BACKGROUND Cocoa is rich in flavonoids, has anti-oxidative properties and increases the bioavailability of nitric oxide (NO). Adequate renal tissue oxygenation is crucial for the maintenance of renal function. The goal of this study was to investigate the effect of cocoa-rich dark chocolate (DC) on renal tissue oxygenation in humans, as compared to flavonoid-poor white chocolate (WC). METHODS Ten healthy volunteers with preserved kidney function (mean age ± SD 35 ± 12 years, 70% women, BMI 21 ± 3 kg/m2) underwent blood oxygenation level-dependent magnetic resonance imaging (BOLD-MRI) before and 2 hours after the ingestion of 1 g/kg of DC (70% cocoa). Renal tissue oxygenation was determined by the measurement of R2* maps on 4 coronal slices covering both kidneys. The mean R2* (= 1/T2*) values in the medulla and cortex were calculated, a low R2* indicating high tissue oxygenation. Eight participants also underwent BOLD-MRI at least 1 week later, before and 2 hours after the intake of 1 g/kg WC. RESULTS The mean medullary R2* was lower after DC intake compared to baseline (28.2 ± 1.3 s-1 vs. 29.6 ± 1.3 s-1, p = 0.04), whereas cortical and medullary R2* values did not change after WC intake. The change in medullary R2* correlated with the level of circulating (epi)catechines, metabolites of flavonoids (r = 0.74, p = 0.037), and was independent of plasma renin activity. CONCLUSION This study suggests for the first time an increase of renal medullary oxygenation after intake of dark chocolate. Whether this is linked to flavonoid-induced changes in renal perfusion or oxygen consumption, and whether cocoa has potentially renoprotective properties, merits further study.

The stroke-prone spontaneously hypertensive rat (SHRSP) is a genetically determined model of “salt-sensitive” stroke and hypertension whose full phenotypic expression is said to require a diet high in Na+ and low in K+. We tested the hypothesis that dietary Cl− determines the phenotypic expression of the SHRSP. In the SHRSP fed a normal NaCl diet, supplementing dietary K+ with KCl exacerbated hypertension, whereas supplementing either KHCO3 or potassium citrate (KB/C) attenuated hypertension, when blood pressure (BP) was measured radiotelemetrically, directly and continually. Supplemental KCl, but not KB/C, induced strokes, which occurred in all and only those rats in the highest quartiles of both BP and plasma renin activity (PRA). PRA was higher with KCl than with KB/C. These observations demonstrate that with respect to both severity of hypertension and frequency of stroke the phenotypic expression of the SHRSP is (i) either increased or decreased, depending on whether the anionic component of the potassium salt supplemented is, or is not, Cl−; (ii) increased by supplementing Cl− without supplementing Na+, and despite supplementing K+; and hence (iii) both selectively Cl−-sensitive and Cl−-determined. The observations suggest that in the SHRSP selectively supplemented with Cl− the likelihood of stroke depends on the extent to which both BP and PRA increase.

Primary aldosteronism (PA) is a common form of endocrine hypertension previously believed to account for less than 1% of hypertensive patients. Hypokalemia was considered a prerequisite for pursuing diagnostic tests for PA. Recent studies applying the plasma aldosterone/plasma renin activity ratio (ARR) as a screening test have reported a higher prevalence. This study is a retrospective evaluation of the diagnosis of PA from clinical centers in five continents before and after the widespread use of the ARR as a screening test. The application of this strategy to a greater number of hypertensives led to a 5- to 15-fold increase in the identification of patients affected by PA. Only a small proportion of patients ( between 9 and 37%) were hypokalemic. The annual detection rate of aldosterone-producing adenoma (APA) increased in all centers ( by 1.3-6.3 times) after the wide application of ARR. Aldosterone-producing adenomas constituted a much higher proportion of patients with PA in the four centers that employed adrenal venous sampling ( 28 - 50%) than in the center that did not (9%). In conclusion, the wide use of the ARR as a screening test in hypertensive patients led to a marked increase in the detection rate of PA. Copyright © 2004 by The Endocrine Society

