HIV-positive adolescents face a number of challenges in dealing with their disease and its treatment. In this qualitative study, twenty-nine HIV-positive adolescents aged 13 to 20 years (22 girls), who live in Switzerland, were asked, in a semi-structured interview (duration of 40-110 minutes), to describe their perceptions and experiences with the disease itself and with therapeutic adherence. While younger adolescents most often thought of their disease as fate, older adolescents usually knew that they had received it through vertical transmission, although the topic appeared to be particularly difficult to discuss for those living with their HIV-positive mothers. Based on their attending physician's assessment, 18 subjects were judged highly adherent, 4 fairly and 7 poorly adherent. High adherence appeared linked with adequate psychological adjustment and effective coping mechanisms, as well as with the discussion and adoption of explicit medication-taking strategies. The setting and organisation of health care teams should allow for ongoing discussions with HIV-positive adolescents that focus on their perceptions of their disease, how they cope with it and with the treatment, and how they could improve their adherence.

Dental injuries are common and the incidence of maxillofacial injuries has increased over the recent decades in Finland. Accidental injuries are the global leading cause of death among children over the age of one year and among adults under the age of 40 globally. Significant resources and costs are needed for the treatment of these patients. The prevention is the most economical way to reduce trauma rates and costs. For the prevention it is crucial to know the prevalences, incidences and risk factors related to injuries. To improve the quality of treatment, it is essential to explore the causes, trauma mechanisms and management of trauma. The above mentioned was the aim of this thesis. With a large epidemiological cohort study (5737 participants) it was possible to estimate lifetime prevalence of and risk factors for dental trauma in general population (Study I). The prevalence of dental fractures was 43% and the prevalence of dental luxations and avulsions was 14%. Male gender, a history of previous non-dental injuries, mental distress, overweight and high alcohol consumption were positively associated with the occurrence of dental injuries Study II was conducted to explore the differences in type and multiplicity of mandibular fractures in three different countries (Canada, Finland and Kuwait). This retrospective study showed that the differences in mandibular fracture multiplicity and location are based on different etiologies and demographic patterns. This data can be exploited for planning of measures to prevent traumatic facial fractures. The etiology, management and outcome of 63 pediatric skull base fracture (Study III) and 20 pediatric frontobasal fracture patients (Study IV) were explored. These retrospective studies showed that, both skull base fracture and frontobasa fracture are rare injuries in childhood and although intracranial injuries and morbidity are frequent, permanent neurological or neuropsychological deficits are infrequent. A systematic algorithm (Study V) for computer tomography (CT) image review was aimed at clinicians and radiologists to improve the assessment of patients with complex upper midface and cranial base trauma. The cohort study was cross sectional and data was collected in the Turku and Oulu University Hospitals. A novel image-reviewing algorithm was created to enhance the specificity of CT for the diagnosis of frontobasal fractures. The study showed that an image-viewing algorithm standardizes the frontobasal trauma detection procedure and leads to better control and assessment. The purpose of the retrospective subcranial craniotomy study (VI) was to review the types of frontobasal fractures and their management, complications and outcome when the fracture is approached subcranially. The subcranial approach appears to be successful and have a reasonably low complication rate. It may be recommended as the technique of choice in multiple and the most complicated frontal base fractures where the endoscopic endonasal approach is not feasible.

Introduction: Canada’s aging population is diverse and this diversity will continue to grow for the next two decades (Government of Canada, 2002; Katz, 2005; Statistics Canada, 2010). Objective: to examine the relationship between dementia family caregivers’ traditionally-based beliefs about caregiving, their caregiving experience, and their well-being. Method: exploratory secondary data analysis of cross-sectional survey data from 76 community caregivers of persons with dementia in Ontario. Results: traditional values for caregiving was independently associated with coping resources and health status but not depression symptoms. Caregiver self-efficacy and social support both partially mediated the relationship between beliefs about caregiving and caregiver health status. Discussion: Findings from this exploratory study are consistent with stress process models of culture and caregiving. The finding that self-efficacy was associated with traditional values and that it mediated the relationship between traditional values and caregiver well-being is new to the literature.

