Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) is an emergent pathogen in Brazil. However, there are no data on the prevalence of CA-MRSA. We report here the first well-characterized case of severe life-threatening CA-MRSA infection in a child living in Rio de Janeiro city. The patient had many complications including hematogenous osteomyelitis and involvement of multiple sites requiring drainage of soft-tissue abscess, and pleural and pericardial empyema. The MRSA isolates recovered were genotyped using PFGE, SCCmec typing and multilocus sequence typing. Disk diffusion tests were performed following Clinical and Laboratory Standards Institute recommendations. In addition, the presence of Panton-Valentine leukocidin (PVL) was assessed by PCR amplification, using specific primers for lukF-pv (encoding for the F subunit of the PVL). The bacterial isolates were related to the ST30-SCCmecIV lineage (Oceania Southwest Pacific clone), a PVL producer CA-MRSA previously detected in Porto Alegre, RS, Brazil. Also, the isolates analyzed were susceptible to all non-β-lactam antibiotics tested. The present report demonstrates that disseminated CA-MRSA disease is also occurring in Rio de Janeiro. Thus, the empirical treatment of moderate or severe infections suspected of being associated with CA-MRSA needs to be reviewed in order to allow prompt initiation of an effective therapy that also covers these microorganisms.

The objective of the present cross-sectional study was to assess the prevalence and the clinical and laboratory features of hepatitis C virus (HCV)-positive patients with type 2 diabetes mellitus (DM) attending either an outpatient clinic or hemodialysis units. Serologic-HCV testing was performed in 489 type 2 DM patients (303 outpatients and 186 on dialysis). A structured assessment of clinical, laboratory and DM-related complications was performed and the patients were then compared according to HCV infection status. Mean patient age was 60 years; HCV positivity (HCV+) was observed in 39 of 303 (12.9%) outpatients and in 34 of 186 (18.7%) dialysis patients. Among HCV+ patients, 32 were men (43.8%). HCV+ patients had higher serum levels of aspartate aminotransferase (0.90 ± 0.83 vs 0.35 ± 0.13 µKat/L), alanine aminotransferase (0.88 ± 0.93 vs 0.38 ± 0.19 µKat/L), gamma-glutamyl transferase (1.57 ± 2.52 vs 0.62 ± 0.87 µKat/L; P < 0.001), and serum iron (17.65 ± 6.68 vs 14.96 ± 4.72 µM; P = 0.011), and lower leukocyte and platelet counts (P = 0.010 and P < 0.001, respectively) than HCV-negative (HCV-) patients. HCV+ dialysis patients had higher diastolic blood pressure than HCV- patients (87.5 ± 6.7 vs 81.5 ± 6.0 mmHg; P = 0.005) and a lower prevalence of diabetic retinopathy (75 vs 92.7%; P = 0.007). In conclusion, our study showed that HCV is common among subjects with type 2 DM but is not associated with a higher prevalence of chronic diabetic complications.

We investigated the association between pulse wave velocity (PWV) and HIV infection, antiretroviral treatment-related characteristics, viral load, immune status, and metabolic changes in a cross-sectional study nested in a cohort of HIV/AIDS patients who have been followed for metabolic and cardiovascular changes since 2007. The study included patients recruited from the cohort (N = 261) and a comparison group (N = 82) of uninfected individuals, all enrolled from April to November 2009. Aortic stiffness was estimated using the carotid-femoral PWV (Complior-Artech, Paris, France). The groups were similar with respect to age, metabolic syndrome, diabetes mellitus, Framingham score, and use of antihypertensive and hypolipidemic medications. Hypertension was more frequent among the controls. Individuals with HIV had higher triglyceride, glucose and HDL cholesterol levels. Among individuals with HIV/AIDS, those with a nadir CD4+ T-cell count <200 cells/mm³ had a higher PWV (P = 0.01). There was no statistically significant difference when subjects were stratified by gender. Heart rate, age, male gender, and blood pressure were independently correlated with PWV. Nadir CD4+ T-cell count did not remain in the final model. There was no significance difference in PWV between HIV-infected individuals and uninfected controls. PWV was correlated with age, gender, and blood pressure across the entire population and among those infected with HIV. We recommend cohort studies to further explore the association between inflammation related to HIV infection and/or immune reconstitution and antiretroviral use and PWV.

