Soft-tissue and bone necrosis, although rare in childhood, occasionally occur in the course of infectious diseases, either viral or bacterial, and seem to be the result of hypoperfusion on a background of disseminated intravascular coagulation. Treatment consists in correction of septic shock and control of necrosis. Necrosis, once started, shows extraordinarily rapid evolution, leading to soft-tissue and bone destruction and resulting in anatomic, functional, psychological, and social handicaps. Ten mutilated children were treated from January 1986 to January 1999 in Hospital de Dona Estefaˆ nia, Lisbon, Portugal. One was recovering from hemolytic-uremic syndrome with a severe combined immunodeficiency, another malnourished, anemic child had malaria, and three had chicken pox (in one case complicated by meningococcal septicemia). There were three cases of meningococcal and two of pyocyanic septicemia (one in a burned child and one in a patient with infectious mononucleosis). The lower limbs (knee,leg, foot) were involved in five cases, the face (ear, nose, lip) in four, the perineum in three, the pelvis (inguinal region, iliac crest) in two, the axilla in one, and the upper limb (radius, hand) in two. Primary prevention is based on early recognition of risk factors and timely correction. Secondary prevention consists of immediate etiologic and thrombolytic treatment to restrict the area of necrosis. Tertiary prevention relies on adequate rehabilitation with physiotherapy and secondary operations to obtain the best possible functional and esthetic result.

Introduction: bronchial asthma is a chronic disease that affects a high percentage of adolescents, with a significant restriction of daily activities, and is a cause of school absenteeism. The relationships between adolescents and asthma disease in school were assessed, with a view to improving knowledge about the asthmatic adolescent. Methods: a survey was conducted in the Lisbon metropolitan area, covering urban (Lisbon) and rural (Lourinh˜a ) zones and including 1879 students and 81 teachers from the 7th to 9th high school years. The study groups were asthmatic students, their peers, and teachers. A self-administered questionnaire was applied to collect information. The results were compared with a reference group of 91 asthmatic students attending our Department of Immunoallergy-Hospital Dona Estefânia. Cotinine urinary measurements were made in a sample of asthmatics and a control group. Results: the prevalence of current asthma among students was 10%. Estimates of asthma annual burden among 7th to 9th year students from Lisbon and Lourinh˜a high schools included 4,307 days missed from school, 4,148 medical consultations and a minimum of 351 hospital emergency care and 80 hospital admissions. Exposure to passive smoking was not significantly different between asthmatic students and theirs peers. Cotinine urinary measurements did not discriminate between exposed and non-exposed individuals. Cigarette smoking was almost as common among adolescent asthmatics (5.4%) as it was in non-asthmatic subjects (6.7%). However, 55% of asthmatics mentioned active and passive smoking as an asthma exacerbating factor. Asthmatic students, theirs peers and teachers showed a deficient knowledge about asthma (mean group scores: 17.6; 14.2 and 17.7 of a possible 30), particularly in the areas related to asthma recognition and its management. Asthmatics attending our Allergy Department had the highest scores. All groups showed tolerance in the sense of a positive and understanding attitude toward a person with asthma. However, traditional beliefs about asthma disease (dependence, inferiority...) were confirmed. A positive correlation between knowledge levels and tolerance attitudes was found. Conclusion: in view of the dimension of the asthma problem in adolescence and its social and economic impact, it is justifiable to assess the need for the implementation of asthma education programs in schools in order to improve asthma management by the adolescents and their schools.

Objective: To compare measurements of the upper arm cross-sectional areas (total arm area,arm muscle area, and arm fat area of healthy neonates) as calculated using anthropometry with the values obtained by ultrasonography. Materials and methods: This study was performed on 60 consecutively born healthy neonates: gestational age (mean6SD) 39.661.2 weeks, birth weight 3287.16307.7 g, 27 males (45%) and 33 females (55%). Mid-arm circumference and tricipital skinfold thickness measurements were taken on the left upper mid-arm according to the conventional anthropometric method to calculate total arm area, arm muscle area and arm fat area. The ultrasound evaluation was performed at the same arm location using a Toshiba sonolayer SSA-250AÒ, which allows the calculation of the total arm area, arm muscle area and arm fat area by the number of pixels enclosed in the plotted areas. Statistical analysis: whenever appropriate, parametric and non-parametric tests were used in order to compare measurements of paired samples and of groups of samples. Results: No significant differences between males and females were found in any evaluated measurements, estimated either by anthropometry or by ultrasound. Also the median of total arm area did not differ significantly with either method (P50.337). Although there is evidence of concordance of the total arm area measurements (r50.68, 95% CI: 0.55–0.77) the two methods of measurement differed for arm muscle area and arm fat area. The estimated median of measurements by ultrasound for arm muscle area were significantly lower than those estimated by the anthropometric method, which differed by as much as 111% (P,0.001). The estimated median ultrasound measurement of the arm fat was higher than the anthropometric arm fat area by as much as 31% (P,0.001). Conclusion: Compared with ultrasound measurements using skinfold measurements and mid-arm circumference without further correction may lead to overestimation of the cross-sectional area of muscle and underestimation of the cross-sectional fat area. The correlation between the two methods could be interpreted as an indication for further search of correction factors in the equations.

