7%); B. hominis (26.5%); Giardia lamblia (18.2%); Entamoeba coli (17.1%); Ascaris lumbricoides (16.6%); Iodamoeba bütschlii (9.4%); and ancylostomatidae (7.7%). B. hominis was only identified by the techniques of direct wet mount, sedimentation in formalin-ether and staining by iron hematoxylin, though the latter was less sensitive than the other methods. The high frequency of B. hominis demonstrated by this study indicates the need to include laboratory techniques that enable identification of the parasite on a routine basis.
