INTRODUCTION: Schizophrenia is a chronic mental disorder associated with impairment in social functioning. The most widely used scale to measure social functioning is the GAF (Global Assessment of Functioning), but it has the disadvantage of measuring at the same time symptoms and functioning, as described in its anchors. OBJECTIVES:Translation and cultural adaptation of the PSP, proposing a final version in Portuguese for use in Brazil. METHODS: We performed five steps: 1) translation; 2) back translation; 3) formal assessment of semantic equivalence; 4) debriefing; 5) analysis by experts. Interrater reliability (Intraclass correlation, ICC) between two raters was also measured. RESULTS: The final version was applied by two independent investigators in 18 adults with schizophrenia (DSM-IV-TR). The interrater reliability (ICC) was 0.812 (p < 0.001). CONCLUSION: The translation and adaptation of the PSP had an adequate level of semantic equivalence between the Portuguese version and the original English version. There were no difficulties related to understanding the content expressed in the translated texts and terms. Its application was easy and it showed a good interrater reliability. The PSP is a valid instrument for the measurement of personal and social functioning in schizophrenia.

Undergraduate medical education is moving from traditional disciplinary basic science courses into more integrated curricula. Integration models based on organ systems originated in the 1950s, but few longitudinal studies have evaluated their effectiveness. This article outlines the development and implementation of the Organic and Functional Systems (OFS) courses at the University of Minho in Portugal, using evidence collected over 10 years. It describes the organization of content, student academic performance and acceptability of the courses, the evaluation of preparedness for future courses and the retention of knowledge on basic sciences. Students consistently rated the OFS courses highly. Physician tutors in subsequent clinical attachments considered that students were appropriately prepared. Performance in the International Foundations of Medicine examination of a self-selected sample of students revealed similar performances in basic science items after the last OFS course and 4 years later, at the moment of graduation. In conclusion, the organizational and pedagogical approaches of the OFS courses achieve high acceptability by students and result in positive outcomes in terms of preparedness for subsequent training and long-term retention of basic science knowledge.

Objective The purpose of this research was to make a cross-cultural adaptation of the Medication Adherence Rating Scale (MARS) for psychiatric patients to the Brazilian context. Methods The procedure consisted of four phases: translation of the original scale, back-translation, review by an Expert Committee and Pre-test study with a patients’ sample. Results The Expert Committee corrected the items’ translation when necessary and modified the scale administration format and its instructions from self-report to face-to-face interview form in order to ensure easy understanding by the target population. During Pre-test, the instructions and most of the items were properly understood by patients, with the exception of three of them which had to be changed in order to ensure better understanding. The Pre-test sample was composed by 30 psychiatric patients, with severe and persistent disorders mainly single (46.7%), female (60.0%), with a mean age of 43.8 years old and an average of five years of education. Conclusion The Brazilian version of MARS scale is now adapted to the Brazilian Portuguese language and culture and is easily understood by the psychiatric target population. It is necessary to do further research to evaluate the scale psychometric qualities of validity and reliability in order to use it in Brazil.

The authors consider the possibility of using color Doppler of the ductus venosus and the measurement of nuchal translucency as a screening test for alterations in fetal cardiac functions in the first trimester of gestation. Review of the literature suggests that the combination of the ultrasonographic measurement of nuchal translucency and Doppler at 10 and 14 weeks of gestation can be effective in detecting certain cardiac abnormalities. This conclusion, however, is preliminary and needs to be further investigated.

PURPOSE: To compare peak exercise oxygen consumption (VO2peak) of healthy individuals with asymptomatic individuals with probable heart disease. METHODS: Ninety-eight men were evaluated. They were divided into two groups: 1) 39 healthy individuals (group N) with an age range of 50±4.6 years; and 2) 59 asymptomatic individuals with signs of atherosclerotic and/or hypertensive heart disease (group C) with an age range of 51.9±10.4 years. In regard to age, height, body surface area, percentage of fat, lean body mass, and daily physical activity, both groups were statistically similar. Environmental conditions during the ergometric test were also controlled. RESULTS: Maximal aerobic power (watts), VO2peak, maximal heart rate, and maximal pulmonary ventilation were lower in group C (p<0.01) than in group N; weight, however, was lower in group N (p=0.031) than in group C. Differences in the respiratory gas exchange index, heart rate at rest, and the maximal double product of the two groups were not statistically significant. CONCLUSION: Signs of probable heart disease, even though asymptomatic, may reduce the functional capacity, perhaps due to the lower maximal cardiac output and/or muscle metabolic changes.

