Objective: The etiology of hypoactive sexual desire disorder (HSDD) is known to be multifactorial, involving biological, psychosexual, and context-related factors. The objective of the present study was to analyze the factors associated with female HSDD and to stratify these factors according to their importance. Methods: This was a population-based, hierarchical study conducted in Brazil, based on data from previous research on the Brazilian Sexual Life Study, conducted between November 2002 and February 2003 in various Brazilian cities. The primary study consisted of a self-administered and anonymous questionnaire, addressing sociodemographic parameters, general health, life habits, behavior, and complaints related to sexual function. The association between HSDD and various other factors was assessed. The data were evaluated by hierarchical multiple regression analysis. Results: The prevalence of HSDD in this sample was 9.5%. Associations were found with cardiovascular disease, breast cancer, posttraumatic stress, poorer education level, being older, being married, a lack of information on sexuality in childhood/adolescence, and a limited sexual repertoire. Women who consumed moderate amounts of alcohol were found to be less likely to have HSDD. Conclusions: Analysis of the associated factors classified in order of importance and analysis of the characteristics of the sexual relationships provide additional information to currently available data on the traditional concepts of HSDD.

Objective The sexual function and enjoyment questionnaire (SFEQ) was developed to assess and detect changes in sexual function in men and women. The aim of the present study was to use the SFEQ to evaluate the effects of the serotonin-noradrenaline reuptake inhibitor, milnacipran, in the treatment of depression in two culturally different populations. Methods The SFEQ was employed in two studies investigating milnacipran in the treatment of major depression: a 12-week open study in Brazil and a 6-week randomised controlled study in Europe. Results At endpoint, 61.3% (Brazil) and 78.4% (Europe) of patients had >= 50% reduction of baseline Hamilton Depression Rating (HAMD) score. All SFEQ items showed improvements in sexual function in both Studies at endpoint, 60% (Brazil) and 56% (Europe) of patients stating that their Sexual desire was as great as or greater than it had been before their depressive episode. Conclusions Milnacipran appears to improve sexual function in parallel with improvement in other symptoms of depression. The SFEQ is a sensitive instrument for measuring changes in sexual function and appears to be unaffected by Cultural differences as shown by similar findings in Brazil and Europe. Vagina agenesis is a rare entity. Mayer-Rokitansky-Kuster-Hauser syndrome is the most significant cause of vagina agenesis, whereas the second most common cause is complete androgen syndrome. Surgical treatment can propitiate a vaginal reconstruction, but sexual function depends on several factors that affect sexual performance. Many reports focus on the intraoperative and postoperative results and only describe the global approach to these patients, but reports focusing on the management of these patients to enable them to have a normal sexual life are lacking. This case report highlights a multidisciplinary treatment for this kind of morbidity and emphasizes the necessity of incorporating careful attention to sexual health in the treatment of these patients so that they may achieve a good therapeutic response, resulting in a pleasurable sexual life and a good quality of living.

Chronic disease and its treatments can have a negative impact on sexual functioning and sexual satisfaction. Aim of study - To explore and to compare sexual function and sexual satisfaction in people with stable chronic diseases.

One facet of self-transcendence is creative self-forgetfulness (CSF; tendency to be attentionally absorbed in mindaltering experiences). Proneness to mind-altering attentional absorption and other aspects of self-transcendence were previously related to vaginal intercourse frequency, sexual arousability, and female coital orgasm. Given that sexual responsiveness is enhanced by focused attention, itwas testedwhether CSF correlates with sexual responsiveness, and if maladaptive defenses, openness to experience, and testosterone explain the hypothesized relationships. One hundred thirty-nine Portuguese (98 women) provided saliva samples for testosterone determination by luminescence immunoassays before and after a romantic movie scene and reported how intensely they felt sexual desire and arousal during the movie. CSF was measured by the Temperament and Character Inventory– Revised, maladaptive defenses by the immature defenses subscale of the Defense Style Questionnaire (DSQ-40), male and female past month desire by the desire dimensions of the International Index of Erectile Function (IIEF), and Female Sexual Function Index (FSFI). Female desire and arousal during themoviewere independently predicted by CSF, openness to experience and testosterone, but not by immature defenses. Female past month desire was independently predicted by CSF, testosterone, and less immature defenses. Possible psychobiological processes linking self-transcendence and sexual responsiveness are discussed.

