As proviral human immunodeficiency virus type 1 (HIV-1) DNA can replenish and revive viral infection upon attivation, its analysis, in addition to RNA viral load, could be considered a useful marker during the follow-up of infected individuals, to evaluate reservoir status, especially in HAART-treated patients when RNA viral load is undetectable by current techniques and the antiretroviral efficacy of new, more potent therapeutic regimens. Standardized methods for the measurement of the two most significant forms of proviral DNA, total and non-integrated, are currently lacking, despite the widespread of molecular biology techniques. In this study, total and 2-LTR HIV-1 DNA proviral load, in addition to RNA viral load, CD4 cell count and serological parameters, were determined by quantitative analysis in peripheral blood mononuclear cells (PBMC) in naïve or subsequently HAART-treated patients with acute HIV-1 infection in order to establish the role of these two DNA proviral forms in the course of HIV infection. The study demonstrated that HAART-treated individuals show a significant decrease in both total and 2-LTR circular HIV-1 DNA proviral load compared with naïve patients: these findings confirm that HIV-1 reservoir decay correlates with therapeutic effectiveness. The persistence of small amounts of 2-LTR HIV-1 DNA form, which is considered to be a molecular determinant of infectivity, in PBMC from some patients demonstrates that a small rate of replication is retained even when HAART is substantially effective: HAART could not eradicate completely the infection because HIV is able to replicate at low levels. Plasma-based viral RNA assays may fail to demonstrate the full extent of viral activity. In conclusion, the availability of a new standardized assay to determine DNA proviral load will be important in assessing the true extent of virological suppression suggesting that its quantification may be an important parameter in monitoring HIV infection.

This thesis was carried out in the context of a co-tutoring program between Centro Ceramico Bologna (Italy) and Instituto di Tecnologia Ceramica, Castellón de la Plana (Spain). The subject of the thesis is the synthesis of silver nanoparticles and at their likely decorative application in the productive process of porcelain ceramic tiles. Silver nanoparticles were chosen as a case study, because metal nanoparticles are thermally stable, and they have non-linear optical properties when nano-structured, and therefore they develop saturated colours. The nanoparticles were synthesized by chemical reduction in aqueous solution, a method chosen because of its reduced working steps and energy costs. Besides such a synthesis method uses non-expensive and non-toxic raw material. By adopting this synthesis technique, it was also possible to control the dimension and the final shape of the nanoparticles. Several syntheses were carried out during the research work, modifying the molecular weight of the reducing agent and/or the firing temperature, in order to evaluate the influence such parameters have on the Ag-nanoparticles formation. The syntheses were monitored with the use of UV-Vis spectroscopy and the average dimension as well as the morphology of the nanoparticles was analysed by SEM. From the spectroscopic data obtained from each synthesis, a kinetic study was completed, relating the progress of the reaction to the two variables (ie temperature and molecular weight of the reducing agent). The aim was finding equations that allow the establishing of a relationship between the operating conditions during the synthesis and the characteristics of the final product. The next step was finding the best method of synthesis for the decorative application. For such a purpose the amount of nanoparticles, their average particle size, the shape and the agglomeration are considered. An aqueous suspension containing the nanoparticles is then sprayed over the fired ceramic tiles and they are subsequently thermally treated in conditions similar to the industrial one. The colorimetric parameters of the obtained ceramic tiles were studied and the method proved successful, giving the ceramic tiles stable and intense colours.

