Introduction: There is a growing public perception that serious medical error is commonplace and largely tolerated by the medical profession. The Government and medical establishment's response to this perceived epidemic of error has included tighter controls over practising doctors and individual stick-and-carrot reforms of medical practice. Discussion: This paper critically reviews the literature on medical error, professional socialization and medical student education, and suggests that common themes such as uncertainty, necessary fallibility, exclusivity of professional judgement and extensive use of medical networks find their genesis, in part, in aspects of medical education and socialization into medicine. The nature and comparative failure of recent reforms of medical practice and the tension between the individualistic nature of the reforms and the collegiate nature of the medical profession are discussed. Conclusion: A more theoretically informed and longitudinal approach to decreasing medical error might be to address the genesis of medical thinking about error through reforms to the aspects of medical education and professional socialization that help to create and perpetuate the existence of avoidable error, and reinforce medical collusion concerning error. Further changes in the curriculum to emphasize team working, communication skills, evidence-based practice and strategies for managing uncertainty are therefore potentially key components in helping tomorrow's doctors to discuss, cope with and commit fewer medical errors.

Approximately 200 million people, 5% aged 15-64 worldwide are illicit drug or substance abusers (World Drug Report, 2006). Between 2002 and 2005, an average of 8.2% of 12 year olds and older in the Miami, Fort Lauderdale metropolitan areas used illicit drugs (SAMHSA, 2007). Eight percent of pregnant women, aged 15 to 25, were more likely to have used illicit drugs during pregnancy than pregnant women aged 26 to 44. Alcohol use was 9.8% and cigarette use was 18% for pregnant women aged 15 to 44 (SAMHSA, 2005). Approximately a quarter of annual birth defects are attributed to the exposure of drugs or substance abuse in utero (General Accounting Office, 1991). Physical, psychological and emotional challenges may be present for the illicit drug/substance abuse (ID/SA) mother and infant placing them at a disadvantage early in their relationship (Shonkoff & Marshall, 1990). Mothers with low self efficacy have insecurely attached infants (Donovan, Leavitt, & Walsh, 1987). As the ID/SA mother struggles with wanting to be a good parent, education is needed to help her care for her infant. In this experimental study residential rehabilitating ID/SA mothers peer taught infant massage. Massage builds bonding/attachment between mother and infant (Reese & Storm, 2008) and peer teaching is effective because participants have faced similar challenges and speak the same language (Boud, Cohen, & Sampson 2001). Quantitative data were collected using the General Self-Efficacy and Maternal Attachment Inventory-Revised Scale before and after the 4-week intervention program. A reported result of this study was that empowering ID/SA mothers increased their self-efficacy, which in turn allowed the mothers to tackle challenges encountered and created feelings of being a fit mother to their infants. This research contributes to the existing database promoting evidence-based practice in drug rehabilitation centers. Healthcare personnel, such as nurse educators and maternal-child health practitioners, can develop programs in drug rehabilitation centers that cultivate an environment where the ID/SA rehabilitating mothers can peer teach each other, while creating a support system. Using infant massage as a therapeutic tool can develop a healthy infant and nurture a more positive relationship between mother and infant.

