Background: Excessive consumption of energy is a decisive factor of obesity, but a simple quantitative assessment of consumption between obese and eutrophic individuals not always explains the problem, raising questions about the importance of the qualitative aspects of food. Therefore, the purpose of this study was to evaluate the differences in nutrient composition and meal patterns between eutrophic and obese schoolchildren. Methods. The diet of 83 children (42 obese and 41 eutrophic), aged between 7 and 11 years of age, was assessed by two non-consecutive dietary recalls. After the software analysis of macro and micronutrients composition, the different types and amount of legumes, fruits and vegetables were analyzed to verify the dietary patterns. Results: No differences were verified in energy consumption between the groups (eutrophic = 1934.2 672.7 kcal, obese = 1835.8 621.2 kcal). In general, children showed consumption within the recommended ranges of carbohydrates, lipids and proteins. The average consumption of fiber was higher in the eutrophic group (20.7 g) when compared to the obese group (14.8 g). The dietary fiber was strongly correlated with the number of servings of beans (r = 0.77), when compared to fruits (r = 0.44) and leafy vegetables (r = 0.13). It was also observed that the higher the consumption of fiber and beans, the lower the proportion of dietary fat (r = -0.22) in the diet. Generally, there was a low consumption of fiber (20.7 g = eutrophic group/14.8 g = obese group), beans (1.1 portions in the eutrophic and obese groups), fruits (0.7 portions eutrophic group and 0.6 obese group) and vegetables (1.3 eutrophic group and 1.1 obese group). Conclusions: It is concluded that the obesity was more related to a dietary pattern of low intake of dietary fiber than excessive energy consumption and macronutrients imbalance. © 2011 de Oliveira et al; licensee BioMed Central Ltd.

BACKGROUND: Brazilian blood centers ask candidate blood donors about the number of sexual partners in the past 12 months. Candidates who report a number over the limit are deferred. We studied the implications of this practice on blood safety. STUDY DESIGN AND METHODS: We analyzed demographic characteristics, number of heterosexual partners, and disease marker rates among 689,868 donations from three Brazilian centers between July 2007 and December 2009. Donors were grouped based on maximum number of partners allowed in the past 12 months for each center. Chi-square and logistic regression analysis were conducted to examine associations between demographic characteristics, number of sex partners, and individual and overall positive markers rates for human immunodeficiency virus (HIV), human T-lymphotropic virus Types 1 and 2, hepatitis B virus, hepatitis C virus, and syphilis. RESULTS: First-time, younger, and more educated donors were associated with a higher number of recent sexual partners, as was male sex in Sao Paulo and Recife (p < 0.001). Serologic markers for HIV and syphilis and overall were associated with multiple partners in Sao Paulo and Recife (p < 0.001), but not in Belo Horizonte (p = 0.05, p = 0.94, and p = 0.75, respectively). In logistic regression analysis, number of recent sexual partners was associated with positive serologic markers (adjusted odds ratio [AOR], 1.2-1.5), especially HIV (AOR, 1.9-4.4). CONCLUSIONS: Number of recent heterosexual partners was associated with HIV positivity and overall rates of serologic markers of sexually transmitted infections. The association was not consistent across centers, making it difficult to define the best cutoff value. These findings suggest the use of recent heterosexual contacts as a potentially important deferral criterion to improve blood safety in Brazil.

Background: This study is the first to investigate the Brazilian Amazonian Forest to identify new D-xylose-fermenting yeasts that might potentially be used in the production of ethanol from sugarcane bagasse hemicellulosic hydrolysates. Methodology/Principal Findings: A total of 224 yeast strains were isolated from rotting wood samples collected in two Amazonian forest reserve sites. These samples were cultured in yeast nitrogen base (YNB)-D-xylose or YNB-xylan media. Candida tropicalis, Asterotremella humicola, Candida boidinii and Debaryomyces hansenii were the most frequently isolated yeasts. Among D-xylose-fermenting yeasts, six strains of Spathaspora passalidarum, two of Scheffersomyces stipitis, and representatives of five new species were identified. The new species included Candida amazonensis of the Scheffersomyces clade and Spathaspora sp. 1, Spathaspora sp. 2, Spathaspora sp. 3, and Candida sp. 1 of the Spathaspora clade. In fermentation assays using D-xylose (50 g/L) culture medium, S. passalidarum strains showed the highest ethanol yields (0.31 g/g to 0.37 g/g) and productivities (0.62 g/L.h to 0.75 g/L.h). Candida amazonensis exhibited a virtually complete D-xylose consumption and the highest xylitol yields (0.55 g/g to 0.59 g/g), with concentrations up to 25.2 g/L. The new Spathaspora species produced ethanol and/or xylitol in different concentrations as the main fermentation products. In sugarcane bagasse hemicellulosic fermentation assays, S. stipitis UFMG-XMD-15.2 generated the highest ethanol yield (0.34 g/g) and productivity (0.2 g/L.h), while the new species Spathaspora sp. 1 UFMG-XMD-16.2 and Spathaspora sp. 2 UFMG-XMD-23.2 were very good xylitol producers. Conclusions/Significance: This study demonstrates the promise of using new D-xylose-fermenting yeast strains from the Brazilian Amazonian Forest for ethanol or xylitol production from sugarcane bagasse hemicellulosic hydrolysates.

