Aim: The aim of this study was to determine care home managers' knowledge of palliative care using the palliative care quiz for nursing (PCQN). Background: Palliative care is strongly advocated for all people living with advancing incurable illness. Within acare home setting there should be a particular emphasis on the importance of palliative care, particularly for those residents who, because of their advancing age, are likely to live with non-malignant diseases such as dementia, chronic obstructive pulmonary disease or heart failure to name a few. Methods: Before the beginning of a workshop on optimising palliative care for people living in care homes, 56 care home managers (all nurses) completed the PCQN, a validated questionnaire that is used to assess a nurse's knowledge of palliative care, as part of a learning exercise. Results: The quiz consisted of 20 questions for which participants could answer true, false or don't know. The average score was 12.89 correct answers out of a possible 20 (64.45%). Conclusion: This study highlights the need to develop the knowledge and competence of care home managers in relation to palliative care. This is particularly important given the increasing number of people who are living with non-malignant disease within a care home setting.

Published to coincide with the International Day of Older Persons, this viewpoint explores some of the findings from Adult Social Services Environments and Settings(ASSET), a research project that was funded by the NIHR School for Social Care Research from February 2012 to April 2014. The project explored how adult social care services are commissioned and delivered in extra care housing and retirement villages. It is a timely paper, coming hot off the heels of the recent Commission on Residential Care, chaired by former Care Services Minister, Paul Burstow MP. This recommended greater clarification on what constitutes housing with care.

O envelhecimento populacional é uma realidade mundial que altera a sociedade de forma complexa, implicando a necessidade de criação de estratégias de adaptação a esta realidade. A população idosa portuguesa apresenta também um aumento exponencial, e este fenómeno está frequentemente associado à perda de capacidades e dependência e ao aumento da incidência de patologias crónicas, como por exemplo as úlceras crónicas nos membros inferiores. Esta realidade verifica-se através de uma maior procura de cuidados de saúde e também no aumento dos encargos para a família e segurança social. Assim, a aquisição de conhecimentos sobre as implicações da úlcera crónica nos membros inferiores permite, aos profissionais de saúde, melhorar a prestação de cuidados às pessoas idosas, possibilitando a gestão eficaz de recursos e a melhoria da qualidade de vida dos utentes. Neste contexto, emergiu o nosso estudo que tem como objetivo conhecer as principais implicações da presença de uma úlcera crónica, nos membros inferiores, no quotidiano de pessoas idosas. A metodologia utilizada foi qualitativa, com um tipo de estudo exploratório-descritivo, em que foram realizadas 16 entrevistas a pessoas idosas portadoras de úlcera crónica nos membros inferiores e posteriormente analisadas as narrações de vivências ou experiências significativas dos participantes, utilizando as etapas metodológicas da análise de conteúdo segundo Bardin (2011). Os resultados encontrados foram incluídos em três áreas temáticas: Sentimentos e preocupações vividos com o aparecimento e desenvolvimento da úlcera crónica, Alterações no quotidiano das pessoas idosas com úlcera crónica e a Rede de apoio da pessoa idosa com úlcera crónica. A primeira área temática demostrou que as pessoas idosas apresentam sentimentos negativos de tristeza e dor em relação às suas vivências com a úlcera crónica, e receios futuros relacionados com a incerteza da evolução da úlcera, verificando-se alguma ambivalência entre a esperança e o desespero. As alterações no quotidiano verificaram-se na mobilidade física prejudicada, na interferência em atividades de vida diária e através da necessidade de tratamento. Na mobilidade física prejudicada foi o caminhar o mais mencionado pelos participantes e na interferência em atividades de vida diária foram as atividades domésticas, sociais e de lazer. A terceira área temática incluiu a rede de apoio da pessoa idosa com úlcera crónica identificando a família, a instituição e o convivente significativo como o principal apoio dos participantes. A família apresentou um papel de destaque, através do apoio prestado pelo cônjuge e pelos filhos. A realização deste estudo proporcionou conhecer melhor a realidade das pessoas idosas com úlceras crónicas nos membros inferiores, os seus sentimentos, dificuldades e/ou incapacidades, permitindo aos profissionais de saúde aumentar os conhecimentos e elaborar estratégias para auxiliar no seu dia-a-dia, ambicionando-se uma melhoria na prestação de cuidados às pessoas idosas, famílias e sociedade.