OBJECTIVE: To assess by Doppler echocardiography the structural and functional alterations of rat heart with surgical induced extensive myocardial infarction. METHODS: Five weeks after surgical ligature of the left coronary artery, 38 Wistar-EPM rats of both sexes, 10 of them with extensive infarction, undergone anatomical and functional evaluation by Doppler echocardiography and then euthanized for anatomopathological analysis. RESULTS: Echocardiography was 100% sensible and specific to anatomopathological confirmed extensive miocardial infarction. Extensive infarction lead to dilatation of left ventricle (diastolic diameter: 0.89cm vs.0.64cm; systolic: 0.72cm vs. 0.33cm) and left atrium (0.55cm vs. 0.33cm); thinning of left ventricular anterior wall (systolic: 0.14cm vs. 0.23cm, diastolic: 0.11cm vs. 0.14cm); increased mitral E/ A wave relation (6.45 vs. 1.95). Signals of increased end diastolic ventricle pressure, B point in mitral valve tracing in 62.5% and signs of pulmonary hypertension straightening of pulmonary valve (90%) and notching of pulmonary systolic flow (60%) were observed in animals with extensive infarction. CONCLUSION: Doppler echocardiography has a high sensitivity and specificity for detection of chronic extensive infarction. Extensive infarction caused dilatation of left cardiac chambers and showed in Doppler signals of increased end diastolic left ventricular pressure and pulmonary artery pressure.

OBJECTIVE: To evaluate the influence of systolic or diastolic dysfunction, or both on congestive heart failure functional class. METHODS: Thirty-six consecutive patients with a clinical diagnosis of congestive heart failure with sinus rhythm, who were seen between September and November of 1998 answered an adapted questionnaire about tolerance to physical activity for the determination of NYHA functional class. The patients were studied with transthoracic Doppler echocardiography. Two groups were compared: group 1 (19 patients in functional classes I and II) and group 2 (17 patients in functional classes III and IV). RESULTS: The average ejection fraction was significantly higher in group 1 (44.84%±8.04% vs. 32.59%±11.48% with p=0.0007). The mean ratio of the initial/final maximum diastolic filling velocity (E/A) of the left ventricle was significantly smaller in group 1 (1.07±0.72 vs. 1.98±1.49 with p=0.03). The average maximum systolic pulmonary venous velocity (S) was significantly higher in group 1 (53.53cm/s ± 12.02cm/s vs. 43.41cm/s ± 13.55cm/s with p=0.02). The mean ratio of maximum systolic/diastolic pulmonary venous velocity was significantly higher in group 1 (1.52±0.48 vs. 1.08±0.48 with p=0.01). A predominance of pseudo-normal and restrictive diastolic patterns existed in group 2 (58.83% in group 2 vs. 21.06% in group 1 with p=0.03). CONCLUSION: Both the systolic dysfunction index and the patterns of diastolic dysfunction evaluated by Doppler echocardiography worsened with the evolution of congestive heart failure.

OBJECTIVE: To study mitral valve function in the postoperative period after correction of the partial form of atrioventricular septal defect. METHODS: Fifty patients underwent surgical correction of the partial form of atrioventricular septal defect. Their mean age was 11.8 years and 62% of the patients were males. Preoperative echocardiography showed moderate and severe mitral insufficiency in 44% of the patients. The mitral valve cleft was sutured in 45 (90%) patients (group II - GII). Echocardiographies were performed in the early postoperative period, and 6 and 12 months after hospital discharge. RESULTS: The patients who had some type of arrhythmia in the postoperative period had ostium primum atrial septal defect of a larger size (2.74 x 2.08 cm). All 5 patients in group I (GI), who did not undergo closure of the cleft, had a competent mitral valve or mild mitral insufficiency in the preoperative period. One of these patients began to have moderate mitral insufficiency in the postoperative period. On the other hand, in GII, 88.8% and 82.2% of the patients had competent mitral valve or mild mitral insufficiency in the early and late postoperative periods, respectively. CONCLUSION: The mitral valve cleft was repaired in 90% of cases. Echocardiography revealed competent mitral valve or mild mitral insufficiency in 88.8% and 82.2% of GII patients in the early and late postoperative periods, respectively.