Specialized setae placed on proximal segments of appendages in tarantulas have been related to sound production (stridulation), used in defense or sexual communication. The surface structure of called stridulatory setae of Acanthoscurria suina Pocock, 1903 was studied by SEM. Three morphological types of setae were recognized and at least two of them could be involved in stridulation. Their role in sexual communication was tested by experimental removal. Our results showed no differences in the sexual success between the setaeless and control individuals. Consequently, a defensive function for stridulatory setae seems to be more likely than a sexual function.

PURPOSE OF REVIEW: Recent findings in the physiology and neurobiology of ejaculation have expanded our understanding of male sexual function and have allowed the development of new instruments to investigate ejaculatory and orgasmic disorders. RECENT FINDINGS: The evidence-based definition of lifelong premature ejaculation has set a model in the evaluation and treatment outcome of sexual dysfunction. New instruments to objectively assess arousal, orgasm and the expulsion phase of ejaculation such as functional MRI, dynamic pelvic ultrasound, PET scans and validated questionnaires have lead to a better understanding of sexual dysfunction in men. Animal models, developments in neurobiology and clinical experience have transformed a purely psychoanalytical approach to ejaculatory and orgasmic function into a novel multidisciplinary, scientifically sound and evidence-based discipline of medicine. SUMMARY: Ejaculation is an integral part of normal sexual function. Ejaculatory dysfunction is common and may cause substantial disruption to the quality of a patient's life. A better understanding of the epidemiology, pathophysiology, neuroscience and genetics of ejaculatory and orgasmic function will eventually lead to the development of new, effective methods of treatment of disorders of ejaculation and orgasm in men.

Female sexual dysfunctions, including desire, arousal, orgasm and pain problems, have been shown to be highly prevalent among women around the world. The etiology of these dysfunctions is unclear but associations with health, age, psychological problems, and relationship factors have been identified. Genetic effects explain individual variation in orgasm function to some extent but until now quantitative behavior genetic analyses have not been applied to other sexual functions. In addition, behavior genetics can be applied to exploring the cause of any observed comorbidity between the dysfunctions. Discovering more about the etiology of the dysfunctions may further improve the classification systems which are currently under intense debate. The aims of the present thesis were to evaluate the psychometric properties of a Finnish-language version of a commonly used questionnaire for measuring female sexual function, the Female Sexual Function Index (FSFI), in order to investigate prevalence, comorbidity, and classification, and to explore the balance of genetic and environmental factors in the etiology as well as the associations of a number of biopsychosocial factors with female sexual functions. Female sexual functions were studied through survey methods in a population based sample of Finnish twins and their female siblings. There were two waves of data collection. The first data collection targeted 5,000 female twins aged 33–43 years and the second 7,680 female twins aged 18–33 and their over 18–year-old female siblings (n = 3,983). There was no overlap between the data collections. The combined overall response rate for both data collections was 53% (n = 8,868), with a better response rate in the second (57%) compared to the first (45%). In order to measure female sexual function, the FSFI was used. It includes 19 items which measure female sexual function during the previous four weeks in six subdomains; desire, subjective arousal, lubrication, orgasm, sexual satisfaction, and pain. In line with earlier research in clinical populations, a six factor solution of the Finnish-language version of the FSFI received supported. The internal consistencies of the scales were good to excellent. Some questions about how to avoid overestimating the prevalence of extreme dysfunctions due to women being allocated the score of zero if they had had no sexual activity during the preceding four weeks were raised. The prevalence of female sexual dysfunctions per se ranged from 11% for lubrication dysfunction to 55% for desire dysfunction. The prevalence rates for sexual dysfunction with concomitant sexual distress, in other words, sexual disorders were notably lower ranging from 7% for lubrication disorder to 23% for desire disorder. The comorbidity between the dysfunctions was substantial most notably between arousal and lubrication dysfunction even if these two dysfunctions showed distinct patterns of associations with the other dysfunctions. Genetic influences on individual variation in the six subdomains of FSFI were modest but significant ranging from 3–11% for additive genetic effects and 5–18% for nonadditive genetic effects. The rest of the variation in sexual functions was explained by nonshared environmental influences. A correlated factor model, including additive and nonadditive genetic effects and nonshared environmental effects had the best fit. All in all, every correlation between the genetic factors was significant except between lubrication and pain. All correlations between the nonshared environment factors were significant showing that there is a substantial overlap in genetic and nonshared environmental influences between the dysfunctions. In general, psychological problems, poor satisfaction with the relationship, sexual distress, and poor partner compatibility were associated with more sexual dysfunctions. Age was confounded with relationship length but had over and above relationship length a negative effect on desire and sexual satisfaction and a positive effect on orgasm and pain functions. Alcohol consumption in general was associated with better desire, arousal, lubrication, and orgasm function. Women pregnant with their first child had fewer pain problems than nulliparous nonpregnant women. Multiparous pregnant women had more orgasm problems compared to multiparous nonpregnant women. Having children was associated with less orgasm and pain problems. The conclusions were that desire, subjective arousal, lubrication, orgasm, sexual satisfaction, and pain are separate entities that have distinct associations with a number of different biopsychosocial factors. However, there is also considerable comorbidity between the dysfunctions which are explained by overlap in additive genetic, nonadditive genetic and nonshared environmental influences. Sexual dysfunctions are highly prevalent and are not always associated with sexual distress and this relationship might be moderated by a good relationship and compatibility with partner. Regarding classification, the results supports separate diagnoses for subjective arousal and genital arousal as well as the inclusion of pain under sexual dysfunctions.

