Since 11 September 2001, the religious dimension of conflict has been the focus of increasing attention. In The Clash of Civilizations, Huntington has identified the West in religious-cultural terms, as Christian with a dominant democratic culture emphasizing tolerance, moderation and consensus. The persistence of conflict in Northern Ireland among 'White' Protestant and Catholic Christians undermines this simplistic argument and demands a more subtle understanding of the role of religion and fundamentalism in contemporary conflict. Modernization theory - which is echoed among some theorists of globalization - had predicted the declining importance of religion as the world became industrialized and increasingly interconnected. This is echoed by those who argue that the Northern Ireland conflict is 'ethno-national' and dismiss the role of religion. On the other hand, others have claimed that the conflict is religious and stress the role of Protestant fundamentalism. This article draws on new evidence from Northern Ireland of the complex and subtle ways in which religion impacts on the conflict there, incorporating insights about the pragmatism of fundamentalist Protestants and how religious actors are contributing to conflict transformation. This analysis leads to three broader conclusions about understanding conflicts with religious dimensions. First, the complexity of religion must be understood, and this includes a willingness to recognize the adaptability of fundamentalisms to particular contexts. Second, engaging with fundamentalists and taking their grievances seriously opens up possibilities for conflict transformation. Third, governments and religious actors within civil society can play complementary roles in constructing alternative (religious) ideologies and structures as part of a process of transformation. In a world in which the impact of religion is persistent, engaging with the religious dimension is a vital part of a broader-based strategy for dealing with conflict. © 2008 Journal of Peace Research.

OBJECTIVES: To demonstrate how individual participant data (IPD) meta-analyses have impacted directly on the design and conduct of trials and highlight other advantages IPD might offer.

STUDY DESIGN AND SETTING: Potential examples of the impact of IPD meta-analyses on trials were identified at an international workshop, attended by individuals with experience in the conduct of IPD meta-analyses and knowledge of trials in their respective clinical areas. Experts in the field who did not attend were asked to provide any further examples. We then examined relevant trial protocols, publications, and Web sites to verify the impacts of the IPD meta-analyses. A subgroup of workshop attendees sought further examples and identified other aspects of trial design and conduct that may inform IPD meta-analyses.

RESULTS: We identified 52 examples of IPD meta-analyses thought to have had a direct impact on the design or conduct of trials. After screening relevant trial protocols and publications, we identified 28 instances where IPD meta-analyses had clearly impacted on trials. They have influenced the selection of comparators and participants, sample size calculations, analysis and interpretation of subsequent trials, and the conduct and analysis of ongoing trials, sometimes in ways that would not possible with systematic reviews of aggregate data. We identified additional potential ways that IPD meta-analyses could be used to influence trials.

CONCLUSIONS: IPD meta-analysis could be better used to inform the design, conduct, analysis, and interpretation of trials.

AIMS: Rheumatic heart disease (RHD) accounts for over a million premature deaths annually; however, there is little contemporary information on presentation, complications, and treatment.

METHODS AND RESULTS: This prospective registry enrolled 3343 patients (median age 28 years, 66.2% female) presenting with RHD at 25 hospitals in 12 African countries, India, and Yemen between January 2010 and November 2012. The majority (63.9%) had moderate-to-severe multivalvular disease complicated by congestive heart failure (33.4%), pulmonary hypertension (28.8%), atrial fibrillation (AF) (21.8%), stroke (7.1%), infective endocarditis (4%), and major bleeding (2.7%). One-quarter of adults and 5.3% of children had decreased left ventricular (LV) systolic function; 23% of adults and 14.1% of children had dilated LVs. Fifty-five percent (n = 1761) of patients were on secondary antibiotic prophylaxis. Oral anti-coagulants were prescribed in 69.5% (n = 946) of patients with mechanical valves (n = 501), AF (n = 397), and high-risk mitral stenosis in sinus rhythm (n = 48). However, only 28.3% (n = 269) had a therapeutic international normalized ratio. Among 1825 women of childbearing age (12-51 years), only 3.6% (n = 65) were on contraception. The utilization of valvuloplasty and valve surgery was higher in upper-middle compared with lower-income countries.

