To discuss and share knowledge around advances in the care of patients with thrombotic disorders, the Third International Symposium of Thrombosis and Anticoagulation was held in So Paulo, Brazil, from October 14-16, 2010. This scientific program was developed by clinicians for clinicians, and was promoted by four major clinical research institutes: the Brazilian Clinical Research Institute, the Duke Clinical Research Institute of the Duke University School of Medicine, the Canadian VIGOUR Centre, and the Uppsala Clinical Research Center. Comprising 3 days of academic presentations and open discussion, the symposium had as its primary goal to educate, motivate, and inspire internists, cardiologists, hematologists, and other physicians by convening national and international visionaries, thought-leaders, and dedicated clinician-scientists. This paper summarizes the symposium proceedings.

Patients with diabetes mellitus (DM) have high platelet reactivity and are at increased risk of ischaemic events and bleeding post-acute coronary syndromes (ACS). In the PLATelet inhibition and patient Outcomes (PLATO) trial, ticagrelor reduced the primary composite endpoint of cardiovascular death, myocardial infarction, or stroke, but with similar rates of major bleeding compared with clopidogrel. We aimed to investigate the outcome with ticagrelor vs. clopidogrel in patients with DM or poor glycaemic control. We analysed patients with pre-existing DM (n = 4662), including 1036 patients on insulin, those without DM (n = 13 951), and subgroups based on admission levels of haemoglobin A1c (HbA1c; n = 15 150). In patients with DM, the reduction in the primary composite endpoint (HR: 0.88, 95% CI: 0.76-1.03), all-cause mortality (HR: 0.82, 95% CI: 0.66-1.01), and stent thrombosis (HR: 0.65, 95% CI: 0.36-1.17) with no increase in major bleeding (HR: 0.95, 95% CI: 0.81-1.12) with ticagrelor was consistent with the overall cohort and without significant diabetes status-by-treatment interactions. There was no heterogeneity between patients with or without ongoing insulin treatment. Ticagrelor reduced the primary endpoint, all-cause mortality, and stent thrombosis in patients with HbA1c above the median (HR: 0.80, 95% CI: 0.70-0.91; HR: 0.78, 95% CI: 0.65-0.93; and HR: 0.62, 95% CI: 0.39-1.00, respectively) with similar bleeding rates (HR: 0.98, 95% CI: 0.86-1.12). Ticagrelor, when compared with clopidogrel, reduced ischaemic events in ACS patients irrespective of diabetic status and glycaemic control, without an increase in major bleeding events.

Ventilation distribution can be assessed by SPECT with Technegas. This study was undertaken in piglets with different degrees of ventilation inhomogeneity to compare PET using (68)Ga-labeled pseudogas or ""Gallgas"" with Technegas. Methods: Twelve piglets were studied in 3 groups: control, lobar obstruction, and diffuse airway obstruction. Two more piglets were assessed for lung volume (functional residual capacity). Results: In controls, SPECT and PET images showed an even distribution of radioactivity. With lobar obstruction, the absence of ventilation of the obstructed lobe was visible with both techniques. In diffuse airway obstruction, SPECT images showed an even distribution of radioactivity, and PET images showed more varied radioactivity over the lung. Conclusion: PET provides detailed ventilation distribution images and a better appreciation of ventilation heterogeneity. Gallgas with PET is a promising new diagnostic tool for the assessment of ventilation distribution.

O artigo tem por objetivo investigar a internacionalização dos investimentos portugueses no Brasil de meados dos anos 1990 até o começo da década de 2000. Foram registradas cerca de 400 empresas lusitanas no país em 2003. Previamente, colocam-se várias questões empíricas e teóricas, e destaca-se a relevância do caso à luz dos grandes modelos de referência em matéria de internacionalização de empresas, em particular a escola nórdica (ou modelo de Uppsala) e o paradigma OLI de John Dunning. Enfatiza-se em seguida a contribuição dos determinantes culturais, globalmente considerados, desde o início do processo de investimento luso-brasileiro. Nesse contexto, o papel da língua comum é enfatizado. De passagem, analisam-se diversos tópicos, inclusive o da influência por vezes paradoxal da proximidade cultural sobre o investimento internacional. Por outro lado, os resultados a que foi possível chegar ajudam a preencher as lacunas deixadas pelas insuficiências teóricas ligadas à negligência do movimento internacional de fatores.

Vegetation attributes were consistent with the successional stage of each dome in the primary sere; however, the geomorphologic units did not follow the same pattern. The influence of the rates of plant colonization and soil formation are responsible for the decrease of the successional rates from footslopes, to summits, to slopes. The vegetation successionally changes from Juniperus scrub, to Juniperus wood and forest, and there is little species replacement since the similarity in species composition is high between the 3 domes.

A globalização abriu um vasto leque de oportunidades para as empresas/negócios em todo o mundo. Usando a metodologia qualitativa e o estudo de caso, procuramos analisar o processo de internacionalização de três PME portuguesas com o objetivo de perceber a sua abordagem aos mercados internacionais. Os resultados indicam que o modelo Uppsala é útil para compreender o processo de internacionalização das PME. Contudo, também indicam que há empresas que iniciam a sua expansão internacional muito rapidamente e para mercados geograficamente dispersos, contrariando alguns princípios do modelo Uppsala. Para os casos em análise, a exportação constitui o principal modo de entrada nos mercados externos. Adicionalmente, as razões/fatores que motivam e influenciam a internacionalização, em conjunto com o perfil dos gestores, o modelo de negócio, bem como as networks em que a empresa poderá estar inserida, podem ser os principais impulsionadores da decisão e do reforço do processo de internacionalização.

Although literature is lacking in the topic of internationalization of services, we manage to apply both the Uppsala model and the Eclectic Theory to the healthcare service. A cross-case study analysis with three international hospitals is done in order to define an internationalization pattern and conditions for a successful process. This is then applied to Associação Protectora dos Diabéticos de Portugal with the purpose of defining an internationalization strategy to the Association.

This Work Project was focused, firstly, in an internal analysis to assess the possible competitive advantage sources of the firm and to identify the key weakness that the firm should improve before advancing with the internationalization strategy. Completed the analysis was possible to verify that the source of sustainable competitive advantage of Relance is the use of specific raw materials and that its biggest weaknesses are the lack of capacity and the excessive dependence on key employees. Overall, it was possible to conclude that the firm is in position of competing in the international markets. Secondly, due to its crucial importance, the country selection was made with a deep objective analysis, resulting with the selection of Brazil as the country with the highest market potential. Regarding the strategy to actually enter in Brazil’s market, after the study of 12 modes of entry it was possible to conclude that there is no ideal mode. However, the study performed allowed to undertake the conclusion that in order to obtain long-term optimization on the process, the adaptation, to the firm´s situation, of the Uppsala model progressive stages is the ideal. At the course of this Work Project it was inferred that the critical success factors are the choice of the distributor and the promotion of Relance as a premium brand.