Once considered rare, primary aldosteronism (PAL) is now regarded as the commonest potentially curable and specifically treatable form of hypertension. At Greenslopes Hospital Hypertension Unit (GHHU), the decision in 1991 to screen all (and not just hypokalemic or resistant) hypertensives by aldosterone/renin ratio (ARR) testing led to a 10-fold increase in detection rate of PAL and four-fold increase in removal rate of aldosterone-producing adenomas (APAs). The GHHU/Princess Alexandra Hospital Hypertension Unit PAL series stands at 977 patients and 250 APAs removed with hypertension cured in 50-60% (remainder improved). Reliable detection requires that interfering medications are withdrawn (or their effects considered) before ARR measurement, and reliable methods (such as fludrocortisone suppression testing) to confirm PAL. Adrenal venous sampling is the only dependable way to differentiate APA from bilateral adrenal hyperplasia. Genetic testing has facilitated detection of alucocorticoid-remediable, familial PAL. Identification of mutations causing the more common familial variety described by GHHU in 1991 should further aid in detection of PAL. (C) 2003 Elsevier Ireland Ltd. All rights reserved.

Monogenic mutations leading to excessive activation of the mineralocorticoid pathway result, almost always, in suppressed renin and hypertension in adult life and sometimes in hypokalaemia and alkalosis, which can be severe. In most of these syndromes, precise molecular changes in specific steroidogenic or effector genes have been identified, permitting appreciation of (1) pathophysiology, (2) great diversity of phenotype and (3) possibility of genetic methods of diagnosis. Yet to be achieved elucidation of the genetic basis of familial hyperaldosteronism type 11, the most common and clinically significant of them, will enhance detection of primary aldosteronism, currently the commonest specifically treatable and potentially curable form of hypertension. While classic, complete-phenotype presentations of monogenic forms of mineralocorticoid hypertension are rarely recognised, more subtle genetic expression causing less florid manifestations could represent a significant proportion of so-called 'essential hypertension.'

The genetic basis of primary hypertension is not known. Renin is important in blood pressure and volume control and a HindIII restriction fragment length polymorphism (RFLP) is present within the human renin gene locus. To examine whether there is a relationship between this RFLP and primary hypertension, DNA and renin analyses were performed on leukocytes and plasma from hypertensive and normotensive individuals. In hypertensives the frequencies of alleles for the HindIII RFLP were found to be 0.55 and 0.45, compared with 0.60 and 0.40 in the total population of 231 subjects examined, a difference that was not statistically significant. There also appeared to be no significant difference in renin activity in plasma for hypertensive patients of each genotype, nor in their pre- or post-treatment blood pressures. We thus conclude that, within the limits of the present study, the suspected genetic abnormalities associated with primary hypertension in man do not appear to be related to a HindIII RFLP in the renin gene.

The effect of malathion on jugular plasma concentrations of follicle-stimulating hormone (FSH), estradiol (E2), progesterone (P4) and acetylcholinesterase (AchE) on conception in dairy cattle during a cloprostenol (prostaglandin F2-alpha analogue, PG)-induced estrus was studied. Malathion (1 mg/kg, intraruminally) given at the onset of estrus (48 h after PG) did not alter the plasma FSH or E2 concentrations but significantly (P < 0.05) inhibited plasma P4 concentration. The mean P4 concentration in the malathion-treated group on days 8 and 12 were 0.8 +/- 0.4 and 1.0 +/- 0.5 ng/ml, as compared to 2.6 +/- 0.0 and 2.4 +/- 0.3 ng/ml in the control group. There was a nonsignificant (P > 0.05) inhibition of plasma AchE activity in malathion-treated cattle. Conception was 16.6% in malathion-treated cows and 50% in controls. Inhibition of progesterone secretion and poor conception occurred after the single intraruminal dose of malathion at the onset of estrus.

