A colestase crônica na infância e na adolescência interfere diretamente no cres-cimento e no desenvolvimento do indivíduo e produz conseqüências clínicas relacionadas com a má absorção das vitaminas lipossolúveis da dieta. A vitamina E exerce um importante papel na estrutura e na função dos sistemas nervoso e musculoesquelético. A vitamina D tem reconhecida influência sobre a fisiopatologia da osteopenia colestática que se manifesta como osteoporose, raquitismo ou osteomalácia. A realização de dosagens plasmáticas dessas vitami-nas é essencial para detectar precocemente suas deficiências, bem como para monitorizar uma adequada suplementação. Essas dosagens não são realizadas de rotina no nosso meio. Os objetivos do presente estudo foram verificar os níveis plasmáticos de vitami-nas D e E em uma amostra de crianças e adolescentes com colestase crônica; verificar o esta-do nutricional e a ingestão de macro e micronutrientes desses pacientes; verificar o uso de su-plemento de vitaminas, o tempo de colestase; e realizar avaliação neurológica para estabelecer eventual relação com os níveis plasmáticos de vitamina E. A amostra constou de 22 crianças e adolescentes com colestase crônica que con-sultavam no ambulatório ou estiveram internadas na Unidade de Gastroenterologia Pediátrica do Hospital de Clínicas de Porto Alegre no período de dezembro de 2000 a abril de 2002. Como controles, participaram 17 crianças eutróficas e normais do ponto de vista gastroentero-lógico com faixa etária correspondente. Foram realizadas avaliação nutricional e avaliação neurológica. Foi pesquisado o tempo de colestase e o uso de suplemento de vitaminas lipossolúveis. A técnica utilizada para as dosagens da vitamina E foi a cromatografia líquida de alta precisão (HPLC) e as dosagens plasmáticas de vitamina D pela técnica de radioimunoensaio. A prevalência de desnutrição variou entre 23,8% a 63,0% considerando as diferentes medidas e padrões utilizados. O inquérito alimentar realizado demonstrou uma ingestão calórica média de 89,33 ± 27,4% em relação ao recomendado para idade com uma distribui-ção dos macronutrientes em relação às calorias ingeridas dentro dos valores de referência para o grupo em questão, havendo, porém, uma pobre ingestão de micronutrientes como ferro e zinco. O exame neurológico foi alterado em 43% dos pacientes colestáticos, em que foram constatadas vinte alterações neurológicas em nove pacientes. Não obtivemos resultados con-fiáveis para os níveis plasmáticos de vitamina E, apesar de realizar 3 etapas para validação. O valor médio de vitamina D entre os pacientes foi de 13,7 ± 8,39 ng/ml, enquanto que no grupo controle foi de 25,58 ± 16,73 ng/ml (P = 0,007), havendo uma prevalência de hipovitaminose D entre esses pacientes de 36%. Não foi observada relação entre estado nutricional, tempo de colestase ou uso de suplemento oral de vitaminas lipossolúveis e os níveis plasmáticos refe-ridos. Concluímos que a média de níveis plasmáticos de vitamina D nas crianças e nos adolescentes colestáticos do estudo foi significativamente menor do que nos controles nor-mais sem relação significativa com estado nutricional, tempo de colestase ou uso de suple-mento de vitaminas. As alterações neurológicas foram freqüentes e a prevalência de desnutri-ção nos pacientes foi semelhante à encontrada na literatura. A ingesta calórica foi deficiente havendo porém, um equilíbrio dos macronutrientes e ingestão insuficiente de ferro e zinco.