Viruses are the major cause of lower respiratory tract infections in childhood and the main viruses involved are Human Respiratory Syncytial Virus (HRSV), Human Metapneumovirus (HMPV), Influenzavirus A and B (FLUA and FLUB), Human Parainfluenza Virus 1, 2 and 3 (HPIV1, 2 and 3) and Human Rhinovirus (HRV). The purposes of this study were to detect respiratory viruses in hospitalized children younger than six years and identify the influence of temperature and relative air humidity on the detected viruses. Samples of nasopharyngeal washes were collected from hospitalized children between May/2004 and September/2005. Methods of viral detection were RT-PCR, PCR and HRV amplicons were confirmed by hybridization. Results showed 54% (148/272) of viral positivity. HRSV was detected in 29% (79/272) of the samples; HRV in 23.1% (63/272); HPIV3 in 5.1% (14/272); HMPV in 3.3% (9/272); HPIV1 in 2.9% (8/272); FLUB in 1.4% (4/272), FLUA in 1.1% (3/272), and HPIV2 in 0.3% (1/272). The highest detection rates occurred mainly in the spring 2004 and in the autumn 2005. It was observed that viral respiratory infections tend to increase as the relative air humidity decreases, showing significant association with monthly averages of minimal temperature and minimal relative air humidity. In conclusion, viral respiratory infections vary according to temperature and relative air humidity and viral respiratory infections present major incidences it coldest and driest periods.

PURPOSE Fixation of anterior cruciate ligament (ACL) substitutes with non-physiological anteroposterior translation (APT) worsens outcome. The aim was to present a technique for physiological APT adjustment of the transplant in ACL reconstruction and its outcome at midterm. METHODS In a consecutive series of 28 patients (age 32 ± 11 years, 24 male), chronic ACL deficiency was treated by bone-patella-tendon-bone reconstruction. Transplant APT was adjusted to that of the contralateral uninjured ACL, measured 3, 6, and 12 months postoperatively using the Rolimeter. At a median follow-up of 5.3 years (3-8 years), 82% of the patients were re-evaluated with APT measurement and using IKDC-, Tegner-, Lysholm-Scores, conventional radiographs and MRI. RESULTS No differences in APT (mean ± SD) between uninjured and reconstructed knees were observed after adjustment (6 ± 1 versus 6 ± 1 mm, n.s.). Three months postoperatively, a statistically significant increase in APT (7 ± 1 mm) and a further increase at midterm (9 ± 2 mm) were observed. Patients scored "normal" or "nearly normal", respectively, in 79% (IKDC) and 4 (3-9) points (Tegner; median, range) or 89 ± 9 points (Lysholm; mean ± SD). Radiological evaluation showed no, minimal or moderate joint degeneration in 5, 20 and 75% of patients, respectively. MRI confirmed intact ACL transplants in all patients. CONCLUSION ACL reconstruction using the presented technique was considered successful, as patients did not suffer from subjective instability, radiographic analysis did not provide evidence for graft rupture at midterm. However, APT increase and occurrence of degenerative changes in reconstructed knees at the midterm might not be prevented even by restoration of a physiological APT in ACL reconstruction. The Rolimeter can be used for quick and easy intraoperative indirect control of the applied tension to the ACL transplant by measuring the APT to obtain physiological tensioning resulting in a satisfying outcome at midterm. The European Eye Epidemiology (E3) consortium is a recently formed consortium of 29 groups from 12 European countries. It already comprises 21 population-based studies and 20 other studies (case-control, cases only, randomized trials), providing ophthalmological data on approximately 170,000 European participants. The aim of the consortium is to promote and sustain collaboration and sharing of data and knowledge in the field of ophthalmic epidemiology in Europe, with particular focus on the harmonization of methods for future research, estimation and projection of frequency and impact of visual outcomes in European populations (including temporal trends and European subregions), identification of risk factors and pathways for eye diseases (lifestyle, vascular and metabolic factors, genetics, epigenetics and biomarkers) and development and validation of prediction models for eye diseases. Coordinating these existing data will allow a detailed study of the risk factors and consequences of eye diseases and visual impairment, including study of international geographical variation which is not possible in individual studies. It is expected that collaborative work on these existing data will provide additional knowledge, despite the fact that the risk factors and the methods for collecting them differ somewhat among the participating studies. Most studies also include biobanks of various biological samples, which will enable identification of biomarkers to detect and predict occurrence and progression of eye diseases. This article outlines the rationale of the consortium, its design and presents a summary of the methodology.

