AbstractEpithelial ovarian tumors are the most common malignant ovarian neoplasms and, in most cases, eventual rupture of such tumors is associated with a surgical procedure. The authors report the case of a 54-year-old woman who presented with spontaneous rupture of ovarian cystadenocarcinoma documented by computed tomography, both before and after the event. In such cases, a post-rupture staging tends to be less favorable, compromising the prognosis.

Context: Several studies have been carried out to describe how general practitioners (GPs) follow smoking cessation counseling's recommendations and to identify GP-related factors that influence their practice. But limited information exists about practice organization's elements influencing smoking counseling and more generally about prevention activities in primary care (PC) in Switzerland. Objectives: Explore the feasibility of this study. Investigate determinants of PC practices associated with smoking cessation counseling in Switzerland. Explore the GP's attitude towards prevention and the organizational characteristics of PC practices. Design: Pilot Web-based survey in a representative sample of Swiss French speaking GPs. Results : 44 GP's responded; 66% were men; mean age = 57.5 years; 59% were in group practices; 1 GP was currently smoking and 32% were ex-smokers; 57% of GPs found very important to perform health prevention activities. 57% of GPs were trained (>5 hours throughout the career) to provide smoking cessation counseling. 84% of GP's always inquire about smoking status during first consultations, compared to 69% during emergency consultations for respiratory symptoms and only 9% during non-respiratory emergency consultation. Factors positively associated with smoking status inquiring were: young age (<57.5) of GP's for first consultations, rural location and solo practice for emergency consultations. Smoking cessation counseling is systematically provided by 48% of GPs during emergency consultations for respiratory symptoms and by 20% of GPs during follow-up consultations. Factor positively associated with smoking cessation counseling was solo practice. Discussion: In Switzerland, there are missed opportunities to provide smoking cessation counseling, especially in emergency situations, possibly in people who have rarely contacts with the health care system. Despite a small sample, this study showed the potential associations between organizational and GP-related factors and smoking cessation counseling. These findings will be further explored in a national study among 200 GPs' practices.

L'encéphalite à tique (TBE, tick-borne encephalitis) est une infection à Flavivirus endémique dans 27 pays européens et la maladie virale transmise par les tiques la plus commune en Europe centrale et en Europe de l'est, ce qui en fait un problème majeur de santé publique. En Suisse, 98 à 172 cas par an sont recensés et sont associés avec des symptômes cliniques sévères (bulletin de l'OFSP). Les foyers viraux ont tendance à s'étendre, et de nouveaux apparaissent, probablement à cause des changements climatiques. La lutte la plus efficace contre cette maladie est représentée par la vaccination. Deux vaccins sont disponibles en Europe (FSME-Immun® et Encepur®). Pour les deux, le taux de séroconversion post-vaccination approche les 100% et après trois ans post-vaccination les anticorps sont encore élevés à 95% (98% chez les enfants) pour le FSME-Immun® et de 96 à 100% pour le Encepur®. Néanmoins, des cas d'encéphalite à tique ont été observés chez des patients ayant reçu une vaccination appropriée et réussie, que ce soit des adultes ou des enfants, dont plusieurs cas en Suisse. Ce projet à pour but d'essayer d'identifier les cas de breakthrough du vaccin contre la TBE en Suisse, d'étudier la description clinique, l'histoire vaccinale, les caractéristiques du diagnostic de laboratoire (sérologie et PCR) ainsi que les potentiels facteurs de risque pour un échec du vaccin et ce afin d'estimer l'importance du problème en Suisse. Méthode Identifier les patients ayant développé une encéphalite à tique malgré une anamnèse de vaccination au moyen du système de déclaration obligatoire de l'OFSP. Étudier les dossiers des patients pour détailler l'histoire de vaccination, les signes et symptômes de la maladie, les caractéristiques de laboratoire, en particulier ceux prouvant l'encéphalite à tique (sérologie et PCR). Résultats Le travail de recherche aboutit à la création d'un tableau contenant les informations générales, vaccinales et cliniques, notamment sérologiques, sur les patients recensés. Des tendances sont ainsi tirées sur la base de ce tableau. Toutefois, un tableau exhaustif n'a pas pu être obtenu, en raison du manque parfois important d'informations dans les dossiers médicaux des divers hôpitaux concernés. Discussion-conclusion Il existe, en Suisse du moins, un manque manifeste de critères diagnostiques pour l'encéphalite à tique en situation post-vaccinale. Ceci entraîne une incertitude importante au niveau de l'exactitude du diagnostic de nos patients, étant donné que le vaccin qu'ils ont reçu est réputé avoir une excellente efficacité, proche de 100%.