The expansion of the specialty of sports and exercise medicine (SEM) is a relatively recent development in the medical community and the role of the SEM specialist continues to evolve and develop. The SEM specialist is ideally placed to care for all aspects of physical activity not only in athletes but also in the general population. As an advocate for physical activity the SEM specialist plays a broad role in advising safe effective sports and recreation participation; screening for disease related to sports participation; examining and contributing to the evidence behind treatment strategies and evaluating any potential negative impact of sports injury prevention measures. In this thesis I will demonstrate the breadth of the role the Sports and Exercise Medicine Specialist from epidemiology to in-depth examination of treatment strategies. In Chapter 2, I examined the epidemiology of sports and recreation related injury (SRI) in Ireland, an area that has previously been poorly studied. We report on 3,172 SRI (14% of total presentations) presentations to the ED over 6 months. Paediatric patients (4-16 yrs) were over represented comprising 39.9% of all SRI presentation compared to 16% of total ED presentations and 18% of the general population. These injuries were serious (32% fractures) and though 49% of injuries occurred during organised competition/practice, 41.5% occurred during recreation-most often at home. In Chapter 3, I examined risk factors associated with hand injury in hurling. The previous chapter highlighted the importance of a firm evidence base underpinning treatment strategies. When measures to improve welfare are introduced not only must potential benefits be measured, so too must potential unwanted adverse outcomes. In this study I examined a cohort of adult hurlers who had presented to the ED with a hurling related injury in order to highlight the variables associated with hand injury in this population. I found the athletes who wore a helmet were far more likely (OR 3.15 95% CI (1.51-6.56) p= 0.002) to suffer a hand injury than athletes who did not. Very few of those interviewed (4.9%) used hand protection compared to 65% who used helmet and faceguard. The introduction of the helmet and faceguard in hurling has undeniably decreased the incidence of head and face injury in hurling. However in tandem with this intervention several observational studies have demonstrated an increase in the occurrence of hurling related hand injuries. This study highlights the importance of being cognisant of unanticipated or unintended consequences when implementing a new treatment or intervention. In Chapter 4, I examined the role of population screening as applied to sport and exercise. This is a controversial area –cardiac screening in the exercising population has been the subject of much debate. Specifically I define the prevalence of exercise induced bronchoconstriction (EIB) using a specifically designed sports specific field-testing protocol. In this study I found almost a third (29%) of a full international professional rugby squad had confirmed asthma or EIB, as compared with 12-15% of the general population. Despite regular medical screening, 5 ‘new’ untreated cases (12%) were elicited by the challenge test and in the group already on treatment for asthma/EIB; over 50% still displayed EIB. In Chapter 5, I examined the evidence supporting current treatment options for iliotibial band friction syndrome (ITBFS). The practice of sports medicine has traditionally been ‘eminence based’ rather than ‘evidence based’. This may be problematic as some of these practices are based upon flawed principles- for example the treatment of iliotibial band friction syndrome (ITBFS). In this chapter, using cadaveric and biomechanical studies I expand upon the growing base of evidence clarifying the anatomy and biomechanics of the area-thereby re-examining the principles on which current treatments are based. The role of the SEM specialist is broad; we chose to examine specific examples of some of the roles that they execute. An understanding of the epidemiology of SRI presenting to the ED has implications for individual patients, sports governing bodies and health resource utilisation. Population screening is an important tool in health promotion and disease prevention in the general population. Screening in SEM may have similar less well-recognised benefits. The SEM specialist needs to be conversant in screening for medical conditions concerning physical activity. A comprehensive understanding of the pathophysiology of a disease is required for its diagnosis and treatment. Due to the ongoing evolution of SEM many treatments are eminence-based rather than evidence‐based practice. Continued re-examination of the fundamentals of current practice is essential. An awareness of potential unwanted side effects is essential prior to the introduction of any new treatment or intervention. The SEM specialist is ideally placed to advise sports governing bodies on these issues prior to and during their implementation.

El arteterapia permite una aproximación creativa biográfica particularmente valiosa en la etapa final de la vida. La persona enferma presenta múltiples necesidades – físicas, emocionales, sociales y espirituales – que solo una atención holística puede pretender abarcar, tal como lo contempla la filosofía de los cuidados paliativos. El arteterapeuta integrado en el equipo interdisciplinar contribuye a aliviar y acompañar el sufrimiento del paciente y su familia. Se presentan aquí las bases teóricas y la metodología de la intervención, así como el marco sanitario en el cual se inscribe.