Opisthorchis viverrini is a food home trematode, important because of the sheer numbers of people infected and its serious morbidities such as hepatobiliary diseases and cholangiocarcinoma (CHCA). Although infections are identified throughout Southeast Asia, the epi-center is northeast Thailand, where high prevalence coexists with a high incidence of CHCA. We examined whether there are crosscultural differences in the magnitude of genetic and environmental contributions to risk of becoming a regular smoker and of persistence in smoking in men and women. Standard methods of epidemiologic and genetic analysis were applied to questionnaire data on history of cigarette use obtained from large samples of male and female like-sex twins from three different countries: Australia (N = 2284 pairs), Sweden (N = 8651 pairs), and Finland (N = 10,948 pairs). Samples were subdivided into three age groups (AG), 18-25 years, 26-35 years, and 36-46 years of age. The magnitude of genetic influence for lifetime smoking was found to be consistent across country and AG for women (46%) and men (57%), and estimates of the contribution from environmental influences shared by twin and co-twin could be equated across all countries by AG for the women (from youngest to oldest AG: 45%, 35%, and 26%), but not for men, with separate estimates obtained for the Scandinavian (33%, 29%, and 19%) and the Australian men (26%, 9%, and 11 %). There was no evidence for an important role for shared environmental influences on persistent smoking, and the genetic contribution was found to be consistent in magnitude in men and women, and the same across country and AG (52%). There are strong genetic influences on smoking behavior, and that risk of becoming a smoker (but not persistence in smoking) may be modified by experiences shared by twins that differ by AG and, at least for men, cultural background.

This paper evaluates three hypotheses about the relationship between cannabis use and psychosis in the light of recent evidence from prospective epidemiological studies. These are that: ( 1) cannabis use causes a psychotic disorder that would not have occurred in the absence of cannabis use; ( 2) that cannabis use may precipitate schizophrenia or exacerbate its symptoms; and ( 3) that cannabis use may exacerbate the symptoms of psychosis. There is limited support for the first hypothesis. As a consequence of recent prospective studies, there is now stronger support for the second hypothesis. Four recent prospective studies in three countries have found relationships between the frequency with which cannabis had been used and the risk of receiving a diagnosis of schizophrenia or of reporting psychotic symptoms. These relationships are stronger in people with a history of psychotic symptoms and they have persisted after adjustment for potentially confounding variables. The absence of any change in the incidence of schizophrenia during the three decades in which cannabis use in Australia has increased makes it unlikely that cannabis use can produce psychoses that would not have occurred in its absence. It seems more likely that cannabis use can precipitate schizophrenia in vulnerable individuals. There is also reasonable evidence for the third hypothesis that cannabis use exacerbates psychosis.

