Background: The incidence and outcome of Herpes zoster (HZ) in systemic lupus erythematosus (SLE) are not completely defined as well as the relevance to HZ of disease and therapy factors. Objective: To determine HZ features in SLE. Patients and Methods: SLE patients ( 1997 update of the American College of Rheumatology classification criteria) with definitive HZ infection were identified from our Lupus Clinic computerized database of 1145 patients. Results: HZ was diagnosed in 51 SLE patients (4.45%) with an annual incidence rate of 6.4 events/1000 patient-years. At HZ diagnosis, mean disease duration was 9.78 +/- 8.37 years, median Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) was 1, and only 17.6% had SLEDAI >= 8. Frequency of manifestations and immunosuppressor use were similar between patients with and without HZ. Forty-two patients (82.5%) with HZ were under prednisone with concomitant immunosuppressive therapy in 66.7%. Thirty-five patients (68.6%) were using immunosuppressors: azathioprine (39.2%), cyclophosphamide (9.8%), and mycophenolate mofetil ( 9.8%). The mean lymphocyte count was 1219 +/- 803/mm(3) (43.1% < 1000/mm(3) and 17.6% < 500/mm(3)). Only patients using azathioprine and cyclophosphamide had lymphocyte counts < 500/mm(3) (15% and 40%). All patients received acyclovir, 19.6% had postherpetic neuralgia, and recurrence occurred in only 7.8%. Thoracic nerves were the most involved site (56.8%) followed by lumbar (23.5%). Bacterial suprainfection occurred in 11.7% but was not associated with therapy, lymphocyte count, or SLEDAI scores ( P > 0.05). Conclusion: This is the largest cohort to determine that HZ is a late SLE complication with some peculiar features, such as good prognosis and typical dermatomal distribution. In addition, we have identified that the major trigger factor for this viral infection in SLE is therapy, particularly the concomitant use of corticosteroid and immunosuppressors, and not active disease.

Introduction. Chagas` disease is endemic in South America. Objective. This research reviewed the experience with cardiac transplantation in Chagas` disease, emphasizing reactivation, immunosuppression, and mortality. Methods. Over 25 years from March 1985 to March 2010, 107/409 (26.2%) patients with Chagas` disease underwent heart transplantation, patients including 74 (71.1%) men and 72 (67.2%), in functional class IV with 33 (30.8%) on vasopressors and 17 (10.7%) on mechanical circulatory support. Results. The diagnosis of disease reactivation was performed by identifying the parasite in the myocardium (n = 23; 71.8%) in the subcutaneous tissue (n = 8; 25.0%), in blood (n = 11; 34.3%), or in central nervous tissue (n = 1; 3.1%). Hospital mortality was 17.7% (n = 19) due to infection (n = 6; 31.5%), graft dysfunction (n = 6; 31.5%), rejection (n 4; 21.1%), or sudden death (n = 2; 10.5%). Late mortality was 27 (25.2%) cases, which were distributed as: rejection (n = 6; 22.2%), infection (n = 6; 22.2%), (n = lymphoma 4; 14.8%), sarcoma (n = 2; 7.4%), for constrictive pericarditis (n = 2; 7.4%) reactivation of Chagas` disease in the central nervous system (n = 1; 7.1%). Conclusions. Transplantation in Chagas` disease has peculiar problems that differ from other etiologies due to the possibility of disease reactivation and the increased possibility of emergence of cancers. However, transplantation is the only treatment able to modify the natural progression of the disease in its terminal phase. Early diagnosis and rapid introduction of benzonidazole reverses the histological patterns. Immunosuppression, especially steroids, predisposes to the development of cancer and disease reactivation.

