Relationships between oral health status in children with disability and their mothers’ depressive symptoms Aim. The purpose of the present study was to evaluate the relationships between oral health status in children with chronic medical conditions and their mothers’ depressive symptoms. Methods. Fifty-one children (25 male and 26 female, ranging from 2 to 18 years) affected by chronic systemic diseases followed at the Sant’Orsola-Malpighi Hospital in Bologna, ,and, were referred with their mothers at the Dental Department of Bologna. Children were subclassified in 3 groups according to the ASA classification and orally examined for hygiene status, gingival condition and dental caries. The indexes used were O’Leary plaque Index (PI), bleeding on probing index (BOP), dmft/DMFT. Mothers were interviewed on knowledge about oral diseases prevention for their children and daily management (hygiene habits, sugared aliments consumption). Statistical analysis was performed through the use of linear regression. Results. The relationships between ASA and IP as well as between ASA and BOP are statistically significant (α = 0,01). Seventy percent of patients and their relatives in ASA groups 3 and 4 never received information on oral health and prevention of oral diseases by paediatricians and/or dentists. The 53% of mothers present depressive symptoms. The relationships between degree of depressive symptoms and dmft/DMFt as well as between degree of depressive symptoms and sugared aliments daily consumption are statistically significant (α = 0,05). Conclusion. Our results give support to the hypothesis of an association between degree of systemic disease and oral hygiene status. The psychological mothers condition seams to play a role on the oral conditions of their sons. Our analysis shows the needs for an interdisciplinar approach in order to promote the oral health of children with disability.

La ricerca si è focalizzata su due degli aspetti di interesse odontoiatrico più diffusi: la carie dentaria e la parodontite cronica. Il problema della carie dentaria è stato studiato in una popolazione di 39 soggetti affetti da cardiopatia congenita in cui la scarsa igiene orale è fattore di rischio per problematiche di salute generale e soprattutto per lo sviluppo di endocardite infettiva. I dati osservati e confrontati con quelli di un omogeneo gruppo di controllo dimostrano che nella dentatura decidua questi bambini hanno più denti cariati, come dimostrato dalla significativa differenza dell'indice dmft. Nella dentatura permanente non si osservano differenze tra i due gruppi. La carica microbica totale rilevata nella saliva e la presenza di Streptococcus mutans non mostrano differenze tra i due gruppi. I problemi di parodontite cronica sono stati studiati in un gruppo di 352 soggetti italiani adulti in cui si è definita la prevalenza dei 6 più importanti patogeni parodontali e la possibile correlazione con parametri clinici (pus, sanguinamento al sondaggio - BOP, profondità di sondaggio della tasca parodontale – PPD). Tra le 6 specie batteriche ricercate, quello di più frequente riscontro è stato Fusobacterium nucleatum (95%), mentre quello con carica batterica più alta è stato Tannerella forsythia. La carica batterica di Porphyromonas gingivalis, Treponema denticola, Tannerella forsythia e Fusobacterium nucleatum ha mostrato una correlazione diretta con il BOP e la presenza di pus. Inoltre, si è riscontrato che la carica batterica di tutte le specie (tranne Aggregatibacterium actinomycetemcomitans) aumenta all'aumentare del PPD. Tra le variabili studiate, PPD rappresenta il più importante fattore di rischio per la presenza di parodontopatogeni, mentre BOP è un indicatore di rischio per la ricerca del complesso rosso.

Parents may feel guilty about their children's oral problems, which can affect their quality of life. The aim of this study was to assess the presence of parental guilt and its association with early childhood caries (ECC), traumatic dental injuries (TDI) and malocclusion (AMT) in preschool children. All 2 to 5 year-old children (N = 305), and their parents, seeking dental care at the University of São Paulo Dental School one-week Screening Programme, were asked to participate in the study, and 260 agreed. Children were examined by two calibrated dentists, and their parents answered a socioeconomic and ECOHIS questionnaire; the question on guilt was used as the dependent variable. Regression analyses examined the association between parental guilt and ECC, TDI, AMT and socioeconomic factors. A total of 35.8% of parents felt guilty. This was only associated with caries severity. No association was found between guilt and TDI, AMT or socioeconomic factors. ECC was present in 63.8% of the children; the mean (± sd) dmf-t score was 7.29 (± 2.78). Thus, the number of parents feeling guilty increases with the increase of their children's dental caries severity. Parental guilt is related to caries but is not associated with TDI or AMT.

