Abstract OBJECTIVE Analyzing the elements that compose the environment of pregnant women who have experienced intimate partner violence in the light of Levine's Nursing Theory. METHOD A qualitative, descriptive study conducted from September to January 2012, with nine pregnant women in a Municipal Health Center in Rio de Janeiro. The interviews were semi-structured and individual. The theoretical framework was based on Levine's Nursing Theory. RESULTS Thematic analysis evidenced the elements that composed the external environment, such as violence perpetrated by intimate partners before and during pregnancy, violence in childhood and adolescence, alcohol consumption and drug use by the partner, unemployment, low education and economic dependency, which affected health and posed risks to the pregnancy. CONCLUSION Violence perpetrated by an intimate partner was the main external factor that influenced the internal environment with repercussions on health. This theory represents a tool in nursing care which will aid in detecting cases and the fight against violence.

La dysplasie pseudorhumatoïde progressive (PPRD) est une arthropathie non- inflammatoire causée par des mutations récessives du gène WISP3. Elle se manifeste pendant la petite enfance avec une raideur progressive des articulations et des douleurs. Les patients sont référées habituellement aux rhumatologues pédiatres et aux orthopédistes, et seulement dans un deuxième temps, vers les généticiens et/ou experts dans les dysplasies osseuses. Pour cette raison le diagnostic clinique, qui repose sur les signes radiologiques typiques et l'expérience clinique, est souvent retardé et les patients peuvent recevoir des traitements anti-inflammatoires et immunosuppresseurs innécessaires. Nous reportons ici une large série de patients atteints de PPRD avec confirmation du diagnostic au niveau moléculaire, et nous soulignons les caractéristiques cliniques et radiologiques de la maladie. Il existe une fenêtre d'âge dans laquelle les signes radiologiques sont spécifiquement reconnaissables. Des anomalies spondyloépiphyseales très légères peuvent apparaître avant l'âge de 9 ans et une perte de cartilage non-spécifique qui ressemble à une ostéoarthrite avancée est présente chez les jeunes adultes. Les articulations interphalangiennes sont les premières à être atteintes, suivis par les genoux et les hanches. L'atteint de la colonne arrive chez les enfants plus grands et dans l'adolescence. Une cyphose est fréquente et une scoliose survienne chez une minorité de patients. Des signes d'ostéoarthrite avancée comme des formations ostéophytiques et/ou des calcifications périarticulaires sont observés chez les jeunes adultes atteints de PPRD et peuvent être responsables d'une certaine inflammation secondaire. L'analyse moléculaire du gène WISP3 par séquençage de l'ADN génomique permet de confirmer le diagnostic dans la plupart des cas. Néanmoins, des splicings alternatives causés par des mutations introniques peuvent être détectés dans le cDNA des fibroblastes. Dans le cas où l'analyse génomique ne montre aucune mutation chez un individu présentant les signes et symptômes typiques de la maladie, une biopsie de peau est indiquée pour analyse moléculaire du cDNA. La prise en charge de la PPRD est symptomatique et largement insatisfaisante. Le remplacement des articulations les plus atteintes est souvent nécessaire dès l'adolescence afin de diminuer les douleurs et maintenir la mobilité.

This research aims to provide a contribution towards understanding how and why certain people can display disobedience behaviors, to overcome unjust situations, and withstand persecutions deployed by authority. The paper presents a hermeneutic content analysis of the autobiographical speeches and texts of Gandhi, M. L. King and Mandela. Our results show that parents' value orientation, experience of injustice during childhood and exploration of alternative viewpoints during adolescence play a crucial role in structuring prosocial disobedience. Findings show also that social responsibility and ingroup communication are important conditions for facing persecutions without dropping original goals.