CONCLUSION: Rheumatic heart disease patients were young, predominantly female, and had high prevalence of major cardiovascular complications. There is suboptimal utilization of secondary antibiotic prophylaxis, oral anti-coagulation, and contraception, and variations in the use of percutaneous and surgical interventions by country income level.

Understanding the spatial distribution of organisms is a central topic in ecology. The European roe deer (Capreolus capreolus) population is in Portugal and Norway at the southwestern and northern edge of its distribution, respectively. Understanding the factors that act on these populations enlightens both local aspects concerning their conservation and wider scale aspects of the species bioclimatic envelope, which is crucial for being better able to predict the impacts of environmental change. The main aim of this thesis was to evaluate roe deer distribution in Portugal and Norway, two countries with contrasting landscapes, seasonality and with different anthropogenic pressure. The interspecific relationship with sympatric ungulates was also analysed. By using pellet group counts, we investigated habitat use of roe deer, identifying the major environmental descriptors, to understand the importance of forest structure, vegetation characteristics, landscape structure and human disturbance on their distribution. The analyses were based on presence – absence data and were carried out at two spatial scales. The results showed that habitat use of roe deer was different across countries. In Portugal, at the local scale, roe deer distribution was positively associated with high density of shrubs, especially heather and brambles, while the presence of red deer had a negative effect on their distribution. At a broad scale, roe deer was negatively associated with spatial heterogeneity, namely mean shape index and made less use of areas close to agricultural fields. In Norway, at the local scale, roe deer made more use of areas with high cover of deciduous trees and patches containing juniper and Vaccinium sp.. At a broad scale, roe deer use patches near edges between fields and forest. In both countries, roe deer make use of areas further away from roads. While in Norway roe deer in both summer and winter are always close to houses, in Portugal they are either far (summer) or indifferent (winter). Anthropogenic disturbance is better tolerated in Norway, where the importance of the critical season seems to be higher. Human disturbance may contribute to roe deer habitat loss in Portugal, while roe deer are able to persist close to humans in managed landscapes in Norway. In fact, some of the differences observed could be more due to the indirect impacts of human exploitation (e.g. presence of free-ranging dogs and hunting regulation) rather than the actual human presence or land-use per se. I conclude that the methodology and tools developed here are readily expandable to address similar questions in different contexts. Wildlife management would benefit greatly from a more holistic/integrative approach and that should include human aspects, as human disturbance is expected to continue increasing.

«Realismo e Lirismo em Terra Sonâmbula e Chuva Braba» é um trabalho de leitura que reflecte a nossa percepção em relação a dois mundos particulares que se constroem a partir das obras dos dois escritores africanos. Com efeito, optamos por uma estrutura pragmática do estudo, centrando a nossa atenção na leitura e interpretação dos romances, sem incluirmos um capítulo específico de referências teóricas. Tal estratégia permitiu cruzar o quadro conceptual com as informações textuais resultantes do processo de análise e interpretação do «corpus» do trabalho. As duas obras estabelecem pontos de intersecção no domínio linguístico e cultural como consequência de partilha de um passado histórico, político e social. A localização geográfica de Cabo Verde, a fome prolongada, por um lado, e a guerra catastrófica que abalou Moçambique entre 1976 e 1992, por outro, permitiram extrapolar recorrências temáticas inspiradas em impressões e experiências dos autores, relacionadas com práticas e vivências que, no trato literário, ganharam uma dimensão lírico-realista de grande valor hermenêutico. A insularidade e a continentalidade que opõem Cabo Verde e Moçambique, assim como a fome e a guerra que os caracterizam respectivamente, a procura de um espaço literário a partir das marcas de crioulidade e moçambicanidade compõem um conjunto de valores estéticos que configuram o imaginário cultural dos dois países africanos de língua portuguesa. Esta tese pesquisa as imagens e os aspectos fundamentais ínsitos nos dois romances, procurando mostrar até que ponto, a partir de temáticas de fome e guerra se pode construir narrativas lírico-realistas. O estudo permitiu observar que as imagens de sofrimento, desolação e desassossego constituem, geralmente, o paradigma estético da escrita lírica e realista de Mia Couto e Manuel Lopes.