Einleitung: In Deutschland leiden derzeit etwa eine Million Menschen an einer Demenzerkrankung. Aufgrund der demografischen Entwicklung ist mit einem deutlichen Anstieg der Häufigkeit solcher Erkrankungen in den kommenden Jahren zu rechnen. Demenz ist in höherem Alter die häufigste Ursache von Pflegebedürftigkeit. Da diese Krankheiten in der Regel nicht heilbar sind, liegt der Fokus der Pflege auf der Verzögerung des Voranschreitens der Erkrankung sowie der Aufrechterhaltung von Funktionsfähigkeit und Lebensqualität der Betroffenen. Fragestellung: Wie ist die Evidenz für pflegerische Konzepte für Patienten mit Demenz hinsichtlich gebräuchlicher Endpunkte wie kognitive Funktionsfähigkeit, Fähigkeit zur Durchführung von Aktivitäten des täglichen Lebens, Lebensqualität, Sozialverhalten? Wie ist die Kosten-Effektivität der betrachteten Pflegekonzepte zu bewerten? Welche ethischen, sozialen oder juristischen Aspekte werden in diesem Kontext diskutiert? Methoden: Auf Basis einer systematischen Literaturrecherche werden randomisierte kontrollierte Studien (RCT) mit mindestens 30 Teilnehmern zu folgenden Pflegekonzepten eingeschlossen: Validation/emotionsorientierte Pflege, Ergotherapie, sensorische Stimulation, Entspannungsverfahren, Realitätsorientierung und Reminiszenz. Die Studien müssen ab 1997 (für den ökonomischen Teil ab 1990) in deutscher oder englischer Sprache publiziert worden sein. Ergebnisse: Insgesamt 20 Studien erfüllen die Einschlusskriterien. Davon befassen sich drei Studien mit der Validation/emotionsorientierte Pflege, fünf Studien mit der Ergotherapie, sieben Studien mit verschiedenen Varianten sensorischer Stimulation, je zwei Studien mit der Realitätsorientierung und der Reminiszenz und eine Studie mit einem Entspannungsverfahren. Keine signifikanten Unterschiede zwischen Interventions- und Kontrollgruppe berichten zwei von drei Studien zur Validation/emotionsorientierten Pflege, zwei von fünf Studien zur Ergotherapie, drei von sieben Studien zur sensorischen Stimulation, beide Studien zur Reminiszenz, und die Studie zur Entspannung. Von den verbleibenden zehn Studien berichten sieben teilweise positive Ergebnisse zugunsten der Intervention und drei Studien (Ergotherapie, Aromatherapie, Musik/Massage) berichten positive Effekte der Intervention hinsichtlich aller erhobenen Zielkriterien. Sechs Publikationen berichten ökonomische Ergebnisse von Pflegemaßnahmen. Eine Studie berichtet Zusatzkosten von 16 GBP (24,03 Euro (2006)) pro Patient pro Woche für Beschäftigungstherapie. Zwei weitere Veröffentlichungen geben inkrementelle Kosten von 24,30 USD (25,62 Euro (2006)) pro gewonnenen Mini-mental-state-examination-(MMSE)-Punkt pro Monat bzw. 1.380.000 ITL (506,21 Euro (2006)) pro gewonnenen MMSE-Punkt an. Zwei Publikationen berichten über Mischinterventionen, wobei einmal die Zusatzkosten für ein Aktivitätsprogramm (1,13 USD (1,39 Euro (2006)) pro Tag pro Pflegebedürftigem) und einmal der zeitliche Mehraufwand für die Betreuung mobiler Demenzpatienten (durchschnittlich 45 Minuten zusätzliche Pflegezeit pro Tag) berichtet wird. Hinsichtlich ethisch-sozialer Aspekte wird vor allem die Selbstbestimmung von Demenzpatienten diskutiert. Aus einer Demenzdiagnose lässt sich danach nicht zwingend schließen, dass die Betroffenen nicht eigenständig über eine Studienteilnahme entscheiden können. Im juristischen Bereich versucht die Regierung mit dem Pflege-Weiterentwicklungsgesetz (PfWG) die finanzielle Lage und die Betreuung der Pflegenden und Gepflegten zu verbessern. Weitere Fragestellungen rechtlicher Natur betreffen die Geschäftsfähigkeit bzw. die rechtliche Vertretung sowie die Deliktfähigkeit von an Demenz erkrankten Personen. Diskussion: Es gibt nur wenige methodisch angemessene Studien zu den in diesem Bericht berücksichtigten pflegerischen Konzepten für Demenzkranke. Die Studien haben überwiegend kleine Fallzahlen, und weisen erhebliche methodische Unterschiede hinsichtlich der Einschlusskriterien, der Durchführung, und der erfassten Zielkriterien auf. Diese Heterogenität zeigt sich auch in den Ergebnissen: in der Hälfte der eingeschlossen Studien gibt es keine positiven Effekte der Intervention im Vergleich zur Kontrollgruppe. Die andere Hälfte der Studien berichtet zum Teil positive Effekte bezüglicher unterschiedlicher Zielkriterien. Die ökonomischen Studien sind methodisch und thematisch nicht dazu geeignet die aufgeworfenen Fragestellungen zu beantworten. Ethische, soziale und juristische Aspekte werden diskutiert, aber nicht systematisch im Rahmen von Studien erfasst. Schlussfolgerung: Basierend auf der derzeitigen Studienlage liegt für keines der untersuchten Pflegekonzepte ausreichende Evidenz vor. Fehlende Evdienz bedeutet in diesem Kontext jedoch nicht zwingend fehlende Wirksamkeit. Vielmehr sind weitere Studien zu diesem Thema notwendig. Wünschenswert wären insbesondere Studien, die in Deutschland unter den Rahmenbedingungen des hiesigen Ausbildungs- und Pflegesystems durchgeführt werden. Dies gilt auch für die gesundheitsökonomische Bewertung der Pflege

