Objetivos: O presente estudo tem como objectivo analisar as redes sociais e pessoais de idosos portugueses com filhos e sem filhos, relativamente às características estruturais, funcionais e relacionais-contextuais. Metodologia: Para a avaliação das variáveis em estudo foram utilizados: o Instrumento de Análise da Rede Social Pessoal, versão para idosos (IARSP – Idosos) (Guadalupe, 2010; Guadalupe & Vicente, 2012) para avaliar as dimensões da rede social pessoal; um inquérito por questionário para caracterização da amostra a nível sociodemográfico e sociofamiliar. Participantes: A amostra é constituída por 418 idosos, com uma média de idades de 76 anos (DP=7,62), entre os 65 anos e os 98 anos, maioritariamente do sexo feminino (63,9%), casados ou em união facto (51,0%) e com escolaridade (63,9,1%). A maioria dos idosos inquiridos vive com agregado familiar (80,4%) em zonas rurais (64,8) e não usufrui de qualquer tipo de apoio de resposta social (71,5%). Resultados: Os idosos com filhos são maioritários (n=364; 87,1%) e os sem filhos os minoritários (n=54; 12,9%) na nossa amostra. Dos que têm filhos, 29,9% (109) têm filhos únicos e 70,1% (255) têm mais do que um filho (67% com 1 ou 2 filhos), sendo os filhos maioritariamente de ambos os sexos (n=158; 43,4%). Os resultados sugerem que o facto de os idosos terem ou não terem filhos reflete-se sobretudo em diferenças nas características estruturais das redes. Os idosos com filhos apresentam uma média mais elevada no tamanho da rede, na proporção das relações de familiares, e satisfação da rede e reciprocidade de apoio (p ≤ 0,001), a nível das características funcionais. Já relativamente à proporção das relações de amizade, vizinhança na rede, são os idosos sem filhos que apresentam uma média substantivamente mais elevada (p ≤ 0,003) do que a outra subamostra, apresentando também uma maior durabilidade das relações interpessoais (p < 0,03). As correlações entre o número total de filhos/as com as características estruturais e funcionais da rede, indicam-nos que quanto maior é o número de filhos maior é o tamanho da rede, a proporção das relações familiares, o apoio emocional, a reciprocidade de apoio, a satisfação com a rede e com o suporte social. Conclusões: O nosso estudo revela que as redes sociais pessoais dos idosos diferenciam-se a nível estrutural segundo o facto de terem ou não filhos, mas também nalgumas variáveis funcionais. Os idosos com filhos apresentam redes mais alargadas e muito centradas nas relações familiares na rede (sensivelmente menos um terço) do que os idosos sem filhos. Os últimos apresentam redes menores (com cerca de 2 pessoas a menos) e mais investidas nas relações de amizade e de vizinhança (com proporções que representam mais do dobro destes vínculos). / Goals: The present study has the purpose to analyze the personal social networks of Portuguese elder with our without sons/daughters, in their structural, functional and contextual relations. Methodology: We used for the evaluation of the variables: The Personal Social Network Analysis Tool, regarding elderly people (IARSP – elderly people) (Guadalupe, 2010; Guadalupe & Vicente, 2012), to evaluate the personal social network dimensions; a survey through an inquiry to characterize the sample at sociodemographic and at sociofamily level. Participants: The sample includes 418 elderly with an average age of 76 years old (DP = 7,62) between 65 and 98 years old, mainly women (63,9%), married or living as a couple (51,0%) with education (63,9,1%). Most of the elderly people who answered the survey live in family household (80,4%) in rural areas (64,8) and do not benefit from any type of social services support (71,5%). Results: The majority is elderly people with sons/daughters (n=364, 87,1%) and the ones without are the minority (n= 54, 12,9%) in our sample. Within those who are parents, 29,9% (109) have only one child and 70,1% (255) have more than one child (67% with 1 or 2), and the children are mainly of both sexes (n= 158; 43,4%). The results suggest that the fact that the elderly have or not have children is reflected especially in differences in the structural characteristics of the networks. Elderly people with children have a higher average in the network size, in the proportion of family relationships, and satisfaction of the network and reciprocal support (p ≤ 0,001). Regarding the proportion of friendly relations, network neighborhood, are the elderly without children who have a substantially higher mean (p ≤ 0,003) than the other subsample, also featuring improved durability of interpersonal relationships (p < 0,003). The correlations between the total number of children/with the structural and functional characteristics of the network, indicate that the greater the number of children, the greater the size of the network, the proportion of family relationships, emotional, support, reciprocity support and satisfaction with social network. Conclusion: Our study reveals that the personal social networks of the elderly differ in the structural level, according to whether they have sons/daughters or not, but also in some functional variables. The elderly with children have networks much more broader and centered on family relationships (roughly one-third) than elderly without children. These elderly without children have smaller networks (about 2 fewer people) and more centered in relations of friendship and neighborhood (with proportions that are more than the double compared to the other subsample).

