Objectives: Our aim was to study the effect of combination therapy with aspirin and dipyridamole (A+D) over aspirin alone (ASA) in secondary prevention after transient ischemic attack or minor stroke of presumed arterial origin and to perform subgroup analyses to identify patients that might benefit most from secondary prevention with A+D. Data sources: The previously published meta-analysis of individual patient data was updated with data from ESPRIT (N=2,739); trials without data on the comparison of A+D versus ASA were excluded. Review methods: A meta-analysis was performed using Cox regression, including several subgroup analyses and following baseline risk stratification. Results: A total of 7,612 patients (5 trials) were included in the analyses, 3,800 allocated to A+D and 3,812 to ASA alone. The trial-adjusted hazard ratio for the composite event of vascular death, non-fatal myocardial infarction and non-fatal stroke was 0.82 (95% confidence interval 0.72-0.92). Hazard ratios did not differ in subgroup analyses based on age, sex, qualifying event, hypertension, diabetes, previous stroke, ischemic heart disease, aspirin dose, type of vessel disease and dipyridamole formulation, nor across baseline risk strata as assessed with two different risk scores. A+D were also more effective than ASA alone in preventing recurrent stroke, HR 0.78 (95% CI 0.68 – 0.90). Conclusion: The combination of aspirin and dipyridamole is more effective than aspirin alone in patients with TIA or ischemic stroke of presumed arterial origin in the secondary prevention of stroke and other vascular events. Research suggests that those suspected of sexual offending might be more willing to reveal information about their crimes if interviewers display empathic behaviour. However, the literature concerning investigative empathy is in its infancy, and so as yet is not well understood. This study explores empathy in a sample of real-life interviews conducted by police officers in England with suspected sex offenders. Using qualitative methodology, the presence and type of empathic verbal behaviours displayed was examined. Resulting categories were quantitatively analysed to investigate their occurrence overall, and across interviewer gender. We identified four distinct types of empathy, some of which were used significantly more often than others. Female interviewers displayed more empathic behaviour per se by a considerable margin.

While there is extensive research regarding the way users in social networking sites (SNSs) connect and communicate with each other, literature on consumer-brand relationships in SNSs is scarce. This paper hypothesizes and tests the impact of varying the source of communication in Facebook brand pages on key characteristics of brand equity, examining whether this impact is conditioned by relationship closeness expectations. More specifically, two experiments assess how relationship closeness expectations vary according to brand category and brand affiliation and how the use of a spokes-character as the source of communication in brand pages versus communicating institutionally affects consumer’s attitudes towards two real-world brands. To measure these variables, structured questionnaires were conducted with three groups of undergraduate students. The results suggest that the appropriateness of opting for a more “informal” source of communication in brand pages such as a spokes-character varies depending on whether this is in(congruent) with existing relationship closeness expectations. Implications for researchers, brand and social media managers are presented.

The article presents a rationale for communicative, conceptual, cognitive and procedural challenges experienced by litigants in person in financial remedy proceedings. The article also explores oscillation between written and spoken legal genres and narrative development strategies which litigants in person have to use throughout different stages (from the early stages of starting proceedings, filling in court forms and providing documentation, through the negotiation process to interaction in court). While legal professionals express themselves in paradigmatic legal mode influenced by legal acts and legislation, litigants in person tend to express themselves in narrative mode similar to everyday storytelling. The objective is to investigate obstacles litigants in person experience during the process originally designed by legal professionals for legal professionals. The article evaluates different options for empowering lay people involved in legal proceedings and argues for the need to provide more specific support for different stages of family proceedings.

Background: Although numerous studies and metanalysis have shown the beneficial effect of statin therapy in CVD secondary prevention, there is still controversy such the use of statins for primary CVD prevention in patients with DM. The purpose of this study was to evaluate the occurrence of total major adverse cardio-vascular events (MACE) in a cohort of patients with type 2 diabetes complicated by nephropathy treated with statins, in order to verify real life effect of statin on CVD primary prevention. Methods: We conducted an observational prospective multicenter study on 564 patients with type 2 diabetic nephropathy free of cardiovascular disease attending 21 national outpatient diabetes clinics and followed them up for 8 years. 169 of them were treated with statins (group A) while 395 were not on statins (group B). Results: Notably, none of the patients was treated with a high-intensity statin therapy according to last ADA position statement. Total MACE occurred in 32 patients from group A and in 68 patients from group B. Fatal MACE occurred in 13 patients from group A and in 30 from group B; nonfatal MACE occurred in 19 patients from group A and in 38 patients from group B. The analysis of the Kaplan-Meier survival curves showed a not statistically significant difference in the incidence of total (p 0.758), fatal (p 0.474) and nonfatal (p 0.812) MACE between the two groups. HbA1c only showed a significant difference in the incidence of MACE between the two groups (HR 1.201, CI 1.041-1.387, p 0.012). Conclusions: These findings suggest that, in a real clinical setting, moderate-intensity statin treatment is ineffective in cardiovascular primary prevention for patients with diabetic nephropathy.

