Esta tese apresenta conceitos, estratégias e métodos de avaliação aplicados sobre as organizações de cuidados da saúde das pessoas com HIV/AIDS, procurando argumentar sobre a adequação de certos modos de articulação e significação. A forma de perceber e de pensar as organizações implica na forma de perceber e de pensar a avaliação das organizações. Avaliar é atribuir valor e os valores são históricos e contextuais. A avaliação das organizações de cuidados da saúde das pessoas com HIV/AIDS foi realizada a partir dos pilares da qualidade de Donabedian (eficácia, eficiência, efetividade, equidade, aceitabilidade, legitimação e otimização), que funcionam como territórios temáticos onde são produzidas informações por diferentes atores internos e externos. Foram identificadas referências para futuras indagações, críticas e atividades de avaliação. A avaliação pode fazer parte dos processos de comunicação, discussão e de decisão das organizações para melhoria da sua qualidade, no sentido de maior satisfação dos seus clientes usuários.

Trata dos resultados obtidos pela Coordenação Norte do Projeto QUALlS II Fundação Zerbini, mediante ações de saúde implementadas junto às famílias portadoras de hipertensão arterial sistêmica e/ou diabetes melito. Aborda o comportamento das equipes de saúde da família com base na Teoria de Liderança Transformacional e aponta possível relação entre este e as mudanças obtidas no cotidiano das famílias estudadas

Identifica o grau de satisfação do usuáio de um hospital geral com relação a uma série de atributos dos serviços oferecidos estabelecendo sua relação com as dimensões da qualidade. Analisa comparativamente a satisfação do usuário com a qualidade dos serviços e a percepção gerencial dessa qualidade. Os achados foram obtidos por meio de estudo descritivo com método de pesquisa Survey e uso de questionário estruturado no Hospital Universitário da Universidade de São Paulo

Trata da avaliação de hospitais gerais ocorrida no município de Jundiai - SP no ano de 1995. Aborda: o contexto das políticas públicas de saúde nacional e local; os conceitos de avaliação; os instrumentos de avaliação e seu uso. Estuda especialmente este processo de avaliação, explorando sua realização, seus produtos e resultados e o instrumento utilizado.

Este trabalho analisa como aprimorar um programa de avaliação de desempenho de fornecedores de serviços jurídicos de cobrança de crédito imobiliário realizado em uma grande instituição financeira brasileira. A partir de revisão bibliográfica, determinou-se um escopo do gerenciamento de fornecedores e restrições à sua aplicação, assim como a localização da avaliação de desempenho de fornecedores neste escopo. Posteriormente, delineou-se um quadro referencial para a avaliação de desempenho de fornecedores composto por quatro dimensões: avaliação do sistema da qualidade do fornecedor, avaliação da gestão empresarial empreendida pelo fornecedor, avaliação da adequação ao uso do produto ou serviço fornecido e avaliação do relacionamento entre empresa e fornecedor. Adicionalmente, foram detectados alguns pontos em outros processos relativos ao gerenciamento de fornecedores que causam impacto na avaliação de desempenho do fornecedor. Por meio de entrevistas com membros da organização compradora e de um de seus maiores fornecedores, verificou-se que o quadro referencial se mostrou adequado à avaliação dos fornecedores de serviços jurídicos de cobrança de crédito imobiliário e, dessa forma, puderam ser encontradas lacunas e sugestões de melhoria para o programa em vigor, cumprindo o papel pretendido para o estudo de caso.

Este artigo aborda aspectos relacionados ao uso de modelos de avaliação externa de serviços de saúde no Brasil, sobretudo acreditação hospitalar, a partir da experiência da autora com a implementação de modelos de avaliação da qualidade em hospitais, bem como de sua reflexão sobre acreditação enquanto objeto de estudo da pós-graduação. A autora aponta alguns dos resultados do uso destes modelos em hospitais e as especificidades de sua aplicação. Esses resultados são enumerados: aprimoramento da gestão, padronização de processos, treinamento de pessoal, trabalho em equipe, atenção focada no paciente, motivação, reconhecimento externo, mudança de cultura. A autora problematiza o modo como tais modelos têm sido empregados nos hospitais brasileiros