Con un diseño experimental se realizó un estudio clínico, controlado, aleatorizado La muestra incluyó 200 pacientes, el grupo experimental comprendió 100 pacientes en las que se drenó la sangre de cordón umbilical 50 anémicas y 50 no anémicas, y el grupo de control con pinzamiento del cordón lo integraron 100 pacientes, 50 anémicas y 50 no anémicas. Resultados: al comparar la duración del tercer período del parto del grupo con drenaje se obtuvo una media de 4,6 ñ 1,4 min y en el grupo con pinzamiento 9,07 ñ 2,5 min. La diferencia fue significativa (P = 0,0001).Cuando se comparó la duración del tercer período del parto de 1 a 5 min vs 6 a 10 min, la mayoría de las pacientes del grupo con drenaje, estuvo entre 1 a 5 min, con un RR 0.239 (IC 95: 0.188 - 0.358), RRA 70.1, RRR 76.1, NNT 1.426, en las anémicas, y un RR 0.250 (IC 95: 0.179 - 0.383), RRA 66.1, RRR 75, NNT 1.513 en las no anémicas. El volumen de drenaje en el grupo de estudio, tuvo una media de 60.3 ñ 19.5 ml en las anémicas y 56.9 ñ 18 ml en las no anémicas (P = 0.369). La hemorragia del tercer período del parto del grupo con drenaje tuvo una media de 197.6 ñ 36 ml vs el grupo con pinzamiento 277.4 ñ 49 ml con un valor (P = 0,0001) lo cual es estadísticamente significativo a favor del drenaje. Cuando se comparó la hemorragia del tercer período del parto de menor que 250 ml vs 250 a 500 ml la mayoría de las pacientes del grupo con drenaje, estuvo en menor que 250 ml, con un RR 0.070 (IC 95: 0.025 - 0.168), RRA 80, RRR 93, NNT 1.25, en las anémicas, y un RR 0.074 (IC 95: 0.020 - 0.246), RRA 50, RRR 92.6, NNT 2 en las no anémicas. Conclusiones. La maniobra del drenaje de sangre del cordón umbilical reduce el tiempo y la hemorragia del tercer período del parto en pacientes con anemia leve y no anémicas sin producir efectos deletéreos

Background: Constipation is an intestinal dysfunction. Prebiotics, such as inulin, can improve bowel function by positively influencing intestinal biota. Aim: To analyze the scientific evidence for the role of inulin in improving bowel function in patients with chronic constipation. Methods: A meta-analysis of randomized controlled clinical trials was conducted, grounded on a literature search for the period 1995-2013 (descriptors: inulin & constipation) on PubMed, ScieLo and Central Trials Register Cochrane databases. A total of 24 articles were found, 5 of them were selected for this meta-analysis, involving 252 subjects (experimental group: n = 144, control group: n = 108). The quality of the studies was assessed using the Jadad scale. Results: We found a significant overall effect of inulin on stool frequency (DEM = 0.69, 95%CI: 0.04, 1.34), stool consistency (Bristol scale) (DEM = 1.07, 95% CI: 0.70, 1.45), transit time (DEM = -0.57, 95% CI: -0.99, -0.15) and hardness of stool (RR = 0.42, 95% CI: 0.26, 0.70). Pain and bloating do not improve with inulin intake. Conclusions: inulin intake has a positive effect on bowel function.

