An integrated approach to climate change impact assessment is explored by linking established models of regional climate (SDSM), water resources (CATCHMOD) and water quality (INCA) within a single framework. A case study of the River Kennet illustrates how the system can be used to investigate aspects of climate change uncertainty, deployable water resources, and water quality dynamics in upper and lower reaches of the drainage network. The results confirm the large uncertainty in climate change scenarios and freshwater impacts due to the choice of general circulation model (GCM). This uncertainty is shown to be greatest during summer months as evidenced by large variations between GCM-derived projections of future tow river flows, deployable yield from groundwater, severity of nutrient flushing episodes, and Long-term trends in surface water quality. Other impacts arising from agricultural land-use reform or delivery of EU Water Framework Directive objectives under climate change could be evaluated using the same framework. (c) 2006 Elsevier B.V. The River Lugg has particular problems with high sediment loads that have resulted in detrimental impacts on ecology and fisheries. A new dynamic, process-based model of hydrology and sediments (INCA- SED) has been developed and applied to the River Lugg system using an extensive data set from 1995–2008. The model simulates sediment sources and sinks throughout the catchment and gives a good representation of the sediment response at 22 reaches along the River Lugg. A key question considered in using the model is the management of sediment sources so that concentrations and bed loads can be reduced in the river system. Altogether, five sediment management scenarios were selected for testing on the River Lugg, including land use change, contour tillage, hedging and buffer strips. Running the model with parameters altered to simulate these five scenarios produced some interesting results. All scenarios achieved some reduction in sediment levels, with the 40% land use change achieving the best result with a 19% reduction. The other scenarios also achieved significant reductions of between 7% and 9%. Buffer strips produce the best result at close to 9%. The results suggest that if hedge introduction, contour tillage and buffer strips were all applied, sediment reductions would total 24%, considerably improving the current sediment situation. We present a novel cost-effectiveness analysis of our results where we use percentage of land removed from production as our cost function. Given the minimal loss of land associated with contour tillage, hedges and buffer strips, we suggest that these management practices are the most cost-effective combination to reduce sediment loads.

O câncer de pulmão é um problema de saúde pública global que tem uma elevada taxa de letalidade e poderia ser evitado em grande parte, com a redução do tabagismo. Após o diagnóstico desta enfermidade as chances de cura são pequenas e as probabilidades de sobrevida, muito baixas. O propósito desta tese é analisar a tendência de mortalidade por câncer de pulmão e identificar fatores associados à sobrevida, em uma coorte de pacientes com câncer de pulmão, atendida no Hospital do Câncer I do Instituto Nacional de Câncer José Alencar Gomes da Silva (HCI/INCA). Para atingir os objetivos, foram analisadas bases de dados secundários fornecidas pelo Registro Hospitalar de Câncer e pelo Sistema de Informações sobre Mortalidade. Além disso, prontuários médicos também foram consultados. Três abordagens de análise  estudo de tendência temporal, estudo de perfil e análise de sobrevida  foram utilizadas para ilustrar diferentes aspectos do câncer de pulmão. Os resultados obtidos foram organizados prioritariamente no formato de artigos. O primeiro artigo trata da tendência de mortalidade por câncer de pulmão no Brasil entre 1980 e 2007 sob a ótica de uma metodologia que permite estimar os efeitos associados à idade na data do óbito, ao período de ocorrência do óbito e à coorte de nascimento da população estudada. Nesse artigo foram detectados resultados positivos, em relação à redução das taxas de mortalidade entre homens jovens. Entre as mulheres, destaca-se que o efeito coorte estimado indica que este grupo está experimentando atualmente uma tendência semelhante à que os homens experimentaram décadas atrás.O segundo artigo aborda o perfil dos indivíduos com câncer de pulmão, que foram atendidos no HCI/INCA entre 2000 e 2007, por meio da Análise de Correspondência Múltipla. Quatro grupos de pacientes foram identificados: fumantes, não fumantes, portadores de tumores do tipo carcinoma de pequenas células e portadores de tumores em estádios iniciais.

