L’Ifremer coordonne, sur l’ensemble du littoral métropolitain, la mise en oeuvre de réseaux d’observation et de surveillance de la mer côtière. Ces outils de collecte de données sur l’état du milieu marin répondent à deux objectifs : · servir des besoins institutionnels en fournissant aux pouvoirs publics des informations répondant aux exigences de la Directive Cadre sur l’Eau (DCE), des conventions régionales marines (OSPAR et Barcelone) et de la réglementation sanitaire relative à la salubrité des coquillages des zones de pêche et de production conchylicoles ; · acquérir des séries de données nourrissant les programmes de recherche visant à mieux comprendre le fonctionnement des écosystèmes côtiers et à identifier les facteurs à l’origine des changements observés dans ces écosystèmes. Le dispositif comprend : le réseau d’observation et de surveillance du phytoplancton et des phycotoxines (REPHY) qui porte aussi sur l’hydrologie et les nutriments, le réseau d’observation de la contamination chimique (ROCCH), le réseau de contrôle microbiologique (REMI) et le réseau de surveillance benthique (REBENT). Ces réseaux sont mis en oeuvre par les Laboratoires Environnement Ressources (LER) qui opèrent également des observatoires de la ressource : l'observatoire national conchylicole (RESCO) qui évalue la survie, la croissance et la qualité des huîtres creuses élevées sur les trois façades maritimes françaises ; et le réseau de pathologie des mollusques (REPAMO). Pour approfondir les connaissances sur certaines zones particulières et enrichir le diagnostic de la qualité du milieu, plusieurs Laboratoires Environnement Ressources mettent aussi en oeuvre des réseaux régionaux : sur la côte d’Opale (SRN), sur le littoral normand (RHLN), dans le bassin d’Arcachon (ARCHYD) ainsi que dans les étangs languedociens et corses (RSL). Les prélèvements et les analyses sont effectués sous démarche qualité. Les analyses destinées à la surveillance sanitaire des coquillages sont réalisées par des laboratoires agréés. Les données obtenues sont validées et saisies par les laboratoires. Elles intègrent la base de données Quadrige² qui héberge le référentiel national des données de la surveillance des eaux littorales et forme une composante du Système national d’information sur l’eau (SIEau). Les bulletins régionaux annuels contiennent une synthèse et une analyse des données collectées par les réseaux pour les différentes régions côtières. Des représentations graphiques homogènes pour tout le littoral français, assorties de commentaires, donnent des indications sur les niveaux et les tendances des paramètres mesurés. Les stations d’observation et de surveillance figurant sur les cartes et les tableaux de ces bulletins régionaux s’inscrivent dans un schéma national. Une synthèse des résultats portant sur l’ensemble des côtes françaises métropolitaines complète les bulletins des différentes régions. Business journalism comes under persistent criticism for serving its historic readership of brokers and business people while lacking sufficient autonomy and failing to sufficiently question or challenge powerful corporate and economic interests. This is a dominant theme in media criticism of the savings and loan crisis⁠ and 2008 financial crisis.⁠ Against this backdrop, this dissertation asks: Is this critique valid, and if so, how can business journalism improve? To engage these questions, this dissertation examines the question of autonomy in business journalism in an unlikely place: the trade press. The central case study is coverage of the savings and loan crisis by the National Thrift News, a small financial services newspaper that won a George Polk award for its reporting in 1988. How could a small trade newspaper succeed in some instances when larger news organizations failed to connect the dots? The National Thrift News created a newsroom environment that celebrated reporter autonomy and independence. In some cases, it used its insider knowledge and consistent beat reporting to serve both its core readers and the broader society by uncovering savings and loan corruption. This study will highlight a long-running debate among theorists of journalistic professionalism by arguing that the commercial and advertising model in journalism does not inevitably compromise journalistic independence⁠ but rather can help pave a way forward for a more independent press. It therefore challenges the political economy critique of journalism, which holds that external forces such as capitalism harm press independence. This case suggests journalistic independence and individual agency remain powerful forces in newsrooms. Lastly, the dissertation argues that in an era of media downsizing, the trade press can perform an even more useful watchdog role over industry if the mainstream news media acknowledges and pursues some of the innovative trade reporting.

