The use and abuse of Psychoactive Substances (PAS) in contemporaneity corresponds to a social issue and a public health issue. Few social phenomena entail more costs with justice and health, family difficulties, and appearances in the media than the PAS abuse comsumption. The government power has been facing this situation allocating investments and developing public policies. Despite the current Mental Health Policy, based on the principles of Psychiatric Reform that prioritizes outpatient services, the number of investments from various government spheres and families requests for admissions continue increasing. This study aimed to understand the pathos experienced by an individual toward the involuntary internment of a family member who is an abusive user of PAS. The research also aimed to investigate what led that individual to choose this type of treatment. The Psychoanalysis was the theoretical basis of this work, and the exercise of the psychoanalytic method, from the collection of bibliographic references up to the interpretation of the semi-structured interview, conducted in depth, was intended. The findings of this research gave us the oportunity of thinking about how the social callings to the family were made, especially in regard of atention and care with their family members who are user of PAS and how it affects this family individual. It also allowed to discuss how the public policies that preconize involuntary internment, affectivity, prohibitionist and mono-disciplinarity – that cross the State in the attention given to this issue – are formulated and implemented. The interview analysis showed us how happen the agencying of pathos, the libidinal aspects of joy and guilt, the desire to punish and atonement, working in family relations and in caring relations, especially in the decision for involuntary internment. The survey also made possible to understand how a mother, facing the chaotic scene of public health, helpless, finds in the involuntary internment a way to reverberate her affections.

BACKGROUND: Involuntary job loss is a major life event associated with social, economic, behavioural, and health outcomes, for which older workers are at elevated risk. OBJECTIVE: To assess the 10 year risk of myocardial infarction (MI) and stroke associated with involuntary job loss among workers over 50 years of age. METHODS: Analysing data from the nationally representative US Health and Retirement Survey (HRS), Cox proportional hazards analysis was used to estimate whether workers who suffered involuntary job loss were at higher risk for subsequent MI and stroke than individuals who continued to work. The sample included 4301 individuals who were employed at the 1992 study baseline. RESULTS: Over the 10 year study frame, 582 individuals (13.5% of the sample) experienced involuntary job loss. After controlling for established predictors of the outcomes, displaced workers had a more than twofold increase in the risk of subsequent MI (hazard ratio (HR) = 2.48; 95% confidence interval (CI) = 1.49 to 4.14) and stroke (HR = 2.43; 95% CI = 1.18 to 4.98) relative to working persons. CONCLUSION: Results suggest that the true costs of late career unemployment exceed financial deprivation, and include substantial health consequences. Physicians who treat individuals who lose jobs as they near retirement should consider the loss of employment a potential risk factor for adverse vascular health changes. Policy makers and programme planners should also be aware of the risks of job loss, so that programmatic interventions can be designed and implemented to ease the multiple burdens of joblessness.