Having well-trained staff is key to ensuring good quality autism services, especially since people affected with autism generally tend to have higher support needs than other populations in terms of daily living as well as their mental and physical health. Poorly-trained staff can have detrimental effects on service provision and staff morale and can lead to staff burn-out as well as increased service user anxiety and stress. This paper reports on a survey with health, social care, and education staff who work within the statutory autism services sector in the UK that explored their knowledge and training with regards to autism. Interview data obtained from staff and service users offer qualitative illustrations of survey findings. Overall, the findings expose an acute lack of autism specific training that has detrimental impacts. At best this training was based on brief and very basic awareness raising rather than on in-depth understanding of issues related to autism or skills for evidence-based practice. Service users were concerned with the effects that lack of staff training had on the services they received. The paper concludes with a discussion of policy routes to achieving quality staff training based on international best practice. The focus is on improving the quality of life and mental health for services users and staff as well as making potentially significant cost-savings for governments.

Enquadramento: É expectável que o pós-parto seja um período de alegria. Todavia, nem sempre é assim, já que pode ser um período marcado por momentos de tristeza, cansaço e desânimo. Reconhecendo as vantagens da amamentação para a saúde e bem-estar do recémnascido e da mãe, acreditamos que esta possa ser preventiva da depressão pós-parto. Objetivos: Obter a melhor evidência científica para compreender se a amamentação tem efeito preventivo na depressão pós-parto. Método: Foi efetuada uma revisão integrativa da literatura sobre a relação entre a amamentação e a depressão pós-parto através das bases de dados: EBSCO host, LILACS, PubMed, SciELO, Repositórios institucionais e Google Académico. Selecionaram-se quinze artigos que obedeceram aos critérios de inclusão deste estudo. Resultados: A maioria dos estudos identifica a amamentação como sendo preventiva da depressão pós-parto, destacando a importância da amamentação para a saúde mental da puérpera e consequentemente para a diminuição das hipóteses de desenvolver depressão pósparto. Cinco artigos identificam-na também como sendo um fator de risco e um estudo não encontrou uma associação clara entre a amamentação e a depressão pós-parto. Conclusões: São descritos vários benefícios da amamentação, os quais terão um efeito preventivo contra a depressão pós-parto. Uma mulher informada e apoiada acerca da amamentação será capaz de se sentir confiante no seu novo papel. É essencial um investimento constante por parte da equipa de saúde, iniciado durante o processo do planeamento do casal para a gravidez, e mantendo-se ao longo da gravidez, parto e pós-parto. Objetivo: Identificar estructuras estandarizadas para la formulación de preguntas clínicas en el marco de la práctica basada en la evidencia. Método: Se realizó una revisión de la literatura. La búsqueda bibliográfica se efectuó en las bases de datos MEDLINE, CINAHL, EMBASE y LILACS, y en el buscador académico Google Scholar. Se efectuaron estrategias de búsqueda sensibles y acordes a cada base de datos. Límites de búsqueda: intervalo de tiempo (enero de 1995 a abril de 2015), idioma (artículos en inglés). Se usaron palabras clave libres y descriptores del Medical Subject Headings (MeSH) pertinentes: evidence based practice, question, formulation, well-built question, framework. Resultados: Se encontraron 10 manuscritos que aportaron el diseño original de estructuras para la formulación de preguntas clínicas en el ámbito de la práctica basada en la evidencia. El modelo PICO es la estructura más conocida y comúnmente utilizada en investigación cuantitativa y de él derivan los modelos PICOT, PICOTT, PICOS, PIPOH, PECORD, PESICO. En el campo de la gestión sanitaria la estructura ECLIPSE se erige para formulación de preguntas relacionadas con la gestión. Por último, para la búsqueda de evidencias cualitativas se han configurado los modelos SPICE y SPIDER adecuando sus componentes al fenómeno cualitativo. Conclusiones: Estos modelos estandarizadas se comportan como un instrumento idóneo para guiar la estrategia de búsqueda y delimitar el área de interés. Dada la gran variedad de piezas que integran las estructuras, su conocimiento exhaustivo acrecienta sus usos potenciales. Estas estructuras no deben ser consideradas como una guía rígida a la que ceñirse ineludiblemente.