Background-Peculiar aspects of Chagas cardiomyopathy raise concerns about efficacy and safety of sympathetic blockade. We studied the influence of beta-blockers in patients with Chagas cardiomyopathy. Methods and Results-We examined REMADHE trial and grouped patients according to etiology (Chagas versus non-Chagas) and beta-blocker therapy. Primary end point was all-cause mortality or heart transplantation. Altogether 456 patients were studied; 27 (5.9%) were submitted to heart transplantation and 202 (44.3%) died. Chagas etiology was present in 68 (14.9%) patients; they had lower body mass index (24.1+/-4.1 versus 26.3+/-5.1, P=0.001), smaller end-diastolic left ventricle diameter (6.7+/-1.0 mm versus 7.0+/-0.9 mm, P=0.001), smaller proportion of beta-blocker therapy (35.8% versus 68%, P<0.001), and higher proportion of spironolactone therapy (74.6% versus 57.8%, P=0.003). Twenty-four (35.8%) patients with Chagas disease were under beta-blocker therapy and had lower serum sodium (136.6+/-3.1 versus 138.4+/-3.1 mEqs, P=0.05) and lower body mass index (22.5+/-3.3 versus 24.9+/-4.3, P=0.03) compared with those who received beta-blockers. Survival was lower in patients with Chagas heart disease as compared with other etiologies. When only patients under beta-blockers were considered, the survival of patients with Chagas disease was similar to that of other etiologies. The survival of patients with beta-blockers was higher than that of patients without beta-blockers. In Cox regression model, left ventricle end-diastolic diameter (hazard ratio, 1.78; CI, 1.15 to 2.76; P=0.009) and beta-blockers (hazard ratio, 0.37; CI, 0.14 to 0.97; P=0.044) were associated with better survival. Conclusions-Our study suggests that beta-blockers may have beneficial effects on survival of patients with heart failure and Chagas heart disease and warrants further investigation in a prospective, randomized trial.

The Edinburgh-Cape Blue Object Survey is a major survey to discover blue stellar objects brighter than B similar to 18 in the southern sky. It is planned to cover an area of sky of 10 000 deg(2) with \b\ > 30 degrees and delta < 0 degrees. The blue stellar objects are selected by automatic techniques from U and B pairs of UK Schmidt Telescope plates scanned with the COSMOS measuring machine. Follow-up photometry and spectroscopy are being obtained with the SAAO telescopes to classify objects brighter than B = 16.5. This paper describes the survey, the techniques used to extract the blue stellar objects, the photometric methods and accuracy, the spectroscopic classification, and the limits and completeness of the survey.

Kabuki syndrome is a genetic disorder of unknown etiology characterized by mental retardation, growth deficiency, and peculiar face (i.e., long palpebral fissures, eversion of the lateral third of the lower eyelids, prominent ears, and broad and depressed nasal tip). Oral manifestations commonly observed in Kabuki syndrome may comprise cleft lip/palate, bifid tongue and uvula, malocclusion, and dental abnormalities. We evaluated the dental findings of eight patients with Kabuki syndrome. One presented cleft palate; three presented caries; and seven had missing teeth, with the upper lateral incisors and inferior central incisors being the most commonly absent. All missing teeth were permanent, and there was no alteration of dental chronology or morphology. Because most patients had mixed dentition, the presence or absence of primary teeth was assessed through the parents` reports. One patient presented an absent upper canine, which had not been reported previously in the literature. Dental findings may be helpful for clinical diagnosis, or they may be an additional finding to substantiate the diagnosis of Kabuki syndrome in children with mild phenotype.