INTRODUCTION: Chylothorax is a rare but serious postoperative condition in children with congenital heart disease. Conventional medical treatment consists of specific long-term dietary modification, and surgical reintervention, such as lymphatic duct ligation, may be indicated in refractory cases. In recent years, an additional conservative treatment, octreotide, a synthetic analog of somatostatin, has been used in management of congenital and postoperative chylothorax. METHODS: The objective of this work was to analyze the efficacy and safety of this treatment for chylothorax after congenital heart surgery. We reviewed the records of sixteen patients with chylothorax after surgery for congenital heart disease between January 1999 and December 2007, and collected the following data: demographic information; type of surgical procedure; onset, duration and management of chylothorax and treatment; and duration of hospital stay. To analyze efficacy we compared these parameters in children receiving conventional treatment only with those receiving octreotide. To analyze safety we compared the adverse effects of both treatments. Octreotide was administered at a dose of 4 to 10 microg/kg/hour, with monitoring of side effects. RESULTS: The incidence of chylothorax in our population was 1.6%. It occurred more often after Glenn and Fontan procedures (8 patients). Octreotide was begun three days after diagnosis of chylothorax and continued for a median of seventeen days (ranging from 4 to 26 days), until complete resolution. Side effects were frequent (in 3 of the 8 patients) but of no clinical relevance. All patients responded to the therapy and there was no indication for further surgical intervention. DISCUSSION AND CONCLUSIONS: Octreotide is safe and effective in the treatment of postoperative chylothorax in children with congenital heart disease. It is a useful adjunctive therapy to the conventional treatment of this complication.

O AA, após definir a U.I como um sintoma, um sinal e uma condição ilustra com estudos epidemiológicos que estamos perante um problema médico, social e económico maior. Por outro lado a fisioterapia da U.I. é explicada no contexto do equilíbrio entre as forças expulsivas e retencionistas na fase de enchimento. Deste modo explica a U.I por urgência devido a bexiga hiperactiva e a U.I. ao stress por hipermobilidade do colo da bexiga e ou lesão do esfíncter intrínseco. O diagnóstico e a severidade destes diferentes tipos de U.I. é apresentado tendo como base: a história clínica; o pad teste ; o registo diário demicções; o Q tip test. Os exames complementares como: O Rx simples do aparelho urinário; a urinocultura; os estudos urodinâmicos; a eco transvaginal avaliam os factores de continência alterados. O tratamento da U.I. é exposto tendo como base a fisiopatologia, e exemplificando de uma forma sistemática com casos clínicos.

Apresenta-se um caso clínico de angiomiolipoma renal complicado por hemorragia, numa doente de 44 anos. Avalia-se o contributo diagnóstico e terapêutico da arteriografia, que neste caso permitiu uma apreciação rigorosa da vascularização das lesões e resolveu a situação hemorrágica através da embolização trans-catéter percutânea.

Exame físico (incluindo toque rectal). Ultrasonografia renal e da bexiga e/ou UIV Cistoscopia com descrição de tamanho, e localização do tumor (o diagrama da bexiga deve ser incluído). Urina Tipo II; Citologia urinária, RTU com biópsia da base tumoral. Biópsias de todas as áreas suspeitas; biópsias randomizadas na presença da citologia positiva, tumor >3cm, ou tumor não papilar; biópsia da uretra prostática em casos de Cis ou suspeita de carcionoma in situ. Quando o tumor da bexiga é invasivo e está indicado um tratamento radical, é mandatório RX do tórax, UIV e/ou tomografia axial computorizada abdominal e pélvica, Ultasonografia hepática, cintigrafia óssea se houver sintomas ou se fosfatase alcalina for elevada.

Apresentamos o caso clínico de um doente de vinte e um anos, sexo masculino, raça branca, toxicodependente, que se queixou, no Serviço de Urgência, de febre, emagrecimento, dor abdominal e diarreia. Foi excluído o diagnóstico inicial de apendicite. Baseados nos estudos radiológicos e endoscópicos, foi feito o diagnóstico de doença de Crohn ileo-cecal e iniciado tratamento com ácido 5-aminosalicílico e corticosteroides. Dada a má resposta clínica, foi adicionada 5-mercaptopurina. Seis semanas mais tarde, o doente ficou neutropénico e a febre reapareceu. Os radiogramas e tomografia computorizada (TC) revelaram um derrame pleural direito e uma lesão cavitada nodular no segmento posterior do lobo superior do pulmão direito. Foi realizada uma biópsia pleural e foram identificados bacilos álcool-ácido resistentes (BAAR). Foi iniciado tratamento específico para a tuberculose e as imagens pulmonares desapareceram, mas a febre e as dores abdominais persistiram.