PURPOSE: To determine whether the treatment of dental caries with ozone was possible in apprehensive children and to ascertain whether ozone reverses caries in open single-surface lesions. Further, the influence of ozone on laser fluorescence was investigated. METHODS: 82 lesions in 28 children with at least two open single-surface lesions were assessed. The children were anxious and were judged by the referring dentist as non-treatable. For each test lesion, which was treated with ozone, a control lesion was left without ozone treatment. Hardness and laser fluorescence values were assessed and the changes for hardness and laser fluorescence values in the test lesion were compared with the values in the control lesion after 2, 4, 6, and 8 months. RESULTS: 94 percent of the children were treatable and 93% lost their dental anxiety. The hardness values improved significantly in the ozone-treated test lesions after 4, 6, and 8 months (P< 0.05) compared with baseline while the control lesions had no significant change in hardness at any recall interval. Comparing the differences between test and control teeth over time, the laser fluorescence values improved, however the improvement was not statistically significant (P> 0.05). The use of ozone resulted in an average reduction of 13% of the laser fluorescence values immediately after the ozone treatment.

During and after an erosive challenge, behavioral factors play a role in modifying the extent of erosive tooth wear. The manner that dietary acids are introduced into the mouth (gulping, sipping, use of a straw) will affect how long the teeth are in contact with the erosive challenge. The frequency and duration of exposure to an erosive agent is of paramount importance. Night-time exposure (e.g. baby bottle-feeding) to erosive agents may be particularly destructive because of the absence of salivary flow. Health-conscious individuals tend to ingest acidic drinks and juices more frequently and tend to have higher than average oral hygiene. While good oral hygiene is of proven value in the prevention of periodontal disease and dental caries, frequent toothbrushing with abrasive oral hygiene products may enhance erosive tooth wear. Unhealthy lifestyles such as consumption of designer drugs, alcopops and alcohol abuse are other important behavioral factors.

When considering data from many trials, it is likely that some of them present a markedly different intervention effect or exert an undue influence on the summary results. We develop a forward search algorithm for identifying outlying and influential studies in meta-analysis models. The forward search algorithm starts by fitting the hypothesized model to a small subset of likely outlier-free studies and proceeds by adding studies into the set one-by-one that are determined to be closest to the fitted model of the existing set. As each study is added to the set, plots of estimated parameters and measures of fit are monitored to identify outliers by sharp changes in the forward plots. We apply the proposed outlier detection method to two real data sets; a meta-analysis of 26 studies that examines the effect of writing-to-learn interventions on academic achievement adjusting for three possible effect modifiers, and a meta-analysis of 70 studies that compares a fluoride toothpaste treatment to placebo for preventing dental caries in children. A simple simulated example is used to illustrate the steps of the proposed methodology, and a small-scale simulation study is conducted to evaluate the performance of the proposed method. Dental caries is a common preventable childhood disease leading to severe physical, mental and economic repercussions for children and their families if left untreated. A needs assessment in Harris County reported that 45.9% of second graders had untreated dental caries. In order to address this growing problem, the School Sealant Program (SSP), a primary preventive initiative, was launched by the Houston Department of Health and Human Services (HDHHS) to provide oral health education, and underutilized dental preventive services to second grade children from participating Local School Districts (LSDs). ^ To determine the effectiveness and efficiency of the SSP, a program evaluation was conducted by the HDHHS between September 2007 and June 2008 for the Oral Health Education (OHE) component of the SSP. The objective of the evaluation was to assess short term changes in oral health knowledge of the participants and determine if these changes, if any, were due to the OHE sessions. An 8-item multiple choice pre/post test was developed for this purpose and administered to the participants before and immediately after the OHE sessions. ^ The present project analyzed pre and post test data of 1,088 second graders from 22 participating schools. Changes in overall and topic-specific knowledge of the program participants before and after the OHE sessions were analyzed using the Wilcoxon's signed rank test. ^ Results. The overall knowledge assessment showed a statistically significant (p <0.001) increase in the dental health knowledge of the participants after the oral health education sessions. Participants in the higher scoring category (7-8 correct responses) increased from 9.5% at baseline to 60.8% after the education sessions. Overall knowledge increased in all school regions with the highest knowledge gains seen in the Central and South regions. Males and females had similar knowledge gains. Significant knowledge differences were also found for each of the topic specific categories (functions of teeth, healthy diet, healthy habits, dental sealants; p<0.001) indicating an increase in topic specific knowledge of the participants post-health education sessions. ^ Conclusions. The OHE sessions were successful in increasing the short term oral health knowledge of the participants. ^