Introducción. Los Trastornos Musculoesqueléticos (TME) son lesiones de músculos, tendones, nervios y articulaciones que tienen como principal síntoma el dolor y se presentan con gran frecuencia en trabajos que requieren una actividad física importante o como consecuencia de malas posturas sostenidas durante largos periodos de tiempo. Objetivo: Determinar la prevalencia de síntomas osteomusculares y su asociación con factores ergonómicos en una población de trabajadores administrativos de una empresa de servicios en Bogotá, Colombia, en el año 2015. Método: Se realizó un estudio de corte transversal. Se analizaron datos secundarios correspondientes a una base de datos de una población de 450 trabajadores pertenecientes al área administrativa de una empresa de servicios, de los cuales se incluyeron 150 registros que tenían la información completa de las variables del estudio. Se tuvieron en cuenta variables sociodemográficas, ocupacionales y la presencia de síntomas osteomusculares. Incluyó también el registro de condiciones de la estación de trabajo recolectada mediante inspecciones de puestos de trabajo (registro fotográfico y formato estandarizado para condiciones observadas). Se realizó un análisis descriptivo considerando las medidas de tendencia central para las variables cuantitativas y distribución de frecuencias para las variables cualitativas. Resultados: Predominó el género femenino (56%) dentro de la población estudiada, con una edad media para ambos géneros de 35 años (75%), (DS ± 7.0). El síntoma más frecuentemente reportado fue el dolor en cuello (17%) seguido por dolor en muñeca (11%) y dolor lumbar (8.7%). Del total de los trabajadores evaluados, el 80% correspondió al cargo ejecutivo, que reúne funciones administrativas especializadas complejas y/o de supervisión directa. De la población femenina objeto de estudio el 76% se desempeñaban en el cargo ejecutivo. Las condiciones ergonómicas por mejorar de mayor frecuencia fueron altura y distancia de la pantalla (42%) y cables sueltos (30%). La frecuencia de síntomas fue más alta en el rango de edad de 31 a 45 años. Conclusiones: En este estudio se encontró que los síntomas osteomusculares más frecuentes se ubicaron en el segmento axial (cuello y región lumbar) y en mano y que las condiciones de altura y distancia de la pantalla y cables sueltos presentaron las frecuencias más altas de condiciones por mejorar. Se hace necesaria la implementación de programas de prevención de TME, la vigilancia de casos y la intervención de las condiciones de los puestos de trabajo. Se debe promover la identificación y promoción de conductas saludables dentro del ámbito laboral y la prevención de condiciones que favorezcan la aparición del riesgo biomecánico.

Introducción: El cáncer gástrico es uno de los más frecuentes a nivel mundial y Colombia se sitúa entre los países de mayor incidencia en este tipo de patología. Objetivo: Describir las características epidemiológicas, clínicas, el tratamiento administrado y los desenlaces inmediatos de los pacientes con diagnóstico de cáncer gástrico atendidos en el Hospital Universitario Mayor de Bogotá entre los años 2011 y 2014. Metodología: Se realizó un estudio observacional descriptivo con diagnóstico de cáncer gástrico. Se realizaron análisis univariados por medio de proporciones para las variables cualitativas y medidas de tendencia central para las variables cuantitativas según la distribución. Resultados: Un total de 189 pacientes fueron analizados. El dolor fue el síntoma más frecuente en los pacientes (30.7%) y el principal signo encontrado fue una masa palpable en abdomen (9,5%). Los pacientes fueron sometidos a diferentes abordajes terapéuticos, la mayoría recibieron manejo paliativo no quirúrgico (52.9%) y la opción quirúrgica más usada en los pacientes fue la gastrectomía total (20.6%), y la subtotal (16,4) seguidas de quimioterapia y/o radiación perioperatoria. Los pacientes que sobrevivieron a los 2 años fueron 7,4% del total. Conclusiones: El registro de los pacientes con cáncer gástrico es bueno en el Méderi-Hospital Universitario Mayor es bueno y permite caracterizar los pacientes, la presentación de la patología y los resultados del tratamiento que concuerdan con los presentados en contextos similares en la literatura.

