Jorge Lobo's disease is a cutaneous and subcutaneous mycosis that affects patients in the Amazon region. The number of patients is relatively small, but the real situation of the disease as public health problem is not known, because Jorge Lobo's disease is not a notifiable disease. This study aims to report the clinical evolution in patients affected and to determine the prevalence and areas of occurrence of the disease. A retrospective study was carried out based on the analysis of the clinical records, which included a collection of photographs of patients in the Department of Sanitary Dermatology, in Rio Branco, and patients seen in the interior of the state. In a decade, in Rio Branco, 249 cases of the disease were reported, 30 were females and 219 males. Of these patients, 153 had localized lesions, 94 of them were on one ear, 55 had multifocal lesions and 41 had disseminated lesions. The average time between the onset of symptoms and diagnosis was 19 years. The average age at the time of diagnosis was 53 years, and ages ranged from 14 to 96 years.

Rickettsia typhi is the causal agent of murine typhus; a worldwide zoonotic and vector-borne infectious disease, commonly associated with the presence of domestic and wild rodents. Human cases of murine typhus in the state of Yucatán are frequent. However, there is no evidence of the presence of Rickettsia typhi in mammals or vectors in Yucatán. The presence of Rickettsia in rodents and their ectoparasites was evaluated in a small municipality of Yucatán using the conventional polymerase chain reaction technique and sequencing. The study only identified the presence of Rickettsia typhi in blood samples obtained from Rattus rattus and it reported, for the first time, the presence of R. felis in the flea Polygenis odiosus collected from Ototylomys phyllotis rodent. Additionally, Rickettsia felis was detected in the ectoparasite Ctenocephalides felis fleas parasitizing the wild rodent Peromyscus yucatanicus. This study’s results contributed to a better knowledge of Rickettsia epidemiology in Yucatán.

This study longitudinally investigated the association between Triatoma dimidiata infestation, triatomine infection with Trypanosoma cruzi and household/backyard environmental characteristics in 101 homesteads in Molas and Yucatan, Mexico, between November 2009 (rainy season) and May 2010 (dry season). Logistic regression models tested the associations between insect infestation/infection and potential household-level risk factors. A total of 200 T. dimidiata were collected from 35.6% of the homesteads, mostly (73%) from the peridomicile. Of all the insects collected, 48% were infected with T. cruzi. Infected insects were collected in 31.6% of the homesteads (54.1% and 45.9% intra- and peridomiciliary, respectively). Approximately 30% of all triatomines collected were found in chicken coops. The presence of a chicken coop in the backyard of a homestead was significantly associated with both the odds of finding T. dimidiata (OR = 4.10, CI 95% = 1.61-10.43, p = 0.003) and the presence of triatomines infected with T. cruzi (OR = 3.37, CI 95% = 1.36-8.33, p = 0.006). The results of this study emphasize the relevance of chicken coops as a putative source of T. dimidiata populations and a potential risk for T. cruzi transmission.

Studies on host-parasite interaction in Jorge Lobo's disease are scarce, with no report in the literature on the phagocytosis of Lacazia loboi by phagocytic mononuclear cells. Thus, the objective of the present study was to assess the phagocytic activity of blood monocytes in the presence of L. loboi in patients with the disease and in healthy subjects (controls) over 3 and 24 hours of incubation. Statistical analyses of the results showed no significant difference in percent phagocytosis of the fungus between patient and control monocytes. With respect to incubation time, however, there was a significant difference, in that percent phagocytosis was higher at 3 hours than at 24 hours (p <0.01). These results suggest that monocytes from patients with the mycosis are able to phagocyte the fungus, as also observed in control individuals.

A doença de Jorge Lobo é uma micose cutânea/subcutânea de evolução crônica, causada pelo fungo Lacazia loboi. Devido às características epidemiológicas e poucos estudos relacionados aos aspectos imunológicos dessa doença, o objetivo do trabalho foi investigar uma possível associação das especificidades HLA de classe II em 21 pacientes portadores da doença de Jorge Lobo, comparando com indivíduos sadios de mesma etnia. As tipificações HLA foram realizadas pelo método de PCR-SSP. O resultado não revelou qualquer tipo de associação entre os antígenos HLA e doença de Jorge Lobo. Embora sem significância estatística, foi observada a diminuição da freqüência do antígeno HLA-DR7 no grupo dos pacientes em relação aos controles (0% x 18%), sugerindo uma associação negativa (protetora) entre HLA-DR7 e doença de Jorge Lobo. Contudo, estudos devem ser continuados, objetivando melhor entendimento nos mecanismos envolvidos na suscetibilidade e/ou proteção dessa doença.

O trabalho que se segue consiste exactamente na identificação, estudo e edição do texto transmitido por estes testemunhos; tendo em conta que existem consideráveis diferenças entre os textos das duas edições, foi necessário proceder-se a uma edição crítica, cujos critérios serão adiante definidos. O facto de existirem edições do século seguinte, e divergentes entre si, sugere que tanto esta comédia como o seu Autor terão gozado de uma considerável fortuna junto do público: uma peça de teatro que continua a ser reeditada em Portugal e em Espanha muito tempo depois da sua primeira edição, era forçosamente importante. Nesse sentido, tudo o que nela se contém deve ser encarado como representativo da sua época e do gosto do público do tempo; por isso, tornou-se necessário elaborar e apresentar ao leitor um conjunto de índices – personalidades, topónimos, autores, entidades mitológicas – do mundo referencial existente na comédia. Este trabalho é, assim, constituído por duas grandes partes: na primeira, faz-se uma revisão crítica do estado dos conhecimentos sobre Jorge Ferreira de Vasconcelos e a sua obra, e desta comédia no conjunto da obra do Autor. Na segunda parte, procede-se à identificação da tradição impressa da obra e, naturalmente, ao estabelecimento do respectivo texto crítico. Um anexo, constituído pelos já referidos índices, completa o trabalho.

Purpose: Higher myopic refractive errors are associated with serious ocular complications that can put visual function at risk. There is respective interest in slowing and if possible stopping myopia progression before it reaches a level associated with increased risk of secondary pathology. The purpose of this report was to review our understanding of the rationale(s) and success of contact lenses (CLs) used to reduce myopia progression. Methods: A review commenced by searching the PubMed database. The inclusion criteria stipulated publications of clinical trials evaluating the efficacy of CLs in regulating myopia progression based on the primary endpoint of changes in axial length measurements and published in peerreviewed journals. Other publications from conference proceedings or patents were exceptionally considered when no peer-review articles were available. Results: The mechanisms that presently support myopia regulation with CLs are based on the change of relative peripheral defocus and changing the foveal image quality signal to potentially interfere with the accommodative system. Ten clinical trials addressing myopia regulation with CLs were reviewed, including corneal refractive therapy (orthokeratology), peripheral gradient lenses, and bifocal (dual-focus) and multifocal lenses. Conclusions: CLs were reported to be well accepted, consistent, and safe methods to address myopia regulation in children. Corneal refractive therapy (orthokeratology) is so far the method with the largest demonstrated efficacy in myopia regulation across different ethnic groups. However, factors such as patient convenience, the degree of initial myopia, and non-CL treatments may also be considered. The combination of different strategies (i.e., central defocus, peripheral defocus, spectral filters, pharmaceutical delivery, and active lens-borne illumination) in a single device will present further testable hypotheses exploring how different mechanisms can reinforce or compete with each other to improve or reduce myopia regulation with CLs.