The world of competitive sport affords an individual the opportunity to enter a spiritual community adding meaning that transcends one’s current understanding of life (Parry et al. 2007). Previously established dimensions of sports fanship (group affiliation, psychological commitment and team identification) share characteristics commonly associated with religious or spiritual affiliation indicating that fans may generate substantial life meaning from observing sporting encounters. In the present study, 12 male basketball fans (M=32.42; SD=7.97) completed semi-structured interviews immediately prior to viewing a competitive match at the 2011 European Basketball Championships (Vilnius, Lithuania). Interviews were structured under four headings; 1) the most memorable moment as a basketball fan, 2) thoughts, feelings and emotions attached to their team, 3) affiliation to the team in the context of the fan’s meaning of life, 4) connection with members of the fan’s sporting community. Interpretative Phenomenological Analysis was employed to analyse interview transcripts. Raw data clustered into four dimensions; 1) devotion, 2) obscure emotions, 3) connectedness, and 4) universal values. The results indicate that sports fanship is characterised by, and synonymous with, an established understanding of spirituality derived from membership of wider spiritual and/or religious communities. The findings hold implications for the marketing and membership of local, amateur and professional sports clubs and brands, as well as community development, health and welfare.

This thesis offers an original account of what personal well-being can be. Any account of education, it is believed, has to do with and aims at personal well-being. I approach this view on well-being not in a positive but in a negative way. I put forward some items that in certain circumstances can be taken by and called sources or forms of disorder. In the absence of such forms or sources of disorder, I assume that a certain order, prudential or moral, takes place and that constitutes the well-being of the person. The concept of ‘absence of disorder’ is introduced and argued as an educationally appropriate view of personal well-being which is the central educational aim. Therefore, ‘absence of disorder’ is positioned as the central aim of education. This concept is illuminated, for practical reasoning, by a list of seven possible forms of disorder: Comparison, Corruption, Dependency, Division, Fear, Self-disintegration and Violence. As a view of personal well-being, ‘absence of disorder’ is initially rooted in informed desire satisfaction, via the introduction of the concept of entropy. Prudentially, the agent’s informed desire is satisfied by living a life with low build up of entropy or disorder. But, in a second move such a base is also provided by the Levinasinian concept of ‘disinterest’ as a root for ‘what is to be a human’. Such ‘disinterest’ is related to the concepts of love and of ‘action for its own sake’. It is at this final approach that an attempt is made towards the approximation of the ethical and the prudential aspects of social practices. Even if only to some extent successful, the argument is directed to the following conclusion: an education aiming at ‘absence of disorder’ may promote prudential well-being and give us some confidence in simultaneously favouring moral education.

Eugénio de Andrade (1923-2005) é um dos mais celebrados e traduzidos poetas portugueses contemporâneos, com mais de trinta volumes de poesia, e vários prémios nacionais e internacionais. Como poeta atento e turista curioso, Andrade viajou com frequência para Espanha, França, Itália e Grécia, ao encontro de outros escritores (Vicente Aleixandre, Dámaso Alonso, etc.), e visitou diversas cidades e locais históricos. Recorrentemente, menciona Madrid, Valverde del Fresno, Roma, Delfos, Tebas, Súnion, Veneza, Brindisi, Corfu, etc. Em resultado destas viagens, existem, na sua poesia, poemas em prosa e crónicas, abundantes referências aos países, cultura e paisagem natural do Mediterrâneo. Eugénio de Andrade captura poeticamente o ―genius loci‖, realçando os laços entre os povos, a fauna, a flora e o clima. Desde os primeiros escritos até ao seu último livro, o poeta coerentemente apresenta uma ―visão mediterrânica‖. Recorrendo às suas obras e a excertos de algumas das entrevistas que concedeu, este artigo exemplifica, analisa e avalia estes aspectos, explorando uma faceta menos conhecida da obra eugeniana.

