The relationship between platelet count and outcome in patients with acute venous thromboembolism (VTE) has not been consistently explored. RIETE is an ongoing registry of consecutive patients with acute VTE. We categorised patients as having very low- (<80,000/µl), low- (80,000/µl to 150,000/µl), normal- (150,000/µl to 300,000/µl), high- (300,000/µl to 450,000/µl), or very high (>450,000/µl) platelet count at baseline, and compared their three-month outcome. As of October 2012, 43,078 patients had been enrolled in RIETE: 21,319 presenting with pulmonary embolism and 21,759 with deep-vein thrombosis. In all, 502 patients (1.2%) had very low-; 5,472 (13%) low-; 28,386 (66%) normal-; 7,157 (17%) high-; and 1,561 (3.6%) very high platelet count. During the three-month study period, the recurrence rate was: 2.8%, 2.2%, 1.8%, 2.1% and 2.2%, respectively; the rate of major bleeding: 5.8%, 2.6%, 1.7%, 2.3% and 4.6%, respectively; the rate of fatal bleeding: 2.0%, 0.9%, 0.3%, 0.5% and 1.2%, respectively; and the mortality rate: 29%, 11%, 6.5%, 8.8% and 14%, respectively. On multivariate analysis, patients with very low-, low-, high- or very high platelet count had an increased risk for major bleeding (odds ratio [OR]: 2.70, 95% confidence interval [CI]: 1.85-3.95; 1.43 [1.18-1.72]; 1.23 [1.03-1.47]; and 2.13 [1.65-2.75]) and fatal bleeding (OR: 3.70 [1.92-7.16], 2.10 [1.48-2.97], 1.29 [0.88-1.90] and 2.49 [1.49-4.15]) compared with those with normal count. In conclusion, we found a U-shaped relationship between platelet count and the three-month rate of major bleeding and fatal bleeding in patients with VTE.

Quantitative determinations of agglutination of hemocytes from oysters, Crassostrea virginica, by the Lathyrus odoratus lectin at five concentrations revealed that clumping of hemocytes from oysters infected with Perkinsus marinus is partially inhibited. Although the nature of the hemocyte surface saccharide, which is not D(+)-glucose, D(+)mannose, or alpha-methyl-D-mannoside, remains to be determined, it may be concluded that this molecule also occurs on the surface of P. marinus. It has been demonstrated that the panning technique (Ford et al. 1990) is qualitatively as effective for determining the presence of P. marinus in C. virginica as the hemolymph assay method (Gauthier & Fisher 1990).

Objective: To produce age-related normograms for serum antimullerian hormone (AMH) level in infertile women without polycystic ovaries (non-PCO).Design: Retrospective cohort analysis.Setting: Fifteen academic reproductive centers.Patient(s): A total of 3,871 infertile women.Intervention(s): Blood sampling for AMH level.Main Outcome Measure(s): Serum AMH levels and correlation between age and different percentiles of AMH.Result(s): Age-related normograms for the 3rd, 10th, 25th, 50th, 75th, 90th, and 97th percentiles of AMH were produced. We found that the curves of AMH by age for the 3rd to 50th percentiles fit the model and appearance of linear relation, whereas the curves of >75th percentiles fit cubic relation. There were significant differences in AMH and FSH levels and in antral follicle count (AFC) among women aged 24-33 years, 34-38 years, and >= 39 years. Multivariate stepwise linear regression analysis of FSH, age, AFC, and the type of AMH kit as predictors of AMH level shows that all variables are independently associated with AMH level, in the following order: AFC, FSH, type of AMH kit, and age.Conclusion(s): Age-related normograms in non-PCO infertile women for the 3rd to 97th percentiles were produced. These normograms could provide a reference guide for the clinician to consult women with infertility. However, future validation with longitudinal data is still needed. Background and Objectives: Studies show that inflammation can contribute to an increase in resting energy expenditure in patients with chronic kidney disease; however, findings about total energy expenditure (TEE) have not been reported. The aim of this study was to evaluate the effects of inflammation on TEE and physical activity energy expenditure in hemodialysis (HD) patients.Design: This was a cross-sectional study.Setting: This study was conducted from Hopital Edouard Herriot, Lyon, France.Patients: This study included 24 HD patients and 18 healthy subjects.Main Outcome Measure: TEE and step counts were measured over a 7-day period by the SenseWear Pro2 Armband in 24 HD patients (15 patients with C-reactive protein,5 mg/L, aged 67.0 +/- 6 14.7 years, and 9 with C-reactive protein >5 mg/L, aged 69.0 +/- 6 18.0 years) and compared with 18 healthy subjects (62.3 +/- 6 15.3 years).Results: Mean estimated TEE measured with SenseWear Pro2 Armband was significantly lower (25.5 +/- 4.1 kcal/kg/day) in patients with inflammation when compared with those without inflammation (32.0 +/- 6.7 kcal/kg/day) and with healthy subjects (31.8 +/- 6 7.0 kcal/kg/day) (P = .012). There was a difference in the physical activity (step counts) between patient groups (P < .05). Healthy subjects and patients without inflammation walked more (8,107 +/- 5,419 and 6,016 +/- 3,752 steps/day, respectively) as compared with patients with inflammation (2,801 +/- 2,754 steps/day, P = .001).Conclusion: Our findings suggest that patients with inflammation have a lower TEE when compared with healthy subjects and patients without inflammation. INTRODUCTION: The risk that hip preserving surgery may negatively influence the performance and outcome of subsequent total hip replacement (THR) remains a concern. The aim of this study was to identify any negative impact of previous hip arthroscopy on THR. METHODS: Out of 1271 consecutive patients who underwent primary THR between 2005 and 2009, 18 had previously undergone ipsilateral hip arthroscopy. This study group (STG) was compared with two control groups (CG, same approach, identical implants; MCG, paired group matched for age, BMI and Charnley categories). Operative time, blood loss, evidence of heterotopic bone and implant loosening at follow-up were compared between the STG and the MCG. Follow-up WOMAC were compared between the three groups. RESULTS: Blood loss was not found to be significantly different between the STG and MCG. The operative time was significantly less (p < 0.001) in the STG. There was no significant difference in follow-up WOMAC between the groups. No implant related complications were noted in follow-up radiographs. Two minor complications were documented for the STG and three for the MCG. CONCLUSION: We have found no evidence that previous hip arthroscopy negatively influences the performance or short-term clinical outcome of THR.