Here we investigated the possible association between the carboxypeptidase A (CPA)-like activity of the rat mesenteric arterial bed (MAB) perfusate and the ability of this fluid of forming angiotensin (Ang) 1-9 and Ang 1-7 upon incubation with Ang I and Ang II, respectively. Initially, we observed that anion exchange chromatography of the perfusate would consistently split the characteristic Z-Val-Phe-hydrolyzing activity of CPA-like enzymes into five distinct peaks, whose proteolytic activities were then determined using also Ang I and Ang II as substrates. The resulting proteolytic profile for each peak indicated that rat MAB perfusate contains a complex mixture of carboxypeptidases; tentatively, five carboxypeptidases were distinguished based on their substrate preferences toward Z-Val-Phe. Ang I and Ang II. The respective reactions, namely, Z-Val-Phe cleavage, Ang I to Ang 1-9 conversion and Ang II to Ang 1-7 conversion, were inhibited by 1,10-phenanthroline and nearly fully blocked by potato carboxypeptidase inhibitor. Also, all the CPA-like activity peaks prepared by anion exchange chromatography were tested negative for contaminating Ang I-converting enzyme-2, cathepsin A and prolylcarboxypeptidase. Overall, our results indicate that rat MAB perfusate contains a multiplicity of Ang I and Ang II-processing CPA-like enzymes whose proteolytic specificities suggest they might perform peculiar regulatory roles in the local resin-angiotensin system. Leprosy is a chronic infection caused by Mycobacterium leprae, a bacillus that presents a peculiar tropism for the skin and peripheral nerves. The clinical spectrum of leprosy ranges from the tuberculoid form (TT) to the disseminative and progressive lepromatous form (LL). Oral lesions are rare but, when present, occur in the lepromatous form. This article describes the clinical and microscopic findings of three cases of LL with oral manifestations. All patients had the lepromatous form and their leprosy-specific oral lesions occurred in the palate. The diagnosis was based on clinical, serological and histopathological findings, and multidrug therapy for multibacillary leprosy was started and continued for 24 months. All patients completed treatment, but developed reaction episodes which were treated with prednisone and/or thalidomide. The authors emphasize the importance of oral mucosa evaluation by a dental health professional during patient care since oral lesions may act as a source of infection.

We describe the long-term clinical outcome of a patient with Leigh-like syndrome presenting as an early onset encephalopathy and peripheral neuropathy caused by the T8993G mutation in the mitochondrial DNA (mtDNA). Clinical follow-up for 20 years revealed a peculiar pattern of slow disease progression, characterized by the addition of new minor deficits, while worsening of previous symptoms was mild. Brain MRI revealed cerebellar atrophy, diffuse demyelination of corona radiata and parietal white matter, and bilateral and symmetrical putaminal lesions. The proportion of mutant mtDNAs in blood was 72% (+/- 0.02%) and in skeletal muscle was 81% (+/- 0.4%). Leigh-like syndrome caused by the T8993G mtDNA mutation is a progressive disease, although not necessarily associated with an aggressive clinical course. (C) 2009 Elsevier B.V. All rights reserved.

The possibility of obtaining transplantable oral epithelia opens new perspectives for oral treatments. Most of them are surgical, resulting in mucosal failures. As reconstructive material this in vitro epithelia would be also useful for other parts of the human body. Many researchers still use controversial methods; therefore it was evaluated and compared the efficiency of the enzymatic and direct explant methods to obtain oral keratinocytes. To this project oral epithelia fragments were used. This work compared: time needed for cell obtainment, best cell amount, life-span and epithelia forming cell capacity. The results showed the possibility to obtain keratinocytes from a small oral fragment and we could verify the advantages and peculiar restrictions. We concluded that under our conditions the enzymatic method showed the best results: in the cells obtaining time needed, cell amount and life-span. Both methods showed the same capacity to form in vitro epithelia.

Sperm ultrastructure is described for the nudibranch gastropod Cadlinella ornatissima, type species of the genus Cadlinella (Thiele). Although C. ornatissima exhibits most of the sperm features characteristic of other Opisthobranchia and the Pulmonata (a small, rounded acrosomal vesicle, a complex, helical, mitochondrial derivative - partially paracrystalline, coarse fibres associated with the axoneme), it also possesses a number of previously undescribed and possibly unique features (a longitudinally inrolled acrosomal pedestal, an axial structure within the cavity of the acrosomal pedestal, an electron-dense collar at the anterior region of the acrosomal pedestal, the presence of crystalloid bodies within the glycogen helices of the mitochondrial derivative). To our knowledge this is the first report of crystalloid bodies in mature sperm of any mollusc. Collectively this evidence raises questions concerning the affinities and systematic position of Cadlinella within the Nudibranchia. The peculiar nature of the sperm differences, in comparison with other investigated nudibranchs, suggest that Cadlinella is not easily linked to either the Cadlinidae or Chromodorididae, and should be considered incertae sedis.