Groundnut rosette disease (GRD) is the most destructive virus disease of Valencia groundnuts ( Arachis hypogaea L.) in sub-Saharan Africa. Cultural, biological and chemical control measures have received limited success due to small scale farmers’ inability to use them. Use of host plant resistance provides the most effective and economically viable management option for the resource poor farmers. This study was conducted to determine heritability for resistance to GRD in Valencia groundnuts. Six crosses; Valencia C (P1) × ICGV-SM 90704 (P2), Valencia C (P1) × ICGV-SM 96801(P2), Valencia C (P1) × ICGV-SM 99566 (P2), NuMex-M3 (P1) × ICGV-SM 90704 (P2), NuMex-M3 × ICGV-SM 96801 (P2), and NuMex-M3 (P1) × ICGV-SM 99566 (P2), were made to generate F1, F2, BC1P1 and BC1P2 populations. Data on GRD severity were collected on a 1-9 score scale. Genetic Advance as a percentage of the mean (GAM) and heritability were estimated using variance components. Phenotypic Coefficient of Variation (PCV) and Genotypic Coefficient of Variation (GCV) estimates were high (20.04-70.1%) in the six crosses, except for Valencia C × ICGV-SM 96801(18.1%) and NuMex-M3 × ICGV-SM 96801(17.1%), which exhibited moderate GCV values. Broad and narrow sense heritability estimates for GRD disease score ranged from 64.1 to 73.7% and 31 to 41.9%, respectively, in all the crosses. GAM was high in all the crosses (21-50.7%), except for Valencia C x ICGV-SM 96801 (14.67), M3 x ICGV-SM 99566 (18%) and NuMex-M3 x ICGV-SM 96801 (13.5%) crosses that exhibited moderate GAM. The study revealed the presence of variability of GRD resistance, implying that genetic improvement of these exotic materials is possible.

The noise emanating from generators has adverse effects on our health. In view of this, this paper reports the development of soundproof device for 950Watt rated generators which are widely used portable generators. Performance evaluation of the soundproof device was carried out, and the sound pressure level of the generator was reduced by 7.64, 6.24, 6.82, 8.72 and 8.68dB at distances of 0.70, 1.40, 2.10, 2.80 and 3.50m from the generator respectively.

Background and Aim: The prevalence of alcohol use has increased globally. Out-of-school youth are a vulnerable group who might have missed opportunities for learning healthy behaviours in a formal school environment. The purpose of this study was to determine the risk perception, pattern of use, and correlates of alcohol use among out-of-school youth in Lagos, Nigeria. Methods: A cross-sectional study was conducted among 380 out-of-school youth in motor parks in Lagos State, Nigeria, using interviewer administered questionnaires. Results: The lifetime prevalence of alcohol use was 61.1%, while 55.5% were current drinkers. Beer (57.3%) was the most consumed type of alcohol, followed by distilled spirits (29.8%). Using the CAGE scoring system, more than half (57.8%) of the current drinkers had a drinking problem. Almost three quarters (70.1%) had experienced at least one episode of alcohol intoxication within the past month. A considerable number of current drinkers (63.5%) desired to reduce their alcohol intake or stop drinking, while 45.5% had made unsuccessful attempts to do so within the past year. Only 28.9% had received assistance to quit or reduce their drinking and of these less than half (39.3%) received assistance from a professional or healthcare worker. Males were more likely to be current drinkers and to have experienced episodes of alcohol intoxication. Parental and peer drinking were associated with alcohol use but not with intoxication. Conclusions: It is important to design specific programmes to reduce alcohol use among out-of-school youth in these settings.