L’imagerie musicale involontaire (IMIN) est un phénomène mental extrêmement commun. Il peut être défini en tant que type d’imagerie mentale musicale qui devient consciente sans effort ou intentionnalité et qui n’est pas pathologique. La forme la plus connue d’IMIN est le « ver d’oreille », qui se présente généralement comme un court extrait musical tournant en boucle en tête et dont on se débarrasse difficilement. L’objectif principal de la présente thèse est d’investiguer les mécanismes cognitifs sous-tendant le phénomène puisque, malgré l’intérêt répandu dans les médias populaires, son étude expérimentale est récente et un modèle intégré n’a pas encore été proposé. Dans la première étude, l’induction expérimentale a été tentée et les caractéristiques des images mentales d’épisodes d’IMIN ont été investiguées. Dans le laboratoire, des chansons accrocheuses (versus des proverbes) ont été présentées répétitivement aux participants qui devaient ensuite les chanter le plus fidèlement possible. Ils ont par après quitté le laboratoire, une enregistreuse numérique en mains, avec la consigne d’enregistrer une reproduction vocale la plus fidèle possible de ce qu’ils avaient en tête lors de tous leurs épisodes d’IMIN sur une période de quatre jours, ainsi que de décrire leur timbre. L’expérience a été répétée deux semaines plus tard. Douze des dix-huit participants du groupe expérimental ont rapporté des pièces induites comme épisodes d’IMIN, ce qui confirme l’efficacité de la procédure d’induction. La tonalité et le tempo des productions ont ensuite été analysés et comparés à ceux des pièces originales. Similairement pour les épisodes d’IMIN induits et les autres, les tempi produits et, dans une moindre mesure pour les non-musiciens, les tonalités étaient proches des originaux. Le timbre décrit était généralement une version simplifiée de l’original (un instrument et/ou une voix). Trois études se sont ensuite intéressées au lien entre le potentiel d’IMIN et la mémorabilité. Dans une étude préliminaire, 150 chansons du palmarès francophone radiophonique ont été évaluées en ligne par 164 participants, sur leur niveau de familiarité, d’appréciation et de potentiel d’IMIN. Les pièces ont ensuite été divisées en groupes de stimuli à faible et à fort potentiel d’IMIN, qui ont été utilisés dans une tâche typique de rappel libre/reconnaissance, premièrement avec des francophones (pour qui les pièces étaient familières) et ensuite avec des non-francophones (pour qui les pièces étaient non-familières). Globalement, les pièces à fort potentiel d’IMIN étaient mieux rappelées et reconnues que les pièces à faible potentiel. Une dernière étude a investigué l’impact de la variabilité inter-stimulus du timbre sur les résultats précédents, en demandant à une chanteuse d’enregistrer les lignes vocales des pièces et en répétant l’expérience avec ces nouveaux stimuli. La différence précédemment observée entre les stimuli à fort et à faible potentiel d’IMIN dans la tâche de reconnaissance a ainsi disparu, ce qui suggère que le timbre est une caractéristique importante pour le potentiel d’IMIN. En guise de conclusion, nous suggérons que les phénomènes mentaux et les mécanismes cognitifs jouant un rôle dans les autres types de souvenirs involontaires peuvent aussi s’appliquer à l’IMIN. Dépendamment du contexte, la récupération mnésique des pièces peut résulter de la répétition en mémoire à court terme, de l’amorçage à court et long terme ou de l’indiçage provenant de stimuli dans l’environnement ou les pensées. Une des plus importantes différences observables entre l’IMIN et les autres souvenirs involontaires est la répétition. Nous proposons que la nature même de la musique, qui est définie par la répétition à un niveau micro- et macro-structurel en est responsable.