High rates of nutrient loading from agricultural and urban development have resulted in surface water eutrophication and groundwater contamination in regions of Ontario. In Lake Simcoe (Ontario, Canada), anthropogenic nutrient contributions have contributed to increased algal growth, low hypolimnetic oxygen concentrations, and impaired fish reproduction. An ambitious programme has been initiated to reduce phosphorus loads to the lake, aiming to achieve at least a 40% reduction in phosphorus loads by 2045. Achievement of this target necessitates effective remediation strategies, which will rely upon an improved understanding of controls on nutrient export from tributaries of Lake Simcoe as well as improved understanding of the importance of phosphorus cycling within the lake. In this paper, we describe a new model structure for the integrated dynamic and process-based model INCA-P, which allows fully-distributed applications, suited to branched river networks. We demonstrate application of this model to the Black River, a tributary of Lake Simcoe, and use INCA-P to simulate the fluxes of P entering the lake system, apportion phosphorus among different sources in the catchment, and explore future scenarios of land-use change and nutrient management to identify high priority sites for implementation of watershed best management practises.

Background: Antimicrobials are used to directly control bacterial infections in pet (ornamental) fish and are routinely added to the water these fish are shipped in to suppress the growth of potential pathogens during transport. Methodology/Principal Findings: To assess the potential effects of this sustained selection pressure, 127 Aeromonas spp. isolated from warm and cold water ornamental fish species were screened for tolerance to 34 antimicrobials. Representative isolates were also examined for the presence of 54 resistance genes by a combination of miniaturized microarray and conventional PCR. Forty-seven of 94 Aeromonas spp. isolates recovered from tropical ornamental fish and their carriage water were tolerant to >= 15 antibiotics, representing seven or more different classes of antimicrobial. The quinolone and fluoroquinolone resistance gene, qnrS2, was detected at high frequency (37% tested recent isolates were positive by PCR). Class 1 integrons, IncA/C broad host range plasmids and a range of other antibiotic resistance genes, including floR, blaTEM21, tet(A), tet(D), tet(E), qacE2, sul1, and a number of different dihydrofolate reductase and aminoglycoside transferase coding genes were also detected in carriage water samples and bacterial isolates. Conclusions: These data suggest that ornamental fish and their carriage water act as a reservoir for both multi-resistant bacteria and resistance genes.

Steady state and dynamic models have been developed and applied to the River Kennet system. Annual nitrogen exports from the land surface to the river have been estimated based on land use from the 1930s and the 1990s. Long term modelled trends indicate that there has been a large increase in nitrogen transport into the river system driven by increased fertiliser application associated with increased cereal production, increased population and increased livestock levels. The dynamic model INCA Integrated Nitrogen in Catchments. has been applied to simulate the day-to-day transport of N from the terrestrial ecosystem to the riverine environment. This process-based model generates spatial and temporal data and reproduces the observed instream concentrations. Applying the model to current land use and 1930s land use indicates that there has been a major shift in the short term dynamics since the 1930s, with increased river and groundwater concentrations caused by both non-point source pollution from agriculture and point source discharges. �

The catchment of the River Thames, the principal river system in southern England, provides the main water supply for London but is highly vulnerable to changes in climate, land use and population. The river is eutrophic with significant algal blooms with phosphorus assumed to be the primary chemical indicator of ecosystem health. In the Thames Basin, phosphorus is available from point sources such as wastewater treatment plants and from diffuse sources such as agriculture. In order to predict vulnerability to future change, the integrated catchments model for phosphorus (INCA-P) has been applied to the river basin and used to assess the cost-effectiveness of a range of mitigation and adaptation strategies. It is shown that scenarios of future climate and land-use change will exacerbate the water quality problems, but a range of mitigation measures can improve the situation. A cost-effectiveness study has been undertaken to compare the economic benefits of each mitigation measure and to assess the phosphorus reductions achieved. The most effective strategy is to reduce fertilizer use by 20% together with the treatment of effluent to a high standard. Such measures will reduce the instream phosphorus concentrations to close to the EU Water Framework Directive target for the Thames.