OBJETIVO: traduzir e validar o Female Sexual Function Index (FSFI) para grávidas brasileiras. MÉTODOS: participaram da pesquisa 92 gestantes assistidas em ambulatório de pré-natal de baixo risco, com diagnóstico da gravidez confirmado por ultra-sonografia precoce. Inicialmente, traduzimos o questionário FSFI para a língua portuguesa (do Brasil), de acordo com os critérios internacionais. Foram realizadas adaptações culturais, conceituais e semânticas do FSFI, em função das diferenças da língua, para que as gestantes compreendessem as questões. Todas as pacientes responderam duas vezes ao FSFI, no mesmo dia, com dois entrevistadores diferentes, com intervalo de uma hora de uma entrevista para a outra. Em seguida, 7 a 14 dias depois, o questionário foi novamente aplicado numa segunda entrevista. Foram avaliadas a confiabilidade (consistência interna intra e interobservador) e a validade do construto (para demonstrar que o questionário avalia a função sexual). RESULTADOS: adaptações culturais foram necessárias para obtermos a versão final. A consistência interna intra-observador (alfa de Chronbach) dos diversos domínios oscilou de moderada a forte (0,791 a 0,911) e a consistência interobservador variou de 0,791 a 0,914. Na validação do construto, foram obtidas correlações de moderada a forte entre os escores finais (gerais) do FSFI e do Quociente Sexual Feminino (QS-F), que tem a capacidade de avaliar a função sexual feminina. CONCLUSÕES: o FSFI foi adaptado à língua portuguesa e à cultura brasileira, apresentando significante confiabilidade e validade, podendo ser incluído e utilizado em futuros estudos da função sexual de grávidas brasileiras.

OBJETIVO: Avaliar a influência dos sintomas climatéricos na função sexual de mulheres de meia-idade. MÉTODOS: Estudo populacional de corte transversal, com amostra de 370 mulheres entre 40 e 65 anos, atendidas nas Unidades Básicas de Saúde da cidade de Natal, no estado do Rio Grande do Norte, Brasil. Aplicou-se um questionário referente s características sociodemográficas, clínicas e comportamentais das mulheres. A função sexual foi avaliada pelo Female Sexual Function Index (FSFI), enquanto os sintomas do climatério pelo Menopause Rating Scale (MRS). RESULTADOS: No grupo estudado, 67% das mulheres apresentaram risco de disfunção sexual (FSFI≤26,5). Todos os domínios do FSFI (desejo, excitação, lubrificação, orgasmo, satisfação e dor) apresentaram escores mais baixos nas mulheres com risco de disfunção sexual (p<0,001). Os domínos excitação, orgasmo e dor foram os que mais contribuíram para os baixos escores do FSFI. Os sintomas somatovegetativos, urogenitais e psicológicos do MRS apresentaram-se mais elevados nas mulheres com risco de disfunção sexual, sendo significativos para todas as comparações (p<0,001). A análise de regressão logística revelou que as chances de mulheres com riscos de disfunção sexual apresentarem fogachos, humor depressivo, problemas sexuais e ressecamento vaginal foram, respectivamente, 2,1 (IC95% 1,2 - 3,5); 2,4 (IC95% 1,5 - 4,1); 2,3 (IC95% 1,4 - 3,8) e 2,2 (IC95% 1,3 - 3,6) vezes maior, quando comparadas quelas sem risco. CONCLUSÃO: Os sintomas climatéricos parecem influenciar a função sexual de mulheres na meia-idade.