It has been proposed that long-term consumption of diets rich in non-digestible carbohydrates (NDCs), such as cereals, fruit and vegetables might protect against several chronic diseases, however, it has been difficult to fully establish their impact on health in epidemiology studies. The wide range properties of the different NDCs may dilution their impact when they are combined in one category for statistical comparisons in correlations or multivariate analysis. Several mechanisms have been suggested to explain the protective effects of NDCs, including increased stool bulk, dilution of carcinogens in the colonic lumen, reduced transit time, lowering pH, and bacterial fermentation to short chain fatty acids (SCFA) in the colon. However, it is very difficult to measure SCFA in humans in vivo with any accuracy, so epidemiological studies on the impact of SCFA are not feasible. Most studies use dietary fibre (DF) or Non-Starch Polysaccharides (NSP) intake to estimate the levels, but not all fibres or NSP are equally fermentable. The first aim of this thesis was the development of the equations used to estimate the amount of FC that reaches the human colon and is fermented fully to SCFA by the colonic bacteria. Therefore, several studies were examined for evidence to determine the different percentages of each type of NDCs that should be included in the final model, based on how much NDCs entered the colon intact and also to what extent they were fermented to SCFA in vivo. Our model equations are FC-DF or NSP$ 1: 100 % Soluble + 10 % insoluble + 100 % NDOs¥ + 5 % TS** FC-DF or NSP 2: 100 % Soluble + 50 % insoluble + 100 % NDOs + 5 % TS FC-DF* or NSP 3: 100 % Soluble + 10 % insoluble + 100 % NDOs + 10 % TS FC-DF or NSP 4: 100 % Soluble + 50 % insoluble + 100 % NDOs + 10 % TS *DF: Dietary fibre; **TS: Total starch; $NSP: non-starch polysaccharide; ¥NDOs: non-digestible oligosaccharide The second study of this thesis aimed to examine all four predicted FC-DF and FC-NSP equations developed, to estimate FC from dietary records against urinary colonic NDCs fermentation biomarkers. The main finding of a cross-sectional comparison of habitual diet with urinary excretion of SCFA products, showed weak but significant correlation between the 24 h urinary excretion of SCFA and acetate with the estimated FC-DF 4 and FC-NSP 4 when considering all of the study participants (n = 122). Similar correlations were observed with the data for valid participants (n = 78). It was also observed that FC-DF and FC-NSP had positive correlations with 24 h urinary acetate and SCFA compared with DF and NSP alone. Hence, it could be hypothesised that using the developed index to estimate FC in the diet form dietary records, might predict SCFA production in the colon in vivo in humans. The next study in this thesis aimed to validate the FC equations developed using in vitro models of small intestinal digestion and human colon fermentation. The main findings in these in vitro studies were that there were several strong agreements between the amounts of SCFA produced after actual in vitro fermentation of single fibre and different mixtures of NDCs, and those predicted by the estimated FC from our developed equation FC-DF 4. These results which demonstrated a strong relationship between SCFA production in vitro from a range of fermentations of single fibres and mixtures of NDCs and that from the predicted FC equation, support the use of the FC equation for estimation of FC from dietary records. Therefore, we can conclude that the newly developed predicted equations have been deemed a valid and practical tool to assess SCFA productions for in vitro fermentation.

Les troubles anxieux sont parmi les troubles psychiatriques les plus souvent diagnostiqués chez les adolescents. Ces troubles sont souvent accompagnés de nombreuses comorbidités, dont des difficultés de sommeil. L’objectif principal de cette thèse est de caractériser l’activité corticale à l’éveil et pendant le sommeil à l’aide de l’EEG quantifié chez une population d’adolescents présentant un trouble anxieux, et de la comparer à un groupe témoin d’adolescents. Dans un second temps, on cherche à savoir si l’activité EEG des patients anxieux corrèle avec différentes mesures cliniques. Deux études permettent de répondre à ces objectifs, une première portant sur l’activité EEG au cours de l’éveil, et une seconde portant sur l’activité EEG au cours du sommeil (SL et SP). La première étude démontre que l’activité EEG des deux groupes ne présente pas de différence à l’EEG le soir. Par contre, le matin, les patients anxieux présentent une activité significativement supérieure à celle des contrôles aux électrodes centrales (0,75-10 Hz et 13-20 Hz) ainsi qu’aux électrodes occipitales (2,5-7,75 Hz). Dans la seconde étude, nous avons analysé l’activité EEG absolue et relative en SL et en SP. Nous avons trouvé une activité absolue significativement supérieure à l’EEG de la région centrale chez les participants du groupe anxieux : en SLP (stades 3 et 4) sur l’ensemble des bandes de fréquence, en stade 2 sur les bandes de fréquence thêta, alpha et beta seulement. Finalement, en SP, les différences sont trouvées en alpha et beta, et non en thêta et delta. Les résultats obtenus à ces deux études suggèrent la présence de mécanismes de synchronisation et de filtrage inadéquats au niveau de la boucle thalamo-corticale, entraînant une hypervigilance du SNC. Quant aux corrélations entre l’activité EEG et les mesures cliniques, les résultats obtenus dans les deux études révèlent que les fréquences lentes (thêta et delta) de l’activité d’éveil le matin corrèlent à la fois avec l’anxiété de trait et d’état et les fréquences rapides (Alpha et Beta) de l’EEG du sommeil corrèlent sélectivement avec l’anxiété d’état. Il semble donc exister un lien entre les mesures cliniques et l’activité EEG. Une hausse d’activité EEG pourrait être un indicateur de la sévérité accrue des symptômes anxieux.