Cysticercosis results from the ingestion Taenia solium eggs directly by faecal-oral route or contaminated food or water. Human tapeworm carriers who have become infected after ingesting pork meat contaminated with cysticerci release these eggs. Cysticercosis occurs after tapeworm eggs are ingested by an intermediate host (pig or human) and then hatch, migrate, and lodge in the host's tissues, where they develop onto larval cysticerci. When they lodged in the central nervous system of humans, results in the disease condition called Neurocysticercosis (NCC), with a heterogeneous manifestations depending of the locations of cysts, number, size and their stage of evolution (1). Consequently the prognostic ranges from asymptomatic to situations leading to death in 2% to 9.8%. of cases (7) In swine’s there are few studies, but recent works have proved that animals, for the same reasons, also have neurological abnormalities, expressed by seizures, stereotypic walk in circles, chewing motions with foamy salivation included tonic muscle contractions followed by a sudden diminution in all muscle tone leading to collapse (2). Conventional domestic wastewater treatment processes may not be totally effective in inactivating parasites eggs from Taenia solium, allowing some contamination of soils and agricultural products (11). In Portugal there are some evidence of aggregation of human cysticercosis cases in specific regions, bases in ecological design studies (6). There are few information about human tapeworm carriers and social and economic factors associated with them. Success in knowledge and consequently in lowering transmission is limited by the complex network of biological and social factors that maintain the spread. Effective control of mostly zoonosis require One Health approach, after a real knowledge and transparency in the information provided by the institutions responsible for both animal and human health, allowing sustained interventions targeted at the transmission cycle's crucial nodes. In general, the model used to control, reflects a rural reality, where pigs are raised freely, poor sanitation conditions and incipient sanitary inspection. In cysticercosis, pigs are obligate intermediate hosts and so considered as first targets for control and used as sentinels to monitor environmental T. solium contamination (3). Usually environmental contamination with Taenia spp. eggs is a key issue in most of studies with landscape factors influencing presence of Taenia spp. antigens in both pigs and humans (5). Soil-related factors as well as socio-economic and behavioural factors are associated with the emergence of significant clustering human cysticercosis (4,5). However scarce studies has been produced in urban environmental and in developed countries with the finality to characterize the spatial pattern. There are still few data available regarding its prevalence and spatial distribution; Transmission patterns are likely to exhibit correlations as housing conditions, water supply, basic sanitation, schooling and birthplace of the individual or relatives, more than pigs rearing free, soil conditions (9). As a matter of fact, tapeworm carriers from endemic zones can auto-infect or transmit infection to other people or arrive already suffering NCC (as a result of travelling to or being a citizen from an endemic cysticercosis country) to a free cysticercosis country. Transmission is fecal-oral; this includes transmission through person-to-person contact, through autoinfection, or through contaminated food This has been happening in different continents as North America (5.4–18% been autochthonous), Europe and Australia (7). Recently, case reports of NCC have also emerged from Muslim countries. (10). Actually, different papers relate an epidemic situation in Spain and Portugal (7, 8). However the kind of study done does not authorize such conclusion. There are no evidence that infections were acquired in Portugal and there are not characterized the mode of transmission. Papers with these kind of information will be allow to have economic consequences resulted from artificial trade barriers with serious consequences for pig producers and pig meat trade. We need transparency in information’s that allow provide the basis to support the development and targeting of future effective control programmes (and prove we need that). So, to have a real picture of the disease, it is necessary integrate data from human, animal and environmental factors surrounding human and pig cases to characterize the pattern of the transmission. The design needs to be able to capture unexpected, and not common outcomes (routine data). We need to think “One Health” to get a genuine image of the situation.