Comme dans la plupart des pays francophones d’Afrique Subsaharienne, la question du vieillissement ou la situation des personnes âgées ne sont pas encore d’actualité au Niger, principalement à cause de la jeunesse de sa population d’une part et d’un intérêt plus porté sur les enfants, les adolescents et les mères d’autre part. Pourtant le Niger fait face à une crise économique sans précédent qui peut avoir des conséquences néfastes sur les conditions de vie des personnes âgées. D’un côté, selon la coutume, la personne âgée (généralement grand parent) s’occupe principalement des petits enfants (orphelins ou non) qui lui sont confiés par leurs parents vivant dans la même localité ou ailleurs, ou qui sont décédés. De l’autre, l’absence d’un jeune adulte dans un ménage où vit au moins une personne âgée est considérée comme un phénomène social préoccupant dans les pays à forte prévalence de VIH/SIDA. Le Niger fait partie des pays où la proportion des personnes âgées vivant avec des petits enfants en l’absence de leurs parents adultes est la plus élevée. Cependant, malgré une forte mortalité adulte, l’absence de données fiables ne permet pas de le classer parmi les pays à forte mortalité adulte due au VIH/SIDA. La raison de cette situation est donc à chercher dans les différences individuelles et communautaires. Jusqu’au début des années 1990, la plupart des études sur les personnes âgées réalisées en Afrique Subsaharienne étaient basées sur les études qualitatives, tandis que les plus récentes sont faites à partir des données des recensements ou enquêtes sociodémoraphiques et économiques. Les conditions de vie des personnes âgées et les conséquences de la pauvreté et du VIH/SIDA sur celles-ci sont les principaux thèmes jusque-là couverts à l’aide des données existantes. Mais, il manque encore de données longitudinales essentielles à l’analyse de certains aspects du cycle de vie des personnes âgées. L’étude n’étant pas sociologique, c’est à l’aide de données démographiques quantitatives, plus précisément le recensement général de la population, que nous tenterons d’expliquer le phénomène sur une base exploratoire. L’analyse au niveau individuel a été faite à l’aide de la régression logistique sous STATA, tandis qu’au niveau contextuel, nous avons utilisé l’analyse multiniveau à l’aide du logiciel HLM (version 6.0). Les résultats indiquent que la vie en l’absence d’un jeune adulte et dans un ménage à génération coupée dépendent principalement du statut sociodémographique de la personne âgée au Niger. Par exemple, il ressort que le mariage avantage l’homme âgé, tandis que le veuvage l’isole plus que la femme âgée. Au niveau contextuel, ce sont les facteurs socioéconomiques qui influencent les conditions de vie des personnes âgées. L’étude montre, en effet, que le degré d’urbanisation d’une commune augmente le risque d’isolement d’une personne âgée qui y réside, alors que le niveau de pauvreté le réduit. Toutefois, nos résultats sont à prendre avec prudence parce qu’en premier lieu il n’existe pas d’études références sur le sujet tant au Niger que dans la sous-région d’Afrique francophone sahélienne. Ensuite, parce que le phénomène étudié pourrait être mesuré de plusieurs manières en fonction du contexte et des données disponibles, et que l’analyse approfondie des effets du statut matrimonial nécessiterait une plus grande connaissance du phénomène chez les personnes âgées. Enfin, compte tenu de la faible prévalence du VIH/SIDA au Niger, les principaux facteurs explicatifs de la vie dans un ménage à génération coupée (aussi bien pour les personnes âgées que pour les enfants) pourraient être le confiage des enfants ou la mortalité adulte due aux autres causes telles que le paludisme, la tuberculose et les maladies infectieuses. Toutefois, l’absence d’informations relatives à ces aspects dans les données utilisées n’a pas permis de les intégrer dans notre étude. Ainsi, compte tenu de la difficulté d’appréhender les contours du phénomène, les futurs programmes en faveur des personnes âgées au Niger et en Afrique Subsaharienne francophone doivent se baser sur des études concrètes relatives aux dimensions sociale et économique du phénomène. Introducción: Existen condiciones que definen la gravedad de accidentes laborales de tipo riesgo biológico como el contacto con una fuente conocida o desconocida, tipo de exposición según el fluido de contacto, el elemento causante y el tipo de lesión que causa. Objetivo: Describir e identificar la ocurrencia de los accidentes de trabajo con riesgo biológico que tienen la población trabajadora del sector salud y afines, afiliados a una ARL reportados durante el año 2014, suministradas en una base de datos de