BACKGROUND: Post-abortion contraceptive use in India is low and the use of modern methods of contraception is rare, especially in rural areas. This study primarily compares contraceptive use among women whose abortion outcome was assessed in-clinic with women who assessed their abortion outcome at home, in a low-resource, primary health care setting. Moreover, it investigates how background characteristics and abortion service provision influences contraceptive use post-abortion. METHODS: A randomized controlled, non-inferiority, trial (RCT) compared clinic follow-up with home-assessment of abortion outcome at 2 weeks post-abortion. Additionally, contraceptive-use at 3 months post-abortion was investigated through a cross-sectional follow-up interview with a largely urban sub-sample of women from the RCT. Women seeking abortion with a gestational age of up to 9 weeks and who agreed to a 2-week follow-up were included (n = 731). Women with known contraindications to medical abortions, Hb < 85 mg/l and aged below 18 were excluded. Data were collected between April 2013 and August 2014 in six primary health-care clinics in Rajasthan. A computerised random number generator created the randomisation sequence (1:1) in blocks of six. Contraceptive use was measured at 2 weeks among women successfully followed-up (n = 623) and 3 months in the sub-set of women who were included if they were recruited at one of the urban study sites, owned a phone and agreed to a 3-month follow-up (n = 114). RESULTS: There were no differences between contraceptive use and continuation between study groups at 3 months (76 % clinic follow-up, 77 % home-assessment), however women in the clinic follow-up group were most likely to adopt a contraceptive method at 2 weeks (62 ± 12 %), while women in the home-assessment group were most likely to adopt a method after next menstruation (60 ± 13 %). Fifty-two per cent of women who initiated a method at 2 weeks chose the 3-month injection or the copper intrauterine device. Only 4 % of women preferred sterilization. Caste, educational attainment, or type of residence did not influence contraceptive use. CONCLUSIONS: Simplified follow-up after early medical abortion will not change women's opportunities to access contraception in a low-resource setting, if contraceptive services are provided as intra-abortion services as early as on day one. Women's postabortion contraceptive use at 3 months is unlikely to be affected by mode of followup after medical abortion, also in a low-resource setting. Clinical guidelines need to encourage intra-abortion contraception, offering the full spectrum of evidence-based methods, especially long-acting reversible methods. Within The Creative Unconscious and Pictorial Sign I explore the dialogue that exists between social language and personal expression to understand how creativity is mediated. I consider how the involuntary inventiveness of artistic creativity and the structuring function of language come to negotiate what artists can experience and represent. My Doctoral practice attempts to question the influence of orthodox postmodernist views and allow sensual and direct experiences to be located within improvisation and spontaneous approaches to image making. I ask if it is possible for a humanistic and psychological interpretation of creativity to move beyond the copy and quotation that some postmodern theories of simulation and the hyperreal advance; but to retain the communicative function of visual expression and the model of a social form of signification instead of naïvely promoting unintelligible and personal languages.

OBJECTIFS: La libération de dopamine par les afférences à sérotonine (5-HT) du striatum constitue un déterminant pré-synaptique important des dyskinésies induites par la L-Dopa (DILs), un effet délétère du traitement pharmacologique de la maladie de Parkinson. En effet, les axones 5-HT seraient en mesure de libérer de façon non-physiologique de la dopamine lorsque la L-Dopa est administrée au patient, contribuant ainsi à l’expression des DILs. Certaines afférences striatales 5-HT contiennent un transporteur vésiculaire du glutamate (VGluT3) et nous croyons que sa présence puisse avoir un effet synergique sur la libération de dopamine. L’objectif général de ce mémoire est donc d’évaluer la quantité de VGluT3 présent au sein des axones 5-HT et de mesurer son implication dans l’expression des DILs. MÉTHODES : Dix-huit souris C57/Bl6 ont été séparées en trois groupes expérimentaux. Douze souris ont reçu une injection intracérébrale de 6- hydroxydopamine (6-OHDA) dans le faisceau prosencéphalique médian afin de léser les afférences dopaminergiques du striatum. Six souris lésées ont reçu des injections systémiques de L-Dopa (12 jours, 1 fois/jour). Six autres souris ont reçu une injection intracérébrale du véhicule afin de servir de contrôle. La sévérité des mouvements involontaires anormaux induits par la L-Dopa (équivalent des dyskinésies) a été quantifiée selon une échelle reconnue. Un double marquage en immunofluorescence pour le transporteur membranaire de la 5-HT (SERT) et le VGluT3 a permis d’évaluer la densité des varicosités SERT+ et SERT+/VGluT3+ dans le striatum dorsal et de comparer ces données entre les trois groupes expérimentaux. RÉSULTATS: Chez les trois groupes de souris, un faible pourcentage des varicosités axonales 5-HT sont également VGluT3+. Ces varicosités doublement marquées sont souvent retrouvées sur une même branche axonale. Aucune différence significative n’a été observée entre les trois groupes expérimentaux en ce qui a trait à la proportion de varicosités SERT+ qui contiennent le VGluT3+. CONCLUSION: Nos données expérimentales ne nous permettent pas de conclure que la densité des varicosités axonales SERT+ ou SERT+/VGluT3+ au sein du striatum dorsal varie en fonction de la sévérité des mouvements involontaires anormaux induits par l’administration de L-Dopa.