OBJECTIVE: To differentiate the nature of functional cardiorespiratory limitations during exercise in individuals with chronic obstructive pulmonary disease (COPD) or congestive heart failure (CHF) and to determine indicators that may help their classifications. METHODS: The study comprised 40 patients: 23 with COPD and 17 with CHF. All individuals underwent maximal cardiopulmonary exercise testing on a treadmill. RESULTS: The values of peak gas exchange ratio (R peak), peak carbon dioxide production (VCO2 peak), and peak oxygen ventilatory equivalent (V E O2 peak) were higher in the patients with CHF than in those with COPD, and, therefore, those were the variables that characterized the differences between the groups. For group classification, the differentiating functions with the R peak, VCO2 peak (L/min), and V E O2 peak variables were used as follows: group COPD: - 44.886 + 78.832 x R peak + 5.442 x VCO2 peak + 0.336 x V E O2 peak; group CHF: - 69.251 + 89.740 x R peak + 8.461 x VCO2 peak + 0.574 x V E O2 peak. The differentiating function, whose result is greater, correctly classifies the patient's group as 90%. CONCLUSION: The R peak, VCO2 peak, and V E O2 peak values may be used to identify the cause of the functional cardiorespiratory limitations in patients with COPD and CHF.

TEMA I. MECANISMOS NO CLÁSICOS EN EL CONTROL DE LA BIOSÍNTESIS DE HORMONAS TIROIDEAS. El objetivo general de este aspecto del proyecto es el estudio de los mecanismos bioquímicos y moleculares que regulan la hormonogénesis tiroidea en diferentes condiciones funcionales. En proyectos anteriores hemos demostrado que la endotoxina bacteriana lipopolisacárido (LPS) y el oxido nítrico (NO) inducen modificaciones en la biosíntesis de hormonas tiroideas. En base a estos resultados se propone investigar el mecanismo responsable de la estimulación de la captación de ioduro y la expresión de NIS ejercida por LPS y los factores que regulan la producción de NO en la célula tiroidea. Por otra parte se investigarán posibles factores hormonales reguladores de la absorción de ioduro a nivel intestinal y su relación con el eje hipófiso-tiroideo, así como los mecanismos involucrados en la expresión de NIS en enterocitos. Como extensión con aplicación clínica y en base a la experiencia del grupo en el estudio de proteínas que participan en la biosíntesis hormonal tiroidea, se realizará una pesquisa de posibles mutaciones en el transportador de ioduro en pacientes con Hipotiroidismo congénito.TEMA II. ESTUDIO DEL EFECTO DE LAS HORMONAS TIROIDEAS EN LA INICIACIÓN DE LA RESPUESTA INMUNE EN RATÓN. INTERACCIÓN CON GLUCOCORTICOIDES. En nuestro grupo demostramos recientemente un efecto novel de las hormonas tiroideas sobre la maduración/función de células presentadoras de antígenos especializadas: células dendríticas (DC), derivadas de médula ósea de ratón. En este proyecto se propone: 1) profundizar el mecanismo molecular involucrado en el efecto de triiodotironina (T3) sobre DC: rol del receptor de T3-señalamiento intracelular; capacidad de DC tratadas con T3 de estimular la citotoxicidad antígeno-específica; estrategia de vacunación en el tratamiento antitumoral. Por su parte, también previamente demostramos la característica de los glucocorticoides (GC) de interaccionar con el mecanismo de acción de las hormonas tiroideas en la expresión final de los efectos T3-específicos. Considerando el uso terapéutico de los GC en diversos estados clínico-patológicos inmunes, se propone: 2) Estudio del efecto de dexametasona (GC de síntesis) sobre el mecanismo de acción de T3 a nivel de DC.