OBJETIVO: Avaliar a função sexual de mulheres após a menopausa com diagnóstico de síndrome metabólica. MÉTODOS: Estudo caso-controle, incluindo 195 mulheres após o período da menopausa (amenorreia ≥1 ano, FSH ≥30 mUI/mL e idade entre 43 a 69 anos), atendidas no Departamento de Obstetrícia e Ginecologia da Faculdade de Ciências Médicas da Santa Casa de São Paulo e nas Unidades Básicas de Saúde do Programa de Saúde da Família da cidade de São Paulo. Foram coletados dados clínicos e avaliados o índice de massa corpórea e a circunferência abdominal. Na análise bioquímica foram solicitadas dosagens de colesterol total, HDL e LDL colesterol, triglicerídeos e glicemia de jejum. Foram consideradas com síndrome metabólica as mulheres que apresentaram três ou mais critérios diagnósticos: circunferência abdominal maior do que 88 cm; triglicerídeos ≥150 mg/dL; HDL colesterol <50 mg/dL; pressão arterial ≥130/85 mmHg e glicemia de jejum≥110 mg/dL. As participantes foram divididas nos Grupos Controle (n=87) e Síndrome Metabólica (n=108). Empregou-se o questionário Female Sexual Function Index (FSFI) para avaliar a função sexual. RESULTADOS: A média de idade foi de 54,0±4,7 anos. O índice de disfunção sexual em mulheres com síndrome metabólica foi significativamente superior ao do Grupo Controle, quando considerado o FSFI <26,5 (90/108 [83,3%] versus 42/87 [48,2%], p<0,001) ou FSFI <23 (62/108 [57,4%] versus 16/87 [18,39%], p<0,001). Os domínios desejo, excitação, lubrificação, orgasmo (p<0,001) e satisfação (p=0,002) apresentaram escores inferiores nas mulheres portadoras da síndrome metabólica. Para o escore de dor não houve diferença significante (p=0,57). Todos os componentes do diagnóstico da síndrome metabólica estiveram associados a maiores níveis de disfunção sexual (p<0,001). CONCLUSÃO: Mulheres após a menopausa com síndrome metabólica apresentam mais frequentemente disfunção sexual do que aquelas na mesma faixa etária que não são portadoras da síndrome.

OBJETIVO: Adaptar o Pregnancy and Sexual Function Questionnaire (PSFQ) para uso no Brasil, além de avaliar suas propriedades psicométricas. MÉTODOS: Foi realizado um estudo de adaptação e validação com mulheres no último trimestre de gestação, residentes em Rio Branco, Acre. O questionário foi traduzido para a língua portuguesa, revisado e avaliado por um grupo de especialistas, tendo sido realizado um pré-teste. A validação do constructo do instrumento foi avaliada por análise fatorial, a consistência interna, pelos coeficientes alfa de Cronbach e ômega de McDonald e a reprodutibilidade, pela estatística kappa por teste-reteste em uma amostra das gestantes. RESULTADOS: A análise fatorial demonstrou a presença de seis domínios: subjetividade, dor e desconforto; frequência e receptividade; desejo; satisfação; orgasmo e estímulo. A consistência interna segundo o alfa foi de 0,6, enquanto na análise do ômega foi de 0,7. Na amostra, o valor de kappa foi superior a 0,7 em todas as questões. CONCLUSÃO: A versão em português do PSFQ pode ser utilizada para avaliar a função sexual durante a gravidez.