This paper reports the results of a postal survey of intermediate care co-ordinators (ICCs) on the organization and delivery of intermediate care services for older people in England, conducted between November 2003 and May 2004. Questionnaires, which covered a range of issues with a variety of quantitative, ‘tick-box’ and open-ended questions, were returned by 106 respondents, representing just over 35% of primary care trusts (PCTs). We discuss the role of ICCs, the integration of local systems of intermediate care provision, and the form, function and model of delivery of services described by respondents. Using descriptive and statistical analysis of the responses, we highlight in particular the relationship between provision of admission avoidance and supported discharge, the availability of 24-hour care, and the locations in which care is provided, and relate our findings to the emerging evidence base for intermediate care, guidance on implementation from central government, and debate in the literature. Whilst the expansion and integration of intermediate care appear to be continuing apace, much provision seems concentrated in supported discharge services rather than acute admission avoidance, and particularly in residential forms of post-acute intermediate care. Supported discharge services tend to be found in residential settings, while admission avoidance provision tends to be non-residential in nature. Twenty-four hour care in non-residential settings is not available in several responding PCTs. These findings raise questions about the relationship between the implementation of intermediate care and the evidence for and aims of the policy as part of NHS modernization, and the extent to which intermediate care represents a genuinely novel approach to the care and rehabilitation of older people.