Human papillomavirus (HPV)-related head and neck squamous cell carcinoma (HNSCC) is considered to be a distinct clinical entity with better prognosis than the classical tobacco- and alcohol-associated tumors. The increasing incidence of this neoplasia during the last decades highlights the need to better understand the role of HPV in the development of these cancers. Although the proportion of HNSCC attributed to HPV varies considerably according to anatomical site, overall approximately 25% of all HNSCC are HPV-DNA positive, and HPV-16 is by far the most prevalent type. In this review we discuss the existing evidence for a causal association between HPV infection and HNSCC at diverse anatomical head and neck subsites.

The emergence of ganciclovir (GCV) resistance during the treatment of human cytomegalovirus (HCMV) infection is a serious clinical challenge, and is associated with high morbidity and mortality. In this case report, we describe the emergence of two consecutive mutations (A594V and L595W) related to GCV resistance in a patient with HCMV retinitis and long-term HIV progression after approximately 240 days of GCV use. Following the diagnosis of retinitis, the introduction of GCV did not result in viral load reduction. The detected mutations appeared late in the treatment, and we propose that other factors (high initial HCMV load, previous GCV exposure, low CD4+ cell count), in addition to the presence of resistance mutations, may have contributed to the treatment failure of HCMV infection in this patient.

Zika virus (ZIKV), a mosquito-borne flavivirus, belongs to the Flaviviridae family, genus Flavivirus. ZIKV was initially isolated in 1947 from a sentinel monkey in the Zika forest, Uganda. Little clinical importance was attributed to ZIKV, once only few symptomatic cases were reported in some African and Southeast Asiatic countries. This situation changed in 2007, when a large outbreak was registered on the Yap Island, Micronesia, caused by the Asian ZIKV lineage. Between 2013 and 2014, ZIKV spread explosively and caused many outbreaks in different islands of the Southern Pacific Ocean and in 2015 autochthonous transmission was reported in Brazil. Currently, Brazil is the country with the highest number of ZIKV-positive cases in Latin America. Moreover, for the first time after the discovery of ZIKV, the Brazilian scientists are studying the possibility for the virus to cause severe congenital infection related to microcephaly and serious birth defects due to the time-spatial coincidence of the alarming increase of newborns with microcephaly and the Brazilian ZIKV epidemic. The present review summarizes recent information for ZIKV epidemiology, clinical picture, transmission, diagnosis and the consequences of this emerging virus in Brazil.

Pertussis or whooping cough is a human respiratory tract infection and a vaccine-preventable disease that is caused by Bordetella pertussis bacteria. Pertussis vaccination has been part of the Finnish national vaccine program since 1952. Despite extensive vaccinations, the incidence of pertussis has increased in many countries during the last decades. Large epidemics have been observed also in countries with high vaccine coverage. Inter-individual variation in immune responses is always encountered after vaccination. Low vaccine responses may cause vulnerability to pertussis even straight after vaccination. Reasons for low responses are not fully understood. The innate immune system is responsible for the initial recognition of pathogens and vaccine antigens. The role of innate immunity on pertussis immunity has not been thoroughly investigated. Mannose-binding lectin (MBL) and toll-like receptor 4 (TLR4) are important molecules of the innate immune system and in the recognition of pathogens. Cytokines form a signaling network that have a notable role in immune responses after infections as well as after vaccinations. Single nucleotide polymorphism (SNP) is common in genes encoding these molecules and the polymorphisms have been reported to affect vaccine response after viral and bacterial vaccines. This study investigated the gene polymorphisms of MBL2, TLR4 and interleukin (IL)-10 promoter and their association with vaccine responses after acellular pertussis (aP) vaccination in Finnish adolescents and infants. Cell-mediated immune responses were investigated ten years after the previous pertussis vaccinations in young adults. In addition, the role of MBL deficiency in pertussis infection susceptibility was evaluated. The results of this study show that subjects with TLR4 polymorphism had lower antibody production and persistence after aP vaccination compared with normal allele. A specific SNP in the TLR4 gene was associated with decreased antibody responses and persistence in adolescents after aP booster vaccination. Cell-mediated immune responses were partly detected ten years after the previous vaccination; booster vaccine clearly enhanced the responses. In addition, subjects with IL-10 polymorphism had altered cell-mediated immune responses. MBL deficiency was found to be more frequent in pertussis patients than healthy controls but the polymorphism of MBL2 was not associated with antibody responses after acellular pertussis vaccination. The novel finding of this study was that genetic variation in the innate immune system seems to play a role in altered pertussis vaccine responses as well as in pertussis infection. These new findings enlighten the mechanisms behind the low responses after pertussis vaccination and help to predict risk factors related to this phenomenon.

