Objectives: To investigate the effectiveness of valerian for the management of chronic insomnia in general practice. Design: Valerian versus placebo in a series of n-of-1 trials, in Queensland, Australia. Results: Of 42 enrolled patients, 24 (57%) had sufficient data for inclusion into the n-of-1 analysis. Response to valerian was fair for 23 (96%) participants evaluating their 'energy level in the previous day' but poor or modest for all 24 (100%) participants' response to 'total sleep time' and for 23 (96%) participants' response to 'number of night awakenings' and 'morning refreshment'. As a group, the proportion of treatment successes ranged from 0.35 (95% CI 0.23, 0.47) to 0.55 (95% CI 0.43, 0.67) for the six elicited outcome sleep variables. There was no significant difference in the number (P = 0.06), distribution (P = 1.00) or severity (P = 0.46) of side effects between valerian and placebo treatments. Conclusions: Valerian was not shown to be appreciably better than placebo in promoting sleep or sleep-related factors for any individual patient or for all patients as a group. (C) 2003 Elsevier Ltd. The occurrence of hand grindstones at the Cogotas I archaeological sites is considered to be a common feature. Given that a distant-provenance raw material is frequently involved, determination of its source is a basic factor in the search for a better understanding of resource management and for any Political Economy approach. To progress in these directions an overall study should be planned, using selected grindstones with a view to covering diverse sub-zones of the Cogotas I dispersal area, especially because of its considerable distance from the granite basement source. Such a study may today includes diverse analytical procedures combining successive geographic, petrographic, mineralogical and geochemical criteria. To check the plausibility of the proposed methodology, a preliminary test has been carried out on two granite grindstones, obtained at the archaeological excavation at the Castronuño (Valladolid) Cogotian site, which is fifty km away from an inferred source area that was presumably located at Peñausende (Zamora). The result obtained validates the proposed operational process, yielding a generalizable knowledge to other similar situations.

El presente estudio revisa la caracterización de los enunciados realizativos, deteniéndose en aquellos que no presentan estructura oracional. Así, analizaremos las expresiones no verbales que presentan las características propiamente performativas: construcciones preposicionales encabezadas por núcleos ´locativos`, ´sumativos` y ´sustractivos` y apelaciones enfáticas. En este sentido, podremos determinar el valor de las preposiciones que introducen sintagmas equivalentes a predicados con sentido realizativo. Por otro lado, distinguiremos dos modos de enunciar en relación con la descortesía y la adulación: insultos y halagos. Dichas expresiones se describirán desde una perspectiva gramatical y prosódica, de modo que se atenderá a su capacidad para reproducir enunciados realizativos convencionales y de orientar el acento y la inflexión melódica a la transformación cognitiva y emocional de la realidad del individuo receptor.

AIMS: Our aims were to evaluate the distribution of troponin I concentrations in population cohorts across Europe, to characterize the association with cardiovascular outcomes, to determine the predictive value beyond the variables used in the ESC SCORE, to test a potentially clinically relevant cut-off value, and to evaluate the improved eligibility for statin therapy based on elevated troponin I concentrations retrospectively.
METHODS AND RESULTS: Based on the Biomarkers for Cardiovascular Risk Assessment in Europe (BiomarCaRE) project, we analysed individual level data from 10 prospective population-based studies including 74 738 participants. We investigated the value of adding troponin I levels to conventional risk factors for prediction of cardiovascular disease by calculating measures of discrimination (C-index) and net reclassification improvement (NRI). We further tested the clinical implication of statin therapy based on troponin concentration in 12 956 individuals free of cardiovascular disease in the JUPITER study. Troponin I remained an independent predictor with a hazard ratio of 1.37 for cardiovascular mortality, 1.23 for cardiovascular disease, and 1.24 for total mortality. The addition of troponin I information to a prognostic model for cardiovascular death constructed of ESC SCORE variables increased the C-index discrimination measure by 0.007 and yielded an NRI of 0.048, whereas the addition to prognostic models for cardiovascular disease and total mortality led to lesser C-index discrimination and NRI increment. In individuals above 6 ng/L of troponin I, a concentration near the upper quintile in BiomarCaRE (5.9 ng/L) and JUPITER (5.8 ng/L), rosuvastatin therapy resulted in higher absolute risk reduction compared with individuals <6 ng/L of troponin I, whereas the relative risk reduction was similar.
CONCLUSION: In individuals free of cardiovascular disease, the addition of troponin I to variables of established risk score improves prediction of cardiovascular death and cardiovascular disease.

Este artículo describe los métodos, técnicas de enseñanza y recursos didácticos, utilizados por una muestra de docentes de I y II Ciclo de la Educación General Básica en la Dirección Regional Educativa de Heredia al enseñar Probabilidad y Estadística, que surge como resultado de una investigación realizada en el 2009, en Costa Rica. Para obtener los datos necesarios se aplicó un cuestionario auto-administrado a dicha muestra de docentes. En relación con los métodos, dentro de los hallazgos más importantes se encontró una preferencia de los educadores entrevistados, hacia estrategias pedagógicas tradicionales (usan con mayor frecuencia los métodos: deductivo, expositivo, estudio de textos, colectivo). En cuanto a técnicas de enseñanza, se señalaron con uso frecuente la lluvia de ideas, las actividades lúdicas y el trabajo en grupos, que si bien son buenas herramientas para propiciar un proceso de enseñanza y aprendizaje en donde el estudiante interactúe con situaciones problemáticas, también pueden emplearse en lecciones tradicionales. La técnica elaboración de proyectos resultó de uso poco frecuente. Además, dentro los principales recursos didácticos elegidos por los docentes, para enseñar Probabilidad y Estadística, se destacan la pizarra y el material fotocopiado; aunque también se mencionaron las actividades lúdicas y el material concreto.
