We evaluated insulin release and insulin sensitivity in women with basal and/or postprandial hyperglycemia but normal oral glucose tolerance test (OGTT) in previous pregnancy (GHG). These women were individually matched with females without previous hyperglycemia (NGT). Both groups consisted of normal glucose-tolerant women at the time of this study. They underwent OGTT (75g; n= 32 pairs) and hyperglycemic clamp experiments (10mmoll-1; n=27 pairs) with plasma glucose, insulin, and C-peptide measurements and calculation of insulinogenic index, first- and second-phase insulin release, and insulin sensitivity index (ISI). The GHG group showed higher glycosylated hemoglobin levels (6.2±0.6% versus 5.8±0.8%; P<0.05); lower insulinogenic index at 30min (134.03±62.69pmolmmol-1 versus 181.59±70.26pmolmmoll-1; P<0.05) and diminished C-peptide response in relation to glucose (4.05±0.36nmolmmol-1 versus 4.23±0.36nmolmmol-1; P<0.05) at OGTT. Both groups did not show difference in insulin secretion and ISI by hyperglycemic clamp technique. We concluded that in up to 12 years from index pregnancy, women with previous GHG, presenting normal glucose tolerance and well-matched with their controls, showed β-cell dysfunction without change in ISI. As women with previous GHG are at risk of type 2 diabetes, β-cell dysfunction may be its primary defect. © 2003 Elsevier B.V. All rights reserved.

Diarrhea caused by Cryptosporidium sp is frequent in patients with AIDS, but involvement of other organs of the digestive tract is uncommon. We report a case of Cryptosporidium-associated obstruction of the biliary tract mimicking cancer of the head of the pancreas in a 43-year-old woman with AIDS.

Polymorphous low-grade adenocarcinoma (PLGA) is a subtype of salivary gland carcinoma with a marked predilection for the minor salivary glands. The diagnosis of this lesion can be a challenge in incisional biopsies because of its similarity to other salivary neoplasms and the difficulty of analyzing its infiltrative nature and neurotropism. Diagnosis can be facilitated, however, by immunohistochemistry, a technique that has become a fundamental tool to the pathologist. The purpose of this study was analyze the immunohistochemical aspects of 4 cases that were examined in the São José dos Campos School of Dentistry. The patients included 4 women, ages 32 to 57 years (mean, 50 years), who reported an intraoral painless lesion, ranging in size from 1 to 3 cm (mean, 1.75 cm) with a variable evolution period. Histologically, the lesions showed similar aspects, characterized by polymorphous growth pattern, including solid, cribriform, trabecular, papillary, and single-file arrays. The tumors showed positivity to cytokeratins (Cks) 7 and 8 and vimentin, variable expression for Ck 14, and a negative reaction to Cks 13 and 19 and muscle-specific actin. PLGA is a tumor of the salivary gland that must be distinguished from other neoplasms for therapeutic and prognostic considerations, and the panel of antibodies employed in this study was effective for its diagnosis.

Adenocarcinoma of the lung that metastasizes to the mandible is very uncommon; only a few cases have been described in the English-language literature. This article presents a metastasis from adenocarcinoma of the lung affecting the mandible of a 64-year-old woman, in which the first discovered metastatic lesion was detected before the primary tumor. The immunoreactivity for human thyroid transcription factor-1 (TTF-1) in the oral lesion was essential for determining the site and type of the primary tumor, as the patient showed no clinical or radiographic evidence of a tumor in the thyroid gland. After the primary tumor in the lung was diagnosed, radiotherapy and chemotherapy were initiated; unfortunately, the patient died two months after the start of treatment. This article emphasizes the importance of a well-conducted examination for diagnosing metastatic oral lesions.

Background: The adenocarcinoma of the appendix is a rare disease, generally identified as an acute appendicitis in the clinical presentation. Preoperative diagnosis is extremely difficult and uncommon. Objective: To present two cases of adenocarcinoma of the appendix, unique for presenting in early ages, and a complete literature review on this topic. Cases report: First case: a 24-year-old man submitted to appendectomy for acute inflammatory abdomen. Sent to hospital due to a histopathological diagnosis of mucinous adenocarcinoma of the appendix. Right hemicolectomy, epiploectomy, ganglion and partial abdominal wall resection were performed. The patient is now in chemotherapy. Second case: a 32-year-old man presented, during appendectomy, with vegetant lesion on the apex of the appendix. The histopathological study revealed the presence of adenocarcinoma of the appendix. Right hemicolectomy with ganglion resection was performed. The patient does not present signs of recurring disease. Conclusion: The macroscopic and clinical similarity between adenocarcinoma of the appendix and acute appendicitis makes it important to check out histopathological study results. The cases above reinforce this requirement, as the authors have found adenocarcinoma of the appendix in young patients.