Tamoxifen (TX), a drug used in the treatment of breast cancer, may cause hepatic changes in some patients. The consequences of its use on the liver tissues of rats with or without diabetes mellitus (DM) have not been fully explored. The purpose of this multidisciplinary study was to evaluate the correlation between plasma hepatic enzyme levels and the presence of iron overload in the hepatic tissue of female Wistar rats with or without streptozotocin-induced DM and using TX. Female rats were studied in control groups: C-0 (non-drug users), C-V (sorbitol vehicle only) and C-TX (using TX). DM (diabetic non-drug users) and DM-TX (diabetics using TX) were the test groups. Sixty days after induced DM, blood samples were collected for glucose, alanine aminotransferase (ALT), aspartate aminotransferase (AST) alkaline phosphatase (ALP) and bilirubin measures. Hepatic fragments were processed and stained with hematoxylin and eosin (H&E), Masson s trichrome, Perls. The hepatic iron content was quantified by atomic absorption spectrometry. AST, ALT and ALP levels were significantly elevated in the DM and DM-TX groups, with unchanged bilirubin levels. Liver iron overload using Perls stain and atomic absorption spectrometry were observed exclusively in groups C-TX and DM-TX. There was positive correlation between AST, ALT and ALP levels and microscopic hepatic siderosis intensity in group DM-TX. In conclusion, TX administration is associated with liver siderosis in diabetic and non-diabetic rats. In addition, TX induced liver iron overload with unaltered hepatic function in 2 non-diabetic rats and may be a useful tool for investigating the biological control of iron metabolism

Alterations in the neuropsychomotor development of children are not rare and can manifest themselves with varying intensity at different stages of their development. In this context, maternal risk factors may contribute to the appearance of these alterations. A number of studies have reported that neuropsychomotor development diagnosis is not an easy task, especially in the basic public health network. Diagnosis requires effective, low-cost, and easy - to-apply procedures. The Denver Developmental Screening Test, first published in 1967, is currently used in several countries. It has been revised and renamed as the Denver II Test and meets the aforementioned criteria. Accordingly, the aim of this study was to apply the Denver II Test in order to verify the prevalence of suspected neuropsychomotor development delay in children between the ages of 0 and 12 months and correlate it with the following maternal risk factors: family income, schooling, age at pregnancy, drug use during pregnancy, gestational age, gestational problems, type of delivery and the desire to have children. For data collection, performed during the first 6 months of 2004, a clinical assessment was made of 398 children selected by pediatricians and the nursing team of each public health unit. Later, the parents or guardians were asked to complete a structured questionnaire to determine possible risk indicators of neuropsychomotor development delay. Finally the Denver II Developmental Screening Test (DDST) was applied. The data were analyzed together, using Statistical Package for Social Science (SPSS) software, version 6.1. The confidence interval was set at 95%. The Denver II Test yielded normal and questionable results. This suggests compromised neuropsychomotor development in the children examined and deserves further investigation. The correlation of the results with preestablished maternal risk variables (family income, mother s schooling, age at pregnancy, drug use during the pregnancy and gestational age) was strongly significant. The other maternal risk variables (gestational problems, type of delivery and desire to have children) were not significant. Using an adjusted logistic regression model, we obtained the estimate of the greater likelihood of a child having suspected neuropsychomotor development delay: a mother with _75 4 years of schooling, chronological age less than 20 years and a drug user during pregnancy. This study produced two manuscripts, one published in Acta Cirúrgica Brasileira , in which an analysis was performed of children with suspected neuropsychomotor development delay in the city of Natal, Brazil. The other paper (to be published) analyzed the magnitude of the independent variable maternal schooling associated to neuropsychomotor development delay, every 3 months during the first twelve months of life of the children selected.. The results of the present study reinforce the multifactorial characteristic of development and the cumulative effect of maternal risk factors, and show the need for a regional policy that promotes low-cost programs for the community, involving children at risk of neuropsychomotor development delay. Moreover, they suggest the need for better qualified health professionals in terms of monitoring child development. This was an inter- and multidisciplinary study with the integrated participation of doctors, nurses, nursing assistants and professionals from other areas, such as statisticians and information technology professionals, who met all the requirements of the Postgraduate Program in Health Sciences of the Federal University of Rio Grande do Norte

