Histoplasmosis is a fungal disease that is endemic in Brazil. It may present as chronic pulmonary infection or in disseminated form. Disseminated histoplasmosis frequently affects the adrenal gland; however, unilateral involvement in immunosuppressed patients is the usual presentation. We report a case of an elderly immunocompetent male with history of weight loss, fever and bilateral adrenal mass who was successfully treated with itraconazole.

Comparou-se a suscetibilidade à anfotericina B de Candida spp isoladas de candidemias, sendo: 41 do Hospital Universitário de Santa Maria, 56 do Hospital de Clínicas de Porto Alegre e 47 da Santa Casa, Complexo Hospitalar de Porto Alegre. Os testes foram baseados no documento M27-A2 do Clinical Laboratory Standards Institute. Todavia, foram empregadas 20 concentrações de anfotericina B, variáveis entre 0,1 e 2µg/ml. Os testes foram realizados nos meios RPMI 1640 com glicose, antibiotic medium 3 e yeast nitrogen base dextrosado. O caldo antibiotic medium 3 gerou amplas faixas de concentrações inibitórias mínimas e concentrações fungicidas mínimas quando comparado aos demais. As variações de suscetibilidade entre os hospitais foram melhor detectadas no antibiotic médium 3; os isolados do Hospital Universitário de Santa Maria evidenciaram menor sensibilidade do que os da Santa Casa, Complexo Hospitalar de Porto Alegre (p < 0,05). As causas das variações de suscetibilidade não foram avaliadas mas apontam para a necessidade de vigilância da suscetibilidade a anfotericina B.

As hepatites B e C continuam sendo um importante problema de saúde pública no Brasil. Neste estudo, determinou-se a prevalência de marcadores sorológicos para as hepatites B e C em indivíduos do Estado do Pará, atendidos no Laboratório Central de Saúde Pública do Pará, no período de janeiro de 2002 a dezembro de 2005. Foram realizados 11.282 exames para a pesquisa do HBsAg, 2.342 para o anti-HBc e 5.542 para o anti-vírus da hepatite C. A prevalência de HBsAg foi de 3,6% e predominou na faixa etária de 20 a 29 anos, enquanto que o anti-HBc foi observado em 37,7% dos indivíduos. A prevalência do antivírus da hepatite C foi de 3,6% e predominou entre indivíduos acima de 50 anos. Assim, as freqüências dos marcadores encontradas no Pará foram mais altas que em vários outros estados do país, sugerindo a necessidade de medidas de saúde publica mais eficazes no combate a estes agravos na região.

The purpose of this study was to compare the agar dilution and broth microdilution methods for determining the minimum inhibitory concentration (MIC) of fluconazole, itraconazole, ketoconazole, griseofulvin and terbinafine for 60 dermatophyte samples belonging to the species Trichophyton rubrum, Trichophyton mentagrophytes and Microsporum canis. The percentage agreement between the two methods, for all the isolates with < 2 dilutions that were tested was 91.6% for ketoconazole and griseofulvin, 88.3% for itraconazole, 81.6% for terbinafine and 73.3% for fluconazole. One hundred percent agreement was obtained for Trichophyton mentagrophytes isolates evaluated with ketoconazole and griseofulvin. Thus, until a reference method for testing the in vitro susceptibility of dermatophytes is standardized, the similarity of the results between the two methods means that the agar dilution method may be useful for susceptibility testing on these filamentous fungi.

Nos dias de hoje, as tecnologias deixaram de ser tabus para qualquer faixa etária, tor-nando-se uma necessidade emergente. Os smartphones estão entre os dispositivos mais utilizados pela população em geral no quotidiano. Adicionando aos novos estilos de vida atuais, a medicação diária também tem vindo a crescer nos últimos tempos, sendo esta utilizada em casos de hipertensão, colesterol elevado, diabetes ou outras doenças perpétuas, como também na toma de multivitamínicos, medicação para aumento de produtividade, contracetivos ou antibióticos e anti-inflamatórios em casos de constipa-ções ou gripes. Aliado a um estilo de vida célere, a toma de medicação no momento indicado facilmente é esquecida, tornando os tratamentos ineficientes. A aplicação “Hora do comprimido”, desenvolvida para dispositivos Android, permite auxiliar os utilizadores na toma da sua medicação, contínua ou temporária, facilitando o quotidiano do utilizador, viabilizando assim um tratamento completo, sem falhas nas tomas ou feitas fora de horas.

The aims of this study were to carry out a serological survey of canine leishmaniasis and identify the phlebotomine fauna in the urban area of Bonito, Mato Grosso do Sul. The serological survey was conducted on a sample of 303 dogs, by means of the indirect immunofluorescence test. Phlebotomines were captured using automated light traps. The serological survey found that 30% of the dogs were seropositive, both from the center and from all districts of the town. A total of 2,772 specimens of phlebotomines were caught and the species most found was Lutzomyia longipalpis (90.4%), which corroborated its role as the vector of for canine visceral leishmaniasis in the region. Phlebotomines of the species Bichromomyia flaviscutellata (the main vector for Leishmania (Leishmania) amazonensis) and Nyssomyia whitmani (the vector for Leishmania (Viannia) brasiliensis) were also caught. The findings indicate the need for continuous epidemiological surveillance, with attention towards diminishing the vector breeding sites and the transmission of these diseases in that region.