The effect of induction of parturition with a PGF(2)alpha analog on plasma concentration of prolactin (PRL) and its effects on colostrum concentration of IgG and chitotriosidase (ChT) activity were studied in 16 pregnant Majorera goats. Treated goats, those in which parturition was induced, had greater concentrations of PRL than control goats 24 h before parturition (P < 0.05) and 48 h after parturition (P < 0.05). Control goats had greater concentrations of PRL than treated goats 96 h after parturition (P < 0.05). Plasma concentration of IgG did not differ between groups during the experimental period, but colostrum concentrations of IgG were greater in control goats than in treated goats at parturition (P < 0.05). Plasma ChT activity decreased during the period 72 h before parturition to 24 h after parturition in control and treated goats. Time evolution after partum affected the colostrum ChT activity, being greater at parturition than after parturition in both groups (P < 0.05). In summary, concentration of IgG in colostrum is slightly diminished if parturition is induced. Induction of parturition causes an early increase in PRL, which is most likely responsible for preterm suppression of IgG transport into mammary secretions. (C) 2011 Elsevier Inc. Introduction: Unaccustomed eccentric exercise often results in muscle damage and neutrophil activation. We examined changes in plasma cytokines stress hormones, creatine kinase activity and myoglobin concentration, neutrophil surface receptor expression, degranulation, and the capacity of neutrophils to generate reactive oxygen species in response to in vitro stimulation after downhill running. Methods: Ten well-trained male runners ran downhill on a treadmill at a gradient of -10% for 45 min at 60% V̇O2max. Blood was sampled immediately before (PRE) and after (POST), 1 h (1 h POST), and 24 h (24 h POST) after exercise. Results: At POST, there were significant increases (P < 0.01) in neutrophil count (32%), plasma interleukin (IL)-6 concentration (460%), myoglobin (Mb) concentration (1100%), and creatine kinase (CK) activity (40%). At 1 h POST, there were further increases above preexercise values for neutrophil count (85%), plasma Mb levels (1800%), and CK activity (56%), and plasma IL-6 concentration remained above preexercise values (410%) (P < 0.01). At 24 h POST, neutrophil counts and plasma IL-6 levels had returned to baseline, whereas plasma Mb concentration (100%) and CK activity (420%) were elevated above preexercise values (P < 0.01). There were no significant changes in neutrophil receptor expression, degranulation and respiratory burst activity, and plasma IL-8 and granulocyte-colony stimulating factor concentrations at any time after exercise. Neutrophil count correlated with plasma Mb concentration at POST (r = 0.64, P < 0.05), and with plasma CK activity at POST (r = 0.83, P < 0.01) and 1 h POST (r = 0.78, P < 0.01). Conclusion: Neutrophil activation remains unchanged after downhill running in well-trained runners, despite increases in plasma markers of muscle damage.