Aims: Inflammatory bowel diseases (IBD) appearing during childhood and adolescence compromise peak bone mass acquisition and increase fracture risk. The structural determinants of bone fragility in IBD however remain unknown. Methods: We investigated volumetric bone mineral density (vBMD), trabecular and cortical bone microstructure at distal radius and tibia by high-resolution pQCT (XtremeCT, Scanco, Switzerland), aBMD at distal radius, hip and spine and vertebral fracture assessment (VFA) by DXA in 107 young patients (mean age 22.8 yrs, range 12.2-33.7 yrs; 62 females and 45 males) with Crohn's disease (n=75), ulcerative colitis (n=25), undetermined colitis (n=2), and no definitive diagnosis (n=5), and in 389 healthy young individuals. Results: Mean disease duration was 6.1 yrs, 89/107 IBD patients received corticosteroids, 83 other immunomodulators, and 59 vitamin D. Clinical fractures were reported by 38 patients (mean age at 1st fracture, 12.6 yrs), the vast majority of the forearm, arm or hand; 5 had vertebral crush fractures (Grade 1 or 2) and 11 had multiple fractures. As compared to healthy controls (matched 2:1 for age, sex, height and fracture history), the 102 patients with established IBD had similar weight but significantly lower aBMD at all sites, lower trabecular (Tb) BV/TV and number, and greater Tb separation and inhomogeneous Tb distribution (1/SD TbN) at both distal radius and tibia, lower tibia cortical thickness (CTh), but no differences in cortical vBMD nor bone perimeter. Among IBD's, aBMD was not associated with fractures (by logistic regression adjusted for age, age square, sex, height, weight and protein intake). However, radius and tibia Tb BV/TV, thickness and SD 1/TbN, as well as radius Tb separation and perimeter, were significantly associated with fracture risk (fully adjusted as above), whereas cortical vBMD and CTh were not. After adjustment for aBMD at radius, respectively at femur neck, radius SD 1/TbN and tibia BV/TV, TbTh and perimeter remained independently associated with fracture risk. Conclusions: Young subjects with IBD have low bone mass and poor bone microarchitecture compared to healthy controls. Alterations of bone microarchitecture, particularly in the trabecular bone compartment, are specifically associated with increased fracture risk during growth.

OBJECTIVE: The objective of the study is to evaluate cross-sectional and longitudinal changes in children's commuting to school in a representative sample of a Brazilian city. METHODS: Two school-based studies were carried out in 2002 (n=2936; 7-10years old) and 2007 (n=1232; 7-15years old) in Florianopolis, Brazil. Cross-sectional data were collected from children aged 7 to 10years in 2002 and 2007. Longitudinal analyses were performed with data from 733 children participating in both surveys. Children self-reported their mode of transportation to school using a validated illustrated questionnaire. Changes were tested with chi square statistics and McNemar's test. RESULTS: Cross-sectional data showed a 17% decline in active commuting; a decrease from 49% in 2002 to 41% in 2007. On the other hand, active commuting among the 733 children increased as they entered adolescence 5years later, rising from 40% to 49%. CONCLUSION: Active commuting to school decreased in Brazilian children aged 7-10years over a five year period; whereas, it increased among children entering adolescence. Policies should focus on safety and environmental determinants to increase active commuting.

This study explored the links between having older siblings who get drunk, satisfaction with the parent-adolescent relationship, parental monitoring, and adolescents' risky drinking. Regression models were conducted based on a national representative sample of 3725 8th to 10th graders in Switzerland (mean age 15.0, SD = .93) who indicated having older siblings. Results showed that both parental factors and older siblings' drinking behaviour shape younger siblings' frequency of risky drinking. Parental monitoring showed a linear dose-response relationship, and siblings' influence had an additive effect. There was a non-linear interaction effect between parent-adolescent relationship and older sibling's drunkenness. The findings suggest that, apart from avoiding an increasingly unsatisfactory relationship with their children, parental monitoring appears to be important in preventing risky drinking by their younger children, even if the older sibling drinks in such a way. However, a satisfying relationship with parents does not seem to be sufficient to counterbalance older siblings' influence.