la IPS encargada del manejo de los eventos, con el fin de destacar su impacto y métodos de prevención de los mismos a través de la ARL. Materiales y métodos: Se realizó un estudio de corte transversal en el que se analizaron datos secundarios procedentes de una base de datos que incluyo los eventos sobre riesgo biológico reportados a una ARL entre el 1 de febrero del 2014 y el 31 de diciembre de 2014, se reportaron 1806 accidentes de tipo riesgo biológico donde se analizaron características sociodemográficas, tipo de fluido, tipo de fuente, tipo de accidente y tipo de elemento. Resultados: Se evidencio que los auxiliares de la salud fueron el grupo con mayor accidentalidad (65%) y los técnicos (49.9%). El sitio de mayor frecuencia de ocurrencia de los accidentes fue el intrahospitalario con 89.5%. La mayor accidentalidad se relacionó con el contacto con sangre y se presentó en trabajadores con nivel universitario (89.8%), seguido por los técnicos (84.6%) y auxiliares (67.3%). Se observó una ocurrencia de accidentes entre trabajadores con nivel primaria y secundaria con un porcentaje de 95.7% y 91.6% respectivamente, quienes además tienen mayor riesgo a tener accidentes con fuentes desconocidas, mientras que los técnicos (68.5%) y los universitarios (80.1%) fueron con fuente conocida. Dentro del grupo de auxiliares se destaca accidentes con fuente desconocida con 53.4%. Debido a la manipulación de elementos cortopunzantes con las manos, los dedos fueron la parte anatómica más afectada (84.1%) con elementos de alto riesgo y específicamente con sangre (77.1%). Conclusiones: El personal del área de la salud fue el más afectado dado el contacto directo con pacientes. Las auxiliares de servicios de salud fueron las más afectadas por tener contacto directo en la manipulación de elementos cortopunzantes con fluidos de alto riesgo como la sangre, y los dedos la parte anatómica más afectada con fuente conocida.

Introdução: O adiamento das altas clínicas nas Unidades de Cuidados Continuados Integrados (UCCI) por motivos sociais é actualmente considerado um dos principais motivos que impedem a integração atempada de novos clientes na Rede Nacional de Cuidados Continuados, RNCC, daqui em diante designada como a REDE. Este atraso tem impacto ao nível da recuperação e estabilização dos utentes, bem como ao nível de eficiência e eficácia da UCCI, não podendo deixar de se considerarem os aspectos sociais e económicos. Objectivo Geral: Identificar os determinantes que influenciam as altas clínicas em UCCI. Métodos e População do Estudo: Este é um estudo de caso colectivo, em que os dados observacionais, transversais, são recolhidos por meio de questionário de auto-relato (por cada área de intervenção directa) e por análise dos processos de consulta de pacientes. O objecto desta pesquisa abrange dois grupos: o grupo de amostra composto por 70 profissionais de saúde que lidam directamente com os utentes e o grupo amostra composto de utentes internados na UCCI L Nostrum, com alta clínica entre 1/1/2011 e 31/12/2012, e que foram integrados através da REDE. Foram recolhidos os dados de 293 utentes sendo objecto de estudo os casos de 83 utentes integrados através da REDE e com prolongamento de internamento por motivos sociais. Resultados: Na percepção dos profissionais de saúde, as respostas institucionais apresentam-se como a condicionante mais indicada, tanto para os utentes em geral, com 22 indicações (88%) como para os utentes da REDE, com 10 indicações (40%). Relativamente aos motivos familiares há referência de 76% para os utentes em geral e de 36% para os utentes da REDE. Os motivos económicos também apresentam, para os profissionais inquiridos, um valor expressivo (68%) nos utentes em geral, estando nos da REDE este factor condicionante a par com os motivos familiares (36%). Os motivos estruturais têm menor expressão tanto nos utentes em geral (32%) como nos utentes da REDE (16%). “Outros” para os utentes em geral, refere-se a dependência funcional (4%). Nos motivos familiares, para os utentes em geral, 23 (92%) foi mais vezes indicada a insuficiência de suporte familiar, para os utentes da REDE, 13 (52%). A ausência de suporte familiar, para os utentes em geral, representa 48% das respostas, seguindo-se o suporte inadequado (28%) e a ausência de cuidadores (24%). Para os utentes da REDE, o suporte inadequado apresenta-se como segundo motivo (7%), seguindo-se a ausência de suporte familiar (16%). Na percepção dos profissionais, os utentes da REDE estão também condicionados pela distância geográfica (8%) da sua área residencial. Em termos estruturais, os motivos mais assinalados pelos profissionais para a generalidade dos utentes foram as barreiras