Cropp and Gabric [Ecosystem adaptation: do ecosystems maximise resilience? Ecology. In press] used a simple phytoplanktonzooplankton-nutrient model and a genetic algorithm to determine the parameter values that would maximize the value of certain goal functions. These goal functions were to maximize biomass, maximize flux, maximize flux to biomass ratio, and maximize resilience. It was found that maximizing goal functions maximized resilience. The objective of this study was to investigate whether the Cropp and Gabric [Ecosystem adaptation: do ecosystems maximise resilience? Ecology. In press] result was indicative of a general ecosystem principle, or peculiar to the model and parameter ranges used. This study successfully replicated the Cropp and Gabric [Ecosystem adaptation: do ecosystems maximise resilience? Ecology. In press] experiment for a number of different model types, however, a different interpretation of the results is made. A new metric, concordance, was devised to describe the agreement between goal functions. It was found that resilience has the highest concordance of all goal functions trialled. for most model types. This implies that resilience offers a compromise between the established ecological goal functions. The parameter value range used is found to affect the parameter versus goal function relationships. Local maxima and minima affected the relationship between parameters and goal functions, and between goal functions. (C) 2003 Elsevier B.V. All rights reserved.

Este estudo traz à tona nossa intenção de pesquisa: Compreender a história de vida de um sujeito com a Síndrome de Klinefelter, o Ramon. A Síndrome de Klinefelter é muito peculiar e vem ganhando destaque na área médica, não por sua prevalência na população, mas por sua complexidade. Na área da educação, a produção acerca dessa síndrome é incipiente encontramos apenas um estudo em Portugal. No nosso país, no entanto, ressalta-se o ineditismo dessa pesquisa. Nesse estudo pensamos, juntamente com Lev Semionovich Vigotski e outros autores que imprimem em seus textos raízes sócio-históricas, num Ramon para além do biológico, ou seja, para além dos seus limites orgânicos: um sujeito rico em subjetividade que foi valorizada. Objetivamos, assim, compreender a subjetividade desse sujeito que é singular na coletividade, assim, pensar no Ramon é pensar nos outros, nos seus pares e nas intrínsecas redes de dialogismos tecidas na imensa trama que é a vida. Buscamos também observar como ocorreu a inclusão desse sujeito no âmbito escolar. A fim de atingir os objetivos traçados, utilizamos a perspectiva histórico-cultural do desenvolvimento humano atrelando os pressupostos dessa opção teórica à metodologia história de vida. Para entender os detalhes, os indícios, as miudezas, os resquícios da história de vida do sujeito que parecem insignificantes, mas que são imprescindíveis para se compreender alguns processos de grande dimensão, nos apoiamos no paradigma indiciário de Ginzburg. Nessa pesquisa, nossos encontros com esse jovem de 22 anos são descritos e analisados, levando em conta os aspectos subjetivos. Nesses encontros, ouvimos várias narrativas para compor a história de vida do sujeito pesquisado: a do próprio Ramon, da sua mãe Marlene e das professoras da APAE e da Educação de Jovens e Adultos, ouvimos, portanto, Ramon em diferentes contextos: em casa, na APAE, na escola de ensino comum (EJA) e na casa da professora da APAE. Para colher os dados, recorremos às entrevistas biográficas semiestruturadas, as quais foram adequadas à singularidade de cada sujeito ouvido. Todas as entrevistas foram gravadas e transcritas em sua totalidade e os dados obtidos foram analisados levando-se em conta o contexto histórico e social de Ramon, assim, observamos as relações dialógicas estabelecidas por Ramon com seus pares e tentamos compreendê-las para melhor entender a construção subjetiva desse sujeito que para além de biológico, é social, cultural, que aprende, apreende e que, para além disso...muito nos ensina!