Sommaire La situation actuelle des infections nosocomiales dans les établissements de santé est préoccupante. En ce qui concerne le Clostridium difficile (C. difficile), son émergence et son impact sur la morbidité et la mortalité sont bien connus. De plus, la population vieillissante est à risque élevé d’avoir des diarrhées associées au C. difficile et, de ce fait, de se retrouver en isolement de contact. Les personnes âgées sont déjà vulnérables au moment d’une hospitalisation, alors qu’en est-il lors d’un isolement de contact relié à une infection au C. difficile? Dans cette perspective, les connaissances sur l’expérience des personnes âgées lors d’un isolement étant peu développées, cette étude s’est intéressée au vécu des personnes âgées durant l’isolement de contact et aux effets de cet isolement sur leur vécu. Le but de cette étude phénoménologique, prenant appui sur la Théorie de l’humain en devenir de Parse (2003), était de décrire et comprendre l’expérience de personnes âgées de 75 ans et plus lors d’un isolement de contact en milieu hospitalier causé par une infection au C. difficile. Ainsi, des entrevues semi-structurées furent réalisées auprès de cinq personnes âgées qui ont accepté de participer à l’étude, puis la transcription de leurs propos fut analysée selon la méthode proposée par Giorgi (1997). De cette analyse sont ressortis trois thèmes : 1) Vivre les effets du Clostridium difficile; 2) Vivre de l’inquiétude et 3) Vivre de la déception dans la relation avec le personnel soignant. Poussant plus loin l’analyse des thèmes et sous-thèmes, il a été possible de proposer que l’essence de l’expérience de personnes âgées de 75 ans et plus lors d’un isolement de contact en milieu hospitalier causé par une infection au C. difficile était de « vivre à la fois en conjonction avec le besoin d’être isolé pour protéger son univers et en séparation avec le besoin de recevoir des soins qui respectent leur dignité, et ce, malgré la vulnérabilité induite par leur état de santé ». Ces résultats pourront sensibiliser les personnes soignantes qui accompagnent au quotidien les personnes âgées en isolement. La compréhension de leur vécu pourra favoriser la mise en place de soins davantage centrés sur la personne qui tiennent compte de leurs inquiétudes, de leurs craintes et de l’importance de préserver leur dignité. L’inflammation est un procédé complexe qui vise l’élimination de l’agent causal de dommages tissulaires en vue de faciliter la réparation du tissu affecté. La persistance de l’agent causal ou l’incapacité à résoudre l’inflammation mène à un dérèglement homéostatique chronique qui peut avoir une incidence sur la morbidité et la mortalité. L’athérosclérose est une condition inflammatoire chronique des vaisseaux sanguins dont l’origine est multifactorielle. L’hypertension et l’état infectieux représentent respectivement des facteurs de risque classiques et émergents du développement de cette maladie. Les fondements initiaux de l’inflammation font intervenir l’immunité innée, la première ligne de défense dont disposent les cellules pour répondre à un signal de danger. Le but de cette thèse est d’examiner le rôle pro-inflammatoire d’une famille de kinases essentielles à l’immunité innée, soit celle des kinases de IkappaB (IKK) et des kinases IKK-related. Les kinases IKKalpha et IKKbeta forment le complexe IKK avec la molécule adaptatrice NEMO/IKKgamma. Ce complexe est chargé d’effectuer la phosphorylation de l’inhibiteur de NF-kappaB, IkappaBalpha, ce qui mène à sa dégradation et à la libération du facteur de transcription NF-kappaB. Nous montrons que le peptide vasoactif angiotensine II (AngII) induit l’activité phosphotransférase d’IKKbeta dans les VSMC par immunoprécipitation de NEMO puis essai kinase in vitro. Grâce à une approche ARN interférence (ARNi) dirigée contre IKK, nous montrons que cette kinase est responsable de la phosphorylation de p65/RelA. Nous montrons que le mécanisme d’induction de NF-kappaB par l’AngII est atypique, puisqu’il ne module pas IkappaBalpha, et montrons à l’aide d’inhibiteurs pharmacologiques que l’activation de p65 est indépendante des voies MEK-ERK-RSK, PI3K et de la transactivation du récepteur de l’EGF. Les kinases IKK-related Tank-binding kinase 1 (TBK1) et IKK-i sont quant à elles principalement activées suite à une infection bactérienne ou virale. Ces kinases phosphorylent directement le facteur de transcription interferon regulatory factor (IRF)-3. Nous montrons que le cytomégalovirus humain, un pathogène associé à l’athérosclérose, a la capacité d’induire l’activation de TBK1 dans les VSMC. L’usage d’ARNi dirigé contre TBK1 et IKKi montre que les 2 kinases sont impliquées dans l’activation d’IRF-3. De plus, nous montrons à l’aide d’une lignée de VSMC exprimant une version dominante négative d’IRF-3 que ce dernier est essentiel à la synthèse des chimiokines RANTES et IP-10, tel qu’analysé par RT-PCR. Par ailleurs, il a récemment été montré que les kinases IKK-related étaient étroitement liées à la transformation oncogénique, et que TBK1 était pro-angiogénique. Or, l’angiogenèse est le plus souvent modulée par la réponse hypoxique qui est d’ailleurs commune à la majorité des processus inflammatoires. Le facteur de transcription hypoxia inducible factor (HIF)-1 module l’angiogenèse, l’inflammation et la survie cellulaire. Nous montrons à l’aide de cellules Tbk1 et Ikbke -/- et d’une approche lentivirale que TBK1 est spécifiquement impliquée dans l’induction traductionnelle de HIF-1alpha en condition de stress hypoxique. L’expression de TBK1 est induite sous ces conditions, et cette kinase module la phosphorylation de ERK, RSK, Akt et TSC1. Les résultats originaux présentés dans cette thèse montrent donc que les kinases IKK et IKK-related exercent leurs actions pro-inflammatoires par des mécanismes distincts.