1) Background: The most common methods to evaluate clarithromycin resistance is the E-Test, but is time consuming. Resistance of Hp to clarithromycin is due to point mutations in the 23S rRNA. Eight different point mutations have been related to CH resistance, but the large majority of the clarithromycin resistance depends on three point mutations (A2142C, A2142G and A2143G). A novel PCR-based clarithromycin resistance assays, even on paraffin-embedded biopsy specimens, have been proposed. Aims: to assess clarithromycin resistance detecting these point mutation (E-Test as a reference method);secondly, to investigate relation with MIC values. Methods: Paraffin-embedded biopsies of patients Hp-positive were retrieved. The A2142C, A2142G and A2143G point mutations were detected by molecular analysis after DNA extraction by using a TaqMan real-time PCR. Results: The study enrolled 86 patients: 46 resistant and 40 sensible to CH. The Hp status was evaluated at endoscopy, by rapid urease test (RUT), histology and hp culture. According to real-time PCR, 37 specimens were susceptible to clarithromycin (wild type dna) whilst the remaining 49 specimens (57%) were resistant. A2143G is the most frequent mutation. A2142C always express a resistant phenotype and A2142G leads to a resitant phenotype only if homozigous. 2) Background: Colonoscopy work-load for endoscopy services is increasing due to colorectal cancer prevention. We tested a combination of faecal tests to improve accuracy and prioritize the access to colonoscopy. Methods: we tested a combination of fecal tests (FOBT, M2-PK and calprotectin) in a group of 280 patients requiring colonoscopy. Results: 47 patients had CRC and 85 had advanced adenoma/s at colonoscopy/histology. In case of single test, for CRC detection FOBT was the test with the highest specificity and PPV, M2-PK had the highest sensitivity and higher NPV. Combination was more interesting in term of PPV. And the best combination of tests was i-FOBT + M2-PK.

Il trauma cranico é tra le piú importanti patologie traumatiche. Ogni anno 250 pazienti ogni 100.000 abitanti vengono ricoverati in Italia per un trauma cranico. La mortalitá é di circa 17 casi per 100.000 abitanti per anno. L’Italia si trova in piena “media” Europea considerando l’incidenza media in Europa di 232 casi per 100.000 abitanti ed una mortalitá di 15 casi per 100.000 abitanti. Degli studi hanno indicato come una terapia anticoagulante é uno dei principali fattori di rischio di evolutiviá di una lesione emorragica. Al contrario della terapia anticoagulante, il rischio emorragico correlato ad una terapia antiaggregante é a tutt’oggi ancora in fase di verifica. Il problema risulta rilevante in particolare nella popolazione occidentale in quanto l’impiego degli antiaggreganti é progressivamente sempre piú diffuso. Questo per la politica di prevenzione sostenuta dalle linee guida nazionali e internazionali in termini di prevenzione del rischio cardiovascolare, in particolare nelle fasce di popolazione di etá piú avanzata. Per la prima volta, é stato dimostrato all’ospedale di Forlí[1], su una casistica sufficientemente ampia, che la terapia cronica con antiaggreganti, per la preven- zione del rischio cardiovascolare, puó rivelarsi un significativo fattore di rischio di complicanze emorragiche in un soggetto con trauma cranico, anche di grado lieve. L’ospedale per approfondire e convalidare i risultati della ricerca ha condotto, nell’anno 2009, una nuova indagine. La nuova indagine ha coinvolto oltre l’ospedale di Forlí altri trentuno centri ospedalieri italiani. Questo lavoro di ricerca vuole, insieme ai ricercatori dell’ospedale di Forlí, verificare: “se una terapia con antiaggreganti influenzi l’evolutivitá, in senso peggiorativo, di una lesione emorragica conseguente a trauma cranico lieve - moderato - severo in un soggetto adulto”, grazie ai dati raccolti dai centri ospedalieri nel 2009. Il documento é strutturato in due parti. La prima parte piú teorica, vuole fissare i concetti chiave riguardanti il contesto della ricerca e la metodologia usata per analizzare i dati. Mentre, la seconda parte piú pratica, vuole illustrare il lavoro fatto per rispondere al quesito della ricerca. La prima parte é composta da due capitoli, che sono: • Il capitolo 1: dove sono descritti i seguenti concetti: cos’é un trauma cra- nico, cos’é un farmaco di tipo anticoagulante e cos’é un farmaco di tipo antiaggregante; • Il capitolo 2: dove é descritto cos’é il Data Mining e quali tecniche sono state usate per analizzare i dati. La seconda parte é composta da quattro capitoli, che sono: • Il capitolo 3: dove sono state descritte: la struttura dei dati raccolti dai trentadue centri ospedalieri, la fase di pre-processing e trasformazione dei dati. Inoltre in questo capitolo sono descritti anche gli strumenti utilizzati per analizzare i dati; • Il capitolo 4: dove é stato descritto come é stata eseguita l’analisi esplorativa dei dati. • Il capitolo 5: dove sono descritte le analisi svolte sui dati e soprattutto i risultati che le analisi, grazie alle tecniche di Data Mining, hanno prodotto per rispondere al quesito della ricerca; • Il capitolo 6: dove sono descritte le conclusioni della ricerca. Per una maggiore comprensione del lavoro sono state aggiunte due appendici. La prima tratta del software per data mining Weka, utilizzato per effettuare le analisi. Mentre, la seconda tratta dell’implementazione dei metodi per la creazione degli alberi decisionali.