Objectif : Abstract Le but de cette étude consiste à étudier un éventuel lien entre le dosage du traitement de substitution par la Méthadone® pendant la grossesse et les issues obstétricales (rupture prématurée des membranes, menace d'accouchement prématuré), ainsi que néonatales (telles que le retard de croissance intrautérin, l'adaptation néonatale, le sevrage néonatal aux opiacés et l'hypoglycémie néonatale). Nous évaluerons également le développement psychomoteur de l'enfant à court terme (jusqu'à 18 mois de vie) via l'échelle de Griffiths. Méthode : Il s'agit d'une étude rétrospective sur 50 femmes enceintes sous Méthadone® suivies au CHUV et ayant accouché entre les années 2000 et 2010, ainsi que sur leurs enfants suivis par l'Unité du Développement du CHUV et évalués moyennant l'échelle du développement psychomoteur appelée Griffiths (il s'agit de 26 enfants entre 6-9 mois et 20 entre 18-19 mois). Pour ce faire, nous avons parcouru les différentes archives du CHUV (informatiques et papiers) dans un premier temps. Ces données ont été ensuite saisies dans un tableau Excel avant d'être analysées via STATA. Résumé des résultats : En fonction du dosage de la Méthadone®, 27% (dose plus faible) à 47 % (dose plus élevée) des femmes de notre collectif accouchent prématurément (p = 0.139). 48 % de leurs nouveau-nés présentent un retard de croissance intra-utérin (RCIU). Ce risque est d'autant plus élevé que la Méthadone est faiblement dosée (p = 0.073). Inversement au RCIU, le risque d'hypoglycémie néonatale croît avec la dose maternelle de Méthadone® (p = 0.148). La survenue du syndrome de sevrage néonatal aux opiacés ainsi que sa durée sont significativement plus importantes lorsque le dosage maternel de Méthadone est élevé (p = 0.022 ; p = 0.0118) ou lors de la prise concomitante de benzodiazépines (p = 0.004 ; p = 0.0129). La prise d'autres substances illicites a elle aussi tendance à prolonger le sevrage (p = 0.065). Entre 6-9 mois de vie, il y a plus de microcéphalie (périmètre crânien inférieur au P10) lorsque les enfants reçoivent une dose plus faible in utéro (p = 0.005). Le développement psychomoteur est quant à lui plus favorable lorsque le traitement de substitution est fortement dosé (p = 0.039) et que l'enfant vit chez sa mère biologique (p = 0.050) ou bénéficie d'un contact maternel régulier (p = 0.008). L'effet du dosage de la Méthadone® (p = 0.683) et du lieu de vie (p = 0.211) sur le développement psychomoteur ont néanmoins tendance à s'estomper entre 18-19 mois de vie. Conclusions : Bien qu'un traitement de substitution par la Méthadone hautement dosé augmente la survenue et la durée du syndrome de sevrage néonatal aux opiacés, il y a maintenant des indices pour un meilleur outcome de l'enfant lorsque la substitution est importante (moins de RCIU, de microcéphalie et un développement psychomoteur plus favorable). A propos de l'issue néonatale, tous les enfants nés de mères toxicodépendantes semblent être à risque d'hypoglycémie néonatale. Implications pratiques : Il serait désormais préférable d'augmenter les doses de substitution des futures mères toxicomanes d'autant plus lorsque celles-ci le réclament et tous leurs enfants devraient bénéficier d'une alimentation précoce et de contrôles glycémiques, même s'ils sont eutrophiques.

Fibrose pulmonaire idiopathique La fibrose pulmonaire idiopathique (FPI, Idiopathic Pulmonary Fibrosis) est une maladie diffuse du parenchyme pulmonaire de cause inconnue caractérisée par une aggravation de la dyspnée, une réduction du volume pulmonaire et une altération des échanges gazeux. Elle est la plus fréquente de pneumopathies interstitielles idiopathiques (IIPs, Idiopathic Interstitial Pneumonias), avec une incidence annuelle estimée entre 4,6 et 16,3 pour 100'000 personnes et une prévalence de 13 à 20 cas pour 100 000 personnes. Son évolution est progressive, irréversible et de mauvais pronostic avec une survie médiane de 2 à 3.5 ans et une survie à 5 ans de moins de 20%.