Introducción El motivo principal por el que acuden los pacientes a las consultas de podología es el dolor producido por los callos y callosidades plantares. El dolor producido por las callosidades y callos plantares provocan en el paciente cambios de presiones y alteraciones en el apoyo, dificultando la deambulación correcta. Existen numerosos estudios sobre la eliminación de callosidades en pacientes diabéticos, con AR, pero pocos en personas sanas. La eliminación de estos callos y callosidades se puede realizar mediante deslaminación mecánica con bisturí o mediante queratolíticos. Objetivos Objetivo principal Analizar el efecto de la deslaminación mecánica con bisturí de las callosidades y callos plantares sobre el dolor y la calidad de vida en sujetos sanos Objetivos secundarios Determinar la existencia de modificaciones en los parámetros de la marcha con la eliminación de callosidades y callos plantares Observar las diferencias y efectividad de tratamientos de la eliminación de callosidades plantares mediante la técnica de deslaminación mecánica con bisturí versus parches de ácido salicílico Comprobar los cambios producidos en los parámetros psíquicos y fiscos del paciente antes y después de las diferentes técnicas de eliminación de las callosidades empleadas Método Se realizan dos estudios: un estudio cuasi experimental aleatorizado no controlado, en el que a un grupo de 34 pacientes con callosidades plantares dolorosas se les mide el dolor con una escala visual analógica y para analizar los parámetros de la marcha, la paltaformaWin-Track, antes del tratamiento de deslaminación mecánica con bisturí y a las 24 horas. El segundo estudio es un ensayo clínico aleatorizado inscrito en Australian New ZelandClinicalstrials y aprobado por el Comité ético de la Universidad de Málaga, en el que 62 participantes con callosidades plantares dolorosas se dividieron en dos grupos de tratamiento. El grupo A recibió tratamiento con parche de ácido salicílico y el grupo B recibió tratmiento de deslaminación con bisturí. Se utilizó la escala visual analógica para la medida de dolor antes, inmediatamente después de la intervención, a las 2 semanas y a las 6 semanas. Para el dolor y la discapacidad funcional del pie se utilizó el cuestionario Manchester FootPain and Disability antes del tratamiento, a las 2 semanas y a las 6 semanas. Para medir la calidad de vida general se utilizó el cuestionario SF-12 Conclusiones La deslaminación mecánica con bisturí de los callos y callosidades plantares es efectiva para su eliminación a nivel de la sensación de dolor, aunque no tanto en lo que se refiere a la mejora de calidad de vida. No hay resultados significativos de que la eliminación mecánica con bisturí de callos y callosidades plantares modifican los parámetros de la marcha medido con la plataforma Win-track. Se observa como la deslaminación mecánica con bisturí para la eliminación de callos y callosidades plantares pueden ser más efectiva a corto plazo que la eliminación mediante parche con ácido salicílico. Background The prevalence of geriatric syndromes (falls, immobility, intellectual or memory impairment, and incontinence) is unknown in many resource-poor countries. With an aging population such knowledge is essential to develop national policies on the health and social needs of older people. The aim of this study was to provide a preliminary survey to explore the prevalence of falls and other geriatric syndromes and their association with known risk factors in people aged > 60 years in urban Blantyre, Malawi. Methods This was a cross-sectional, community survey of adults aged > 60 years. Subjects were recruited at home or in the waiting areas of chronic care clinics. They were interviewed to complete a questionnaire on ageassociated syndromes and comorbid problems. The Abbreviated Mental Test (AMT) and Timed Up and Go (TUG) tests were carried out. Results Ninety-eight subjects were studied; 41% reported falling in the past 12 months, 33% of whom (13% of all subjects) were recurrent fallers. Twenty-five percent reported urine incontinence, 66% self-reported memory difficulties, and 11% had an AMT score < 7. A history of falling was significantly associated with urine incontinence (p=0.01), selfreported memory problems (p=0.004) and AMT score < 7 (p=0.02). Conclusions Geriatric syndromes, including falls, appear to be prevalent in older people in Blantyre, Malawi. Falling is associated with cognitive impairment and urinary incontinence. There is an urgent need for more understanding of geriatric problems in this setting to develop national policies on health and social needs of older people. It is likely that many of the contributory factors to falls would be amenable to multifactorial interventions similar to those found to be effective in developed countries.