La méthylation de l'ADN est une marque épigénétique importante chez les mammifères. Malgré le fait que la méthylation de la cytosine en 5' (5mC) soit reconnue comme une modification épigénétique stable, il devient de plus en plus reconnu qu'elle soit un processus plus dynamique impliquant des voies de méthylation et de déméthylation actives. La dynamique de la méthylation de l'ADN est désormais bien caractérisée dans le développement et dans le fonctionnement cellulaire des mammifères. Très peu est cependant connu concernant les implications régulatrices dans les réponses immunitaires. Pour se faire, nous avons effectué des analyses du niveau de transcription des gènes ainsi que du profilage épigénétique de cellules dendritiques (DCs) humaines. Ceux-ci ont été faits avant et après infection par le pathogène Mycobacterium tuberculosis (MTB). Nos résultats fournissent le premier portrait génomique du remodelage épigénétique survenant dans les DCs en réponse à une infection bactérienne. Nous avons constaté que les changements dans la méthylation de l'ADN sont omniprésents, identifiant 3,926 régions différentiellement méthylées lors des infections par MTB (MTB-RDMs). Les MTB-RDMs montrent un chevauchement frappant avec les régions génomiques marquées par les histones associées avec des régions amplificatrices. De plus, nos analyses ont révélées que les MTB-RDMs sont activement liées par des facteurs de transcription associés à l'immunité avant même d'être infecté par MTB, suggérant ces domaines comme étant des éléments d'activation dans un état de dormance. Nos données suggèrent que les changements actifs dans la méthylation jouent un rôle essentiel pour contrôler la réponse cellulaire des DCs à l'infection bactérienne.