BACKGROUND: Recommendations for statin use for primary prevention of coronary heart disease (CHD) are based on estimation of the 10- year CHD risk. We compared the 10-year CHD risk assessments and eligibility percentages for statin therapy using three scoring algorithms currently used in Europe. METHODS: We studied 5683 women and men, aged 35-75, without overt cardiovascular disease (CVD), in a population-based study in Switzerland. We compared the 10-year CHD risk using three scoring schemes, i.e., the Framingham risk score (FRS) from the U.S. National Cholesterol Education Program's Adult Treatment Panel III (ATP III), the PROCAM scoring scheme from the International Atherosclerosis Society (IAS), and the European risk SCORE for low-risk countries, without and with extrapolation to 60 years as recommended by the European Society of Cardiology guidelines (ESC). With FRS and PROCAM, high-risk was defined as a 10- year risk of fatal or non-fatal CHD>20% and a 10-year risk of fatal CVD≥5% with SCORE. We compared the proportions of high-risk participants and eligibility for statin use according to these three schemes. For each guideline, we estimated the impact of increased statin use from current partial compliance to full compliance on potential CHD deaths averted over 10 years, using a success proportion of 27% for statins. RESULTS: Participants classified at high-risk (both genders) were 5.8% according to FRS and 3.0% to the PROCAM, whereas the European risk SCORE classified 12.5% at high-risk (15.4% with extrapolation to 60 years). For the primary prevention of CHD, 18.5% of participants were eligible for statin therapy using ATP III, 16.6% using IAS, and 10.3% using ESC (13.0% with extrapolation) because ESC guidelines recommend statin therapy only in high-risk subjects. In comparison with IAS, agreement to identify eligible adults for statins was good with ATP III, but moderate with ESC. Using a population perspective, a full compliance with ATP III guidelines would reduce up to 17.9% of the 24′ 310 CHD deaths expected over 10 years in Switzerland, 17.3% with IAS and 10.8% with ESC (11.5% with extrapolation). CONCLUSIONS: Full compliance with guidelines for statin therapy would result in substantial health benefits, but proportions of high-risk adults and eligible adults for statin use varied substantially depending on the scoring systems and corresponding guidelines used for estimating CHD risk in Europe.

Approximately 1 million people in the United States and over 30 million worldwide are living with human immunodeficiency virus type 1 (HIV-1). While mortality from untreated infection approaches 100%, survival improves markedly with use of contemporary antiretroviral therapies (ART). In the United States, 25 drugs are approved for treating HIV-1, and increasing numbers are available in resource-limited countries. Safe and effective ART is a cornerstone in the global struggle against the acquired immunodeficiency syndrome. Variable responses to ART are due at least in part to human genetic variants that affect drug metabolism, drug disposition, and off-site drug targets. Defining effects of human genetic variants on HIV treatment toxicity, efficacy, and pharmacokinetics has far-reaching implications. In 2010, the National Institute of Allergy and Infectious Diseases sponsored a workshop entitled, Pharmacogenomics A Path Towards Personalized HIV Care. This article summarizes workshop objectives, presentations, discussions, and recommendations derived from this meeting.

The adult hippocampus generates functional dentate granule cells (GCs) that release glutamate onto target cells in the hilus and cornus ammonis (CA)3 region, and receive glutamatergic and γ-aminobutyric acid (GABA)ergic inputs that tightly control their spiking activity. The slow and sequential development of their excitatory and inhibitory inputs makes them particularly relevant for information processing. Although they are still immature, new neurons are recruited by afferent activity and display increased excitability, enhanced activity-dependent plasticity of their input and output connections, and a high rate of synaptogenesis. Once fully mature, new GCs show all the hallmarks of neurons generated during development. In this review, we focus on how developing neurons remodel the adult dentate gyrus and discuss key aspects that illustrate the potential of neurogenesis as a mechanism for circuit plasticity and function.