In the United States, dental caries is the most common chronic illness in children, occurring five to eight times as frequently as asthma. 11 Dental caries is an unmet health need, disproportionately affecting minority groups and individuals with low socio-economic status.15,34,36 School-Based Sealant Programs were developed to target children at risk, to provide dental services in a closer geographic area, to offer low cost preventive dental services, and to educate families about oral health and prevention.1 There is scientific, evidence based literature that shows the effectiveness of dental sealants preventing dental decay. 13^ Currently, there is no central source for cataloging School-Based Sealant Programs (SBSPs). Information is scattered around publications and documents. For instance, the National Health and Nutrition Examination Survey (NHANES) does not have information about all the existing SBSPs. ^ This literature review determined which are the most common characteristics of SBSPs in the U.S. and determined the extent to which these programs provide sealants to children of low socio-economic status. The method utilized was an electronic database search. Pubmed and EBESCO host databases were searched with Mesh terms like “dental school sealant programs”, “community dentistry”, “school based sealant programs” and “oral preventive programs”. Results were organized in terms of location, population served, providers, funding source and data shared. ^ The searches produced 77 studies, from which 40 were included in this work. Only 18 U.S. states were represented in the results; however these findings are very consistent with the Best Practice Approach – School Based Sealant Programs3. Most of the SBSPs provide their services to children from low income families, and utilized the lower labor cost providers permitted by their state regulations. The author intends that this thesis work will become an aide in the development of future programs, and as evidence for the sustainability of these programs.^

O objetivo principal deste estudo foi avaliar a efetividade de dois intervalos de retorno clinico na incidência de cárie em pré-escolares que apresentaram alto risco de cárie. Também, foram avaliadas como desfechos secundários a paralisação de lesões de cárie já existentes, o nível de ansiedade, e a qualidade de vida relacionada à saúde bucal (QVRSB), de acordo com cada intervalo empregado. A amostra foi composta por 224 crianças entre 3 a 5 anos de idade, de ambos os gêneros, com alto risco de cárie, de acordo com os critérios clínicos adaptados da Secretaria da Saúde do Estado de Saúde de São Paulo. As crianças foram alocadas aleatoriamente em dois grupos: Grupo 1 (G1) - exame clínico bucal mais, orientação de cuidados de higiene bucal e dieta com intervalos de retornos clínicos de 4 meses; Grupo 2 (G2) - exame clínico bucal mais, orientação de cuidados de higiene bucal e dieta com intervalos de retornos de 8 meses. Os exames clínicos bucais foram realizados por um único examinador treinado e calibrado, cego aos grupos de estudo em que as crianças estavam alocadas. O exame clínico bucal abrangeu a detecção e atividade de lesões de cárie de acordo com o índice ICDAS (International Caries Detection and Asessment System) e as características clínicas de lesões de cárie ativas e inativas foram associadas com os critérios descritos no manual do Comitê ICDAS. Um dentista externo cego aos grupos de estudo realizou as orientações sobre cuidados de saúde bucal, avaliou o nível de ansiedadeatravés da Escala de Imagem Facial, e a QVRSB mensurada pelo B-ECOHIS. Todos os grupos foram acompanhados durante um período de 8meses, totalizando duas consultas de retorno para o G1 e uma consulta para o G2. Todas as análises foram realizadas por intenção de tratar (ITT). A curto prazo, os resultados apresentam que retorno de 4 e 8 meses apresentaram-se similares em todos os desfechos do estudo para pré-escolares com alto de risco de cárie a nível de saúde pública.