In 2007, an H3N2 influenza A virus was isolated from Canadian mink. This virus was found to be phylogenetically related to a triple reassortant influenza virus which emerged in Canadian swine in 2005, but it is antigenically distinct. The transmission of the virus from swine to mink seems to have occurred following the feeding of animals with a ration composed of uncooked meat by-products of swine obtained from slaughterhouse facilities. Serological analyses suggest that the mink influenza virus does not circulate in the swine population. Presently, the prevalence of influenza virus in Canadian farmed and wild mink populations is unknown. The natural occurrence of influenza virus infection in mink with the presence of clinical signs is a rare event that deserves to be reported.

White Spot Syndrome Virus (WSSV) is the most devastating disease affecting shrimp culture around the world. Though, considerable progress has been made in the detection and molecular characterization of WSSV in recent years, information pertaining to immune gene expression in shrimps with respect to WSSV infection remains limited. In this context, the present study was undertaken to understand the differential expression of antimicrobial peptide (AMP) genes in the haemocytes of Penaeus monodon in response to WSSV infection on a time-course basis employing semi-quantitative RT-PCR. The present work analyzes the expression profile of six AMP genes (ALF, crustin-1, crustin-2, crustin-3, penaeidin-3 and penaeidin-5), eight WSSV genes (DNA polymerase, endonuclease, immediate early gene, latency related gene, protein kinase, ribonucleotide reductase, thymidine kinase and VP28) and three control genes (18S rRNA, β-actin and ELF) in P. monodon in response to WSSV challenge. Penaeidins were found to be up-regulated during early hours of infection and crustin-3 during late period of infection. However, ALF was found to be up-regulated early to late period of WSSV infection. The present study suggests that AMPs viz. ALF and crustin-3 play an important role in antiviral defense in shrimps. WSSV gene transcripts were detected post-challenge day 1 itself and increased considerably day 5 onwards. Evaluation of the control genes confirmed ELF as the most reliable control gene followed by 18S rRNA and β-actin for gene expression studies in shrimps. This study indicated the role of AMPs in the protection of shrimps against viral infection and their possible control through the up-regulation of AMPs

Introducción: La obstrucción intestinal es una patología de alta prevalencia e impacto en los servicios de cirugía general a nivel mundial. El manejo de esta entidad puede ser médico o quirúrgico. Cuando se requiere intervención quirúrgica, se busca evitar el desarrollo de isquemia intestinal y resecciones intestinales; durante el postoperatorio, pueden existir complicaciones. El objetivo de este estudio es identificar los factores asociados al desarrollo de complicaciones post operatorias en un grupo de pacientes con obstrucción intestinal mecánica llevados a manejo quirúrgico. Metodología: Estudio analítico tipo casos y controles en un grupo de pacientes con diagnóstico de obstrucción intestinal mecánica llevados a manejo quirúrgico de su patología. Los casos corresponden a los pacientes con complicaciones postoperatorias y los controles aquellos que no presentaron complicaciones. Se identificaron factores asociados a complicación post operatoria mediante modelos estadísticos bivariados y multivariados de regresión logística para factores como edad, sexo, antecedente quirúrgico, presentación clínica, paraclínica y diagnóstico postoperatorio de malignidad, entre otras. Resultados: Se identificaron un total de 138 pacientes (54 casos y 129 controles). Los rangos de edad entre 55-66 años y mayor de 66 años fueron asociados con complicaciones postoperatorias (OR 3,87 IC95% 1,58-9,50 y OR 3,62 IC95% 1,45-9,08 respectivamente). El déficit de base inferior a 5 mEq/litro se relaciona con complicaciones postoperatorias (OR 2,64 IC95% 1.33-5,25) Otras pruebas de laboratorio, características radiológicas, hallazgos de malignidad en el postoperatorio y la evolución de los pacientes no fueron asociados con complicaciones. Conclusiones: Las disminución de las complicaciones durante el manejo quirúrgico de obstrucción intestinal mecánica continúa siendo un reto para la cirugía general. Factores no modificables como edad avanzada y modificables como el equilibrio ácido base deben ser tenidos en cuenta dada su correlación en el desarrollo de complicaciones postoperatorias.