Switzerland has the lowest adolescent fertility rate in Western Europe. According to data collected in 1993 as part of the Swiss Multicentre Adolescent Survey on Health, 5% of 1,726 sexually active adolescents in a group of 3,993 15-20-year-old women enrolled in academic or vocational classes had ever been pregnant; most of these women (80%) had terminated their pregnancy. Adolescents who had ever been pregnant did not differ significantly from those who had not by demographic characteristics. Multiple logistic regression analysis identified seven factors associated with pregnancy: having had four or more sexual partners; not having used contraceptives at first intercourse; ever use of less-effective contraceptive methods; having used illicit drugs during the last 30 days; living apart from one's parents; recently experiencing stress; and perceiving a lack of future prospects.

Aims: To provide 12-month prevalence and disability burden estimates of a broad range of mental and neurological disorders in the European Union (EU) and to compare these findings to previous estimates. Referring to our previous 2005 review, improved up-to-date data for the enlarged EU on a broader range of disorders than previously covered are needed for basic, clinical and public health research and policy decisions and to inform about the estimated number of persons affected in the EU. Method: Stepwise multi-method approach, consisting of systematic literature reviews, reanalyses of existing data sets, national surveys and expert consultations. Studies and data from all member states of the European Union (EU-27) plus Switzerland, Iceland and Norway were included. Supplementary information about neurological disorders is provided, although methodological constraints prohibited the derivation of overall prevalence estimates for mental and neurological disorders. Disease burden was measured by disability adjusted life years (DALY). Results: Prevalence: It is estimated that each year 38.2% of the EU population suffers from a mental disorder. Adjusted for age and comorbidity, this corresponds to 164.8 million persons affected. Compared to 2005 (27.4%) this higher estimate is entirely due to the inclusion of 14 new disorders also covering childhood/adolescence as well as the elderly. The estimated higher number of persons affected (2011: 165 m vs. 2005: 82 m) is due to coverage of childhood and old age populations, new disorders and of new EU membership states. The most frequent disorders are anxiety disorders (14.0%), insomnia (7.0%), major depression (6.9%), somatoform (6.3%), alcohol and drug dependence (>4%), ADHD (5%) in the young, and dementia (1-30%, depending on age). Except for substance use disorders and mental retardation, there were no substantial cultural or country variations. Although many sources, including national health insurance programs, reveal increases in sick leave, early retirement and treatment rates due to mental disorders, rates in the community have not increased with a few exceptions (i.e. dementia). There were also no consistent indications of improvements with regard to low treatment rates, delayed treatment provision and grossly inadequate treatment. Disability: Disorders of the brain and mental disorders in particular, contribute 26.6% of the total all cause burden, thus a greater proportion as compared to other regions of the world. The rank order of the most disabling diseases differs markedly by gender and age group; overall, the four most disabling single conditions were: depression, dementias, alcohol use disorders and stroke. Conclusion: In every year over a third of the total EU population suffers from mental disorders. The true size of "disorders of the brain" including neurological disorders is even considerably larger. Disorders of the brain are the largest contributor to the all cause morbidity burden as measured by DALY in the EU. No indications for increasing overall rates of mental disorders were found nor of improved care and treatment since 2005; less than one third of all cases receive any treatment, suggesting a considerable level of unmet needs. We conclude that the true size and burden of disorders of the brain in the EU was significantly underestimated in the past.Concerted priority action is needed at all levels, including substantially increased funding for basic, clinical and public health research in order to identify better strategies for improved prevention and treatment for isorders of the brain as the core health challenge of the 21st century. As escolhas vocacionais são momentos importantes no ciclo do desenvolvimento humano, sendo particularmente relevantes as que ocorrem ainda na adolescência. Em Cabo Verde são escassos os estudos sobre o desenvolvimento vocacional e os projectos vocacionais dos jovens cabo-verdianos. O presente estudo tem como objectivo analisar as atitudes vocacionais e a construção de projectos futuros de adolescentes. Para tal, foram inqueridos 100 alunos de 9º ano de escolaridade de uma escola secundária de Cabo Verde, dos quais 45 são rapazes e 55 raparigas. Esses alunos responderam o questionário sociodemográfico, o Inventário de Desenvolvimento de Carreira, a Escala de Atribuição para Carreira e uma versão adaptada do Inventário de Percepção de Barreira na Carreira. Os resultados mostraram que a maioria dos participantes apresenta projectos, variando no grau de especificação. As raparigas foram mais específicas nos seus projectos. Nas atitudes de planeamento e exploração vocacional as diferenças de género não foram significativas. A análise das crenças atribucionais para a carreira permitiu observar diferenças mais significativas de género, na subescala causalidade interna associado ao fracasso, diferença esta, favorável aos rapazes. Nas percepções de barreiras, a diferença mais significava entre raparigas e rapazes verificaram-se na variável: restrição de oportunidades. Por fim, discute-se a importância da orientação vocacional e apresenta as limitações e implicações do estudo.