Mechanistic catchment-scale phosphorus models appear to perform poorly where diffuse sources dominate. We investigate the reasons for this for one model, INCA-P, testing model output against 18 months of daily data in a small Scottish catchment. We examine key model processes and provide recommendations for model improvement and simplification. Improvements to the particulate phosphorus simulation are especially needed. The model evaluation procedure is then generalised to provide a checklist for identifying why model performance may be poor or unreliable, incorporating calibration, data, structural and conceptual challenges. There needs to be greater recognition that current models struggle to produce positive Nash–Sutcliffe statistics in agricultural catchments when evaluated against daily data. Phosphorus modelling is difficult, but models are not as useless as this might suggest. We found a combination of correlation coefficients, bias, a comparison of distributions and a visual assessment of time series a better means of identifying realistic simulations.

Although there are signs of decline, homicides and traffic-related injuries and deaths in Brazil account for almost two-thirds of all deaths from external causes. In 2007, the homicide rate was 26.8 per 100 000 people and traffic-related mortality was 23.5 per 100 000. Domestic violence might not lead to as many deaths, but its share of violence-related morbidity is large. These are important public health problems that lead to enormous individual and collective costs. Young, black, and poor men are the main victims and perpetrators of community violence, whereas poor black women and children are the main victims of domestic violence. Regional differentials are also substantial. Besides the sociocultural determinants, much of the violence in Brazil has been associated with the misuse of alcohol and illicit drugs, and the wide availability of firearms. The high traffic-related morbidity and mortality in Brazil have been linked to the chosen model for the transport system that has given priority to roads and private-car use without offering adequate infrastructure. The system is often poorly equipped to deal with violations of traffic rules. In response to the major problems of violence and injuries, Brazil has greatly advanced in terms of legislation and action plans. The main challenge is to assess these advances to identify, extend, integrate, and continue the successful ones.

Xenomorellia Malloch, a subgenus of Morellia Robineau-Desvoidy, is revised to include two new species, Morellia (Xenomorellia) inca Nihei and Carvalho sp. nov. from South America, and M. (X.) maia Carvalho and Nihei sp. nov. from Costa Rica and Mexico. Diagnoses for M. (X.) holti (Malloch) and M. (X.) montanhesa (Albuquerque) are provided, as well as an identification key to the four species of the subgenus. A cladistic analysis was performed to test the monophyly of Xenomorellia and to recover the phylogenetic relationships among its species. Tree searches resulted in one single most-parsimonious cladogram, wherein the monophyly of Xenomorellia is supported, as well as a sister-group relationship with the Neotropical subgenus Trichomorellia Stein. Xenomorellia was divided into two clades: one with Caribbean-Andean species (maia + inca), and another with species from southeastern South America (holti + montanhesa).

Exposure of phosphatidylserine (PS) on cellular membranes and membrane-derived microvesicles stimulates a number of anti-inflammatory responses involved in malignant processes. Herein we show that B16F10 cells, a highly metastatic melanoma cell line, produce large quantities of PS-containing microvesicles in vitro. Tumor microvesicles increased TGF-beta(1) production by cultured macrophages and, in vivo, enhanced the metastatic potential of B16F10 cells in C57BL/6 mice, both effects being reversed by annexin V. Most strikingly, microvesicles induced melanoma metastasis in BALB/c mice, which are normally resistant to this tumor cell line. Altogether, this is the first demonstration that tumor-derived microvesicles favor the establishment of melanoma metastasis in a PS-dependent manner, possibly by down-regulating the host`s inflammatory and/or anti-tumoral immune responses. Avanços científicos têm propiciado melhor qualidade de vida aos pacientes e maior expectativa de vida à população. Os hospitais públicos que buscam se manter como referência em sua área de especialização são induzidos a realizar investimentos significativos para oferecer os novos tratamentos, embora a remuneração auferida por meio do Sistema Único de Saúde não costume acompanhar o preço das tecnologias. Portanto, ao gestor de saúde cabe a árdua tarefa de compatibilizar receitas estáveis com custos crescentes. Contrario senso, experiências de mensuração de custos em hospitais públicos ainda são remotas e pouco relatadas em periódicos acadêmicos. Nesse sentido, este artigo consiste no estudo de caso da implantação de sistema de informação de custos no Instituto Nacional de Câncer (INCA). Identificou-se mediante análise documental e entrevistas em profundidade que os fatores determinantes para influenciar a gestão do INCA a implantar, a partir de 2009, um sistema que lhe permita mensurar e analisar custos foram: transparência com o dinheiro público, irregularidade na dotação orçamentária e necessidade de contratualização com o governo central, principalmente devido à ameaça externa de redução drástica nos recursos humanos.