PURPOSE: In obesity prevention, understanding psychosocial influences in early life is pivotal. Reviews reported contradictory results and a lack of longitudinal studies focusing on underlying lifestyle factors. This study tested whether psychosocial Quality-Of-Life (QOL) was associated with pre-schoolers' lifestyle and adiposity changes over one school year and whether lifestyle moderated the latter. It was hypothesised that QOL might not impact adiposity in everybody but that this might depend on preceding lifestyle. METHOD: Longitudinal data from 291 Swiss pre-schoolers (initially 3.9-6.3 years) was available. The following measures were used in longitudinal regressions: psychosocial QOL by PedsQL, adiposity (BMI z-score, waist, fat%), diet (food frequency), sedentary time and accelerometer-based activity. RESULTS: Concerning lifestyle, low psychosocial QOL was only related to unfavourable changes in diet (less fruit β = 0.21 and more fat intake β = -0.28) and lower physical activity (β = 0.21). Longitudinal QOL-adiposity relations appeared only after moderation by lifestyle factors (beta-range 0.13-0.67). Low psychosocial QOL was associated with increased adiposity in children with an unhealthy diet intake or high sedentary time. By contrast, low psychosocial QOL was associated with decreasing adiposity in high fruit consumers or more physically active pre-schoolers. CONCLUSION: Results emphasise the need for testing moderation in the QOL-adiposity relation. An unhealthy diet can be a vulnerability factor and high physical activity a protective factor in QOL-related adiposity. Consequently, QOL and lifestyle should be targeted concurrently in multi-factorial obesity prevention. The environment should be an 'activity encouraging, healthy food zone' that minimises opportunities for stress-induced eating. In addition, appropriate stress coping skills should be acquired.

The spectrophotometric determination of Cd(II) using a flow injection system provided with a solid-phase reactor for cadmium preconcentration and on-line reagent preparation, is described. It is based on the formation of a dithizone-Cd complex in basic medium. The calibration curve is linear between 6 and 300 µg L-1 Cd(II), with a detection limit of 5.4 µg L-1, an RSD of 3.7% (10 replicates in duplicate) and a sample frequency of 11.4 h-1. The proposed method was satisfactorily applied to the determination of Cd(II) in surface, well and drinking waters.

A flow system coupled to a tungsten coil atomizer in an atomic absorption spectrometer (TCA-AAS) was developed for As(III) determination in waters, by extraction with sodium diethyldithiocarbamate (NaDDTC) as complexing agent, and by sorption of the As(III)-DDTC complex in a micro-column filled with 5 mg C18 reversed phase (10 µL dry sorbent), followed by elution with ethanol. A complete pre-concentration/elution cycle took 208 s, with 30 s sample load time (1.7 mL) and 4 s elution time (71 µL). The interface and software for the synchronous control of two peristaltic pumps (RUN/ STOP), an autosampler arm, seven solenoid valves, one injection valve, the electrothermal atomizer and the spectrometer Read function were constructed. The system was characterized and validated by analytical recovery studies performed both in synthetic solutions and in natural waters. Using a 30 s pre-concentration period, the working curve was linear between 0.25 and 6.0 µg L-1 (r = 0.9976), the retention efficiency was 94±1% (6.0 µg L-1), and the pre-concentration coefficient was 28.9. The characteristic mass was 58 pg, the mean repeatability (expressed as the variation coefficient) was 3.4% (n=5), the detection limit was 0.058 µg L-1 (4.1 pg in 71 µL of eluate injected into the coil), and the mean analytical recovery in natural waters was 92.6 ± 9.5 % (n=15). The procedure is simple, economic, less prone to sample loss and contamination and the useful lifetime of the micro-column was between 200-300 pre-concentration cycles.

Stable carbon and nitrogen isotopes in skin and bone of South American sea lions from Brazil and Uruguay were analysed to test the hypothesis that trophic overlap between the sexes is lower during the pre-breeding season than throughout the rest of the year. The isotopic values of skin and bone were used to infer the trophic relationships between the sexes during the pre-breeding period and year round, respectively. Prey species were also analysed to establish a baseline necessary for interpreting the stable isotope ratios of skin and bone. Standard ellipse areas, estimated using Bayesian inference in the SIBER routine of the SIAR package in R, suggested that males and females used a wide diversity of foraging strategies throughout the year and that no differences existed between the sexes. However, the diversity of foraging strategies was largely reduced during the pre-breeding period, with all the individuals of each sex adopting similar strategies, but with the two sexes differing considerably in stable isotope values and the ellipse areas of males and females not overlapping at all. Nevertheless, the results revealed a general increase in the consumption of pelagic prey by both sexes during the pre-breeding period. The progressive crowding of individuals in the areas surrounding the breeding rookeries during the pre-breeding period could lead to an increase in the local population density, which could explain the above reported changes.