The National School Nutrition Program is the oldest program in the country directed at food and nutrition safety. Its aims are to offer nutritional food as a supplement to students of public and philanthropic schools. Studying school nutrition transcends the investigation as a purely social program, given that it reaches the fields of public health, nutrition and food, using a wide variety of approaches. Thus, it is characterized by a multidisciplinary study, where the disciplines work side by side in distinct aspects of a single problem. Aim: This study aims to assess hygiene practices during the preparation of meat-based meals in public schools in the city of Natal, Brazil. Methods: A list was applied at 27 schools to identify the procedures of good food preparation practices. In addition, cooking and meal distribution temperature were measured and a microbiological analysis of the final preparation and of the water used in preparing it was performed. For microbiological analyses of the food, we analyzed coliforms at 45°C, coliforms at 35°C and Enterococcus, and for the water, we analyzed thermotolerant coliforms and total coliforms, using the methods recommended by APHA, 1995. Results: Most of the schools did not meet the required standards in all the variables related to good food preparation practices, except for the time spent preparing the meat, in which 89% were within the norm. Cooking temperature of the meals was within the standard; however, the temperature at distribution and the time spent dispensing the meals were inadequate. Of the 27 schools, 22 (81.5%) showed the presence of coliforms at 35° C in at least one meal sample and 18 (66.7%) had values above the recommended limit for coliforms at 45°C. The presence of E. coli was identified in 6.1% of the samples analyzed. The presence of Enterococcus was not found at any of the schools. With respect to the water, the North district of the city was the only one that did not meet the standards for the two indicators evaluated. The contamination found was not associated with the hygiene or food storage problems observed. Conclusions: The results show that the hygiene-sanitary conditions of meat-based public school meals were unsatisfactory, demonstrating the need for improvements in the production process to preserve the health of the student population. This study evaluated the effect of indole-3-acetic acid (IAA) in the activation of goats preantral follicles (FOPA). Were used four pairs of ovaries of adult mixed breed goats. Each ovarian pair was divided into 23 fragments. One fragment was fixed for histology and other fragment was using to follicular isolation procedure. The remaining fragments were cultured in 1.0 ml of Minimum Essential Medium (MEM) or MEM supplemented with IAA at concentrations of 10, 40, 100, 500 or 1000 ng/mL. The in vitro culture was performed at 39°C in incubator with 5% CO2 for 1, 3 and 5 days. After in vitro culture was evaluated histological integrity and viability of FOPA. The addition of 100 ng/ml of IAA to MEM showed a significant increase in follicles transition in the third day of in vitro culture, characterizing follicular activation. Moreover, this concentration was obtained maintaining the histological integrity of PAF by the fifth day of in vitro culture. The viability test confirmed the results of histology. Thus, we conclude that IAA can promote the activation of goats FOPA

The occurrence of human cancer in the municipality of Lucrécia (RN, Brazil) has shown high frequencies (INCA, 2007). Tests of micronucleus (MN) and nuclear abnormalities (NA) were performed to assess the genotoxic potential of water from the Lucrécia dam, which is located in a semi-arid region under the influence of crop irrigation and irregular rainfall. Water samples were collected in this source for analyzing the concentration of cyanobacteria, metals and radioactivity. Erythrocytes of Nile tilapia (Oreochromis niloticus) were collected in dam and cells of human oral mucosa in the urban area of this municipality for the bioassays of MN and NA. In fish were also analyzed concentrations of metals in samples of liver and gills. The genotoxicity tests with biological samples of fish and humans have shown significant increases in the frequencies of MN and NA (p ≤ 0.05) and are indicative of increased DNA damage in relation to the control groups. In conclusion, the results obtained from water samples and biological municipality of Lucrécia indicates that the presence of chemical and microbiological pollutants, and increase of genotoxic in human of this municipality. We suggest the implementation of advanced water treatment, to prevent further degradation of the aquatic environment and decrease in the life quality. This research of environmental quality assessment was performed in order to contemplate a multi and interdisciplinary character of this water resource and that can induce genotoxic damage in the organisms in this study region

Contextualization: Several studies have examined the mobility of this group of children, however little is known about the impact of motor function in activities of daily living, considering the seriousness of their neuromotor damage. Objective: Identify the functional differences of children with Cerebral Palsy with different levels of motor dysfunction and correlate these differences with the areas of mobility, self-care and social function in functional ability and caregiver´s assistance of these children. Methods: An l analytical cross-section search was developed, which were part 70 children / families aged from 4 to 7.5 years, in the Rehabilitation Center for Children. As tools were used the Pediatric Evaluation Disability Inventory (PEDI) and the Gross Motor Function Classification System (GMFCS). Data analysis was performed by ANOVA and Pearson's correlation tests. Results: The results show the functional variability of children CP in different severity levels of motor disfunction This variation was observed in the areas of mobility, self-care and social function. The results also showed a strong correlation between the domains mobility and self-care, mobility and social function. Conclusions: The variability shown by the children with CP, suggests the use of PEDI and GMFCS as this association appears to increase the understanding of how the gross motor functions are related to activities of daily living, describing the best commitments and their degree of impact on functional activities. This correlation demonstrates how mobility is crucial to evaluate the performance and guide the therapeutic practice, to develop the children´s potencial, and guide the caregiver in stimulation