Abstract PURPOSE: To compare differences in the occurrence and changed domains of sexual dysfunction in obese and non-obese Brazilian women. METHODS: Female Sexual Function Index, based on six domains, to investigate 31 sexual dysfunction incidence for obese compared to 32 non-obese women, was used. Statistical analysis using ANOVA and MANOVA were performed to compare total scores of Female Sexual Function Index among groups and to identify the differences among domains, Student t -test was used. Statistical significant level was established for all tests for p<0.05. RESULTS: No difference in female sexual dysfunction frequency between obese (25.8%) and non-obese women (22.5%) was found. However, an important distinction in which aspects of sexual life were affected was found. While the obese group was impaired in three domains of sexual life (desire, orgasm, and arousal), in the control group five aspects were dysfunctional (desire, orgasm, arousal, pain and lubrication). Future research exploring psychological outcomes in obese females, such as body image and measures of positive and negative effect, might better characterize the female sexual dysfunction in this group. CONCLUSIONS: Obesity does not appear to be an independent factor for allow quality of female sexual life. However, disturbance associated to obesity indicates a low frequency of disorder in physical domains, suggesting that psychological factors seem to be mainly involved in the sexual dysfunction in obese women.

Provoked vestibulodynia (PVD) is a prevalent women’s sexual pain disorder, which is associated with sexual function difficulties. Attachment theory has been used to understand adult sexual outcomes, providing a useful framework for examining sexual adaptation in couples confronted with PVD. Research to date indicates that anxious and avoidant attachment dimensions correlate with worse sexual outcomes in community and clinical samples. The present study examined the association between attachment, pain, sexual function and sexual satisfaction in a sample of 101 couples in which the women presented with PVD. The Actor-Partner Interdependence Model was used in order to investigate both actor and partner effects. This study also examined the role of sexual assertiveness as a mediator of these associations via structural equation modeling. Women completed measures of pain intensity and both members of the couple completed measures of romantic attachment, sexual assertiveness, sexual function and satisfaction. Results indicated that attachment dimensions did not predict pain intensity. Both anxious and avoidant attachment were associated with lower sexual satisfaction. Only attachment avoidance predicted lower sexual function in women. Partner effects indicated that higher sexual assertiveness in men predicted better sexual function in women, while higher sexual assertiveness in women predicted higher sexual satisfaction in men. Finally, women’s sexual assertiveness was found to be a significant mediator of the relationship between their attachment dimensions, sexual function and satisfaction. Findings highlight the importance of examining how anxious and avoidant attachment may lead to difficulties in sexual assertiveness and to less satisfying sexual interactions in couples where women suffer from PVD.

Introduction Provoked vestibulodynia (PVD) is suspected to be the most frequent cause of vulvodynia in premenopausal women. Based on the onset of PVD relative to the start of sexual experience, PVD can be divided into primary (PVD1) and secondary PVD (PVD2). Studies comparing these PVD subgroups are inconclusive as to whether differences exist in sexual and psychosocial functioning. Aim The aim of this study was to compare the pain, sexual and psychosocial functioning of a large clinical and community-based sample of premenopausal women with PVD1 and PVD2. Methods A total of 269 women (n = 94 PVD1; n = 175 PVD2) completed measures on sociodemographics, pain, sexual, and psychosocial functioning. Main Outcome Measures Dependent variables were the 0–10 pain numerical rating scale, McGill–Melzack Pain Questionnaire, Female Sexual Function Index, Global Measure of Sexual Satisfaction, Beck Depression Inventory-II, Painful Intercourse Self-Efficacy Scale, Pain Catastrophizing Scale, State-Trait Anxiety Inventory Trait Subscale, Ambivalence over Emotional Expression Questionnaire, Hurlbert Index of Sexual Assertiveness, Experiences in Close Relationships Scale—Revised, and Dyadic Adjustment Scale-Revised. Results At first sexual relationship, women with PVD2 were significantly younger than women with PVD1 (P < 0.01). The average relationship duration was significantly longer in women with PVD2 compared with women with PVD1 (P < 0.01). Although women with PVD1 described a significantly longer duration of pain compared with women with PVD2 (P < 0.01), no significant subtype differences were found in pain intensity during intercourse. When controlling for the sociodemographics mentioned earlier, no significant differences were found in sexual, psychological, and relational functioning between the PVD subgroups. Nevertheless, on average, both groups were in the clinical range of sexual dysfunction and reported impaired psychological functioning. Conclusions The findings show that there are no significant differences in the sexual and psychosocial profiles of women with PVD1 and PVD2. Results suggest that similar psychosocial and sex therapy interventions should be offered to both subgroups of PVD.