Abstract Background: Communication is a basic tool in nursing, a crucial element of care. The quality of the interactions that take place between the nurse and the user/family influence their satisfaction and security felt with the care received. Objectives: To identify the communication skills and interpersonal relationship of nursing students in health care; identify the sociodemographic and academic variables influencing communication skills and interpersonal relationship of nursing students in health care. Methodology: Quantitative study, cross-sectional, descriptive and correlational. The data collection instrument was a questionnaire with questions concerning the socio-demographic and academic characterization; basic skills of interview and clinical communication in health care; learning of clinical communication skills and range of communication skills and interpersonal relationship. The sample consisted of 374 nursing students from two Portuguese schools. Results: The majority were female (80.5%), in the age group of 18-21 years. The students recognize the importance of clinical communication skills and interpersonal relations in nursing practice (82.4%); agreed on the teaching methods of communicational skills (54.3%). Evaluated their training in the area as good (71.7%). Age, semester and school influenced communication skills and interpersonal relationship of students (p <0.5) Conclusion: The results obtained allow us to state that the education / training of nursing student in the relational context is of fundamental importance in building capacity for competent professional practice.

Reflections on communication in the learning process at the level of the seventh year of secondary education based on the methodological and pedagogical approaches, considered in research of a multidisciplinary team of the CIDE of UNA (National University), with the aim of building the “teaching performance profile for the seventh year of Costa Rican education”. It demonstrates the role of teachers to ensure favorable communication links to the learning process, not only to capture the attention and interest of students through varied and systematic motivations, but also by the honesty, openness, personal example and involvement in the task, problems and aspirations of students.

In this paper, I analyzed, according to Discourse Analysis approach, the narratives of drug-addicted people which were found on a website of the drug rehabilitation and treatment clinic www.ctiviva.com.br. I could find common characteristics among the reported narratives that integrate the group’s opinion into the same ideological position. I could also find a collective subject, considering the narratives made by the drug-addicted people who were under treatment into the reported clinic whose site was mentioned before. That site displays the 10 narratives that composed this corpus research. That analysis focused the construction of the process that indicates a social position of group, considering three categories: key-expressions; key-ideas and anchorage, according to Lefèvre (2005). The first one indicates excerpts of testimonials that facilitates the key-words` identification; the second one, the key-words, indicates the meanings of each testimonial, and the last one, the anchorage, indicates the ideology of the discourse. Managing such categories enabled the identification of all the narratives as situation category, as proposed by Bertaux (2010). The results indicated two criteria that establish relationships of power: family and the good treatment. The construction of a collective subject evidenced the ideologies of the clinic and of the treatments that were given to the patients.

The analysis of opiates is of vital interest in drug abuse monitoring and research. This review presents a general overview of the electrochemical methods used for detection and quantification of opiates in a variety of matrices. Emphasis has been placed on the voltammetric methods used for study and determination of morphine, codeine, and heroin. Specific issues that need to be solved and better explained as well as future trends in the use of electrochemical methods in the examination of opiates are also discussed.

OBJETIVO: Este trabalho apresenta resultados acerca das propriedades psicométricas da Escala de atitudes frente ao HIV/AIDS. Os dados, provenientes de uma amostra de 549 alunos entre universitários, ensinos médio e ensino fundamental. MÉTODOS: Os dados foram tratados pelo método dos componentes principais da análise fatorial. A análise final, postulado um eigenvalue mínimo de 2, resultou cinco fatores. Foram eliminados itens que apresentaram carga fatorial menor que 0,30. Neste estudo, o menor alfa observado foi de 0,79. Portanto, é provável que todos os 47 itens do instrumento final elaborado meçam o mesmo construto: atitude frente ao HIV/AIDS. RESULTADOS: Escores inferiores a 96 foram considerados fraco grau de conhecimento sobre HIV/AIDS; entre 96 e 192 moderado grau de conhecimento e acima de 192 alto grau de conhecimento sobre HIV/AIDS. Foram estabelecidos os fatores: 1, 2 e 3, sendo fator geral de percepção da informação técnico-científica; fator de percepção da informação técnico-científica versus sexualidade e preconceito; fator de percepção da informação técnico-científica no uso de drogas, respectivamente. CONCLUSÕES: O alfa de Cronbach encontrado para a escala como um todo foi de 0,859, sugerindo fortemente a existência da fidedignidade do instrumento que se mostrou útil para avaliar o grau de conhecimento acerca do HIV/AIDS e o risco decorrente do desconhecimento, entre estudantes.

Introdução: com o aumento da toxicodependência, que engloba um problema mundial, também cresce o número de doentes com suas devidas consequências, que são patologias sistémicas, englobando a saúde oral, que é uma das partes do corpo que mais sofre com a toxicodependência. Objetivos: Esta tese tem como objetivo a revisão da literatura sobre “toxicodependência” e “medicina dentária”, correlacionando estes temas com patologias e complicações na cavidade oral, nomeadamente, bruxismo, desgaste dental, xerostomia, halitose, cárie rampante e doença periodontal. Metodologia: Foi realizada uma revisão bibliográfica com base em artigos publicados em revistas e teses disponíveis em bibliotecas online, nos últimos dez anos. As palavraschaves usadas foram, assim como a conjugação entre elas e seus devidos idiomas: toxicodependência, desintoxicação, abstinência, cannabis, álcool, metanfetamina, heroína, opiáceos, tabaco, nicotina, metadona, erosão dentária, xerostomia, cáries, bruxismo, entre outras. Conclusão: Preconiza-se que haja uma maior atenção e conhecimento por parte do médico dentista sobre esta condição, bem como das complicações associadas, uma vez que é um dos principais profissionais de saúde a ter o contacto e a oportunidade de tratar os pacientes sintomáticos, como também os que estão em fase de recuperação e reinserção social.