The present work concerns with the study of debris flows and, in particular, with the related hazard in the Alpine Environment. During the last years several methodologies have been developed to evaluate hazard associated to such a complex phenomenon, whose velocity, impacting force and inappropriate temporal prediction are responsible of the related high hazard level. This research focuses its attention on the depositional phase of debris flows through the application of a numerical model (DFlowz), and on hazard evaluation related to watersheds morphometric, morphological and geological characterization. The main aims are to test the validity of DFlowz simulations and assess sources of errors in order to understand how the empirical uncertainties influence the predictions; on the other side the research concerns with the possibility of performing hazard analysis starting from the identification of susceptible debris flow catchments and definition of their activity level. 25 well documented debris flow events have been back analyzed with the model DFlowz (Berti and Simoni, 2007): derived form the implementation of the empirical relations between event volume and planimetric and cross section inundated areas, the code allows to delineate areas affected by an event by taking into account information about volume, preferential flow path and digital elevation model (DEM) of fan area. The analysis uses an objective methodology for evaluating the accuracy of the prediction and involve the calibration of the model based on factors describing the uncertainty associated to the semi empirical relationships. The general assumptions on which the model is based have been verified although the predictive capabilities are influenced by the uncertainties of the empirical scaling relationships, which have to be necessarily taken into account and depend mostly on errors concerning deposited volume estimation. In addition, in order to test prediction capabilities of physical-based models, some events have been simulated through the use of RAMMS (RApid Mass MovementS). The model, which has been developed by the Swiss Federal Institute for Forest, Snow and Landscape Research (WSL) in Birmensdorf and the Swiss Federal Institute for Snow and Avalanche Research (SLF) takes into account a one-phase approach based on Voellmy rheology (Voellmy, 1955; Salm et al., 1990). The input file combines the total volume of the debris flow located in a release area with a mean depth. The model predicts the affected area, the maximum depth and the flow velocity in each cell of the input DTM. Relatively to hazard analysis related to watersheds characterization, the database collected by the Alto Adige Province represents an opportunity to examine debris-flow sediment dynamics at the regional scale and analyze lithologic controls. With the aim of advancing current understandings about debris flow, this study focuses on 82 events in order to characterize the topographic conditions associated with their initiation , transportation and deposition, seasonal patterns of occurrence and examine the role played by bedrock geology on sediment transfer.

This research activity studied how the uncertainties are concerned and interrelated through the multi-model approach, since it seems to be the bigger challenge of ocean and weather forecasting. Moreover, we tried to reduce model error throughout the superensemble approach. In order to provide this aim, we created different dataset and by means of proper algorithms we obtained the superensamble estimate. We studied the sensitivity of this algorithm in function of its characteristics parameters. Clearly, it is not possible to evaluate a reasonable estimation of the error neglecting the importance of the grid size of ocean model, for the large amount of all the sub grid-phenomena embedded in space discretizations that can be only roughly parametrized instead of an explicit evaluation. For this reason we also developed a high resolution model, in order to calculate for the first time the impact of grid resolution on model error.