The risk of cardiovascular diseases and sleep-disordered breathing increases after menopause. This cross-sectional study focuses on overnight transcutaneous carbon dioxide (TcCO2) measurements and their power to predict changes in the early markers of cardiovascular and metabolic diseases. The endothelial function of the brachial artery, the intima-media thickness of the carotid artery, blood pressure, glycosylated hemoglobin A1C and plasma levels of cholesterols and triglycerides were used as markers of cardiovascular and metabolic diseases. The study subjects consisted of healthy premenopausal women of 46 years of age and postmenopausal women of 56 years of age. From wakefulness to sleep, the TcCO2 levels increased more in postmenopausal women than in premenopausal women. In estrogen-users the increase in TcCO2 levels was even more pronounced than in other postmenopausal women. From the dynamic behaviour of the nocturnal TcCO2 signal, several important features were detected. These TcCO2 features had a remarkable role in the prediction of endothelial dysfunction and thickening of the carotid wall in healthy premenopausal women. In addition, these TcCO2 features were linked with blood pressure, lipid profile and glucose balance in postmenopausal women. The nocturnal TcCO2 profile seems to contain significant information, which is associated with early changes in cardiovascular diseases in middle-aged women. TcCO2 might not only measure the tissue carbon dioxide levels, but the TcCO2 signal variation may also reflect peripheral vasodynamic events caused by increased sympathetic activity during sleep.

Atherosclerosis is a vascular inflammatory disease causing coronary artery disease, myocardial infarct and stroke, the leading causes of death in Finland and in many other countries. The development of atherosclerotic plaques starts already in childhood and is an ongoing process throughout life. Rupture of a plaque and the following occlusion of the vessel is the main reason for myocardial infarct and stroke, but despite extensive research, the prediction of rupture remains a major clinical problem. Inflammation is considered a key factor in the vulnerability of plaques to rupture. Measuring the inflammation in plaques non-invasively is one potential approach for identification of vulnerable plaques. The aim of this study was to evaluate tracers for positron emission tomography (PET) imaging of vascular inflammation. The studies were performed with a mouse model of atherosclerosis by using ex vivo biodistribution, autoradiography and in vivo PET and computed tomography (CT). Several tracers for inflammation activity were tested and compared with the morphology of the plaques. Inflammation in the atherosclerotic plaques was evaluated as expression of active macrophages. Systematic analysis revealed that the uptake of 18F-FDG and 11C-choline, tracers for metabolic activity in inflammatory cells, was more prominent in the atherosclerotic plaques than in the surrounding healthy vessel wall. The tracer for αvβ3 integrin, 18Fgalacto- RGD, was also found to have high potential for imaging inflammation in the plaques. While 11C-PK11195, a tracer targeted to receptors in active macrophages, was shown to accumulate in active plaques, the target-to-background ratio was not found to be ideal for in vivo imaging purposes. In conclusion, tracers for the imaging of inflammation in atherosclerotic plaques can be tested in experimental pre-clinical settings to select potential imaging agents for further clinical testing. 18F-FDG, 18F-galacto-RGD and 11C-choline choline have good properties, and further studies to clarify their applicability for atherosclerosis imaging in humans are warranted.

To assess the psychological and family factors associated with suicidal ideation in preadolescent children, we studied a sample of 361 students, average age 9 years old. Twogroups were formed, on the basis of the presence (n = 34) or absence (n = 44) of suicidal ideation. Suicidal ideation was assessed with the Children’s Depression Inventory and the Children’s Depression Rating Scale-Revised. Depression, hopelessness, self-esteem, and perceived family environment were compared in both the suicidal ideation and the control groups. Students with suicidal ideation generally presented greater depressive symptoms and hopelessness, and lower self-esteem and family expressiveness, although there weredifferences both between sexes, and when the variable depression was controlled. Identifying these risk factors in pre-adolescents may have an impact on prevention of suicidal behavior at higher risk ages

This article offers a panorama of mathematics training for future teachers at pre-school level in Spain. With this goal in mind, this article is structured infour sections: where we come from, where we are, where we’re going and where we want to go. It offers, in short, a brief analysis that shows the efforts made to ensure there is sufficient academic and scientific rigour in teachers’ studies at pre-school in general and students’ mathematics education in particular. Together with a description of the progress made in recent years, it also raises some questions for all those involved in training future teachers for this educational stage