É descrito um envenenamento pela ingestão de vísceras de um baiacu-pintado (Sphoeroides testudineus) por uma criança de dois anos, que apresentou sudorese fria, fraqueza muscular progressiva, parada cardiorrespiratória e morte. São discutidos os riscos do consumo da carne e vísceras de baiacus, fato comum em certas regiões do Brasil.

INTRODUÇÃO: Este trabalho mostra a padronização e o uso do método imunoenzimático utilizando células infectadas como antígeno (EIA-ICC) no diagnóstico sorológico rotineiro do dengue. MÉTODOS: Na otimização do teste, com a dose de 1.000 TCID50 de vírus do dengue tipo 3 (DENV-3), foram utilizadas 100.000 células C636 infectadas 1000 TCID50 (DENV-3). RESULTADOS: Os resultados obtidos com EIA-ICC foram comparados com o kit comercial de dengue HUMAN. Os resultados foram altamente coincidentes; o EIA-ICC mostrou-se moderadamente sensível e com alta especificidade. O teste foi usado no diagnóstico sorológico de 1.797 amostras sorológicas de casos suspeitos de dengue durante a epidemia de Ribeirão Preto, em 2006. Na avaliação sorológica, 228 amostras foram positivas para IgM contra DENV-3, e 235 amostras foram positivas para IgG contra DEV-3, e em 35 amostras detectou-se positividade para IgM e IgG. CONCLUSÕES: O EIA-ICC mostrou-se confiável e simples sendo adequado ao diagnóstico sorológico do dengue.

INTRODUCTION: Infection with Leishmania chagasi is the most common clinical presentation for visceral leishmaniaisis in endemic areas. The municipality of Raposa is an endemic area in State of Maranhão, Brazil, and have had registration cases of visceral leishmaniasis disease. For this reason, a cross- sectional study was conducted to evaluate the risk factors for infection with L. chagasi detected by Montenegro skin test. METHODS: The sample comprised 96% of the inhabitants of the villages of Maresia, Pantoja, and Marisol located in the municipality of Raposa, corresponding to 1,359 subjects. Data were collected using a questionnaire. Univariate and multivariate logistic regression models were applied to evaluate the association between the variables studied and infection of L. chagasi. RESULTS: The variables associated with infection upon nonadjusted analysis were a straw roof, mud walls, floors of beaten earth, presence of sand flies inside or outside of the dwelling, and bathing outdoors. Adjusted analysis showed that the presence of sand flies inside/outside the dwelling was a risk factor, and age younger than 10 years was a protective factor against asymptomatic infection. CONCLUSIONS: The results highlight the extent to which precarious living conditions of the population strengthen the epidemiological chain of visceral leishmaniasis.

INTRODUCTION: Leprosy is an infectious disease caused by Mycobacterium leprae. The aim of this study was to describe the epidemiological, clinical, and operational aspects of leprosy carriers. METHODS: A cross-sectional study leprosy patients assisted in São Luis, MA, was performed. RESULTS: Of the 85 cases analyzed, 51.7% were male participants, and 60% were brown. Concerning the age, 54.8% of women were between 35 and 49 years, and 57.6% of men were between 20 and 34 years. Lepromatous leprosy was found in 42.3% of cases, and the multibacillary form was found in 72.9%. The skin smear was positive in 42.3%. The occurrence of reaction was found in 43.5% of cases, and 83.5% had no Bacillus Calmette-Guérin scar. Leprosy in the family was reported by 44.7% of the patients. Most of the individuals (96.4%) lived in houses made of brick with more than three rooms (72.6%) and two persons per room (65.1%). Concerning the level of education, 41.4% of women and 34.1% of men had more than one to three years of education. The most evaluated age group in the beginning of the treatment was that of 35 to 49 years with a Grade 0 incapability (64.5%), and that in the end was the age group of 20 to 34 (29.9%) with Grade 0, 30.7% Grade 1, and 11.5% Grade 2. CONCLUSIONS: The frequency of multibacillary forms found in this study and the cases in family members point out delayed diagnoses. Thus, early diagnosis and appropriate treatment are important in decreasing the outcome of disabilities.