IDENTIFICACIÓN ZEB1 (Zinc Finger E-box Binding Homeobox) es un factor de transcripción funcionalmente asociado con la diferenciación de células como miocitos, neuronas, células de sostén y linfocitos T, además de estar involucrado en la Transición Epitelial-Mesenquimatosa (EMT) de los tumores sólidos epiteliales. Aún no se ha revelado en profundidad la participación de ZEB1 en los procesos de proliferación y diferenciación en los que participa. Estamos interesados en los mecanismos de regulación de ZEB1 y los factores que intervienen en los procesos de diferenciación y transformación celular. HIPÓTESIS 1. Las vías de señalamiento regulan el estado de fosforilación y la función de ZEB1 en la célula normal, el cual se desregularía en la célula neoplásica llevando a cambios en la función normal de ZEB1 y consecuentemente a metástasis. 2. IGF-1 es la señal que, en asociación con el supresor de tumores CCN6, juega un rol causal en la regulación de ZEB1 y esto a su vez en la metástasis del cáncer de mama. OBJETIVO GENERAL: establecer el rol funcional de ZEB1, su interrelación con otros factores y su regulación en los procesos de diferenciación y transformación celular. OBJETIVOS ESPECIFICOS (incluye Materiales y Métodos) 1. Estudiar la participación de vías de señalización sobre la función biológica de ZEB1 en células normales y neoplásicas. Analizaremos la participación de señales intracelulares en la fosforilación de ZEB1 por experimentos de ganancia/pérdida de función de la vía (por uso de inhibidores farmacologicos, mutantes silenciadoras y siRNAs), lo cual sera evaluado en EMSAs, ChIP, transfecciones, inmunofluoresc, etc. 2. Estudiar el rol de IGF-1 y CCN6 sobre la expresión y el estado de fosforilación de ZEB1 en tumores mamarios benignos, no invasivos e invasivos y metastatizantes. A) Se estudiará la expresión y localización subcelular de ZEB1 en líneas celulares de cáncer mamario y en xenotransplantes de ratón con variada expresión de CCN6. B) Investigar la relevancia de la fosforilación de ZEB1 mediada por IGF-1 en el EMT por experimentos con ganancia/pérdida de función. RESULTADOS ESPERADOS Esperamos poder delinear la/s vía/s de señalización intracelular que fosforilan ZEB1 y así conocer sobre la regulación del mismo. Podremos establecer algunas bases para entender la biología básica del cáncer de mama e identificar blancos terapéuticos. IMPORTANCIA Un amplio conocimiento de los factores de transcripción y sus vías de señalamiento es necesario para el desarrollo tanto de pruebas diagnósticas como para la identificación de nuevos blancos terapéuticos para neoplasias. De modo que resulta de gran importancia clínica determinar el rol de ZEB1, sus proteínas y vías reguladoras en el proceso de oncogénesis. El desarrollo del proyecto prevé la formación de dos tesistas. Se continuaran colaboraciones con dos grupos extranjeros y se iniciara una tercera. ZEB1 (Zinc Finger E-box Binding Homeobox) is a transcription factor involved in cell differentiation and Epithelial Mesenchymal Transition (EMT) of epithelial tumors. We are interested in the study of mechanisms of regulation (pre and post transcriptional). S.A.1. To investigate post translational mechanisms of ZEB1 regulation in normal and cancer cells. We will analyze the involvement of intracellular signals in phosphorylation of ZEB1 by gain- and lost-of-function experiments. S.A.2. A) To determine the role of IGF-1 signaling and CCN6 in regulating the expression of hypo- and hyperphosphorylated forms of ZEB1 in benign and malignant breast cell lines and in xenograft mouse models by overexpressing and inhibiting CCN6 in breast cancer cells. B) To investigate the relevance of CCN6-mediated ZEB1 phosphorylation to EMT, breast cancer invasion and metastasis. The role of CCN6 on ZEB1 phosphorylation and regulation of E-cadherin, induction of EMT, invasion and metastasis of breast cells will be investigated using gain- and loss-of-function experiments.