La détection du délirium à l’aide d’outils est importante pour pouvoir intervenir le plus rapidement et efficacement possible. Les activités de TC retenues étaient une capsule clinique sur les manifestations du délirium évaluées par un ODD et une carte aide-mémoire sur des interventions infirmières possibles. Les taux d’utilisation de l’ODD ont été évalués en pré et post implantation d’une intervention de TC (devis pré-post test) à partir d’une revue de 242 dossiers médicaux de patients (avant n=121 ; après n=121). Aucune différence significative n’a été notée entre les périodes pré et post intervention de TC (p > .99). Une explication réside dans le taux déjà élevé (> 85%) d’utilisation de l’ODD observé avant l’intervention de TC pour deux des trois quarts de travail. L’intervention de TC basée sur les barrières et les facilitateurs a été appréciée par les infirmières et elle pourrait avoir le potentiel de promouvoir une pratique basée sur les résultats probants.

Objetivo: Identificar las barreras para la unificación de una Historia Clínica Electrónica –HCE- en Colombia. Materiales y Métodos: Se realizó un estudio cualitativo. Se realizaron entrevistas semiestructuradas a profesionales y expertos de 22 instituciones del sector salud, de Bogotá y de los departamentos de Cundinamarca, Santander, Antioquia, Caldas, Huila, Valle del Cauca. Resultados: Colombia se encuentra en una estructuración para la implementación de la Historia Clínica Electrónica Unificada -HCEU-. Actualmente, se encuentra en unificación en 42 IPSs públicas en el departamento de Cundinamarca, el desarrollo de la HCEU en el país es privado y de desarrollo propio debido a las necesidades particulares de cada IPS. Conclusiones: Se identificaron barreras humanas, financieras, legales, organizacionales, técnicas y profesionales en los departamentos entrevistados. Se identificó que la unificación de la HCE depende del acuerdo de voluntades entre las IPSs del sector público, privado, EPSs, y el Gobierno Nacional.

Durante os últimos anos tem sido preocupação dos fisioterapeutas conduzir a investigação para melhorar a eficácia, eficiência e efectividade dos tratamentos que prestam e que conduzam aos melhores resultados possíveis, com base na melhor evidência científica disponível. Esta investigação tinha como objectivo principal perceber qual o contributo que a Gestão da Qualidade dá às práticas em fisioterapia, investigar quais as barreiras e obstáculos, que os fisioterapeutas percepcionam na implementação da prática baseada na evidência na sua prática clínica e examinar as atitudes perante a investigação em fisioterapia. A amostra foi constituída por treze fisioterapeutas e a recolha dos dados feita através de uma entrevista semi-estruturada. Verificou-se que no geral, os fisioterapeutas, revelam atitudes e comportamentos positivos relativos à Prática Baseada na Evidência, tendo ainda dificuldade em usá-la a seu favor. Constatou-se que os principais obstáculos á implementação da Prática Baseada na Evidência são de ordem individual, institucional e organizacional. Sendo que o maior obstáculo se prende com a existência de recursos disponíveis para acesso à informação. /ABSTRACT: During the last years it’s been the physical therapists concern to lead the research to improve efficacy, efficiency e effectiveness of treatments which they provide and that they lead to the best achievable results, based in the best scientific evidence available. This research had as its main goal, understand what contribute does the quality management give to the physiotherapy practice, discover which barriers and obstacles are perceived, by the physical therapists, in the evidence-based practice implementation and analyze their attitudes and behavior towards physiotherapy research. The sample was formed by thirteen physical therapists and the data was collected by a semi- structured interview. The results showed that generally, the physical therapists demonstrated positive attitude and behavior regarding Evidence-Based Practice having, nevertheless, difficulties using it in their profit and benefit. The study revealed that the principle obstacles perceived, in the implementation of Evidence-Based Practice are of individual, institutional and organizational order. The biggest obstacle perceived is the availability of resources to achieve information.