Em virtude da reserva de vagas prevista em legislação, a presença de pessoas com deficiência - PcDs é cada vez mais frequente nas organizações públicas. Percebe-se que nem sempre essas organizações estão preparadas, quer seja arquitetonicamente quer seja por uma cultura de inclusão, para receber e gerir essa força de trabalho. Diante desse contexto, o presente estudo procurou discutir as práticas de gestão das pessoas com deficiência na área pública, a partir do enfoque da inclusão, utilizando o caso do Instituto Nacional de Câncer José Alencar Gomes da Silva - INCA. Para a construção do estudo foi realizada uma pesquisa qualitativa com análise documental e com entrevistas semiestruturadas. No intuito de abarcar a visão dos diferentes grupos de pessoas que estão diretamente envolvidos com a temática no INCA foram entrevistados três tipos de profissionais, a saber: servidores com deficiência, chefias diretamente ligadas a essas PcDs e chefia da área de gestão de pessoas. Constata-se uma preocupação do instituto em cumprir a determinação legal de ocupação das cotas reservadas no quadro de pessoal para as PcDs, porém ainda não foram desenvolvidos ou adaptados instrumentos institucionais de gestão que permitam a inclusão efetiva desses profissionais. Verificou-se também que a adaptação do INCA às dimensões de acessibilidade - arquitetônica, comunicacional, metodológica, instrumental, programática e atitudinal – ainda é incipiente. Formalmente não há nenhum elemento que impeça a participação das PcDs nos subsistemas de gestão de pessoas do INCA, contudo nem sempre são criadas e institucionalizadas as ferramentas necessárias à autonomia e à plena participação desses profissionais.

The occurrence of human cancer in the municipality of Lucrécia (RN, Brazil) has shown high frequencies (INCA, 2007). Tests of micronucleus (MN) and nuclear abnormalities (NA) were performed to assess the genotoxic potential of water from the Lucrécia dam, which is located in a semi-arid region under the influence of crop irrigation and irregular rainfall. Water samples were collected in this source for analyzing the concentration of cyanobacteria, metals and radioactivity. Erythrocytes of Nile tilapia (Oreochromis niloticus) were collected in dam and cells of human oral mucosa in the urban area of this municipality for the bioassays of MN and NA. In fish were also analyzed concentrations of metals in samples of liver and gills. The genotoxicity tests with biological samples of fish and humans have shown significant increases in the frequencies of MN and NA (p ≤ 0.05) and are indicative of increased DNA damage in relation to the control groups. In conclusion, the results obtained from water samples and biological municipality of Lucrécia indicates that the presence of chemical and microbiological pollutants, and increase of genotoxic in human of this municipality. We suggest the implementation of advanced water treatment, to prevent further degradation of the aquatic environment and decrease in the life quality. This research of environmental quality assessment was performed in order to contemplate a multi and interdisciplinary character of this water resource and that can induce genotoxic damage in the organisms in this study region

The chronicles of historical sources are difficult because they are a mix of myths and facts experienced by their authors. Starting from this premise, this study analyzes some of the Spanish and indigenous sources on the conquest of the Inca people and the description of this world and people who were so different. Among the chroniclers chosen, the article highlights the reports by Jose de Acosta on the Mundus Novus and by Cieza de Leon and Garcilaso de la Vega on the myths of the first Inca chief; finally, Francisco de Xerez, Juan de Betanzos, among others, on the event of the imprisonment and murder of Atahualpa, which resulted in the conquest of the "Tahuantinsuyu" by the Spanish.