A área da Endodontia está em constante progresso. Os materiais utilizados nos instrumentos Endodônticos, primordialmente, eram construídos com base em cordas de piano. Seguiu-se uma fase em que estes eram de aço de carbono, mas sofriam corrosão significativa devido ao cloro presente no hipoclorito de sódio, bem como aos processos de esterilização a vapor. Foi necessário evoluir novamente e foram introduzidos os instrumentos de aço inoxidável. Estes apresentavam alta resistência e dureza, mas algumas desvantagens devido à falta de flexibilidade. Atualmente, os instrumentos de NiTi proporcionam uma melhor flexibilidade e efeito de memória de forma. A fratura de instrumentos em Endodontia pode ocorrer por dois grandes fatores: a torção e a flexão por fadiga cíclica, podendo também ser a conjugação de ambos. Fatores anatômicos, como a curvatura e a largura do canal ou outros fatores como ciclos de esterilização, número de usos, etc., podem influenciar uma fratura mais precoce dos instrumentos. A incidência da fratura de instrumentos, embora seja pouco frequente, pode ser reduzida a um mínimo absoluto se os clínicos usarem as características de torque e de stress adequadas. Um bom conhecimento dos procedimentos clínicos, da anatomia, dos materiais e a utilização de instrumentos como o microscópio podem ajudar a prevenir ou a resolver a fratura dos instrumentos. No entanto, a melhor forma de prevenir a fratura é a sua prevenção. A desinfeção é o procedimento mais importante para o sucesso de um tratamento Endodôntico, portanto para que isto seja possível, é necessária uma boa conformação canalar. A presença de um instrumento no interior do canal pode comprometer a desinfecção, especialmente caso tenha ocorrido numa fase precoce da preparação canalar. Aquando da fratura de um instrumento, deve-se refletir sobre os procedimentos a seguir, podendo-se optar por várias abordagens, nomeadamente pela manutenção do instrumento no canal e obturação incorporando o fragmento, pela remoção do segmento através de diversas técnicas (ultrassons ou técnicas de microtubos, etc.), e ainda pela realização do bypass ou pela cirurgia Endodôntica. Em última instância pode ser realizada a extração do elemento dentário.

Background: Several materials have been used for tissue engineering purposes, since the ideal matrix depends on the desired tissue. Silk biomaterials have come to focus due to their great mechanical properties. As untreated silkworm silk has been found to be quite immunogenic, an alternative could be spider silk. Not only does it own unique mechanical properties, its biocompatibility has been shown already in vivo. In our study, we used native spider dragline silk which is known as the strongest fibre in nature. Methodology/Principal Findings: Steel frames were originally designed and manufactured and woven with spider silk, harvesting dragline silk directly out of the animal. After sterilization, scaffolds were seeded with fibroblasts to analyse cell proliferation and adhesion. Analysis of cell morphology and actin filament alignment clearly revealed adherence. Proliferation was measured by cell count as well as determination of relative fluorescence each after 1, 2, 3, and 5 days. Cell counts for native spider silk were also compared with those for trypsin-digested spider silk. Spider silk specimens displayed less proliferation than collagen-and fibronectin-coated cover slips, enzymatic treatment reduced adhesion and proliferation rates tendentially though not significantly. Nevertheless, proliferation could be proven with high significance (p<0.01). Conclusion/Significance: Native spider silk does not require any modification to its application as a biomaterial that can rival any artificial material in terms of cell growth promoting properties. We could show adhesion mechanics on intracellular level. Additionally, proliferation kinetics were higher than in enzymatically digested controls, indicating that spider silk does not require modification. Recent findings concerning reduction of cell proliferation after exposure could not be met. As biotechnological production of the hierarchical composition of native spider silk fibres is still a challenge, our study has a pioneer role in researching cellular mechanics on native spider silk fibres.