To evaluate the effect of acute exercise and exercise training at the anaerobic threshold (AT) intensity on aerobic conditioning and insulin secretion by pancreatic islets, adult male Wistar rats were submitted to the lactate minimum test (LMT) for AT determination. Half of the animals were submitted to swimming exercise training (trained), 1 h/day, 5 days/week during 8 weeks, with an overload equivalent to the AT. The other half was kept sedentary. At the end of the experimental period, the rats were submitted to an oral glucose tolerance test and to another LMT. Then, the animals were sacrificed at rest or immediately after 20 minutes of swimming exercise at the AT intensity for pancreatic islets isolation. At the end of the experiment mean workload (% bw) at AT was higher and blood lactate concentration (mmol/L) was lower in the trained than in the control group. Rats trained at the AT intensity showed no alteration in the areas under blood glucose and insulin during OGTT test. Islet insulin content of trained rats was higher than in the sedentary rats while islet glucose uptake did not differ among the groups. The static insulin secretion in response to the high glucose concentration (16.7 mM) of the sedentary group at rest was lower than the sedentary group submitted to the acute exercise and the inverse was observed in relation to the trained groups. physical training at the AT intensity improved the aerobic condition and altered insulin secretory pattern by pancreatic islets. © 2010 Landes Bioscience.

Peptides isolated from animal venoms have shown the ability to regulate pancreatic beta cell function. Characterization of wasp venoms is important, since some components of these venoms present large molecular variability, and potential interactions with different signal transduction pathways. For example, the well studied mastoparan peptides interact with a diversity of cell types and cellular components and stimulate insulin secretion via the inhibition of ATP dependent K + (K ATP) channels, increasing intracellular Ca 2+ concentration. In this study, the insulin secretion of isolated pancreatic islets from adult Swiss mice was evaluated in the presence of synthetic Agelaia MP-I (AMP-I) peptide, and some mechanisms of action of this peptide on endocrine pancreatic function were characterized. AMP-I was manually synthesized using the Fmoc strategy, purified by RP-HPLC and analyzed using ESI-IT-TOF mass spectrometry. Isolated islets were incubated at increasing glucose concentrations (2.8, 11.1 and 22.2 mM) without (Control group: CTL) or with 10 μM AMP-I (AMP-I group). AMP-I increased insulin release at all tested glucose concentrations, when compared with CTL (P < 0.05). Since molecular analysis showed a potential role of the peptide interaction with ionic channels, insulin secretion was also analyzed in the presence of 250 μM diazoxide, a K ATP channel opener and 10 μM nifedipine, a Ca 2+ channel blocker. These drugs abolished insulin secretion in the CTL group in the presence of 2.8 and 11.1 mM glucose, whereas AMP-I also enhanced insulin secretory capacity, under these glucose conditions, when incubated with diazoxide and nifedipine. In conclusion, AMP-I increased beta cell secretion without interfering in K ATP and L-type Ca 2+ channel function, suggesting a different mechanism for this peptide, possibly by G protein interaction, due to the structural similarity of this peptide with Mastoparan-X, as obtained by modeling. © 2012 Elsevier Ltd.

As neoplasias gástricas são a segunda maior causa de morte por câncer e apesar das descobertas sobre a fisiopatologia das células tumorais, o câncer é considerado como, no melhor das hipóteses, minimamente controlado pela medicina moderna. O carcinoma gástrico é uma das poucas neoplasias malignas nas quais os agentes infecciosos tem um importante papel etiológico. O objetivo do presente trabalho foi pesquisar a prevalência e o grau de associação da infecção por Helicobacter pylori e do vírus de Epstein-Barr em adenocarcinoma gástrico, em uma população do norte do Brasil. Foram analisadas 125 amostras de adenocarcinoma gástrico que foram submetidas à técnica de PCR para detecção de H. pylori e da cepa cagA de H. pylori, à técnica de hibridização in situ para detecção do EBV e à análise histopatológica para determinação de características clínico-patológicas e epidemiológicas. Observou-se o maior acometimento de pacientes do sexo masculino (68%) e de faixa etária acima de 50 anos (78%). A prevalência encontrada para H. pylori foi de 88%, e foi considerada alta quando comparada a estudos anteriores na região norte. A prevalência encontrada para o EBV foi de 9,6%. Os pacientes positivos para H. pylori-cagA+ apresentaram um risco relativo aumentado para adenocarcinoma do tipo intestinal. A frequência para os estádios III e IV foi de 82,4%, evidenciando que o diagnóstico desta neoplasia é geralmente realizado tardiamente. Os casos positivos para urease apresentaram um fator de risco relativo (OR=4,231) maior que quatro vezes, para H. pylori-cagA+, que é a cepa mais virulenta de H. pylori. Não houve significância estatística para a associação entre H. pylori e EBV na população estudada, porém os casos positivos para EBV apresentaram 100% de positividade para H. pylori, sugerindo uma possível atuação sinérgica destes agentes na carcinogênese gástrica.