físicas à mobilidade (80%) e a habitação sem condições básicas de habitabilidade (78%). Os mesmos motivos foram assinalados para os utentes da REDE, barreiras físicas à mobilidade (40%) e habitação sem condições de habitabilidade (28%). No entanto, relativamente aos utentes em geral, a ausência de habitação (29%) e a distância geográfica (4%) também foram motivos assinalados. Dos motivos económicos percebidos pelos profissionais, a insuficiência de rendimentos é o factor mais assinalado pela generalidade dos utentes (84%) e pelos da REDE (68%), seguida da percepção da capacidade de reposta limitada das instituições, 64% para a generalidade dos utentes e 28% para os da REDE e por fim os tipos de respostas insuficientes para as necessidades individuais dos utentes (20% dos utentes em geral e 12% da REDE). No total dos dois anos, 2011 e 2012, verificaram-se na UCCI L Nostrum 293 prorrogações (100%) das quais 210 (71,6%) foram consideradas dentro do prazo e justificadas com motivos clínicos, enquanto 83 (28,3%) foram efectivamente protelamentos por motivos sociais, tendo em conta que nestes casos os utentes já não tinham critérios clínicos que justificassem a sua permanência na UCCI. Das 210 prorrogações consideradas dentro do prazo e justificadas com motivos clínicos, 93 (44,3%) foram-no por tempo de espera para transferência de UCCI. Em 2011, dos 146 utentes com alta protelada (100%), 50 utentes (34,2%) permaneceram na UCCI por motivos sociais, enquanto em 2012 houve registo de 33 casos de protelamento (22,4%) em 147 (100%) altas prorrogadas. Conclusões: Dos factores identificados como motivo de protelamento nos 83 utentes, estritamente por motivos sociais, destaca-se o protelamento de alta por espera de integração em equipamento/resposta adequada, nomeadamente lar ou serviços de apoio domiciliário (79,5%), seguindo-se a insuficiência de rendimentos do utente/familiares para contratação de serviços ou resposta institucional (74,7%), a inexistência de condições habitacionais para regresso ao domicílio (63,9%) e a insuficiência de suporte familiar (54,2%). Regista-se também a inadequação do suporte familiar (31,3%), a inexistência de suporte familiar (28,9%) e, em menor percentagem, a ausência de condições estruturais (13,3%). A ausência de domicílio (sem abrigo) (8,4%) e a ausência de rendimentos (4,8%) também foram factores inibidores da alta clínica. Dos 293 utentes identificados que tiveram protelamento da alta por motivos sociais verificou-se que 144 (49,1%) dos utentes permaneceram unicamente pela existência de condicionantes institucionais e familiares/estruturais. Aspectos éticos: ao longo deste estudo, foram assegurados e respeitados, todos os procedimentos de garantia da confidencialidade e rigor na recolha dos dados, e a não interferência nas dinâmicas da instituição, dos utentes e dos profissionais.

The workplace is a manageable community-based setting for ensuring proper nutrition. This study aimed to evaluate dietary quality and associated factors among adult workers at a cosmetics factory in the metropolitan area of Sao Paulo, Brazil. This factory was actively participating in the Brazilian Workers` Meal Program, which was created to ensure workers` nutritional health. In this cross-sectional study, data on 202 adult workers were assessed using questionnaires (sociodemographic, anthropometric, and lifestyle characteristics) administered during August and September 2006. Dietary intake, measured by 24-hour dietary recall, was used to calculate the modified Healthy Eating Index (HEI). A repeated administration of the 24-hour dietary recall was applied in a random subsample to calculate the modified HEI adjusted for the within-person variation in intake. Mean adjusted modified HEI scores were analyzed using multiple linear regression adjusted for energy. The mean adjusted modified HEI score was 72.3 +/- 8.0. The lowest adjusted modified HEI components scores were ""milk and dairy products"" (4.4 +/- 3.2) and ""sodium"" (3.7 +/- 3.1). Two percent of workers had ""poor diet"" (adjusted modified HEI score <51 points) and the majority (87%) had ""diet that needs modification"" (adjusted modified HEI score between 51 and 80), despite their participation in the meal program. Adjusted modified HEI scores were considerably higher for men (74.7 +/- 7.0) than for women (66.9 +/- 8.2) and for normal body mass index (calculated as kg/m(2)) (73.3 +/- 7.8) than for overweight/obese (70.9 +/- 8.1). Based on these results, the vast majority of workers were found to have diets that needed improvement. Individuals with higher-quality diets were more likely to have lower body mass index and to be male. J Am Diet Assoc. 2010;110:786-790.