Els nous implants de genoll pretenen millorar la flexió màxima aconseguida amb els implants convencionals. El nostre estudi pretén demostrar que els pacients intervinguts quirúrgicament, per a la implantació d’una artroplàstia total de genoll d’alta flexió, són capaços d’aconseguir un balanç articular i una flexió màxima superior a l’aconseguida pels pacients tractats amb el model convencional. La flexió màxima post-operatòria, a l’any de la intervenció, va ser 13º major en el grup de pacients portadors d’un implant d’alta flexió, respecte al grup de pacients portadors d’un implant convencional. Estudio clínico prospectivo de 70 pacientes sometidos a tiroidectomía total (TT) El objetivo del estudio es analizar el valor de la determinación de paratirina (PTHi) a las 24 horas de la TT como indicador de riesgo de hipoparatiroidismo definitivo. Cuarenta y cuatro pacientes (62,9%) presentaron hipocalcemia y 27 (38,6%) una deficiencia de PTHi a las 24 h de la TT. Una concentración de PTHi a las 24 h postTT 5,8 pg/mL predice con una sensibilidad del 100% y una especificidad del 81,5% la evolución a hipoparatiroidismo definitivo, con un valor predictivo negativo del 100%. Estudi clínic prospectiu de 70 pacients sotmesos a tiroïdectomia total (TT). L’objectiu de l’estudi és analitzar el valor de la determinació de paratirina (PTHi) a les 24 hores de la TT com a indicador de risc d’hipoparatiroïdisme definitiu. Quaranta-quatre pacients (62,9%) presentaren hipocalcèmia i 27 (38,6%) una deficiència de PTHi a les 24 hores de la TT. Mitjançant un estudi observacional prospectiu, hem descrit i classificat la ferida quirúrgica en el postoperatori immediat de 159 pacients que se’ls hi va implantar una artroplàstia total de genoll, mitjançant una escala descriptiva fiable i reproduïble creada pels autors (estudi de validesa). Es va observar que l’aspecte de la ferida quirúrgica en el postoperatori immediat no tenia relació amb la funcionalitat de la pròtesis als 6 mesos de seguiment ni amb el dolor que patia el pacient en l’estància hospitalària. Si que vam observar correlació estadísticament significativa entre el pitjor aspecte de la ferida i l’aparició d’infeccions protèsiques profundes.

La formació de partícules de polietilè és la principal causa de osteòlisi i per tant de fracàs a l’artroplàstia total de genoll. El nostre objectiu és comparar, in vivo, el nombre de partícules que es formen amb un polietilè convencional amb un altament entrecreuat, esperant que el segon cas el nombre sigui més petit. Utilitzem una tècnica de aïllament de partícules en líquid sinovial ja descrita, que reproduïm a l’estudi preliminar. Presentem un protocol d’estudi prospectiu a doble cec, on s’estudiaran pacients amb artroplàstia de genoll d’un any d’evolució.