O câncer gástrico permanece como grave problema de saúde pública, com elevada morbidade e mortalidade. Os diagnósticos ocorrem, na maioria dos casos, em estágios avançados da doença, situação na qual as opções terapêuticas disponíveis apresentam reduzida eficácia. Não obstante o avanço do conhecimento a respeito da carcinogênese do adenocarcinoma gástrico, em especial sobre mecanismos genéticos e epigenéticos envolvidos, a aplicabilitadade clínica destes conhecimentos permanece limitada. Objetivando identificar potenciais biomarcadores no câncer gástricos, foi realizado estudo utilizando microarray, comparando-se expressões gênicas em adenocarcinomas gástricos e amostras pareadas de mucosa gástrica não neoplásica. Os resultados preliminares demonstraram diferenças significativas de expressão em 53 genes. Dentre estes, foram selecionados os genes TFF1 e TFF2 para validação da expressão por PCR em tempo real em 78 amostras adicionais. As expressões de TFF1 e TFF2 foram significativamente reduzidas em amostras de adenocarcinoma gástrico, quando comparas aos tecidos pareados não neoplásicos (p<0,05). Adicionalmente, a expressão do gene TFF2 foi significantemente mais reduzida no tipo intestinal do que no tipo difuso. A expressão dos dois genes apresentou uma forte correlação, o padrão de expressão semelhante sugere que TFF1 e TFF2 podem partilhar elementos reguladores comuns. Essa hipótese é intensificada devido a pequena distancia física entre eles. Os resultados sugerem fortemente a participação de TFF1 e TFF2 na carcinogenese gástrica e demonstram um potencial para utilização clínica dos referidos genes como biomarcadores e eventuais alvos terapêuticos no adenocarcinoma gástrico.

O câncer gástrico representa um grave problema de saúde pública mundial. A alta incidência de tumores avançados com baixa sobrevida pelas metástases, sobretudo no norte do país, nos fez realizar o estudo comparativo das linhagens de adenocarcinomas gástricos metastáticos (AGP01) com adenocarcinomas gástricos sem metástases (ACP02) através da avaliação proteômica da via de mobilidade celular, que possam ter relação com a formação dessas metástases. Foi realizado estudo proteômico das linhagens AGP01 e ACP02 através da técnica da cromatografia líquida de alta performance 2D Nanoultra (UPLC) em conjunto com nanoESI-MSE (MudPIT) e análise funcional das proteínas diferencialmente expressas no programa Ingenuity Pathways Analysis (IPA). Observamos 19 proteínas com aumento da expressão na linhagem AGP01 em relação a ACP02, as quais apresentam relação com movimento, organização e morfologia celular, onde podemos sugerir que as proteínas ACTB, ANXA1, LGALS1, IQGAP1, EZR, MSN, MYH9 e S100A11, de acordo com nossos achados e corroborados pela literatura pesquisada, tem associação com a metástase de adenocarcinomas gástricos. Outras proteínas se mostraram em forte expressão em nosso estudo, mas na literatura pesquisada sua expressão tem relação com as vias de disseminação apenas de outros tumores, como: mama (RAB5C), pulmão (PLS1 e CAP1), reto (ACTN1) e GIST (SYNE2). Conflitantes com nosso estudo, as expressões das proteínas CAPZA1, FLNA e FLNC, foram observadas na literatura como um inibidor de avanço tumoral, enquanto que as expressão das proteínas MYL6, MYL6B, e ACTN2, aparecem pela primeira vez como tendo relação com a mobilidade celular, invasão e metástase em câncer.