A descriptive study was developed to monitor air fungal contamination in one Portuguese maternity. Sixty air samples were collected through impaction method. Air sampling was performed in food storage facilities, kitchen, food plating, canteen, pharmacy, sterilization areas, genecology wards, intensive care unit, operating rooms, urgency and also, outside premises, since this was the place regarded as reference. Besides air samples, forty three samples were collected by swabbing the surfaces using a 10 by 10 cm square stencil. Simultaneously, temperature, relative humidity and particles counting (PM10) were registered. Twenty three species of fungi were identified in air, being the two most commonly isolated the genera Penicillium (41,5%) and Cladosporium (28,4%). Regarding yeasts, only Rhodotorula sp. (45,2%), Trichosporon mucoides (51,6%) and Cryptococcus neoformans (3,2%) were found. Thirteen species of fungi were identified in surfaces, being the most frequent the Penicillium genus (91,6%). Concerning yeasts found in surfaces, four species were identified being Rhodotorula sp. (29,1%) the most frequent. There was no coincidence between prevailing genera indoors and outside premises. Moreover, some places presented fungal species different from the ones isolated outside. In the inside environment, Aspergillus species were isolated in air and surfaces. There was no significant relationship (p>0,05) between fungal contamination and the studied environmental variables. De las motivaciones de su actitud, apunto como hipótesis el uso que hace Zambrano de Ortega como respaldo (involuntario) de su razón poética: para legitimar, con la continuidad que da a entender entre ambos, una metodología con la que la poesía reemplaza a la filosofía en sus funciones para ampliar los márgenes de lo cognoscible. [en] María Zambrano hides or diminishes the differences that confronted Ortega. Also, the different roles they assign to art: she does not make a critique of escapism that raises the aesthetics of vital reason. To explain the motivations for her attitude, I propose the hypothesis that Zambrano used Ortega in support (involuntary) of her poetic reason: to legitimize her idea, poetry replaces philosophy in its functions to expand the margins of the knowable.

In 2012, were estimated 6.7 million cases of healthcare-associated infections (HAI) either in long-term care facilities or acute-care hospitals from which result 37,000 deaths configuring a serious public health problem. The etiological agents are diverse and often resistant to antimicrobial drugs. One of the mechanisms responsible for the emergence of drug resistance is biofilm assembly. Biofilms are defined as thin layers of microorganisms adhering to the surface of a structure, which may be organic or inorganic, together with the polymers that they secrete. They are dynamic structures which experience different stages of organization with the ageing and are linked to an increase in bacterial resistance to host defense mechanisms, antibiotics, sterilization procedures other than autoclaving, persistence in water distribution systems and other surfaces. The understanding of bacteria organization within the biofilm and the identification of differences between planktonic and sessile forms of bacteria will be a step forward to fight HAIs.

Shame is a social emotion with adaptive functions involved in human be-havior and social interactions. This emotion is regarded as an involuntary response associated with increased self-awareness, loss of status and self-devaluation (Gilbert, 1998), that may render individuals more prone to psychopathology (Gilbert, 1998; Pinto-Gouveia & Matos, 2011). Thus, identifying and assessing feelings of shame in childhood is essential in addressing the actual impact of shame on individual’s developmental trajectory. The Other As Shamer Scale (OAS; Goss, Gilbert & Allan, 1994) is a widely used measure of external shame, adapted and translated to several languages — including Portuguese (Matos, Pinto-Gouveia, Gilbert, Duarte & Figueiredo, 2015) — to adult and to adolescent populations (OASB-A - Other As Shamer Brief for adolescents; translated and adapted by Cunha, Xavier, Cherpe & Pinto-Gouveia, 2014). The current study aims to adapt and to explore the psychometric proper-ties of the brief OAS in a sample of Portuguese children attending to elementary schools.