O objetivo deste estudo foi relacionar dor de dente com condição socioeconômica, acesso aos serviços de saúde bucal e estilo de vida em adolescentes do município de Sobral, Ceará, desenvolvido com delineamento transversal analítico, com amostra composta por 688 indivíduos. A prevalência de dor de dente no grupo pesquisado foi de 31,8%. Para avaliar a relação entre as variáveis independentes e a dor de dente, realizou-se teste de associação pelo qui-quadrado, estimando-se a razão de prevalências por meio da regressão de Poisson. Os fatores que mais demonstraram relação com a dor de dente foram severidade da cárie, motivo do atendimento odontológico relacionado com urgência, frequência ao dentista e recebimento de escova na escola. Observou-se que a alta prevalência de dor de dente em adolescentes está diretamente relacionada às condições de acesso, assim como às características das ações desenvolvidas pelos serviços de saúde. Assim como há necessidade da implantação de serviços vinculados à promoção de saúde, pautados pela equidade e integralidade, é necessária a implantação de serviços de urgência que não simplesmente intervenham na dor de forma mutiladora, mas a encarem como mecanismo de estímulo ao desenvolvimento de procedimentos de prevenção das doenças bucais.

OBJECTIVE: To describe the distribution of malocclusion and its associated factors in Brazilian adolescents. METHODS: Data from 7,328 subjects aged 12 years and 5,445 adolescents aged 15-19 years were analyzed. The adolescents took part in the Brazilian Oral Health Survey (SBBrasil 2010). The outcome was severe malocclusion according to the dental aesthetic index. The independent variables were sex, skin color, monthly household income, possessions, number of individuals in the household, untreated dental caries, missing teeth and dental appointments or lack thereof, frequency, and reason. Logistical regression analysis was carried out, considering the complex sampling cluster design, based on a hierarchical model. RESULTS: The prevalence of severe malocclusion was 6.5% and 9.1% in the 12-year-olds and the 15-19-year-olds, respectively. After adjustment, those with lighter- skinned black or black skin were 1.59 (95%CI 1.08;2.34) times more likely to present the outcome compared with those with white skin. The loss of one or more first molars increased 2.66 (95%CI 1.26;5.63) the chance to present severe malocclusion by the age of 12. Adolescents aged 15-19 whose household income was below R$ 1,500.00 (OR 2.69 [95%CI 1.62; 4.47]) and those who had seen a dentist for treatment (OR 2.59 [95%CI 2.55;4.34]) had the greatest chance of having severe malocclusion compared with those with higher incomes and those who visited the dentist for prevention.

The SBBrasil 2010 Project (SBB10) was designed as a nationwide oral health epidemiological survey within a health surveillance strategy. This article discusses methodological aspects of the SBB10 Project that can potentially help expand and develop knowledge in the health field. This was a nationwide survey with stratified multi-stage cluster sampling. The sample domains were 27 State capitals and 150 rural municipalities (counties) from the country's five major geographic regions. The sampling units were census tracts and households for the State capitals and municipalities, census tracts, and households for the rural areas. Thirty census tracts were selected in the State capitals and 30 municipalities in the countryside. The precision considered the demographic domains grouped by density of the overall population and the internal variability of oral health indices. The study evaluated dental caries, periodontal disease, malocclusion, fluorosis, tooth loss, and dental trauma in five age groups (5, 12, 15-19, 35-44, and 65-74 years).