Background: Over the last 10 years, Australia's spontaneous vaginal birth rate has decreased approximately 1% each year and the caesarean section rate has increased approximately 1% each year. This trend has serious implications for the health of women and babies. As midwives we are the caretakers of normal birth and therefore partly responsible for its decline and the solution to its decline. Although antenatal education is in a potentially powerful position to promote normal birth, a structured review conducted as part of this thesis found that it does not realise that potential. Currently framed in pathogenesis, antenatal education in particular and maternity services in general, are in need of reframing. The theory of salutogenesis may offer a new lens as it focuses on health rather than illness. Sense of coherence is the cornerstone of salutogenesis and is a predictive indicator of health. What is unclear is the role pregnant women's sense of coherence plays in their birthing outcomes. This study explored associations between pregnant women's sense of coherence, their pregnancy choices, their anticipated labour choices, their labour and birthing outcomes as well as factors associated with modification to sense of coherence from the antenatal to postnatal periods. Methods: After a comprehensive review of the literature, questionnaire development and psychometric tool testing and modification, a longitudinal survey was conducted where eligible women completed a questionnaire before the 30th week of pregnancy (Phase One) and approximately 8 weeks after birth (Phase Two). Eligible women were less than 30 weeks pregnant with a single fetus, could read and write in English and lived in the Australian Capital Territory in Australia. Phase One provided information on women's sense of coherence scores, Edinburgh Postnatal Depression Scale (EPDS) scores, Support Behaviour Inventory (SBI) scores, pregnancy choices including care proVider, planned place of birth, planned birth type and anticipated epidural use and demographics. Phase Two provided information on women's sense of coherence scores, EPDS scores and their labour and birthing outcomes. Findings: 1074 women completed Phase One representing a 61.3% response rate. 753 women completed Phase Two representing a 70.1% retention rate between phases. Compared to women with low sense of coherence, women with high sense of coherence were older, reported fewer pregnancy conditions such as diabetes or hypertension, were less likely to have depressive symptoms, were more likely to feel well supported, were less likely to experience a caesarean section and more likely to experience an assisted vaginal birth. Sense of coherence was not associated with women's pregnancy choices. Higher EPDS scores, lower sense of coherence and greater satisfaction with birth were associated with an increase in women's sense of coherence from the antenatal to the postnatal period. Decreased birth satisfaction and experiencing epidural anaesthesia in labour and assisted vaginal birth were associated with a decrease in sense of coherence from the antenatal to the postnatal period. Conclusion: Strong sense of coherence in pregnant women halved the likelihood of experiencing caesarean section compared to women with low sense of coherence. Sense of coherence is a modifiable predictor of women's childbearing health and was found to be raised by birth satisfaction and lowered by birth dissatisfaction and labour interventions. These important findings add to the limited body of knowledge about sense of coherence and childbearing.

Purpose: This study was aimed to evaluate the antioxidant activity of the methanol extract of Euphorbia spinidens (Euphorbiaceae) and its effect on Herpes simplex virus type-1 (HSV-1) replication. Methods: The methanol extract of aerial parts of E. spinidens collected from Khorasan State in North- Eastern part of Iran was used in this study. Total phenolic, flavonoid contents and the antioxidant activity were evaluated using Folin-Ciocalteu method, aluminum chloride colorimetric method and β- carotene-linoleate model system, respectively. Both the cytotoxic and antiviral effects of the crude extract on Vero cell line were determined by quantifying the viability of Vero cells using 3-(4,5- dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)2H-tetrazolium (MTS) assay. Results: Total phenolic and flavonoid contents of E.spinidens were 70 ± 1 mg of gallic acid equivalent/g of dry extract (mg GAE/g extract) and 49.66 ± 1.00 mg rutin equivalent/g of dry extract (mg RTN/g extract), respectively. Antioxidant activity was 44 ± 1 % compared with the standard, buthylated hydroxytuloene (BHT). The 50 % cytotoxic concentration (CC50) of the extract on Vero cells was 5.072 ± 0.063 mg/ml and its antiviral concentration of 50 % effectiveness (EC50) value was 0.34 ± 0.003 mg/ml. Conclusion: The findings of this study show that the methanol extract of E. spinidens has high content of phenolic and flavonoid compounds with good antioxidant activity. Furthermore, this extract has significant antiviral effect on HSV-1 probably due to the inhibition of viral replication.