I ett internationellt perspektiv har Sverige hög amningsfrekvens, men under 2000-talet har amningsfrekvensen sjunkit och stora regionala skillnader ses. Kvinnorna rekommenderas att helamma sex månader. Mödravårdcentralen (MVC) informerar om bröstmjölkens hälsoeffekter som till exempel att bröstmjölken skyddar barn mot allergier. Forskning visar att överviktiga och feta kvinnor har kortare amningsduration, men få studier behandlar svenska förhållanden. Syfte: Syfte med fördjupningsarbetet är att studera samband mellan mammans BMI relaterat till amningsduration och mammans skattning av barnets hälsa de tre första levnadsåren. Metod: Studien är en kvantitativ retrospektiv tvärsnittstudie. Data insamlades via enkäter, 418 deltagare inkluderades. Data har analyserats med Statistical Package for the Social Sciences (SPSS). För deskriptiv och jämförande analys har parametriska och icke-parametriska analyser genomförts. Resultat: De kvinnor som inte ammade skattade sitt barns hälsa sämre vid tre års ålder i jämförelse med kvinnor som ammat någon period. Vid tre års ålder skattade kvinnor med övervikt och fetma sitt barns hälsa sämre. Slutsats: Det är viktigt att kunna identifiera kvinnor som är behov av stöd för att initiera amning och kunna bidra till anpassat stöd till dem. AbstractSweden reports high duration of breastfeeding compared to international findings. During the last century, the frequency of breastfeeding duration has decreased in Sweden, with large regional differences. Recommendation for exclusive breastfeeding is six months. Midwives provide information about healthy benefits of breast milk, such as breast milk protecting baby´s against allergies. Research has presented associations between obesity and short duration of breastfeeding. However, there is a lack of knowledge regarding women in a Swedish context. Aim: To investigate the relation between mothers BMI, duration of breastfeeding and maternal valuation of the baby´s health during the first three year of life. Method: The study is a quantitative retrospective cross-sectional study. Data was collected through questionnaires, 418 participants were included. Data was analyzed using the SPSS. For descriptive and comparative analysis parametric and nonparamateric statistics have been used.Results: Women who did not breastfed perceived their baby´s health worse at three years compared with women who did breastfeed. Women with overweight and obesity also perceived the health of their three year old worse than the other maternal group. Conclusions: It´s important to identify women who need support to initiate breastfeeding and give them customized support.

Bakgrund: Internet används globalt inom många områden och aktuell forskning visar att gravida kvinnor använder Internet till att söka information om graviditet och förlossning. En graviditet innebär många frågor hos kvinnan och kvinnor söker efter information för att få en trygghet och försäkran om att allt är normalt med deras hälsa och barnet i magen. Syfte: Syftet är att undersöka gravida kvinnors informationssökning via Internet avseende graviditet och förlossning. Metod: Beskrivande tvärsnittstudie. Datainsamling skedde via en enkätundersökning och data analyserades med deskriptiv och jämförande ansats. Resultat: Nästan alla kvinnor i undersökningen använde Internet för att söka information om graviditet och förlossning. Kvinnorna söker information om barnets utveckling, kost/näring och graviditetslängd i hög utsträckning. Högutbildade kvinnor har andra kriterier för trovärdighet på hemsidor än kvinnor med lägre utbildning. Slutsats: Barnmorskor bör vara medvetna om gravida kvinnors användning av Internet samt själva skapa sig en uppfattning om informationen som de gravida kvinnorna läser på Internet. Vår studie visar att barnmorskors rekommendation av hemsidor på Internet har stor betydelse för gravida kvinnor.