INTRODUCCIÓN: Los carbapenémicos (CP) son una de las últimas líneas de tratamiento para infecciones por microorganismos multirresistentes (MDR), especialmente Gram-negativos productores de betalactamasas de espectro extendido. Es creciente la preocupación a nivel mundial por el aumento de aislamientos resistentes a CP, en EEUU hasta 60% de las infecciones nosocomiales son causadas por bacterias MDR. En la Unión Europea, cerca de 25.000 pacientes mueren anualmente por esta causa. En Latinoamérica hay una tendencia creciente en las tasas de resistencia.OBJETIVO: Identificar y describir factores protectores o de riesgo, relacionados con colonización o infección por Gram negativos resistentes a CP en pacientes adultos hospitalizados, mediante una revisión sistemática de la literatura.MÉTODOS: Revisión sistemática de literatura, búsqueda de estudios observacionales analíticos en las bases de datos PubMed, Embase, Scopus, BVS, Scielo y búsqueda de literatura gris, publicados desde el 01/01/2004 al 15/04/2015. Se evalúo la calidad de los estudios con escala Newcastle-Ottawa y FLC Osteba. RESULTADOS: Se seleccionaron 36 estudios de alta calidad, diseño de casos y controles. Los factores de riesgo estadísticamente significativos observados son estancia en UCI OR:36.46, insuficiencia renal aguda OR:6.23, diálisis OR:10.80 ventilación mecánica OR:17.5, cateterismo vesical OR:14.3, uso de carbapenémicos OR:18,52,quinolonas OR17.30, cefepime OR:28.05, glicopéptidos OR:19.1; metronidazol OR:4.17, p:0.03, colistina OR:12.1, linezolid OR:7 CONCLUSIÓN: Pese a que hay alta heterogeneidad en las variables incluidas en los estudios, se encontró que los factores de riesgo principales para adquirir GNR-CP en pacientes hospitalizados son: antecedente de insuficiencia renal aguda y diálisis, ventilación mecánica, cateterismo vesical, estancia en UCI y uso previo de antibióticos carbapenémicos, quinolonas, cefepime, glicopéptidos, metronidazol, linezolid y colistina. No se hallaron factores protectores. factores de riesgo

Sorghum (Sorghum bicolor L.) plants were grown in split pots in three Rothamsted soils with different soil pH values and phosphorus (P) contents. Ammonium addition resulted in higher plant dry weight and P content than comparable nitrate treatments. The pH of soils in the rhizosphere (0.51-mm average thickness) differed from the bulk soil depending on nitrogen (N) form and level. Ammonium application resulted in a pH decrease, but nitrate application slightly increased pH. To examine the effect of rhizosphere acidification on mobilization of phosphate, 0.5 M NaHCO3 extractable phosphate was measured. The lowering rhizosphere pH enhanced the solubility of P in the soil and maybe availability of P to plants. Rhizosphere-P depletion increased with increasing ammonium supply, but when N was supplied as nitrate, P depletion was not related to increasing nitrate supply. Low P status Hoosfield soils developed mycorrhizal infection., and as a result, P inflow was increased. Geescroft soil, which initially had a high P status, did not develop mycorrhizal infection, and P inflow was much smaller and was unaffected by N treatments. Therefore, plant growth and P uptake were influenced by both rhizosphere pH and indigenous mycorrhizal infection.