During adolescence, nutrition needs are high; however the literature shows that few adolescents are following standardized nutritional requirements. A few weeks before an intervention about nutrition to high school adolescents in Lausanne, they were invited to fill in a self-reported questionnaire about their nutrition modes and habits, and their self-image satisfaction (N = 198). Results show that only 5% of youth are eating 5 fruits and vegetables per day and only 29% 3 to 5 dairy products. 21% of female and 6% of boys are not satisfied about their self-image, and those exhibiting a poor self-image tend to adopt health compromising eating patterns in a higher proportion. During adolescence it is important not only to investigate the nutritional habits but also one's self image.

Introduction: Le tabagisme est une cause majeure de maladies et de décès dans la population en Valais. Près d'un valaisan sur trois et une valaisanne sur quatre sont fumeurs. L'initiation au tabagisme a lieu le plus souvent à l'adolescence. Il est dès lors important de connaître les habitudes tabagiques des écoliers valaisans. Méthodes: Tous les quatre ans, le Valais et les autres cantons suisses participent à l'enquête Health Behaviour in School aged Chidren (HBSC), qui est réalisée dans plus de 40 pays sous l'égide de l'Organisation mondiale de la santé. L'objectif est de récolter et d'analyser des données pertinentes sur les comportements de santé des écoliers de 11 à 15 ans, et notamment sur le tabagisme. L'enquête HBSC 2010 a été menée dans 71 classes auprès de 1'198 écoliers en Valais. Les données ont été récoltées par questionnaire. Résultats: De nombreux écoliers valaisans de 11-15 ans ont déjà fumé au moins une fois dans leur vie. Alors que très peu d'écoliers sont des fumeurs quotidiens avant l'âge de 14 ans, près d'un écolier sur six (17% des garçons et des filles) affirme fumer quotidiennement à 15 ans. Entre 2002 et 2010, les proportions d'écoliers ayant fumé au moins une fois ou fumant quotidiennement ont diminué. Les écoliers de 14-15 ans fumant régulièrement (au moins une fois par semaine) rapportent avoir acheté le plus souvent eux mêmes leurs cigarettes ou se les êtres procurées auprès de connaissances. 42% des écoliers et 44% des écolières ont au moins leur mère, leur père ou un de leurs frères et soeurs qui fument. Conclusions et perspectives : Le tabagisme est fréquent dans la population valaisanne, et ce dès l'adolescence. La prévalence du tabagisme a légèrement diminué chez les écoliers entre 2002 et 2010. Toutefois, l'augmentation marquée de l'initiation tabagique entre 11 et 15 ans et la prévalence importante des fumeurs quotidiens parmi les écoliers de 15 ans confirment que la prévention du tabagisme est nécessaire dès le plus jeune âge. L'accessibilité des écoliers au tabac devrait être évaluée et des mesures devraient être renforcées pour la diminuer.