This review provides an update on current evidence surrounding epidemiology, treatment and prevention of lower respiratory tract infection, with special reference to pneumonia and influenza, in care home residents. The care home sector is growing and provides a unique ecological niche for infections, housing frail older people with multiple comorbidities and frequent contact with healthcare services. There are therefore considerations in the epidemiology and management of these conditions which are specific to care homes. Opportunities for prevention, in the form of vaccination strategies and improving oral hygiene, may reduce the burden of these diseases in the future. Work is needed to research these infections specifically in the care home setting and this article highlights current gaps in our knowledge.

O presente relatório pretende descrever a intervenção psicomotora com população idosa em contexto de estágio profissionalizante no centro de dia do Centro Social da Trafaria da Santa Casa de Misericórdia de Almada, no ano de 2014-2015 no âmbito do Ramo de Aprofundamento de Competências Profissionais do Mestrado de Reabilitação Psicomotora da Faculdade de Motricidade Humana da Universidade de Lisboa. Encontra-se estruturado em duas partes relativas ao enquadramento e realização da prática profissional. Deste modo, são referidos os aspetos teóricos de envelhecimento, intervenção psicomotora, gerontopsicomotricidade, e é ainda realizada uma caracterização da instituição de forma a fundamentar e enquadrar a prática profissional. Referente à realização da prática profissional são abordadas as duas etapas da intervenção realizadas, especificando a intervenção psicomotora, na segunda etapa, realizada com um grupo de oito pessoas idosas e um estudo caso, verificando-se no geral uma manutenção e melhoria das competências psicomotoras. No final do relatório são referidas as conclusões finais, sendo uma análise crítica e reflexiva acerca da intervenção psicomotora neste contexto, limitações e propostas.

El objetivo del estudio es evaluar la mortalidad a un año en pacientes con fractura de cadera, mayores de 65 años tratados en un programa establecido de orto-geriatría. 298 se trataron de acuerdo al protocolo de orto-geriatría, se calculo la mortalidad a un año, se establecieron los predictores de mortalidad orto-geriátrico. La sobrevida anual se incremento de 80% a 89% (p = .039) durante los cuatro años de seguimiento del programa y disminuyo el riesgo de mortalidad anual postoperatorio (Hazard Ratio = 0.54, p = .049). La enfermedad cardiaca y la edad maor a 85 años fueron predictores positivos para mortalidad.

Methods: Cross-sectional study with non probabilistic sample of 333 elderly, performed in a hospital, homes and day centers for the elderly. The data collection protocol included socio-demographic data, Questions to elicit Elder Abuse (Carney, Kahan & Paris, 2003 adap. By Ferreira Alves & Sousa, 2005), scale of instrumental activities of daily living Lawton and Brody and Katz index to assess the level of independence in activities of daily living. Objectives: To evaluate the association between abuse and neglect in the elderly, instrumental activities of daily living and level of independence in activities of daily living. Results: Emotional abuse is significantly correlated with the level of independence in activities of daily living (p = 0.000), older people with less independence tend to have higher levels of emotional abuse. The total abuse is significantly correlated with the levels of independence in activities of daily living (p = 0.002), less independent elderly tend to suffer greater abuse and neglect. There were no statistically significant associations between abuse and neglect and instrumental activities of daily living. Conclusions: The less independent elderly are more vulnerable to situations of abuse and neglect, being more exposed to emotional abuse. These results point to the need for health professionals/ nurses develop prevention interventions, including strategies to support carers and early screening in less independent elderly.

The aim of this study was to analyze the prevalence of hypertension and control practices among the elderly. The survey analyzed data from 872 elderly people in São Paulo, Brazil, through a cluster sampling, stratified according to education and income. A Poisson multiple regression model checked for the existence of factors associated with hypertension. The prevalence of self-reported hypertension among the elderly was 46.9%. Variables associated with hypertension were self-rated health, alcohol consumption, gender, and hospitalization in the last year, regardless of age. The three most common measures taken to control hypertension, but only rarely, are oral medication, routine salt-free diet and physical activity. Lifestyle and socioeconomic status did not affect the practice of control, but knowledge about the importance of physical activity was higher among those older people with higher education and greater income. The research suggests that health policies that focus on primary care to encourage lifestyle changes among the elderly are necessary.