O objetivo geral desse estudo foi analisar os efeitos de dois treinamentos de força diferenciados, em volume e intensidade, em algumas variáveis relacionadas à saúde da população idosa, tais como, força isométrica (FI) e dinâmica de membros superiores (FMS) e inferiores (FMI), ativação muscular do quadríceps (EMG), consumo máximo de oxigênio (VO2máx.), tempo de exaustão em esteira (TE) e densidade mineral óssea (DMO). Vinte e oito mulheres pós-menopáusicas, com perda óssea (osteoporose ou osteopenia), com e sem reposição hormonal, foram divididas em três grupos experimentais: (1) treinamento de força (GF - n=9) com intensidades entre 35 e 80% de 1RM, (2) treinamento em circuito (GC - n=10) com intensidades entre 35 e 60% de 1RM , e (3) um grupo controle (GCON - n=9). Os grupos GF e GC treinaram 3 vezes por semana durante 24 semanas, enquanto o GCON não realizou nenhum tipo de exercício físico sistemático. Em todos os grupos (GF, GC e GCON), as variáveis analisadas foram comparadas através de análise de variância (ANOVA) para medidas repetidas e, em caso de diferenças significativas, foi utilizado o teste Post-Hoc de Bonferroni. Em todas as análises, o nível de significância de p<0,05 foi considerado. Após as 24 semanas de treinamento foram observados aumentos significativos nas variáveis analisadas, somente em GF e GC. No entanto, enquanto o GF teve as variáveis FI (112 ± 18,4Nm vs. 149,8 ± 23,4Nm), FMS (7,3 ± 0,75kg vs. 9,4 ± 0,96kg), FMI (46,7 ± 5,5kg vs. 65,2 ± 8,9kg), EMG (138 ± 35µV vs. 208 ± 51µV), VO2máx. (21,7 ± 2,7ml.kg-1.min-1 vs. 26,6 ± 2,2 ml.kg-1.min-1) e TE (562,6 ± 98,3s vs. 671,7 ± 72,7s), modificadas; o GC apresentou modificações apenas nas variáveis FI (124,1 ± 23,2Nm vs. 146,7 ± 22,3Nm), FMS (6,8 ± 1,3kg e 8,5 ± 1,2kg); FMI (41,4 ± 7,8kg para 60,1 ± 9,5kg), VO2máx. (22,1 ± 2,3 ml.kg-1.min-1 vs. 26,2 ± 2,3 ml.kg-1.min-1) e TE (573,2 ± 66,5s, pós: 669 ± 75,3s). A DMO não foi modificada em nenhum grupo experimental. No GCON não houve qualquer modificação das variáveis analisadas. Esses resultados sugerem que tanto o treinamento de força como o treinamento em circuito interferem positivamente na força e ativação muscular, e no condicionamento cardiorespiratório de mulheres pós-menopáusicas. No entanto a DMO parece não ser alterada com esses tipos de treinamento, em um período de 24 semanas.

The relationship between lipid peroxidation, antioxidant defense and diabetic osteopenia remains unclear. This study evaluated the relationship between lipid peroxidation index, antioxidant defense parameter and bone metabolism in a premenopausal diabetic model by measurements such as thiobarbituric acid-reactive substances concentration (TBARS) and reduced glutathione (GSH) content in brain homogenates, histomorphometric analysis, biomechanical testing and bone mineral density (BMD). Female Wistar rats with regular estrous cycle were divided into two groups: Group 1: control rats (n = 15) and Group 2: diabetic rats (n = 15). Diabetes mellitus was induced by alloxan and confirmed by glycemia 250 mg/dL. The experimental period understood 1 and 5 after days induction and 45, 75 and 120 days after the installation of diabetes mellitus.The lipid peroxidation index, measured by TBARS concentration, showed a significant increase (p<0.05) in diabetic animals in comparison to animals control. However, the antioxidant parameter, measured by GSH content, was significantly decrease (p<0.05) in diabetic animals. Histomorphometric analysis showed a significant increase (p<0.05) in femoral trabecular separation together with a significant decrease (p<0.05) in trabecular thickness and reduced trabecular bone volume in diabetic rats. Moreover, biomechanical testing and BMD values were significant decrease (p<0.05) in diabetic group. Thus, our results demonstrated that increased lipid peroxidation and altered antioxidant defense could be related to the development of oxidative stress and diabetic osteopenia in premenopausal rats

Diabetes Mellitus (DM) and osteoposes are chronic diseases with great socioeconomic consequences, mainly due to the late complications and consequent disabilities. The potential effects of DM on bone metabolism remain a very conroversial issue, and disagreement exists with regard to the clinical implications of diabetic osteopenia and the mechanism of its ocurrence. The issue is further complicated by the contribuicion of the especific factors, such as duration of disease an dthe degree of metabolic control. The objective of this study is to identify the osteopathy in children and adolescents with DM 1 assisted in the hospital of pediatrics, UFRN, through biochemical markers of bone and mineral metabolism and the extent of bone mineral density. The study was composed by 74 diabetics type 1 patients (DM1) of both gender and aged 6 to 20 yars. Normoglicêmic group was composed by 97 healthy subjects of both genders, which showed the same age range of DM1, in addition to same socioeconomic class. These individuals qere students from the networks of public education in the city of Natal-RN, randomly invited to paticipate in our study. Both groups DM1 and NG were divided intofour subgroups, according to the classification of tanner , T1, T2, T3, T4 for achieving a benchmark. Diabetic individuals showed up with a poor glycemic control. the group DN1 T4 showed an incresead value for total protein, albumin, urea and microalbumiuria are predictors of grumelura injury in DM1 patients . The total alkaline phosphatase activitywas kept on high levels for both groups because they are in a stature development age. For osteocalcin there were decreased levels for groups Dm1 T1, T2, and T3 when compared to their NG (s), suggesting that this decrease could be associated with reduction in the number and/or differentiation os osteoblasts thereby contributing to reducing bone formation. There were no changes in the activity of TRAP. The serum concentrations of total and ionized calcium, phosphorus and magnesium were included within the RV. It was observed that the BMD (Z- SCORE ) has always been within the RV for both groups, despite to DM1 T4. Taking all together, our results support the hypothesis that children and adolescents with type 1 DM present the risk in the long run to suffer a reduction in the bone mass, associated to poor glicemic control and disease duration. It could limit the bone growth and increase the probality of development of osteopenia, as well as other complications surch as retinopathy and renal failure