Purpose: To evaluate the effectiveness of intravenous thrombolysis in combination with nicorandil in the treatment of acute ST-segment elevation myocardial infarction (STEMI). Methods: Patients who developed acute STEMI and underwent intravenous thrombolysis in the hospital were selected and divided into observation group (n = 128) and control group (n = 114). Besides thrombolytic therapy, the observation group was also given 20 mg of nicorandil. The control group received conventional thrombolytic therapy only. Clinical effects and rehabilitation of patients were observed. Results: Cardiac troponin I (cTNI) level of the observation group was 4.0 ± 1.5, 8.3 ± 2.8 and 9.8 ± 3.9 after 4, 12 and 24 h, respectively, which is much lower than 5.8 ± 1.4, 11.4 ± 2.7 and 13.2 ± 4.2 in the control group (p < 0.05). ST-segment resolution of observation group was higher (44 ± 14, 52 ± 17, 69 ± 21 and 80 ± 18) % at different time points, compared with the control group (p < 0.05). The proportion of patients with Curtis-Walker score > 3 points, and ventricular wall motion score (4.70 %; 1.38 ± 0.11) in the observation group were both lower than those of the control group (21.00 %; 1.43 ± 0.15) (p < 0.05). The difference in adverse cardiac events between the observation group (N = 6, 4.70 %) and control group (N = 12, 10.50 %) was not statistically significant (p > 0.05) Conclusion: Combining intravenous thrombolysis with nicorandil therapy can enhance myocardial perfusion level, reduce myocardial damage, improve cardiac function and decrease risk of arrhythmia for acute STEMI patients.

Las enfermedades raras o huérfano son una problemática que ha tomado mucha importancia en el contexto mundial del presente siglo, estas se han definido como crónicas, de difícil tratamiento de sus síntomas y con baja prevalencia en la población; muchas de estas enfermedades cursan con varios tipos de discapacidad, siendo el objetivo del presente trabajo el enfocarse en aquellas enfermedades raras que cursan con discapacidad intelectual. Para poder profundizar en estas enfermedades se realizó una revisión teórica sobre las enfermedades raras, así como de la discapacidad psíquica y su importancia a nivel mundial y nacional. A partir de estas definiciones, se revisaron en profundidad 3 enfermedades raras que cursan con discapacidad intelectual en el contexto colombiano, como son: el síndrome de Rett, el síndrome de Prader-Willi y el síndrome de X frágil. En cada una de estas enfermedades además se explicaron los tipos de diagnóstico, intervención, prevención, grupos de apoyo y tipos de evaluación que más se usan en el contexto nacional

The study and preservation of museum collections requires complete knowledge and understanding of constituent materials that can be natural, synthetic, or semi-synthetic polymers. In former times, objects were incorporated in museum collections and classified solely by their appearance. New studies, prompted by severe degradation processes or conservation-restoration actions, help shed light on the materiality of objects that can contradict the original information or assumptions. The selected case study presented here is of a box dating from the beginning of the 20th century that belongs to the Portuguese National Ancient Art Museum. Museum curators classified it as a tortoiseshell box decorated with gold applications solely on the basis of visual inspection and the information provided by the donor. This box has visible signs of degradation with white veils, initially assumed to be the result of biological degradation of a proteinaceous matrix. This paper presents the methodological rationale behind this study and proposes a totally non-invasive methodology for the identification of polymeric materials in museum artifacts. The analysis of surface leachates using 1H and 13C nuclear magnetic resonance (NMR) complemented by in situ attenuated total reflection infrared spectroscopy (ATR FT-IR) allowed for full characterization of the object s substratum. The NMR technique unequivocally identified a great number of additives and ATR FT-IR provided information about the polymer structure and while also confirming the presence of additives. The pressure applied during ATR FT-IR spectroscopy did not cause any physical change in the structure of the material at the level of the surface (e.g., color, texture, brightness, etc.). In this study, variable pressure scanning electron microscopy (VP-SEM-EDS) was also used to obtain the elemental composition of the metallic decorations. Additionally, microbiologic and enzymatic assays were performed in order to identify the possible biofilm composition and understand the role of microorganisms in the biodeterioration process. Using these methodologies, the box was correctly identified as being made of cellulose acetate plastic with brass decorations and the white film was identified as being composed mainly of polymer exudates, namely sulphonamides and triphenyl phosphate.