Durante la década de los años 90 Costa Rica tomó conciencia del problema que significa el mal manejo de los desechos sólidos. En la provincia de Heredia durante estos años se formó una comisión interinstitucional e intermunicipal con el objetivo de darle una solución integral provincial a esta problemática. La Universidad Nacional, a través de la Escuela de Ciencias Geográficas (Progra­ma Morfoclimatología Aplicada y Dinámica Exógena, MADE), formó parte de esta comisión, para la cual realizó la primera etapa de un estudio tendiente a identificar los sitios óptimos para la instalación de un relleno sanitario provincial. La metodología utilizada se basó en la que se desarrolló en el Plan Nacional de Manejo de Desechos elaborado por el Gobierno de Costa Rica y la GTZ (Deutsche Gesellschaft für Technische Zusammenarbeit). Se identificaron cinco áreas técnicamente aptas, todas en el sector sur de la provincia, estas, dos presentan las condiciones menos adversas y, por lo tanto, consideran las áreas candidatas, estas se ubican al este de la provincia y son parte del cantón de San Isidro. Se recomienda realizar los estudios de detalle, considerados indispensables. En este sentido, recientemente la Empresa de Servicios Públicos Heredia (ESPH) ha utilizado como base esta investigación para efectuar mencionados estudios. ABSTRACT During the 1990s, Costa Rica began concentrating on the problem of poor  solid waste management. In the province of Heredia during these years, inter-institutional and inter-municipal commission was formed to seek integral solution to this problem at the province level. The Universic Nacional in Costa Rica, through the School of Geographical Sciences (Progr Morfoclimatología Aplicada y Dinámica Exógena, MADE), formed part of this commission and did a preliminary study in order to identify the optimum sites for installing a provincial sanitary refill. The methodology used was based on the one developed in the National Waste Management Plan designed by the government of Costa Rica and German agency GTZ (Deutsche Gesellschaft für Technische Zusammenarbeit ). Five technically suitable áreas were identified, all located in the southern sec of the province. Of these, two present less adverse conditions and therefore considered candidate áreas. These are located in the eastern part of this sector  and are in the cantón of San Isidro. Further detailed studies are recommended and are considered crucial, this sense, the Empresa de Servicios Públicos de Heredia (ESPH) has utilized this research as base study to perform the abo ve mentioned studies.

In this thesis, we investigate the role of applied physics in epidemiological surveillance through the application of mathematical models, network science and machine learning. The spread of a communicable disease depends on many biological, social, and health factors. The large masses of data available make it possible, on the one hand, to monitor the evolution and spread of pathogenic organisms; on the other hand, to study the behavior of people, their opinions and habits. Presented here are three lines of research in which an attempt was made to solve real epidemiological problems through data analysis and the use of statistical and mathematical models. In Chapter 1, we applied language-inspired Deep Learning models to transform influenza protein sequences into vectors encoding their information content. We then attempted to reconstruct the antigenic properties of different viral strains using regression models and to identify the mutations responsible for vaccine escape. In Chapter 2, we constructed a compartmental model to describe the spread of a bacterium within a hospital ward. The model was informed and validated on time series of clinical measurements, and a sensitivity analysis was used to assess the impact of different control measures. Finally (Chapter 3) we reconstructed the network of retweets among COVID-19 themed Twitter users in the early months of the SARS-CoV-2 pandemic. By means of community detection algorithms and centrality measures, we characterized users’ attention shifts in the network, showing that scientific communities, initially the most retweeted, lost influence over time to national political communities. In the Conclusion, we highlighted the importance of the work done in light of the main contemporary challenges for epidemiological surveillance. In particular, we present reflections on the importance of nowcasting and forecasting, the relationship between data and scientific research, and the need to unite the different scales of epidemiological surveillance.