OBJETIVO: avaliar a qualidade de vida de pacientes com osteoporose e osteopenia, acompanhadas em ambulatórios especializados em osteoporose e climatério, comparando-as com pacientes com densidade mineral óssea (DMO) normal. MÉTODOS: estudo de série de casos transversal, observacional, que se propôs a analisar, por meio do questionário Medical Outcomes Study 36 Short-Form Health Survey (SF-36), a qualidade de vida de mulheres com osteopenia e osteoporose. Foram avaliadas 124 mulheres na pós-menopausa divididas em três grupos: 55 pacientes com diagnóstico densitométrico de osteoporose, 35 com o de osteopenia e 34 que apresentavam DMO normal. Os três grupos foram comparados com relação aos dados demográficos, características clínicas e de estilo de vida e aos diferentes domínios do SF-36. RESULTADOS: as pacientes dos grupos osteopenia e DMO normal apresentaram menor idade média (56,7±7,1 e 52,9±5,4 anos), maior índice de massa corpórea (IMC) (28,6±3,7 e 30,9±5,1 kg/m²) e menor tempo de menopausa (8,4±5,9 e 5,8±4,5 anos) quando comparadas ao grupo osteoporose (61,8±10,1 anos, IMC de 25,7±5,3 kg/m², 15,5±7,5 anos, respectivamente; p<0,05). de acordo com o SF-36, não houve diferença significativa entre os grupos com relação aos domínios, à exceção do domínio vitalidade, que se mostrou superior no grupo osteoporose. Com relação à impressão pessoal sobre seu estado de saúde, das pacientes que o consideraram bom, um maior percentual pertencia ao grupo osteoporose, e entre aquelas que o consideraram ruim, um percentual menor pertencia ao grupo osteopenia. CONCLUSÃO: a qualidade de vida foi similar em mulheres com osteoporose e osteopenia, em relação às com DMO normal, à exceção do domínio vitalidade, que foi superior, paradoxalmente, nas pacientes com osteoporose.

Considering that osteopenia and osteoporosis are diabetes mellitus complications, and that tamoxifen (TAM) is an anti-estrogenic drug used in breast cancer treatment, this drug may have a beneficial action preventing accentuaded bone loss associated to diabetes. Female Wistar rats (n=60) weighting 180-250g were divided in four groups: Group C, control animals (n=5); Group T, animals treated with TAM (n=5); Group D, diabetic animals (n=5); and Group DT, diabetic animals treated with TAM (n=5). Oestrus cycle was evaluated before the beggining of experimental period to select the animals with regular cycle. This evaluation continued throughout the study period and for all studied groups. Diabetes was induced by a intra perithoneal injection of streptozotocin (STZ) in a concentration of 45 mg/Kg of body weight. Those animals with serum glicose levels 250 mg/dL were considered diabetics. Animals were sacrificed in the periods of 30, 60 and 90 days after diabetes onset. Left femur histomorphometric measurements and serum biochemical analysis (glycemia, alkaline phosphatase, tartaric-resistant acid phosphatase, calcium, phosphorous, magnesium, total proteins, albumin, globulins, urea and creatinine) were done. Histomorphometric results showed a progressive bone loss in Group D animals when compared to those from Group C all over the experimental period, becoming accentuaded in the 90 days period. In relation to Groups T and DT, values approcimated to those obtained for control group were found during the whole period of study. Those data may indicate a bone mass recovery or a diminished bone loss due to diabetes when animals were treated with TAM. During the whole experimental period animals of groups D and DT maintained glycemic levels above 250 mg/dL whereas animals of groups C and T maintained those levels below 150mg/dL. Alkaline phosphatase activity was increased in all study periods for groups D and DT when compared to group C animals over the 90 days period. Tartarate-resistant acid phosphatase activity was showed unaltered in all periods of study and for all groups. Calcium and magnesium results were also unaltered, maintaining reference levels for all groups in all experimental periods. Phosphorous levels were increased in groups D and DT when compared to groups C and T in the 30 days period. However no difference was found in the periods of 60 and 90 days for this test. No difference was found for total proteins levels for groups C, T, D and DT over the study period. Albumin levels were reduced in DT group in the 60 days period and in D and DT groups in the 90 days period. Urea levels were significantly increased in the 30, 60 and 90 days study periods in groups D and DT when compared to groups C and T. Creatinine results showed a significantly increase in the 90 days period for groups D and DT when compared to groups C and T, and maintaining unaltered in the 30 and 60 days periods. These results suggest that the treatment with TAM may reduce bone loss caused by diabetes mellitus