Realizamos uma análise da atividade da Consciência Infeliz (unglücklichesBewusstsein), tal como foi exposta por G.W.F. Hegel em sua obraFenomenologia do Espírito(PhänomenologiedesGeistes), de 1807.A Consciência Infeliz é uma denominação hegeliana referente a uma consciência religiosa que se cinde em duas; um destes seus lados, ela aliena de si e tem por sua essência que reside no além, o Imutável; ao outro lado, ela mesma, assevera como o Mutável, inessente, residente no aquém. Toda a sua atividade resume-se a unir isto que ela põe como o infinitamente desunido, a saber, ela e sua essência, pois a consciência ainda não é ciente de que esta essência absoluta que ela opôs a si mesma nada mais é do que ela mesma. Isto resulta num tender singular para seu objeto Universal absoluto e ao mesmo tempo não querer maculá-lo com esta sua singularidade; numa atividade que deve absolutamente ser e não-ser, busca de algo que não pode nem deve ser buscado. Enquanto herdeira do pensamento estóico e cético, a Consciência Infeliz aparece como consciência contraditória, curvada sobre si mesma e sempre dolorida, que além de efetivar um movimento de negação para com o mundo do aquém e tudo o que lhe diz respeito, busca se libertar da dor que é ser portadora desta contraditoriedade que surge justamente daquela sua atitude negativa. A fim de que possamos fundamentar esta atitude Infeliz, realizamos em nosso primeiro capítulo uma investigação acerca de suas características peculiares nas esferas anteriores ao seu aparecimento, a saber, a esfera do Entendimento (Verstand), a dialética do Senhor e do Escravo e do Estoicismo e Ceticismo. No segundo capítulo, discorremos acerca do conceito e da atividade da Consciência Infeliz, bem como procuramos investigar a necessidade de sua superação a partir de uma análise de sua suprassunção no momento da Razão (Vernunft). Por fim, em nosso terceiro e último capítulo, procuramos refletir sobre em que medida se poderia afirmar que as consciências contemporâneas continuam agindo de maneira infeliz, e para tanto, nos apoiamos em breves leituras de S. Freud, pensador do ―mal-estar‖ moderno e Z.Bauman, pensador do ―mal-estar‖ contemporâneo.

No âmbito da Democracia Participativa, cujos elementos foram inseridos na democracia brasileira pela Constituição Federal de 1988 (CF/88), ao legitimar a participação do usuário na administração pública direta e indireta, prevendo mecanismos de controle social, destacamos as ouvidorias públicas, e dentre elas as ouvidorias universitárias, como instrumentos de interlocução e intermediação entre os cidadãos e as instituições públicas. Embora existam importantes estudos acerca das ouvidorias públicas, sobretudo nas áreas da saúde e judiciária, ainda há escassez no campo da gestão pública quanto a estudos específicos sobre as ouvidorias universitárias, restando uma lacuna que necessita ser preenchida. Nesse sentido, a presente pesquisa teve o propósito de investigar a atuação da Ouvidoria Geral da Universidade Federal do Espírito Santo (UFES), buscando responder à seguinte questão: Qual a importância da Ouvidoria Geral da UFES na gestão pública dessa instituição? E, com isso, trazer à tona aspectos relevantes e peculiares das ouvidorias universitárias, que certamente alcançam, a exemplo da Ouvidoria Geral da UFES, ouvidorias de outras instituições públicas de ensino superior, dando maior visibilidade a um instituto por vezes desconhecido ou tratado sem a atenção devida. Para tanto, foi realizada uma pesquisa qualitativa a partir do procedimento técnico do estudo de caso, com utilização de análise documental, observação espontânea e aplicação de entrevistas. Assim, além de abordar aspectos estruturais e conjunturais que envolvem a Ouvidoria, esse estudo traz a percepção de cidadãos que utilizaram os serviços do órgão, propiciando a ampliação da discussão sobre o tema na Academia e, porventura, a indução de mudanças organizacionais.