Maternal mortality (MM) is a core indicator of disparities in women's rights. The study of Near Miss cases is strategic to identifying the breakdowns in obstetrical care. In absolute numbers, both MM and occurrence of eclampsia are rare events. We aim to assess the obstetric care indicators and main predictors for severe maternal outcome from eclampsia (SMO: maternal death plus maternal near miss). Secondary analysis of a multicenter, cross-sectional study, including 27 centers from all geographic regions of Brazil, from 2009 to 2010. 426 cases of eclampsia were identified and classified according to the outcomes: SMO and non-SMO. We classified facilities as coming from low- and high-income regions and calculated the WHO's obstetric health indicators. SPSS and Stata softwares were used to calculate the prevalence ratios (PR) and respective 95% confidence interval (CI) to assess maternal characteristics, clinical and obstetrical history, and access to health services as predictors for SMO, subsequently correlating them with the corresponding perinatal outcomes, also applying multiple regression analysis (adjusted for cluster effect). Prevalence of and mortality indexes for eclampsia in higher and lower income regions were 0.2%/0.8% and 8.1%/22%, respectively. Difficulties in access to health care showed that ICU admission (adjPR 3.61; 95% CI 1.77-7.35) and inadequate monitoring (adjPR 2.31; 95% CI 1.48-3.59) were associated with SMO. Morbidity and mortality associated with eclampsia were high in Brazil, especially in lower income regions. Promoting quality maternal health care and improving the availability of obstetric emergency care are essential actions to relieve the burden of eclampsia.

The vast majority of maternal deaths in low-and middle-income countries are preventable. Delay in obtaining access to appropriate health care is a fairly common problem which can be improved. The objective of this study was to explore the association between delay in providing obstetric health care and severe maternal morbidity/death. This was a multicentre cross-sectional study, involving 27 referral obstetric facilities in all Brazilian regions between 2009 and 2010. All women admitted to the hospital with a pregnancy-related cause were screened, searching for potentially life-threatening conditions (PLTC), maternal death (MD) and maternal near-miss (MNM) cases, according to the WHO criteria. Data on delays were collected by medical chart review and interview with the medical staff. The prevalence of the three different types of delays was estimated according to the level of care and outcome of the complication. For factors associated with any delay, the PR and 95%CI controlled for cluster design were estimated. A total of 82,144 live births were screened, with 9,555 PLTC, MNM or MD cases prospectively identified. Overall, any type of delay was observed in 53.8% of cases; delay related to user factors was observed in 10.2%, 34.6% of delays were related to health service accessibility and 25.7% were related to quality of medical care. The occurrence of any delay was associated with increasing severity of maternal outcome: 52% in PLTC, 68.4% in MNM and 84.1% in MD. Although this was not a population-based study and the results could not be generalized, there was a very clear and significant association between frequency of delay and severity of outcome, suggesting that timely and proper management are related to survival.

This study aimed to characterize which regulatory logics (other than government regulation) result in healthcare output, using a two-stage qualitative study in two municipalities in the ABCD Paulista region in São Paulo State, Brazil. The first stage included interviews with strategic actors (managers and policymakers) and key health professionals. The second phase collected life histories from 18 individuals with high health-services utilization rates. An analysis of the researchers' involvement in the field allowed a better understanding of the narratives. Four regulatory systems were characterized (governmental, professional, clientelistic, and lay), indicating that regulation is a field in constant dispute, a social production. Users' action produces healthcare maps that reveal the existence of other possible health system arrangements, calling on us to test shared management of healthcare between health teams and users as a promising path to the urgent need to reinvent health.

The 2005 National Institutes of Health (NIH) Consensus Conference proposed new criteria for diagnosing and scoring the severity of chronic graft-versus-host disease (GVHD). The 2014 NIH consensus maintains the framework of the prior consensus with further refinement based on new evidence. Revisions have been made to address areas of controversy or confusion, such as the overlap chronic GVHD subcategory and the distinction between active disease and past tissue damage. Diagnostic criteria for involvement of mouth, eyes, genitalia, and lungs have been revised. Categories of chronic GVHD should be defined in ways that indicate prognosis, guide treatment, and define eligibility for clinical trials. Revisions have been made to focus attention on the causes of organ-specific abnormalities. Attribution of organ-specific abnormalities to chronic GVHD has been addressed. This paradigm shift provides greater specificity and more accurately measures the global burden of disease attributed to GVHD, and it will facilitate biomarker association studies.