Objectives:This study evaluated the influence of ovariectomy 8 weeks after implant placement on bone integrated to titanium implants.Materials and methods:Thirty-eight female rats were submitted to a titanium implant at the tibiae proximal methaphysis. After a healing period of 8 weeks the animals were randomly divided into three groups: control (CTL-10 animals), sham-operated (SHAM-14 animals) and ovariectomy (OVX-14 animals). The CTL group was then sacrificed in order to confirm integration of the implant. The SHAM group was submitted to fictitious surgery and the OVX group was submitted to bilateral ovariectomy. After 12 weeks post-implant placement, the SHAM and OVX groups were sacrificed. In order to confirm the systemic osteopenia in rats, a dual-energy X-ray absorptiometry (DXA) was performed. For the evaluation of bone density, digital radiographs were taken. The grey level of the bone adjacent to implant was measured using image software and the bone density was calculated at six points on both sides of the implant.Results:Densitometry measurements of the femur confirmed systemic bone mass loss in the OVX group. Individualized bone analyses of different regions surrounding the implant showed a significantly lower radiographic bone density (P < 0.05) in the cancellous region of the OVX group (77.48 +/- 23.39 grey levels) when compared with the CTL and SHAM groups (91.61 +/- 32.10 and 102.57 +/- 32.50 grey levels, respectively).Conclusions:The present study showed a decrease of the radiographic bone density in the cancellous region of bone around titanium implants placed 8 weeks before ovariectomy in rats.

Bone loss associated with cyclosporin A (CsA) therapy can result in serious morbidity to patients. Intermittent administration of 1,25 Vitamin D and calcitonin reduces osteopenia in a murine model of postmenopausal osteoporosis. The purpose of this study was to evaluate the effects of this therapeutic approach on CsA-induced alveolar bone loss in rats. Forty male Wistar rats were allocated to four experimental groups according to the treatment received during 8 weeks: (1) CsA (10 mg/kg/day, s.c.); (2) 1,25 Vitamin D (2 mu g/kg, p.o.; in weeks 1, 3, 5, and 7) plus calcitonin (2 mu g/kg, i.p.; in weeks 2, 4, 6, and 8); (3) CsA concurrently with intermittent 1,25 Vitamin D and calcitonin administration; and (4) the control treatment group (vehicle). At the end of the 8-week treatment period, serum concentrations of bone-specific alkaline phosphatase, tartrate-resistant acid phosphatase (TRAP-5b), osteocalcin, interleukin (IL)-1 beta, IL-6, and tumor necrosis factor alpha (TNF-alpha) were measured and an analysis of bone volume, bone surface, number of osteoblasts, and osteoclasts was performed. CsA administration resulted in significant alveolar bone resorption, as assessed by a lower bone volume and an increased number of osteoclasts, and increased serum bone-specific alkaline phosphatase, TRAP-5b, IL-1 beta, IL-6, and TNF-alpha concentrations. The intermittent administration of calcitriol and calcitonin prevented the CsA-induced osteopenic changes and the increased serum concentrations of TRAP-5b and inflammatory cytokines. Intermittent calcitriol/calcitonin therapy prevents CsA-induced alveolar bone loss in rats and normalizes the production of associated inflammatory mediators.