A hipertensão é uma das mais importantes causas de morte prematura no mundo. Estudos sobre a Euterpe oleracea Mart. (açaí), uma planta típica do Brasil e rica em polifenóis, têm mostrado grande potencial terapêutico contra a hipertensão, uma vez que seus benefícios podem ser associados às ações antioxidante, vasodilatadora e anti-hipertensiva. O rato espontaneamente hipertenso (SHR) é um modelo experimental utilizado para o estudo da hipertensão essencial. Neste estudo, investigamos o efeito do tratamento crônico do extrato hidroalcoólico do caroço de açaí (ASE) sobre a hipertensão de SHR. Animais SHR e Wistar receberam tratamento com ASE (200 mg/Kg/dia) na água de beber, ou veículo, desde 21 dias até 4 meses de idade e tiveram a pressão arterial sistólica (PAS) aferida por pletismografia de cauda. Os efeitos vasodilatadores da acetilcolina (ACh) e nitroglicerina (NG) foram estudados em leito arterial mesentérico (LAM) perfundido e pré-contraído com norepinefrina. A atividade das enzimas superóxido dismutase (SOD), catalase (CAT), glutationa peroxidase (GPx), os níveis de malondialdeído (MDA), a carbonilação de proteínas e os níveis de nitrito foram avaliados em plasma, LAM, coração e rim por espectrofotometria. A expressão das proteínas SOD e eNOS foram avaliadas por western blot em LAM e as alterações vasculares pela espessura da túnica média em aorta. A PAS foi maior (p<0.05) nos animais SHR, e reduzida pelo tratamento com ASE. O efeito vasodilatador reduzido da ACh em SHR foi recuperado pelo ASE e o da NG não foi diferente entre os grupos. Não houve diferença nos níveis de glicose e insulina em SHR comparados aos controles. Entretanto, a insulina se apresentou reduzida no grupo SHR+ASE. O nível de renina foi maior nos SHR e normalizado pelo ASE (p<0.05). Os níveis de MDA não foram diferentes entre SHR e controles, entretanto o tratamento com ASE reduziu esses níveis em rim de SHR (p<0.05). Os níveis de carbonilação de proteínas foram maiores em amostras de rim e coração de SHR e o ASE reduziu o dano sobre proteínas (p<0.05), não tendo diferença em plasma e LAM. A atividade da SOD foi menor em amostras de rim nos animais SHR e aumentada pelo tratamento com ASE (p<0.05). Entretanto, a atividade aumentada da SOD em coração e LAM dos SHR, foi reduzida pelo tratamento com ASE, não havendo diferença em amostras de plasma. Não houve diferença na atividade da GPx em amostras de LAM e coração dos diferentes grupos, porém sua atividade foi aumentada em rim dos SHR, e o tratamento com ASE normalizou essa atividade. Em plasma, a atividade da GPx foi reduzida em SHR e aumentada pelo tratamento (p<0.05). A atividade da enzima CAT foi reduzida em plasma e rim de SHR e o ASE aumentou sua atividade. Não houve diferença em amostras de LAM, entretanto em amostras de coração o tratamento aumentou a atividade da CAT em SHR (p<0.05). Em amostras de plasma, coração e rim, não houve diferença nos níveis de nitrito entre os diferentes grupos, porém em amostras de LAM foram menores em SHR e SHR+ASE (p<0.05). A expressão das proteínas eNOS e SOD apresentaram-se aumentadas em SHR (p<0.05) sem alteração com o tratamento. Os SHR apresentaram um aumento na espessura da camada média da aorta que foi reduzido (p<0.05) pelo ASE. Este estudo demonstrou que o tratamento crônico com ASE em SHR reduziu a hipertensão, preveniu a disfunção endotelial e o remodelamento vascular. O aumento da defesa antioxidante e redução do dano oxidativo devem contribuir para os efeitos benéficos de ASE. Portanto, sugerimos que o ASE pode ser uma ferramenta importante para o tratamento das alterações cardiovasculares associadas à hipertensão essencial.

Alguns trabalhos demonstraram que a planta, Euterpe oleracea Mart. (açaí) é rica em polifenóis e que uma dieta rica em polifenóis pode estar envolvida na proteção contra o risco cardiovascular. Desta forma, o objetivo deste estudo foi avaliar o efeito do tratamento com o extrato hidroalcoólico do caroço do açaí (ASE) (200mg/Kg/dia), sobre as alterações renais, cardiovasculares, metabólicas, morfológicas e o estresse oxidativo na prole adulta com dezesseis semanas, cujas mães foram submetidas a uma dieta hipoprotéica durante a gestação. Quatro grupos de ratas foram alimentados com dietas experimentais: controle (20% de proteínas); controle + ASE (20% de proteína + ASE); hipoprotéica (6% de proteína); hipoprotéica + ASE (6% de proteína + ASE) durante a gestação. Após o desmame, todos os filhotes passaram a ser alimentados com uma dieta controle e foram sacrificados com 4 meses de idade. A pressão arterial sistólica (PAS) foi medida por pletismografia de cauda e o efeito vasodilatador da acetilcolina (ACh) e nitroglicerina (NG) foi avaliado em leito arterial mesentérico (LAM) perfundido. Foram determinados o peso corporal, níveis séricos de colesterol total, triglicerídeos, lipoproteína de alta densidade (HDL), albumina, uréia, creatinina, glicose, insulina, resistência à insulina (índice de Homa IR), sódio, potássio e a renina plasmática. Foi avaliada a depuração de creatinina, volume de urina, excreção fracionada de sódio (EFNa) e o número de glomérulos renais. O dano oxidativo, níveis de nitrito e a atividade das enzimas antioxidantes: superóxido dismutase, catalase e glutationa peroxidase foram dosados no plasma e homogenato de rim. O peso corporal foi menor no grupo hipoprotéico e recuperado com ASE. A PAS foi aumentada no grupo hipoprotéico e revertida pelo tratamento com ASE. Os níveis de renina plasmática foram aumentados no grupo hipoprotéico e reduzidos com ASE. A resposta vasodilatadora à ACh em LAM estava reduzida no grupo hipoprotéico com ASE houve uma melhora dessa resposta.O efeito vasodilatador da NG não foi diferente entre os grupos. No grupo hipoprotéico também foi observado o aumento dos níveis de triglicerídeos, colesterol total, uréia, creatinina, glicose e insulina, os mesmos foram reduzidos pelo ASE. Não houve diferença nos níveis de HDL, sódio, potássio, depuração de creatinina e volume urinário nos grupos estudados. Os níveis de malondialdeído e carbonilação de proteínas estavam aumentados e os níveis de nitrito reduzidos no grupo hipoprotéico e foram revertidos pelo ASE. As atividades das enzimas antioxidantes no plasma e no rim foram reduzidas no grupo hipoprotéico e aumentadas pelo ASE, com exceção da catalase que não apresentou diferença entre os grupos. Os animais hipoprotéicos apresentaram redução no número de glomérulos renais e aumento da EFNa e o tratamento com ASE preveniu as alterações renais encontradas neste modelo. Em conclusão, o ASE parece proteger a prole, cujas mães foram expostas a uma dieta com pouca proteína durante a gestação, através do efeito anti-hipertensivo, vasodilatador e antioxidante observado neste trabalho.