As Contas Nacionais ocupam uma posição de destaque no conjunto de todas as estatísticas económicas produzidas por um determinado país ou região, pois o sistema no qual assenta a sua produção, estabelece um padrão internacionalmente aceite em termos de conceitos, definições, classificações e metodologias permitindo a comparabilidade entre as várias estatísticas produzidas ao nível nacional e internacional, assim como a adaptação a qualquer país e a harmonização entre os diferentes sistemas estatísticos. No seu cerne está a medição do nível de crescimento e desempenho de uma economia, assim como a determinação da capacidade (+) / necessidade (‐) de financiamento de uma economia A produção das contas nacionais baseia‐se no Sistema de Contas Nacionais das Nações Unidas, cujo objetivo é a medição do nível de atividade económica (produto interno bruto), através do registo dos fluxos e stocks gerados entre os agentes económicos de uma economia, e entre estes e o resto do mundo, com base em padrões e recomendações internacionalmente aceites. O presente trabalho apresenta uma proposta de modelo de produção das estatísticas de contas nacionais financeiras em Moçambique, com vista a completar o modelo atualmente existente que comporta apenas a produção das contas não financeiras. Os resultados apresentados pelo modelo estão organizados em matrizes que resumem as operações financeiras realizadas entre os agentes económicos, onde se cruzam os instrumentos financeiros com os diversos setores institucionais residentes e o resto do mundo enquanto intervenientes nas operações realizadas. Nesse sentido, são apresentados, (i) quadros de património que apresentam o stock de ativos financeiros e passivos existentes num dado momento, assim como o saldo que representa o património líquido, (ii) a matriz “quem a quem” que resume as operações em forma de matriz tridimensional, onde aparecem os instrumentos financeiros e as duas partes intervenientes na operação na qualidade de credor e devedor, e (iii) a matriz de variação de stocks que, para efeitos deste trabalho, é apresentado como proxy da matriz de transações em ativos financeiros e passivos realizados num determinado momento, assim como a poupança financeira.

Introduction Methicillin-resistant Staphylococcus aureus (MRSA) strains have been responsible for many nosocomial outbreaks. Within hospitals, colonized employees often act as reservoirs for the spread of this organism. This study collected clinical samples of 91 patients admitted to the intensive care unit (ICU), hemodialysis/nephrology service and surgical clinic, and biological samples from the nasal cavities of 120 professionals working in those environments, of a University Hospital in Recife, in the State of Pernambuco, Brazil. The main objective of this study was to determine the occurrence and dissemination of methicillin- and vancomycin-resistant Staphylococcus spp. Methods The isolates obtained were tested for susceptibility to oxacillin and vancomycin and detection of the mecA gene. In addition, the isolates were evaluated for the presence of clones by ribotyping-polymerase chain reaction (PCR). Results MRSA occurrence, as detected by the presence of the mecA gene, was more prevalent among nursing technicians; 48.1% (13/27) and 40.7% (11/27) of the isolates were from health professionals of the surgical clinic. In patients, the most frequent occurrence of mecA-positive isolates was among the samples from catheter tips (33.3%; 3/9), obtained mostly from the hemodialysis/nephrology service. Eight vancomycin-resistant strains were found among the MRSA isolates through vancomycin screening. Based on the amplification patterns, 17 ribotypes were identified, with some distributed between patients and professionals. Conclusions Despite the great diversity of clones, which makes it difficult to trace the source of the infection, knowledge of the molecular and phenotypic profiles of Staphylococcus samples can contribute towards guiding therapeutic approaches in the treatment and control of nosocomial infections.

Introduction Leprosy is a chronic infectious disease that is caused by Mycobacterium leprae. The objective of this study was to evaluate the risk factors that are associated with neural alterations and physical disabilities in leprosy patients at the time of diagnosis. Methods A prospective cross-sectional study was conducted on 155 leprosy patients who participated in a program that aimed to eliminate leprosy from São Luis, State of Maranhão. Results Patients who were 31-45 years of age, were older than 60 years of age or had a partner were more likely to have a disability. Patients with partners were 1.14 times more likely (p = 0.025) to have disabilities of the hands. The frequency of disabilities in the feet among the patients with different clinical forms of leprosy was statistically significant. Conclusions The identification of risk factors that are associated with neural alterations and physical disabilities in leprosy patients is important for diagnosing the disease because this approach enables physicians to plan and prioritize actions for the treatment and monitoring of patients.

Epstein-Barr virus (EBV)-related post-transplant lymphoproliferative disease (PTLD) is one of the most serious complications associated with solid organ and hematopoietic stem cell transplantation. PTLD is most frequently seen with primary EBV infection post-transplant, a common scenario for pediatric solid organ recipients. Risk factors for infection or reactivation of EBV following solid organ transplant are stronger immunosuppressive therapy regimens, and being seronegative for receptor. For hematopoietic stem cell transplantation, the risk factors relate to the type of transplant, human leukocyte antigen disparity, the use of stronger immunosuppressants, T-cell depletion, and severe graft-versus-host disease. Mortality is high, and most frequent in patients who develop PTLD in the first six months post-transplant. The primary goal of this article is to provide an overview of the clinical manifestations, diagnosis, accepted therapies, and management of EBV infection in transplant recipients, and to suggest that the adoption of monitoring protocols could contribute to a reduction in related complications.