Les commotions cérébrales ont longtemps été considérées comme une blessure ne comportant que peu ou pas de conséquences. Cependant, la mise à la retraite forcée de plusieurs athlètes de haut niveau, liée au fait d'avoir subi des commotions cérébrales multiples, a porté cette question au premier plan de la culture scientifique et sportive. Malgré la sensibilisation croissante du public et la compréhension scientifique accrue des commotions cérébrales, il reste encore beaucoup d’inconnus au sujet de ces blessures. En effet, il est difficile de comprendre comment cette atteinte peut avoir des effets si profonds malgré le fait qu’elle n’entraîne apparemment pas de conséquences physiques apparentes lorsque les techniques traditionnelles d’imagerie cérébrale sont utilisées. Les techniques de neuroimagerie fonctionnelle ont cependant contribué à répondre aux nombreuses questions entourant les conséquences des commotions cérébrales ainsi qu'à accroître la compréhension générale de la physiopathologie de commotions cérébrales. Bien que les techniques de base telles que l'imagerie structurelle comme les scans TC et IRM soient incapables de détecter des changements structurels dans la grande majorité des cas (Ellemberg, Henry, Macciocchi, Guskiewicz, & Broglio, 2009; Johnston, Ptito, Chankowsky, & Chen, 2001), d'autres techniques plus précises et plus sensibles ont été en mesure de détecter avec succès des changements dans le cerveau commotionné. Des études d’IRM fonctionelle ont entre autres établi une solide relation entre les altérations fonctionnelles et les symptômes post-commotionels (Chen, Johnston, Collie, McCrory, & Ptito, 2007; Chen et al., 2004; Chen, Johnston, Petrides, & Ptito, 2008; Fazio, Lovell, Pardini, & Collins, 2007). Les mesures électrophysiologiques telles que les potentiels évoqués cognitifs (ERP) (Gaetz, Goodman, & Weinberg, 2000; Gaetz & Weinberg, 2000; Theriault, De Beaumont, Gosselin, Filipinni, & Lassonde, 2009; Theriault, De Beaumont, Tremblay, Lassonde, & Jolicoeur, 2010) et la stimulation magnétique transcrânienne ou SMT (De Beaumont, Brisson, Lassonde, & Jolicoeur, 2007; De Beaumont, Lassonde, Leclerc, & Theoret, 2007; De Beaumont et al., 2009) ont systématiquement démontré des altérations fonctionnelles chez les athlètes commotionnés. Cependant, très peu de recherches ont tenté d'explorer davantage certaines conséquences spécifiques des commotions cérébrales, entre autres sur les plans structural et métabolique. La première étude de cette thèse a évalué les changements structurels chez les athlètes commotionnés à l’aide de l'imagerie en tenseur de diffusion (DTI) qui mesure la diffusion de l'eau dans la matière blanche, permettant ainsi de visualiser des altérations des fibres nerveuses. Nous avons comparé les athlètes commotionnés à des athlètes de contrôle non-commotionnés quelques jours après la commotion et de nouveau six mois plus tard. Nos résultats indiquent un patron constant de diffusion accrue le long des voies cortico-spinales et dans la partie du corps calleux reliant les régions motrices. De plus, ces changements étaient encore présents six mois après la commotion, ce qui suggère que les effets de la commotion cérébrale persistent bien après la phase aiguë. Les deuxième et troisième études ont employé la spectroscopie par résonance magnétique afin d'étudier les changements neurométaboliques qui se produisent dans le cerveau commotionné. La première de ces études a évalué les changements neurométaboliques, les aspects neuropsychologiques, et la symptomatologie dans la phase aiguë post-commotion. Bien que les tests neuropsychologiques aient été incapables de démontrer des différences entre les athlètes commotionnés et non-commotionnés, des altérations neurométaboliques ont été notées dans le cortex préfrontal dorsolatéral ainsi que dans le cortex moteur primaire, lesquelles se sont avérées corréler avec les symptômes rapportés. La deuxième de ces études a comparé les changements neurométaboliques immédiatement après une commotion cérébrale et de nouveau six mois après l’atteinte. Les résultats ont démontré des altérations dans le cortex préfrontal dorsolatéral et moteur primaire dans la phase aiguë post-traumatique, mais seules les altérations du cortex moteur primaire ont persisté six mois après la commotion. Ces résultats indiquent que les commotions cérébrales peuvent affecter les propriétés physiques du cerveau, spécialement au niveau moteur. Il importe donc de mener davantage de recherches afin de mieux caractériser les effets moteurs des commotions cérébrales sur le plan fonctionnel.