Objective Activation of the renal renin-angiotensin system in patients with diabetes mellitus appears to contribute to the risk of nephropathy. Recently, it has been recognized than an elevation of prorenin in plasma also provides a strong indication of risk of nephropathy. This study was designed to examine renin-angiotensin system control mechanisms in the patient with diabetes mellitus.Methods We enrolled 43 individuals with type 2 diabetes mellitus. All individuals were on a high-salt diet to minimize the contribution of the systemic renin-angiotensin system. After an acute exposure to captopril (25 mg), they were randomized to treatment with either irbesartan (300 mg) or aliskiren (300 mg) for 2 weeks.Results All agents acutely lowered blood pressure and plasma aldosterone, and increased renal plasma flow and glomerular filtration rate. Yet, only captopril and aliskiren acutely increased plasma renin and decreased plasma angiotensin II, whereas irbesartan acutely affected neither renin nor angiotensin II. Plasma renin and angiotensin II subsequently did increase upon chronic irbesartan treatment. When given on day 14, irbesartan and aliskiren again induced the above hemodynamic, renal and adrenal effects, yet without significantly changing plasma renin. Irbesartan at that time did not affect plasma angiotensin II, whereas aliskiren lowered it to almost zero.Conclusion The relative resistance of the renal renin response to acute (irbesartan) and chronic (irbesartan and aliskiren) renin-angiotensin system blockade supports the concept of an activated renal renin-angiotensin system in diabetes, particularly at the level of the juxtaglomerular cell, and implies that diabetic patients might require higher doses of renin-angiotensin system blockers to fully suppress the renal renin-angiotensin system. New technologies and sterilization agents for heat-sensitive materials are under intense investigation. Plasma sterilization, an atoxic low-temperature substitute for conventional sterilization, uses various gases that are activated by an electrical discharge, generating reactive species that promote lethality in microorganisms. Here, assays were performed using pure O-2 and O-2 + H2O2 mixture gas plasmas against a standard load of Bacillus atrophaeus spores inoculated on glass carriers inside PVC catheters. The sterilization efficiency was studied as a function of plasma system (reactive ion etching or inductively coupled plasma), biological monitor lumen diameter, gas, radio frequency power, and sub-lethal exposition time. After sterilization, the biological monitors were disassembled and the surviving bacteria were grown in trypticase soy broth using the most probable number technique. Plasma antimicrobial activity depended on the catheter's internal diameter and radio frequency powers. The N-2 + H2O2 mixture exhibited higher microbial efficacy than pure N-2 in both plasma systems.