OBJETIVO: avaliar a prevalência do hipotiroidismo subclínico e suas repercussões sobre o perfil lipídico e a densidade mineral óssea (DMO) em mulheres na pós-menopausa. Métodos: trata-se de estudo transversal com recuperação de dados de prontuários de pacientes acompanhadas em ambulatório de climatério. Critérios de inclusão: mulheres na pós-menopausa com dosagem do hormônio estimulador da tiróide (TSH) e de tiroxina livre (T4-L). Critérios de exclusão: hipertiroidismo e carcinoma de tiróide. Considerou-se hipotiroidismo subclínico valores de TSH superiores a 5,0 mUI/mL e T4-L normal. Foram selecionadas 320 pacientes (idade 55,2±6,4 anos) divididas em 3 grupos: função tiroideana normal (n=208), hipotiroidismo subclínico (n=53) e hipotiroidismo clínico sob tratamento (n=59). Foram analisados dados clínicos, uso de terapia hormonal, índice de massa corpórea (IMC=kg/m²), perfil lipídico (colesterol total, HDL, LDL, triglicerídeos) e DMO da coluna lombar e fêmur. Na análise estatística, as diferenças entre as médias dos grupos foram comparadas utilizando-se a análise de variância (ANOVA). Para múltipla comparação, assumindo que a variância era diferente entre os grupos, utilizou-se o método de Tukey. RESULTADOS: o hipotiroidismo subclínico foi diagnosticado em 16,1% dos casos. Os grupos foram homogêneos quanto às características clínicas, IMC e perfil lipídico e uso de terapêutica hormonal. Nas pacientes com hipotiroidismo subclínico ou clínico encontrou-se menor freqüência de osteopenia na coluna lombar e fêmur quando comparadas às eutiroidianas (p<0,001). Houve correlação negativa entre os valores de TSH e DMO da coluna lombar e fêmur (p<0,001). Não se constatou correlação entre os valores de TSH e idade, tempo de menopausa, IMC e perfil lipídico. O total de usuárias de terapia hormonal foi de 65,1%, duração média de 3,43±2,42 anos, não diferindo entre os grupos. CONCLUSÃO: o hipotiroidismo subclínico com prevalência de 16,1% na pós-menopausa associou-se à baixa DMO, mas sem repercussões sobre o perfil lipídico.

We investigated the importance of daily free activity in the cage and body weight gain during the recovering of bone structural and mechanical properties in growing rats after hindlimb unloading. Eight-week-old male Wistar rats were randomly divided into control (CG, n=24) and suspended (SG, n=24) groups. Animals from SG underwent a four-week hindlimb unloading period by tail-suspension. Animals from CG and those from SG after release were kept in collective cages and sacrificed at the age of 12, 16 and 20 weeks. Both femurs were removed and its area, bone mineral density (BMD), resistance to failure and stiffness were determined. Four-week hindlimb unloading decreased (p < 0.05) body weight (CG, 373.00 +/- 9.47 vs. SG, 295.86 +/- 9.19 g), BMD (CG, 0.19 +/- 0.01 vs. SG, 0.15 +/- 0.01 g/cm(2)), bone resistance to failure (CG, 147.75 +/- 5.05 vs. SG, 96.40 +/- 5.95 N) and stiffness (CG, 0.38 +/- 0.01 vs. SG, 0.23 +/- 0.02 N/m). Eight weeks of free activity in cage recovered (p > 0.05) the body weight (CG, 472.75 +/- 14.11 vs. SG, 444.75 +/- 18.91 g), BMD (CG, 0.24 +/- 0.01 vs. SG, 0.22 +/- 0.01 g/cm(2)), bone resistance to failure (CG, 195.73 +/- 10.06 vs. SG, 178.45 +/- 8.48 N) and stiffness (CG, 0.56 +/- 0.02 vs. SG, 0.47 +/- 0.03 N/m) of SG animals. Body weight correlated strongly with bone structural and mechanical properties (p < 0.0001). In conclusion, free activity in the cage associated with body weight gain restored bone structural and mechanical properties in growing rats after hindlimb unloading.

Background: Many studies have shown that physical exercises are able to stimulate bone formation and increase bone mass, constituting a therapeutic modality to treat bone loss due to osteoporosis. However, some points about the intensity, duration and frequency of the exercises remain confusing and contradictory. Thus, the aim of this study was to determine the effects of a progressive loading exercise program on femur of osteopenic rats. To induce osteopenia we used the animal model of ovariectomy (OVX). Forty animals was studied and divided into 4 groups: sham-operated sedentary (SS); ovariectomy-sedentary (OS); sham-operated training (ST) and ovariectomy training (OT). The trained groups performed jumps into water: 4 series of 10 jumps each, with an overload of 50% to 80% of the animal's body weight, during 8 weeks. Femora were submitted to a physical properties evaluation, a biomechanical test, calcium and phosphorus content measurement and a morphometric histological evaluation. Results: osteopenic animals showed a decrease of bone strength and lower values of bone weights, bone density and calcium content. The exercised osteopenic rats showed higher values of geometrical, physical properties, bone strength and calcium content compared to controls. The results of the present study indicate that the progressive loading exercise program had stimulatory effects on femora of osteopenic rats. It seems that the intensity and duration of the protocol used produced bone structural adaptations, which contributed to reverse bone loss due to ovariectomy.