Objectives: This in vitro study assessed the effect of a 4% TiF4 varnish on demineralisation and remineralisation of sound enamel and artificial carious enamel lesions, respectively.Methods: Bovine sound and carious enamel (n = 110) were randomly allocated to each type of varnish: Duraphat (R))-D (NaF, 2.26%F, pH 4.5, Colgate-Brazil, n = 30), Duofluorid (R)-F (NaF, 2.71%F, pH 8.0, FGM-Brazil, n = 30), TiF4-T (2.45%F, pH 1.0, FGM-Brazil, n = 30) and no-fluoride-P (FGM-Brazil, pH 5.0, n = 20). For the formation of artificial enamel caries, half of the blocks were immersed in 32 mL buffer acetate solution (16 h), whereas the other half was maintained sound. The varnishes were applied onto the enamel surfaces. Thus, the samples were subjected to pH cycles (37 degrees C) for 7 days. The response variables tested were surface and cross-sectional hardness. Data were tested using Kruskal-Wallis test (p < 0.05).Results: All F varnishes significantly reduced demineralisation and increased remineralisation in comparison to placebo. The TiF4 did not significantly reduce the surface enamel softening when compared with the other F varnishes, but it decreased the loss of subsurface hardness to the same extent. In enamel blocks with previous artificial carious lesions, the TiF4 significantly improved the rehardening compared to the other varnishes up to 30 mu m depth.Conclusions: The TiF4 varnish was able to decrease the demineralisation and increase the remineralisation of previously sound and carious enamel, respectively. It was equally effective compared to NaF varnishes on reducing the demineralisation at subsurface, but it was more effective on improving the remineralisation at surface and subsurface. The objective of this study is to measure the impact of the national subsidy scheme on the olive and fruit sector in two regions of Albania, Shkodra and Fier. From the methodological point of view, we use a non- parametric approach based on the propensity score matching. This method overcomes problem of the missing data, by creating a counterfactual scenario. In the first step, the conditional probability to participate in the program was computed. Afterwards, different matching estimators were applied to establish whether the subsidies have affected sectors performance. One of the strengths of this study stays in the data. Cross-sectional primary data was gathered through about 250 interviews.. We have not found empirical evidence of significant effects of government aid program on production. Differences in production found between beneficiaries and non-beneficiaries disappear after adjustment by the conditional probability of participating into the program. This suggests that subsidized farmers would have performed better than the subsidized households even in the absence of production grants, revealing program self-selection. On the other hand, the scheme has affected positively the farm structure increasing the area under cultivation, but yields has not increased for beneficiaries compared to non beneficiaries. These combined results shed light on the reason of the missed impact. It could be reasonable to believe that the new plantation, in particular in the case of olives, has not yet reached full production. Therefore, we have reasons to believe on positive impacts in the future. Concerning some qualitative results, the extension of area under cultivation is strongly conditioned by the small farm size. This together with a thin land market makes extremely difficult the expansion beyond farm boundaries.

The objective of this study was to explore whether it is possible to describe based on the International Classification of Functioning, Disability and Health (ICF) relevant aspects of functioning and disability affected in multiple sclerosis (MS) as well as environmental factors relevant to persons with MS. The specific aim was to identify most relevant 'Body functions', 'Body structures', 'Activities and participation', as well as 'Environmental factors' in patients with MS using the ICF. Additionally, different MS forms were compared with respect to the identified problems. A multi-centre study was conducted in an empirical cross-sectional design. Data from 205 individuals with MS were collected in rehabilitation centres: disease related data, socio-demographic data, single interviews based on the Extended ICF Checklist and a patient questionnaire including ratings on general health and functioning status, Beck Depression Inventory II (BDI-II) and Comorbidity Questionnaire (SCQ). The 129 ICF categories identified represent a comprehensive classification of functioning in MS from the clinical perspective. Differences between MS forms were observed for several ICF categories, EDSS, general health and functioning status, but not for BDI and SCQ. The study showed that it is possible to describe based on the ICF the spectrum in functioning and disability affected in MS as well as environmental factors relevant to persons with MS.