Estima-se que aproximadamente 30 milhões de brasileiros apresentem hipertensão arterial e a despeito da grande quantidade de hipotensores disponíveis, acredita-se que apenas 2,7 milhões estejam sendo tratados adequadamente. Recentemente vários estudos clínicos, epidemiológicos e experimentais têm mostrado uma associação entre o consumo de alimentos ricos em cacau e a redução da pressão arterial assim como relacionando este efeito a uma possível ação dos flavonóides do cacau sobre a função endotelial. Objetivos: avaliar em pacientes hipertensos primários, estágio 1, o efeito da administração dos flavonóides do chocolate amargo 70% de cacau sobre: a pressão arterial; a função endotelial e as possíveis correlações entre as variações da pressão arterial e da função endotelial. Tipo de estudo: experimental, clínico e aberto. Casuística: 20 pacientes, sem distinção de raça ou sexo, com hipertensão arterial primária no estágio 1, sem tratamento anti-hipertensivo prévio, eutróficos, com sobrepeso ou obesos grau I, com idades entre 18 e 60 anos. Local do estudo: Disciplina de Fisiopatologia Clínica e Experimental Clinex. Universidade do Estado do Rio de Janeiro. Variáveis estudadas: pressão arterial, PCR-US, IL-6, TNF-α, VCAM, ICAM, E-selectina, LDL-OX, colesterol total, LDL-colesterol, HDL-colesterol, triglicérides, glicemia, insulina, HOMA, índice de massa corporal, circunferência de cintura, circunferência de quadril, relação cintura quadril e percentual de gordura corporal. Resultados: o chocolate-cacau 70% reduziu de forma significativa a pressão arterial avaliada pelo método oscilométrico casual. Através deste método observamos que a pressão arterial sistólica reduziu de forma significativa após 4 semanas de tratamento, (V0: 146,50 1,28; V1: 140,40 3,02; V2: 138,50 2,44; V3: 140,60 2,50; V4: 136,90 2,60; V4 vs. V0, p<0,001) enquanto a pressão arterial diastólica apresentou redução significativa a partir de 2 semanas de tratamento e assim permanecendo até o final do estudo (V0: 93,2 0,74; V1: 87,50 1,8; V2: 86,05 1,67; V3: 88,35 1,48; V4: 87,45 1,78; V2 vs. V0, p< 0,05 e V4 vs. V0, p<0,03). A pressão arterial avaliada pelo método de monitorização ambulatorial da pressão arterial durante 24h (MAPA) não modificou de maneira significativa após a intervenção. Observamos reduções expressivas, embora não estatisticamente significativas nas concentrações de PCR-US, TNF-α, LDL-OX, IL-6, VCAM, ICAM e E-selectina As correlações da PCR-US com IL-6 e ICAM foram significativas (r=0,3; p=0,05 e r=0.45, p=0,04) e de IL-6 com ICAM forte mas sem significância (r=0,42, p=0,06). As demais variáveis avaliadas não se modificaram de forma significativa após 4 semanas de consumo de chocolate-cacau 70%. Conclusões: os resultados do presente estudo sugerem que o chocolate-cacau 70% tem efeito benéfico sobre a função endotelial e controverso em relação ao comportamento da pressão arterial
